Abstract: Summary: Background: People with diabetes are at increased risk of hospitalisation, morbidity, and mortality following SARS-CoV-2 infection. Long-term outcomes for people with diabetes previously hospitalised with COVID-19 are, however, unknown. This study aimed to determine the longer-term physical and mental health effects of COVID-19 in people with and without diabetes. Methods: The PHOSP-COVID study is a multicentre, long-term follow-up study of adults discharged from hospital between 1 February 2020 and 31 March 2021 in the UK following COVID-19, involving detailed assessment at 5 and 12 months after discharge. The association between diabetes status and outcomes were explored using multivariable linear and logistic regressions. Findings: People with diabetes who survived hospital admission with COVID-19 display worse physical outcomes compared to those without diabetes at 5- and 12-month follow-up. People with diabetes displayed higher fatigue (only at 5 months), frailty, lower physical performance, and health-related quality of life and poorer cognitive function. Differences in outcomes between diabetes status groups were largely consistent from 5 to 12-months. In regression models, differences at 5 and 12 months were attenuated after adjustment for BMI and presence of other long-term conditions. Interpretation: People with diabetes reported worse physical outcomes up to 12 months after hospital discharge with COVID-19 compared to those without diabetes. These data support the need to reduce inequalities in long-term physical and mental health effects of SARS-CoV-2 infection in people with diabetes. Abstract: Background The long-term outcomes of COVID-19 hospitalisation in individuals with pre-existing airway diseases are unknown. Methods Adult participants hospitalised for confirmed or clinically suspected COVID-19 and discharged between 5 March 2020 and 31 March 2021 were recruited to the Post-hospitalisation COVID-19 (PHOSP-COVID) study. Participants attended research visits at 5 months and 1 year post discharge. Clinical characteristics, perceived recovery, burden of symptoms and health-related quality of life (HRQoL) of individuals with pre-existing airway disease (i.e., asthma, COPD or bronchiectasis) were compared to the non-airways group. Results A total of 615 out of 2697 (22.8%) participants had a history of pre-existing airway diseases (72.0% diagnosed with asthma, 22.9% COPD and 5.1% bronchiectasis). Abstract: Summary: Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. Copyright © 2023 The Author(s). Background: Sleep disturbance is common following hospital admission both for COVID-19 and other causes. The clinical associations of this for recovery after hospital admission are poorly understood despite sleep disturbance contributing to morbidity in other scenarios. We aimed to investigate the prevalence and nature of sleep disturbance after discharge following hospital admission for COVID-19 and to assess whether this was associated with dyspnoea. Methods: CircCOVID was a prospective multicentre cohort substudy designed to investigate the effects of circadian disruption and sleep disturbance on recovery after COVID-19 in a cohort of participants aged 18 years or older, admitted to hospital for COVID-19 in the UK, and discharged between March, 2020, and October, 2021. Participants were recruited from the Post-hospitalisation COVID-19 study (PHOSP-COVID). Follow-up data were collected at two timepoints: an early time point 2–7 months after hospital discharge and a later time point 10–14 months after hospital discharge. Sleep quality was assessed subjectively using the Pittsburgh Sleep Quality Index questionnaire and a numerical rating scale. Sleep quality was also assessed with an accelerometer worn on the wrist (actigraphy) for 14 days. Participants were also clinically phenotyped, including assessment of symptoms (ie, anxiety [Generalised Anxiety Disorder 7-item scale questionnaire], muscle function [SARC-F questionnaire], dyspnoea [Dyspnoea-12 questionnaire] and measurement of lung function), at the early timepoint after discharge. Actigraphy results were also compared to a matched UK Biobank cohort (non-hospitalised individuals and recently hospitalised individuals). Multivariable linear regression was used to define associations of sleep disturbance with the primary outcome of breathlessness and the other clinical symptoms. PHOSP-COVID is registered on the ISRCTN Registry (ISRCTN10980107). Findings: 2320 of 2468 participants in the PHOSP-COVID study attended an early timepoint research visit a median of 5 months (IQR 4–6) following discharge from 83 hospitals in the UK. Data for sleep quality were assessed by subjective measures (the Pittsburgh Sleep Quality Index questionnaire and the numerical rating scale) for 638 participants at the early time point. Sleep quality was also assessed using device-based measures (actigraphy) a median of 7 months (IQR 5–8 months) after discharge from hospital for 729 participants. After discharge from hospital, the majority (396 [62%] of 638) of participants who had been admitted to hospital for COVID-19 reported poor sleep quality in response to the Pittsburgh Sleep Quality Index questionnaire. A comparable proportion (338 [53%] of 638) of participants felt their sleep quality had deteriorated following discharge after COVID-19 admission, as assessed by the numerical rating scale. Device-based measurements were compared to an age-matched, sex-matched, BMI-matched, and time from discharge-matched UK Biobank cohort who had recently been admitted to hospital. Compared to the recently hospitalised matched UK Biobank cohort, participants in our study slept on average 65 min (95% CI 59 to 71) longer, had a lower sleep regularity index (–19%; 95% CI –20 to –16), and a lower sleep efficiency (3·83 percentage points; 95% CI 3·40 to 4·26). Similar results were obtained when comparisons were made with the non-hospitalised UK Biobank cohort. Overall sleep quality (unadjusted effect estimate 3·94; 95% CI 2·78 to 5·10), deterioration in sleep quality following hospital admission (3·00; 1·82 to 4·28), and sleep regularity (4·38; 2·10 to 6·65) were associated with higher dyspnoea scores. Poor sleep quality, deterioration in sleep quality, and sleep regularity were also associated with impaired lung function, as assessed by forced vital capacity. Depending on the sleep metric, anxiety mediated 18–39% of the effect of sleep disturbance on dyspnoea, while muscle weakness mediated 27–41% of this effect. Interpretation: Sleep disturbance following hospital admission for COVID-19 is associated with dyspnoea, anxiety, and muscle weakness. Due to the association with multiple symptoms, targeting sleep disturbance might be beneficial in treating the post-COVID-19 condition. Background: Most studies of immunity to SARS-CoV-2 focus on circulating antibody, giving limited insights into mucosal defences that prevent viral replication and onward transmission. We studied nasal and plasma antibody responses one year after hospitalisation for COVID-19, including a period when SARS-CoV-2 vaccination was introduced. Methods: In this follow up study, plasma and nasosorption samples were prospectively collected from 446 adults hospitalised for COVID-19 between February 2020 and March 2021 via the ISARIC4C and PHOSP-COVID consortia. IgA and IgG responses to NP and S of ancestral