Anjiomatoid Fibröz Histiositom(AFH), genellikle çocuklarda ve genç erişkinlerde, nadir olarak görülen bir yumuşakdoku tümörüdür. Litaratürde pek çok farklı yerleşim yeri tanımlanmıştır.Bu olgu sunumunda 4 yaşında erkek hastanınkoltuk altında şişlik ile baş vurması ve operasyondan sonra patolojik olarak AFH tanısı alması tartışılmıştır. Hastanınbütün radyolojik verileri yorumlanmış ve patolojik tanısıyla karşılaştırılmıştır.Sonuç olarak; AFH radyolojik vepatolojik tanılarına yönelik, bu hastalarının değerlendirilmesinde takibinde uygulanması gereken yöntemlerle ilgiliçıkarımlarda bulunulmuştur. , Angiomatoid fibrous histiocytoma (AFH), is a rare soft tissue tumor most commonly occurring in children and youngadult. Many different settlessments are desicribed in literature. In this case report, it has been discussed that a four-yearoldmale patien was admitted with swelling in his armpit ande he had pathologically diagnosis and AFH after surgery.All radiological data of the patient have been interpreted and compared with his pathological diagnosis. As a result,there has been inferences about the methods to be applied in the evaluation and follow-up of these patiens forradiological and pathological diagnoses of AFH.

Treatment of fractures in patients with developmental dysplasia of hip is not clear. In patients without any pre-existing hip pathology, open or closed reduction and internal fixation is used to treat hip fractures. Total arthroplasty is used as a salvage procedure in hip fractures of young patients. Total arthroplasty serves perfect functional results in elderly patients with developmental dysplasia of hip. We presented two multi-trauma patients with developmental dysplasia of hip, and their hip fractures were treated with total arthroplasty. Painless and functional hip joints were obtained, and patients were mobilized early to prevent systemic complications. We think that total arthroplasty is the most convenient treatment method for hip fractures in patients with developmental dysplasia of hip.

14. Bilateral isolated olecranon fractures after trauma: Report of two cases

Olecranon fractures represent approximately 10% of all fractures around the elbow in adults. Olecranon fractures occur by a fall or blunt trauma on the posterior tip of elbow, or by the forces generated by triceps muscle during a fall. Olecranon fractures were seen as unilateral fractures, however bilateral olecranon fractures were seen as pathological fractures. Anatomic restoration of the articular surface, repair of the extensor mechanism and full range of motion with stability are the main goals of treatment in olecranon fractures. Operative management should be performed when there is any articular incongruity or extensor mechanism deficit. Tension band fixation, intramedullary fixation, plating and proximal fragment excision are the main operative techniques which were used for the olecranon fractures. In this study, we reported two cases of bilateral traumatic olecranon fractures in patients without any concomitant systemic disease. Tension band wiring technique was used for the treatment of bilateral olecranon fractures. We preferred transcortical tension band wiring because it serves adequate stabilization and fragment compression with minimal instrumentation. When we reviewed the literature, we could not find isolated bilateral olecranon fractures in a patient without any concomitant disease. Treatment and rehabilitation of bilateral fractures served successful results with tension band wiring as unilateral fractures.

