5. Satisfaction with videoconferencing support for levodopa-carbidopa intestinal gel: An observational study

Abstract

Background Levodopa-carbidopa intestinal gel (LCIG) is a continuously delivered Parkinson's disease therapy intended to stabilize plasma levodopa levels. Patients receiving LCIG require education and follow-up. Some LCIG support programs use video-assisted telenursing. Objective To examine how videoconferencing impacts satisfaction with LCIG support programs. Methods FACILITATE CARE ( F easibility of video- A ssisted C are for I ntestinal L evodopa I nfusion with T elenursing – observ A tional T rial E valuating patient and C aregiver A cceptance in RE al life) was a 12-week, prospective, open-label, 2-arm, parallel-group, observational study assessing satisfaction with LCIG support in patients who self-assigned to video or audio-only arms. Patients aged 18–85 years had completed LCIG titration and owned a videoconferencing device (video arm only). A visual analog scale measured satisfaction (1–10, 10 being most satisfied). Results Patients’ mean (standard deviation) ages were 67.9 (7.4, n = 26) and 71.1 (6.2, n = 15) years in the video and audio arms, respectively. Patients, caregivers, and physicians in both groups reported satisfaction scores of 8–10 with LCIG support personnel, communication access, and assistance with becoming independent. At week 12, the Modified Caregiver Strain Index least square means change from baseline was lower in the video vs. audio arm (−2.3 [1.0] vs. 1.6 [1.2]). LCIG support personnel travel time was lower in the video vs. audio arm (125.7 [70.2] vs. 203.0 [70.0] minutes). Conclusions LCIG support programs are associated with high patient, caregiver, and physician satisfaction; video and audioconferencing satisfaction are similarly high. Video-assisted telenursing may be a convenient communication avenue and may reduce caregiver burden. Registration ClinicalTrials.gov; NCT04500106.

Abstract

A new recursive construction of $N$-ary error-correcting output code (ECOC) matrices for ensemble classification methods is presented, generalizing the classic doubling construction for binary Hadamard matrices. Given any prime integer $N$, this deterministic construction generates base-$N$ symmetric square matrices $M$ of prime-power dimension having optimal minimum Hamming distance between any two of its rows and columns. Experimental results for six datasets demonstrate that using these deterministic coding matrices for $N$-ary ECOC classification yields comparable and in many cases higher accuracy compared to using randomly generated coding matrices. This is particular true when $N$ is adaptively chosen so that the dimension of $M$ matches closely with the number of classes in a dataset, which reduces the loss in minimum Hamming distance when $M$ is truncated to fit the dataset. This is verified through a distance formula for $M$ which shows that these adaptive matrices have significantly higher minimum Hamming distance in comparison to randomly generated ones., Comment: 20 pages, 75 figures

Abstract

New bounds on classification error rates for the error-correcting output code (ECOC) approach in machine learning are presented. These bounds have exponential decay complexity with respect to codeword length and theoretically validate the effectiveness of the ECOC approach. Bounds are derived for two different models: the first under the assumption that all base classifiers are independent and the second under the assumption that all base classifiers are mutually correlated up to first-order. Moreover, we perform ECOC classification on six datasets and compare their error rates with our bounds to experimentally validate our work and show the effect of correlation on classification accuracy., Comment: 14 pages, 11 figures

19. Exploring ND-011992, a quinazoline-type inhibitor targeting quinone reductases and quinol oxidases

Abstract

Abstract Bacterial energy metabolism has become a promising target for next-generation tuberculosis chemotherapy. One strategy to hamper ATP production is to inhibit the respiratory oxidases. The respiratory chain of Mycobacterium tuberculosis comprises a cytochrome bcc:aa 3 and a cytochrome bd ubiquinol oxidase that require a combined approach to block their activity. A quinazoline-type compound called ND-011992 has previously been reported to ineffectively inhibit bd oxidases, but to act bactericidal in combination with inhibitors of cytochrome bcc:aa 3 oxidase. Due to the structural similarity of ND-011992 to quinazoline-type inhibitors of respiratory complex I, we suspected that this compound is also capable of blocking other respiratory chain complexes. Here, we synthesized ND-011992 and a bromine derivative to study their effect on the respiratory chain complexes of Escherichia coli. And indeed, ND-011992 was found to inhibit respiratory complex I and bo 3 oxidase in addition to bd-I and bd-II oxidases. The IC50 values are all in the low micromolar range, with inhibition of complex I providing the lowest value with an IC50 of 0.12 µM. Thus, ND-011992 acts on both, quinone reductases and quinol oxidases and could be very well suited to regulate the activity of the entire respiratory chain.

