3. Designing a Model Shoreline: Fourth-Grade Students Create a Habitat for Terrapins While Reducing Erosion

Abstract

This article describes a lesson in which fourth-grade students create a habitat for terrapins while reducing erosion. A client and their needs were presented to the students: The city purchased a piece of shoreline on the bay previously owned by a private company. The company had started to develop the land, exposing soil up to the water's edge. The city council wants to hire an environmental engineering firm to redesign the shoreline area to solve two problems: (1) too much erosion into the bay; and (2) not enough habitat for terrapins. The goals for student teams were: (1) to design a model shoreline that addresses the problems and is as affordable as possible for the city council; and (2) to present their best model to the city council to compete for the job. After a demonstration of models to begin the unit, the students design their own model.

19. The individual and health sector costs of asthma: the first year of a longitudinal study in New South Wales

Abstract

Objective: To identify the resources used and the costs incurred by people with asthma for health care and non-health care products and services to manage asthma. Methods: A prospective, longitudinal study, using self-reported and administrative data, commenced in 2001 in New South Wales (NSW). Data sources included two six-monthly surveys, hospital admission data from NSW Health, and pharmaceutical benefits and Medicare data. A cohort of 245 people with asthma, aged between 5-75 years, was recruited from the general population and from hospital emergency departments. The study measured the use and cost of health care services and products, including alternative therapies and home modifications. Costs to both the health care system and individuals were measured. Results: General practitioner visits and asthma medications were the items of health care most commonly used. Medications were also the largest component of individual costs for health care in terms of the average payment and the number of people facing an out-of-pocket cost, although home equipment and modifications were the most expensive individual items. The distribution of individual costs was highly skewed, ranging from $0 to $4,882 per person per annum (median $89). Admitted hospital care was the largest component of the cost to the health care system. Conclusion: While individual costs were not large for the majority, some people faced substantial costs, with the highest of these being for non-health care items. Implications: Asthma management policies may need to incorporate schemes to alleviate the impact of costs for the minority experiencing significant out-of-pocket expenses.

The Six-Minute Walk Test (6MWT) is widely used as an outcome measure in exercise rehabilitation. However, the repeatability of the 6MWT performed at home in survivors of a critical illness has not been evaluated. Objective. The purpose of this study was to evaluate, in survivors of a critical illness: (1) the repeatability of the 6MWT performed at home, (2) the effect on estimates of change in functional exercise capacity if only one 6MWT was performed at follow-up assessments, and (3) the relationship between the physical functioning (PF) score of the 36-Item Short-Form Health Survey questionnaire (SF-36) and the 6MWT. Design. Repeated measures of the 6MWT and SF-36 were obtained. Methods. Eligible participants had an intensive care unit (ICU) length of stay of ≥48 hours and were mechanically ventilated for ≥24 hours. Two 6MWTs and the SF-36 were conducted in participants' homes at weeks 1, 8, and 26 after hospital discharge. Results. One hundred seventy-three participants completed the study. The participants had a mean age of 57 years (SD=16), a mean Acute Physiology and Chronic Health Evaluation II (APACHE II) score on admission of 19 (SD=10), a mean ICU length of stay of 9 days (SD=8), and a mean mechanical ventilation time of 140 hours (SD=137). Of the 173 participants, 110 performed two 6MWTs at weeks 1, 8, and 26. There were significant mean increases in 6-minute walk distance in the second test of 15 m (P Limitations. Some study participants were unable to perform a second 6MWT, and these participants may have differed in important aspects of function compared with those individuals who completed two 6MWTs. Conclusions. In survivors of a critical illness, the 6MWT in the home environment should be performed twice at each assessment to give an accurate reflection of change in exercise capacity over time. The SF-36 PF score was a strong indicator of 6-minute walk distance in early recovery from a critical illness.

El presente estudio permitió describir la Competencia Matemática Comunicar (CMC) en los estudiantes de grado noveno de la Institución Educativa Libardo Madrid Valderrama (IELMV), mediante un estudio descriptivo en el que se usó como técnicas de investigación la revisión documental y la observación participante a las comunidades de aprendizaje durante el desarrollo de cuatro tareas matemáticas sobre el objeto matemático Ecuaciones Lineales. Con esto fue posible aflorar algunas de las características de esta competencia, entre las que se pueden mencionar los procesos de decodificación, codificación, traducción, descripción, argumentación y persistencia. Igualmente, las comunidades de aprendizaje se convirtieron en un elemento clave para la descripción de la CMC. The present study allowed to describe the communicative mathematical competence (CMC) in ninth grade students of the Libardo Madrid Valderrama Educational Institution (IELMV), through a descriptive study in which documentary review and participant observation was used as research techniques. learning communities during the development of four mathematical tasks on the mathematical object Linear equations. With this it was possible to emerge some of the characteristics of this competence, among which can be mentioned the processes of decoding, coding, translation, description, argumentation and persistence. Likewise, the learning communities became a key element for the characterization of the CMC.

Abstract

Background: There is good evidence of the positive effects of person-centered care (PCC) on agitation in dementia. We hypothesized that a person-centered environment (PCE) would achieve similar outcomes by focusing on positive environmental stimuli, and that there would be enhanced outcomes by combining PCC and PCE. Methods: 38 Australian residential aged care homes with scope for improvement in both PCC and PCE were stratified, then randomized to one of four intervention groups: (1) PCC; (2) PCE; (3) PCC +PCE; (4) no intervention. People with dementia, over 60 years of age and consented were eligible. Co-outcomes assessed pre and four months post-intervention and at 8 months follow-up were resident agitation, emotional responses in care, quality of life and depression, and care interaction quality. Results: From 38 homes randomized, 601 people with dementia were recruited. At follow-up the mean change for quality of life and agitation was significantly different for PCE (p = 0.02, p = 0.05, respectively) and PCC (p = 0.0003, p = 0.002 respectively), compared with the non-intervention group (p = 0.48, p = 0.93 respectively). Quality of life improved non-significantly for PCC+PCE (p = 0.08), but not for agitation (p = 0.37). Improvements in care interaction quality (p = 0.006) and in emotional responses to care (p = 0.01) in PCC+PCE were not observed in the other groups. Depression scores did not change in any of the groups. Intervention compliance for PCC was 59%, for PCE 54% and for PCC+PCE 66%. Conclusion: The hypothesis that PCC+PCE would improve quality of life and agitation even further was not supported, even though there were improvements in the quality of care interactions and resident emotional responses to care for some of this group.
