Keskin, Gülsen Aydın (Balikesir Author), Hybrid flowshop scheduling problem (HFSP) is a mixture of two classical scheduling problems as parallel machine scheduling (PMS) and flowshop scheduling (FS). In the HFSP, a series of jobs are processed respectively in a set of stages that at least one of these stages has more than one parallel machine (identical, uniform or unrelated). HFSP is a widespreadly studied subject in the literature and there are various application areas such as transportation, healthcare management, agricultural activities, cloud computing, and the most common manufacturing. Therefore, it will be useful to present a review study including recent papers and developments related to this problem for researchers. For this aim, in this paper, a systematic literature survey has been conducted with respect to HFSPs by means of Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology which enables to realize systematic review and meta-analyses in a specified subject. 172 articles which are published in the 2010-2020 year interval, related to production scheduling and including a mathematical programming model to express scheduling problems have been determined as a result of this methodological review process. These articles have been statistically analyzed according to many features such as year, country, journal, number of stages, type of parallel machines, constraints, objective functions, solution methods, test instances and type of parameters. The results of statistical analyses have been presented through charts so as to provide a visual demonstration to researchers. Furthermore, it has been aimed to answer 14 predetermined research questions by means of analyses realized in the scope of this review study. Consequently, it has revealed the existing literature, recent developments and future research suggestions related to HFSP and therefore it is possible to say that this review paper provides a beneficial road map for researchers studying in this field. Amaç: 3. basamak bir çocuk hastanesinde çocuk cerrahisi polikliniğine başvuran veya konsülte edilen labial füzyonlu hastaları retrospektif olarak değerlendirmeyi amaçladık. Gereç ve Yöntem: 01/01/2005-31/12/2020 tarihleri arasında çocuk cerrahisi kliniğine labial füzyon nedeni ile başvuran veya konsülte edilen 889 hasta tanı anındaki yaşları, başvuru şikâyetleri, hangi kliniklerden çocuk cerrahisine konsülte edildiği, tedavileri, nüks ve komplikasyonlar açısından retrospektif olarak değerlendirildi.. Bulgular: Hastaların yaş ortalaması 2,21(0,1-11) yıl idi. Hastaların çoğu asemptomatik idi (%82,5). Hastaların %72,3’ü ebeveynlerin fark etmesi ile direk çocuk cerrahisi polikliniğine getirildi. Bunun dışında pediatri poliklinikleri, pediatrik endokrinoloji polikliniği, pediatrik nefroloji polikliniği ve çocuk acilden hastalar kliniğimize konsülte edildi. İlk muayenede, 885 hastaya manuel seperasyon ve 4 hastaya cerrahi uygulandı. Tüm hastalara işlemden sonra 15 gün boyunca ılık suya oturma banyosu ve topikal östrojen tedavisi uygulandı. Manuel seperasyon yapılan 80 (%9) hastada nüks tespit edildi. Nüks olan hastaların 78’ine yeniden manuel seperasyon yapılırken, sert fibrotik füzyonu olan iki hastaya (%2,5) ise cerrahi seperasyon yapıldı.Sonuç: Labial füzyon tedavisinde, manuel seperasyon ve topikal östrojenli krem tedavi kombinasyonunu, poliklinik ortamında güvenilir şekilde uygulanabilir olması ve nüks oranın düşük olmasından dolayı önermekteyiz. Sert, kalın ve fibrotik labial füzyonlarda ise cerrahi seperasyon tercih edilmelidir., Aim: We aimed to retrospectively evaluate the patients with labial fusion who presented to or were referred to the pediatric surgery clinic in a tertiary pediatric hospital.Material and Method: Between 01/01/2005 and 31/12/2020, 889 patients admitted or consulted to the pediatric surgery clinic due to labial fusion, age at the time of diagnosis, complaints on admission, which clinic referred the patient to the pediatric surgery clinic, treatments, recurrence, and complications were evaluated retrospectively.Results: The mean age of the patients was 2.21 (0.1–11) years. Most of the patients (82.5%) were asymptomatic. Parents noticed the condition and brought their child directly to the pediatric surgery clinic in 72.3% of the cases. Otherwise, patients were consulted to the pediatric surgery clinic from pediatric clinics, the pediatric endocrinology clinic, the pediatric nephrology clinic, or the pediatric emergency department. During the initial examination, manual separation was performed in 885 patients, and surgery was required for four patients. After the procedures, hydrotherapy with warm water and topical estrogen therapy were applied to all patients for 15 days. Recurrence was detected in 80 (9.0%) patients who were treated by manual separation. Manual separation was performed again in 78 of the patients, while surgical separation was performed in two (2.5%) patients who had severe fibrotic fusions.Conclusion: In the treatment of labial fusion, we recommend the combination of manual separation and topical estrogen cream treatment because it can be applied safely in the clinic, and the recurrence rate is low. Surgical separation is preferred in severe, thick, and fibrotic labial fusions.

