1. Female genital mutilation/cutting incidence, diagnostic capacities, and obstetric outcomes among migrant women: a single-center retrospective analysis in a 10-year birth cohort in Austria

Abstract

Abstract Introduction Practical experience in the care of women with female genital mutilation/cutting (FGM/C) is uncommon in Austria. However, affected women require specialized gynecological and obstetric care. In our region, there is currently neither an official counseling center nor specially trained medical personnel to address the special needs of women after FGM/C. The aim of this study was to determine the potential need for obstetric care for women who have undergone FGM/C in our region. Methods We retrospectively reviewed women presenting for delivery at the LKH University Hospital Graz from 1.1.2010 until 31.12.2020 regarding the place of birth and/or the nationality of the mother to filter out women from a country with known FGM/C prevalence according to the UNICEF Global Database. Data on the documentation of FGM/C as well as demographic maternal data and peripartal parameters were gathered. Periods before and after the European refugee crisis in 2015 were compared. Results During the study period, a total of 35,628 deliveries took place at our hospital. 856 (2.4%) deliveries of 539 women were included due to nationality or birthplace in a country with known FGM/C prevalence. We found only 17/539 (3.2%) documented FGM/C cases. The estimated FGM/C prevalence among those patients was, however, 208/539 (38,6%). Women affected by FGM/C in our collective were most frequently from Nigeria, Egypt, Iraq, Ghana, and Somalia. No statistically significant increase in deliveries during the study period in the overall study cohort was observed, with the exception of deliveries of Somali women (p = 0.000). Discussion The discrepancy between documented and expected FGM/C rates (3,2% vs. 38,6%) in our collective suggests that most cases of FGM/C go undetected among women delivering in Austria. These data show the great need for special training for obstetricians and targeted contact points for affected women.

Abstract

Introduction/Background* Numerous societies, including the Austrian Society of Obstetrics & Gynecology (OEGGG) in 2015, have recommended prophylactic bilateral salpingectomy (PBS) at the time of benign gynecologic surgery with the intent of ovarian cancer risk reduction. We evaluated implementation and feasibility of PBS at benign, minimally invasive hysterectomy in public hospitals in the Austrian province of Styria in 2014 vs. 2018 (before and after the official recommendation in 2015). Methodology We reviewed surgical consent forms and operative notes of patients undergoing vaginal or laparoscopic hysterectomy for benign indications in Styria in 2014 and 2018. Ethics approval was obtained. Result(s)* 1,256 benign, minimally invasive hysterectomies were identified (580 in 2014, 676 in 2018). 68% of patients were consented for PBS in 2014 and 94% in 2018 (P Conclusion* PBS at minimally invasive hysterectomy was widely performed in Styria even before the official recommendation in 2015, and increased thereafter to 83% overall in 2018. PBS was accomplished somewhat more often at laparoscopic than at vaginal hysterectomy.

Abstract

This is the first collaborative Enhanced Recovery After Surgery Society guideline for optimal perioperative care for vulvar and vaginal surgeries. An Embase and PubMed database search of publications was performed. Studies on each topic within the Enhanced Recovery After Surgery vulvar and vaginal outline were selected, with emphasis on meta-analyses, randomized controlled trials, and prospective cohort studies. All studies were reviewed and graded according to the Grading of Recommendations, Assessment, Development and Evaluation system. Abstract

Introduction: The purpose of these guidelines is to provide help in decision-making concerning the diagnosis, treatment and follow-up of patients with higher grade perineal tears (perineal tears grades 3 and 4) and thus minimise the risk for persisting complaints. Methods: In 2007 the Austrian Urogynecology Working Group (Arbeitsgemeinschaft fur Urogynakologie und rekonstruktive Beckenbodenchirurgie Osterreich [AUB]) published the “Guidelines for Management of 3rd and 4th Grade Perineal Tears after Vaginal Delivery” („Leitlinie zum Management von Dammrissen III. und IV. Grades nach vaginaler Geburt“). In 2011 the guidelines were updated including literature appearing up to publication date November 30, 2011. The realisation of consensus was supported by the DELPHI method. Results: Evidence-based recommendations founded on consensus were presented on the topics epidemiology, risk factors, classification, diagnosis, surgical management and follow-up of higher grade perineal tears (grade 3 and 4 perineal tears) after vaginal delivery.

