Background. The study of the functional connectivity by means of magnetic resonance imaging (MRI) in asymptomatic LRRK2 mutation carriers could contribute to the characterization of the prediagnostic phase of LRRK2 associated Parkinson's disease (PD). Objective. To characterize MRI functional patterns during resting state in asymptomatic LRRK2 mutation carriers. Methods. We acquired structural and functional MRI data of 18 asymptomatic LRRK2 mutation carriers and 18 asymptomatic LRRK2 mutation noncarriers, all first-degree relatives of LRRK2-PD patients. Starting from resting state data, we analyzed the functional connectivity of the striatocortical and the nigrocortical circuitry. Structural brain data was analyzed by voxel based morphometry, cortical thickness and volumetric measures. Results: Asymptomatic LRRK2 mutation carriers had functional connectivity reductions between the caudal motor part of the left striatum and ipsilateral precuneus and superior parietal lobe. Connectivity in these regions correlated with subcortical gray matter volumes in mutation carriers. Asymptomatic carriers also showed increased connectivity between the right substantia nigra and bilateral occipital cortical regions (occipital pole and cuneus bilaterally, and right lateral occipital cortex). No intergroup differences in structural MRI measures were found. In LRRK2 mutation carriers, age and functional connectivity correlated negatively with striatal volumes. Additional analyses including only subjects with the G2019S mutation revealed similar findings. Conclusion: Asymptomatic LRRK2 mutation carriers showed functional connectivity changes in striatocortical and nigrocortical circuits compared with noncarriers. These findings support the concept that altered brain connectivity precedes the onset of classical motor features in a genetic form of PD.

Previous functional MRI studies have revealed changes in the default-mode network (DMN) in Parkinson's disease (PD). The purpose of this work was to evaluate changes in the connectivity patterns of a set of cognitively relevant, dynamically interrelated brain networks in association with cognitive deficits in PD using resting-state functional MRI. Sixty-five non-demented PD patients and 36 matched healthy controls (HC) were included. Thirty-four percent of PD patients were classified as having mild cognitive impairment (MCI) based on performance in the three mainly-affected cognitive domains in Parkinson's disease (attention/executive, visuospatial/visuoperceptual and declarative memory). Data-driven analyses through independent-component analysis (ICA) was used to identify the DMN, the dorsal attention network (DAN) and the bilateral frontoparietal networks (FPN), which were compared between groups using a dual-regression approach. Additional seed-based analyses using a-priori defined regions of interest were used to characterize local changes in intra and inter-network connectivity. ICA results revealed reduced connectivity between the DAN and right frontoinsular cortical regions in MCI patients, which correlated with worse performance in attention/executive functions. The DMN, on the other hand, displayed increased connectivity with medial and lateral occipito-parietal regions in MCI patients; these increases correlated with worse visuospatial/visuoperceptual performance. In line with data-driven results, seed-based analyses mainly revealed reduced within-DAN, within-DMN and DAN-FPN connectivity, as well as increased DAN-DMN coupling in MCI patients. Our findings demonstrate differential connectivity changes affecting the networks evaluated, which we hypothesize to be related to the pathophysiological bases of different types of cognitive impairment in PD.

Background: One of the most common neuropsychiatric symptoms in PD is apathy, affecting between 23 and 70% of patients and thought to be related to frontostriatal dopamine deficits. In the present study, we assessed functional resting-state frontostriatal connectivity and structural changes associated with the presence of apathy in a large sample of PD subjects and healthy controls, while controlling for the presence of comorbid depression and cognitive decline. Methods: Thirty-one healthy controls (HC) and 62 age, sex and education-matched PD patients underwent resting-state functional MRI. Apathy symptoms were evaluated with the Apathy Scale (AS). The 11 Beck Depression Inventory-II items that measure dysphoric mood symptoms as well as relevant neuropsychological scores were used as nuisance factors in connectivity analyses. Voxel-wise analyses of functional connectivity between frontal lobes (limbic, executive, rostral motor and caudal motor regions), striata (limbic, executive, sensorimotor regions) and thalami were performed. Subcortical volumetry/shape analysis and fronto-subcortical voxel-based morphometry were performed to assess structural changes. Results: Twenty-five PD patients were classified as apathetic (PD-A) (AS>13). PD-A patients showed functional connectivity reductions compared with HC and with non-apathetic patients (PD-NA), mainly in left-sided circuits, and predominantly involving limbic striatal and frontal territories. Similarly, severity of apathy negatively correlated with connectivity in these circuits. No significant effects were found in structural analyses. Conclusions: Our results indicate that the presence of apathy in PD is associated with functional connectivity reductions in frontostriatal circuits, predominating in the left hemisphere and mainly involving its limbic components.

Graph‐theoretical analyses of functional networks obtained with resting‐state functional magnetic resonance imaging (fMRI) have recently proven to be a useful approach for the study of the substrates underlying cognitive deficits in different diseases. We used this technique to investigate whether cognitive deficits in Parkinson's disease (PD) are associated with changes in global and local network measures. Thirty‐six healthy controls (HC) and 66 PD patients matched for age, sex, and education were classified as having mild cognitive impairment (MCI) or not based on performance in the three mainly affected cognitive domains in PD: attention/executive, visuospatial/visuoperceptual (VS/VP), and declarative memory. Resting‐state fMRI and graph theory analyses were used to evaluate network measures. We have found that patients with MCI had connectivity reductions predominantly affecting long‐range connections as well as increased local interconnectedness manifested as higher measures of clustering, small‐worldness, and modularity. The latter measures also tended to correlate negatively with cognitive performance in VS/VP and memory functions. Hub structure was also reorganized: normal hubs displayed reduced centrality and degree in MCI PD patients. Our study indicates that the topological properties of brain networks are changed in PD patients with cognitive deficits. Our findings provide novel data regarding the functional substrate of cognitive impairment in PD, which may prove to have value as a prognostic marker. In a previous functional MRI (fMRI) study, we found that Parkinson's disease (PD) patients presented dysfunctions in the recruitment of recognition memory networks. We aimed to investigate the changes in these networks over time. We studied 17 PD patients and 13 age and sex-matched healthy subjects. In both groups fMRI (recognition memory paradigm) and neuropsychological assessments were obtained at baseline and follow-up. In order to analyze changes over time in functional networks, modelfree (independent component analysis) analyses of the fMRI data were carried out. After that, a cross-correlation approach was used to assess the changes in the strength of functional connectivity. At follow-up, patients showed reduced recruitment of one network, including decreased activations in orbitofrontal cortices, middle frontal gyri, frontal poles, anterior paracingulate cortex, superior parietal lobes and left middle temporal gyrus, as well as decreased deactivation in anterior paracingulate gyrus and precuneus. Cross-correlation analyses over time showed a decrease in the strength of functional connectivity between middle frontal gyrus and superior parietal lobe in PD patients. Model-free fMRI and cross-correlation connectivity analyses were able to detect progressive changes in functional networks involved in recognition memory in PD patients at early disease stages and without overt clinical deterioration. Functional connectivity analyses could be useful to monitor changes in brain networks underlying neuropsychological deficits in PD.

Parkinson's disease (PD) patients differed from controls of similar age in visuospatial and visuoperceptual functions at diagnosis moment, and these deficits have been shown to be neuropsychological markers of evolution to dementia. The aim of this study was to relate these dysfunctions with measures of brain. The sample of this study consisted of 92 PD patients and 36 healthy subjects matched by age, sex and education. All subjects were evaluated with Judgment of Line Orientation, Visual Form Discrimination and Facial Recognition Tests and magnetic resonance imaging at 3 Tesla. We found significant differences between patients and controls in all three tests and in the mean of cortical thickness, gray matter volume and ventricular system. All visuospatial and visuoperceptual tests correlated with the measures of global atrophy suggesting that they are reflecting the brain degeneration associated to PD.

We provide experimental support for the notion that genetic factors play a critical role in such adaptation. We induced a controlled brain disruption using repetitive transcranial magnetic stimulation (rTMS) and show that APOE status determines its impact on distributed brain networks as assessed by functional MRI (fMRI).Twenty non-demented elders exhibiting mild memory dysfunction underwent two fMRI studies during face-name encoding tasks (before and after rTMS). Baseline task performance was associated with activation of a network of brain regions in prefrontal, parietal, medial temporal and visual associative areas. APOE ε4 bearers exhibited this pattern in two separate independent components, whereas ε4-non carriers presented a single partially overlapping network. Following rTMS all subjects showed slight ameliorations in memory performance, regardless of APOE status. However, after rTMS APOE ε4-carriers showed significant changes in brain network activation, expressing strikingly similar spatial configuration as the one observed in the non-carrier group prior to stimulation. Similarly, activity in areas of the default-mode network (DMN) was found in a single component among the ε4-non bearers, whereas among carriers it appeared disaggregated in three distinct spatiotemporal components that changed to an integrated single component after rTMS. Our findings demonstrate that genetic background play a fundamental role in the brain responses to focal insults, conditioning expression of distinct brain networks to sustain similar cognitive performance.

The objective of this study is to contribute to the establishment of a memory profile in subjects with attention deficit disorder with hyperactivity (ADHD). We examined fifty-one adolescents: 16 ADHD and 35 controls, aged between 14 and 16. Neuropsychological exploration included tests of short-term memory and of declarative and implicit (incidental and procedural) memory. Subjects with ADHD presented deficits in their mnesic abilities. These deficits do not involve all type of memory, but are selective. Declarative memory and, especially, short-term memory show higher deficits than implicit or non-declarative memory: incidental learning is preserved and procedural learning is similar to that obtained by control subjects, although this forn of learning presents more diflculties for ADHD subjects learning with a higher difficulty.

