Viral respiratory infections are among the most common causes of disease in humans, particularly in young children, and remain a major public health problem worldwide. For many geographic regions, there is limited epidemiological information on the main causative agents of these diseases. In this article, we investigated, in a prospective study, the viral agents leading to acute respiratory disease in children younger than 15 years of age who were admitted to the pediatric emergency unit of a major teaching hospital in Erbil City, capital of the Kurdistan region, Iraq. Nasopharyngeal samples obtained from 269 hospitalized children were analyzed for viral respiratory pathogens using the xTAG Respiratory Virus Panel Fast assay, and the data were correlated with the clinical and demographic information available for these patients. One or more respiratory virus(es) were detected in 203 out of 269 (75.5%) samples. The most frequent viruses were enterovirus/rhinovirus (n = 88; 32.7%), respiratory syncytial virus (n = 55; 20.4%), and human metapneumovirus (n = 36; 13.4%). In 42 samples (15.6%), coinfections with 2 or more respiratory viruses were detected, with enterovirus/rhinovirus, respiratory syncytial virus, human metapneumovirus, and adenovirus being identified as the most common agents in viral coinfections in these patients.

4. Comparative Analysis of the Fatty Acid Pattern of Silybum marianum with Nigella sativa by Gas Chromatography-Mass Spectrometry

Many studied were conducted to evaluate the antihepatotoxic and antioxidant activities of Silybum marianum and proved these actions. The Naturally grown seed in Iraqi-Kurdistan Region also were studied for its chemical contents and biological activities. Vegetable oils occur in various plant parts mainly concentrated in the seeds. In this study comparison was made between the fatty acid patterns of two plant seeds, Silybum marianum and Nigella sativa. Seed sample of Silybum marianum and Nigella sativa were exposed for extraction and isolation of the fatty acid contents using two different solvents (petroleum ether and n-hexane) at 60-80oC using soxhlet apparatus and the oily extract were purified and analysed by GC-MS triplet system. Result showed that extraction with petroleum ether yielded 23% and 24% of total weight of the seeds of Silybum marianum and Nigella sativa respectively and comparable results with n-hexane extract yielded 17% and 22% of total seed weight of Silybum marianum and Nigella sativa respectively showing significant differences in the fatty acids patterns of both plants under research. La pathologie traumatique du pied et de la cheville est un motif frequent de consultation aux urgences. Les fractures de la marge posterieure du tibia, anciennement denommees fractures de la malleole posterieure, sont frequentes. Elles sont le plus souvent associees a d’autres lesions ligamentaires ou osseuses touchant les differents elements de la cheville et/ou du pied. Les fractures isolees de la marge posterieure du tibia sont relativement rares. On estime leur frequence a environ 1 % de toutes les fractures de la cheville. Le mecanisme de survenue fait intervenir habituellement une hyperflexion plantaire du pied combinee a une compression axiale de la marge posterieure du tibia par le talus. Nous rapportons le cas clinique d’un patient victime d’une chute d’escalier chez qui le diagnostic a ete pose secondairement grâce a la radiographie de la cheville de profil en rotation externe. L’evolution a ete favorable malgre le retard de prise en charge.

Les osteophytes tibiotaliens anterieurs peuvent etre responsables de douleurs chroniques de la cheville par conflit anterieur osseux. Ces osteophytes sont secondaires a des microtraumatismes repetes ou a des fractures de la cheville. Ces situations sont retrouvees au cours de la pratique de sport. Les patients presentent un tableau mixte associant des douleurs et une raideur de l’articulation tibiotalienne parfois mal toleree. Le but de ce travail etait de presenter la technique operatoire arthroscopique du traitement du conflit anterieur osseux de la cheville avec enraidissement et d’en presenter les premiers resultats a court terme sur la mobilite tibiotalienne et la douleur par l’etude de quatre cas cliniques.

