Abstract

BACKGROUND: Mid-treatment cross-border migration of patients with TB increases the risk of treatment interruption. OBJECTIVE: To establish a cross-border referral process for patients with TB in Japan, and enhance their access to health facilities and treatment outcomes. DESIGN: This prospective cohort study describes and assesses the process of foreign-born patients with TB who returned to their home countries during treatment, focusing on their access to healthcare facilities and treatment outcomes. RESULTS: We enrolled 135 foreign-born patients with TB, and confirmed that 112 (83.0%) were referred to and accessed healthcare facilities after returning to their home countries. Of 102 patients due to complete treatment as of July 2023, 87 (85.3%) completed their treatment. We did not identify significant differences in the treatment success rate among patient characteristics, except between the patients with confirmed access to a healthcare facility and those without (P < 0.001). We confirmed that 49/87 (56.3%) patients had completed treatment with official data. CONCLUSION: The access and treatment success rates of the cross-bordered patients with TB from Japan were >80%; however, we should further improve this proportion by confirming the treatment outcomes with official data.

Abstract

We reconstructed surface mass balance (SMB) around Dome Fuji, Antarctica, over the last 5000 years using the data from 15 shallow ice cores and seven snow pits. The depth–age relationships for the ice cores were determined by synchronizing them with a layer-counted ice core from West Antarctica (WAIS Divide ice core) using volcanic signals. The reconstructed SMB records for the last 4000 years show spatial patterns that may be affected by their locations relative to the ice divides around Dome Fuji, proximity to the ocean, and wind direction. The SMB records from the individual ice cores and snow pits were stacked to reconstruct the SMB history in the Dome Fuji area. The stacked record exhibits a long-term decreasing trend at -0.037±0.005 kg m−2 per century over the last 5000 years in the preindustrial period. The decreasing trend may be the result of long-term surface cooling over East Antarctica and the Southern Ocean and sea ice expansion in the water vapor source areas. The multidecadal to centennial variations of the Dome Fuji SMB after detrending the record shows four distinct periods during the last millennium: a mostly negative period before 1300 CE, a slightly positive period from 1300 to 1450 CE, a slightly negative period from 1450 to 1850 CE with a weak maximum around 1600 CE, and a strong increase after 1850 CE. These variations are consistent with those of previously reconstructed SMB records in the East Antarctic plateau. The low accumulation rate periods tend to coincide with the combination of strong volcanic forcings and solar minima for the last 1000 years, but the correspondence is not clear for the older periods, possibly because of the lack of coincidence of volcanic and solar forcings or the deterioration of the SMB record due to a smaller number of stacked cores.

Abstract

The retrieval of continuous ice core records of more than 1 Myr is an important challenge in palaeo-climatology. For identifying suitable sites for drilling such ice, knowledge of the subglacial topography and englacial layering is crucial. For this purpose, extensive ground-based ice radar surveys were carried out over Dome Fuji in the East Antarctic plateau during the 2017/18 and 2018/19 austral summers by the Japanese Antarctic Research Expedition, on the basis of ground-based radar surveys conducted over the previous ∼ 30 years. High-gain Yagi antennae were used to improve the antenna beam directivity, thereby significantly decreasing hyperbolic features of unfocused along-track diffraction hyperbolae in the echoes from mountainous ice–bedrock interfaces. We combined the new ice thickness data with the previous ground-based data, recorded since the 1980s, to generate an accurate high-spatial-resolution (up to 0.5 km between survey lines) ice thickness map. This map revealed a complex landscape composed of networks of subglacial valleys and highlands. Based on the new map, we examined the roughness of the ice–bed interface, the bed surface slope, the driving stress of ice and the subglacial hydrological condition. These new products and analyses set substantial constraints on identifying possible locations for new drilling. In addition, our map was compared with a few bed maps compiled by earlier independent efforts based on airborne radar data to examine the difference in features between datasets. Our analysis suggests that widely available bed topography products should be validated with in situ observations where possible.

