1. Processes and outcomes of an emotion‐focused family therapy two‐chair intervention for transforming problematic parenting patterns

Abstract

Emotion-focused family therapy (EFFT) is a therapy model which includes a two-chair intervention aimed at facilitating awareness and an interruption of problematic patterns of parenting as well as a reconnection to healthy caregiving instincts. The present study employed a task analysis to examine the process of this intervention with parents in a therapeutic setting and report on preliminary outcomes. Four trained EFFT therapists conducted chair work interventions with four parents (total of 16) and parents completed questionnaires directly following the intervention. Results indicated that most parents experienced significant emotional resolution of their love-based fears and an increase in confidence in supporting their children struggling with mental health issues. Use of the intervention in clinical settings is an effective and efficient way to support parents in responding to their children's mental health challenges.

Abstract

How couples handle marital conflict may depend on what issues they are facing, as some issues may be more difficult to resolve than others. What is unclear, however, is what issues happy couples face and how these issues may be different for couples depending on their developmental stage. To explore this possibility, the current study used both self-reports and observations drawn from two separate samples of happily married couples-one early in middle adulthood (N = 57 couples; average marital duration = 9 years) and one in older adulthood (N = 64 couples; average marital duration = 42 years). Results indicated that all issues were relatively minor, but early middle-aged couples reported more significant problems than did older couples. As to determining the most salient topic for happy couples, it depended on the spouses' gender, developmental stage, and how salience was assessed (i.e., highest rated issue vs. most discussed issue). Only moderate links were found between what happy couples said was their most serious concern and what they actually tried to resolve during observations of marital problem-solving, but there were differences in how spouses behaved based on the proportion of their time discussing certain topics. Findings suggest that more attention should be devoted to understanding what marital issues happy couples discuss and why, as doing so may reveal how couples maintain their marital happiness.Las maneras en las que las parejas manejan el conflicto conyugal puede depender de qué problemas enfrentan, ya que algunos problemas pueden ser más difíciles de resolver que otros. Sin embargo, lo que no está claro es qué problemas enfrentan las parejas felices y cómo estos pueden ser diferentes para las parejas según su etapa de desarrollo. Para analizar esta posibilidad, el presente estudio utilizó autoinformes y observaciones tomados de dos muestras distintas de parejas felizmente casadas: una a principios de la mediana edad (N = 57 parejas; duración conyugal promedio = 9 años) y una en la tercera edad (N = 64 parejas; duración conyugal promedio = 42 años). Los resultados indicaron que todos los problemas fueron relativamente menores, pero las parejas de principios de la mediana edad informaron problemas más importantes que las parejas mayores. En cuanto a la determinación del problema más prominente para las parejas felices, dependió del género de los cónyuges, la etapa de desarrollo y cómo se evaluó la prominencia (p. ej.: el problema más puntuado frente al problema más debatido). Abstract

Given that parental love is essential for children's optimal development, the current study gathered examples of how parental love was demonstrated within parent-child relationships. Fifty-eight two-parent, financially stable families consisting of a mother, father, and young child (3-7 years old) from the Midwest were interviewed regarding how they demonstrated or perceived parental love. Results from an inductive thematic analysis revealed considerable variability in how parental love was demonstrated, with five themes emerging that overlapped between parents and their children: playing or doing activities together, demonstrating affection, creating structure, helping or supporting, and giving gifts or treats. Some gendered patterns among these themes were found with mothers emphasizing physical and verbal affection and fathers highlighting their more prominent role as playmates. The lay examples provided by parents and children in this exploratory study extend previous conceptualizations of parental love and underscore the importance of parents being attuned and responsive to the specific needs of their children.

Abstract

Although compassionate love has been identified as a key characteristic of healthy relationships, it is unknown how it changes over time. Thus, both self- and spouse-reported compassionate love toward a spouse were assessed from a sample of 64 older married couples to begin to understand the course of compassionate love and to identify predictors of potential changes over a 1-year period. In contrast to prior theoretical and cross-sectional work, results indicated that compassionate love modestly declined over two waves spaced approximately 17 months apart. Although health was largely unrelated to compassionate love, wives’ attachment avoidance emerged as a consistent, negative predictor of compassionate love for both husbands and wives. These preliminary findings raise some concern for older spouses as they transition into more caregiving roles, as the need for compassionate love is theorized to be greater later in life. Future research should focus on replicating these findings in more diverse samples of older adults to capture how compassionate love develops amid the unique challenges associated with aging.