A 42-year-old housewife was admitted to our clinic because of shoulder pain. At X-ray and CT, multiple calcifications and ossifications were seen in the subscapular bursa and glenohumeral joint. MRI findings were consistent with synovial chondromatosis. Surgical treatment was planned but the patient refused. Thus conservative treatment was initiated for pain relief. The shoulder joint is a rare localization in this disorder. Although our patient's symptoms resolved with conservative treatment, we think that it would be better to perform surgical treatment because of the increased size and number of loose bodies and minimal or moderate chondral damage. Hemofili; X'e bağlı resesif geçiş gösteren, faktör 8 veya 9 eksikliği sonucu gelişen, kalıtımsal, nadir görülen bir kanama bozukluğudur. Hemofili 5000-10000 doğumda bir görülür. Hemofili A faktör 8 eksikliği, hemofili B ise faktör 9 eksikliği sonucu gelişir. Hemofili A olguları vakaların %80'ını kapsar. Hastalık seyrinde eklem içi ve kas içi kanama atakları sık görülür. Eklemde tekrarlayan kanama atakları sinovyal membranda inflamasyona sebep olur (kronik sinovit). Bunun neticesinde eklemde gerçekleşen dejenerasyon hemofilik artropati ile sonuçlanır.Çalışmamızdaki amaç; son evre diz hemofilik artropatili hastalara uygulanan total diz artroplastisinin klinik, radyografik ve fonksiyonel orta dönem sonuçlarının cerrahi öncesi dönem ile karşılaştırarak yöntemin etkinliğini belirlemektir.Çalışmamıza Karadeniz Teknik Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji kliniğinde 2012-2020 yılları arasında 11 hastaya uygulanan toplam 14 total diz artroplastisi vakası dahil edilmiştir. Hastalar dosyalar üzerinden retrospektif değerlendirilmiştir. Değerlendirmede Amerikan Diz Cemiyeti skorlaması, Total diz artroplastisi Radyolojik değerlendirme skorlaması ve Visuel Analog Skalası kullanılmıştır. Hemofilik artropati evrelemesi için Arnold- Hilgartner ve Petterson skalası kullanılmıştır. Çalışmamızda elde ettiğimiz sonuçları preoperatif dönem ve postoperatif dönem olmak üzere ayrı ayrı değerlendirip literatür verileri ile karşılaştırdık. Hemofilik artropatili hastalara uygulanan total diz artroplastisinin eklem hareket açıklığını dramatik arttırdığını, ağrı skorlarını belirgin iyileştirdiğini ve fonksiyonel bir diz elde etmede etkin bir tedavi yöntemi olduğunu gördük. Yakın takip, multidisipliner ekip anlayışı ile komplikasyonların en aza indirgenebildiğini gözlemledik. Evaluation of Midterm Outcomes Of Total Knee Arthroplasty In Hemophilic Arthropathic Patients: Retrospective studyHaemophilia is an X-linked, recessive inherited, congenital bleeding disorder caused by a deficiency of coagulation factor 8 or 9. Hemophilia has an estimated frequency of approximately one in 5,000- 10,000 births. Hemophilia A is caused by factor 8 deficiency and hemophilia B is caused by factor 9 deficiency. Hemophilia A is more common than hemophilia B, representing %80 of the total hemophilia population. Intra-articular and intra-muscular bleeding occurs commonly. Recurrent bleeding attacks in the joint cause inflammation of the synovial membrane (chronic sinovitis). As a result progressive degeneration in the joint causes hemophilic arthropathy.The aim of this study is to evaluate the clinical, radiographic and functional midterm results of total knee arthroplasty performed to patients with end stage knee hemophilic arthropathy, compare preoperatively and determine the effectiveness of this method.In the period 2012-2020, 14 total knee arthroplasties were perfomed in 11 men with severe haemophilic arthropathy included in this study. Patients were evaluated retrospectively. American Knee Society scoring, Total knee arthroplasty radiological evaluation scoring and Visuel Analogue Scale were used to evaluation. Arnold-Hilgartner and Petterson scales were used for staging of hemophilic arthropathy.We evaluated the results obtained in our study separately in preoperative and postoperative periods and compared them with the literature datas. We found that total knee arthroplasty performed to patients with hemophilic arthropathy increases joint range of motion, improves pain scores and is an effective treatment for obtaining a functional knee.We observed that complications could be minimized with close follow-up and multidisciplinary team working. 95