We studied a patient who had recognition difficulty for manipulable objects. MRI showed a lesion in the left occipito-parietotemporal area. Differential diagnosis of agnosia, aphasia and apraxia is discussed. We believe this “object meaning amnesia” constitutes a distinct subtype of semantic amnesia.

Abstract

Samples of the carbonaceous asteroid Ryugu were brought to Earth by the Hayabusa2 spacecraft. We analyzed 17 Ryugu samples measuring 1 to 8 millimeters. Carbon dioxide–bearing water inclusions are present within a pyrrhotite crystal, indicating that Ryugu’s parent asteroid formed in the outer Solar System. The samples contain low abundances of materials that formed at high temperatures, such as chondrules and calcium- and aluminum-rich inclusions. 20. Prognostic impact of atrial fibrillation in patients with hypertrophic cardiomyopathy in a community-based Japanese cohort: results from Kochi RYOMA study

Abstract

Background The prognostic impact of atrial fibrillation (AF) in patients with hypertrophic cardiomyopathy (HCM) is not fully elucidated. Purpose The aim of this study was to examine the prevalence and prognostic impact of AF in a prospectively assembled community-based HCM patient cohort in an aged Japanese community. Methods In 2004, we established a cardiomyopathy registration network in Kochi Prefecture, Japan, consisting of 9 hospitals, and finally 293 patients with HCM were followed. Results The ages at registration and at diagnosis were 63±14 and 56±16 years, respectively, and 197 patients (67%) were men. 86 patients (29%) showed AF. During follow-up period of 6.1±3.2 years, 44 patients died. In those patients, HCM-related deaths occurred in 23 patients with an annual mortality rate of 1.3%. Regarding HCM-related adverse events including HCM-related deaths, appropriate ICD discharge, heart failure admission and hospitalization for embolic events, a total of 77 cardiovascular events in 70 patients occurred. Multivariate analysis revealed that presence of AF, left ventricular (LV) outflow obstruction, NYHA functional class III, and lower LV fractional shortening at registration were significant predictors of these adverse events. During the follow-up period, additional 31 patients (11%) developed new-onset AF. Importantly, the incidence of HCM-related adverse events was significantly higher in patients with new AF observed from its onset compared with those with AF at registration (log-rank p=0.029) (Figure 1). Conclusions In an unselected HCM registry in an aged Japanese community, presence of AF, particularly new-onset AF, was associated with unfavorable clinical outcomes. 22. Measurements and correlation of diffusion coefficients of ibuprofen in both liquid and supercritical fluids

Abstract

The diffusion coefficients of ibuprofen in both liquid and supercritical fluids are presented for the first time. The diffusion values in liquid ethanol were measured by the Taylor dispersion method at temperatures from 298.15 K to 333.15 K and pressures from 0.10 MPa to 30.00 MPa. Those in supercritical CO2 were carried out by the chromatographic impulse response method at 308.15 K–353.15 K and up to 40.00 MPa. The determined diffusion activation energies in supercritical CO2 were Ea =18.6 kJ/mol to 11.0 kJ/mol at P = 15.00 MPa to 35.00 MPa, and those in liquid ethanol were Ea =16.3 kJ/mol to 15.1 kJ/mol at P = 0.10 MPa to 30.00 MPa, respectively. All the diffusion data of ibuprofen in liquid ethanol and in supercritical CO2 were well correlated by the hydrodynamic equation proposed in this study, with an average absolute relative deviation of 2.2 % for 115 data points.

Objectives To estimate the annual prevalence of different diagnostic categories by age, breed and sex in insured cats in Japan for which veterinary care claims had been made, and to identify if there is a pattern in these host factors. Materials and Methods Data from 48,187 cats insured for veterinary care in Japan in the period from April 2012 to March 2013 comprising 26,003 males and 22,184 females were analysed to calculate the annual prevalence of 18 diagnostic categories of disease by age, breed and sex. Results The prevalence was highest for urinary system disorders (12·2% for males and 10·0% for females), followed by digestive disorders (11·6% for males and 10·7% for females) and dermatological diseases (8·7% for males and 9·0% for females). The male cats had a higher prevalence than female cats for most diagnostic categories. The prevalence of cardiovascular, urinary, endocrine and neoplastic disorders increased with age; infectious and parasitic diseases had high prevalence at young ages, and the prevalence of respiratory, musculoskeletal disorders and injuries had bimodal peaks. Dermatological disorders had a high prevalence at all ages. A large variation in prevalence was observed between breeds for otic, dermatological, dental and cardiovascular disorders. Clinical Significance The findings can be used to increase awareness of patterns of health disorders in different categories of cat.

