Zonguldak Bülent Ecevit Üniversitesi Merkez Farabi Kampüsü Sezai Karakoç Kültür Merkezi’nde, 12 Kasım 2022 tarihinde “Travma anestezisinde Hasta Güvenliği, Zonguldak’ta Sonbahar Sempozyumu-III “(https://www.zbeu-travmaanestezisi.org) gerçekleştirildi. Sempozyum oturumlarındaki sözlü bildirilerin özetlerinin sunulması amaçlanmıştır. Kongre süresince toplam 15 sözel bildiri sunulmuştur. Böylece, travma anestezisinde hasta güvenliği ve travma hastasının yönetiminde güncel yaklaşımların bilim dünyası ile paylaşılması sağlanmıştır., At the Zonguldak Bulent Ecevit University, Central Farabi Campus Sezai Karakoc Cultural Center, 12 November 2022, Patient Safety In Trauma Anesthesia, Autumn Symposium In Zonguldak-III (https://zeg.beun.edu.tr/)” was held. It is aimed to present abstracts of the oral presentations in the symposium sessions. During the symposium, 15 presentations were presented. Thus, Patient safety in trauma anesthesia and current approaches in the management of trauma patients were shared with the scientific world.

Objectives: This study aims to investigate the effect of McCoy, Macintosh laryngoscope, and C-MAC video-laryngoscopes on optic nerve sheath diameter (ONSD) and hemodynamic responses to laryngoscopy and intubation. Methods: This prospective randomized study was conducted in Zonguldak Bülent Ecevit University Hospital, Zonguldak, Turkey, between July 2019 and January 2020. Informed written consent was obtained from all patients. Patients with previous intracranial/ocular surgery or glaucoma were excluded from the study. The patients were randomized to use McCoy, Macintosh, and C-MAC (30 per group). Intubations were performed by the same person. Mean arterial pressure, heart rate (HR), and ONSD were recorded before the induction and repeated in 1, 3, 5, and 10 minutes after the intubation. Results: The effects of laryngoscopy and intubation on hemodynamic responses and ONSD were similar between groups (p>0.05). While the comparison within groups showed ONSD increase in McCoy group and HR and ONSD increase in the Macintosh group compared to baseline 1 min after the intubation, no change was observed in hemodynamic responses and ONSD measurements in the C-MAC® group (p>0.05). Conclusions: In this study, there was no significant difference between the groups in terms of ONSD and hemodynamic responses to laryngoscopy and intubation. It was observed that there were no significant changes in ONSD values just in C-MAC® video-laryngoscope group. Therefore, intubations with C-MAC® video-laryngoscope are thought to be more appropriate for patients with an increase in intracranial pressure.

Apert syndrome is an autosomal dominant inherited mandibulofacial dysostosis characterized by craniosynostosis, syndactyly, high forehead, broad nose, maxillary hypoplasia, synostosis of cervical vertebrae, organ malformations, and mental retardation. It is rarely encountered and as there is little knowledge of the anesthesia practice for this syndrome in the literature, we present our anesthesia experience of a case undergoing bilateral syndactyly surgery., Apert sendromu, kraniyosinostoz, sindaktili, yüksek alın, düz kemer burun, maksiller hipoplazi, servikal vertebralarda sinostoz, organ malformasyonları, konjenital kalp hastalıkları ve mental retardasyon ile karakterize ve sıklıkla otozomal dominant geçiş gösteren bir mandibulofasiyal dizostozdur. Nadir karşılaşılması ve literatürde bu sendroma ait anestezi pratiğinin az olması nedeniyle bilateral sindaktili ameliyatı yapılan olgudaki anestezi deneyimimiz sunulmuştur.

