OBJECTIVE Post-operative radiotherapy (PORT) in non-small cell lung cancer (NSCLC), especially after complete resection, has long been an unresolved dilemma and debated among therapeutic disciplines. We aimed to evaluate the effects of different radiotherapy volumes and techniques on local-regional recurrence patterns and PORT results in patients with NSCLC. METHODS The results of 389 patients who underwent surgery and received PORT at 11 centers were analyzed retrospectively. The surgical margin was positive or closes in 100 (26%) patients. The PORT dose was a median of 50 Gy (36-60 Gy). Intensity-modulated RT methods were used in 68 (17.5%) patients. RESULTS The first recurrence of the patients who developed relapse, local recurrence was found in 77 (19.8%) patients, distant recurrence was found in 95 (24%) patients, and both recurrences was found in 30 (8%) patients. The median time to locoregional relapse was 14 months (1.84-59.7 months). Local-regional recurrence was not significantly higher in patients with positive surgical margins than in negative pa-tients (39% vs. 29%, p=0.1), but the dose administered to these patients was also higher. Mediastinal recurrence occurred in 28 (19%) patients who did not receive radiotherapy to the mediastinum; 25 of these recurrences (89%) were just near or outside the field. Cardiac events became 7% in all groups and did not change according to chosen mediastinal radiotherapy volume. CONCLUSION A clear description of the PORT volumes according to the localization of the primary tumor and the involved lymph nodes would be beneficial in terms of establishing the recurrence/toxicity balance better.

Amaç: Gastrointestinal stromal tümörler (GIST'ler), gastrointestinal sistemin en sık görülen mezenkimal neoplazmlarıdır. GIST' lerin biyolojik davranışı çok değişkendir ve mevcut risk sınıflandırma kriterleri ile malignite potansiyellerini tahmin etmek oldukça güçtür. Bu nedenle, çalışmamızda GIST' li hastalarda sağ kalım sonuçlarını tahmin etmek için Ki-67’nin prognostik önemini analiz etmeyi amaçladık.Yöntem: 2006 ve 2017 yılları arasında cerrahi sonrası tedavi altına alınan hastalar çalışmaya dahil edildi., Tüm vakaların histolojik örneklerini geriye dönük olarak tek patolog tarafından yeniden değerlendirildi. Bulgular: 104 hasta çalışmaya dahil edildi. Ortalama takip süresi 73 aydı (6- 148 ay). Takip süresince 104 hastanın 7' sinde (% 7) lokal nüks, 26' sında (% 25) uzak metastaz mevcut olup 11' i (% 11) ex idi. Tüm hastalar için ortalama genel sağ kalım (OS) 133 (124-140) aydı. Ortalama hastalıksız sağ kalım (DFS) 117 (107-127) aydı. Takip süresince hastalığın progresyonu, rekürrensi ve yaş artışı OS için önemli prognostik faktörlerdi. Mitoz sayısı, büyüme paterni, tümör yerleşimi ve Ki-67 indeksi DFS için önemli prognostik faktörlerdi. Çok değişkenli analizlere göre, Ki-67 hastalığın ilerlemesi veya nüksünü tahmin etmede tek prognostik faktördü (p = 0.04).Sonuç: OS' yi etkileyen en önemli prognostik faktörler; yaş, hastalık progresyonu veya nüx idi. Bu sonuçlar ışığında takip süresi uzadıkça Ki-67’nin genel sağ kalımı da etkileyeceğini düşünüyoruz., Objective: The biological behavior of gastrointestinal stromal tumors (GISTs) varies widely and it is diffucult to predict their malignant potential with the current risk classification criterias. Therefore, we aimed to analyse the prognostic importance of Ki-67 LI for estimating survival outcomes in patients with GISTs.Methods: For the last 11 years, between 2006 and 2017, who had been treated after surgery were included. A single pathologist re-defined the histologic examples of all cases retrospectively.Results: Totally, 104 patients were included in the study. The median follow-up time was 73 months (range; 6 to 148 months). Seven of the 104 patients (7%) had local recurrence, 26 of the 104 patients (25%) had distant metastases and 11 of the 104 patients (11%) died during the follow-up period. The mean OS was 133 (range; 124 to 141) and the mean DFS was 117 (range; 107 to 127) months for patients. The disease progression or recurrence during follow up and increasing age were the significant prognostic factors for OS. Mitotic count, growth pattern, tumor location and Ki-67 LI were the significant prognostic factors for DFS. According to multivariate analyses, the Ki-67 LI was the only prognostic factor for estimating disease progression or recurrence (p=0.04). Conclusion: The most important prognostic factors that affect OS were the age and disease progression or recurrence of disease. Ki-67 LI was the only prognostic factor for estimating disease progression or recurrence. As the follow-up period increases, we think that Ki-67 LI also will affect overall survival.