Abstract

SUMMARYJapan has been free from rabies since 1958. A strict import regimen has been adopted since 2004 consisting of identification of an animal with microchip, two-time rabies vaccination, neutralizing antibody titration test and a waiting period of 180 days. The present study aims to quantitatively assess the risk of rabies introduction into Japan through the international importation of dogs and cats and hence provide evidence-based recommendations to strengthen the current rabies prevention system. A stochastic scenario tree model was developed and simulations were run using @RISK. The probability of infection in a single dog or cat imported into Japan is estimated to be 2·16 × 10−9 [90% prediction interval (PI) 6·65 × 10−11–6·48 × 10−9]. The number of years until the introduction of a rabies case is estimated to be 49 444 (90% PI 19 170–94 641) years. The current import regimen is effective in maintaining the very low risk of rabies introduction into Japan and responding to future changes including increases in import level and rabies prevalence in the world. However, non-compliance or smuggling activities could substantially increase the risk of rabies introduction. Therefore, policy amendment which could promote compliance is highly recommended. Scenario analysis demonstrated that the waiting period could be reduced to 90 days and the requirement for vaccination could be reduced to a single vaccination, but serological testing should not be stopped.

Abstract

Modification of an electroporation buffer using Ca(NO3)2 and elevated pH (9–10) appeared to have a favorable effect on gene transfer to creeping bentgrass (Agrostis palustris Huds.) Penncross protoplasts, resulting in an increase in the transformation frequency of about twofold. Following electroporation with the plasmid pARK22 containing the bar gene, a total of 278 bialaphos-resistant cell colonies were obtained from four experiments. The bialaphos-resistant regenerants proved to be transgenic by Southern hybridization of the amplified DNA. All the tested transgenic plants showed herbicide (HERBIE) resistance at the field rate of 0.5–1% (vol/vol). Ammonia contents in leaves after spraying with the herbicide increased less in transgenic plants than in untransformed control plants.

Abstract

Data of 98 rabies cases in dogs and cats from the 1948-1954 rabies epidemic in Tokyo were used to estimate the probability distribution of the incubation period. Lognormal, gamma and Weibull distributions were used to model the incubation period. The maximum likelihood estimates of the mean incubation period ranged from 27.30 to 28.56 days according to different distributions. The mean incubation period was shortest with the lognormal distribution (27.30 days), and longest with the Weibull distribution (28.56 days). The best distribution in terms of AIC value was the lognormal distribution with mean value of 27.30 (95% CI: 23.46-31.55) days and standard deviation of 20.20 (15.27-26.31) days. There were no significant differences between the incubation periods for dogs and cats, or between those for male and female dogs.

Abstract

Intracellular redox state is a critical factor for fundamental cellular functions, including regulation of the activities of various metabolic enzymes as well as ROS production and elimination. Genetically-encoded fluorescent redox sensors, such as roGFP (Hanson, G. T., et al. (2004)) and Redoxfluor (Yano, T., et al. (2010)), have been developed to investigate the redox state of living cells. However, these sensors are not useful in cells that contain, for example, other colored pigments. We therefore intended to obtain simpler redox sensor proteins, and have developed oxidation-sensitive fluorescent proteins called Oba-Q (oxidation balance sensed quenching) proteins. Our sensor proteins derived from CFP and Sirius can be used to monitor the intracellular redox state as their fluorescence is drastically quenched upon oxidation. These blue-shifted spectra of the Oba-Q proteins enable us to monitor various redox states in conjunction with other sensor proteins.