Resume Le kyste synovial intraosseux des os du carpe est une cause rare de douleur du poignet. Peu de cas ont ete rapportes. Nous rapportons un cas tres rare de kyste synovial intraosseux du capitatum, chez une fille âgee de 16 ans qui se plaignait de douleur chronique du poignet droit. Le bilan radiologique a montre une lesion osteolytique avec sclerose marginale. Le diagnostic a ete confirme par l’etude anatomopathologique et le traitement a consiste en un curetage suivi d’autogreffe d’os spongieux. Apres un recul de huit mois, le capitatum etait completement consolide et le resultat fonctionnel satisfaisant.

Resume La prise en charge de patients âges de 30 a 50 ans et presentant une laxite anterieure du genou par rupture du LCA associee a une arthrose femorotibiale interne, est tres difficile. De recentes publications ont montre que l’association d’une osteotomie tibiale a une ligamentoplastie intra-articulaire permettait de freiner l’evolution de l’arthrose tout en corrigeant la laxite du genou. Nous rapportons l’experience de 15 patients du service de traumatologie de l’hopital militaire Moulay Ismail de Meknes presentant une laxite anterieure avec arthrose femorotibiale interne. Ces patients ont beneficie d’une osteotomie de valgisation associee a une ligamentoplastie du LCA.

The combination of elbow dislocation and perilunate dislocation is rare. The prognosis of this condition depends mainly on that of the wrist. While some cases of floating forearm have been reported, no bilateral affection has -of yet- to our knowledge, been published. The authors report a case of a young patient who presented with bilateral floating forearm after a fall from a height. The treatment was surgical at the wrist. The functional result is similar to cases reported in the literature.

Resume Entre 1996 et 2005, 50 patients (54 epaules) ont beneficie d’une intervention de Latarjet pour instabilite anterieure de l’epaule. Nous avons, retrospectivement, evalue les resultats cliniques et radiologiques de cette technique operatoire. L’intervention a ete realisee pour le traitement d’une luxation recidivante dans 51 cas, subluxation recidivante douloureuse dans deux cas et une epaule douloureuse dans un cas. Tous les patients ont eu une evaluation radiographique avant l’intervention et lors du controle le plus recent. Selon le score de Rowe, 51 (94 %) des 54 epaules ont obtenu un resultat bon ou excellent. Au plus grand recul, 51 epaules etaient indemnes d’arthrose glenohumerale.

La survenue d’une fracture pathologique complique la conduite therapeutique. Mais lorsque la surface articulaire est epargnee, le kyste anevrismal osseux de la rotule peut etre traite avec succes par curetage et greffe corticospongieuse comme dans notre observation.

The authors report a case of fibularis muscle syndrome in a 37-year-old man with no history of trauma to or chronic instability in the ankle. Surgical exploration discovered a split lesion in the fibularis brevis tendon, associated with the supernumerary muscle. Based on clinical history and perioperative observations, the fissure syndrome appears to have resulted from a retromalleolar conflict in the fibularis retinaculum secondary to anatomi- cal abnormality. Tendon repair and accessory muscle excision allowed complete recovery.

Osteoblastoma is a benign but painful bone tumour, which can only be treated surgically. Localization at the elbow is rare and poses diagnostic problems. The tumour in this location causes joint stiffnesses due to pain and the regional inflammatory response. A delay in diagnosis can, despite successful treatment of the tumour, cause loss of the full range of movements of the elbow. We report a case illustrating this aspect.

27. [Intraosseous ganglion cyst of the capitate: a case report]

Intraosseous cyst of the carpal bones is a rare cause of wrist pain. Few cases have been reported. We report a rare case of intraosseous ganglion cyst of the capitate, in a girl aged 16 who plain of chronic pain of the right wrist. The radiological assessment showed an osteolytic lesion with marginal sclerosis. The diagnosis was confirmed by histopathology and treatment consisted of curettage with autologous cancellous bone graft. At 8 months follow-up, the capitate is fully consolidated and the functional result is satisfactory.

Aneurysmal bone cyst is a tumor-like lesion which accounts for about 1% of all biopsied tumors and tumor-like lesions of bone. We report an extremely rare case of aneurysmal bone cyst of the patella in an 18-year-old patient revealed by a pathological fracture. The unexpected appearance of a pathological fracture affects the whole therapeutical management. However, in the case of a well-preserved articular surface, curettage and corticocancellous grafting are considered to be successful methods of treatment for aneurysmal bone cyst of the patella, as revealed in our study.

