Background We investigated the notification trends of sexually acquired chlamydia (chlamydia) and its association with testing in Sweden before (1992–2004) and after (2009–2018) the discovery of a new variant of Chlamydia trachomatis (nvCT). Methods We applied monthly time series analysis to study chlamydia trends and annual time series to study chlamydia rates adjusted for testing. We analyzed incidence nationally and by county group (based on able and unable to detect nvCT at time of discovery). Results We present data on 606,000 cases of chlamydia and 9.9 million persons tested. We found a U-shaped chlamydia trend during the period 1992–2004, with an overall increase of 83.7% from 1996 onward. The period 2009–2018 began with a stable trend at a high incidence level followed by a decrease of 19.7% during the period 2015–2018. Peaks were seen in autumn and through during winter and summer. Similar results were observed by groups of county, although with varying levels of increase and decrease in both periods. Furthermore, increased testing volume was associated with increased chlamydia rates during the first period (P = 0.019) but not the second period. Conclusions Our results showed that chlamydia trends during the period 2009–2018 were not driven by testing, as they were during the period 1992–2004. This suggests less biased notified chlamydia rates and thus possibly a true decrease in chlamydia incidence rates. It is important to adjust case rates for testing intensity, and future research should target other potential factors influencing chlamydia rates.

BACKGROUND: Relationship status is an important factor associated with condomless anal intercourse (CAI) amongst men who have sex with men (MSM). METHODS: A multi-centre bio-behavioural survey with MSM was conducted in 13 European cities (n = 4901) exploring factors associated with CAI via bivariate and multivariate multilevel logistic regression analyses. RESULTS: Likelihood of CAI with casual partners was associated with being 'out' to a majority (AOR = 1.19;95% CI 1,1.42); knowing their HIV status (AOR = 1.86; 95% CI 1.25,2.76); using substances (1-2 AOR = 1.39; 95% CI 1.16,1.63, 2+ AOR = 1.81; 95% CI 1.35,2.42); being older (AOR = 0.98; 95% CI 0.97,0.99); successful sero-communication (AOR = 0.79; 95% CI 0.67,0.94); and, not having a recent HIV test (AOR = 0.78; 95% CI 0.66,0.92). CAI with steady partners was associated with successful sero-communication (AOR = 2.72; 95% CI 2.72,3.66); not having a recent HIV test (AOR = 1.26; 95% CI 1.09,1.46), and; being older (AOR = 0.99; 95% CI 0.98,0.99). CONCLUSIONS: Understandings of partner type and/or relationship status in relation to CAI amongst MSM can potentially play an important role in the development of culturally appropriate HIV/STI prevention and risk-reduction efforts targeting at-risk MSM. Our results speak to the need to consider segmented and tailored public health and health promotion initiatives for MSM with differing CAI behaviours and relationship profiles. publishersversion published

Aims: Methods for prediction of the peak of the influenza from early observations are suggested. These predictions can be used for planning purposes. Methods: In this study, new robust methods are described and applied on weekly Swedish data on influenza-like illness (ILI) and weekly laboratory diagnoses of influenza (LDI). Both simple and advanced rules for how to predict the time and height of the peak of LDI are suggested. The predictions are made using covariates calculated from data in early LDI reports. The simple rules are based on the observed LDI values while the advanced ones are based on smoothing by unimodal regression. The suggested predictors were evaluated by cross-validation and by application to the observed seasons. Results: The relation between ILI and LDI was investigated and it was found that the ILI variable is not a good proxy for the LDI variable. The advanced prediction rule regarding the time of the peak of LDI had a median error of 0.9 weeks, and the advanced prediction rule for the height of the peak had a median deviation of 28%. Este documento tiene como fin proporcionar una visión actualizada de los niveles y tendencias de las principales variables demográficas en Costa Rica. La agenda se inicia con una descripción del cambio histórico registrado en la mortalidad y la fecundidad, cuantificando también la magnitud de la migración interna. En el Capítulo Dos se analiza el comportamiento de la mortalidad infantil y en el Tres, la reducción en la mortalidad de los adultos. El Capítulo Cuatro se dedica al análisis de la fecundidad considerando el tamaño de familia, la fecundidad reciente, así como la conducta reproductiva en el espaciamiento de los hijos. Utilizando el esquema de los determinantes próximos de la fecundidad, el documento continua en el Capítulo Cinco con un análisis de la edad a la primera unión marital, el estado conyugal, la disolución y el tiempo vivido en la primera unión. Estos son factores que miden el grado de exposición a concebir, lo que se complementa con una descripción de la práctica sexual premarital. El comportamiento reproductivo postparto, es el tema que trata el Capítulo Seis, el cual analiza los patrones de lactancia como principal determinante de la esterilidad temporal postparto. Estos factores se consideran en el contexto de reanudación de la actividad sexual y el grado de protección anticonceptiva. Este último aspecto es tratado más detenidamente en el Capítulo Siete, que describre el conocimiento de métodos y fuentes, el aprendizaje y la difusión de la planificación familiar, así como el uso pasado y los niveles actuales de práctica anticonceptiva. El Capítulo Ocho considera en detalle, las actitudes de las mujeres costarricenses hacia el aborto provocado y el cambio experimentado en el período 1976-1993. Las cifras sobre actitudes se complementan con estimaciones de los niveles de práctica del aborto inducido en Costa Rica. Finalmente, el Capítulo 9 intenta organizar la población de Costa Rica en una tipología de hogares. Esta parte destaca los aspectos relacionados con el tamaño y composición, la jefatura femenina y el ciclo vital de la familia. Muchas son las fuentes de datos a las que se ha recurrido para preparar los artículos descritos. Merecen destacarse las Estadísticas Vitales, los Censos de Población, especialmente los de 1973 y 1984 y las Encuestas de Hogares, labores encomendadas a la Dirección General de Estadística y Censos. Las encuestas de fecundidad han sido también indispensables, en especial la Encuesta Nacional de Salud Reproductiva de 1 993, llevada a cabo por la Caja Costarricense de Seguro Social, que ha sido la última en una secuencia de investigaciones en el campo que incluye la encuesta Nacional de Fecundidad de 1976 y la Encuesta de Fecundidad y Salud de 1986, también usadas aquí. La constelación de fuentes de datos y membresía de los autores anticipa ya la existencia de una amplia cooperación interinstitucional. Sin embargo, la lista no estaría completa sin nombrar y agradecer especialmente el apoyo logístico del Ministerio de Planificación Nacional y Política Económica y la ayuda financiera del Fondo de Población de las Naciones Unidas. Esperamos que la cooperación de los autores y el esfuerzo institucional contribuyan a entender mejor la dinámica demográfica de Costa Rica.