El déficit neuropsicológico asociado al alcholismo abarca diversas funciones: memoria, razonamiento abstracto, solución de problemas y ha- bilidades perceptivo-motoras. Existe además una interacción entre la edad y las al- teraciones citadas. Añadir por ultimo que tanto los signos de atrofia en la TC, como la reducción de flujo sanguineo y los trastornos neuropsico- lógicos son parcialmente reversibles con la abstinencia. [eng] INTRODUCTION: Parkinson’s disease (PD) and Dementia with Lewy bodies (DLB) appear as neurodegenerative disorders with a wide range of symptomatology that differs among patients, among which are different levels of cognitive impairment. For instance, mild cognitive impairment (MCI) in PD contributes to a specific clinical profile with a higher risk of developing dementia. Looking at PD and DLB together provides evidence of the existence of different subtypes within both diseases. In recent years, complex imaging techniques, such as magnetic resonance imaging (MRI), have been used to study pathologies of the brain. MRI measures can be used to better characterize the brain basis of PD and DLB symptomatology, such as MCI. The reconstruction of the whole-brain connectome is a complex approach that can help to describe the heterogeneous symptomatology in neurodegenerative disorders. In addition, MRI in combination with new data-driven methods, such as cluster analysis, has been used to group patients according to their similarities, which allows subtypes to be identified. Until now, most studies in PD have described subtypes based on clinical and neuropsychological data, and just a few have used MRI measures to identify subtypes with different brain patterns. As DLB research is still in a relatively early stage, no cluster analyses have been yet performed based on MRI data. OBJECTIVES AND HYPOTHESES: Given this context, the current Doctoral Thesis focuses on the heterogeneity that characterizes PD and DLB. The main objectives were to identify subtypes based on structural MRI measures in PD and DLB, as well as to characterize the structural brain connectivity of PD associated with MCI. We hypothesized that there would be PD subtypes with different patterns of grey and white matter alterations that would be associated with particular clinical and cognitive profiles. We also hypothesized that there would be DLB subtypes characterized by different grey matter (GM) patterns, and that these patterns would explain specific symptomatology of the disease and would be differentially associated to concomitant brain changes seen in cerebrovascular and Alzheimer’s diseases. Finally, we expected that PD-MCI would present a characteristic pattern of impaired structural connectivity. In order to examine and clarify these issues, the current Doctoral Thesis is presented as a compendium of three studies. METHODS: In Study 1, to identify subtypes in PD, we performed a hierarchical cluster analysis based on multimodal imaging using the Ward’s linkage method. We performed the analysis in a sample of 62 PD patients. GM volumes of cortical and subcortical brain regions as well as fractional anisotropy (FA) white matter (WM) measures were combined. Once the subtypes were identified, voxel-based morphometry (VBM) and tract-based spatial statistics (TBSS) analyses were carried out in order to compare the pattern of GM and WM of the PD subtypes to the 33 healthy control group. Demographical, clinical and neuropsychological data were used to characterize the subtypes. In Study 2, the sample consisted of 27 PD-MCI and 35 PD without MCI, as well as 51 healthy controls. In this study we applied threshold-free network-based statistics (TFNBS), a novel technique based on whole-brain probabilistic tractography data useful to study structural connectivity. We complemented the analysis with TBSS and graph theory analyses (global and local measures). Study 3 included 165 DLB subjects from the Mayo Clinic and 3 centres from the European DLB consortium (E-DLB). We performed a cluster analysis based on GM volumes using a random forest method, and characterized the subtypes based on GM volumes, clinical, demographical data as well as tau, β-amyloid and cerebrovascular biomarkers. Additionally, we characterized cognitive trajectories of the subtypes in a 3-year follow- up. RESULTS: In Study 1, we identified 3 PD subtypes which mainly differed in GM patterns, while WM involvement appeared to be more limited. PD1 (24%) was characterized by temporo-parieto-occipital GM atrophy and subcortical atrophy, as well as FA reductions mainly affecting fronto-occipital WM tracts. This subtype was the oldest and had the worse neuropsychological profile. The second subtype, PD2 (34%), was characterized by GM atrophy limited to frontal and temporal cortical regions, and a third subtype, PD3 (42%), with non-detectable GM atrophy or WM impairment, and preserved cognitive profile. In Study 2, we found that PD patients had fewer streamlines (NOS) compared with healthy controls. Structural connectivity impairments were present in PD with and without MCI. However, the pattern and degree of connectivity impairment were different. PD-MCI showed a higher number of abnormal connections, primarily involving those between deep GM structures and cortical regions and posterior cortico-cortical connections, mainly in the temporal and occipital regions. PD without MCI showed fewer altered connections, and unlike PD-MCI, they were mainly located in the bilateral prefrontal cortex. What is more, the logistic regression and ROC curve analysis showed that the decreased NOS in the impaired connections characteristic of PD-MCI, were able to discriminate between both PD groups with high accuracy. The TBSS analysis revealed that only PD-MCI had reduced FA values compared to controls. The graph theory analysis showed PD groups differed in local graph measures. In Study 3, three DLB subtypes with the same disease evolution were identified based on their GM volumes. The cortical predominant subtype (30%) was characterized by widespread reduced cortical GM, older age, worse cognition at baseline and faster cognitive decline over 3 years. The second subtype, the fronto-occipital subtype (46%), had lower GM volumes in frontal and occipital regions. Finally, the subcortical predominant subtype (24%) was characterized by the greatest GM volumes, and relatively low GM volumes in the basal ganglia, as they were the only brain regions where the 3 subtypes had equivalent GM volumes. The subcortical predominant subtype was also characterized by the highest frequency of cognitive fluctuations. CONCLUSIONS: Our overall findings support the existence of different PD and DLB subtypes that can be identified by means of cluster analyses based on MRI data, which are in turn associated with specific cognitive profiles, and that cognitive impairment in PD is also associated to a specific pattern of structural connectivity impairment. These results contribute to clarifying the basis of heterogeneity in DLB and PD and give further information about which characteristics could be considered biomarkers of worse prognosis, with the final aim of approaching a more personalized medicine., [cat] INTRODUCCIÓ: La malaltia de Parkinson i la Demència amb cossos de Lewy són malalties neurodegeneratives que es presenten amb una àmplia varietat de símptomes, els quals difereixen entre pacients. El deteriorament cognitiu lleu, per exemple, és un possible símptoma de la malaltia de Parkinson que contribueix en un perfil clínic concret amb un elevat risc de desenvolupar demència. Tot plegat aporta evidència de l’existència de diferents subtipus (grups) dins d’ambdues malalties. En els últims anys, s’han utilitzat tècniques d’imatge complexes, com la imatge per ressonància magnètica (IRM), per estudiar les malalties que afecten al cervell. Les diferents mesures obtingudes de la IRM, es poden utilitzar per caracteritzar les bases de la simptomatologia de la malaltia de Parkinson i de la Demència amb cossos de Lewy, com per exemple el deteriorament cognitiu lleu. La reconstrucció del connectoma de tot el cervell és una aproximació complexa que pot ajudar a descriure la simptomatologia heterogènia de les malalties neurodegeneratives, a més, la IRM en combinació amb noves tècniques guiades per les dades, com els anàlisis de clúster, s’han utilitzat per agrupar els pacient d’acord a les seves similituds, el que permet trobar subtipus de pacients. Fins ara, la majoria dels estudis en la malaltia de Parkinson han trobat subtipus basant les anàlisis en dades clíniques i neuropsicològiques, i només escassos treballs s’han basat en dades derivades d’IRM. En la Demència amb cossos de Lewy, degut a que la recerca es troba en fases més primerenques que no pas la recerca en la malaltia Parkinson, encara no s’ha realitzat cap anàlisis de clúster basat en dades d’IRM. OBJECTIUS I HIPÒTESIS: Donat aquest context, la Tesi Doctoral que aquí es presenta es centra en l’heterogeneïtat que caracteritza la malaltia de Parkinson i a la Demència amb cossos de Lewy. Els objectius principals han estat identificar subtipus basats en dades d’IRM en Parkinson i Demència amb cossos de Lewy, així com caracteritzar la connectivitat cerebral a nivell estructural del deteriorament cognitiu lleu en la malaltia de Parkinson. Hem hipotetitzat que hi haurien subtipus en la malaltia de Parkinson que presentarien diferents patrons d’alteració de la substància grisa i la substància blanca que s’associarien amb un perfil clínic i cognitiu específic. En aquesta línia també hem hipotetitzat que hi haurien subtipus dins de la Demència amb cossos de Lewy que es caracteritzarien per diferents patrons d’alteració de la substància grisa, i que aquests patrons, podran explicar diferències en la simptomatologia de la malaltia i es trobaran diferencialment associats a marcadors biològics d’altres processos degeneratius associats a l’envelliment, com els canvis que es troben en processos cerebrovasculars i en la malaltia d’Alzheimer. Finalment, esperaríem que el deteriorament cognitiu lleu en la malaltia de Parkinson presentés un patró característic de connectivitat estructural alterada. Amb la intenció de donar resposta a aquestes qüestions, l’actual Tesi Doctoral es presenta com un compendi de 3 estudis. MÈTODES: En l’estudi 1, per tal d’identificar subtipus en la malaltia de Parkinson, es va dur a terme un anàlisis de clúster jeràrquic aglomeratiu, utilitzant dades multimodals d’IRM. L’anàlisi es va dur a terme en una mostra de 62 pacients i es van combinar volums de substància grisa de regions corticals i subcorticals amb mesures d’anisotropia fraccional de la substància blanca. Un cop identificats els subtipus, es va dur a terme una morfometria basada en vòxels (voxel-based morphometry (VBM), en anglès) i una estadística espacial basada en tractes (Tract-based spatial statistics (TBSS), en anglès) per tal de comparar els patrons de substància grisa i blanca amb un grup de 33 controls sans. A més, vam utilitzar dades demogràfiques, clíniques i neuropsicològiques per caracteritzar els subtipus. A l’estudi 2, la mostra consistia en 27 pacients amb malaltia de Parkinson i deteriorament cognitiu lleu i 35 pacients sense deteriorament cognitiu, així com de 51 controls sans. En aquest estudi vam aplicar una tècnica nova que en anglès rep el nom de threshold-free network-based statistics (TFNBS), aquesta tècnica resulta útil per estudiar la connectivitat estructural i la vam utilitzar amb dades de tractografia probabilística de tot el cervell. A més, vam complementar l’anàlisi amb TBSS i mesures globals i locals de graf. A l’estudi 3, vam incloure 165 pacients amb Demència amb cossos de Lewy que procedien de la Clínica Mayo i de 3 centres que formen part del consorci europeu de Demència amb cossos de Lewy, abreviat com a E-DLB. Vam realitzar un anàlisi de clúster basat en random forest en el que hi vam introduir volums de substància grisa de regions corticals i subcorticals de tot el cervell. En conseqüència, els subtipus els vam caracteritzar en base als volums de substància grisa, però també en base a dades clíniques, demogràfiques i de biomarcadors de tau, β-amiloide i malaltia cerebrovascular. A més, vam caracteritzar les trajectòries cognitives dels subtipus amb un seguiment de 3 anys. RESULTATS: A l’estudi 1, vam definir 3 subtipus en la malaltia de Parkinson que principalment diferien en substància grisa, mentre que la implicació de la substància blanca era molt baixa. El primer subtipus (PD1, 24%) es caracteritzava per presentar atròfia de la substància grisa a nivell de còrtex temporal, parietal i occipital així com en regions subcorticals. A més, presentava reduccions de l’anisotropia fraccional principalment en tractes fronto-occipitals de substància blanca. Aquest subtipus, el PD1, incloïa als pacients de més edat amb el pitjor perfil neuropsicològic. El segon subtipus (PD2, 34%) es caracteritzava per atròfia de la substància grisa en regions frontals i temporals. El tercer subtipus (PD3, 42%), no presentava alteracions detectables ni a nivell de substància grisa ni de blanca, i presentava un perfil cognitiu preservat. En l’estudi 2, vam trobar que els pacients amb malaltia de Parkinson presentaven menor número de fibres en nombroses connexions cerebrals en comparació amb controls sans. Es van trobar alteracions en la connectivitat estructural tant en pacients amb deteriorament cognitiu lleu com sense deteriorament cognitiu, tot i així, el patró i grau de les alteracions en connectivitat estructural eren diferents entre grups de pacients. Els pacients amb deteriorament cognitiu lleu presentaven major número de connexions alterades, les quals principalment involucraven connexions corticals en regions temporals i occipitals així com connexions entre l’escorça cerebral i els nuclis grisos de la base. Els pacients amb malaltia de Parkinson però sense deteriorament cognitiu, presentaven menys connexions alterades que, a diferència dels pacients amb deteriorament cognitiu lleu, es localitzaven principalment en regions bilaterals del còrtex prefrontal. A més, la regressió logística i la corba ROC van mostrar que el número reduït de fibres en les connexions alterades que caracteritzaven el grup de pacients amb deteriorament cognitiu lleu, permetien la discriminació entre els dos grups de pacients amb una precisió elevada. L’anàlisi de TBSS va mostrar que només el pacients amb deteriorament cognitiu lleu presentaven reduccions en l’anisotropia fraccional en comparació al grup de controls sans, i l’anàlisi de graf va mostrar que els 2 grups de pacients diferien en mesures locals. En l’estudi 3, es van identificar 3 subtipus dins de la Demència amb cossos de Lewy en base als volums de substància grisa. El subtipus amb predominança cortical (30%) es caracteritzava per una reducció generalitzada dels volums corticals de substància grisa, incloïa els pacients de més edat, pitjor cognició global i deteriorament cognitiu més ràpid en el transcurs de 3 anys. El segon subtipus, el fronto-occipital (46%), presentava menors volums de substància grisa en regions frontals i occipitals. Finalment, el subtipus amb predominança subcortical (24%), es caracteritzava per majors volums de substància grisa en regions corticals i volums relativament baixos en estructures subcorticals, donat que eren les úniques estructures en les que no hi havia diferències entre els 3 subtipus. Aquest últim subtipus amb predominança subcortical, també es caracteritzava per una major freqüència de pacients amb fluctuacions cognitives. CONCLUSIONS: En conjunt, les troballes presentades en aquesta Tesi Doctoral donen suport a l’existència de diferents subtipus, tant en la malaltia de Parkinson com en la Demència amb cossos de Lewy, que poden ser identificats mitjançant anàlisis de clúster basats en dades d’IMR. A més, aquests subtipus reflecteixen perfils cognitius específics. El deteriorament cognitiu en la malaltia de Parkinson també es troba associat a un patró concret d’alteracions en connectivitat estructural. Així, aquests resultats contribueixen a aclarir les bases de l’heterogeneïtat descrita en la malaltia de Parkinson i en la Demència amb cossos de Lewy, i aporten informació rellevant sobre quines característiques podrien ser considerades com biomarcadors de pitjor pronòstic, amb l’objectiu final d’apropar-nos a una medicina més personalitzada.