Osteoblastoma is a benign but painful bone tumour, which can only be treated surgically. Localization at the elbow is rare and poses diagnostic problems. The tumour in this location causes joint stiffnesses due to pain and the regional inflammatory response. A delay in diagnosis can, despite successful treatment of the tumour, cause loss of the full range of movements of the elbow. We report a case illustrating this aspect.

Entrapment of the ulnar nerve at the wrist is rare compared with the more commun site at the elbow. This condition was secondary to reccurent blunt trauma, ganglion cyst, vasculitic disorders, and fibrous bands. Compression of the ulnar nerve caused by pisiform-hamate coalition is rare entity. One case of this condition is reported.

Hemangioma is an uncommon bone neoplasm that usually affects the craniofacial bones and spine, accounting for less than 1% of all primary bone tumors. The authors report a case of isolated hemangioma involving the proximal humerus in a 22-year-old man. Radiographically, the lesion was a well-defined osteolytic process with marginal sclerosis. The CT images demonstrated an osteolytic lesion with coarse trabecular pattern and partial cortical breakthrough. Magnetic resonance imaging showed a multilocular and hemorrhagic lesion with fluid-fluid level, that was suggestive of aneurysmal bone cyst.

Bone and joint involvement is observed in 3 to 5 p. 100 of all cases of tuberculosis and in 15 p. 100 of extrapulmonary tuberculosis. Spinal and weight-bearing joints are most frequently involved. We report three cases of bone tuberculosis with rare localizations that raised difficult diagnostic problems. Our patients had tuberculosis of the sternum, the iliac bone and in the third case, the spine but solely in the posterior arch of the fourth cervical vertebra. X-ray evidenced bone lysis and invasion of adjacent tissues. The cold abscesses were evacuated surgically allowing bacteriological and histological diagnosis. The patients were successfully treated with anti-tuberculosis regimens. An infectious etiology should be considered in patients with bone lesions in order to obtain the samples necessary for positive diagnosis.

We report seven cases of traumatic dislocation of the great toe, detailing the anatomy, the mechanism of injury and the radiographic diagnosis. We propose an additional classification based on three hereto unreported cases.Between october 1994 and october 1997, we treated seven patients with traumatic dislocation of the first metatarso-phalangeal joint of the great toe. There were six men and one woman, mean age 35 years (range 24 - 44 years). Dislocation was caused by motor vehicle accidents in four cases and by falls in three. Diagnosis was made on anteroposterior, lateral and medial oblique radiographs. According to Jahss' classification, there was one type I and three type IIB dislocations. There was also one open lateral dislocation and two dorsomedial dislocations. Only these dorsomedial dislocations required open reduction, done via a dorsal approach. Mean follow-up was 17.5 months (range 9 - 24 months) in six cases. One patient was lost to follow-up. The outcome was good in six cases and poor in one (dorsomedial dislocation).Dislocation of the first metatarso-phalangeal joint of the great toe is an uncommon injury. In 1980, Jahss reported two cases and reviewed three others described in the literature. He proposed three types of dislocation based on the feasibility of closed reduction (type I, II and IIB). In 1991, Copeland and Kanat reported a unique case in which there was an association of IIA and IIB lesions. They proposed an addition to the classification (type IIC). In 1994, Garcia Mata et al. reported another case which had not been described by Jahss and proposed another addition. All dislocations reported to date have been sagittal dislocations. Pathological alteration of the collateral ligaments has not been previously reported. In our experience, we have seen one case of open lateral dislocation due, at surgical exploration, to medial ligament rupture and two cases of dorsomedial dislocation due, at surgical exploration, to lateral ligament rupture.We propose another additional classification with pure lateral dislocation (type III) and dorso-lateral dislocation (type IL or IIL+), which are related to the formerly described variants.