ÖZET All-sütür Ankor Materayali İle Onarım Yapılan Rotator Manşet Yırtıklı Hastaların Klinik Sonuçları ve Yerinden Çıkan(Pullout) Ankorların Nedenlerinin AraştırılmasıOmuz eklemi sportif ve günlük yaşamsal aktivitelerin yerine getirilebilmesi açısından önemlidir. Eklem işlevinin büyük bir kısmını rotator manşet kası oluşturmaktadır. Rotator manşet yırtıklarında konservatif ve cerrahi tedavi yöntemleri uygulanmaktadır. Rotator manşet tamirlerinin başarısını artırmak için birçok cerrahi teknik tanımlanmış ve materyal geliştirilmiştir. Bu çalışmanın amacı, all-sütür ankor materyali ile rotator manşet onarımı yapılan hastaların klinik sonuçlarını ortaya koymak, sonuçları literatürdeki diğer ankor materyalleri ile karşılaştırmak ve pullout olduğu durumların nedenlerini belirlemektir. Kliniğimizde, Ocak 2015-Aralık 2018 tarihleri arasında klinik ve radyolojik olarak rotator manşet yırtığı tanısı konulan 131 hastaya all-sütür ankor ile artroskopik tek sıra tekniğiyle rotator manşet tamiri yapılmıştır. Hastaların 79'u kadın (%60,3), 52'si erkek (%39,7) ve ortalama yaşları 60,78 (35-80) yıl olarak hesaplanmıştır. Hastalar ortalama 28,63 ay (14 ay-54 ay) takip edilmiştir. ÖZETAmaç : Bu çalışmada EMA'nın adölesan iskelet gelişimi üzerindeki etkilerinin tespit edilmesi için; EMA'nın adölesan dönemde epifizyel büyüme plağı üzerine olabilecek muhtemel etkilerini araştırmayı amaçladıkMateryal metod: Sıçanlar rastgele 3 eşit gruba ayrıldılar. Kontrol grubuna (K) ilave herhangi bir uygulama yapılmadı. EMA etkisine maruz bırakılan gruptaki (E) sıçanlar her gün 1 saat süreyle kesintisiz 900 MHz EMA etkisine maruz bırakıldı. Sham grubuna (S) her gün bir saat EMA uygulanmaksızın pleksiglass fanus içinde tutuldu. Deney bitiminde sakrifiye edilen sıçanların bacakları diz ekleminden ampute edildi. Preperatta ratgele seçilen 3 farklı bölgeden epifizin total uzunluğu ve her bir zonun ( dinlenme, proliferasyon, hipertrofi) uzunlukları ölçüldü. Ayrıca her bir preperatta rastgele seçilen 3 farklı alanda; epifizyal plağın kahverengi nukleuslu TUNEL (+) kıkırdak hücreleri apopitotik olarak kabul edilerek apopitotik hücrelerin sayısı yüzde olarak ifade edildi.Bulgular : Apopitozun fizis katmanlarından en fazla hipertrofi katmanında gözükmesine rağmen elektromanyetik alana maruz kalan farelerin kontrol gruplarıyla kıyaslandığında hipertrofi katmanlarında apopitoz açısından anlamlı bir ilişki olmadığı görülmekte fakat proliferatif ve dinlenme katmanlarında anlamlı değişiklikler olduğu saptanmıştır. Fizis katmanları arasındaki uzunluklar karşılaştırıldığında toplam fizis katmanlarının uzunluğunun elekromanyetik alana maruz kalmayla değişmediği bunun yanında dinlenme katmanında elektromanyetik alana maruziyet ile anlamlı değişiklikler olduğu gözlemlenmiştir. Elektromanyetik alana maruziyet sonucunda fizisteki apoptoz ve katmanların tümünün uzunlukları arasındaki ilişkiye baktığımızda fizis katmanlarının toplam uzunluğu ile apopitoz arasında anlamlı bir ilişki olmadığı bunun yanında dinlenme katmanında EMA'a maruziyet ile apopitoz ve katman yükseklikleri arasında pozitif yönde ve anlamlı bir ilişki olduğu gözlemlenmiştir. Sonuç : Sonuç olarak EMA, hem apopitozis hem de uzunluk anlamında fizisin bazı katmanlarında anlamlı değişikliklere neden olduğu gözlemlenmiştir. EMA 'nın fizisin normal yapısında değişikliklere neden olduğu açıkça belli olmasına rağmen bunun olumlu ya da olumsuz mu olduğu karmaşık olup bu konudaki çalışmaların daha ileri düzeyde yapılması gerekmektedir. Anahtar kelimeler : Elektromanyetik alan , Büyüme plağı , Apopitozis, Adölosan Rat Purpose: In this study, our aim is to determine the probable effects of EMF's on epiphysial growth plate in adolescent period for determining EMF's effects on adolescent skeleton development and preventing the problems that might be stem from those effects in adultry period.Material/Method: Rats are randomly divided into three equal groups. Any implementation is not applied to first group which is specified as control group (K). Rats are subjected to EMF effect (E) for one hour at same the time of every day. Sham groups rats are kept in bell glass every day for one hour without subjecting to EMF effects. At the end of the experiment, sacrificed rats' leg's knees are amputed. From three different area, total length of epiphysis and each zone's lengths are given in terms of micrometer. Also, In three different area that are randomly chosen at each preparate, chondrocytes are identified in three different zones (dinlenme, proliferasyon ve hipertrofik) of epiphysial plate. Tunel (+) cartilage cells with brown nucleus are accepted as apopitotik and apopitotik cells are stated in terms of percentages.Findings: Despite the fact that apopitoz is observed mostly in hipertrofi layer, rats which are subjected to EMF do not have any meaningful relation in the sense of apopitos in hipertrofi layers compared to control groups. Though, meaningful changes are observed in proliferatif and dinlenme layers. When lengths between fizis layers are compared, It is observed that total lengths of fizis layers do not change with the EMF exposure. On the other hand, In dinlenme layer, meaningful changes are observed with EMF exposure. When we examine the relation between lengths of apoptoz in fizis and lengths of all layers, It is observed that as a result of EMF exposure there is no meaningful relation between total lengths of fizis layer and apopitoz. Besides, a positively and meaningful relation is observed between EMF exposure and apopitoz and layer heights.Result: As a result, EMA has some meaningful effects on some layers of fizis in the context of both apopitozis and length. Although, It is obvious that EMA causes changes on normal structure of fizis, It is complicated to determine whether these are positive or negative and studies in this area are needed to be conducted at advanced levels.Keywords: Electromagnetic field, growth plate, apopitozis, adolescent rat 75