[eng] BACKGROUND: The present Doctoral Thesis is focused on Rapid-eye-movement (REM) sleep behavior disorder (RBD) and olfactory dysfunction as biomarkers of Synucleinopathies, such as Parkinson’s disease (PD), dementia with Lewy bodies (DLB), and multiple system atrophy (MSA). RBD is a parasomnia characterized by loss of atonia during REM sleep that provokes dream-enacting behaviors. Clinically isolated RBD (IRBD) is considered a prodromal stage of alpha-Synucleinopathies since almost 75% of patients after 12 years of the disease develop a PD or atypical parkinsonism. Specifically, in a multicentric study, 56.5% of IRBD patients converted to parkinsonism as the first manifestation, while 43.5% developed dementia first. Subtle motor symptoms, hyposmia, and cognitive impairment are the best predictors of phenoconversion in IRBD. OBJECTIVES AND HYPOTHESES: The main objectives of this Thesis are: 1) To characterize structural and functional brain substrates underlying Idiopathic REM sleep behavior disorder as well as to relate these findings to cognitive performance; 2) To study olfactory dysfunction as a preclinical and clinical biomarker of alpha- Synucleinopathies, and its progression throughout the disease; 3) To investigate progressive brain degeneration throughout IRBD, and to investigate how these changes relate to cognitive decline. The main hypotheses are: 1) IRBD will be characterized by specific changes in brain structure and functional connectivity that will be associate with olfactory and cognitive impairment, 2) IRBD patients will show structural brain changes over time and a cognitive decline superior to that seen in normal aging, 3) Smell dysfunction will be identified in IRBD and PD, and it is expected to progress slightly with the disease course. MATERIAL AND METHODS: This Doctoral Thesis is presented as a compendium of six studies that were carried out to achieve the above-mentioned objectives. RESULTS: In studies 1, 2, and 5, IRBD patients differed from controls in several cognitive domains, namely attention and executive functions, mental processing speed, verbal memory, and semantic fluency. Besides, studies 1 and 5 identified for first-time impairment in facial recognition and visuospatial functions. Contrary to our hypothesis in study 1, we did not find significant correlations between cognitive performance and brain atrophy. However, there was coherence between the structural changes detected by MRI and neuropsychological impairment. For instance, IRBD subjects had facial recognition impairment and cortical atrophy in the fusiform gyrus, a region that is known to be critical for such function. In the same way, patients showed memory impairment and hippocampal atrophy, a core structure for learning and memory. On the other hand, study 2 evidenced the potential role of altered brain functional connectivity in IRBD’s cognitive impairment. We found a positive correlation between mental processing speed and temporoparietal functional connectivity in the IRBD group. In study 5, we reported visual form discrimination decline in IRBD relative to normal aging after less than two years. More importantly, we identified that visual form discrimination worsening over time was explained by progressive reductions of the cortical thickness in the superior parietal. CONCLUSIONS: The present Doctoral Thesis has identified new cognitive deficits in IRBD patients and has shed some light on its progression throughout the course of the disease. In particular, our work has shown the importance of VS/VP functions as a measure able to identify cognitive changes across time in IRBD, and its potential in identifying those patients with progressive neurodegeneration in posterior cortices. Besides, we have exhaustively described the characteristics of severe and early olfactory dysfunction in IRBD and described for the first time its neuroanatomical correlates. The neuroimaging data from this Thesis points towards an early implication of the temporal-occipital-parietal cortices in IRBD. Additionally, our longitudinal work has shown that IRBD patients have brain degeneration of the occipitoparietal and orbitofrontal cortex over time, in line with the degenerative changes reported in PD and DLB. Abnormalities in such regions are supported by the association between impaired cognition and both functional and structural brain changes. Cortical atrophy and disrupted cortico-cortical functional connectivity highlight that cortical degeneration and functional abnormalities already exist in patients with IRBD before they are diagnosed with DLB, PD, or MSA. Thus, suggesting that in prodromal PD, the alpha-synuclein pathology is already involving these structures earlier than would be expected according to the Braak degeneration model proposed form neuropathological data. Cortical atrophy and dysfunction may explain why some patients present neuropsychological impairment before the motor onset. We hope that our research will serve as a basis for further multicentric studies that confirm our findings, and to identify the optimal neuropsychological and neuroimaging markers for diagnostic, prognostic, and efficacy of future neuroprotective therapies., [cat] MARC TEÒRIC: La present Tesi Doctoral estudia el trastorn del comportament del son REM (RBD, per les seves sigles en anglès) i la disfunció olfactiva com a biomarcadors de les alfa- Sinucleinopaties, com ara la malaltia de Parkinson (PD, per les seves sigles en anglès), la demència per cossos de Lewy (DLB, per les seves sigles en anglès) i l’atròfia multisistèmica (MSA, per les seves sigles en anglès). El RBD és una parasòmnia caracteritzada per la pèrdua d’atonia durant el son REM que provoca episodis on els pacients mostren moviments que reflecteixen el contingut dels seus somnis. El RBD que es presenta clínicament aïllat (IRBD, per les seves sigles en anglès), es considera un estadi prodròmic de les Sinucleinopaties, doncs s’ha vist que gairebé el 75% dels pacients després de 12 anys amb diagnòstic d’IRBD acaben sent diagnosticats de malaltia de Parkinson o parkinsonismes atípics. Concretament, en un estudi multicèntric el 56.5% dels pacients amb IRBD varen desenvolupar parkinsonisme com a primera manifestació, mentre que el 43.5% va presentar demència en primer lloc. La simptomatologia motora, la hipòsmia i el deteriorament cognitiu han demostrat ser els millors predictors de conversió en aquests pacients. En aquest context, la comunitat científica ha demostrat un interès creixent en definir els canvis cognitius associats a l’IRBD. Els estudis publicats fins ara conclouen que entre el 15-50% dels pacients tenen deteriorament cognitiu lleu. Malgrat l’evidència ben documentada sobre la presència d’afectació neuropsicològica en pacients amb IRBD, cal una recerca addicional que valori de forma aïllada i específica les funcions visuoespacials i visuoperceptives (VS/VP), ja que l’alteració d’aquestes funcions és característica del perfil neuropsicològic dels pacients amb PD i DLB. Pel que fa al declivi cognitiu al llarg de la malaltia, encara hi ha una gran incertesa. OBJECTIUS I HIPÒTESIS: Els objectius principals d'aquesta Tesi són: 1 Caracteritzar els substrats cerebrals estructurals i funcionals subjacents a l’RBD, així com relacionar aquestes troballes amb el rendiment cognitiu, 2 Estudiar la disfunció olfactiva com a biomarcador clínic i preclínic de les alfa- Sinucleinopaties, i la seva progressió al llarg de la malaltia, 3 Investigar la degeneració cerebral progressiva al llarg de l’IRBD i com aquests canvis es relacionen amb el declivi cognitiu. Les principals hipòtesis són: 1 L'IRBD es caracteritzarà per canvis específics en l'estructura cerebral i la connectivitat funcional que estarà associada a un deteriorament olfactiu i cognitiu, 2 Els pacients amb IRBD mostraran canvis cerebrals estructurals amb el pas del temps i un declivi cognitiu superior a l’observat en l’envelliment normal, 3 S'espera identificar reducció de la capacitat olfactòria en l'IRBD i la PD, així com que progressi lleugerament amb el curs de la malaltia. MATERIAL I MÈTODES: La present Tesi Doctoral consta de sis estudis realitzats per donar resposta als objectius esmenats anteriorment. CONCLUSIONS: La present Tesi Doctoral ha identificat nous dèficits cognitius en pacients amb IRBD i ha perfilar la seva progressió al llarg de l’evolució de la malaltia. En particular, el nostre treball ha demostrat la importància de les funcions VS/VP com a mesura capaç d’identificar els canvis cognitius a través del temps en l’IRBD i el seu potencial per identificar aquells pacients amb neurodegeneració progressiva en el còrtex posterior. A més, hem descrit de forma exhaustiva les característiques de la disfunció olfactiva precoç i greu de l’IRBD i hem descrit per primera vegada els seus correlats neuroanatòmics.