The Buschke-Loewenstein tumour (BLT) is a sexually transmissible benign tumour of viral origin. It is characterized by a large tumour volume, the possibility of local recurrence and especially its potential for malignant transformation. We report a case of scrotal BLT in a 49-year-old patient with a history of recurrent scrotal fistula, with negative HIV serology, but in whom the search for Human Papilloma Virus (HPV) was not performed. The tumour arising from the scrotal skin was resected and the testis was found to be normal. Histology confirmed transformation of the tumour into a squamous cell carcinoma. The clinical course was favourable, after a follow-up of 8 months with no local recurrence. In the light of this case of rare tumour, especially in its malignant form and its unusual scrotal site, the authors review the problems related to clinical classification, histology (differential diagnosis with primary squamous cell carcinoma), clinical course and treatment raised by this tumour.

Tuberculosis in an isolated localization in the patella is rarely reported. Five cases were found in the literature. Two new cases together with the radiographic work-up to be performed in case of a "painful" knee are presented. Histology of the curetage specimen is required to reach certain diagnosis and develop an adapted management strategy to protect the functional future of the knee.

This article presents a participative and iterative qualitative risk assessment framework that can be used to evaluate the spatial variation of the risk of infectious animal disease introduction and spread on a national scale. The framework was developed through regional training action workshops and field activities. The active involvement of national animal health services enabled the identification, collection and hierarchization of risk factors. Quantitative data were collected in the field, and expert knowledge was integrated to adjust the available data at regional level. Experts categorized and combined the risk factors into ordinal levels of risk per epidemiological unit to ease implementation of risk-based surveillance in the field. The framework was used to perform a qualitative assessment of the risk of introduction and spread of foot-and-mouth disease (FMD) in Tunisia as part of a series of workshops held between 2015 and 2018. The experts in attendance combined risk factors such as epidemiological status, transboundary movements, proximity to the borders and accessibility to assess the risk of FMD outbreaks in Tunisia. Out of the 2,075 Tunisian imadas, 23 were at a very high risk of FMD introduction, mainly at the borders; and 59 were at a very high risk of FMD spread. To validate the model, the results were compared to the FMD outbreaks notified by Tunisia during the 2014 FMD epizootic. Using a spatial Poisson model, a significant alignment between the very high and high-risk categories of spread and the occurrence of FMD outbreaks was shown. The relative risk of FMD occurrence was thus 3.2 higher for imadas in the very high and high spread risk categories than for imadas in the low and negligible spread risk categories. Our results show that the qualitative risk assessment framework can be a useful decision support tool for risk-based disease surveillance and control, in particular in scarce-data environments., (© 2020 Wiley-VCH GmbH.)

The study aimed at assessing the prevalence of toxoplasmosis due to T. gondii in dromedaries by the ELISA test and to identify associated risk factors. A cross-sectional study was performed on 320 dromedaries (Camelus dromedaries) from January to March 2018 in four provinces of southeastern Algeria (Biskra, El- Oued, Ouargla, and Ghardaia). A structured questionnaire was used to collect data on individual characteristics, husbandry management and health conditions. The ELISA test revealed an overall seroprevalence of 15% (CI95%: 11.5-19.33). Possible factors associated with T. gondii infection were analyzed by univariate analysis and multivariate logistic regression. Age, husbandry system, and study area were identified as risk factors for T. gondii infection. Increased seropositivity was observed in old animals older than 10 years (OR = 2.81; CI: 1.51-3.87; p = 0.003), two times higher in animals living in the intensive husbandry system (OR = 2.42; CI: 1.3-3.07; p = 0.003), and four times higher in animals living in Biskra (OR = 4.2; CI: 1.21-9.85; p = 0.0001) than other provinces. Peritoneal metastasis (PM) is a very serious complication of gastrointestinal and gynecological malignancies which is poorly documented. Modified mesothelial cell layer and their microenvironments can favor fibrin deposition for cancer cell adhesion. Scanning and transmission electron microscopy of peritoneal surface and cancer cell clusters from cancer patients was done. Ascites and its impact on mesothelial cells were assessed by cytokine array. Neprilysin, matrix metalloprotease, epithelial mesenchymal transition (EMT) related molecules (E-cadherin, Snail, Slug, Twist, Vimentin and Fibronectin), tissues factor (TF), endothelial protein C receptors (EPCR) were quantified by q-PCR. Fibrin in the simples were stained using anti fibrin F1E1 antibody. Migration ability was assessed by scratch assay. Cell viability and neprilysin activity were analyzed by bioluminescence. Cancer cells-fibrin interaction was investigated by scanning electron microscopy (SEM) and microcinematography (MCG). Mesothelial cells change their morphology after incubation with carcinomatosis peritoneal fluids in vitro. EMT associated with upregulation of neprilysin, matrix metalloproteinase-2, tissue factor and cytokines secretions such as interleukin-6, and 8, hepatocyte growth factor and granulocyte chemotactic protein-2 mRNA and protein were observed. EPCR expression as a natural anticoagulant was decreased. In parallel, carcinomatosis cell clusters extracted from peritoneal fluids were found to be associated with fibrin. Kinetic analysis of cancer cell-fibrin interaction in vitro studied by MCG showed that fiber filaments generated from clots inhibited cancer cell adhesion on fibrin clots. 41. Cross-Sectional Survey on Toxoplasma gondii Infection in Cattle, Sheep, and Goats in Algeria: Seroprevalence and Risk Factors.