Gelişimsel kalça displazisi olan erişkin hastalarda, cerrahi hazırlık aşamasında hastalık hakkında ayrıntılı bilgi edinilerek, daha etkin cerrahi tedavi uygulanmasına yardımcı olmak amacıyla bu çalışma yapılmıştır. Araştırmaya sadece bir kalça ekleminde yüksekte kalça çıkığı olan (Hartofilakidis Sınıflaması Tip 3) ve diğer kalça ekleminde GKD olmayan 10 hasta dahil edildi. Hastaların hem çıkık hem de sağlam taraflarında femur ve asetabuluma ait birçok uzunluk, açı ve hacim ölçümü yapıldı.Çıkık tarafta femur başı üst noktası ile trokanter minör arası mesafe kısa, trokanter minör ile interkondüler notch arası mesafe ise uzun bulundu. Dolayısı ile trokanter minöre göre uzunluk ayarlaması yapılan total kalça artroplastisi ameliyatlarında bu durumun göz önüne alınması önem arz etmektedir. Çalışmamızda femoral anteversiyon ölçümü, sağlam taraf kalça ekleminde ortalama 33,61, çıkık taraf kalça ekleminde ortalama 26,38 derece olarak bulundu. Trokanter minörün çıkıntısının femura göre rotasyonuna bakıldığında çıkık tarafta belirgin bir şekilde retrovert olduğu belirlendi. Trokanter minörün 20 mm aşağısından itibaren distale doğru gidildikçe çıkık tarafta femoral kanal çapının sağlam tarafa göre daha geniş olduğu tespit dildi. Trokanter minör seviyesinde hem çıkık hem de sağlam tarafta mediolateral çap anteroposterior çaptan fazla, trokanter minörün distalinde ölçüm yapılan tüm seviyelerde ise bu durum tersine dönerek hem çıkık hem de sağlam tarafta anteroposterior çap mediolateral çaptan daha fazla olarak belirlendi. Femur en dar intramedüller kanal çapına bakıldığında çıkık taraf sağlam tarafa göre daha genişti. Ayrıca trokanter minörün 20 mm distal seviyesindeki ortalama korteks kalınlığı, çıkık tarafta anlamlı bir şekilde ince olarak bulundu. Çıkık taraftaki asetabulumun ön arka çapını, ön dudak ve arka dudak kalınlığını, derinliğini, hacmini sağlam tarafa göre daha az bulduk. Sağlam taraf ile çıkık taraf karşılaştırılmasında en büyük orandaki anatomik yetersizlik posterior dudak kalınlığında tespit edildi. Asetabulum hacimlerinin karşılaştırılmasında sağlam taraf asetabulum hacmi, çıkık tarafın 2.3 katı olarak bulundu. Asetabulumun gerek hacim olarak gerekse kemik yapılar olarak farklılıkları TKA esnasında asetabular komponentin uygun bir şekilde uygulanmasının güçlüğünü ortaya koymaktadır.Yüksekte çıkık nedeniyle var olan yapısal anatomik değişikliklere ek olarak vücut postüründen yürüyüş fizyolojisine kadar birçok fonksiyonun bozulması, hastaların çıkık taraftaki ekstremitelerine diğer taraftan daha az yük vermesine ve böylece o taraftaki kemiksel gelişimlerin yetersiz kalmasına neden olmaktadır. Bu durum TKA ameliyatı esnasında göz önünde bulundurulmalı ve oluşabilecek komplikasyonlara karşı tedbirli olunmalıdır. Özellikle TKA planlanan hastalarda daha kolay bir cerrahi teknik uygulayabilmek ve ameliyat sonrasındaki komplikasyon risklerini azaltarak başarıyı arttırabilmek için ameliyat öncesi dönemde cerrahi hazırlık aşamasında çok kesitli bilgisayarlı tomografi çekilmesinin faydalı olabileceğini düşünmekteyiz. This study is aimed to help for performing effective surgical treatment by obtaining detailed information in surgical preparation phase about the disorder in adult patients with developmental dysplasia of the hip (DDH).Patients who have high dislocation only in one and just one hip joint (Hartofilakidis Classification Type 3) were included in this study. Multiple lengths, angles and volumes of femur and acetabulum were measured on both dislocated and normal side of the patients.Our results show that distance between the top of the femoral head and trochanter minor was shorter, whereas the distance between trochanter minor and the intercondylar notch was significantly longer on the dislocated side. Thus, it is important to consider this information in the total hip arthroplasty when using length adjustments based on trochanter minor. In our study, the mean femoral anteversion measurement was found 33.61 degrees on the unaffected side and 26.38 degrees on the dislocated side, respectively. Rotation of trochanter minor according to femur was found significantly retroverted on the dislocated side. Femoral canal diameter was found wider than the unaffected side from 20 mm below the trochanter minor towards the distal end. Mediolateral diameter greater than anteroposterior diameter at the trochanter minor level on both dislocated and unaffected side, anteroposterior diameter was greater than mediolateral diameter on both dislocated and unaffected side at the distal of trochanter minor. The narrowest diameter of the femoral intramedullary canal on the dislocated side was wider than the other side. Also at the level of 20 mm distal to the trochanter minor, mean cortical thickness was also found significantly thin on the dislocated side.Anteroposterior diameter, thickness of anterior and posterior lips, depth and volume of acetabulum on the dislocated side was found lesser than the unaffected side. The most anatomical failure was detected in posterior lip thickness when compared the dislocated side and unaffected side. Volume of acetabulum on the unaffected side was 2.3 times greater than dislocated side. The differences of volume and bone structure of acetabulum reveals the difficulty of appropriate positioning for the acetabular component during total hip arthroplasty.In addition to the structural anatomical changes due to high dislocation, deterioration of many functions such as body posture and walking physiology, the weight load on the dislocated side decreases and this leads to inadequate bone development on the affected side. This situation should be considered for avoiding complications during total hip arthroplasty. Multi-slice computerized tomography assessment may be beneficial in the preoperative period in order to perform an easier surgical procedure, reduce the risk of postoperative complications, and increase the success of surgery, in patients undergoing total hip arthroplasty. 61