SARS-CoV-2, Delta and Omicron (BA.1) variants were measured by electrochemiluminescence and compared with plasma neutralisation data. Findings: Strong and consistent nasal anti-NP and anti-S IgA responses were demonstrated, which remained elevated for nine months (p < 0.0001). Nasal and plasma anti-S IgG remained elevated for at least 12 months (p < 0.0001) with plasma neutralising titres that were raised against all variants compared to controls (p < 0.0001). Of 323 with complete data, 307 were vaccinated between 6 and 12 months; coinciding with rises in nasal and plasma IgA and IgG anti-S titres for all SARS-CoV-2 variants, although the change in nasal IgA was minimal (1.46-fold change after 10 months, p = 0.011) and the median remained below the positive threshold determined by pre-pandemic controls. Samples 12 months after admission showed no association between nasal IgA and plasma IgG anti-S responses (R = 0.05, p = 0.18), indicating that nasal IgA responses are distinct from those in plasma and minimally boosted by vaccination. Interpretation: The decline in nasal IgA responses 9 months after infection and minimal impact of subsequent vaccination may explain the lack of long-lasting nasal defence against reinfection and the limited effects of vaccination on transmission. Background: The risk factors for recovery from COVID-19 dyspnoea are poorly understood. We investigated determinants of recovery from dyspnoea in adults with COVID-19 and compared these to determinants of recovery from non-COVID-19 dyspnoea. Methods: We used data from two prospective cohort studies: PHOSP-COVID (patients hospitalised between March 2020 and April 2021 with COVID-19) and COVIDENCE UK (community cohort studied over the same time period). PHOSP-COVID data were collected during hospitalisation and at 5-month and 1-year follow-up visits. COVIDENCE UK data were obtained through baseline and monthly online questionnaires. Dyspnoea was measured in both cohorts with the Medical Research Council Dyspnoea Scale. We used multivariable logistic regression to identify determinants associated with a reduction in dyspnoea between 5-month and 1-year follow-up. Findings: We included 990 PHOSP-COVID and 3309 COVIDENCE UK participants. We observed higher odds of improvement between 5-month and 1-year follow-up among PHOSP-COVID participants who were younger (odds ratio 1.02 per year, 95% CI 1.01–1.03), male (1.54, 1.16–2.04), neither obese nor severely obese (1.82, 1.06–3.13 and 4.19, 2.14–8.19, respectively), had no pre-existing anxiety or depression (1.56, 1.09–2.22) or cardiovascular disease (1.33, 1.00–1.79), and shorter hospital admission (1.01 per day, 1.00–1.02). Similar associations were found in those recovering from non-COVID-19 dyspnoea, excluding age (and length of hospital admission). Interpretation: Factors associated with dyspnoea recovery at 1-year post-discharge among patients hospitalised with COVID-19 were similar to those among community controls without COVID-19. Abstract: Background The impact of COVID-19 on physical and mental health and employment after hospitalisation with acute disease is not well understood. The aim of this study was to determine the effects of COVID-19-related hospitalisation on health and employment, to identify factors associated with recovery, and to describe recovery phenotypes. Methods The Post-hospitalisation COVID-19 study (PHOSP-COVID) is a multicentre, long-term follow-up study of adults (aged ≥18 years) discharged from hospital in the UK with a clinical diagnosis of COVID-19, involving an assessment between 2 and 7 months after discharge, including detailed recording of symptoms, and physiological and biochemical testing. Multivariable logistic regression was done for the primary outcome of patient-perceived recovery, with age, sex, ethnicity, body-mass index, comorbidities, and severity of acute illness as covariates. A post-hoc cluster analysis of outcomes for breathlessness, fatigue, mental health, cognitive impairment, and physical performance was done using the clustering large applications k-medoids approach. The study is registered on the ISRCTN Registry (ISRCTN10980107). Findings We report findings for 1077 patients discharged from hospital between March 5 and Nov 30, 2020, who underwent assessment at a median of 5·9 months (IQR 4·9–6·5) after discharge. Participants had a mean age of 58 years (SD 13); 384 (36%) were female, 710 (69%) were of white ethnicity, 288 (27%) had received mechanical ventilation, and 540 (50%) had at least two comorbidities. At follow-up, only 239 (29%) of 830 participants felt fully recovered, 158 (20%) of 806 had a new disability (assessed by the Washington Group Short Set on Functioning), and 124 (19%) of 641 experienced a health-related change in occupation. Factors associated with not recovering were female sex, middle age (40–59 years), two or more comorbidities, and more severe acute illness. The magnitude of the persistent health burden was substantial but only weakly associated with the severity of acute illness. Four clusters were identified with different severities of mental and physical health impairment (n=767): very severe (131 patients, 17%), severe (159, 21%), moderate along with cognitive impairment (127, 17%), and mild (350, 46%). Of the outcomes used in the cluster analysis, all were closely related except for cognitive impairment. Three (3%) of 113 patients in the very severe cluster, nine (7%) of 129 in the severe cluster, 36 (36%) of 99 in the moderate cluster, and 114 (43%) of 267 in the mild cluster reported feeling fully recovered. Persistently elevated serum C-reactive protein was positively associated with cluster severity. Interpretation We identified factors related to not recovering after hospital admission with COVID-19 at 6 months after discharge (eg, female sex, middle age, two or more comorbidities, and more acute severe illness), and four different recovery phenotypes. The severity of physical and mental health impairments were closely related, whereas cognitive health impairments were independent. In clinical care, a proactive approach is needed across the acute severity spectrum, with interdisciplinary working, wide access to COVID-19 holistic clinical services, and the potential to stratify care. Abstract: Several studies have identified a negativity [the phonological mismatch negativity (PMN)] preceding the N400 during auditory sentence comprehension. The present study investigated whether the PMN reflects a prelexical or lexical stage of spoken word recognition. Event-related brain potentials (ERPs) were recorded to investigate phonological processing independently from lexical/semantic influences during a task requiring metalinguistic analysis of speech stimuli. Participants were instructed to omit the initial phoneme from a word (“clap” without the /k/) after which they heard a correct (lap) or incorrect (cap, ap, nose) answer. The PMN (peaking at 270 ms) was largest to incorrect items and did not differentiate between items that shared the same rime and items that were phonologically unrelated to the correct choice. Further, the PMN did not differ between word (cap) and nonword (ap) choices. The P300 was largest to correct items but was also seen to choices that rhymed with the correct answer. It is concluded that the PMN serves as a neural marker for the analysis of acoustic input merging with prelexical phonemic expectations.