A cross-sectional study aimed at assessing the seroprevalence and identifying the risk factors for Toxoplasma gondii infection in cattle, sheep, and goats in eight provinces located in two main Algerian agro-ecological zones was carried out from October 2015 to March 2018. Blood sera from 4074 animals of both sexes were tested for the presence of anti- T. gondii IgG antibodies, using the indirect, enzyme-linked, immunosorbent assay technique (ELISA). Moreover, to identify the potential risk factors of T. gondii infection, a survey through a breeders' questionnaires was conducted. Nearly one-fourth of the total number of animals tested (1024/4074)-i.e., 25.1%--were seropositive. The seroprevalence in cattle, sheep, and goats was 28.7%, 25.6%, and 11.9%, respectively. The area, sex, age, and herd size were identified as risk factors for T. gondii infection. Higher seropositivity rates were recorded in cows and goats (odds ratio (OR) = 1.63 and 6.4), in old animals (cattle, OR = 2.1; sheep, OR = 1.9; and goat, OR = 3.9), and in small size herds (cattle, OR = 2.5; sheep, OR = 1.9; goat, OR = 2.2). In conclusion, there is widespread T. gondii infection in cattle, sheep, and goats in these two strategic agricultural areas. We here report 54 cases of humerus diaphyseal fractures treated via Hackethal's bundle nailing. We observed 3 cases of radial nerve palsy. Therapeutic abstention associated with monitoring was the applied attitude toward these patients on presentation to the Emergency Department. It resulted in recovery. Hackethal's bundle nailing technique is relatively easy but it requires rigorous surgical procedure performed by an experienced surgeon. A rate of 7.84% of pseudarthrosis is reported. Hackethal's bundle nailing is a very safe, improved orthopedic treatment with modest cost.

Avian pathogenic E. coli (APEC) is the etiologic agent of avian colibacillosis, the most common disease responsible for chicken morbidity in the world. Although multiple virulence-associated factors were identified, their prevalence in Algeria is still poorly known. In the present research, 92 avian pathogenic E. coli (APEC) isolates were recovered from broilers with clinical signs and lesions of colibacillosis. In addition, 32 E. coli isolates collected from feces of healthy birds (AFEC) were included for comparison. All isolates were investigated by PCR for the presence of a total of 11 virulence-associated genes described for avian pathogenic (iroN, ompT, hlyF, iss, iutA, and fimC) and diarrheagenic E. coli (eae, stx, elt/est, ipaH, and aggR). The sensitivity of 39 APEC isolates to 16 antibiotics was also determined using antimicrobial pretreated microplates. Here, we report that 98% of the examined isolates host at least one of the tested virulence factors. The most prevalent genes in APEC were iutA (90.6%), ompT (86.9%), and iss (85.8%); whereas, iutA (78.1%), fimC (78.1%), and iroN (68.7%) were the highest prevalent genes in AFEC. Our data showed that none of the AFEC isolates harbor any of the tested diarrheagenic genes. Moreover, only elt/est (5.4%), stx (2.1%), and ipaH (2.1%) genes were carried by APEC isolates. We further established that ceftazodime, ceftiofur, mequindox, amoxicillin/clavulanic acid, and meropenem were the most efficient antibiotics against the analyzed APEC isolates. Overall, our findings provide more insights about APEC and AFEC virulence potential in Algeria which could participate in the fight against colibacillosis.

