Objectives COVIDTrach is a UK multicentre prospective cohort study project that aims to evaluate the outcomes of tracheostomy in patients with COVID-19 receiving mechanical ventilation and record the incidence of SARS-CoV-2 infection among healthcare workers involved in the procedure.Design Data on patient demographic, clinical history and outcomes were entered prospectively and updated over time via an online database (REDCap). Clinical variables were compared with outcomes, with logistic regression used to develop a model for mortality. Participants recorded whether any operators tested positive for SARS-CoV-2 within 2 weeks of the procedure.Setting UK National Health Service departments involved in treating patients with COVID-19 receiving mechanical ventilation.Participants The cohort comprised 1605 tracheostomy cases from 126 UK hospitals collected between 6 April and 26 August 2020.Main outcome measures Mortality following tracheostomy, successful wean from mechanical ventilation and length of time from tracheostomy to wean, discharge from hospital, complications from tracheostomy, reported SARS-CoV-2 infection among operators.Results The median time from intubation to tracheostomy was 15 days (IQR 11, 21). 285 (18%) patients died following the procedure. 1229 (93%) of the survivors had been successfully weaned from mechanical ventilation at censoring and 1049 (81%) had been discharged from hospital. Age, inspired oxygen concentration, positive end-expiratory pressure setting, fever, number of days of ventilation before tracheostomy, C reactive protein and the use of anticoagulation and inotropic support independently predicted mortality. Six reports were received of operators testing positive for SARS-CoV-2 within 2 weeks of the procedure.Conclusions Tracheostomy appears to be safe in mechanically ventilated patients with COVID-19 and to operators performing the procedure and we identified clinical parameters that are predictive of mortality.Trial registration number The study is registered with ClinicalTrials.Gov (NCT04572438).

ABSTRACT This study was conducted to evaluate the anticoccidial potential of Artemisia brevifolia extract (ABE) and its effect on biochemical parameters against experimental coccidiosis in broiler chickens. For this purpose, a total of 150 broiler chicks were procured and divided into 5 equal groups (30 birds per group). At one week of age, the first three groups (A, B and C) were orally fed with ABE at dose rates of 100, 200 and 300mg/kg respectively for seven consecutive days. Group D served as infected/medicated and was treated with Baycox® (1ml/L of water). Group E served as the infected/non-medicated control group. At two weeks of age, all groups were infected with 50,000 sporulated oocysts of Eimeria tenella. The anticoccidial potential was evaluated based on lesion score, oocyst score, fecal score, feed conversion ratio, and mortality (%). Furthermore, the effect of ABE on hematological parameters and serum chemistry was also evaluated by following standard procedures. The anticoccidial effects were compared with the standard drug Baycox® (p>0.05). Results of the study showed the anticoccidial activity of ABE in terms of lesion diminishing, oocyst and fecal scores, better feed conversion ratios, and mortality reduction (%). The ABE also improved hematological parameters and stabilized the serum enzymes (ALT, LDH, Creatinine) of infected chicks (p>0.05).

Background: Malaria is one of the most devastating protozoal diseases in under developing coun­tries like Pakistan where health facilities are scarce. It is the second most frequently reported disease with 4.5 million suspected cases in Pakistan. The current study was designed to determine the inci­dence of pediatric malaria in Quetta, Balochistan.Methods: The study was conducted at Children Hospital Quetta (CHQ) during July 2011march 2012. Blood samples were collected from 3418 clinically suspected and were evaluated using thin and thick blood films stained with Giemsa stain.Results: Out of 3418 total of 230 (6.72%) children were found positive for any of the malarial para­sitic infestation. Plasmodium vivax was observed to be more common 54.34 % (n= 125/230) than P. falciparum 44.78% (n=103/230). Male children were 65.21% (150/230) i.e. two times more com­monly affected than female 34.78% (80/230) children. The prevalence among age groups was 7.41% (n=89/1200) in preschool-aged children aged 1-5 years, 7.11% (n=75/1054) in school-aged children aged 6—10 years while 6.78% (n=46/678) in 11-15 years-old children, and 6.66% (n=20/300) in >15 year-olds children. Peak prevalence was noted in summer and mild in winter. Mixed infection of (0.86%: 2/230) P. vivax and P. falciparum was also observed in two cases although no case of P. mala­riae or P. ovale infection was seen during entire study.Conclusion: The results reflect the higher prevalence of malaria in Quetta, Pakistan that poses a signifi­cant health threat and requires urgent attention of high-ups to launch programme to control the disease in the area