Amaç: Çocukluklarda COVID-19 sıklıkla asemptomatik ya da daha selim bir seyre sahip olsa da temas riskinden dolayı sağlık çalışanlarının güvenliği asıl hedef olmalıdır. Çalışmamızda hem hasta hem ameliyathane hem de sağlık çalışanlarının iş güvenliğine ışık tutacak acil ve ertelenemeyen cerrahi olgularında asemptomatik COVID-19 PCR pozitifliğini araştırmayı amaçladık.Gereç ve Yöntemler: Çalışmamız pandemi hastanesine dahil çocuk hastanesinin çocuk cerrahisi kliniğine acil ve ertelenemeyen cerrahi nedenler ile başvuran 18 yaş altı olgular için tasarlanmış olup ileriye dönük yapılmış tek merkezli bir çalışmadır. Çalışmaya Mayıs 2020 -Ekim 2020 tarihleri arası altı aylık izlemde acil ve ertelenemeyen cerrahi yapılan olgular dahil edildi. Belirtilen tarihler arasında başvuran 458 hasta değerlendirildi ve kriterleri karşılayan 433 hasta çalışma kapsamında değerlendirildi. olgulardan preoperatif muayene ve tetkik esnasında COVID-19 için nazofaringeal sürüntü (PCR) örneği alındı.Bulgular: Çalışmaya dahil edilen 433 olgunun 155’i kız (%33.5), 278’i (%66.5) erkekti. Yaş ortalaması 9.5 yıl olarak hesaplandı. Acil vakaların tüm olguların 389’unu (%89.8) oluşturduğu saptandı. Yaş gruplarına göre en sık acil patolojilerin; yenidoğan döneminde intestinal obstrüksiyon veya atreziler, okul öncesi dönemde solunum yoluna yabancı cisim aspirasyonlar, okul çağında akut apandisit olduğu görüldü. Nazal sürüntü PCR sonuçları sadece dört hastada pozitif bulundu, bu oranın tüm çalışma grubunda %0.9 olduğu görüldü, bu olguların hiçbirinde postoperatif komplikasyon ve sağlık çalışanına bulaş saptanmadı.Sonuç: Bu prospektif çalışma ile acil veya ertelenemeyen cerrahi planlanan asemptomatik çocuklarda düşük COVID-19 PCR pozitifliği (%0.9 prevalans) tespit edilmiş olup güvenli prosedür uygulamalarının planlanmasına ışık tutacağı kanaatindeyiz., Objective: Although COVID-19 in childhood is often asymptomatic or has a more benign course, the safety of healthcare workers should be the primary goal because of the risk of contamination. In our study, we aimed to investigate the asymptomatic COVID-19 PCR positivity and its effect on the development of complications in emergency and non-deferred surgery cases that will help both the patient, the operating room and the health care workers.Material and Methods: This is a prospective study that was performed in patients under 18 years of age who were operated on due to emergency and urgent cases in the Ankara City Children’s Hospital, which was serving as an anti-pandemic hospital. The patients were evaluated from May 1 to October 1, 2020. Of the 458 patients, 433 patients who met the criteria were included in the study. COVID-19 Nasopharyngeal swab (PCR) samples were taken from all patients along with preoperative examinations.Results: Of the 433 cases included in the study, 155 (33.5%) were girls and 278 (66.5%) were boys. It was determined that emergency cases constituted 389 (89.8%) of all cases. The most common emergency pathologies according to age groups were intestinal obstruction or atresia in the neonatal period, foreign body aspirations of the respiratory tract in the preschool period, and acute appendicitis in the school age. Nasal swab PCR results were positive in only four patients, this rate was found to be 0.9% in the entire study group, and postoperative complications and transmission to healthcare workers were detected at none.Conclusion: In this prospective study, low COVID-19 PCR positivity (0.9% prevalence) was detected in asymptomatic children scheduled for emergency or non-deferred surgery, and we believe that it will shed light on the planning of safe procedure practices.