Abstract

Drawing upon a relatively understudied population and a unique observational task, the current study sought to examine how older couples' interactional behaviors during a relationship narrative task were associated with marital satisfaction over time. Using observational data from a sample of 64 older, higher-functioning married couples, we analyzed a series of Actor-Partner Independence Models (APIM) to explore how couples' interactional behaviors during a relationship narrative task were associated with spouses' marital satisfaction both concurrently and one year later. Analyses revealed that spouses' behaviors (e.g., expressions of positive affect, negative affect, communication skills, engagement) were associated with their self-reported marital satisfaction both at the time of the narrative and with changes in marital satisfaction. We found particularly robust evidence for the role of husbands' negative affect during the narrative task in predicting changes in both spouses' marital satisfaction over time. Our results indicate that researchers and clinicians should carefully consider the influence of development on the associations between spouses' behaviors and marital satisfaction. Further, those seeking to improve marriages in later life may need to consider the meaningful role that gender appears to play in shaping the marital experiences of older couples.

Abstract

With the urgent need to increase and improve mental health care of the growing population of older adults in the United States, clinical research is warranted to further the knowledge and improve the relevant training for mental health professionals working with older adults. This study drew from two diverse clinical samples of adults ages 18 years to 80 years to examine whether and how initial clinical presentations and changes over time in individual, family-of-origin, and relational measures differed across the life span. Results indicated a variety of linear and curvilinear associations between individual, family-of-origin, and relational measures at intake and age, with some moderation by gender. There were no significant results between the amount of change on those measures and age, indicating that older adults may change in similar fashion to middle-aged and younger adults in psychotherapy. Relevant clinical implications are provided.

Abstract

Compassionate love is an important characteristic of healthy close relationships. However, we know little about how compassionate love is behaviorally manifested. Furthermore, compassionate love may be uniquely experienced among older couples, as age-related challenges may require adjustments by spouses in providing more care for one another. Through interviews with 63 older married couples, this exploratory study examined the behavioral component of compassionate love among older couples as reported from the recipients’ perspective. From the qualitative data analysis, 11 behavioral domains were identified, including caring for physical health and helping with household tasks. The examples varied greatly in magnitude and meaningfulness, from willingly moving around the country to fixing breakfast. Further, couples frequently highlighted acts related to their increasing age-related challenges, underscoring the importance of taking a life-span approach to the study of compassionate love. The results may be useful to further develop interventions promoting compassionate love, especially among older couples.

Abstract

The Systemic Therapy Inventory of Change (STICWe administered both the STIC Initial and a set of validity measures to a relatively large sample of patients. Convergent and discriminant validity were tested using both an examination of observed correlations and confirmatory factor analysis (CFA).The correlations among the observed measures showed that the convergent validity coefficients were generally large, whereas the discriminant validity coefficients were moderate to small. Similarly, CFAs suggested that the STIC total scales and subscales are good indicators of the factors they were intended to measure and that the STIC total scales and subscales are weakly related to the factors they were intended to not measure.The results supported the convergent and discriminant validity of the five scales of the STIC Initial. Clinical or methodological significance of this article: The clinical significance of this article is that it demonstrates that the STIC Initial should be useful for identifying treatment targets including both which systems, in addition to the facets within each system, that require targeting. The methodological significance is twofold. First, the use of CFA for testing convergent and discriminant validity is still relatively rare. Second, we demonstrated how to use CFA for a more stringent test of discriminant validity compared with the original approach described by Cole ( 1987 ).

Abstract

The health benefits associated with marriage are disproportionately large in older adulthood, due in part to the powerful role spouses play in promoting each other’s well-being. What remains unclear is what motivates this caretaking. To determine whether compassionate love plays a role, the current study used an Actor–Partner Interdependence Model to examine how 64 older couples’ compassionate love is linked to their health. Feeling compassionate love was linked to better health for wives. The partner effects, however, painted a more complicated picture, with the receipt of compassionate love appearing to undermine health. Given the unprecedented growth in the number of older adults in the United States, we have a vested interest in determining how compassionate love may help or hinder well-being in later-life marriages.