The measuring of Ca2+, Mg2+, K+ has been carried out. This quantity determination had two purposes. It allows us, on the one hand, to show that the assimilation rate of the studied mineral elements and the synthesis of the dry matter are in relation with the composition of the plant organs. On the other hand, to analyse the Na+/ NH4 + antagonism in constantly keeping the concentration of each anion and cation. Within the adjusted saline water T1C, T2C, T3C changing the proportion of NH4 + from 15% to 25% in the total mineral nitrogen of the modified saline solution T1 mod, T2mod, T3, mod. At the level of the two studied species, tomato and bean, the K+ is very abundant with regard to the cations Ca2+, Mg2+, and Na+ probably due to the weak capacity of the cationic root exchange of these plants. The roots of the tomato plants are less loaded with Na+ than the aerial parts. However, the bean stores much more Na+ at the level of the roots, which migrate with difficulty to the aerial parts [2].

Increasing electrical energy consumption during peak hours leads to increased electrical energy losses and the spread of environmental pollution. For this reason, demand-side management programs have been introduced to reduce consumption during peak hours. This study proposes an efficient energy optimization in Smart Urban Buildings (SUBs) based on Improved Sine Cosine Algorithm (ISCA) that uses the load-shifting technique for demand-side management as a way to improve the energy consumption patterns of a SUBs. The proposed system's goal is to optimize the energy of SUBs appliances in order to effectively regulate load demand, with the end result being a reduction in the peak to average ratio (PAR) and a consequent minimization of electricity costs. This is accomplished while also keeping user comfort as a priority. The proposed system is evaluated by comparing it with the Grasshopper Optimization Algorithm (GOA) and unscheduled cases. Without applying an optimization algorithm, the total electricity cost, carbon emission, PAR and waiting time are equal to 1703.576 ID, 34.16664 (kW), and 413.5864s respectively for RTP. While, after applying GOA, the total electricity cost, carbon emission, PAR and waiting time are improved to 1469.72 ID, 21.17 (kW), and 355.772s respectively for RTP. While, after applying the ISCA Improves the total electricity cost, PAR, and waiting time by 1206.748 ID, 16.5648 (kW), and 268.525384s respectively. Where after applying GOA, the total electricity cost, PAR, and waiting time are improved to 13.72 %, 38.00 %, and 13.97 % respectively. And after applying proposed method, the total electricity cost, PAR, and waiting time are improved to 29.16 %, 51.51 %, and 35.07 % respectively. According to the results, the created ISCA algorithm performed better than the unscheduled case and GOA scheduling situations in terms of the stated objectives and was advantageous to both utilities and consumers. Furthermore, this study has presented a novel two-stage stochastic model based on Moth-Flame Optimization Algorithm (MFOA) for the co-optimization of energy scheduling and capacity planning for systems of energy storage that would be incorporated to grid connected smart urban buildings.

