Apexification aims to induce apical closure of the open root apex with a hard-tissue barrier, against which a root filling can be compacted. Mineral trioxide aggregate (MTA) is a potential apical barrier material with good sealability and a high degree of biocompatibility. The main technical problem associated with placement is to prevent overfill and underfill. This paper demonstrates retreatment of an open apex tooth by placement of an apical barrier using MTA and PRF membrane as an internal matrix followed by application of anatomical post and core. This new technique is particularly advantageous in teeth which have open apex, root canals that are not round, wide canals and thin radicular dentin. Context: Microbial control is of paramount importance in Clinical Endodontics. Therefore, cleaning and disinfection of root canals are essential to achieve endodontic success. Aims: The purpose of this study is to compare the efficacy of EndoVac irrigation system and conventional needle (30 gauges side venting needle) irrigation for removal of debris from the root canal walls at coronal, middle and apical third by using the scanning electron microscopy (SEM). Settings and Design: An in vitro randomized control trial study. Materials and Methods: A total of 20 mandibular premolars with completely formed roots were selected and randomly divided into two groups - Group 1: Irrigation with the Conventional system and Group 2: EndoVac irrigation. After access opening and working length determination biomechanical preparation completed up to a rotary protaper F4 file. Groupwise irrigation with sodium hypochlorite and ethylenediaminetetraacetic acid was done with each canal in between instrumentation. Then, the teeth were sectioned in buccolingual direction and the halves were sputter-coated with gold palladium and coronal, middle and apical third were examined by SEM at x2000 magnification. Statistical Analysis: Mann-Whitney test for comparison between methods, Kruskal-Wallis test for comparison among thirds and Miller test for individual comparisons. Results: The apical, middle and cervical root canal thirds were evaluated and the results were analyzed statistically by the Mann-Whitney test for comparison between methods, Kruskal-Wallis test for comparison among thirds and Miller test for individual comparisons. Conclusions: EndoVac group resulted in significantly less debris at apical third compared with the conventional needle irrigation group. There was no statistical significant difference found in debris removal at coronal and middle third of root canal wall between the EndoVac group and conventional needle irrigation group.

Thorough knowledge of root canal morphology is essential for the success of endodontic treatment. There are rare aberrant, variations in canal number and configuration in maxillary and mandibular molars, which could affect treatment outcome. This paper presents the endodontic management of a maxillary first molar with two palatal canals and three buccal canals and mandibular first molar with two mesial and three distal canals. In this paper this morphology was confirmed using cone-beam computed tomography (CBCT) and surgical operating microscope . CBCT can provide dentists with high quality 3-dimensionals images of dental structure due to it's high spatial resolution. Most periapical radiolucent lesions associated with infections of the root canal system heal uneventfully after endodontic treatment. However, some cases may require periradicular surgery in order to remove pathologic tissue from the periapical region and simultaneously eliminate any source of infection that could not be removed by orthograde root canal treatment. With an adequate technique, surgery can address these issues, although it may be insufficient in some situations. This report describes the healing process after surgery in two cases with a 12 months follow-up. In these cases, apicoectomy was followed by retrograde sealing with Super EBA (Harry J. Bosworth Company, Illinois, USA). The bone defect was filled with PerioGlas (NovaBone, Austin, TX, USA) and covered with a resorbable Guidor membrane (Sunstar, Foster Ave, Chicago, USA). No intraoperative or postoperative complications were observed. After 24 months of follow-up, the patient showed no clinical signs or symptoms associated with the lesion and radiographic examination showed progressive resolution of radiolucency. Background and objectives This in vitro study evaluated and compared the abrasion resistance of types IV and V gypsums, resin-modified type IV and type V gypsums, synthetic gypsum, and epoxy resin die material at two different time intervals spanning a period of 24 hours. Materials and methods A total of 10 dies of each die material were fabricated and tested for abrasion resistance at each time interval. An average of five readings was taken for the test, measuring the rate of abrasion of the materials. Results All the tested properties of the epoxy resin material were far superior to those of the other materials (being tested). Resin-modified type IV gypsum had significantly better abrasion resistance than its type V counterpart at setting. However, when tested at 24 hours, both materials had comparable resistance to abrasion, which was the highest among the gypsum-based materials. Interpretation and conclusion Epoxy die resin had the most superior physical properties of all. A significant decrease in the rate of abrasion (-38 to -83%) was noted, when the gypsumbased products were tested at 24 hours. From the data obtained in this study, it is recommended that when gypsum-based materials are used, to wait at least 24 hours prior to separating casts from impressions (and subsequent laboratory procedures) to avoid damage to the casts and dies.