Abstract

Previous work has underscored the robust links between sanctification of marriage and marital outcomes, and recent developments in the literature suggest that compassionate love, which is important for intimate relationships, may act as a mediator of that relationship. Accordingly, the current study used actor-partner interdependence models to examine the relationship between a spiritual cognition (i.e., perceived sacred qualities of marriage) and marital satisfaction, and to determine whether that relationship is mediated by compassionate love, in a sample of older married couples (N = 64). Results revealed that wives' greater sacred qualities of marriage were significantly and positively linked to marital satisfaction on the part of both spouses, and that these links were partially mediated by couples' reports of compassionate love. These findings highlight the importance of moving beyond simply establishing the existence of the link between global markers of involvement of religion and marriage to understanding how specific spiritual cognitions may foster better relationship quality, especially among older couples.

Abstract

HEK293 cells were stably transfected with rat neuronal nicotinic alpha4 and beta2 subunits. Binding of tritiated cytisine and nicotine to cell homogenates revealed the presence of a single class of high-affinity sites (dissociation constants 0.1 nM and 0.4 nM, respectively). Activation of nicotinic receptors was studied using whole-cell patch clamp methods, and acetylcholine, nicotine, dimethylphenylpiperazinium, and cytisine all produced a conductance increase. Responses desensitized to prolonged applications, at both positive and negative membrane potentials. The conductance was strongly rectifying, and outward currents were essentially absent. Responses were maximal at about 2 mM external calcium ion concentration and were reduced by about one-half at either nominally 0 or 10 mM external calcium. Di-hydro-beta-erythroidine blocked physiological responses to acetylcholine and nicotine (IC50, 2.5 nM), and reduced cytisine binding in a competitive manner (Ki 20 nM). Physostigmine enhanced the response to low concentrations of acetylcholine or nicotine. The anesthetic steroid (+)-3alpha-hydroxy-5alpha-androstane-17beta-carbonitrile blocked responses to acetylcholine (IC50, 1.3 microM), but had no effect on cytisine binding at a concentration of 30 microM. The binding properties of the receptors are those expected for rat neuronal nicotinic receptors composed of alpha4 and beta2 subunits. The pharmacological properties indicate that the responsiveness of the receptors may be allosterically enhanced or inhibited.

Abstract

Steroids, in addition to regulating gene expression, directly affect a variety of ion channels. We examined the action of steroids on human embryonic kidney 293 cells stably transfected to express rat alpha4beta2 neuronal nicotinic receptors. Each steroid that was tested inhibited acetylcholine responses from these receptors, with slow kinetics requiring seconds for block to develop and recover. The action of one steroid [3alpha,5alpha, 17beta-3-hydroxyandrostane-17-carbonitrile (ACN)] was studied in detail. Block showed enantioselectivity, with an IC(50) value of 1.5 microM for ACN and 4.5 microM for the enantiomer. Inhibition curves had Hill slopes larger than 1, indicating more than one binding site per receptor. Block did not require intracellular compounds containing high-energy phosphate bonds and was not affected by analogs of GTP, suggesting that the mechanism does not require the activation of second messengers. Block did not appear to be strongly selective between open and closed channel states or to involve changes in desensitization. A comparison of different steroids showed that a beta-orientation of groups at the 17 position produced more block than alpha-orientated diastereomers. The stereochemistry at the 3 and 5 positions was less influential for block of alpha4beta2 nicotinic receptors, despite its importance for potentiation of gamma-aminobutyric acid(A) receptors. The ability of steroids to block neuronal nicotinic receptors correlated with their ability to produce anesthesia in Xenopus tadpoles, but the concentrations required for inhibition are generally greater. Similarly, the concentrations of endogenous neurosteroids required to inhibit receptors are larger than estimates of brain concentrations.

16. Antibiotic Treatments for Clostridium difficile Infection Are Associated with Distinct Bacterial and Fungal Community Structures.