Hyperleukocytosis is seen in 5-20% of childhood leukemia cases and it is a poor prognostic factor. Leukapheresis is an effective emergency treatment method by reducing the number of blasts and control of neurological symptoms. Here we report a pediatric patient whose physical status was ASA IVE. He was diagnosed as acute leukemia and had neurologic deterioration together with late recovery from anesthesia after central venous catheterization. Morbidity and mortality of central venous catheterization, which is necessary for leukapheresis treatment, is high. Therefore, all the risks of the disorder and the central catheterization procedure should be described in detail to the family by clinician and the anesthesiologist during preoperative evaluation. We suggest using local anesthesia during this procedure in suitable cases and avoiding sedation or general anesthesia. When there is disorientation, we must remember that this can be a sign of a cerebrovascular event, Hiperlölositoz, çocukluk çağı lösemilerinin %5-20’sinde görülen, prognozu kötü etkileyen bir risk faktörüdür. Lökoferez için uygulanacak santral venöz kateterizasyon morbidite ve mortalitesi yüksek bir girişimdir. İşlemden önce, hastayı takip eden klinisyen ve anestezistin, hastalığın ve santral girişimin tüm risklerini aileye ayrıntılı anlatması gerektiği, işlem sırasında mümkün olan olgularda sadece lokal anestezi uygulanması, sedasyon ve genel anesteziden kaçınılması, hastanın bilinç durumunda bozulma olduğunda serebrovasküler olayların akla getirilerek hızla tanı ve tedavi için değerlendirilmesi gerektiği kanaatindeyiz.

Methemoglobin (MetHb), hemoglobin molekülündeki Fe+2’nin Fe+3 şeklinde okside formda olmasını tanımlar. Konjenital ya da edinsel olabilir. Edinsel methemoglobinemi yapan çok sayıda ajan bilinmektedir. Kan MetHb düzeyi %55’in üzerinde ise fatal seydedebilir. Acil müdahale gereklidir. Burada prilokaine bağlı methemoglobinemi gelişen 4 aylık bir erkek hasta sunulmuştur. Tedavide kullanılan metilen mavisi ile dramatik bir düzelme sağlanmış ve hayat kurtarıcı olmuştur., Methemoglobinemia is a disorder characterized by the presence of a higher than normal level of ferric (Fe+3) rarer man ferrous (Fe+2) hemoglobin. It may be in two forms, congenital or acquired methemoglobinemia. It is known that many agents can cause acquired methemoglobinemia. If the blood methemoglobin level reaches 55%, it can be lethal. Prompt care is needed in this situation. Here, a 4-month-old boy with acquired methemoglobinemia due to prilocaine use is presented. Methylene blue was used in the treatment of this case. The treatment enabled a dramatic improvement and was life saving

Amaç: Obstrüktif megaüreter prenatal hidronefroz saptanan olguların %10’nu oluşturur. Prenatal tanılı olguların artmasıyla birlikte obstrüktif megaüreterli olguların izlemi ve tedaviside son yirmi yılda değişiklikler görülmektedir. Kliniğimizde primer obstrüktif megaüreter tanısı ile cerrahi girişimde bulunulan olgular sunularak tedavi sonuçları literatür bilgileri ışığında tartışılacaktır.Gereç ve Yöntem: 2007-2011 yılları arasında primer obstrüktif megaüreter tanısı ile cerrahi girişimde bulunulan 22 olgu hastane kayıtlarından geriye dönük olarak değerlendirildi. Obstrüktif megaüreter tanısı ile izlenen olgularımızda artan renal pelvis çapı, azalan renal fonksiyon, tekrarlayan idrar yolu enfeksiyonu ve flank ağrısı cerrahi müdahale için aranan şartlar oldu.Bulgular: Sekiz kız, 14 erkek toplam 22 olgu geriye dönük olarak değerlendirmeye alındı. Olguların yaşları ortalama 2.23± 2.42 (1 ay–8 yaş) yıldı . Dört olguda sağ üreter, 17 olguda sol üreterde, bir olguda ise bilateral obstrüksiyon saptandı. 11 olgumuz prenatal saptanan hidronefroz nedeni ile araştırılırken tanı aldı. Soliter böbreği olan üç olgumuza bir yaş altında erken dönemde girişimde bulunuldu. Ürosepsiste olan bir olguya ise üreterokutaneostomi açıldı ve bir yaşından sonra definitif onarım yapıldı. 