Surgical treatment of unstable pelvic fractures Type C, has a vertical instability that is not controlled by traction and supine; therefore, orthopedic and functional treatments undertaken by default are sources of complications. The closed reduction with percutaneous sacroiliac fixation solves the problem of vertical instability; but at the cost of learning the method. Five patients with unstable pelvic fractures; were treated by percutaneous sacroiliac fixation. This reliable and useful method in the stabilization of unstable pelvic fractures. Good functional results are predictable based on the severity of pelvic fractures and associated injuries. The low rates of complications and the minimally invasive nature are the advantages of this method.

Antegrade intramedullary nailing has become the gold standard to treat two-, three-, and four-part cephalo-tuberosity fractures. We report a retrospective study of 6 patients who have undergone centro-medullary nailing since January 2012. Mean follow-up was 12 months; the average age was 57 years. Clinical evaluation was based on gross and weighted Constant and Murley score, based on the age and sex, compared to the "normal" body surface area. Radiological assessment allowed us to evaluate bone healing, the occurrence of osteonecrosis of femoral head or of post-traumatic arthrosis. Radiological examination focused even on the state of the tuberosities and on the existence or not of osteolysis of the greater tubercle of the humerus. It was also used to identify the criteria for good reduction, namely cephalic cap angle and diaphyseal axis (αF) in face views. All patients underwent the same surgical procedure. Constant and Murley score for all patients was 64.13 points. Weighted score based on age and sex was 73%. Joint mobility was estimated at an average of 116° for all patients in anterior elevation, 99.9° in lateral elevation, and 42° in external rotation. Average αF angle was 42°. All patients met criteria for good reduction, namely αF. Centro-medullary nailing allows simple and cost-effective bone synthesis with very promising functional results. - Abstract
Anterior cruciate ligament (ACL) mucoid cysts are rare. They are often asymptomatic and unrecognized. Their clinical expression is polymorphic and MRI allows for an early diagnosis confirmed by histology. Arthroscopic resection is the gold standard treatment. - Abstract
Eye applicators with 90Sr/90Y and 106Ru/106Rh beta-ray sources are generally used in brachytherapy for the treatment of eye diseases as uveal melanoma. Whenever, radiation is used in treatment, dosimetry is essential. However, knowledge of the exact dose distribution is a critical decision-making to the outcome of the treatment. The Monte Carlo technique provides a powerful tool for calculation of the dose and dose distributions which helps to predict and determine the doses from different shapes of various types of eye applicators more accurately. The aim of this work consisted in using the Monte Carlo GATE platform to calculate the 3D dose distribution on a mathematical model of the human eye according to international recommendations. Mathematical models were developed for four ophthalmic applicators, two HDR 90Sr applicators SIA.20 and SIA.6, and two LDR 106Ru applicators, a concave CCB model and a flat CCB model. In present work, considering a heterogeneous eye phantom and the chosen tumor, obtained results with the use of GATE for mean doses distributions in a phantom and according to international recommendations show a discrepancy with respect to those specified by the manufacturers. The QC of dosimetric parameters shows that contrarily to the other applicators, the SIA.20 applicator is consistent with recommendations. The GATE platform show that the SIA.20 applicator present better results, namely the dose delivered to critical structures were lower compared to those obtained for the other applicators, and the SIA.6 applicator, simulated with MCNPX generates higher lens doses than those generated by GATE., (Copyright © 2016 Associazione Italiana di Fisica Medica.