Objectives: To investigate extracts of the stem bark of Ziziphus jujuba (L.) Gaertn. var. hysudrica Edgew. (Rhamnaceae) for anti-inflammatory activity and isolate the active principle(s). Methods: The dry powder was macerated separately in three types of solvents to prepare methanol extract (ME), ethyl acetate extract (EE), and chloroform extract (CE). Following in vitro anti-inflammatory screening, the most active extract was selected to isolate the active compound. Both, the active extract and isolated compound were further tested on rats using the carrageenan-induced inflammation model. IntroductionExtracellular vesicles are membrane-bound structures secreted into the extracellular milieu by cells and can carry bioactive molecules. There is emerging evidence suggesting that EVs play a role in the diagnosis, treatment, and prognosis of certain cancers. In this study, we investigate the association of EVs bearing PD-L1 and molecules important in B-cell activation and differentiation with AIDS-NHL risk.MethodsEVs were isolated from archived serum collected prior to the diagnosis of AIDS-NHL in cases (N = 51) and matched HIV+ controls (N = 52) who were men enrolled in the Los Angeles site of the MACS/WIHS Combined Cohort Study (MWCCS). Serum specimens of AIDS-NHL cases were collected at a mean time of 1.25 years (range of 2 to 36 months) prior to an AIDS-NHL diagnosis. The expression of PD-L1 and other molecules on EVs (CD40, CD40L, TNF-RII, IL-6Rα, B7-H3, ICAM-1, and FasL) were quantified by Luminex multiplex assay.Results and discussionWe observed significantly higher levels of EVs bearing PD-L1, CD40, TNF-RII and/or IL-6Rα in AIDS-NHL cases compared with controls. Using multivariate conditional logistic regression models adjusted for age and CD4+ T-cell count, we found that EVs bearing PD-L1 (OR = 1.93; 95% CI: 1.10 – 3.38), CD40 (OR = 1.97, 95% CI: 1.09 – 3.58), TNF-RII (OR = 5.06; 95% CI: 1.99 – 12.85) and/or IL-6Rα (OR = 4.67; 95% CI: 1.40 – 15.53) were significantly and positively associated with AIDS-NHL risk. In addition, EVs bearing these molecules were significantly and positively associated with non-CNS lymphoma: PD-L1 (OR = 1.94; 95% CI: 1.01 – 3.72); CD40 (OR = 2.66; 95% CI: 1.12 – 6.35); TNF-RII (OR = 9.64; 95% CI: 2.52 – 36.86); IL-6Rα (OR = 8.34; 95% CI: 1.73 – 40.15). These findings suggest that EVs bearing PD-L1, CD40, TNF-RII and/or IL-6Rα could serve as biomarkers for the early detection of NHL in PLWH.

This study presents the design and implementation of a piezoelectric power harvesting device to capture vibrational energy from pipelines to self-powered IoT devices. The device utilizes key components along with the PPA-1001 piezoelectric sensor, the STM32F103C8T6 microcontroller, and LTC-3588 energy harvesting power supply. Experimental results verified the system's performance in harvesting power within a specific frequency range of 10 Hz to 50 Hz, with the foremost overall performance at 30 Hz. The device generated the highest voltage of 3.3 V, delivering a power output of 2.18 mW, which is sufficient to power low-power electronic devices. The device maintained solid performance across a temperature range of 40 °C to 50 °C, underscoring its robustness in various environmental situations. The findings highlight the capacity of this form of generation to offer a sustainable power source for remote pipeline tracking, contributing to stronger protection and operational efficiency. Introduction Epilepsy is a neurologic condition characterized by spontaneous jerky body movements. It is a chronic morbid condition mostly diagnosed during childhood. Patients are maintained on long-term medications to prevent recurrent seizures that can damage the brain. Medications used for the management of epilepsy have several side effects and require proper monitoring. Patients with epilepsy are at increased risk of psychiatric comorbidities. Objectives Our aim is to find factors responsible for causing suicidality in patients with epilepsy. Methods A review was conducted using Pubmed database with the search terms [epilepsy] OR [neurological conditions] OR [suicide] OR [suidical attempt] OR [suicidal ideation] OR [depression] OR [psychiatric diseases] OR [mood disorders] OR [anxiety] OR [sleeplessness] which yielded around 800 articles. The number was later reduced to be centered around the main area of interest and produced around 40 articles. Results Results show that many contributing factors play an unavoidable role in promoting suicidal ideation that can lead to suicidal attempts in epilepsy. The stigma associated with epilepsy leads to social isolation, lack of opportunities, financial constraints, and impact on close relationships which can be the reasons for depression. According to the review, the foremost contributing factor is the underlying social, emotional, and economic condition of epilepsy patients. Depression and anxiety are the most prevalent psychiatric comorbidities in epileptic patients. Epileptic patients who develop psychiatric ailments show decreased adherence to medications that further worsen the problem. Conclusions Though, this area in neurology has started getting attention for further research and guidelines. But efforts are still inadequate for this to be put into clinical practice. More desperate actions needed to be taken for proper diagnosis and management of suicidal ideations by proper use of assessment tools so that timely actions are planned. A small percentage of bladder cancers in the general population have been found to harbor DNA viruses. In contrast, up to 25% of tumors of solid organ transplant recipients, who are at an increased risk of developing bladder cancer and have an overall poorer outcomes, harbor BK polyomavirus (BKPyV). To better understand the biology of the tumors and the mechanisms of carcinogenesis from potential oncoviruses, we performed whole genome and transcriptome sequencing on bladder cancer specimens from 43 transplant patients. Nearly half of the tumors from this patient population contained viral sequences. The most common were from BKPyV (N=9, 21%), JC polyomavirus (N=7, 16%), carcinogenic human papillomaviruses (N=3, 7%), and torque teno viruses (N=5, 12%). Immunohistochemistry revealed variable Large T antigen expression in BKPyV-positive tumors ranging from 100% positive staining of tumor tissue to less than 1%. In most cases of BKPyV-positive tumors, the viral genome appeared to be clonally integrated into the host chromosome consistent with microhomology-mediated end joining and coincided with focal amplifications of the tumor genome similar to other virus-mediated cancers. Significant changes in host gene expression consistent with the functions of BKPyV Large T antigen were also observed in these tumors. Lastly, we identified four mutation signatures in our cases, with those attributable to APOBEC3 and SBS5 being the most abundant. Mutation signatures associated with an antiviral drug, ganciclovir, and aristolochic acid, a nephrotoxic compound found in some herbal medicines, were also observed. The results suggest multiple pathways to carcinogenesis in solid organ transplant recipients with a large fraction being virus-associated.