Dabigatranın öncü ilacı olan dabigatran etexilate (Pradaxa, Boehringer Ingelheim), yeni geliştirilen, trombini yüksek afinitede ve spesifitede geri dönüşümlü olarak inhibe eden oral bir antikoagülandır. Mevcut kullanımda olan bazı antikoagülan ajanların aksine, sabit dozda uygulanması, yiyecek ve ilaç etkileşimine girmemesi, moniterizasyon gerektirmemesi avantajlarıdır. Gerek oral kullanımından kaynaklanan kullanım kolaylığı gerekse yan etki güvenilirliği nedeniyle giderek artan sıklıkta kullanılmaktadır.Bu çalışmada dabigatran etexilatın kırık iyileşmesi üzerine etkisi, rat tibialarında kırık modeli oluşturularak radyolojik ve histolojik olarak değerlendirilmiştir. Çalışma için 36 adet, ortalama ağırlıkları 250 gr olan, 4-5 aylık Spraque Dawley cinsi, dişi sıçan kullanıldı. Rastgele seçilerek 6'şarlı 6 gruba ayrılan hayvanların sağ tibialarına kırık oluşturuldu ve internal tespit uygulandı. 1. ve 2. gruba dabigatran etexilate %0,5'lik hidroksietilselüloz içinde çözelti haline getirildikten sonra 10 mg/kg olacak şekilde oral gavaj ile sırasıyla 1.gruba 2 hafta ve 2. gruba 4 hafta boyunca günde tek doz olacak şekilde verildi. 3. ve 4. gruba ise benzer şekilde hazırlanan dabigatran etexilate 50 mg/kg olacak şekilde 3. gruba 2 hafta, 4. gruba 4 hafta verildi. 5. ve 6. gruplar ise sham grubu olarak belirlendi. 5. gruba 2 hafta, 6. gruba ise 4 hafta boyunca sadece %0,5'lik hidroksietilselüloz çözeltisi verildi. 1., 3. ve 5. gruplar 14. günde; 2., 4. ve 6. gruplar 28. günde sakrifiye edilerek tibiaları radyolojik ve histolojik olarak değerlendirildi.Radyolojik ve histolojik değerlendirmede 14 ve 28. günlerde ilgili gruplarda farklılık olmadığı; kırık iyileşmesinin normal seyrinde devam ettiği gözlendi.Sonuç olarak radyolojik ve histolojik değerlendirmelerin eşliğinde dabigatran etexilatın ratlarda kırık iyileşmesi üzerine herhangi bir etkisinin olmadığı kanaatine varıldı. Dabigatran etexilate, prodrug of dabigatran, is a new developed anticoagulant which inhibits reversibly thrombin with high affinity and specifity. Despite currently used anticoagulant agents, no monitoring requirement and administration in a fixed dose without drug-drug or food-drug interactions are its main advantages. With these advantages and low side effects its usage has been widened worlwide.In this experimental study we perform a rat tibia fracture model and evaluate the effect of dabigatran etexilate on fracture healing by both radiological and histological. 36 female Spraque Dawley, mean 250 gr weighted female rats was used. Animals divided to 6 groups, each consisting 6 rats then performed right tibial fracture and internal fixation. Oral dabigatran etexilate for rats was a suspention in %0,5 hydroxyethylcellulose and administered to rats via oral gavage. 1st and 2nd groups had 10 mg/kg daily dose respectively for 2 and 4 weeks. As the same way prepared solution 3rd and 4th groups had 50 mg/kg daily dose for 2 and 4 weeks. 5th and 6th groups assigned as sham groups; only hydroxyethylcellulose solution was administered for 2 and 4 weeks. 1st, 3th and 5th groups sacrificied at 2 weeks; 2nd, 4th and 6th groups sacrificied at 4 weeks. Then their right tibias evaluated radiologically and histologically and the results compared.After radiologic and histological evaluation there was no significant difference between drug groups and sham groups at 14 and 28 days and there was no impairment in fracture healing.As a result, we established dabigatran etexilate has no effect on fracture healing in rats according to radiological and histological evaluations. 58