Abstract

BackgroundThis study aimed to assess if there are sex differences in the functional outcome of intravenous thrombolysis (IVT) among patients with lacunar stroke (LS).MethodsConsecutive patients admitted from 1 January 2014 to 31 January 2020 to hospitals participating in the Swiss Stroke Registry presenting with LS and treated with IVT were included. The study population was then divided into two groups based on patient sex, and a multivariable ordinal logistic regression analysis was performed to uncover sex differences in the modified Rankin Scale (mRS) score at 90 days after stroke.ResultsA total of 413 patients with LS were treated with IVT: 177 (42.9%) women and 236 (57.1%) men. Women were older than men (median age 74 years, 25th–75th percentiles 67–84 years versus 70 years, 25th–75th percentiles 60–80 years, value of p 0.001) and, after adjustment for meaningful variables, showed more frequently increased odds of a higher mRS score at 90 days after stroke (adjusted odds ratio 1.49, 95% confidence interval 1.01–2.19, value of p 0.044).ConclusionThis study showed that female sex increased the odds of a worse functional response to IVT in patients with LS. Future studies should further elucidate the mechanisms underlying such sex differences.

Abstract

We report on the commissioning results of the cold neutron multiplexing secondary spectrometer CAMEA (\textbf{C}ontinuous \textbf{A}ngle \textbf{M}ulti-\textbf{E}nergy \textbf{A}nalysis) at the Swiss Spallation Neutron Source (SINQ) at the Paul Scherrer Institut, Switzerland. CAMEA is optimized for an efficient data acquisition of scattered neutrons in the horizontal scattering plane, allowing for detailed and rapid mapping of low-energy excitations under extreme sample environment conditions.

Methods This multinational cohort study was conducted based on prospectively collected data from the EVA-TRISP (EndoVAscular treatment and ThRombolysis for Ischemic Stroke Patients) collaboration. Consecutive patients (2015–2019) with AIS-LVO attributable to CeAD treated with EVT and/or IVT were included. Primary outcome measures were (1) favorable 3-month outcome (modified Rankin Scale score 0–2) and (2) complete recanalization (thrombolysis in cerebral infarction scale 2b/3). Odds ratios with 95% confidence intervals (OR [95% CI]) from logistic regression models were calculated (unadjusted, adjusted). Secondary analyses were performed in the patients with LVO in the anterior circulation (LVOant) including propensity score matching. Results Among 290 patients, 222 (76.6%) had EVT and 68 (23.4%) IVT alone. Abstract

Most plant conservation strategies generally overlook the intra-specific genetic diversity of crop gene pools. Focusing on forage crops and their wild relatives, we present a novel approach to address the conservation of these species on meadows. Two-thirds of Swiss agricultural land is green land, mostly used for forage purposes, and their genetic diversity is being threatened. We focused here on eight plant associations gathering at least 18 taxa considered priority crop wild relatives of forage crops. Since 2020, about 1,217 high-quality surfaces (representing 1,566 hectares) nationwide have been integrated into an innovative auction-based policy instrument dedicated to conserving these populations. Here, we report the benefits and hurdles of implementing this bottom-up approach and try to estimate the quality of conservation of the forage plants’ CWR gene pool. Although we focus on the Swiss case, our approach to in situ conservation offers opportunities to effectively guide conservation in other contexts. We also discuss possible ways to improve CWR conservation policy, particularly the need to better consider the populations and habitat levels.

Abstract

Background The RACECAT (Transfer to the Closest Local Stroke Center vs Direct Transfer to Endovascular Stroke Center of Acute Stroke Patients With Suspected Large Vessel Occlusion in the Catalan Territory) trial was the first randomized trial addressing the prehospital triage of acute stroke patients based on the distribution of thrombolysis centers and intervention centers in Catalonia, Spain. The study compared the drip‐and‐ship with the mothership paradigm in regions where a local thrombolysis center can be reached faster than the nearest intervention center (equipoise region). The present study aims to determine the population‐based applicability of the results of the RACECAT study to 4 stroke networks with a different degree of clustering of the intervention centers (clustered, dispersed). Methods and Results Stroke networks were compared with regard to transport time saved for thrombolysis (under the drip‐and‐ship approach) and transport time saved for endovascular therapy (under the mothership approach). Population‐based transport times were modeled with a local instance of an openrouteservice server using open data from OpenStreetMap.The fraction of the population in the equipoise region differed substantially between clustered networks (Catalonia, 63.4%; France North, 87.7%) and dispersed networks (Southwest Bavaria, 40.1%; Switzerland, 40.0%). Transport time savings for thrombolysis under the drip‐and‐ship approach were more marked in clustered networks (Catalonia, 29 minutes; France North, 27 minutes) than in dispersed networks (Southwest Bavaria and Switzerland, both 18 minutes). Conclusions Infrastructure differences between stroke networks may hamper the applicability of the results of the RACECAT study to other stroke networks with a different distribution of intervention centers. Stroke networks should assess the population densities and hospital type/distribution in the temporal domain before applying prehospital triage algorithms to their specific setting.