[eng] BACKGROUND AND OBJECTIVES. Parkinson’s disease is a heterogenous neurodegenerative disorder. To characterize homogeneous groups of PD patients, PD phenotypes have been described based on clinical data including motor and non-motor manifestations. This thesis is presented as a compendium of three research studies. The aim was to identify different PD subtypes based on objective MRI measures of cortical thickness. We hypothesized that different patterns of regional brain atrophy would be associated to distinct clinical and cognitive features. METHODS. We have used T1-weighted MRI images acquired with 3T Siemens scanners in two sample of PD patients at different times of the disease evolution: a sample of medicated PD patients (n = 88; disease duration: 8 ± 5.7 years) and a second sample from the Parkinson Progression Marker Initiative (PPMI, https://www.ppmi-info.org/) that enrolled 119 PD newly diagnosed drug naïve patients (n = 77; disease duration: 0.9 ± 1.0 years) with available MRI and neuropsychological assessments. Additionally, the medicated sample was followed-up after four years (n = 45). Both PD samples were compared with two similar groups of healthy elders. Cortical thickness estimation was performed with the FreeSurfer suite v5.1 (https://surfer.nmr.mgh.harvard.edu/). An agglomerative hierarchical cluster analysis technique was used to classify patients from a hypothesis-free data driven approach using Matlab (release 2014b, The MathWorks, Inc., Natick, Massachusetts). For the longitudinal assessment, we computed the symmetrized percent of change of the cortical thickness estimation of both times. RESULTS. In Study 1, we firstly classified patients of the medicated sample according to the vertex-wise cortical thickness data. Three patterns of regional thinning were obtained when comparing them with a sample of healthy controls with similar age and education: (1) a pattern mainly involving temporal and parietal atrophy; (2) a second pattern with frontal and occipital and younger age at disease onset; (3) a third pattern with no manifest atrophy in comparison with controls and reduced processing speed. In Study 2, we classified the PD de novo patients according to their cortical thickness information from the 360 parcellations of the Human Connectome Project Multi-Modal Parcellation version 1.0. Two PD patterns were identified: (1) one pattern with anterior predominance including orbitofrontal, anterior cingulate and temporal atrophy with no cognitive deficits and (2) a posterior-based pattern with lateral occipital and parietal atrophy with associated verbal memory learning and delayed recall deficits as well as visuospatial and processing speed impairment. In Study 3, we assessed the progression of the cortical patterns identified in Study 1 over four years. Pattern 1 patients with initial temporal and parietal widespread atrophy had worse compromise in the activities of daily living. Regarding the other two patterns and the controls group, all groups displayed temporo-parietal progressive decline and reduced processing speed. However, pattern 2 patients with initial prefrontal involvement and younger disease onset had better evolution and focal cortical thinning changes. Pattern 3 patients and controls, that at baseline were the less atrophic groups, displayed extensive symmetrized percent of change in temporal and parietal regions. Despite the similar progression of pattern 3 with controls, pattern 3 patients had more atrophy in the prefrontal cortex over time than controls and more decline in semantic fluency, processing speed and visuospatial function. CONCLUSIONS. PD patients showed different patterns of cortical thinning even at the time of diagnosis, regardless the presence of mild cognitive impairment and medication doses. Our patterned groups of patients based on hypothesis free data-driven methodologies stress the importance to review neuropsychological tools for the diagnosis of PD-MCI. Cortical thickness measures of percent of change revealed sensitive to aging and specific cortical degeneration in PD. Different regional atrophy patterns progress differently over time. It has been observed that initial posterior-based atrophy had worse compromise in the activities of daily living and patients were more likely to progress to dementia, whereas initial prefrontal involvement is linked to a better clinical evolution. Overall, data-driven analyses were able to classify PD patients based on their cortical degeneration depicting distinct clinical manifestations and different progressions. Thus, PD prognosis can be characterized by structural MRI data., [cat] ANTECEDENTS I OBJECTIUS. La malaltia de Parkinson és una malaltia neurodegenerativa molt heterogènia. Per a caracteritzar grups homogenis de malalts, s’han descrit fenotips de pacients basats en manifestacions motores i no motores de la malaltia. Aquesta tesi s’ha elaborat en format de compendi de tres estudis de recerca. L’objectiu va ser identificar diferents subtipus de malaltia de Parkinson basat en mesures objectives de gruix cortical (imatge estructural). Vam hipotetitzar que diferents patrons d’atròfia regional estarien associats a diferents manifestacions clíniques i cognitives de la malaltia. MÈTODES. S’han utilitzat imatges potenciades en T1 de ressonància magnètica estructural amb escàners Siemens de 3T en dues mostres de pacients amb diagnòstic de malaltia de Parkinson en diferents moments evolutius de la malaltia: una mostra de pacients medicats (n = 88; duració de la malaltia: 8 ± 5.7 anys) i una segona mostra extreta de Parkinson Progression Marker Initiative (PPMI, https://www.ppmi-info.org/). Aquesta base de dades pública inclou pacients amb malaltia de Parkinson recent diagnosticats que encara no prenien medicació dopaminèrgica per al maneig de la malaltia (n = 77; duració de la malaltia: 0.9 ± 1.0 anys) i que tenien disponibles imatges de ressonància magnètica i avaluació neuropsicològica. Per a l’anàlisi longitudinal, vam calcular una mesura de percentatge de canvi simètric dels valors de gruix cortical en els dos temps. RESULTATS. En l’estudi 1, primerament vam classificar els pacients medicats d’acord amb la informació de cada un dels vèrtexs que conformen el mantell cortical. Es van obtenir tres patrons d’atròfia regional comparant-los amb la mostra de controls d’edat i educació similars: (1) un patró amb atròfia majoritàriament temporal i parietal; (2) un segon patró amb atròfia frontal i occipital i una edat de debut de la malaltia més precoç; (3) un tercer patró sense atròfia cerebral diferent als controls envellits sans i una reducció en la velocitat de processament. En l’estudi 2, vam classificar els pacients recent diagnosticats i sense medicació (de novo) segons la informació obtinguda de les mitjanes de les 360 parcel·lacions de l’atles Human Connectome Project Multi-Modal Parcellation versió 1.0. Es van identificar dos patrons: (1) un patró amb atròfia predominantment anterior que incloïa regions orbitofrontals, anterior cingulat i temporals i sense alteracions cognitives i (2) un segon patró de base posterior amb atròfia a l’occipital i el parietal laterals i amb alteracions associades en l’aprenentatge i el record de la memòria verbal així com en les habilitats viso-espacials i en la velocitat de processament. En l’estudi 3, vam valorar la progressió dels patrons corticals identificats en l’estudi 1 després de quatre anys de progressió. Els pacients classificats en el patró 1 amb atròfia inicial generalitzada en els lòbuls temporal i parietal van tenir la major proporció de casos de demència i compromís de les activitats de la vida diària. En referència als altres dos patrons d’atròfia i al grup de controls, tots els grups van patir un declivi progressiu en regions temporo-parietals i una reducció en la velocitat de processament. No obstant, els pacients classificats en el patró 2 amb atròfia inicial en el còrtex prefrontal i una edat d’inici de la malaltia més jove, van mostrar la millor evolució i atròfia focal progressiva al llarg del temps. Els pacients en el patró 3 i els controls, que en l’estudi 1 primerament van ser descrits com els menys atròfics, van mostrar atròfia extensa al llarg del temps. Tot i la progressió similar entre aquests dos últims grups esmentats, els pacients en el patró 3 van patir més deteriorament en regions del còrtex prefrontal que els controls i en l’occipital medial en comparació a l’evolució dels pacients en el patró 2. Cognitivament, els pacients del patró 3 van presentar pitjors puntuacions en fluència semàntica, velocitat de processament i habilitats viso-espacials al llarg del temps. CONCLUSIÓ. Els pacients amb malaltia de Parkinson es poden classificar segons diferents patrons de gruix cortical fins i tot en el moment del diagnòstic i independentment de la presència de deteriorament cognitiu lleu i la medicació dopaminèrgica per al maneig de la malaltia. Els nostres grups de pacients identificats a partir de dades objectives lliures d’hipòtesis a priori posen de manifest la rellevància de revisar les eines neuropsicològiques per al diagnòstic de deteriorament cognitiu en la malaltia de Parkinson. Les mesures de percentatge de canvi en el gruix cortical es van mostrar sensibles a l’envelliment sa i també a processos de degeneració cortical específics de la malaltia de Parkinson. Diferents patrons d’atròfia regional progressen de forma diferent al llarg del temps. Hem mostrat que els pacients amb una atròfia inicial de base posterior mostren més compromís en activitats de la vida diària i tendeixen a evolucionar més probablement cap a demència mentre que una atròfia inicialment prefrontal va lligada a una millor evolució clínica. En definitiva, els anàlisis guiats per les dades (data-driven) lliures d’hipòtesis van ser capaços de classificar pacients amb malaltia de Parkinson en base a la seva degeneració cerebral, mostrant diferents manifestacions clínics i progressions al llarg del temps. Per tant, l’evolució de la malaltia es pot caracteritzar a partir de dades de neuroimatge estructural.