ABSTRACT Phytogenic feed additives have been increasingly used in the last decade, and several plants and their metabolites have been investigated for the said purpose. In this context, present study aimed at evaluating the effects of Moringa oleifera as feed additive on layer performance, and egg bioactive compound levels and nutrient profile. HyLine W36 layers (n=200), 50 weeks of age, were randomly distributed in four treatments with five replicates of ten birds each. Four isocaloric (2725 kcal/kg) and isonitrogenous (CP 16%) diets were formulated and supplemented with 0, 0.5, 1.0, or 1.5% (w/w) of dried Moringa oleifera leaf powder (MLM). The results showed positive effects of MLM on egg production, egg mass, and feed conversion ratio, but negative effects on egg quality (p≤0.05). The contents of bioactive compounds, like β-carotene, quercetin, and selenium, in the diet and in the egg yolk were significantly (p≤0.05) higher in the group fed1.5% MLM, with values of 8.90, 48.88, and 0.54 mg/kg feed and 4906, 241 and 56.82 µg/100g yolk, respectively. Creatinine and glucose serum levels and cholesterol levels (serum and eggs) linearly increased as a function of increasing MLM dietary levels (p≤0.05). Antibody titers against Newcastle Disease significantly improved (p≤0.05) in the group fed the diet supplemented with 1.5% MLM. It was concluded that Moringa oleifera used as phytogenic feed additive enriches eggs with bioactive and functional compounds, and improves the production performance and the health status of layers.

ObjectiveThis study examines the value of risk stratification by documented diagnosis of diabetes and objectively measured height and weight (BMI) in COVID-19 severity and mortality in a large sample of patients in an urban hospital located in Southern California.MethodsData from a retrospective cohort study of COVID-19 patients treated at Cedars-Sinai Medical Center between March 8, 2020, and January 25, 2021, was analyzed. Sociodemographic characteristics and pre-existing conditions were extracted from electronic medical records. Univariable and multivariable logistic regression models identified associated risk factors, and a regression causal mediation analysis examined the role of diabetes in the association between obesity and illness severity. All analyses were stratified by age (ResultsAmong individuals ConclusionOur findings have clinical implications in documenting which patients may be at elevated risk for adverse outcomes. More in-depth prospective studies are needed to capture how glycemic regulation may influence prognosis.