Abstract

Clostridium difficile infection (CDI) is the most common nosocomial infection in the United States, being associated with high recurrence and persistence rates. Though the relationship between intestinal dysbiosis and CDI is well known, it is unclear whether different forms of dysbiosis may potentially affect the course of CDI. How this is further influenced by C. difficile -directed antibiotics is virtually uninvestigated. In this study, diarrheal stool samples were collected from 20 hospitalized patients, half of whom were confirmed to have CDI. Analyzing tissue ex vivo and in duplicate, CDI and non-CDI fecal samples ( n = 176) were either not antibiotic treated or treated with metronidazole, vancomycin, or fidaxomicin, the three most common CDI therapies. The microbial community composition, interactions, and predicted metabolic functions were assessed by 16S rRNA gene and internal transcribed spacer sequencing, bipartite network analysis, and phylogenetic investigation of communities by reconstruction of unobserved states. Our results demonstrate that while all C. difficile -directed antibiotics were associated with similar reductions in alpha diversity, beta diversity significantly differed on the basis of the particular antibiotic, with differentiating relative abundances of bacterial and fungal assemblages. With the exception of fidaxomicin, each antibiotic was associated with the emergence of potentially pathogenic fungal operational taxonomic units, with predicted bacterial functions enriched for xenobiotic metabolism that could perpetuate the dysbiosis driving CDI. Toxin-independent mechanisms of colitis related to the relative abundance of pathogenic bacteria and fungi were also noted. This study suggests that a transkingdom interaction between fungi and bacteria may be important in CDI pathophysiology, including being a factor in the historically high persistence and recurrence rates associated with this disease. IMPORTANCE Using human fecal samples and including sequencing for both bacterial and fungal taxa, this study compared the conventional antibiotics used to treat C. difficile infection (CDI) from the perspective of the microbiome, which is particularly relevant, given the relationship between dysbiotic states and the development of CDI. Sequencing and imputed functional analyses suggest that C. difficile -directed antibiotics are associated with distinct forms of dysbiosis that may be influential in the course of CDI. Further, a role for fungal organisms in the perpetuation of the causal dysbiosis of CDI is discussed, suggesting a previously unappreciated, clinically relevant transkingdom interaction that warrants further study.

Abstract

Diverticular disease is commonly associated with the older population in the United States. As individual's age, diverticulae, or herniation of the mucosa through the colonic wall, develop. In 10-25% of individuals, the diverticulae become inflamed, resulting in diverticulitis. The gut ecosystem relies on the interaction of bacteria and fungi to maintain homeostasis. Although bacterial dysbiosis has been implicated in the pathogenesis of diverticulitis, associations between the microbial ecosystem and diverticulitis remain largely unstudied. This study investigated how the cooperative network of bacteria and fungi differ between a diseased area of the sigmoid colon chronically affected by diverticulitis and adjacent non-affected tissue. To identify mucosa-associated microbes, bacterial 16S rRNA and fungal ITS sequencing were performed on chronically diseased sigmoid colon tissue (DT) and adjacent tissue (AT) from the same colonic segment. We found that Pseudomonas and Basidiomycota OTUs were associated with AT while Microbacteriaceae and Ascomycota were enriched in DT. Bipartite co-occurrence networks were constructed for each tissue type. The DT and AT networks were distinct for each tissue type, with no microbial relationships maintained after intersection merge of the groups. Our findings indicate that the microbial ecosystem distinguishes chronically diseased tissue from adjacent tissue.

Abstract

The traditional dental curriculum presents limited opportunities for students to evaluate and treat patients with acute dental trauma. In an effort to increase student experiences, faculty at a dental school initiated and taught an interdisciplinary trauma course from 2014 to 2016. This course was elective, offered in the final year of dental school, and combined student-led and faculty-led case-based discussions of management of traumatic dental injuries with hospital trauma call shadowing. Modifications of the course were made based on student feedback. Preliminary data from student surveys showed a favorable opinion of the course. Attendees were exposed to a higher number of acute traumatic injuries and they expressed greater confidence in their diagnosis and management abilities than their peers., (© 2016 John Wiley & Sons A/S. Abstract