Abstract: The Materials Research Collaborative Access Team (MRCAT) has completed construction of its bending magnet beamline at the Advanced Photon Source, with commissioning ongoing since October 2008. Full operations including General User access will begin in January 2010. The beamline is designed to operate in two distinct modes: pink beam for lithography, photochemistry and tomography; and monochromatic beam for x‐ray absorption spectroscopy and tomography. Pink beam is obtained by means of a 880 mm water cooled Pt mirror combined with filters, while monochromatic beam is selected using a water‐cooled double‐crystal Si(111) monochromator, providing an energy range from below 4 keV to greater than 33 keV. Switching between modes is accomplished in under one hour.

Abstract: A structural theory is presented for the large static plastic deformation of space frames composed of thin walled members. Displacements comparable to the overall structural dimensions are contemplated. The frame is considered to consist of an arbitrary number of beam elements connected at node points. The analysis assumes that plastic deformation is confined to idealized hinges located at the node points. As a basis for a general frame computer program, the equations for a beam element are derived as a relationship between appropriate generalized force and deformation rates. The structural constitutive theory employed for the plastic hinge includes biaxial bending, torsion, and axial extension. It accounts for reduction in the load carrying capacity of the hinge due to local deformation. Predicted force-deformation curves for a space frame are in good agreement with experimental results.

