1. Impact of COVID-19 during pregnancy on placental pathology, maternal and neonatal outcome – A cross-sectional study on anemic term pregnant women from a tertiary care hospital in southern India

Abstract

BackgroundSARS-CoV-2 infection during pregnancy may cause adverse maternal, neonatal and placental outcomes. While tissue hypoxia is often reported in COVID-19 patients, pregnant women with anemia are suspected to be more prone to placental hypoxia-related injuries.MethodsThis hospital-based cross-sectional study was conducted between August-November 2021, during COVID-19 second wave in India. Term pregnant women (N=212) admitted to hospital for delivery were enrolled consecutively. Since hospital admission mandated negative RT-PCR test for SARS-CoV-2 virus, none had active infection. Data on socio-demography, COVID-19 history, maternal, obstetric, and neonatal outcomes were recorded. Pre-delivery maternal and post-delivery cord blood samples were tested for hematological parameters and SARS-CoV-2 IgG. Placentae were studied for histology.ResultsOf 212 women, 122 (58%) were seropositive for SARS-CoV-2 IgG, but none reported COVID-19 history; 134 (63.2%) were anemic. In seropositive women, hemoglobin (p=0.04), total WBC (p=0.009), lymphocytes (p=0.005) and neutrophils (p=0.02) were significantly higher, while ferritin was high, but not significant and neutrophils to lymphocytes (p=0.12) and platelets to lymphocytes ratios (p=0.03) were lower. Neonatal outcomes were similar. All RBC parameters and serum ferritin were significantly lower in anemic mothers but not in cord blood, except RDW that was significantly higher in both, maternal (p=0.007) and cord (p=0.008) blood from seropositive anemic group compared to other groups. Placental histology showed significant increase in villous hypervascularity (p=0.000), dilated villous capillaries (p=0.000), and syncytiotrophoblasts (p=0.02) in seropositive group, typically suggesting placental hypoxia. Maternal anemia was not associated with any histological parameters. Univariate and multivariate logistic regression analyses of placental histopathological adverse outcomes showed strong association with SARS-CoV-2 seropositivity but not with maternal anemia. Abstract

In this paper, we propose a deep learning assisted soft-demodulator for multi-set space-time shift keying (MS-STSK) millimeter wave (mmWave) systems, where we train a neural network (NN) to provide the soft values of the MS-STSK symbol without relying on explicit channel state information (CSI). Thus, in contrast to the conventional MS-STSK soft-demodulator which relies on the knowledge of CSI at the receiver, the learning-assisted design circumvents the channel estimation while also improving the data rate by dispensing with pilot overhead. Furthermore, our proposed learning-aided soft-demodulation substantially reduces the number of cost-function evaluations at the output of the MS-STSK demodulator. We demonstrate by simulations that despite avoiding CSI-estimation and the pilot overhead, our learningassisted design performs closely to the channel-estimation aided design assuming perfect CSI for BER -4, whilst imposing a low complexity. Furthermore, we show by simulations that upon using realistic imperfect CSI at the receiver employing conventional soft-demodulation, the learning-aided softdemodulator outperforms the conventional scheme. Additionally, we present quantitative discussions on the receiver complexity in terms of the number of computations required to produce the soft values.

Abstract

Hybrid beamforming (HBF) relying on a large antenna array is conceived for millimeter wave (mmWave) systems, where the beamforming (BF) gain compensates for the propagation loss experienced. The BF gain required for a successful transmission depends on the user's distance from the base station (BS). For the geographically separated users of a multi-user mmWave system, the BF gain requirements of different users tend to be different. On the other hand, the BF gain is directly related to the number of antenna elements (AEs) of the array. Therefore, in this paper, we propose an HBF design for the downlink of multi-user mmWave systems, where the number of AEs employed at the BS for attaining BF gains per user is dependent on the user's distance. We then propose grouping of the RF chains at the BS, where each group of RF chains serves a specific group of users depending on the nature of the channel. Furthermore, to support the escalating data rate demands, the exploitation of link-adaptation techniques constitutes a promising solution, since the rate can be maximized for each link while maintaining a specific target bit error rate (BER). However, given the time-varying nature of the wireless channel and the non-linearities of the amplifiers, especially at mmWave frequencies, the performance of conventional link adaptation relying on pre-defined threshold values degrades significantly. Therefore, we additionally propose a two-stage link adaptation scheme. Specifically, in the first stage, we switch on or off both the digital precoder and the combiner depending on the nature of the channel, while in the second stage a machine-learning assisted link-adaptation is proposed, where the receiver predicts whether to request spatial multiplexing- or diversity-aided transmission from the BS for every new channel realization. We demonstrate by the simulation that having both a digital precoder and a combiner in a single dominant path scenario is redundant. Furthermore, our simulations show that the learning assisted adaptation provides significantly higher data rates than that of the conventional link-adaptation, where the reconfiguration decision is simply based on pre-defined threshold values.