Introduction: This study compared images displayed on 1 desktop monitor, 1 laptop monitor, and 2 tablets for the detection of contrast and working length interpretation, with a null hypothesis of no differences between the devices., Methods: Three aluminum blocks, with milled circles of varying depth, were radiographed at various exposure levels to create 45 images of varying radiographic density. Six observers viewed the images on 4 devices: Lenovo M92z desktop (Lenovo, Beijing, China), Lenovo Z580 laptop (Lenovo), iPad 3 (Apple, Cupertino, CA), and iPad mini (Apple). Observers recorded the number of circles detected for each image, and a perceptibility curve was used to compare the devices. Additionally, 42 extracted teeth were imaged with working length files affixed at various levels (short, flush, and long) relative to the anatomic apex. Observers measured the distance from file tip to tooth apex on each device. The absolute mean measurement error was calculated for each image. Analysis of variance tests compared the devices. Observers repeated their sessions 1 month later to evaluate intraobserver reliability as measured with weighted kappa tests. Interclass correlation coefficients compared interobserver reliability., Results: There was no significant difference in perceptibility detection between the Lenovo M92z desktop, iPad 3, and iPad mini. However, on average, all 3 were significantly better than the Lenovo Z580 laptop (P values ≤.015). No significant difference in the mean absolute error was noted for working length measurements among the 4 viewing devices (P = .3509)., Conclusions: Although all 4 viewing devices seemed comparable with regard to working length evaluation, the laptop computer screen had lower overall ability to perceive contrast differences., (Copyright © 2015 American Association of Endodontists. Abstract

Introduction: Procedures used in single-visit or multiple-visit approaches to apical barrier creation were used with an experimental apexification model to test the surface hardness of 3 materials. The purpose of this study was to examine the microhardness of the materials after setting in moist or dry conditions., Methods: A simulated open apex and periapical environment model was created using polyethylene tubes placed into a porous block filled with phosphate-buffered saline. White ProRoot Mineral Trioxide Aggregate (MTA; Dentsply Tulsa Dental, Tulsa, OK), EndoSequence Root Repair Material (ESRRM; Brasseler USA, Savannah, GA), and Biodentine (BD; Septodont, Louisville, CO) were mixed and placed into the apical 4 mm of the tubes (N = 15). The moist group had a damp cotton pellet above the test materials (mineral trioxide aggregate or ESSRM) with Fuji II LC (GC America, Alsip, IL) sealing the coronal segment. The dry group had gutta-percha placed directly against the test materials with amalgam sealing the coronal segment. After 10 days of storage in 100% humidity at 37°C, samples were sectioned, and microhardness was independently measured at 2 mm and 4 mm from the apical end. Differences were assessed using analysis of variance and a Tukey post hoc test (α = .05)., Results: Analysis of variance analyses showed no significant effect of wet or dry conditions on resultant material hardness. A Tukey post hoc test showed that using ESRRM and BD would not result in a significant difference in hardness, but using MTA would result in statistically significant different hardness values when compared with ESRRM or BD., Conclusions: Either a moist or dry environment could allow hardening of materials; thus, both methods could be acceptable for clinical treatment procedures., (Copyright © 2014 American Association of Endodontists. Abstract

HEK293 cells were stably transfected with rat neuronal nicotinic alpha4 and beta2 subunits. Binding of tritiated cytisine and nicotine to cell homogenates revealed the presence of a single class of high-affinity sites (dissociation constants 0.1 nM and 0.4 nM, respectively). Activation of nicotinic receptors was studied using whole-cell patch clamp methods, and acetylcholine, nicotine, dimethylphenylpiperazinium, and cytisine all produced a conductance increase. Responses desensitized to prolonged applications, at both positive and negative membrane potentials. The conductance was strongly rectifying, and outward currents were essentially absent. Responses were maximal at about 2 mM external calcium ion concentration and were reduced by about one-half at either nominally 0 or 10 mM external calcium. Di-hydro-beta-erythroidine blocked physiological responses to acetylcholine and nicotine (IC50, 2.5 nM), and reduced cytisine binding in a competitive manner (Ki 20 nM). Physostigmine enhanced the response to low concentrations of acetylcholine or nicotine. The anesthetic steroid (+)-3alpha-hydroxy-5alpha-androstane-17beta-carbonitrile blocked responses to acetylcholine (IC50, 1.3 microM), but had no effect on cytisine binding at a concentration of 30 microM. The binding properties of the receptors are those expected for rat neuronal nicotinic receptors composed of alpha4 and beta2 subunits. The pharmacological properties indicate that the responsiveness of the receptors may be allosterically enhanced or inhibited.