Amaç: Ocak 2017’ de akciğer kanserinin tümör, nod ve metastaz(TNM) evreleme sınıflamasının 8. versiyonu yayımlanmıştır. Bu araştırmada evreIII küçük hücreli dışı akciğer kanseri (KHDAK) tanısıyla küratifkemoradyoterapi (KRT) uygulanan hastalarda 7. ve 8. evreleme sistemininetkilerini karşılaştırmak ve gruplar arasındaki sağkalım farkını incelemek amaçlanmıştır.Gereçve Yöntemler: Ocak 2010 ve Ocak 2017tarihleri arasında KHDAK tanısıyla başvuran hastalardan klinik evre III kabuledilerek küratif KRT uygulanan hasta verileri geriye dönük incelendi. 8.versiyona göre olan T-evre ve TNM evre değişimleri kaydedildi. Eski ve yeni T-evre,N-evre ve TNM evre kategorilerine göre sağkalım analizleri yapıldı.Bulgular: Toplam 112 evre III KHDAK’li hastanın değerlendirildiği çalışmada33 hastanın (%30) T-evresinde değişme saptanırken, 47 (%42) hastanın TNM evresinde değişiklik olmuştur. İleri yaş,takip sırasında uzak organ metastazı gelişmesi, TNM evresi ve N-evresi sağkalımıetkileyen bağımsız prognostik faktörler olarak bulunmuştur.Sonuç: Sekizinci TNM evreleme sınıflamasına göre tümörboyutu alt gruplara bölünerek T-evresinin yeniden sınıflandırılması lokal ilerievre KHDAK’li hastalarda sağkalımı etkileyen bir parametre olarakbulunmamıştır. Evre IIIa ve IIIc hastalar arasında anlamlı, evre IIIa ve IIIbhastalar arasında anlamlılığa yakın bir sağkalım farkı bulunmasına rağmen evreIIIb ve IIIc hastalar arasında anlamlı sağkalım farkı bulunamamıştır. N3hastalığa sahip olmak tek başına sağ kalımı öngörecek bağımsız prognostik birfaktör olarak saptanmıştır., Aim: The eighth tumour, node and metastasis (TNM) staging system for lungcancer was published in January 2017. To compare the newly released 8thversionof TNM staging system with 7th version of TNM staging system instage III non-small cell lung cancer patients (NSCLC) treated with curativechemo-radiotherapy (CRT) and to evaluate the survival differences between thegroups were the aims of this study.Material and Methods: Between January 2010 and January 2017, stage III NSCLCpatients who were treated with curative CRT were identified retrospectively.Changes in T-stage and TNM-stage were according to 8thstaging systemwere recorded. Survival analysis were performed by using T-stage, N-stage andTNM stage in each staging system, respectively.Results: One hundred and twelve patients were included in this study. Of these 33patients (30%) showed change in T-stage, whereas 47 patients (42%) had changein TNM stage. Advanced age, distant organ metastasis during follow-up, TNM stageand N-stage progression was found as independent prognostic factors affectingsurvival. Conclusion: According to 8th TNM staging system, subdivision of T-stage by tumour size did not affect the survival parametersin locally advanced NSCLC patients. On the other hand, we found significantsurvival difference between stage IIIa and IIIc. In regards of survival, althoughthere was a significant difference between patients with IIIa and IIIc, andalmost significance between patients with IIIa and IIIb, there was no significancebetween stage IIIb and IIIc patients. Having N3 nodal disease was found to bean independent prognostic factor of survival.