Abstract: The effect of the spatial distribution of impulsive loads on dynamic snap-through of a shallow circular arch is considered in detail. The Budiansky-Roth criterion is used to establish critical magnitudes of the load for many distributions by numerical integration of an approximate set of equations obtained by Galerkin’s method. It is necessary to distinguish between snap-through on the initial oscillation (immediate) and snap-through occurring during a finite time of the response. Critical magnitudes for both cases are compared to a distribution independent lower bound obtained from an analysis of the critical points. For all distributions considered the lower bound is a less conservative estimate of the critical magnitude for finite time snap-through than for immediate snap-through. The effect of small damping on this conclusion, however, remains an open question.

Abstract: Recent applications of structural plasticity to areas such as vehicle crashworthiness has led to interest in the large deformation plastic collapse of general frames. Even when displacements are comparable to the original structural dimensions, the plasticity is confined to localized regions or “hinges”. This paper reports an experimental study of the behavior of such hinges in thin walled structural members. Due to local deformation the load carrying capacity of the hinge significantly decreases at large rotations. In a companion paper [4] a structural constitutive theory is proposed to account for this behavior. Numerical data for this theory is obtained in the present paper. Finally test results are given for a large deformation combined loading test designed to validate the theory of [4]. The experimental results are in good agreement with the theoretical predictions.