Amaç: Elektrokonvülzif tedavi (EKT), etkin ve yaşam kurtarıcı bir tedavi yöntemi olmakla beraber, etkisi ve yan etkileri hala tartışılmaktadır. Çalışmamızda EKT’nin apne döneminde oksijen (O2) uygulamasının Near Infrared Spektroskopi ile serebral oksijenizayon üzerine ve EKT sonrası komplikasyonlar üzerine etkisini değerlendirmeyi amaçladık. Yöntem: Bu ileriye dönük, gözlemsel çalışmaya ilk kez EKT planlanan 40 hasta katılmıştır. Tüm hasta-lara serebral oksimetre ve bispektral indeks sensörleri (BİS) uygulandı. Hastalar nazal kanül ile 6 L/dk O2 uygula-ması yapılan hastalar Grup O (s=20), O2 uygulanmayan hastalar Grup K (s=20) olarak rastgele ikiye ayrıldı. Nöbet sırasında ve nöbetten 10 dakika sonra hemodinamik veriler, end-tidal karbondioksit (EtCO2), bölgesel serebral doku oksijen satürasyonu (rSO2 ) ve BİS değerleri ile EKT sonrası komplikasyonlar kaydedildi. Sonuçlar: Nöbet sırasında ölçülen rSO2’deki değişim ve demografik özellikler açısından gruplar arasında anlamlı bir fark saptanmadı (p>0.05). EKT sırasında desatürasyon (SpO2, Objective: Electroconvulsive therapy (ECT) is an effective and life-saving treatment modality, while its effects and side effects are still being discussed. In our study, we aimed to demonstrate the effect of O2 administration on the cerebral oxygenation measured by Near Infrared Spectroscopy and the complications after ECT in the apnea period of ECT. Methods: This prospective, observational study was carried out with 40 patients whom ECT had been planned and who were undergoing ECT for the first time. Cerebral oximeter sensors and Bispectral index (BIS) monitoring were applied in all patients as well as routine monitoring. The participants were randomly divided into two groups. The patients who received administration of 6 L/min O2 through nasal cannula were included in Group O (n=20), and the patients who were not administered O2 were included in Group C (n=20). Hemodynamic data after induction, during the seizures and at the 10th min after the seizures, EtCO2, regional cerebral tissue oxygen saturation (rSO2), BIS values and complications after ECT were recorded in both groups. The incidence of dental trauma reported during general anesthesia is 0.06-12.3%. The majority of perioperative dental trauma occurs during laryngoscopy and intubation, it may be observed when excessive force is applied to remove the airway, endotracheal tube or laryngeal mask. Dental injury may vary from simple fracture to restoration loss or avulsion (removal of tooth from the socket). Dental injury generally occurs in the upper front region and the left central incisor is most frequently affected due to the position of the laryngoscope. We aim to share an approach to dental trauma that may be encountered during general anesthesia administration in this case report., Genel anestezi sırasında bildirilen diş travması insidansı % 0.06-12.3’dir. Perioperatif dental travmaların çoğu laringoskopi ile entübasyon sırasında ortaya çıkarken, airway, endotrakeal tüp ya da larengeal maske çıkarılması için aşırı kuvvet uygulandığında da görülmektedir. Dental yaralanmalar basit kırıktan restorasyon kaybı veya avulsiyona (dişin soketinden çıkması) kadar değişebilir. Dental yaralanma genellikle üst ön bölgede meydana gelir ve laringoskopun konumu nedeniyle sol orta kesici diş en sık etkilenir. Genel anestezi uygulamalarında karşılaşılabilecek dental travmalara yaklaşımımızı olgu üzerinden aktarmayı amaçladık.

Objective: This randomized, prospective study was designed to investigate the effect of sevoflurane-nitrous oxide application on the incidence of the hand/arm withdrawal movement caused by rocuronium injection. Materials and Methods: There were 90 individuals undergoing elective surgery included in the study. After preoxygenation, Group T was given intravenous (i.v.) 5 mg/kg thiopental, Group S was given 7% sevoflurane and 40%/60% air/O2, and Group N was given 7% sevoflurane and 40%/60% N2O/O2 for induction. After the eyelash reflex was lost, 0.6 mg/kg rocuronium was applied intravenously over 5 seconds, and then 2 ml saline was administered. Patients' response to rocuronium injection was graded by using a 4-point scale (0-3). Hemodynamic data were recorded. Suda boğulmaya bağlı arrestten sonra erken başlatılan etkili KPR ile uygun yoğun bakım tedavisinin, nörolojik hasar oluşmadan sağkalımı sağladığını düşünmekteyiz.

Jarcho Levin sendromu (JLS) kostavertabral anomalilerin ve bunlara hidrosefali, nöral tüp defekti, kardiyak, renal ve gastrointestinal problemlerin eşlik ettiği nadir görülen bir hastalıktır. Solunum sistemi patolojileri nedeniyle erken yaşta mortalite oranı yüksektir. Nadir karşılaşılması ve literatürde JLS'ye dair anestezi pratiğinin az olması nedeniyle inguinal herni ameliyatı yapılan hastadaki anestezi deneyimimiz sunulmuştur., Jarcho-Levin syndrome (JLS) is a rare disease involving costovertebral anomalies and is accompanied by hydrocephalus, neural tube defect, and cardiac, renal and gastrointestinal problems. Due to respiratory system pathologies, there is a high mortality rate at young ages. Due to its rarity and little information in the literature related to anesthetic practice for this syndrome, we present our anesthetic experience of inguinal hernia surgery in a patient with JLS.