Background and objective: Stroke is the second leading cause of death in Jordan and over the world. Knowledge and awareness towards stroke play a crucial role in the management and prevention of its complications. This study aims to assess the knowledge and awareness about stroke among the Jordanian population and determine factors associated with stroke awareness. Methods: This cross-sectional study through a web-based anonymous questionnaire that needed 10 minutes to be completed. It examined sociodemographic characteristics and recognition of the risk factors, warning signs, stroke consequences, and early response to stroke symptoms. Logistic regression analysis identified the factors associated with poor knowledge of stroke. Results: A total of 573 Jordanian adults participated in this study. The participant's ability to identify at least one early symptom of stroke and the proper response to the symptoms were significantly correlated with the educational level (OR of 3.4 and 2.5, respectively). At least one consequence of stroke was significantly associated with different demographic factors such as gender, socioeconomic income, females versus males and those with medium income versus low income had significantly higher odds (OR of 6.6 and 4.1, respectively). Conclusion: This study revealed a good knowledge and awareness level about stroke among Jordanians correlated mainly with their educational level. Therefore, new strategies should be considered to decrease the prevalence of stroke in Jordan, including the need for engagement in enhanced awareness campaigns.

This paper presents the influence of polyolefin fiber on the flexural and shear attitude on the flat slabs. Three slab sets (80 cm × 80 cm) were tested, each with a thickness of 10 cm. In the first set (S1), the effect of fiber content on the flexural behavior of the flat slab was considered. Therefore, four slab specimens were cast, one of which was considered as a control specimen with no fiber content, while the other three included fibers at 0.5, 1, and 1.5 percent of the total concrete volume. The second series of experiments studied the flexural behavior of flat slabs (S2) with an opening of 15 cm × 15 cm. The first specimen contained nil polyolefin, while the second included 1% polyolefin. In the third set (S3), consideration was taken for 0 and 1% of Polyolefin to realize the shear behavior of the flat slab. The increase in polyolefin fiber content from 0 to 1.5% (for slab set 1) will decrease the deflection from 4.5 mm to 2.3 mm, with an average of 3.58 mm, which is close to the deflection of a 1% polyolefin fiber specimen. Three dimensional models for the tested slabs were simulated numerically via ABAQUS software program. The ratio of the maximum deflection between the experimental and the numerical outcomes were varied with a range of 1.01 to 1.28, with an average of 1.14.

Human immunodeficiency virus (HIV) infection is associated with an increased risk of non-Hodgkin lymphoma (NHL). Even in the era of suppressive antiretroviral treatment, HIV-infected individuals remain at higher risk of developing NHL compared to the general population. To identify potential genetic risk loci, we performed case-control genome-wide association studies and a meta-analysis across three cohorts of HIV+ patients of European ancestry, including a total of 278 cases and 1924 matched controls. We observed a significant association with NHL susceptibility in the C-X-C motif chemokine ligand 12 (CXCL12) region on chromosome 10. A fine mapping analysis identified rs7919208 as the most likely causal variant (P = 4.77e-11), with the G>A polymorphism creating a new transcription factor binding site for BATF and JUND. These results suggest a modulatory role of CXCL12 regulation in the increased susceptibility to NHL observed in the HIV-infected population.

This paper presents experimental study to retrofitted reinforced concrete Hollow Section Bridge. The study was carried out on the White River Bridge structure (Bai xi da Qiao / China). The effect of retrofitting on stress and strain of beams at the critical section was studied. Evaluating the bridges girder after strengthening using new material called PolyurethaneCement material (PUC) as an external material .This study present the strain and deflection before and after strengthening the bridge girders. The results has shown that the overall state of the bridge structural strengthening is in good condition. The enhancement was significant in stiffness of the bridge structure. Regarding to the results of static load test, the experimental values strain and deflection are less than theoretical values, indicating that the stiffness of the structure, overall deformation and integrity satisfy the designed and standard requirements and the working performance are in good condition, and flexure capacity has a certain surplus.

We studied the effect of adding (Dodecyl Benzene Ammonium Sulphonate) as afunction of different ratio( 1% - 8%) where these additives added to the matrix of Iraqi cement (Om Quasar factory for cement) .All the prepared samples were in form of cubic with aside ( 50 mm ) long and evaluated as afunction of dipping time in distilled water . All the prepared samples had a proportion of cement to sand ). The obtained results showed that the (Dodecyl Benzene Ammonium Sulphonate) tends to decrease the compressive strength of the modified cement mixture with increasing curing time with distilled water where the amount of decrease linearly when curing time is 21 days but less decrease in compressive strength obtained when curing time is 3 and 7 days while the relative compressive strength decrease linearly also as a function of adding ratio when curing time is 3 days. Also surfactant (Dodecyl Benzene Ammonium Sulphonate) acting as plasticizer on cement matrix leading to flexible cement. Finally the obtained results showed that there is a critical ratio of (Dodecyl Benzene Ammonium Sulphonate) depending on the kind of used cement and curing conditions enabling us to use this modified cement in many application specially in industrial and oil for the high workability of this cement and the less water needs.