Bu deneysel çalışmada 28 adet Spraque Dawley tipi dişi ratın sağ dizleri kullanıldı. Ratlar kontrol, SF, AS ve HA olmak üzere 4 gruba ayrıldı.Deneysel osteoartrit modeli oluşturmak için; kontrol grubu dışındaki gruplardaki ratlara medial menisektomi uygulandı. Kontrol grubundaki ratlara ise artrotomi sonrası başka bir işlem uygulanmadı.Yeterli kıkırdak hasarını sağlamak için; ratların hareket etmelerine izin verilerek 3 hafta beklenildi. 3. haftanın sonunda SF grubundaki ratların sağ dizlerine 0,1ml serum fizyolojik, AS grubundakilere 0,1ml amniyon sıvısı, HA grubundakilere ise 0,1ml 15mg/ml konsantrasyonunda yüksek molekül ağırlıklı hiyaluronik asit eklem içi injeksiyon yoluyla uygulandı. Bu eklem içi injeksiyonlar aynı şekil ve dozlarda olmak üzere; 4. ve 5. haftaların sonlarında da tekrarlandı.6.hafta sonunda tüm ratlar servikal dislokasyon uygulanılarak öldürüldü. Sağ diz eklemleri alınarak, fiksasyon ve dekalsifikasyon uygulandı. Uygun kesitler Safranin-O boyası ile boyandıktan sonra, ışık mikroskobu altında Mankin skorlama sistemi kullanılarak değerlendirildi.Gruplar arasındaki istatistiksel karşılaştırma Kruskal Wallis varyans analizi (Post- hoc olarak Bonferroni düzeltmeli Mann Whitney U testi) kullanılarak yapıldı. Sonuç olarak AS ve SF grupları arasında anlamlı fark olduğu görüldü (P0,008). İn this experimental study we utilize the right knees of 28 Spraque Dawley type female rats. The rats were divided into 4 groups named as control, SF, AS and HA.İn order to obtain an experimental osteoarthritis model; we perform medial menisectomy to the rats except control group. We did not apply another process after arthrotomy to the rats belong to control group.To supply an adequate cartilage injury; we allow rats to move in a period of 3 weeks. After this period we practise 0,1ml saline solution to the right knees of the rats in SF group, 0,1ml amniotic fluid to the AS group and 0,1ml high molecular weight hyaluronic acid with a concentration of 15mg/ml to the HA group through intraarticular injection. This intraarticular injections were repeated as the same dose and form, at the end of week 4 and 5.At the end of week 6; all rats were euthanized by cervical dislocation. The right knee joints were taken, then fixation and decalcification was applied. The appropriate cross-sections were stained with Safranin-O, then examined under light microscope by using Mankin scoring system.Comparisons among groups were performed by using Kruskal Wallis analysis of variance (Mann Whitney U test with Bonferroni correction as post-hoc test). Objective: This study aimed to evaluate the mid-term clinical outcomes of total knee arthroplasty (TKA) in the management of end-stage hemophilic arthropathy., Methods: Eleven patients (15 knees) undergoing TKA with stiff knees were retrospectively evaluated. TKA was performed in all patients without additional surgical interventions such as posterior capsular release, hamstring release, synovectomy, VY quadricepsplasty, or tibial tubercle osteotomy. All patients were evaluated for clinical and radiological results at follow-ups. Functional evaluation and pain status were assessed using the Knee Society Score and Visual Analogue Scale., Results: The mean age at the time of operation was 40.8 ± 11.8 years (range=30-64 years). The mean follow-up was 51.2 ± 20.6 months (range=24-95). The mean flexion contracture significantly decreased from 17.6 ± 11.3 to 1.7 ± 2.8 degrees, and the mean maximum flexion increased dramatically from 55.6 ± 20.5 to 109.2 ± 16.2 degrees (P < 0.001). Statistical significant improvement in flexion and flexion contracture degrees continued up to the postoperatively 18 months. The mean Knee Society Score increased from 22.7 ± 2.4 points preoperatively to 87.8 ± 3.8 points at the last follow-up (P < 0.001). The mean cost of coagulation factor consumption and blood transfusion accounted for 78% of the total cost., Conclusion: This study has shown that TKA is an effective treatment for relieving pain and improving both ranges of motion and quality of life in managing end-stage hemophilic arthropathy of the knee joint., Level of Evidence: Level IV, Therapeutic Study.

- Abstract
Objectives: This study aims to evaluate traditional open surgery results of osteoid osteomas (OOs) in atypical localizations and explore whether open surgery can be a safe alternative in localizations where radiofrequency ablation (RFA) may not be suitable., Patients and Methods: A total of 26 patients (20 males, 6 females; mean age 23.3±14.2 years; range, 4 to 65 years) having OO in atypical localizations between January 2008 and January 2017 were retrospectively evaluated. All patients underwent traditional open surgery under anesthesia. All patients were followed-up, and their clinical success and complications were evaluated. The pain intensity was assessed using the visual analog scale (VAS)., Results: The postoperative first year VAS score was 0.2±0.5 (range, 0-2) (p<0.005). None of the patients showed any complications. No recurrences were reported at the end of the first year. All patients were relieved from pain due to OO in atypical localizations., Conclusion: Although RFA has been accepted as the preferred treatment for OO, traditional open surgery may still be used for OO in atypical localizations yielding perfect clinical results with fewer complications. It is a safe alternative for localizations such as carpal bones, distal ulna, hand and foot where RFA complications can be seen more frequently. In these localizations, traditional open surgery can be recommended as the first treatment choice.

- Abstract
Neglected patellar and quadriceps tendon rupture is a rare injury, but ipsilateral simultaneous patellar and quadriceps tendon rupture was not described in the literature to our knowledge. In this article, we report a 40-year-old healthy male patient with neglected ipsilateral patellar and quadriceps tendon ruptures treated by peroneus longus tendon autograft. Patient had received some conservative and surgical treatments for patellar fracture before applying to our clinic. After our treatment using peroneus longus autograft and interference nails, patient was immobilized for six weeks in cylindrical cast. Flexion exercises and full weight bearing were started after cast removal. Patient had no complaint at postoperative second year. Patient was a neglected case. Surgical repair and early rehabilitation enabled us to achieve a satisfactory outcome.