Abstract: When a circular cylindrical shell (plane strain) is subjected to a uniform radial impulse, the resulting circular mode may be unstable. In such a case flexural motion is excited, resulting in rather large displacements and stress. A previous nonlinear analysis [1]1 used a linear inextensionality constraint and displacement representation for the flexural response. A formulation employing a nonlinear inextensionality constraint is presented in this paper, and a comparison is made with the earlier work. The most significant result is a fundamental difference between the equations of motion; in this analysis the nonlinear modal coupling is primarily inertial. The condition for stability of the circular mode is unaffected, but substantial differences may occur in the long-term (nonlinear) response.

Abstract: Coupled nonlinear equations of motion in the axial and transverse directions are derived for a column under dynamic end load. Material dissipation is included by employing a Kelvin model for the material behavior. The dynamic response of the column in the presence of small transverse perturbations is investigated for short duration of axial loads. For certain geometric configurations, the inclusion of axial inertia permits parametric resonance of transverse modes due to the transient axial motion after the load is removed. The parametric effects are studied in detail. It is found that the usual simplified theory which neglects axial inertia adequately describes the column response provided the material dissipation is sufficiently large and/or the duration of loading is sufficiently long. For given dissipation parametric excitation becomes important as the load duration decreases. It becomes the dominant effect for very short duration loads.

Abstract: A perturbation method for treating the scattering of plane waves by small surface imperfections on an elastic half space is presented. The solution to the first order approximation is given as convolution integrals of the surface imperfection with kernel functions defined by Fourier inversion integrals. The evaluation of these integrals is discussed and their asymptotic representations determined. The far field scattered displacements are explicitly obtained for arbitrary imperfections. The scattered field consists of a Rayleigh surface wave and four body phases which at the free surface travel with the speed of dilational or distortional waves. Numerical examples are given. In particular the error in the apparent angle of emergence due to the scattered waves is obtained. The body phases exhibit the familiar 3/2 geometric attenuation, but still may make a significant contribution at moderately long distances. A strong dependence of the magnitude of the error on the angle of incidence is demonstrated.

Abstract: A modified linear membrane theory for the pressurized toroid is presented for which the displacement field is nonsingular. The derivation rests on a nonlinear formulation, but the equations are linearized by exploiting the insensitivity of the meridional stress resultant to the deformation. The resulting equilibrium equations are not statically determinate, containing two geometric variables. Two compatability equations complete the formulation. It is possible to construct a quadratic functional such that its vanishing first variation generates the derived boundary-value problem. Approximate solutions are then obtained by direct methods of the variational calculus. The results are compared with previously published nonlinear solutions and show very good agreement for a wide range of the load parameter. The present formulation readily permits generalization to orthotropic toroids.

Abstract: The response of a cylindrical shell to a uniform radial impulse is a simple harmonic motion in which the cross section remains circular. In the presence of small nonuniformities in the impulse, this motion may be unstable. The instability is characterized by the parametric excitation of flexural modes which exchange energy with the basic motion in a cyclic manner. The stability criterion derived here for finite length cylinders shows this instability may occur over a wide range of shell geometries. The flexural modes that exhibit significant growth are identified. With this, the finite time response of the shell is obtained by numerical integration of the nonlinear equations. The nonlinear response is dominated by a few flexural modes that initially have high growth rates. The amplification of these modes results in displacements and stresses considerably in excess of those associated with the unperturbed response. Finally the imperfection sensitivity of the nonlinear motion is examined.