18 olgumuz açık cerrahi girişim ile üreter çapı azaltıldıktan sonra üreteroneosistostomi yapılarak, dört olgumuz da JJ kateter takılarak izlendi.Tartışma: Primer obstrüktif megaüreter ender karşılaşılan üriner sistem anomalisidir. Tedavisinde halen görüş birliği oluşmamıştır. Özellikle antenatal hidronefroz ile tanınan olgularda izlem tartışmalıdır. Endoskopik olarak JJ katerer takılarak izlenebileceği ifade edilse de serimizdeki olguların çoğunda endoskopik kateter yerleştirmek mümkün olmamıştır. Bu tür olgularda gerekli olduğunda geçici üreterokutaneostomi yapılırken bir yaşın üzerinde üreter çapını azaltarak üreteroneosistostomi yapılması uygun seçenektir. Özellikle soliter böbrekli olgularda erken dönemde açık cerrahi girişim yapılması gerekebilir., Aim: Obstructive megaureter composes 10% of the prenatally diagnosed cases of hydronephrosis. During the recent 20 years, some changes have appeared in the follow-up and treatment of the cases with obstructive megaureter in conjunction to the increase in prenatally diagnosed cases. In this article, cases of obstructive megaureter operated in our clinic has been presented and results of treatment have been discussed on the basis of literature knowledge.Materials and methods: 22 cases operated for the diagnosis of obstructive megaureter between 2007 and 2011 were evaluated retrospectively by the hospital records. Increasing renal pelvis diameter, decreasing renal functions, recurrent urinary tract infection and flank pain were the parameters for indication of surgical intervention in these cases of obstructive megaureter.Results: Total of 22 cases (8 girls and 14 boys) with the mean age of 2.23± 2.42 years (age range: 1 month-8 years) were evaluated retrospectively. Obstruction was determined at the right ureter in 4 cases, at the left ureter in 17 cases, and bilateral in one case. Eleven cases were diagnosed during investigation for prenatally identified hydronephrosis. Early surgical intervention was performed in 3 cases with solitary kidney under the age of one. Ureterocutaneostomy was carried out for one case with urosepsis and definitive operation was conducted after one year of age. Ureteroneocystostomy was performed for 18 cases by reducing the diameter of the ureter, whereas 4 cases were followed up after a JJ catheter was placed. Discussion: Primary obstructive megaureter is a rare urinary system abnormalities. There is no consensus on the treatment of this disease, yet. Follow-up is controversial especially in cases recognized as antenatal hydronephrosis. Although these cases could be followed by endoscopically placing a JJ catheter; this has not been feasible in most of our cases. In such cases, a temporary ureterocutaneostomy can be performed when it is necessary. Furthermore, ureteroneocystostomy by reducing the diameter of the ureter has been a prefered option for cases over one year of age. Open surgery might be necessary especially in cases with solitary kidney at an early stage

In der Wertschöpfung der türkischen Landwirtschaft hat die Obst- und Gemüseproduktion große Bedeutung und Tafel- und Industrietomaten sind wiederum das wichtigste Einzelprodukt unter den Gemüsesorten. Das Papier beschreibt die Struktur und Entwicklung des Sektors seit den 1990-er Jahren und geht auf die Betriebsstruktur in der Primärerzeugung sowie die Situation in Handel, Verarbeitung und Vermarktung ein. Regionale Spezifika werden herausgearbeitet. Eine Analyse der Struktur und Dynamik von Verbraucherpreisen für Tafeltomaten in 22 Provinzen schließt sich an. Die Frage der Integration der Märkte für Tafeltomaten zwischen Provinzen wird mit Hilfe von Schwellen-Fehlerkorrekturmodellen untersucht, wobei handelsbehindernde Transaktionskosten berücksichtigt werden. Die Analyse der bilateralen Integrationsbeziehungen zwischen 22 Provinzen zeigt, dass entlang den Küsten ein Ring von Provinzen existiert, deren Tomatenmärkte eng mit denen anderer Provinzen verbunden sind, während die untersuchten Provinzen im Landesinneren in dieser Hinsicht relativ isoliert sind. Bei einigen Provinzpaaren findet Preistransmission nur dann statt, wenn Abweichungen vom interprovinziellen Gleichgewicht eine bestimmte Schwelle überschritten haben. Tomatoes have the largest share among fruits and vegetables, which add substantially to the gross agricultural product in Turkey. We describe the structure and development of this sector from the year 1990 onwards, covering farms in primal production, trade facilities, processing firms, and marketing channels. An analysis of retail prices for table tomatoes in 22 provinces and their dynamics follows. Finally we employ a threshold vector error-correction model to analyse integration among the markets for table tomatoes in the presence of transaction costs. The results show a ring of integrated provinces along the coasts of Turkey, while the interior provinces are rather separated. In some cases price transmission only occurs when deviations from an interprovincial price equilibrium exceed a certain threshold.