[eng] Parkinson’s disease is a common neurodegenerative disorder that manifests with a wide range of non-motor symptoms. Among them, cognitive impairment is present in a high proportion of patients, having a great impact on quality of life and prospective outcome. Initiatives have aimed to describe the features of cognitive impairment in Parkinson’s disease and, although findings are not conclusive, the visuospatial and visuoperceptual neuropsychological domain can be impaired early in the disease course, and these deficits sustain a relationship with disease severity and progression. Moreover, posterior cortically-based deficits are prognostic markers of the emergence of dementia in Parkinson’s disease. Hence, their early identification is an important undertaking in clinical settings. In our studies, we have investigated visuospatial and visuoperceptual impairment in Parkinson’s disease patients, as well as its evolution over time. We assessed 121 Parkinson’s disease patients and 41 healthy matched control subjects, applying well-validated and widely used neuropsychological tests, and followed the sample for an approximate period of four years. We identified the neuroanatomical substrates of these deficits using surface-based cortical thickness measures and tract-based spatial statistics for white matter microstructure integrity parameters. Parkinson’s disease patients exhibit a posterior temporo-parieto-occipital pattern of cortical thinning that is related to the impairment of visual cognition processes; however, these correlates do not appear to follow the classical dissociation described in the current literature of the healthy visual system pathways. In Parkinson’s disease patients with mild cognitive impairment, this degeneration is more pronounced and widespread, and follows a posterior-anterior pattern of cortical thinning. Moreover, after a four-year follow-up, Parkinson’s disease patients with mild cognitive impairment exhibit a differential pattern from cognitively normal patients, that involves temporo-parieto-occipital regions, and is related to changes over time in visuospatial and visuoperceptual measures. Conversely, white matter microstructure appears altered in the posterior corpus callosum in Parkinson’s disease, whereas visuospatial, visuoperceptual and visuoconstructive measures exhibit widespread long- and short-range correlates with white matter degeneration. Our results give support to the hypothesized relationship between impairment in visuospatial and visuoperceptual functions and the degeneration of predominantly posterior cortices over time, establish-ing the structural basis of cognitive decline in Parkinson’s disease. These findings could have translational implications and contribute to the validation of the use of these tests in clinical as well as research settings. Further research involving multimodal approaches of structural and functional neuroimaging techniques in longitudinal settings could shed light on the characterization of the neuroanatomical substrate of cognitive decline in Parkinson’s disease., [spa] La malaltia de Parkinson és un trastorn neurodegeneratiu d’elevada incidència que es manifesta amb una gran varietat de símptomes no-motors. Entre els sintomes no-motors, el deteriorament cognitiu és present en una elevada proporció del pacients i produeix un gran impacte en la seva qualitat de vida i el seu pronòstic. La caracterització del deteriorament cognitiu en la malaltia de Parkinson és un important repte de recerca i, tot i que existeix una elevada disparitat entre les troballes actuals, els dèficits visoespacials i visoperceptius es poden considerar com un domini específic que pot mostrar alteracions de forma primerenca i mostrar una relació amb la gravetat de la malaltia i la seva progressió. Els dèficits originats en el còrtex posterior es consideren un marcador pronòstic de l’aparició de demència a la malaltia de Parkinson, fet que posa de relleu la importància de la seva identificació primerenca en el maneig clínic. En els nostres estudis, hem identificat i catacteritzat el deteriorament visoespacial i visoperceptiu a la malaltia de Parkinson, així com la seva evolució en el temps. Hem recollit dades de 121 pacients i 41 controls sans aparellats segons variables sociodemogràfiques, tot aplicant tests validats i àmpliament utilitzats en l’àmbit clínic. Hem seguit la seva evolució per un període aproximat de quatre anys. Per tal de identificar els substrats neuroanatòmics d’aquests dèficits, hem utilitzat mesures de gruix cortical basades en l’estimació de superfícies, i mesures adreçades a l’avaluació de la integritat de la microestructura de la substància blanca. Els malalts de Parkinson presenten un patró de deteriorament cortical localitzat en regions posteriors temporo-parieto-occipitals, que està relacionat amb els dèficits observats en les funcions visoespacials i visoperceptives. Tot i així, aquests correlats no segueixen la dissociació descrita a la literatura clàssica sobre les vies del sistema visual en subjectes normals. En pacients amb deteriorament cognitiu lleu, els patrons d’atrofia cerebral són mes pronunciats i generalitzats, tot seguint un patró posterior-anterior de pèrdua del gruix cortical. El seguiment realitzat ens ha permès identificar un patró diferencial de degeneració en els pacients amb deteriorament cognitiu lleu, localitzat principalment en la regió temporo-parieto-occipital i que està relacionat amb la pèrdua de funcions visoespacials i visoperceptives. De forma coherent, l’alteració de la microestructura de la substància blanca es localitza a la part posterior del cos callós. Ara bé, la pèrdua de funcions visoespacials, visoperceptives i visoconstructives es relaciona amb la pèrdua de la integritat de regions cerebrals més extenses, que impliquen fascicles intra- i interhemisfèrics. Els resultats evidencien la relació entre el deteriorament de les funcions visoespacials i visoperceptives i la degeneració neuroanatòmica predominantment del còrtex posterior. Aquestes troballes contribueixen a establir la base estructural del deteriorament cognitiu en la malaltia de Parkinson i poden tenir implicacions translacionals, ja que contribueixen a la validació de l’ús d’un conjunt de tests en el maneig clínic i en termes de recerca. En el futur, la combinació d’estudis multimodals estructurals i funcionals de caire longitudinal poden contribuir en la caracterització del substrat neuroanatòmic i neurofuncional del deteriorament cognitiu a la malaltia de Parkinson i determinar-ne el valor pronóstic d’evolució a demència.

[ENG] The present doctoral thesis is focused on describing the effects that different environmental and genetic modulators have on white matter pathways and its consequences measured with diffusion tensor imaging. We chose to focus on two examples of each type of modulators. Firstly, we selected as environment modulating factors: pollutants and video games. On one side, pollution as an external factor that enters passively the brain and may influence developmental trajectories. And on the other hand, we used video games as a good example of active behavior that can modify white matter tracts through practice. Secondly, Down syndrome and Prader-Willi syndrome were selected as representative genetic syndromes that may interfere on white matter growth because, although Down syndrome has higher incidence rate than Prader-Willi syndrome, both show behavioral and cognitive alterations, indicating an abnormal brain development. The results of this doctoral thesis lead to the conclusion that white matter pathways development is not an immutable process and it can be modified by diverse modulators. In the same way, diffusion tensor imaging is a good-quality technique to capture and identify those white matter changes through life., [spa] La presente tesis doctoral se centra en describir los efectos que diferentes moduladores ambientales y genéticos tienen sobre las vías de la sustancia blanca y sus consecuencias a través de imágenes de tensor de difusión. Decidimos centrarnos dos ejemplos de cada tipo de moduladores. En primer lugar, se seleccionó como factores de modulación ambiental: contaminantes y videojuegos. Por un lado, la contaminación es un factor externo que penetra pasivamente el cerebro y puede influir en las trayectorias del desarrollo. Y por otro lado, los videojuegos son un buen ejemplo de comportamiento activo que puede modificar los tractos de la materia blanca a través de la práctica. En segundo lugar, se seleccionaron el síndrome de Down y síndrome de Prader-Willi como síndromes genéticos representativos que pueden interferir en el crecimiento de la materia blanca ya que, aunque el síndrome de Down tiene una tasa de incidencia superior al síndrome de Prader-Willi, ambos muestran alteraciones cognitivas y conductuales fruto de un subdesarrollo de las vías de sustancia blanca. Los resultados de esta tesis doctoral nos llevan a la conclusión de que el desarrollo de vías de sustancia blanca no es un proceso inmutable y puede ser modificado por diversos moduladores. De la misma manera, el tensor de difusión es una técnica adecuada para capturar e identificar los cambios en la sustancia blanca que acontecen a lo largo de la vida

[spa] ANTECEDENTES: Los estudios sobre el Deterioro Cognitivo Leve (DCL) se centran en buscar predictores de conversión a demencia, principalmente Enfermedad de Alzheimer (EA). No obstante, las clasificaciones de DCL no han integrado los nuevos datos genéticos en la EA. OBJETIVOS: 1. Determinar el riesgo de conversión a demencia y detección de los principales factores de riesgo implicados, en una muestra de 550 pacientes con DCL. 2. Evaluación del efecto de 23 marcadores genéticos sobre la conversión a EA en 1.250 sujetos con DCL. 3. Identificación de modelos predictivos univariantes y multivariantes de conversión a EA mediante técnicas estadísticas y estudios de supervivencia. 4. Replicación de los marcadores genéticos significativos en series adicionales de DCL. 5. Identificación de elementos redundantes entre los marcadores genotipados y priorización racional de los más predictivos. 6. Realizar un meta-análisis de los resultados más relevantes. 7. Analizar la asociación entre patrones neuropsicológicos y marcadores genéticos. MÉTODO: Estudio retrospectivo con 550 sujetos mayores de 60 años diagnosticados de DCL en Fundació ACE (ACE), según criterios de Petersen et al.[138,142] y López et al.[102,103] clasificados en cuatro subtipos: amnésico probable (DCL-a-Pr) (n=115), no-amnésico probable (DCL-na-Pr) (n=37), amnésico posible (DCL-a-Ps) (n=234) y no-amnésico posible (DCL-na-Ps) (n=164) con afectación en un único o múltiples dominios. Se realizaron Análisis de la Varianza (ANOVA), Chi-Cuadrado y análisis de supervivencia (estimador Kaplan Meier; Regresiones de Cox) para valorar conversión. Estudios prospectivos: 1) con biomarcadores de neuroimagen: muestra de DCL-a-Pr (n=20) de ACE con Resonancia Magnética (RM) estructural, tomografía por emisión de positrones (PET) con 18F-fluorodesoxiglucosa (PET-FDG) y PET con Compuesto B de Pittsburgh (N-Methyl-[11C]2-(4’-methylaminophenyl) 6-hydroxy-benzotriazol (PET-PIB). La construcción de cinco Composites Cognitivos, las medidas del volumen hipocampal ajustado (VHa) y del ratio del valor de captación estandarizado (SUVR) del PET-FDG se replicaron en la base de datos parental AB255 (n=133 DCL-a-Pr de almacenamiento). Se correlacionaron, ajustando por edad, género y escolaridad, los cinco Composites Cognitivos y el VHa, el SUVR PET-FDG y el SUVR PET-PIB. Replicación con la base de datos “Alzheimer’s Disease Neuroimaging Initiative” (ADNI). 2) Correlación fenotipo-genotipo: muestra de DCL (n=1.250), divididos en cuatro subtipos: DCL-a-Pr (n= 262), DCL-na-Pr (n= 76), DCL-a-Ps (n= 549) y DCL-na-Ps (n= 358) con afectación en un único o múltiples dominios, que cumplían los criterios de inclusión del estudio retrospectivo, con evaluación basal clínica y seguimiento anual para verificar su conversión a EA. Genotipado de ADN germinal mediante tecnología Sequenom. Se realizaron análisis de supervivencia y de regresión lineal mediante PLINK, replicación en series independientes y meta-análisis. Análisis de asociación basal entre endofenotipos neuropsicológicos y marcadores genéticos. RESULTADOS: El fenotipo clínico DCL-a-Pr (de almacenamiento) obtuvo 8,5 veces más riesgo de conversión a demencia, principalmente EA, que el DCL-na-Ps, con el menor riesgo y tasa de conversión a demencia. Los endofenotipos neuropsicológicos que mejor predijeron conversión a demencia para la muestra de DCL fueron la Orientación a la Realidad, Recuerdo Diferido de la Lista Palabras WMS-III y Relojes de Luria. El Composite Cognitivo Recuerdo Diferido, correlacionó con los tres biomarcadores de neuroimagen asociados a la EA prodrómica. Se identificó el locus CLU como factor genético independiente de APOE-ε4 asociado a fenoconversión de DCL a EA y se halló un efecto protector del locus HS3ST1 (rs6448799) asociado a Dígitos Inversos y un efecto de riesgo del locus AP2A2 (rs10751667) asociado a Repetición verbal, en el fenotipo DCL-a-Pr., [eng] BACKGROUND: Mild Cognitive Impairment (MCI) classifications have not integrated new genetic data into Alzheimer's Disease (AD). OBJECTIVES: 1. To determine the risk of conversion to dementia and to detect the main risk factors involved in MCI patients. 2. Evaluation of the effect of genetic markers on the conversion to AD in MCI patients. 3. Identification of predictive models of AD conversion using statistical techniques and survival studies. 4. Replication of significant genetic markers in additional series of MCI. 5. Identification of redundant elements among genotyped markers and rational prioritization of the most predictive ones. 6. Perform a meta-analysis of the most relevant results. 7. To analyze the association between neuropsychological endophenotypes and genetic markers. METHOD: Retrospective study with 550 subjects >60 years diagnosed with MCI at Fundació ACE (ACE), according to Petersen and López criteria, classified into four subtypes: probable amnestic (Pr-a-MCI) (n=115), probable non-amnestic (Pr-na-MCI) (n=37), possible amnestic (Pss-a-MCI) (n=234) and possible non-amnestic (Pss-na-MCI) (n=164) single or multiple domain. Prospective studies: 1) Neuroimaging biomarkers: Pr-a-MCI-storage (n=20) sample with structural MRI, PET-FDG and PET-PIB. Replication with the AB255 database (n=133 Pr-a-MCI-storage). The aHV, SUVR PET-FDG and SUVR PET-PIB were correlated with five Cognitive Composites (CCs). Replication with the ADNI database. 2) Phenotype-genotype correlation: sample of MCI (n=1,250), divided into four subtypes: Pr-a-MCI (n=262), Pr-na-MCI (n=76), Pss-a-MCI (n=549) and Pss-na-MCI (n=358). Genotyping of germinal DNA using Sequenom technology. Survival and linear regression analyses were performed using PLINK, independent series replication and meta-analysis. Basal association analysis between neuropsychological endophenotypes and genetic markers. RESULTS: The clinical phenotype Pr-a-MCI-storage obtained 8.5 times more risk of conversion to dementia, mainly AD, than Pss-na-MCI, with the lowest risk. The neuropsychological endophenotypes that best predicted conversion to dementia were Orientation, Delayed Recall of the Word List from the WMS-III and Luria’s Clocks test. The Delayed Recall CC correlated with the three neuroimaging biomarkers associated with prodromal AD. The CLU locus was identified as a genetic factor independent of APOE-ε associated with phenoconversion from MCI to AD, HS3ST1 locus (rs6448799) was found associated with Backward Digits and AP2A2 locus (rs10751667) associated with verbal Repetition, in the Pr-a-MCI phenotype.

