Amaç:Tavşanda enfekte diz protezi modelinde ozon ve antibiotik kombinasyonunun etkisini araştırmak. Gereç ve Yöntemler:24 Yeni Zelanda tavşanın sağ femoral kondiline polietilen pul ve vida yerleştirildi ve intraartiküler Staphylococcus aureus inoküle edildi. Dört grup incelendi: kontrol, antibiotik (14 gün), ozon (iki hafta boyunca haftada üç kez), ozon+antibiotik. Cerrahi sonrası yedinci günde tüm tavşanlar klinik olarak incelendi ve dizleri NaCI solüsyonu ile irrige edildi. Her gruptan 2 hayvan rastgele seçildi ve infeksiyon belirlendi. Tüm hayvanlar 3. hafta sonunda sakrifiye edildi. Tüm hayvanlardan diz örnekleri alındı, kültür ekimi yapıldı ve histolojik ve histokimyasal olarak Salter scorlama sistemi ile değerlendirildi. Bulgular: Kontrol grubu ile diğer gruplar arasında histolojik-histokimyasal skor ve mikrobiolojik olarak anlamlı fark bulundu fakat diğer gruplar arasınsa fark yoktu. İstatistiki olarak önemli olmasada histolojik-histokimyasal sonuç ozon+antibiotik grubunda en düşüktü ve kültür sonucu en iyi antibiotik grubundaydı. Sonuç: Ozon tedavisi, eklem içi implant bulunan kişilerde enfeksiyonun neden olduğu eklem dejenerasyonunun önlenmesi için alternatif bir seçenek olarak düşünülebilir., AbstractObjectives:To investigate effect of a combination of ozone and antibiotics on the treatment of an infected knee prosthesis in a rabbit model. Methods: Twenty-four New Zealand rabbits underwent implantation of a polyethylene washer and screw into the lateral condyle of the right femur and intraarticular inoculation with Staphylococcus aureus. Four groups were examined: control, antibiotic (14 days), ozone (thrice weekly for two weeks), ozone+antibiotic. After the 7th day of surgery, all animals were clinically examined and their knees were irrigated with an NaCl solution. In all groups, two animals were chosen randomly and assessed for infection. All animals were sacrificed at the end of the 3rd week. Knee samples were taken from all animals, cultured, and evaluated via the histologic-histochemical Salter scoring system.Results: There was a significant difference between the control group and all other groups in histologic-histochemical scores, but no other group-wise differences. Although they did not reach statistical significance, the histological-histochemical scores were lowest in the ozone+antibiotic group and the results of culturing most pronounced in the antibiotic group.Conclusions: Ozone therapy may be considered to be an alternative option for the prevention of joint degeneration cause by infection in individuals with an intraarticular implant.

INTRODUCTION[|]The effect of adolescent idiopathic scoliosis (AIS) on shoulder dysfunction and upper limb functionality is still not poorly understood. In this study, we aimed to investigate the effects of AIS on upper extremity functionality and quality of life.[¤]METHODS[|]Thirty-seven patients who had undergone surgery for adolescent idiopathic scoliosis participated in this study. Upper extremity functionality was evaluated using Turkish version of the Disabilities of the Arm, Shoulder and Hand (DASH-T) questionnaire, hand-grip strength Jamar hand dynamometer, and health-related quality of life was assessed using SF-36, and Scoliosis Research Society-22r Health-Related Quality of Life questionnaire (SRS-22).[¤]RESULTS[|]The mean age of the patients was 17.08+-2.96 years and the mean follow-up period was 34.44+-25.37 months. The average DASH-T score was found 14.79+-17.35. Patients quality of life scores was good level. The average right hand grip strength was 19.84+-8.89 kg while left hand grip strength was 18.97+-8.01 kg. There was no statistically significant difference between right and left hand grip strength (p=0.67). There was a moderate negative correlation with DASH-T and SRS-22 pain (r=-0.46, p=0.01) and SF-36 pain (r=0.54, p=0.01), and a weak positive correlation with SF-36 social function (r=-0.38, p=0.03). There was no statistically significant relationship between Cobb angle (Δ) and SRS-22, SF-36, DASH-T and hand grip strength.[¤]DISCUSSION AND CONCLUSION[|]Further studies may examine the effects of surgery on upper extremity functionality, hand-grip strength, and quality of life in patients with AIS using both objective and patient-reported assessment tools.[¤]