Abstract: A long cylindrical shell bonded to a solid elastic core is subject to external time-dependent loading. The loading is uniform along the generator of the cylinder but may have arbitrary variation around the circumference. The solution is obtained by means of the Laplace transform. Inversion is accomplished by the Residue Theorem, giving the solution in the form of a modal series. Numerical results for impulsive loads demonstrate the wave nature of the motion in the core. The dynamic stress concentration due to the focusing effect is evident. The effect of wave propagation in the core on the motion of the shell varies widely depending upon the material properties of the core. These properties also affect the rate of convergence of the modal solution.

Abstract: When a closed spherical shell is suddenly enveloped by a uniform impulse, the response is a purely radial motion of each shell element. If the impulse is slightly nonuniform, this membrane “breathing mode” may be dynamically unstable due to the interaction of the membrane stress with flexural curvature. The linear solution for axisymmetric motion divides into two sets of modes. One set, the membrane modes, is essentially extensional; the second set, the composite modes, is neither purely extensional nor purely inextensional. The linear response to an arbitrary radial velocity distribution indicates that breathing mode stability may be considered with respect to perturbations exciting only composite modes. Using this result the nonlinear study shows that a cyclic energy exchange may occur between the breathing mode and rather high‐order composite modes. The ensuing deflections and stresses may be considerably in excess of those determined by consideration of the breathing mode only.

Abstract: Shallow shell theory is used to investigate the non-linear plane deformation of a circular cylindrical panel elastically restrained against rotation at the supports. The critical or equilibrium configurations which may exist at zero load are determined. By examining the local stability of the various configurations, the critical rotational stiffness is obtained, above which the shell cannot exhibit dynamic snap-through under impulsive load. Finally for the range of geometries and rotational stiffness for which snap-through may exist, a sufficient condition for stability is given.

Abstract: A number of previous experimental studies on the cord load distribution in loaded and inflated bias-ply tires have revealed greatly different cord load fluctuation patterns between the crown and the side-wall regions of the tire. Typical among these is data taken from reference, where the cord load fluctuation measured at the crown center line of a typical bias-ply tire is as shown in Figure 1. The symmetry of this figure is easily explained by either bending or shell membrane considerations or by some combination of these. On the other hand, cord load fluctuations measured in the side wall of a bias-ply tire typically show fluctuations such as shown in Figures 2 or 3, also taken from the previously cited reference. Both of these latter figures show considerable asymmetry of the cord load as a function of position diagrams as the loaded tire is rolled through the contact patch. Bending of the side wall cannot account for such behavior as shown in Figures 2 or 3, since the changes in radius of curvature in the side wall region are completely symmetric about the center line through the contact patch, denoted by the zero degree point in Figures 2 and 3. Simple pressure vessel considerations are also not capable of explaining such diagrams, since changes in curvature in the side wall region are also completely symmetric and hence would result in symmetric stress distributions.

Abstract: When high-intensity electromagnetic energy impinges on an elastic media, energy is deposited internally by photoelectric absorption and scattering. A spatially nonuniform heating is produced with deformation waves being generated in the media. The transient stresses resulting from such an energy deposition in an initially undisturbed layered media is investigated. The internal energy deposition is considered to be equivalent to an internal heat generation varying exponentially with the distance from the exposed surface. Energy is deposited internally according to a different law in each layer. The energy deposition is assumed to occur in a time negligible in comparison to a characteristic mechanical response time. Only spatial variations normal to the plane of the layers are considered. The equations of linear, uncoupled thermoelasticity, assuming constant material properties, are applied to a two-layer media. A boundary value problem is formulated and solved exactly neglecting heat conduction. Numerical examples are presented for several cases showing the effect of specific energy deposition functions, material thickness ratio, and material parameters. A complete description of interior stress reflection is presented. The influence of energy deposition and other material parameters on the interfacial bond stress is shown. Relatively small changes in layer parameters may have a marked effect on the maximum stress, depending on the manner in which the generated stress waves combine.