Abstract

Millimeter Wave (mmWave) technology coupled with full duplex (FD) communication has the potential of increasing the spectral efficiency. However, the self-interference (SI) encountered in the FD mode and the ubiquitous multi-user interference (MI) contaminates the signal. Furthermore, the system performance may also be limited by channel aging that arises because of the time-varying nature of the channel. Therefore, in this paper, we conceive FD hybrid beamforming (HBF) for K-user multiple-input multiple-output (MIMO)-aided orthogonal frequency division multiplexing (OFDM) using learning-aided channel prediction. We first derive a joint precoder and combiner design for full duplex K-user MIMO-OFDM interference channels, where we aim for minimizing both the residual SI and the MI, followed by an iterative hybrid decomposition technique developed for OFDM systems. Then, we propose a learning-aided channel prediction technique for systems suffering from channel aging relying on a radial basis neural network, where we show by simulation that upon using sufficient training, learning-assisted channel prediction can faithfully estimate the current channel. Furthermore, we demonstrate by simulations that our proposed joint hybrid precoder and combiner design outperforms the popular Eigen beamforming (EBF) technique by about 5 dB for a 128 × 32-element MIMO aided OFDM system having 32 sub-carriers.

Abstract

An adaptive array design is proposed for hybrid beamforming in millimeter-wave (mmWave) communications in the context of cloud radio access networks (C-RAN). More explicitly, the adaptive design focuses on the physical layer aspect of C-RAN. The adaptation is performed at two levels, depending on whether the channel is of line-of-sight (LOS) or non-line-of-sight (NLOS) nature. First, the antenna array architecture can be adapted between a fully connected and a sub-array-connected architecture. Then, the employment of a digital precoder in the baseband is decided based on both the channel conditions and the architecture employed. We show that the proposed adaptive design performs better than the fully connected and sub-array-connected constituent designs, when the channel varies between LOS and NLOS scenarios. Then, we extend our proposed adaptive design to mmWave communications in the context of C-RAN, where we propose an adaptive virtual cell formation algorithm, where a user can be connected to one or two remote radio heads, depending on the channel conditions.

Abstract

Rationale & Objective: It is unclear whether the effect of canagliflozin on adverse kidney and cardiovascular events in those with diabetic kidney disease varies by age and sex. We assessed the effects of canagliflozin among age group categories and between sexes in the Canagliflozin and Renal Endpoints in Diabetes with Established Nephropathy Clinical Evaluation (CREDENCE) study.Study Design: Secondary analysis of a randomized controlled trial.Setting & Participants: Participants in the CREDENCE trial.Intervention: Participants were randomly assigned to receive canagliflozin 100 mg/d or placebo.Outcomes: Primary composite outcome of kidney failure, doubling of serum creatinine concentration, or death due to kidney or cardiovascular disease. Prespecified secondary and safety outcomes were also analyzed. Outcomes were evaluated by age at baseline (Results: The mean age of the cohort was 63.0 ± 9.2 years, and 34% were female. Older age and female sex were independently associated with a lower risk of the composite of adverse kidney outcomes. There was no evidence that the effect of canagliflozin on the primary outcome (a composite of kidney failure, a doubling of serum creatinine concentration, or death from kidney or cardiovascular causes) differed between age groups (HRs, 0.67 [95% CI, 0.52-0.87], 0.63 [0.48-0.82], and 0.89 [0.61-1.29] for ages Limitations: This was a post hoc analysis with multiple comparisons.Conclusions: Canagliflozin consistently reduced the relative risk of kidney events in people with diabetic kidney disease in both sexes and across age subgroups. As a result of greater background risk, the absolute reduction in adverse kidney outcomes was greater in younger participants.Funding: This post hoc analysis of the CREDENCE trial was not funded. Abstract