The integration of stem cell technology and cell sheet engineering improved the potential use of cell sheet products in regenerative medicine. This review will discuss the use of mesenchymal stem cells (MSCs) in cell sheet-based tissue engineering. Besides their adhesiveness to plastic surfaces and their extensive differentiation potential in vitro, MSCs are easily accessible, expandable in vitro with acceptable genomic stability, and few ethical issues. With all these advantages, they are extremely well suited for cell sheet-based tissue engineering. This review will focus on the use of MSC sheets in osteogenic tissue engineering. Potential application techniques with or without scaffolds and/or grafts will be discussed. Finally, the importance of osteogenic induction of these MSC sheets in orthopaedic applications will be demonstrated. © 2017 A. Cagdas Yorukoglu et al.

39 6.0ZET Aksesuar naviküler (AN) ve pes planus (PP) arasında bir çok tartışmaya konu olan bir birliktelik vardır. Bu çalışmanın amacı AN'i olan ayaklarda tibialis posterior tendonunun (TPT) ortaayaktaki dağılımını incelemektir ve bu dağılımdan yola çıkarak AN ile PP birlikteliğini açıklamaya çalışmaktır. AN'i olan ayaklarda AN'in ailesel geçişi ile ilgili bir belirsizlik vardır. Bu konuda yapılan saptamalar bilimsel verilere dayanmamaktadır. Bu çalışmanın bir bölümü olan genetik çalışma AN'in ailesel geçişini incelemeyi amaçlamıştır. Çalışma dört basamakta gerçekleştirilmiştir. İlk olarak 119 kadavra ayağı incelenmiş ve 15 AN saptanmıştır. Bu ayaklardan dördünde tendonun yapışma yerini dolduran ve histolojik incelemede fibrokartilaj özelliğinin gözlendiği yer kaplayan dokular saptanmıştır. Ayrıca bu ayaklardan dokuzunda TPTnin AN'e tamamen yapıştığı, tendonun kasılması taklit edildiğinde naviküler kemik distalindeki tendona ait dokulara bu kuvvetin iletilmediği saptanmıştır. İkinci basamakta AN'i olan 35 ayak MR görüntüleme yöntemi ile incelenmiştir. 35 ayağın 23'ünde tendon yapışma yerinin lateralinde, yapışma bölgesini dolduran bir doku tespit edilmiştir. 11 ayakta tendonun, AN ve naviküler kemiğe yapışma bölgesinde tamamen sonlandığı ve diğer ayaklarda ise belirgin şekilde inceldiği saptanmıştır. Üçüncü basmakta MR incelemesi uygulanan üç olgu öpere edildi. İntraoperatif bulgular hem kadavra incelemesi hem de MR incelemesindeki bulgularla uyumlu olarak tespit edilmiştir. Son basamakta üç olgunun aile bireyleri incelenerek pedigrileri elde edilmiştir. Bu pedigriler incelendiğinde ailesel geçişin otozomal dominant formda olduğu gözlenmiştir. Sonuç olarak tendonun yapışma yeri lateralinde sıkışıklık yaratan fibrokartilaj bir doku saptanmıştır. Bu doku olasılıkla kronik friksiyon kuvvetleri sonucu oluşmuş adaptif bir dokudur. Bulunduğu bölgede sıkışıklık ve buna bağlı olarak ağrı yaratmaktadır. Bu ağrı bir süre sonra kompanzatris olarak ayak medialinin yer ile temasının sağlanmasına ve bu bölge üzerindeki yükün bu şekilde azaltılmasına neden olacaktır. Tendonun AN yapışma bölgesinde sonlanması ya da incelmesi durumunda, tendonun tarslara ve metatarslar giden dallarına kuvvet iletilmeyecektir. Sonuç olarak TPT yetmezliği benzeri bir durum gelişecek ve ayak medial arkı çökecektir. 47
