6. Evaluation of Abdominal Fat Ratio in Patients with Breast Cancer-Related Lymphedema: A Controlled Study.

Abstract

Background: This study aimed to investigate the possible relationship between breast cancer-related lymphedema and central obesity, which is known to be effective in metabolic syndrome and chronic inflammation. Methods and Results: Thirty-six patients (18 lymphedemas, 18 control) enrolled in the study. There was no statistically significant difference in demographic data between the two groups except body mass index (BMI) distribution (p = 0.008). Although 50% of the patients in the study group were obese, this rate was 5.6% in the control group. Although there was a significant difference between the study and control groups in terms of abdominal circumference measurement, there was no significant difference between total and abdominal fat amount and ratios. When BMI and abdominal circumference measurements were evaluated together to predict lymphedema, the area under the curve in abdominal circumference measurement was higher than BMI (0.715 vs. 0.659). In receiver operating characteristic curve analysis, 107 cm of abdominal circumference measurement was determined as cutoff value for lymphedema, with 55% sensitivity and 89% specificity (Youden index: 0.44). Conclusion: Abdominal circumference measurement can be evaluated together with BMI in determining the risk of lymphedema. [ABSTRACT FROM AUTHOR]

Abstract

Diğer tedavi yöntemleriyle tedavi edilemeyen, son dönem karaciğer yetmezliğindeki (SDKY) hastalara önerilen, yaşam süresini ve kalitesini uzatan, tek tedavi yöntemi karaciğer transplantasyonudur. Bu çalışmanın amacı; preop ve postop dönemdeki SDKY hastalarının beslenme durumlarını, biyokimyasal bulgularını ve yaşam kalite düzeylerini değerlendirmektir. Çalışma, Aralık 2014-Mart 2015 tarihleri arasında Özel Şişli Florence Nightingale Hastanesi'ne, karaciğer transplantasyonu olmak üzere yatırılmış, 13'ü kadın, 27'si erkek (n=40), yaş ortalaması 53.1±12.1 yıl olan, karaciğer nakil adayları üzerinde yürütülmüştür. Hastaların kişisel verileri, eşlik eden hastalıkları, transplantasyon şekli, antropometrik özellikleri, biyokimyasal bulguları, beslenme durumları, yaşam kalitesi düzeyleri değerlendirilmiştir. Hastaların enerji ve besin ögeleri alımları, geriye dönük 3 günlük besin tüketim kaydı ile postop 1.ay, postop 3.ay ve postop 6.ay olarak değerlendirilmiştir. Araştırmaya katılan hastaların, postop 3.ayda ve postop 6.ayda günlük ortalama enerji tüketimleri, postop 1.aya göre anlamlı artış göstermiştir (p˂0.05). Subjektif Global Değerlendirme (SGD) sonuçlarına göre preop hastaların, %53.8'i hafif malnütrisyonlu bulunmuştur. Üst Orta Kol Çevresi (ÜOKÇ), triseps ve biseps değerlerine bakıldığında preop dönem ile, postop 7.gün, postop 15.gün, postop 1.aydaki değerler arasında anlamlı bir azalma saptanmıştır (p