Akciğer kanserinde torasik RT' nin asıl amacı tümöre yeterli etkinlikteki dozu uygularken komşu normal dokuları kabul edilebilir oranlarda korumak ve böylece teröpatik oranı en üst düzeyde tutmaktır. Tedaviye bağlı gelişen akut radyasyon özofajiti gbi komplikasyonlar ciddi morbiditeye sebep olurlarken aynı zamanda beklenmedik şekilde tedaviye ara verilmesine de neden olmaktadırlar. Bu durum hastaların yaşam kalitesinin bozulmasına yol açtığı gibi tümör kontrolünün azalmasına ve yaşam süresi oranlarının kısalmasına sebep olmaktadır. Daha etkin tedavi biçimi olan RT ve kemoterapinin birlikte kullanımı ile bu yan ekinin insidansı ve şiddeti de artmaktadır. Bu etkiyi en aza indirgemek amacıyla üç boyutlu konformal RT teknikleri ya da yoğunluk ayarlı RT teknikleri kullanılsa dahi özafagusun tedavi hedefinin çok yakınında olması nedeniyle bu yan etki etkin bir şekilde önlenememektedir. Bu nedenle esas stratejiler akut radyasyon özofajitinin kontrolünde etkili olabilecek radyoprotektör ajanların kullanımına yönelik olmaya başlamıştır.Kanser hastalarında kaslardan sürekli olarak glutamin deplesyonu olmaktadır, bu durum kansere bağlı kaşekside anahtar role sahiptir. Akut radyasyon özofajiti ve buna bağlı bulantı, kusma, ağrı, disfaji ve odynojaji normal doku toleransını daha da bozmaktadır. Bu nedenle Glutamin desteği ile torasik RT' ye bağlı gelişen yan etkilerin azalacağı ve akut radyasyon özofajitinin azalacağı düşünülmüştür.Biz, Gazi Üniversitesi Tıp Fakültesinde küratif amaçlı tedavi ettiğimiz 32 akciğer kanseri tanılı hastayı eş zamanlı ya da tek başına RT ile tedavi ederken hastaların tedaviye bağlı yan etkilerini takip ettik. Bu çalışmadaki esas amacımız torasik RT uygulanan hastalarda oral glutaminin akut radyasyon özofajiti gelişimi ve kilo kaybını önlemedeki etkinliğini değerlendirmektir. İkincil amacımız ise özofajit gelişiminde klinik (serum immünolojik parametreleri ve özafagus sintigrafisi gibi) ve dozimetrik (DVH gibi) parametrelerin rolünü değerlendirmektir. Glutamin alan grupta özofajit şiddeti ve görülme süresi, kilo kaybı, serum sitokin seviyeleri ve özafagus transit zamanları istatistiksel anlamlı olacak şekilde almayan gruptan daha düşük bulunmuştur. Glutamin inflamasyonu baskılaması nedeniyle hem hastalığa hem de tedaviye bağlı gelişen toksisiteyi anlamlı şekilde azaltmıştır.Sonuç olarak, bu çalışmanın sonuçları akciğer kanseri tanısıyla definitif RT uygulanan hastalarda oral glutamin kullanımı ile akut radyasyon özofajit şiddet ve insidansı ile kilo kaybının azaldığını göstermektedir. Bu sonuçlar hastaların hayat kalitelerine olumlu yönde katkıda bulunacaktır. Bu nedenle akciğer kanseri tanısı ile definitif RT uygulanacak hastalara glutamin desteği önerilmektedir. The primary aim of thoracic radiotherapy (TRT) in lung carcinoma is to maximize the therapeutic ratio in order to deliver an effective dose to the tumor while maintaining an acceptable dose to the neighboring normal tissues. However, complications such as acute radiation-induced esophagitis (ARIE) may cause significant morbidity and unplanned treatment delays, These issues may reduce tumor control and survival rates, as well as the patients? quality of life (QOL). The use of more effective regimens combining concomitant radiation and chemotherapy has resulted in increased incidence and severity of this toxicity. To lessen this effect, doses to the esophagus have been minimized by the use of three-dimensional conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy, but toxicity could not be effectively avoided because the proximity of the esophagus target tumor volumes precludes that possibility. Therefore, the primary strategies in controlling ARIE currently revolve around identifying an effective radioprotective agent. In cancer patients, there is a tendency for progressive depletion of glutamine from muscle, which is a key factor in cancer-induced cachexia. ARIE and related complications such as nausea, vomiting, pain, dysphagia, or odynophagiamay furtherworsen the tolerance of normal tissues to radiation. Therefore, it is reasonable to assume that glutamine supplementation may be beneficial in prevention of ARIE and related complications in patients undergoing TRT. This evidence suggests a selective radioprotective role for glutamine, so that, thirty-two patients with lung cancer that thoracic radiotherapy applied alone or concurrent with chemotherapy in Gazi University Medicine School Radiation Oncology Department with curative purposes were fallowed throughout the radiotherapy as regards the side effects of the treatments. The primary objective of the current study was to prospectively evaluate the efficacy of oral glutamine use in the prevention of ARIE and weight loss in lung cancer patients during TRT. Our second objective was to determine the predictive role of clinical (such as; serum immunological parameters and oesophageal transit time) and dosimetric parameters in terms of ARIE incidence and severity. When the toxicity grade, weiğht loss, serum cytokine levels and oesophageal transit time were compared between the group receiving glutamine and group not receiving glutamine, statistically significant improvement was found. Glutamine supress the inflammation related to the disease and treatment and reduce toxicity with statistical significanceIn conclusion, the results of this study suggest a beneficial role for oral glutamine use in prevention and/or delay of ARIE incidence and severity, as well as weight loss in lung cancer patients treated with definitive RT. These findings may potentially contribute positively on treatment results and QOL in patients treated with intensive regimens. Therefore, glutamine can be recommended for patients with lung cancer that receive RT. 116