Abstract: The paper describes two crash tests designed to validate a computer simulation developed for predicting the large dynamic plastic response of vehicle structures under crash conditions. The test structures were idealized quarter scale models consisting of frame and rigid body elements. Both direct and oblique pole impacts are reported. Impact speed was 30 MPH. Predicted and experimental results are compared for the crush displacements, impact force at the pole barrier, and acceleration histories at two points on the "passenger compartment" mass. Good agreement is obtained for the symmetric test. Results for the oblique test are not as uniformly good, but quantitative agreement is still satisfactory. Comparison of dynamic variables is sensitive to both the filtering of the raw test data and the numerical integration procedure employed in the simulation. It is demonstrated that the use of appropriate error measures to select step size insures the efficient convergence to a signal with the frequency content of interest in crashworthiness applications.

Abstract: Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

Abstract: Background: To provide appropriate cost-effective care for an ageing population, realignment of care provision to conditions common in advanced age, notably, dementia and multi-morbidities is required. The use of outcome measures in practice may enable this., Aim: A collaborative baseline audit was undertaken to understand how best to implement outcome measures into services for people with dementia across clinical settings., Methods: An academic institution set up a 6-month collaborative baseline audit in 11 English sites. Measures comprised: symptoms/concerns (Integrated Palliative care Outcome Scale for Dementia); Phase of Illness; functional status (Australia-modified Karnofsky Performance Scale); and dementia severity (Functional Assessment Staging)., Findings: Measures were completed at first assessment for 225 people with dementia across nine sites. Their completion promoted comprehensive assessments, but challenges also existed, including recording the prevalence/severity of non-physical symptoms by proxy., Conclusions: The joining together of clinical academic expertise enabled mobilisation of expert knowledge into and from clinical practice.

Abstract: Background: Guilt is commonly associated with distress and psychopathology. However, there is a lack of validated measures that assess how people cope with this aversive emotional and cognitive experience., Aims: We therefore developed and validated a self-report measure that assesses how people manage their guilt: the Guilt Management Scale (GMS)., Method: The GMS was administered to a non-clinical (n = 339) and clinical (n = 67) sample, alongside other validated measures of guilt severity, coping, thought control and psychological distress. Results from a principal component analysis (PCA) and assessments of test-retest reliability and internal consistency are presented., Results: The PCA yielded a six subscale solution (Self-Punishment, Reparation, People-Focused, Spirituality, Avoidance and Metacognition), accounting for 56.14% of variance. Test-retest reliability and internal consistency was found to be good-excellent for the majority of subscales. Across samples, Self-Punishment was related to higher levels of guilt and distress whilst Metacognition and Reparation were related to less guilt and distress in the non-clinical sample only., Conclusions: This paper provides preliminary evidence for the psychometric properties of the GMS in a non-clinical sample. With development and validation in clinical samples, the GMS could be used to inform psychological formulations of guilt and assess therapy outcomes.

Abstract: Objective: Anti N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is an autoimmune disorder that was only fully discovered recently and neuropsychological outcome data remains sparse. We present the case of BA, a 19-year-old male, which illustrates the cognitive outcome in an untreated case over a time period of over 2½ years., Method: We conducted three cognitive assessments, including tests of memory and executive functioning, over this time period and considered the evidence for reliable change in memory function using the Wechsler Advanced Clinical Solutions (ACS) serial assessment package., Results: Our findings revealed mild memory problems 6 months post-discharge with, at best, static and potentially declining memory functioning at follow-up assessment 12 months post-discharge. However, the results of testing at 30 months post-discharge revealed significant improvements in immediate and delayed memory index performances., Conclusions: Our report of a case of anti-NMDAR encephalitis provides evidence for spontaneous improvements in memory functioning occurring more than 2 years after initial assessment and also demonstrates both the utility and potential limitations of the ACS serial assessment software when used in a relatively typical clinical assessment situation.