Aquesta tesi inclou 6 estudis en els quals s’han utilitzat diversos mètodes d’imatge per ressonància magnètica (IRM) per caracteritzar la connectivitat cerebral i la seva relació amb l’estat cognitiu en joves sans, persones d’edat avançada i pacients amb malaltia d’Alzheimer. Les tècniques d’IRM permeten estudiar la connectivitat estructural a partir del Tensor de Difusió (DTI), i la connectivitat funcional mitjançant la IRM funcional (IRMf), mesurada durant l’execució d’una tasca o durant el repòs. S’ha descrit un conjunt de xarxes neuronals, que inclou la xarxa neuronal per defecte (o Default Mode Network, DMN). Utilitzant IRMf en un grup de joves sans, es va trobar que la connectivitat de la DMN, durant el repòs previ a una tasca correlacionava amb el rendiment. A més, l’estat de les xarxes implicades en la tasca també correlacionà amb l’execució. En un altre grup de joves sans, es van estudiar les xarxes implicades en la memòria de treball amb estímuls facials o espaials. Es van descriure regions i connexions específiques per la memòria de treball amb cares, que implicaren el fusiforme i la regió frontal inferior. Posteriorment, es van estudiar les xarxes de la memòria de treball en l’envelliment. Els subjectes d’edat avançada amb correcta execució mostraren increments d’activitat en regions frontals bilaterals, indicant mecanismes compensatoris. També es varen estudiar patrons de connectivitat global, utilitzant IRM en repòs i graph-theory en un grup de 98 subjectes d’edat avançada. L’edat correlacionà amb un decrement de la connectivitat funcional en les connexions llargues i un increment en les curtes. Els subjectes més grans presentaren més clustering funcional, sobretot en regions frontals i parietals, indicant un increment de la segregació. Les mesures obtingudes es van relacionar amb la memòria visual i verbal. A continuació es vàren estudiar els patrons d’interacció de les xarxes neuronals també en el context de l’envelliment. La connectivitat entre els components de la DMN correlacionà negativament amb l’edat, mentre que la connectivitat entre diferents sistemes augmentà amb l’edat. Aquestes interaccions es relacionaren amb la memòria i les funcions executives. Finalment, es va utilitzar la IRMf en subjectes amb una mutació en el gen Presenilin-1, causant de la malaltia d’Alzheimer. Es van trobar alteracions funcionals en la DMN i en una tasca de memòria episòdica, fins i tot en aquells subjectes que estaven lluny de l’edat d’inici de la malaltia. En resum, hem pogut demostrar la utilitat de la IRMf per a estudiar xarxes de connectivitat cerebral i hem pogut concloure que aquestes correlacionen amb l’estat cognitiu i amb l’execució de les tasques. A més, s’han descrit alteracions en la connectivitat durant l’envelliment sà i en la malaltia d’Alzheimer, fins i tot abans de l’aparició de símptomes clínics. La DMN apareix com un sistema altament implicat en cognició en subjectes joves sans, però també com el sistema més vulnerable tant en l’envelliment com en la malaltia d’Alzheimer., INTRODUCTION: This thesis has been elaborated as a compendium of 6 research studies, in which we have used a variety of methods related with Magnetic Resonance Imaging (MRI) with the objective to characterize brain connectivity and its relationship with cognition in young and aged subjects and in preclinical Alzheimers Disease (AD). Brain Connectivity refers to any pattern of links connecting different areas of the brain. It can be stud­ied at its functional level, by using functional MRI (fMRI), which measures the statistical dependence between brain activity at different regions, or at its structural level, with Diffusion Tensor Imaging (DTI), which estimates the directionality of white matter .ber tracts. FMRI can be studied during the execution of a task (task-fMRI) or during resting state (rs-fMRI). With task-fMRI, it is possible to obtain patterns of brain co-activations, measured by the Blood-Oxigen Level Dependent (BOLD) signal. On the other hand, rs-fMRI allows measuring patterns of brain con­nectivity, known as Resting State Networks (RSNs), as correlations between spontaneous oscillations. Functional connectivity has been used to describe a set of networks in the brain, including the Default Mode Network (DMN), which is deactivated during task and shows high activity levels at rest. RESULTS: In the .rst study, we included 16 healthy young subjects, under rs-fMRI and during a Working Memory (WM) task-fMRI. The connectivity of the DMN at rest correlated with the perfor­mance in the subsequent task. During task-fMRI, we described the DMN and the WM network. The connectivity between these networks was highly negative in the most demanding blocks and during .xation, which also correlated with performance. In addition, within-network connectivity increased with cognitive demands. In the second study, we analyzed the networks involved in two different WM tasks, with visuoper­ceptive (faces) and visuospatial (squares) stimuli. We used task-fMRI and DTI. We found that the fusiform and the inferior frontal gyrus were selectively activated for faces, and that the DTI measures of the tracts connecting these regions correlated with task performance for the facial-WM. In the third study, we analyzed a sample of young and old subjects during a WM task. We found patterns of increased activity in the WM networks in aging that were related to successful performance, indicating compensatory mechanisms. The results of this study supported and extended previous research on fMRI and aging. In study 4, we included a sample of 98 healthy elders, during rs-fMRI. We used graph-theory to an­alyze whole-brain patterns of connectivity. Age correlated with a loss of functional connectivity in long-range connections and an increase in functional connectivity in short-range connections. These changes resulted in increases in clustering coef.cient and larger average minimum path-length of the global network, indicating higher functional segregation and less integration. Increases in clustering were located in frontal and parietal regions and they correlated negatively with visual and verbal memory functions. In study 5, we used a sample of 76 healthy elders during rs-fMRI and we analyzed functional interac­tions between the components of the main RSNs. The anterior and posterior components of the DMN were less connected in older subjects, and the connectivity between different networks increased with age, indicating alterations in the global architecture of functional networks. RSN-interactions corre­lated with the results in memory and executive functions. Finally, in the last study, we included a sample of asymptomatic and symptomatic carriers of the PSEN1 mutation, which causes Familial AD. We found altered functional connectivity and brain ac­tivity at rest and during memory encoding that appear even before the onset of AD. CONCLUSIONS: Overall, we proved the usefulness of fMRI to study brain connectivity networks and we concluded that connectivity signals correlate with cognitive status and with task performance. In addition, we described alterations in connectivity during the healthy aging process and in AD, even before the disease onset. Finally, we identi.ed the DMN as the system showing the highest correlations in healthy subjects but also as a core target for aging and AD.