- Abstract
Aneurysmal bone cyst (ABC) is a benign lesion that commonly involves the metaphysis or metadiaphyseal region of long bones in children. The WHO defined ABC as an expansile osteolytic lesion consisting of blood-filled spaces and channels that are divided by connective tissue septa that may contain osteoid tissue and osteoclast-like giant cells. ABCs are usually treated with intralesional procedures, and variables including surgeon technique, young patient, juxtaphyseal tumors, and tumor location have been associated with recurrence. Juxtaphyseal lesions breaching the physis have been described. There is only one case of primary ABC of the epiphysis in the literature. We present a second case in a 13-year-old boy who presented with a primary ABC of the distal tibial epiphysis, including diagnostic evaluation and treatment.

Objectives: This study aims to perform an experimental examination of the patellar tendon grafts repaired using an interference screw and free tendon graft., Material and Methods: Twelve knees of the lower limbs which were amputated due to circulatory disease were included in this study. The knees were removed from all soft tissues including patellar tendons. A tendon graft was fixed onto tuberositas tibia through the distal portion of the knee using an interference screw. The patellar tendon graft and tuberositas tibia complex were fixed at the lower and upper part of the testing machine. The distraction force of the bone-tendon-bone complex was recorded using the testing machine., Results: Both patellar and tibial ruptures were detected. The pullout force of the tendons was found to be 810 at maximum and 420 N at minimum. The mean pullout force was 600 N in men and 525 N in women with an overall mean of 575 N., Conclusion: Our study results showed that patellar tendon repair with grafting had a similar pullout force compared to other repair techniques. In case of a neglected rupture of the patellar tendon, in particular, grafting is recommended.

Objectives: In this study, we aimed to evaluate possible effects of leptin on fracture healing., Materials and Methods: Between November 2009 and May 2010, 54 male Sprague Dawley rats with a mean age of 16 weeks and a mean body weight of 323 g (range; 295-352 g) were included and randomly divided into three equal groups. Standardized tibial fractures were created to all rats. Group 1 was treated with 0.5 mL physiological saline, group 2 was treated with 0.1 µg/kg leptin within 0.5 mL physiological saline, and group 3 was treated 0.3 µg/kg leptin within 0.5 mL physiological saline for 14 days intraperitoneally. Each group was divided into three subgroups including six rats for the evaluation at second, third and fifth weeks., Results: Radiological evaluation showed that fracture healing of group 3 was better than group 1 (p=0.014) only at fifth week. Histological evaluation revealed fracture healing of group 3 was better than group 1 at third week (p=0.011). At fifth week, both group 2 and group 3 had better fracture healing than group 1 (p=0.006, p=0.002, respectively). There was no statistically significant difference in fracture healing between group 2 and 3 at fifth week (p=0.176)., Conclusion: Leptin has a positive dose-dependent effect on rat tibial fracture healing.

- Abstract
Objectives: We evaluated the long-term radiological outcomes of the patients with Perthes disease who were conservatively treated with brace., Patients and Methods: Thirty-three hips of 27 skeletally mature patients (21 males, 6 females; mean age 25.2±3.4 years; range 20 to 32 years) treated with modified Thomas splint were included in this study. The results were evaluated according to the Stulberg classification., Results: The mean age of the patients was 6.3±2.2 (range 3-12) years at the onset of the disease and the mean follow-up was 18.6±3.2 (range 7.2-24.1) years. According to onset of the disease, good results were obtained for the patients who were less than six-years-old as 77.7%, between 6-9 years old as 84.2%, after nine years old as 40%. However, 100% of the Herring group A hips, 79.2% of the group B hips and 50% of the group C hips were healed with Stulberg class I or II results., Conclusion: We obtained satisfactory results after conservative treatment with Thomas splint in patients who were between 6-9 years old at onset of the disease with Herring group B involvement. It is hard to say whether the good results were stemmed from having good prognosis of these patients, or stemmed from positive effect of using brace on treatment.

- Abstract
A 14-year-old girl who was engaged in farming was admitted with the complaints of right elbow pain and limited range of motion in the elbow joint. Plain radiograph revealed capitellar osteochondritis dissecans. After the loose fragment was removed from the capitellum and the defect was curetted, a palmaris longus tendon graft harvested from the same hand was implanted into the capitellar defect. Elbow joint was immobilized for three weeks. Magnetic resonance imaging showed that tendon graft was successfully adapted into defect. The range of motion was preserved and she had no pain in the elbow joint. In conclusion, we suggest that tendon autograft can be used in the treatment of capitellar osteochondritis dissecans.

- Abstract
Objectives: In this study, the effects of human amniotic fluid on posterolateral spinal fusion were investigated in a rat model., Materials and Methods: Twenty-eight 20-week-old male Sprague Dawley rats were included in the study. The subjects were divided into two main groups as the control group and the amniotic fluid group. Posterolateral spinal fusion was performed by using autografts in all groups whereas 0.3 ml of centrifuged human amniotic fluid was added to the fusion area in amniotic fluid groups. Groups were further divided into subgroups each including seven rats and evaluated radiologically and histologically at the third and sixth weeks., Results: The radiological fusion observed in the amniotic fluid group was significant at the sixth week when compared to that in the control group. The histological fusion quality was significant in the amniotic fluid groups both at the third and sixth weeks when compared to the control groups., Conclusion: We conclude that human amniotic fluid enhances posterolateral spinal fusion. We believe that the growth factors and hyaluronic acid present in the amniotic fluid played a role in this result.