Optical properties of Rhodamine-B thin film prepared by PLD technique have been investigated. The absorption spectra using 1064nm and 532 nm laser wavelength of different laser pulse energies shows that all the curves contain two bands, B band and Q bands with two branches, Q1 and Q2 band and a small shift in the peaks location toward the long wavelength with increasing laser energy. FTIR patterns for Rhodamine-B powder and thin film within shows that the identified peaks were located in the standard values that done in the previous researches. X-ray diffraction patterns of powder and prepared Rhodamine-B thin film was display that the powder has polycrystalline of tetragonal structure, while the thin film has an amorphous structure. SEM images for Rhodamine-B, shows homogeneous distributed cauliflower-like nanostructures attached with each other. FM diagram shows that the average grain size was found to be 68.52 nm and the RMS surface roughness was 1.68 nm.

Non thermal argon plasma needle at atmospheric pressure was constructed. The experimental set up was based on simple and low cost electric components that generate electrical field sufficiently high at the electrodes to ionize various gases which flow at atmospheric pressure. A high AC power supply was used with 9.6kV peak to peak and 33kHz frequency. The plasma was generated using two electrodes. The voltage and current discharge waveform were measured. The temperature of Ar gas plasma jet at different gas flow rate and distances from the plasma electrode was also recorded. It was found that the temperature increased with increasing frequency to reach the maximum value at 15 kHz, and that the current leading the voltage, which demonstrates the capacitive character of the discharge. The electron temperature was measured at about 0.61 eV, and we calculated the electron number density to be 4.38×1015 cm-3.

Low-pressure capacitively coupled RF discharge Ar plasma has been studied using Langmuir probe. The electron temperature, electron density and Debay length were calculated under different pressures and electrode gap. In this work the RF Langmuir probe is designed using 4MHz filter as compensation circuit and I-V probe characteristic have been investigated. The pressure varied from 0.07 mbar to 0.1 mbar while electrode gap varied from 2-5 cm. The plasma was generated using power supply at 4MHz frequency with power 300 W. The flowmeter is used to control Argon gas flow in the range of 600 standard cubic centimeters per minute (sccm). The electron temperature drops slowly with pressure and it's gradually decreased when expanding the electrode gap. As the gas pressure increases, the plasma density rises slightly at low gas pressure while it drops little at higher gas pressure. The electron density decreases rapidly with expand distances between electrodes.

In this work, the spectra for plasma glow produced by pulse Nd:YAG laser (λ=532 and 1064nm) on Ag:Al alloy with same molar ratio samples in distilled water were analyzed by studying the atomic lines compared with aluminum and silver strong standard lines. The effect of laser energies of the range 300 to 800 mJ on spectral lines, produced by laser ablation, were investigated using optical spectroscopy. The electron temperature was found to be increased from 1.698 to 1.899 eV, while the electron density decreased from 2.247×1015 to 5.08×1014 cm-3 with increasing laser energy from 300 to 800 mJ with wavelength of 1064 nm. The values of electron temperature using second harmonic frequency are greater than of 1064 nm, which increased from 2.405 to 2.444 eV, while the electron density decreased from 2.210×1015 to 1.516×1015 cm-3 with increasing laser energy for the same energy range.

In this work, the elemental constituents of smoker and nonsmoker teeth samples of human were analyzed by Laser induced breakdown spectroscopy method (LIBS). Many elements have been detected in the healthy teeth samples, the important once are Ca, P, Mg, Fe, Pb and Na. Many differences were found between (female and male) teeth in Ca, P, Mg, Na and Pb contents. The concentrations of most toxic elements were found significantly in the smoker group. The maximum concentrations of toxic elements such as Pb, Cd and Co were found in older male age above 60 year. Also, it was found that the minimum concentrations of trace elements such as Ca, P and Na exist in this age group. From these results it is clear that the LIBS technique is a powerful tool for fast identification of teeth problems.