Parkinson’s disease is a neurodegenerative process with several motor and non-motor manifestations. Among non-motor symptoms, cognitive decline is a major cause of disability, and its neural bases are poorly understood. In recent years, multimodal neuroimaging techniques have proven to be useful tools in the investigation of the bases of cognitive impairment related to neurological diseases. The objective of this thesis was to evaluate the neuroimaging substrates of Parkinson’s disease-related cognitive and neuropsychiatric manifestations through a network approach, using state-of-the art magnetic resonance imaging techniques to assess associated connectivity and structural changes. In this work, two samples of Parkinson’s disease patients and healthy controls underwent neuropsychological evaluation as well as structural and functional magnetic resonance imaging. One of these samples included 121 Parkinson’s disease patients and 49 healthy controls. The data obtained were used in 2 studies addressing the resting-state functional connectivity changes associated with mild cognitive impairment in Parkinson’s disease; one using a graph-theory approach, and the second assessing large-scale intrinsic connectivity networks through an independent-component analysis/dual regression approach. A third was performed to assess connectivity disruptions in frontostriatal circuits associated with the presence of apathy in Parkinson’s disease. And a fourth study was made assessing cortical thickness changes associated with the presence of mild cognitive impairment in Parkinson’s disease. The second sample included the longitudinal evaluation of 17 Parkinson’s disease patients and 15 healthy controls, and the data obtained resulted in two studies regarding progressive cortical thickness and subcortical volumes in early-stage Parkinson’s disease patients. A seventh study, using subjects from both samples, was performed to evaluate microstructural white matter changes (using diffusion tensor imaging) and gray matter changes (through voxel-based morphometry) related to the presence of facial emotion recognition deficits in Parkinson’s disease. We have found that a high percentage of Parkinson’s disease patients had mild cognitive impairment. These patients showed altered patterns of resting-state functional connectivity characterized by the loss of long range connections and strengthening of local connectivity. From a graph-theory perspective, these changes translated as increased small-world coefficients, modularity and clustering coefficients, which correlated with visuospatial/visuoperceptual and memory performance. The study of large-scale networks showed that patients with mild cognitive impairment had reduced connectivity between the dorsal attention network and the right frontoinsular region, and that this reduction was associated with attention/executive deficits. Additionally, parieto-occipital regions that belong to the dorsal attention network showed reduced connectivity with anterior task­positive regions and loss of the normal anticorrelated pattern with the default mode network; furthermore, these posterior regions showed structural degeneration. Finally, these posterior structural and functional changes were associated with the presence of visuospatial/visuoperceptual deficits. The analysis of Parkinson’s disease patients with mild cognitive impairment revealed a pattern of cortical thinning predominating in parieto­occipito-temporal regions. The longitudinal analysis of progressive structural changes in early Parkinson’s disease revealed that these patients had more marked cortical thinning in frontotemporal regions, even before the onset of clinically evident cognitive manifestations. The functional analysis of a recognition memory network likewise showed signs of progressive connectivity changes without overt clinical deterioration. We conclude that different types of cognitive decline in Parkinson’s disease are associated with different patterns of resting-state functional connectivity, structural connectivity and GM structural changes involving distinct neural systems. Different techniques and different conceptual frameworks can provide useful information in the characterization of the neural bases of cognitive deficits associated with Parkinson’s disease., La malaltia de Parkinson és un procés neurodegeneratiu que té diverses manifestacions motores i no motores. Entre les manifestacions no motores, el deteriorament cognitiu és una causa important i comú de discapacitat; al cap de 20 anys des de l’inici de la malaltia, més de 80% dels pacients desenvolupen demència. Tot i tenir una alta prevalença i representar un factor de pèrdua de qualitat de vida tant pels pacients com per als seus cuidadors, les bases neurals d’aquestes alteracions són poc conegudes. En els últims anys, les tècniques d’imatge multimodals han demostrat ser útils en la investigació de les bases dels dèficits cognitius associats a les malalties neurològiques, i alguns estudis previs han trobat alteracions cerebrals tant estructurals com funcionals en subjectes amb la malaltia de Parkinson. D’altra banda, el concepte de deteriorament cognitiu lleu (DCL), utilitzat per definir la presència d’un rendiment cognitiu inferior al esperat segons l’edat i el nivell educatiu – i un major risc de desenvolupament de demència – s’ha començat a aplicar en el context de la malaltia de Parkinson. A més, en estudis epidemiològics s’ha vist que la presència d’alguns tipus d’alteració cognitiva – concretament, aquells que tenen substrats neurals predominants en regions corticals posteriors, com són dèficits visuoespacials i visuoperceptius – indiquen una major probabilitat de desenvolupar demència. Al contrari, en els dèficits relacionats amb els desequilibris en la neurotransmissió dopaminèrgica (com són els dèficits d’atenció i executius), no s’ha trobat que siguin marcadors de pitjor pronòstic cognitiu. L’objectiu d’aquesta tesi va ser avaluar els substrats neuroanatòmics i neurofuncionals de les diferents alteracions cognitives i neuropsiquiàtriques relacionades amb la malaltia de Parkinson mitjançant un abordatge de xarxes, tot utilitzant tècniques avançades de ressonància magnètica per estudiar les disfuncions de la connectivitat cerebral. En aquesta tesi, es van utilitzar dues mostres de pacients amb malaltia de Parkinson i subjectes sans que es van sotmetre a avaluació neuropsicològica i de ressonància magnètica funcional i estructural. La primera d’aquestes mostres va incloure 121 malalts de Parkinson i 49 controls sans. Les dades obtingudes amb aquesta mostra han sigut utilitzades en cinc estudis (estudis 1, 2, 3, 4 i 5), tres dels quals van abordar la connectivitat funcional cerebral en repòs i les alteracions estructurals associats a la presència de DCL. Per definir la presència de DCL, en dos estudis (estudis 1 i 2) s’han avaluat els déficits en tres dominis cognitius (atenció/funcions executives, memòria i funcions visuoespacials/visuoperceptives); en l’altre estudi (estudi 3), s’ha utilitzat un mqtode de classificació d’acord amb les recents directrius proposades per la Movement Disorder Society Task Force, que consideren el rendiment en cinc dominis (atenció/memòria de treball, funcions executives, memòria, llenguatge i funcions visuoespacials/visuoperceptives). En els estudis 1 i 2, es van avaluar 66 pacients i 36 controls utilitzant tècniques d’avaluació de la connectivitat cerebral en repòs i les alteracions corresponents associats a la presència de DCL. Trenta cinc per cent dels pacients amb malaltia de Parkinson van complir criteris de DCL. Estudi 1: En aquest estudi, s’ha aplicat un abordatge de teoria de grafs per avaluar les alteracions globals de connectivitat. Per tal de reconstruir les xarxes cerebrals, la substància grisa cortical i subcortical s’ha dividit en 90 regions definides amb l’atles Automated Anatomical Labelling. Posteriorment, s’ha calculat la correlació temporal de l’activitat entre cada parell de regions, obtenint-se així una matriu representativa de la connectivitat funcional de tot el cervell. En un primer pas, s’han comparat directament aquestes matrius de correlació per tal d’avaluar el patró de reducció o augment de connectivitat en els diferents subgrups de pacients. Aquest anàlisi va revelar que els pacients amb DCL tenien reduccions de connectivitat, sobretot afectant les connexions interlobulars de llarga distància, així com alguns augments de connectivitat, sobretot en connexions mps curtes. Posteriorment, s’han utilitzat les matrius de correlació per calcular mesures globals i regionals de teoria de grafs. Concretament, s’han avaluat paràmetres que mesuren la integració (l’eficiència d’intercanvi d’informació entre distintes àrees cerebrals): el characteristic path length i l’eficiència global;i paràmetres de segregació, que mesuren la interconnectivitat local: el clustering coefficient i l’eficiència local. També s’ha avaluat el grau de modularitat de les xarxes cerebrals, és a dir, quant de bé aquestes xarxes es podien dividir en mòduls o comunitats de nodes densament interconnectats, amb poques connexions entre diferents mòduls. S’ha observat que els pacients amb DCL presentaven augments en les mesures de segregació i de modularitat. Les anàlisis de correlació han revelat que aquestes mesures es correlacionaven amb el rendiment en funcions visuoespacials/visuoperceptives i de memòria. En l’anàlisi de les mesures de centralitat dels nodes – és a dir, la importància que tenen en el tràfic d’informació dins de la xarxa –, s’ha observat que els nodes que solen ser més centrals en subjectes sans (els anomenats hubs cerebrals) perden centralitat en els pacients amb malaltia de Parkinson. Aquest resultat indica una reorganització d’aquestes xarxes caracteritzat per un augment del flux d’informació neural a travès de nodes que en subjectes sans són menys rellevants. Aquest estudi es va publicar a la revista Human Brain Mapping a l’any 2014 (Cognitive impairment and resting-state network connectivity in Parkinson’s disease). Estudi 2: s’ha utilitzat la mateixa mostra que en l’estudi 1. S’ha utilitzat, però, un altre tipus d’anàlisi amb l’objectiu d’estudiar els patrons de connectivitat dins d’un conjunt de xarxes cerebrals que, segons estudis previs, tenen un paper rellevant en els processos cognitius: la default mode network, la xarxa dorsal de l’atenció i la xarxa frontoparietal. Inicialment, s’ha realitzat un anàlisi de components independents amb les dades de ressonància magnètica funcional en repòs per tal d’identificar les xarxes d’interès a nivell grupal. Posteriorment, s’ha utilitzat la tècnica de dual regression per identificar les mateixes xarxes a nivell individual, i que permet comparacions entre els grups. En aquesta anàlisi, s’ha observat que els pacients amb DCL presentaven reduccions de connectivitat entre la xarxa dorsal de l’atenció i l’ínsula anterior i regions adjacents del lòbul frontal dret. En el grup de pacients, el nivell de connectivitat en aquestes regions es correlacionava amb el rendiment en el domini d’atenció/executiu. A més, s’ha vist que els pacients amb DCL presentaven un augment de connectivitat entre extenses àrees parieto-occipitals i la default mode network; aquestes disfuncions de connectivitat també estaven associats a pitjor rendiment cognitiu, però en aquest cas en el domini visuoespacial/visuoperceptiu. Com anàlisi addicional per definir les alteracions locals de connectivitat, es va realitzar una avaluació de les connexions entre els nodes individuals de les xarxes d’interès; per definir aquests nodes, s’han seleccionat a priori les coordenades disponibles en un estudi de referència. Aquesta anàlisi ha demostrat que els augments de connectivitat de regions corticals posteriors amb la default mode network en realitat estaven caracteritzats per una pèrdua del patró normal d’anticorrelació amb aquesta xarxa observat en controls sans. A més, s’ha vist que aquestes mateixes regions tenien menys connectivitat amb les altres xarxes avaluades. Finalment, s’han realitzat anàlisis complementaris per avaluar la presència de degeneració estructural de la substància grisa que pogués estar associada a les alteracions funcionals observades. L’anàlisi del gruix cortical va revelar la presència de reduccions en els pacients amb DCL en regions parieto-occipitals; aquestes reduccions estaven associades a les alteracions de connectivitat de la default mode network i amb el rendiment visuoespacial/visuoperceptiu. Aquest estudi es va publicar a la revista Human Brain Mapping a l’any 2014 (Functional brain networks and cognitive deficits in Parkinson's disease). Estudi 5: En el tercer estudi en que es va avaluar la connectivitat funcional en repòs, l’objectiu va ser investigar les alteracions en els circuits fronto-estriats en la malaltia de Parkinson amb apatia – un trastorn neuropsiquiàtric molt freqüent en pacients amb aquesta malaltia. Amb aquesta finalitat, es va utilitzar la mateixa mostra que en els dos primers estudis. Es van classificat els subjectes en apàtics o no-apâtics segons la puntuació en l’escala d’apatia de Starkstein. A més, es va tenir en compte la presència de trastorns depressius, que, a més de tenir una alta prevalença en la malaltia de Parkinson, freqüentment coexisteixen amb l’apatia i tenen manifestacions que s’hi solapen. Així doncs, també es va aplicar l’escala de depressió de Beck; la puntuació en els 11 ítems d’aquesta escala que representen els símptomes disfòrics, més específics de la depressió, es va utilitzar com a covariable en totes les anàlisis. Per a realitzar l’anàlisi de connectivitat funcional, es va parcel·lar l’estriat en 3 regions – límbica, executiva i sensorimotora. Així mateix, es va parcel·lar l’escorça frontal en 4 regions – límbica, executiva, rostral motora i caudal motora. Vint pacients (41%) es van classificar com apâtics (puntuació > 13 en l’escala d’apatia). L’anàlisi de connectivitat va revelar que la presència d’apatia estava acompanyada de reduccions, que principalment afectaven les regions límbiques tant del estriat com de l’escoroa prefrontal. Per tal d’avaluar si aquestes alteracions s’acompanyaven de degeneració de les estructures avaluades, es van realitzar anàlisis addicionals amb voxel-based morphometry de les estructures corticals i subcorticals, a més de volumetria i shape analysis dels nuclis del estriat. No s’han obtingut resultats significatius amb cap d’aquests abordatges. Aquest estudi està actualment en revisió en una revista indexada. Estudi 3: En aquest estudi, 90 pacients i 32 controls van ser estudiats mitjançant tècniques d’avaluació del gruix cortical. Trenta dos (52%) pacients van complir criteris de DCL. S’ha observat que el grup de pacients amb DCL presentava reduccions del gruix cortical en regions parieto-temporals, així com augment en mesures d’atròfia global tals com reducció del gruix cortical global i del volum de substància grisa, així com augment del volum ventricular. Les anàlisis de correlació van revelar que el rendiment en totes les proves neuropsicològiques estava associat a la pèrdua de gruix cortical posterior. Aquest estudi es va publicar a la revista Movement Disorders a l’any 2014 (Cortical thinning associated with mild cognitive impairment in Parkinson's disease). Per a la segona mostra de subjectes, es van reclutar 24 pacients amb malaltia de Parkinson inicial i 24 controls sans, els quals es van estudiar mitjançant ressonància magnètica estructural i funcional així com mitjançant exploració neuropsicològica. Disset pacients i 15 controls van ser avaluats longitudinalment, amb una segona avaluació després d’un seguiment mitjà de 35,5 mesos. Estudi 6: En el primer estudi fet amb aquesta mostra, es van incloure 17 pacients i 13 controls, i l’objectiu va ser estudiar la xarxa de memòria de reconeixement. Per això es va utilitzar una seqüència de ressonància magnètica funcional amb un paradigma de memòria de reconeixement que consistia en intentar reconèixer les 35 paraules prèviament apreses entre una llista de 70 paraules. Les dades obtingudes van ser avaluades a través de abordatges convencionals o model-based, així com a través de tècniques model-free (anàlisi de components independents). En la primera anàlisi, es va identificar el patró d’activació associat a la tasca. En l’anàlisi de components independents, també es van identificar els components associats a la tasca; les principals regions activades en la component més associada a la tasca es van utilitzar com a regions d’interès en un anàlisi subsegüent de canvis progressius de connectivitat. Es va trobar una correlació entre la activació de la component independent més associada a la tasca i el rendiment de memòria de reconeixement dels pacients. A més, l’avaluació longitudinal va revelar canvis progressius en la connectivitat entre els principals nodes d’aquesta xarxa, caracteritzats per la pèrdua de connectivitat entre regions frontoparietals i la preservació de la connectivitat frontofrontal (que en els controls sans s’havia reduït). Aquest estudi es va publicar a la revista Journal of Neurology, Neurosurgery and Psychiatry a l’any 2013 (Progressive changes in a recognition memory network in Parkinson's disease). Estudi 7: En aquest segon estudi fet amb la segona mostra, s’ha volgut analitzar el patró d’atròfia progressiva de la substància grisa en la malaltia de Parkinson inicial i la seva relació amb canvis neuropsicològics. Amb aquesta finalitat, les dades de setze pacients i 15 controls van ser avaluades amb tècniques de gruix cortical, volum de substància grisa a través de voxel-based morphometry i volumetria cortical i subcortical. Es van trobar reduccions progressives del gruix cortical en regions frontotemporals bilaterals. L’avaluació neuropsicològica va revelar que els pacients tenien pitjor rendiment en proves d’atenció i de velocitat psicomotora, però aquestes variables no es correlacionaven amb els canvis corticals. Aquest estudi es va publicar a la revista Movement Disorders a l’any 2012 (Progression of cortical thinning in early Parkinson's disease). Un últim estudi (estudi 4) es va realitzar amb subjectes de la primera i de la segona mostra. Es va seleccionar una submostra de 39 pacients i 23 controls amb la finalitat d’avaluar les alteracions de les substàncies blanca i grisa associats als dèficits de reconeixement d’emocions facials en la malaltia de Parkinson. Es van utilitzar imatges potenciades en difusió per l’estudi d’alteracions microestructurals de la substància blanca en els tractes llargs que connecten les estructures cerebrals involucrades en el processament emocional, mitjançant tècniques de imatge per tensor de difusió. A més, les imatges estructurals es van avaluar a través de voxel­based morphometry per estudiar les alteracions de volum de la substància grisa en les estructures cerebrals prèviament descrites com rellevants pel processament d’emocions. El rendiment en el reconeixement d’emocions facials es va mesurar mitjançant la prova d’Ekman. Els pacients amb la malaltia de Parkinson van obtenir un rendiment inferior als controls en la identificació de les emocions negatives (tristesa, ràbia, por i fàstic). L’anàlisi de correlacions va revelar que els dèficits d’identificació de tristesa correlacionaven amb els valors d’anisotropia fraccional –un paràmetre de la microestructura de la substància blanca– sobretot en el fascicle fronto-occipital inferior dret. L’anàlisi de substància grisa, per contra, va revelar que la identificació de tristesa correlacionava amb el volum de l’escoroa orbitofrontal dreta, amígdala i gir postcentral; la identificació de ràbia correlacionava amb el volum de substància grisa de l’estriat ventral, escorça infragenual i gir fusiform occipital dret; i la identificació de fàstic correlacionava amb volum de substància grisa en el còrtex cingulat anterior. Aquest estudi es va publicar a la revista Neuropsychologia a l’any 2012 (Structural correlates of facial emotion recognition deficits in Parkinson's disease patients). Tots aquests estudis ens permeten concloure que un alt percentatge de pacients amb malaltia de Parkinson tenen alteracions cognitives i compleixen criteris de DCL. L’anàlisi de neuroimatge mitjançant diferents estratègies demostra que la presència de DCL en la malaltia de Parkinson s’acompanya d’alteracions estructurals i funcionals. A més, diferents tipus de dèficit cognitiu es relacionen amb diferents patrons d’alteració. A nivell de connectivitat funcional en repòs, els pacients amb malaltia de Parkinson amb DCL presenten un patró predominant de reducció de connectivitat de llarg abast i augment de la connectivitat local, que es tradueix en un augment del caràcter modular i de segregació de les xarxes cerebrals. Així mateix, s’observa una reducció de connectivitat de components de xarxes de connectivitat intrínseca rellevants per al processament cognitiu, que es caracteritza per una reducció de connectivitat d’una xarxa involucrada en processos de l’atenció (xarxa dorsal de l’atenció) i la regió insular anterior dreta. Al mateix temps, s’observa una reducció de connectivitat de regions corticals posteriors amb regions cerebrals anteriors, que s’associa a atròfia cortical posterior i a la presència de dèficits en les funcions visuoespacials/visuoperceptives. L’anàlisi de la connectivitat funcional en repòs revela també que la presència d’apatia en la malaltia de Parkinson s’acompanya de reduccions en circuits fronto-estriats, que sobretot afecten els components del sistema de recompensa, és a dir, l’estriat ventral i l’escorça orbitofrontal. Els resultats de l’anàlisi d’alteracions microestructurals de la substància blanca associats al pitjor reconeixement d’emocions facials semblen indicar que els dèficits de connectivitat estructural també estan implicats en l’ocurrència dels dèficits cognitius en la malaltia de Parkinson. Finalment, en pacients amb malaltia de Parkinson inicial, s’han aportat evidències sobre el poder dels mètodes de neuroimatge per a detectar alteracions estructurals (pèrdua de gruix neocortical) i funcional fins i tot abans de l’inici de símptomes cognitius clínicament evidents. En conclusió, els resultats obtinguts en els diferents estudis d’aquesta tesi ens permeten concloure que distintes tècniques i diferents marcs conceptuals poden proporcionar informació útil per a la caracterització de les bases neurals dels dèficits cognitius i emocionals associats a la malaltia de Parkinson.