The Wireless Multimedia Networks (WMNs) have developed due to the extensive applications of wireless devices and increasing availability of lower cost hardware. The WMNs are used to transmit the multimedia content like audio and video streaming and they can be deployed within a lower budget. These networks can also be used in real-time data applications that demand energy-efficient management and awareness of Quality of Service (QoS). The energy resources are limited in the wireless devices that lead to the significant threats on the QoS for WMNs. An energy-efficient routing technique is needed to handle the dynamic topology of WMN that includes a vital resource as energy. The energy-efficient routing method was proposed in this work for the purpose of data communication based on a cluster head selection from each cluster in addition to the multipath route selection to reduce the network overhead and energy consumption. The cluster heads for each cluster are selected based on Node Coverage & average residual energy parameters.In this work, the proposed energy efficient routing algorithm uses improved genetic algorithm (IGA)based on a cost function for dynamic selection of the best path. The proposed cost function uses link lifetime &average link delay parameters to estimate the link cost. The proposed algorithm’s performance compared with other previous routing methods based on extensive simulation analysis. The results showed that the proposed method achieves better performance over three other routing techniques.

Abstract

Objective: Our objective was to study the levels and relation between CD320 receptors in anaemic mothers in their last trimester of pregnancy and their newborns after birth. Methods: Association between CD320, VitB12, active B12 levels in mothers with anaemia were analysed before they underwent labor and in blood collected from their newborns after labor. Regression analysis was performed in order to assess the relation between the mothers and their neonatal vitamin CD320 levels in relation to their Vit B12 status. Results: Among the 200 pregnant mothers recruited, 59% were anaemic. CD320 levels were significantly higher in anaemic mothers (414.09 ±10.75 pg/ml) when compared with cord blood (372.25 ± 7.81 pg/ml). CD320 levels were higher in both mother and cord blood with Vit B12 deficiency, however, were lower in mothers with active B12 deficiency. CD320 levels in cord blood were positively associated with haemoglobin (r value 0.8, p value 0.42) of mothers and negatively with serum folate (r value -0.15; p value 0.23), Vit B12(r value -0.21; p value0.28) and active Vit B12 levels (r value -0.25; p value0.03) in mothers. Conclusion: CD320 levels in cord blood positively correlated with haemoglobin of mothers and negatively with folic acid and B12 status. A negative trend (p=0.0024) was observed between maternal and neonatal CD320 levels.

Abstract

The conventional Biginelli synthesis is more cumbersome and produces lower yields. Several improved methods are reported in the literature to replace the Biginelli catalyst. The design of biocompatible organic transformation is a major concern and a versatile greener procedure to construct Biginelli analogs is in great demand. Factorial design guided, energy-efficient, and versatile synthesis of 3,4-dihydropyrimidin-2-(1H)-ones (DHPM) was developed. One-factor-at-a time (OFAT) and factorial design (23) studies were utilized for screening the independent variables. The optimum levels of potential variables (benzyl-n-triethylammonium chloride (BTEAC) and glacial acetic acid) were determined through studies. The factorial design (32) analysis inferred the use of BTEAC (10.25 mol%) and glacial acetic acid (7.6 ml) as optimal for the 60 min condensation. Thirteen new 3,4- dihydropyrimidine-2-(1H)-one (DHPM) analogs were synthesized using optimized reaction conditions. The quaternary ammonium ion of BTEAC stabilizes the polarization of carbonyl group in aryl aldehydes and enolizable ketone (alkyl acetoacetate) to facilitate the cyclocondensation, in order to produce DHPMs through N-acyliminium ion and Michael adducts formation. The biocompatible strategy, simple product isolation (non-chromatographic method), and good to excellent yields are attractive features of this new protocol. Hence, the newly developed methodology is superior to the literature methods.