Abstract: Aims: Effect of ante- and postmortem hide clipping on the microbiological quality of beef carcasses., Methods and Results: Bovine carcasses (362) were tested for indicator micro-organisms and the presence of pathogens. Prior to slaughter, hide cleanliness of each animal was categorized on a scale of 1-5 (clean to dirty). Lowest mean aerobic colony counts (ACC) (log(10) 3.0 CFU cm(-2)) came from carcasses where clipping had been performed in lairage, antemortem. ACC from animals clipped online (log(10) 3.2 CFU cm(-2)) were significantly higher (P < 0.05) than those clipped in lairage, but comparable to those carcasses from Category 1 and 2 animals. There were no significant differences in the detection of pathogens from any of the carcass groups. Ultimate pH values for carcasses from Category 3 and 4 animals showed clipping animals in lairage, as opposed to online, resulted in a small, but significant increase (P < 0.05) in pH value (mean pH 5.66 and 5.59, respectively)., Conclusions: Hide clipping does not adversely affect microbiological quality of carcasses, although higher ultimate pH values indicate increases in antemortem stress., Significance and Impact of the Study: Hide clipping carcasses both ante- and postmortem appears to be an effective intervention to minimize transfer of hide microflora to carcasses during slaughtering operations. Online clipping offers advantages for animal welfare and improves safety for operatives.

39. Influence of phonological expectations during a phoneme deletion task: evidence from event-related brain potentials.

Abstract: Several studies have identified a negativity [the phonological mismatch negativity (PMN)] preceding the N400 during auditory sentence comprehension. The present study investigated whether the PMN reflects a prelexical or lexical stage of spoken word recognition. Event-related brain potentials (ERPs) were recorded to investigate phonological processing independently from lexical/semantic influences during a task requiring metalinguistic analysis of speech stimuli. Participants were instructed to omit the initial phoneme from a word ("clap" without the/k/) after which they heard a correct (lap) or incorrect (cap, ap, nose) answer. The PMN (peaking at 270 ms) was largest to incorrect items and did not differentiate between items that shared the same rime and items that were phonologically unrelated to the correct choice. Further, the PMN did not differ between word (cap) and nonword (ap) choices. The P300 was largest to correct items but was also seen to choices that rhymed with the correct answer. It is concluded that the PMN serves as a neural marker for the analysis of acoustic input merging with prelexical phonemic expectations.

Abstract: Spinal cord stimulation (SCS) is a treatment modality for medically intractable chronic pain. This study reports an 11-year experience with SCS assessing long-term pain relief and specifically evaluating complications and revisions. It took the form of a retrospective review of medical/surgical records with a postal questionnaire. The subjects were 102 patients with medically intractable chronic pain who underwent SCS implantation between 1989 and 2000. There were 64 revision operations carried out on 35 patients. These comprised electrode replacement/repositioning (29), generator replacement (23), cable failure (3) and implant removal (5). Five (4.9%) implants became infected and 2 required removal. Clinician-reported pain relief was substantial in 69 (68%) patients. This study adds to the weight of evidence that patients undergoing SCS derive significant benefits in terms of pain relief. 41. EL-4 and macrophages: generation of chemotactic monokines.

Abstract: Various effects of EL-4 lymphoma supernatant fluids on macrophages were studied. Macrophages migrate under agarose to medium conditioned by the EL-4 cell line for up to 40 h. EL-4 conditioned medium after 67 h contained dialyzable migration inhibitors. In addition, upon direct treatment with the EL-4 supernatant, macrophages took on a more elongated or polarized morphology as compared to control medium or another tumour cell line conditioned medium. A 2 h treatment of macrophages with the EL-4 culture supernatant was sufficient to trigger the production and/or release of chemotactic monokines. These monokines were chemotactic for both PMNs and macrophages. In addition, a prior state of activation of the macrophage does not appear to be necessary for the production and/or release of the chemotactic monokines since resident as well as stimulated peritoneal macrophages are equally effective.