Öz Tez çalışmamızın konusu, TRT'nin yeni radyo kanalı olan TRT FM'in, TRT radyoları içindeki yeri ve yayın politikasının incelenmesidir. Amacımız, TRT'nin gelişen teknolojik şartlara ve toplumun değişen beğenilerine karşı yayın politakasında yaptığı değişikliklere (TRT FM örneği ile) açıklık getirmektir. Temel varsayımımız, TRT FM'in diğer radyolara oranla daha çok bilinen ve dinlenen bir radyo olması ve bunun TRT FM'in yayınlarından kaynaklanmasıdır. Bu varsayımları doğrulamak amacıyla yapılmış olan araştırmaların sonuçlarına tezde yer verilmiştir. Tezin kapsamı TRT radyoları ve TRT FM yayınlarıyla sınırlandırılmıştır. Programların çok ayrıntılı olması sebebiyle yayın politikalarının ayrıntılı bir analizi yapılmamıştır, özel radyolara ve 3984 sayılı kanuna da konunun gelişimi açısından zaman zaman değinilmiştir. Tezde radyo yayınlarının içerik analizleri yapılmıştır, önce TRT radyoları (Radyo 1, Radyo 2, Radyo 3, Turizm Radyosu, Bölge Radyoları ve Türkiye'nin Sesi Radyosu) yayın amaçları, vericileri, haber ve müzik programlan, reklamlar ve program türlerinin oranları açısından incelenmiştir. TRT FM, 24 saat süreyle dinlenerek günlük, haftalık ve aylık programlarının içerik analizleri yapılmıştır. Ayrıca günlük, haftalık ve aylık süreli yayınlar takip edilerek konu ile ilgili haberler teze eklenmiştir. Böylece TRT FM yayınları ile diğer TRT radyolarının yayın politikaları karşılaştırılmış ve farklılıklar ortaya konmuştur. TRT FM kısa sürede en çok bilinen ve dinlenen bir TRT radyosu olmuştur. Bu başarı yayınlarından kaynaklanmaktadır. Bu sebeple TRT'nin diğer radyo kanallarının da toplumun istek ve beklentilerine uygun olarak düzenlenmesinin dinlenirlik oranlarını artıracağı kanaatindeyiz. ABSTRACT The subject of our thesis is the study of the place and the broadcasting policy of TRT FM, the new radio channel of TRT. Our aim is to bring clearness to the changes which TRT has made at its broadcasting policy according to the progressing thechnology and the society's likes. Our main proof is that, the TRT FM, which is one of the best known and listenning to radio than the other radios and this is so because of TRT FM's broadcasts. The thesis includes only the TRT radios and the TRT FM programmes. The analysis of the broadcasting policy is not made widely because the programmes are very detained. To improve the subject the Private Radios and the Law 3984 is also mentioned. At this thesis the analysis of the contents of the broadcasts is made. First of all the broadcating aim of TRT Radios (Radiol, Radio 2, Radio 3, Turism Radio, Region Radios and the Voice of Turkey) are studied according to the variety of their transmitters, news, music and advertisement programmes. The content analysis of the daily, weekly and monthly programmes are made by listenning to the TRT FM for 24 hours. Also the news regarding our subject is added to the thesis by tracing the daily, weekly and monthly programmes for a month. By doing this the broadcats of TRT FM and the broadcast policy of the other TRT Radios are compared. TRT FM has become the best known and most listenning to radio of TRT 1 in a very short time. This success depends on its broadcasts. Because of this, we think that it will increase the listening rate of the TRTs other radios, if they organise their programmes according to the needs and wishes of the society. 103
