Objective: The primary treatment for stage I non-small cell lung cancer (NSCLC) in medically inoperable patients is stereotactic body radiation therapy (SBRT). The current study aimed to retrospectively analyze patients who underwent SBRT. Methods: A total of 188 patients with stage I NSCLC treated with SBRT between 2014 and 2020 were enrolled. Local control (LC), progressionfree survival (PFS), overall survival (OS), and treatment-related toxicities were analyzed. Results: Patients were mostly male (65.7%, n=71), with a median age of 68 (56-88). Based on tumor size and location, 69 patients (63.9%) received between 50 and 60 Gy in 5 fractions, 26 patients (24.1%) received 54 Gy in 3 fractions, 11 patients (10.2%) received 60 Gy in 8 fractions, and 2 patients (1.8%) received 60 Gy in 3 fractions. The median follow-up time was 32 months (12-47 months). Locoregional relapse occurred in 11 patients, among whom 4 (3.7%) developed distant metastasis. The 3-year LC, OS, and PFS rates were 89.5%, 83%, and 72%, respectively. Advanced age and presence of chronic obstructive pulmonary disease were associated with a decreased 3-year OS. In smokers and those with large tumor volumes, PFS decreased to 3 years. No grade 3 or 4 treatment-related toxicities were observed. Conclusion: SBRT is a fast, safe, and valuable therapeutic approach for patients with early-stage medically inoperable NSCLC, providing significant tumor control rates with low toxicity. Objective: Treatment of H3K27M-altered midline gliomas is a significant challenge. This study aims to assess the efficacy of radiotherapy for these patients. Materials and Methods: Thirteen patients with histologically confirmed H3K27M-altered midline glioma, treated with radiotherapy+/- temozolomide between 2019 and 2021, were retrospectively analyzed. Clinical and radiological responses, survival times, and prognostic factors were evaluated. All patients were treated with hypofractionated radiotherapy, with a dose of 25-40 Gy. Results: The median age was 42 years (ranging from 19 to 55). Karnofsky Performance Status scores ranged from 50 to 70, with a median score of 50. Hypofractionated radiotherapy was administered to all patients, with 25 Gy given in 5 fractions to 7 patients (53.8%), 39 Gy in 13 fractions to 3 patients (23.1%), and 40 Gy in 15 fractions to 3 patients (23.1%). Clinically, two patients (15.4%) showed symptomatic improvement, five patients (38.8%) remained stable, and six patients (46.2%) had neurological deterioration. Radiologically, one patient had a partial response, six patients (46.2%) had stable disease, and radiological progression occurred in three patients (23.1%). The median progression-free survival and median overall survival time were 123 and 154 days, respectively. Karnofsky Performance Status 50 was associated with a worse prognosis. Conclusion: These results suggest that the efficacy of radiotherapy might be limited for adults with H3K27M-altered midline gliomas with poor performance status. New studies are necessary for a more comprehensive understanding of effective therapeutic strategies. [ABSTRACT FROM AUTHOR]

Amaç: Bu çalışmada batın bölgesi yerleşimli organlardan ileum, kolon, karaciğer ve pankreasta iyonizan radyasyona bağlı hasarın azaltılmasında Myrtus communis ekstresinin koruyucu etkilerinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Rattus türü Norvecigus cinsi olan, dişi sıçanlar (n=30) dört gruba ayrıldı. Kontrol grubuna (n=6) sadece 4 gün süre ile oral serum fizyolojik (SF) uygulandı. İyonizan radyasyon (IR) grupları uygulama günü başlanarak 4 gün oral SF verilen "IR grubu" (n=8), IR günü başlanarak 4 gün oral Myrtus communis (MC) ekstresi uygulanan "MC tedavi grubu" (n=8), IR'den 4 gün önce başlanarak toplam 8 gün oral MC uygulanan "MC ön tedavi grubu" (n=8) düzenlendi. Myrtus communis, oral gavaj yöntemiyle 100 mg/kg dozunda sabah saatlerinde günde bir kez verildi. Işınlama ketanmin anestezisi altında lineer akseleratör cihazında 6 MV foton enerjisiyle ön-arka iki alandan tek fraksiyonda 10 Gy tüm vücut olarak uygulandı. Deneyin dördüncü gününde tüm denekler sakrifiye edilerek alına dokularda Hematoksilen veEozin boyamasıyla histopatolojiye ve biyokimyasal olarak malondialdehit (MDA), glutatyon (GSH), miyeloperoksidaz (MPO) seviyeleri ve Na-K ATPaz aktivitelerine bakıldı. Grupların karşılaştırmasında istatistiksel olarak t testi kullanıldı. Anlamlılık için p<0.05 seçildi. Bulgular: Pankreas, karaciğer ve ileum dokularında, kontrol grubuna göre IR grubunda artma (p<0.001) MPO aktivitesi ve MDA seviyesinde IR grubuna göre MC tedavi ve MC ön tedavi gruplarında anlamlı olarak azalma izlendi (p<0.01). Kontrol grubuna göre IR grubunda azalan GSH seviyesi ve NaK ATPaz aktivitesi, MC tedavi hem de MC ön tedavi grubunda anlamlı olarak artmıştı. Histopatolojik incelemede ileum ve kolon dokularında IR grubunda ciddi düzeyde izlenen villus ve mukozal glandular hasarın ön tedavili ve tedavili MC gruplarında azaldığı izlendi. Pankreasta IR grubunda ekzokrin pankreas asinus yapılarında düzensizlik ve hücre hasarı, Langerhans adacıklarında hücre organizasyonunda düzensizlik ve hücreler arasında genişleme izlenirken MC eklenen gruplarda bu etkilerde azalma izlenmiştir. Karaciğerde IR grubunda sinusoidal konjesyon, hepatik kordların organizasyonunda bozulma ve hepatosit hasarı izlenirken MC eklenen gruplarda oldukça azalmış sinuzoidal konjesyon hasarlı hepatositlerin sayında azalma olduğu görüldü (Şekil 1). Sonuç: Myrtus communisin sıçan gastrointestinal sistem dokularında iyonizan radyasyonun inflamatuvar etkilerine bağlı hasarı anlamlı olarak azalttığı görülmektedir. Çalışmamız bu bitkinin klinikte yeni bir radyoprotektan ajan olarak uygulanabilirliği konusunda in vivo öncü bir çalışma niteliğindedir. [ABSTRACT FROM AUTHOR]