Abstract

Die Schnellschnittuntersuchung hat insbesondere bei Ovarialtumoren eine grose Bedeutung, da eine adaquate praoperative histologische Abklarung nicht moglich ist. Die diagnostische Treffsicherheit ist bei primaren Ovarialkarzinomen und benignen Ovarialtumoren am hochsten, bei Metastasen in den Ovarien und seltenen Ovarialtumoren wie Keimzelltumoren am geringsten. Bei Tumoren des Endometriums, insbesondere dem Endometriumkarzinom, sollte die Diagnostik bereits praoperativ mittels Kurettage oder Biopsie erfolgen. Dem intraoperativen Schnellschnitt kommt die Aufgabe der Bestimmung des Tumorstadiums mit der Option zur einzeitigen Lymphadenektomie zu. Bei „Low-grade“-Stromaneoplasien kann dem Schnellschnitt eine diagnostische Rolle zukommen, ebenso bei anderen klinisch malignitatsverdachtigen mesenchymalen Uterustumoren. Die Schnellschnittuntersuchung pelviner Lymphknoten eroffnet die Moglichkeit der einzeitigen paraaortalen Lymphadenektomie. Die Sentinellymphknotenbiopsie ist nur beim Vulvakarzinom von Bedeutung. Die deutsche Arbeitsgemeinschaft Gynakologische Onkologie (AGO) empfiehlt bei Ovarialtumoren mit fraglicher Schnellschnittdiagnose diagnostische Zuruckhaltung und ein zweizeitiges operatives Vorgehen. Im Falle eines Endometriumkarzinoms wird die Schnellschnittuntersuchung des Uterus und der Lymphknoten derzeit ebenso wenig empfohlen wie bei Sentinellymphknoten, aber auch nicht abgelehnt.

Transobturatorische suburethrale Schlingen scheinen eine Alternative zu etablierten retropubischen Verfahren darzustellen. Vergleiche sind durch die Vielzahl an Systemen erschwert. Verletzungen von Blase, Darm und grossen Gefässen scheinen mit transobturatorischen Systemen seltener als mit TVT, Banderosionen und infektiöse Komplikationen dagegen häufiger. Die Ergebnisse ausreichend grosser randomisierter Studien stehen aus.

Abstract

In the study presented here, the aim was to investigate the frequency and clinical significance of bacteremia with urogenital mycoplasmas in immunocompetent patients following gynecological surgery. Among 56 patients undergoing elective hysterectomy (mean age, 55 years; range, 32-82 years), Mycoplasma hominis and Ureaplasma urealyticum were detected in urine by PCR in 2 and 11 patients, respectively. Pre- and postoperative blood samples of the colonized patients were investigated for the colonizing species by PCR. In 4 of the 11 patients colonized by Ureaplasma urealyticum the pathogen was also detected from one of the postoperative blood samples, while no case of bacteremia with Mycoplasma hominis was detected. The postoperative course was uncomplicated in all patients.

Abstract

Midodrine is a potent and selective alpha1-receptor agonist and its potential to increase urethral closure pressure could be useful in the treatment of female stress incontinence. The aim of this randomized double-blind placebo-controlled multicenter study was to evaluate the efficacy and safety of midodrine for the treatment of stress urinary incontinence. The primary criterion of efficacy was the maximum urethral closure pressure at rest. Voiding diaries, symptom and incontinence questionnaires and patient/investigator global assessment were also used to evaluate its efficacy. After 4 weeks of treatment no significant changes in MUCP were found. The global assessment by the patient and investigator did indicate that patients on active treatment had a more positive assessment than the placebo group. In conclusion, midodrine did not cause significant improvements in urodynamic parameters, but there were subjective improvements in some of the patients in the treated groups. Furthermore midodrine was well tolerated.