Objectives: In this study we evaluated the results of combined proximal femoral valgus extension osteotomy and tectoplasty in the treatment of Herring group C Perthes disease with hinge abduction., Patients and Methods: This study was carried out in 11 male patients who underwent combined proximal femoral valgus extension osteotomy and tectoplasty for hinge abduction related to Perthes disease between January 2002 and February 2009. All patients were assessed as group C according to the Herring lateral pillar classification. The mean age at the time of surgery was nine years and one month with an age range of six years to 11 years and two months. All patients had pain and hinge abduction preoperatively., Results: The mean follow-up was 65 months (range 26 to 111 months). In the patients who received a proximal valgus extension osteotomy and tectoplasty, the postoperative femoral head containment was significantly increased radiographically. The postoperative femoral head containment was evaluated on radiographs which were taken one and three months after the surgery. After healing of the osteotomy, one-year and final control graphies were evaluated. We carried out a comprehensive evaluation of preoperative and postoperative radiographs that included measuring; subluxation ratio, femoral head coverage ratio, femoral head size ratio, Sharp's angle, CE (center-edge) angle, neck-shaft angle, caput index and acetabular depth index. Consequently there were significant radiographic healing and improvements from time of initial follow-up to final follow-up., Conclusion: This study has shown that, in the short-term, combined valgus extension osteotomy and tectoplasty relived pain and corrected deformity in patients with hinge abduction. As growth continues the remodelling of hip joint is influenced in a positive way.

Objectives: We investigated the effect of tibiofemoral angle changes on the results of dome osteotomy in patients with medial compartment osteoarthritis of the knee., Methods: The study included 23 knees of 22 patients (15 women, 7 men; mean age 60 years; range 37 to 73 years) who underwent high tibial dome osteotomy for medial compartment osteoarthritis. A simple modified Charnley external fixator was used for stabilization of the osteotomy. Based on the Ahlback classification, 15 knees had grade I, six knees had grade II, and two knees had grade III osteoarthritis. The patients were evaluated in three groups based on the postoperative tibiofemoral angles obtained; hence, a valgus angle of less than 8 degrees (4 knees), 8 degrees to 12 degrees (10 knees), and greater than 12 degrees (9 knees). Clinical evaluations were made using the American Knee Society scoring system. The mean follow-up period was 5.4 years (range 1 to 10 years)., Results: The mean tibiofemoral angle was 4.4+/-2.9 degrees of varus (range 0 degrees to 10 degrees ) preoperatively, and 11+/-3.8 degrees of valgus (range 5 degrees to 18 degrees ) postoperatively. The mean correction loss at final evaluations was 2.7 degrees . The mean preoperative and postoperative knee scores were 37.7+/-15.8 (range 19 to 77) and 80.2+/-9.2 (range 51 to 93), respectively (p<0.001). The mean functional score increased from 52.3+/-18.8 (range 10 to 80) to 75.6+/-18.5 (range 35 to 100) at final analysis (p<0.001). The knee and functional scores did not differ significantly between three groups of patients having a different range of postoperative tibiofemoral angle (p>0.05)., Conclusion: Our results show that the alignment obtained after high tibial osteotomy does not influence knee and functional scores provided that it is within an acceptable range.

Objectives: The aim of the study was to evaluate the results of anterior cruciate ligament (ACL) reconstruction using a peroneus longus tendon (PLT) autograft., Methods: The study included 29 patients (27 males, 2 females; mean age 30 years; range 21 to 39 years) who underwent ACL reconstruction using a PLT autograft and interference nail fixation. Fourteen patients (48.3%) also had partial meniscectomy during surgery. The results were assessed according to the Lysholm scores and the International Knee Documentation Committee (IKDC) scale at the end of at least five years of follow-up., Results: According to the IKDC scale, 17 patients (58.6%) were rated as normal or nearly normal, and 12 patients (41.4%) were rated as abnormal or severely abnormal. The mean Lysholm score was 83.7 (range 45 to 100), with excellent or good results in 23 patients (79.3%). Radiographic examination showed mild (n=10) or moderate (n=1) degenerative changes in the knee joint. Compared with the normal side, no flexion or extension losses occurred in the affected knees. Stability of the ACL was assessed by the Lachman test, which showed normal findings in 12 patients (41.4%), while nine patients had 1+, five patients had 2+, and three patients had 3+ anteroposterior laxity. Pivot-shift test was negative in 13 patients (44.8%); ten patients had 1+ pivot glide, and six patients had 2+ pivot shift. Two patients (6.9%) complained of mild to moderate pressure pain, paresthesia and dysesthesia at the donor site of PLT. No patient experienced ankle joint dysfunction or difficulty in sports activities due to PLT graft transfer., Conclusion: Our data show that PLT can be an appropriate autograft source for ACL reconstruction, avoiding potential complications of autografts obtained from the knee region.