Abstract

The Yb-based heavy-fermion compound Yb2Ni12P7 with a hexagonal Zr2Fe12P7-type crystal structure was investigated by using the 31P nuclear magnetic resonance (NMR) technique. The complicated NMR line changes its shape gradually with decreasing temperature, implying the presence of some Knight shift components. The temperature dependences of the Knight shift and the nuclear spin-lattice relaxation rate 1/T 1 suggest the delocalization of 4f electrons.

Abstract

Some of the most common climacteric symptoms are somatic symptoms, such as muscle and joint pain, which cause limitations in performing daily tasks. This study aimed to assess whether oral porcine placental extract (PPE) affects patients with shoulder stiffness, one of the most common types of muscle-tendon stiffness, in climacteric women and postmenopausal women with hormone therapy (HT).Sixty-six climacteric women (33 each in the control and study groups) with shoulder stiffness and 54 postmenopausal women (27 each in the control and study groups) with HT-resistant shoulder stiffness were enrolled into this open-label, randomized, controlled study. The control subjects received Toki-shakuyaku-san (TJ23; 7.5 g/day, daily per os), an oral herbal remedy used to alleviate climacteric symptoms, and study subjects received PPE orally for 24 weeks in climacteric women and for 12 weeks in postmenopausal women with HT. Changes in the degree of shoulder stiffness were evaluated by the Visual Analog Scale (VAS).Treatment with PPE was significantly effective in reducing the VAS score for shoulder stiffness during the study period compared with the control group in both climacteric and postmenopausal women with HT. The VAS score at the end of treatment was significantly reduced (p0.01 vs. baseline) by 76.4% for climacteric women and 64.8% for postmenopausal women with HT compared with baseline.PPE is useful as an oral supplement for shoulder stiffness in climacteric women without HT or in postmenopausal women with HT-resistant, long-term shoulder stiffness.

Abstract

We develop a highly efficient quasi-phase-matched adhered ridge waveguide (QPM-ARW) in LiNbO3 as a nonlinear material, and demonstrate tunable wavelength conversion without spectral inversion (SI) and parametric tunable dispersion compensation for a single-mode fiber (SMF) link. The QPM-ARW module with a second harmonic generation efficiency of 700 %/W achieves tunable wavelength conversion with a wavelength-tuning range of at least 25 nm through cascaded sum- and difference-frequency generation (SFG-DFG) process in which the signal and pumps are located symmetrically around the phase matching wavelength. The power penalty of the wavelength conversion is less than 0.6 dB for 43-Gb/s nonreturn-to-zero on-off-keying (NRZ-OOK) signals. We then apply the tunable wavelength conversion without SI to the parametric tunable dispersion compensation scheme, and achieve successful optical tunable dispersion compensation in 43-Gb/s NRZ-OOK transmissions over 53.2-km SMF.

Abstract

Injections of human placental extract have long been used to treat menopausal symptoms. Recently, porcine placental extract (PPE), an oral supplement, has been developed for this purpose. The aim of this study was to assess whether PPE has an impact on climacteric symptoms in perimenopausal and postmenopausal women.Seventy-six women with climacteric symptoms were enrolled into this open-label, randomized, controlled study. The control group (n = 38) underwent 24 weeks of open treatment with Toki-shakuyaku-san (TJ23), an oral herbal remedy used to alleviate climacteric symptoms. The PPE group (n = 38) received three capsules of PPE/day orally for the initial 12 weeks and six capsules/day for the next 12 weeks. Climacteric symptoms were evaluated in both groups using the Simplified Menopausal Index (SMI) score, Zung's Self-Rating Depression Scale (ZSDS) and the Spielberger State-Trait Anxiety Inventory (STAI) before commencing treatment, after 12 weeks of treatment and on completion of treatment.Treatment with PPE was significantly (p0.01) more effective in reducing the SMI, ZSDS and STAI measures at 12 and 24 weeks than TJ23 treatment alone. Treatment with PPE was also significantly effective (p0.01) in reducing the subscale scores of the SMI for items such as hot flushes, insomnia, irritability, depression, fatigue and joint pain. PPE treatment had no significant adverse effects.Oral PPE treatment is another possible option for treating perimenopausal and postmenopausal women with climacteric symptoms.