Abstract

Several mesh repair systems for pelvic organ prolapse (POP) were introduced into clinical practice with limited data on safety, complications or success rates, and impact on sexual function. The Austrian Urogynecology Working Group initiated a registry to assess the use of transvaginal mesh devices for POP repair. We looked at perioperative data, as well as outcomes at 3 and 12 months.Between 2006 and 2010 a total of 20 gynecology departments in Austria participated in the Transvaginal Mesh Registry. Case report forms were completed to gather data on operations, the postoperative course, and results at 3 and 12 months.A total of 726 transvaginal procedures with 10 different transvaginal kits were registered. Intra- and perioperative complications were reported in 6.8%. The most common complication was increased intraoperative bleeding (2.2%). Bladder and bowel perforation occurred in 6 (0.8%) and 2 (0.3%) cases. Mesh exposure was seen in 11% at 3 and in 12% at 12 months. 24 (10%) previously asymptomatic patients developed bowel symptoms by 1 year. De novo bladder symptoms were reported in 39 (10%) at 3 and in 26 (11%) at 12 months. Dyspareunia was reported by 7% and 10% of 265 and 181 sexually active patients at 3 and 12 months postoperatively respectively.The 6.8% rate of intra- and perioperative complications is in line with previous reports. Visceral injury was rare. The 12% rate of mesh exposure is consistent with previous series.

Abstract

Rationale and objectives The authors describe the magnetic resonance (MR) imaging characteristics in patients with gestational trophoblastic disease (GTD) before and after therapy and to correlate these findings with human gonadotropin levels and the specific histology of GTD. Methods Thirteen women (mean age, 30.1 years) with elevated human chorionic gonadotropin (HCG) levels and histologically proven GTD underwent MR examinations of the pelvis. Magnetic resonance imaging was performed on a 1.5-tesla unit. Axial and sagittal proton density-weighted and T2-weighted and sagittal T1-weighted sequences were obtained. Four patients underwent follow-up studies after 4 and 8 weeks to monitor the response to therapy. Gestational trophoblastic disease was histologically proven with curettage in 11 patients and with hysterectomy in two cases. Results Nine patients had a diffusely enlarged uterus with pathologic signal intensities. In four patients, a focal tumor mass was observed. All patients showed loss of the zonal anatomy of the uterus in at least one local area. In 11 patients, no uterus zones could be identified throughout the entire uterus. Pathologic dilated tumor vessels were evident in all patients. In all four cases in which follow-up imaging studies were obtained, uterus size, signal intensities, identification of uterus zones, and uterus vessels returned to normal. Conclusion Magnetic resonance imaging shows trophoblastic tumor infiltration as diffuse uterus enlargement, focal tumor masses, loss of zonal anatomy of the uterus, and pathologic uterine vasculature; this seems to be the most reliable MR imaging finding in patients with GTD. No correlation was found between MR imaging changes and HCG levels or specific histologic types of GTD.

Abstract

Self-expandable metal stents 7 mm in diameter were percutaneously implanted into 13 ureters in 10 patients with malignant ureteral obstruction not amenable to double-J stent placement. In nine ureters, one stent was placed, and in four ureters, two overlapping stents were placed. Primary reconstitution of ureteral patency was achieved in all ureters. After 1-2 weeks, four ureters showed a urothelial reaction encroaching on the lumen of the ureter, and a double-J stent was placed coaxially. One ureter was occluded by urothelial hyperplasia 4 weeks after stent placement, and a double-J stent was therefore placed. One ureter was occluded 8 months after stent placement by distal tumor overgrowth. The other ureters showed no signs of obstruction during a follow-up of 3-14 (average, 5.8) months. Peristalsis was preserved at both ends of the stent in all ureters. The use of this stent alone or in combination with a double-J stent alleviated upper urinary tract obstruction and avoided external drainage in all patients.

Abstract

Serial magnetic resonance imaging studies were performed in 28 patients undergoing primary radiation therapy for invasive cervical cancer. T2-weighted spin-echo pulse sequences with long repetition times (2,500 msec) and echo times (30-100 msec) were used at a field strength of 1.5 T. Eighteen tumors responded promptly to radiation therapy with a volume reduction and significant decrease of signal intensity in the early posttreatment phase (1-3 months) and with total tumor regression at 1-6 months (immediate responders). At 6 months seven tumors were visible as residual tumors with declining signal intensity; all seven of these tumors had resolved at 9 months (delayed responders). Thus, a delayed response with residual tumor at 6 months was still compatible with subsequent clinical cure. The tumors showed progression and no marked change in signal intensity (nonresponders). Primary tumors with a volume of more than 50 cm3 were more likely to have no or delayed response. An early (2-3 months) and significant decrease in the signal intensity and volume of a tumor indicates a favorable response. Large primary tumors may show a delayed response.
