Giriş: Son yıllarda çocuk onkoloji hastalarının yaşam oranlarındaki belirgin düzelme geç yan etkilerin giderek önem kazanmasına neden olmuştur. Retinal toksisite, platin bazlı kemoterapinin akut ve geç yan etkisi olarak görülebilmektedir.Amaç: Platin grubu kemoterapi ilaçlarının neden olduğu retinal toksisitenin araştırılması ve sisplatin tedavisi ile karboplatin tedavisinin olası toksisitesini karşılaştırılmaktır.Materyal ve Metod: Platin grubu kemoterapi almış hastalar tedaviden sonra ortalama 72 (11-150) ay sonra değerlendirilmiştir. Böbrek fonksiyon testleri bozuk olan hastalarla, tanıdan önce göz hastalığı öyküsü olanlar çalışma dışında bırakılmıştır. Tanı yaşı, evresi, patolojisi, uygulanan kemoterapi protokolü, kümülatif karboplatin ve sisplatin dozları, son kemoterapiden sonra geçen süre ve göz muayene verileri (görme keskinliği, kırma kusurları, renkli görme, OCT (RSLK, SFRK), OCTA) retrospektif olarak elde edilmiştir.Kontrol grubu olarak, göz bölümünde muayene edilmiş, bilinen akut ve kronik hastalık öyküsü ve bilinen göz bozukluğu olmayan cins ve yaş olarak uygun 36 çocuk alınmıştır.102Olgu ve kontrol grubunun verileri Mann-Whitney U testi ile karşılaştırılmıştır. Kümülatif kemoterapi dozları ile RSLK ve SFRK değerleri Spearman Korelasyon testi ile analiz edilmiştir.Bulgular: Hastaların 70 gözü ile kontrol grubun 72 gözü çalışmaya dahil edilmiştir. Hasta ve kontrol grubu görme keskinliği, renkli görme, kırma kusuru, OCT ve OCTA verileri yönünden karşılaştırıldığında anlamlı fark saptanmamıştır (p>0.05). Sol göz ortalama SFRK değerleri ile kemoterapinin kesilmesinden itibaren geçen sürenin negatif yönde korele olduğu görülmüştür (p=0,032,s= -0,369). Kümülatif sisplatin dozu arttıkça sol SFRK’nin ve sol nazal superior segmentin RSLK verisinin azalmakta olduğu görülmüştür (p=0,049,p=0,040). Kümülatif karboplatin dozu ile ise SFRK ve RSLK arasında anlamlı ilişki saptanmamıştır. Sonuç: Hastalarımızda kullandığımız platin grubu ilaç dozlarıyla belirgin retinal toksisite gelişmediği görülmektedir. Ancak hasta ve kontrol grubu arasında istatistiksel olarak anlamlı bir fark saptanmasa da kümülatif sisplatin dozu ile SFRK ve RSLK’da azalma gözlenmiş olması sisplatinin karboplatine göre daha fazla retinal toksisiteye neden olabileceğini düşündürmektedir. Sisplatinden sonra sol göz SFRK değerleri ile kemoterapi kesiminden sonra geçen süre arasında negatif korelasyon saptanması platin bazlı kemoterapilerden sonra geçen süre uzadıkça retinal kalınlık değerlerinde azalma olabileceğini ve hastaların görme fonksiyonları açısından yaşam boyu izlenmesi gerektiğini düşündürmektedir.Anahtar Kelimeler: Retinal toksisite, sisplatin, karbopatin Background: According to the improvements in treatment modalities and chemotherapy in the last decades, long term adverse effects became more important. Retinal toxicity is one of the adverse effects of chemotherapy that could be seen both in short and long term.Aim: The aim of this study is to investigate retinal toxicity of platin based chemotherapy and to compare the toxicity between cisplatin and carboplatin.Material and Method: Patients who recieved platin based chemotherapy evaluated after an average of 72 (11-150) months Those with impaired renal functions and previously known ocular disease were excluded from the study. Their age, stage of disease, pathology results, chemotherapy protocols they recieved, cummulative cisplatin and/or carboplatin doses, the duration since last chemotherapy, results of ocular examination (visual acuity, refractive errors, color vision, OCT, RNFL, CFRT, OCTA) has previously been made to investigate long term adverse effects were documented retrospectively from their files.As control group, 36 patients who were previously examined by ophtalmologists for several reasons with no known chronic systemic or ocular diseases and are eligible in age and gender for this study.105Mann-Whitney U test used for the comparisons between patients’ group and control group. Spearman’s Correlation test used for analysing relations between cummulative cisplatin and carboplatin doses and RNFL and CFRT.Results: 70 eyes of patients’ group and 72 eyes of control group were enrolled in this study. No statistically significant difference were found between patients’ and control groups in visual acuity, refractive errors, color vision, OCT and OCTA results (p>0,05). A negative correlation was found between the duration since last chemotherapy and CFRT of the lefte ye. It is found that increase in cummulative cisplatin dose, decreases the CFRT and RNFL in nasal superior quadrant (p=0,049, p=0,040) whereas increase in cummulative carboplatin dose does not effect central foveal thickness or RNFL.Conclusion: According to the results from this study, obvious retinal toxicity does not occur with platin based chemoteraphy within the dosage range our patients recieved. Even though there were no significant difference between patients’ group and control group, decrease in CFRT and RNFL with higher dose of cisplatin led us to think that cisplatin may cause more retinal toxicity than carboplatin does. The negative correlation between CFRT of the left eye and duration since last chemotherapy offers that RNFL might get thinner with time, therefore these patients needs lifelong ophtalmologic follow-up.Keywords: Retinal toxicity, cisplatin, carboplatin