Bu araştırmada 2006 ile 2010 yılları arasında Bülent Ecevit Üniversitesi Tıp Fakültesi'nde kalça kırığı nedeniyle opere edilen hastaların analizi yapılarak mortalite ve morbidite üzerine etkili faktörlerin belirlenmesi amaçlandı. Veriler hasta dosyaları ve bilgisayar kayıtlarının incelenmesiyle elde edildi. Hastaların operasyondan bir yıl sonraki sağlık durumları hastalara telefon ile ulaşılarak öğrenildi. Hastaların yaş, cinsiyet, ASA skoru, kırığın yerleşimi, anestezi tekniği, operasyon öncesi hastanede yatış süresi, operasyon süresi, operasyon sonrası hastanede yatış süresi, kan replasmanı gereksinimi, operasyon tekni- ği, istenen konsültasyon sayısı, hemoglobin, albumin, lenfosit sayıları, yandaş hastalıkları, sigara kullanımı, gelişen komplikasyonlar ve hastaneden çıkış durumları incelendi. Hipertansiyon, kronik obstrüktif akciğer hastalığı ve kronik böbrek yetmezliğinin varlığı mortalite ve morbiditeyi belirleyen önemli unsurlar olarak tespit edildi. Bulgularımıza göre ASA değeri mortalite ve morbidite açısından önemli bir belirteçti. ASA 3 ve ASA 4 gruplarında bir yıl içindeki mortalite oranının yüksek olduğu bulundu. Hastalara uygulanan genel anestezi ve rejyonel anestezi teknikleri karşılaştırıldığında morbidite ve mortalite oranları açısından anlamlı fark olmadığı bulundu. Hastalar uygulanan operasyon tekniğine göre karşılaştırıldığında protez uygulamasında intrameduller çivileme uygulamasına göre mortalite oranı yüksek bulundu. Kan parametrelerinden hemoglobin ve albü- min seviyelerinin mortalite ve morbidite üzerine etkisi bulunmazken; preoperatif lenfosit düşüklüğünün bir yıl içindeki mortalite ile iliş- kili olduğu bulundu. Sonuç olarak, çalışmamızda kalça kırığı nedeniyle opere edilen hastaların mortalite ve morbiditesi üzerine seçilen anestezi yönteminin direkt olarak etkisi olmadığını, mortalite ve morbidite üzerine yandaş hastalıkların, ileri yaşın, yüksek ASA değerinin ve operasyon tekniğinin etkili olduğu sonucuna vardık, In this study data of the patients with hip fracture diagnosis operated at Bülent Ecevit University, Department of Orthopedics and Traumatology between 2006-2010 were analyzed to determine the factors affecting mortality and morbidity. Data of the patients were obtained from patients’ charts and computer records. Health condition of the patients were questioned one year after the surgery by phone calls. We analysed patients’ age, sex, ASA score, location of the fracture, anesthesia technique, hospitalization time before operation, operation period, hospitalization time after operation, situation of blood replacement, surgical technique, number of consultations, levels of hemoglobine, albumine, lymphocyte count, adherent disease, smoking status and complications before discharge from hospital. Presence of hypertension, chronic obstructive lung disease, and chronic renal failure were found to be important factors affecting mortality and morbidity. High ASA score was determined to be an important factor indicating the risk of mortality and morbidity. One year mortality rates were high in ASA 3 and ASA 4 groups. There was no significant difference between general and regional anesthesia techniques in the sense of mortality and morbidity rates. When the types of the operation were compared, mortality rates were higher in prothesis group than those of intramedullary nailing. Although blood parameters like hemoglobine and albumine showed no effect on mortality and morbidity, preoperatively lower lymphocyte levels were associated with mortality within one year. In conclusion, we found out that anesthesia technique did not directly affect the mortality and morbidity in patients operated for hip fracture in our study, but comorbidities, higher age, higher ASA scores and technique of operation were found to be effective factors on mortality
