8. The Prognostic Role of Inflammatory Biomarkers in Metastatic Castration Sensitive Prostate Carcinoma

Abstract

Objectives: Due to limited data in the literature on the prognostic value of inflammatory markers neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), and derived NLR (dNLR) in metastatic castration-sensitive prostate cancer (mCSPC), we aimed to determine the role of this markers in the prognosis of mCSPC. Methods: In this study, inflammatory marker values in mCSPC (NLR0, PLR0, dNLR0) and mCRPC (NLR1, PLR1, dNLR1) were calculated. Characteristics of the patients and the effects of inflammatory markers on overall survival (OS) and cancer-specific survival (CSS) were evaluated using appropriate statistical methods. Results: The median age of 124 patients was 68.71 years. No significant difference was found OS in mCSPC NLR0, dNLR0 and PLR0 groups (p>0,05). While the median CSS was statistically longer in the NLR0, dNLR0 and PLR0 low groups (Median:45.9 vs 35.7 months for NLR0, 47.0 vs 34.6 months for dNLR and 46.2 vs 33.9 months, p=0.037, p=0.036, p=0.041 respectively). There was no significant difference in terms of OS and CSS in NLR1 and dNLR1 groups (p>0.05). The patients with low PLR1 showed statistical significantly better OS and CSS (p=0.027 for OS and p=0.006 for CSS). Conclusion: Although inflammatory markers have prognostic value in many cancers, mCSPC which have heterogeneous and complex structures, are still controversial, and more studies are needed for their routine use. 11. Malignant mesothelioma of tunica vaginalis: Two extremely rare case reports.

Abstract

Introduction: Malignant mesothelioma (MM) of tunica vaginalis is a very rare tumor and it has a poor prognosis. It is not easy to diagnose the disease preoperatively because there is a lack of comprehensive studies and information about the disease. We would like to present our two cases with severe scrotal edema and hydrocele, which were debrided perioperatively due to scrotal necrosis, followed by radical inguinal orchiectomy and subsequently diagnosed as MM of tunica vaginalis. Case 1: A 74-year-old male patient with bilateral scrotal swelling for 6 months. Right radical inguinal orchiectomy was performed. The pathology result of right radical inguinal orchiectomy material was compatible with malignant mesothelioma of tunica vaginalis. Case 2: A 90-year-old male patient with complaints of right scrotal swelling for 3 years. A right radical inguinal orchiectomy was performed. The pathology result of right radical inguinal orchiectomy material was compatible with malignant mesothelioma of tunica vaginalis. Conclusion and discussion: MM has a very poor prognosis therefore early treatment plan should be made. Surgery is the main treatment for malignant mesothelioma of the testis. Although chemotherapy and radiotherapy play an active role in the treatment of metastatic disease, surgery is the first-line therapy in the treatment of the tumor. Also; long-term hydrocele, recurrent epididymitis, scrotal edema can make it harder to detect the tumor diagnosis. MM should be considered in such patients. Necrosis and abscess appearance was not mentioned before in published case reports. In this respect, it should not be ignored the long-term hydrocele situation and the appearance of abscess-epididymitis that cannot be treated properly. [ABSTRACT FROM AUTHOR]

Abstract

Objective: To evaluate the potential significance of the second transurethral resection of a bladder tumor (TURBT) in a population of patients whose primary pathology was high-grade pTa (Ta/HG) and who had received Bacillus Calmette-Guérin (BCG) treatment for at least 12 on oncological outcomes, based on the presence or absence of detrusor muscle. Materials and Methods: Patients with primary Ta/HG tumors (n=207) that met the inclusion criteria were grouped based on the presence of muscle tissue in the first TURBT and whether the secondary TURBT was performed. Progression, recurrence, and disease-free survival rates were compared between the groups. Results: Median follow-up period was 24 (12-205) months. In cases with muscle in the first TURBT, a second TURBT significantly increased the median disease-free survival time compared with those that did not undergo the second TURBT [32 months (12-83) vs 12 months (6-67); p<0.005]. In cases without muscle in the first TURBT, the second TURBT significantly reduced the rate of progression (p<0.05). Regression analysis showed that tumor size >3 cm [95% confidence interval (CI)=1.09-2.96, hazard ratio (HR)=1.79, p=0.021], presence of muscle tissue (95% CI=0.35-0.92, HR=0.57, p=0.022), and multiple tumor (95% CI=1.06-2.90, HR=1.75, p=0.028) were independent factors affecting disease relapse in primary Ta/HG tumor. Conclusions: In patients with primary Ta/HG tumors, if there was no muscle in the first TURBT, a second TURBT should be performed to achieve lower progression rates. If there is muscle in the first TURBT, the second TURBT will only increase the median disease-free survival time. [ABSTRACT FROM AUTHOR]