Abstract

An antenna is a crucial component in the wireless communication system which converts electrical waves to EM waves and vice-versa which leads to revaluation in wireless communication technology. Due to this reason the experiment was performed on a simple circular patch antenna for better radiation along with wide transmission bandwidth. In this paper, a simple circular microstrip antenna structure is designed to operate at dual bands. The proposed antenna consists of a circular radiating patch, a 50-ohm feedline line, a quarter-wave impedance matching transmission line, and a ground plane. It meets significant requirements for large bandwidth, optimized gain, minimum reflections, and small size with high directivity and efficiency. This antenna contains FR4 Epoxy substrate material with permittivity 4.4 and size 36mm x 36mm x 1.67mm. ANSOFT HFSS simulator is used to analyze various parameters like return loss, VSWR, Impedance, and gain. The Resonant Frequency of the designed antenna is 8.77 GHz and 10.6 GHz. with minimum Return Loss of -52 dB and -21.66 dB respectively. It was also observed that 1.005 and 1.2 VSWR at 8.77 GHz and 10.6 GHz respectively.

Abstract

The present work is to study the structural and thermal characteristics of cylinder head of Variable Compression Ratio (VCR) Diesel engine under different loads. The geometrical model of cylinder head is prepared using CATIA. The experiments were carried out at compression ratios of 16.5, 17.5 and 18.5. The results of stress-strain developments, heat flux and temperature variations at different loading conditions were simulated using ANSYS. From the results of analysis, the minimum values of critical stresses, heat flux and maximum factor of safety are obtained at 16.5 compression ratio.

Abstract

Investigations on super alloys has been a growing interest in the mechanical field to find the best working and machining conditions. Super alloys are termed to be one of difficult to cut materials among the engineering materials. Due to their high performance in all the conditions they are extensively used in major and highly economical fields such as aeronautical, Marian, and nuclear plants. Inconel 600 is one such alloys which can be machined by un-conventional process due to its hardness. Present paper deals with Wire-cut Electric Discharge Machine (WEDM) machining of Inconel 600 using un-treated zinc wire. Experiments were planned based on design of experiments to minimise the material and cost. L9 design with varying in pulse on (Ton), pulse off (Toff) and current (IP) with constant voltage and constant di-electrical fluid flow rate. Optimisation of surface roughness and material removal rate was done by considering lower the better criteria for roughness and higher the better criteria for material removal rate. A mathematical model has been generated in co-relation with input parameters and found that for surface roughness R sq is 91.84 and for material removal rate R sq is 92.61. ANOVA analysis was carried out and found that Ton is most contributing factor for roughness and current for material removal rate.

Objective: The present study was undertaken to study the serum IgE levels in recurrent respiratorytract illnesses in children greater than 6 months to less than 5 years of age and to find thecorrelation between them. Methods: This was a prospective observational cross-sectional study. Thestudy was undertaken to determine the correlation of serum IgE levels with recurrent respiratorytract illness. A total of 99 children were included in this study. 2ml of blood was drawn by peripheralvenous phlebotomy in airtight screw-capped plastic vials for measuring serum IgE levels. The SerumIgE level is assessed by chemiluminescence. Results: Out of 99 children, 48 children had elevatedserum IgE levels and 51 had normal serum IgE levels. Moreover, the business world comprises the upcoming entrepreneurs who are unaware of the current trends in marketing. Therefore, a recommendation system is very essential for them. In this paper, a fully automated recommendation system for the upcoming entrepreneurs to become successful in their business is proposed. The system works in three stages. In the first stage, the most transacted product is identified using association rule mining FP growth algorithm. This helps in extracting useful information from the previous transacted data by mining the entire set of frequent patterns. The second stage identifies the most customer preferred company based on review analysis. The multilevel clustering process with the generalization of data review is implemented to achieve an accurate review of the product. It rectifies the problems of shilling attack and gray sheep users commonly seen in single level K-means algorithm by refining the collected data. In the third stage, the reviews are sorted using a polarity shift sentiment classification algorithm. It helps in sort positive and negative reviews thereby rating a company. The top rated company would give the best product. Thus, the best product can be identified. From the experimental analysis, it is understood that the proposed system outperforms the existing recommendation methods. Moreover, this automated system helps the user to get the most accurate result within time. Hence, it would be very beneficial to the upcoming businessmen for flourishing their business in this increasing economic world.