47. Quantitative risk assessment of the introduction of rabies into Japan through the importation of dogs and cats worldwide – ADDENDUM

Abstract

Japan has been free from rabies since 1958. A strict import regimen has been adopted since 2004 consisting of identification of an animal with microchip, two-time rabies vaccination, neutralizing antibody titration test and a waiting period of 180 days. The present study aims to quantitatively assess the risk of rabies introduction into Japan through the international importation of dogs and cats and hence provide evidence-based recommendations to strengthen the current rabies prevention system. A stochastic scenario tree model was developed and simulations were run using @RISK. The probability of infection in a single dog or cat imported into Japan is estimated to be 2·16 × 10-9 [90% prediction interval (PI) 6·65 × 10-11-6·48 × 10-9]. The number of years until the introduction of a rabies case is estimated to be 49 444 (90% PI 19 170-94 641) years. The current import regimen is effective in maintaining the very low risk of rabies introduction into Japan and responding to future changes including increases in import level and rabies prevalence in the world. However, non-compliance or smuggling activities could substantially increase the risk of rabies introduction. Therefore, policy amendment which could promote compliance is highly recommended. Scenario analysis demonstrated that the waiting period could be reduced to 90 days and the requirement for vaccination could be reduced to a single vaccination, but serological testing should not be stopped.

Abstract

On 10 September 2001, the first case of bovine spongiform encephalopathy (BSE) in Japan was confirmed in a five-year-old dairy cow born in Hokkaido and raised in Chiba prefecture. Subsequently, BSE surveillance was enhanced. As a result, 35 additional cases were detected by the end of March 2009, with two to ten cases being detected each year up to the end of 2007. The epidemic appeared to peak in 2006. Cases are detected mostly in dairy cattle; in cattle born in 1996 and 2000; and in cattle born in Hokkaido. Two were atypical cases of BSE and the remainder classical cases. This paper describes and discusses the demographic structure of dairy and beef cattle, surveillance programmes in place and the epidemiological features of the BSE epidemic in Japan.

Abstract

Recently Japan had three outbreaks of avian influenza (Al) in 2004, 2005 and 2007. An outbreak of highly pathogenic avian influenza (HPAI) was recorded in early 2004, the first for 79 years, with four farms being infected with HPAI virus subtype H5N1. In 2005, 41 farms were found to be infected with AI virus subtype H5N2. In early 2007, four farms were infected with HPAI virus subtype H5N1 again. In all of these outbreaks, the disease was eradicated without resorting to vaccination, through a campaign of culling, movement control of chickens in areas around infected premises, and intensive clinical and serological surveillance. This paper describes the nature of the outbreaks, the eradication measures implemented, clinical and serological surveillance techniques used, and the possible sources of infection.

Abstract

Background Soy sauce is well-known as a Japanese traditional seasoning, namely shoyu. Usually, shoyu means sauce made from soy. Shoyu does have not only benefits but also adverse effects. Soy sauce allergy which is not caused by soy or wheat allergy is rare. Patients Our four patients developed cellulites and dermatitis around lips with irritation after a meal with shoyu. The age of the patients was 10, 35, 46 and 51 years; they were all female. Objectives These inflammations can be developed by two causes; first it can be caused by allergic reactions to shoyu; the second, it can be caused by histamine poisoning. It is important to determine whether inflammation is caused by allergic reactions or histamine poisoning. Methods We determined the volume of histamine in some sauces and performed prick test and laboratory tests. Results Four patients had positive reactions by prick test after using some sauces. We suspected that histamine caused their symptoms, but nine normal volunteers had negative reactions. Patient’s specific IgE score to soy and wheat was class 0. The results showed that the sauce made from soybean and broad bean contained histamine, but histamine in other sauces was not detected. Conclusions In this study, we confirmed by prick test, four cases of soy sauce allergy, which was caused by some products during brewing. When patients with inflammations around mouth, after a meal containing or using soy sauce, are examined, it should be considered whether dermatitis or cellulites were developed by allergic reaction or by histamine poisoning.