[spa]INTRODUCCIÓN: El daño axonal difuso que se sufre después de un traumatismo craneoencefálico, produce alteraciones cerebrales estructurales y funcionales que se pueden visualizar y analizar mediante imágenes por resonancia magnética (IRM). Estas alteraciones pueden explicar los déficits cognitivos. OBJETIVO: Identificar las posibles alteraciones funcionales y estructurales cerebrales en pacientes con daño axonal difuso y su relación con sus déficits cognitivos mediante la aplicación de pruebas neuropsicológicas y IRM MÉTODOS/ESTUDIOS: La tesis incluye cuatro artículos de investigación en los que se combinan técnicas por tensor de difusión, resonancia magnética funcional, estudio de grosor cortical y volumetría junto con tests neuropsicológicos. La batería de test escogida incluye diferentes pruebas para la evaluación de los dominios cognitivos más frecuentemente alterados en estos pacientes: funciones ejecutivas, aprendizaje y memoria, atención y velocidad de procesamiento mental. Los pacientes incluidos en los estudios se caracterizan principalmente por ser pacientes adultos jóvenes (20-45 años) con TCE grave (GCS≤8) en estado crónico (≤2años desde el traumatismo craneo-encefálico), con evidencia de patología cerebral difusa en la IRM clínica, Los pacientes se han emparejado con un grupo control de acuerdo con las variables edad, escolaridad y dominancia manual. RESULTADOS: 1) El decremento de anisotropía fraccional (AF) en fascículos largos asociativos explica los déficits de memoria de trabajo mientras que una disrupción más local se relaciona más con déficits de memoria declarativa; 2) a pesar de una afectación general de sustancia blanca en todos los tractos cerebrales evidenciado por una reducción de AF y una reducción generalizada cortical, los déficits de memoria declarativa se explican por una alteración cortico-subcortical de sustancia blanca y por la reducción cortical en el área del precuneus; 3) La alteración de la conectividad estructural explica las alteraciones cerebrales de activación funcional y explica los déficits de memoria de trabajo; 4) El incremento de actividad funcional del lóbulo frontal en estado de reposo está asociado a un mejor rendimiento cognitivo. CONCLUSIÓN: La alteración de la conectividad estructural, especialmente la alteración de conexiones con el lóbulo frontal, junto con la alteración de la actividad funcional basal medida en estado de reposo, son las alteraciones principales causantes de las alteraciones cognitivas después de un traumatismo craneo-encefálico de carácter difuso., [eng]Traumatic Brain Injury (TBI) can be defined as an alteration in brain function or evidence of brain pathology caused by an external force. In this thesis the focus of attention is on brain diffuse axonal injury (DAI). DAI is widespread damage to axons throughout the white matter in the brain, caused by sustained acceleration and deceleration forces at the moment of injury (Adams, 1982). Is characterised by the presence of microhaemorrhages mainly in the cerebral hemispheres, corpus callosum, and brainstem. However, other regions such as the thalamus, basal ganglia and fornices are also susceptible (Adams, 1989). Diffuse damage makes the main contribution to neurological dysfunction in TBI patients and typically associated with poor prognosis (Smith and Meaney, 2000). In the absence of significant focal contusions, diffuse brain damage may be not detected on neuroimaging MRI initial scans and macroscopical brain examination. However, microscopic primary damage can be appreciated. In a first stage, primary damage, which is the direct consequence of the direct impact either by mechanical or external forces, produces tissue segmentation. Secondary damage then occurs. This includes a cascade of biological reactions which provokes cytoskeletal disorganization, protein accumulation, metabolic, hypoxic and microvascular damage, and excitotoxicity. As DAI is an evolving process, thus traumatic axonal injury (TAI) has more recently been suggested as a more appropriate term for describing diffuse axonal damage. These secondary processes can result in injury or complete disruption of critical white matter pathways between the cortex and deep grey matter structures (Buki & Povlishock, 2006). Taking into account that this disruption of white matter pathways may be causing cognitive deficits, the main hypothesis of this thesis is that white matter damage is responsible for the alterations in connectivity making the main contribution to cognitive deficits after diffuse, chronic and severe TBI. Moreover, the main objective to identify possible structural and functional connectivity alterations in the brains of patients suffering from diffuse axonal injury and to relate them with cognitive impairment by applying neuropsychological assessment and magnetic resonance imaging techniques. To investigate the connectivity alteration and their relationship with cognitive impairment after chronic TBI, we performed four studies. The first study aimed to investigate the role of white matter damage in declarative and working memory deficits after diffuse TBI focusing on the main associative fasciculi. Given that memory impairment is one of the most common deficits of patients with severe TBI, and taking into account the white matter alterations resulting from the first study, we performed this second study with the aim of assessing structural brain damage in subjects with diffuse and chronic severe TBI and to study its relationship with declarative memory impairment. The third study was the first multimodal approach study in TBI to explore the functional and structural patterns of connectivity underlying working memory impairment after severe traumatic axonal injury. And finally, in the fourth study, we wanted to take advantage of the analysis of resting state fMRI and aimed to explore abnormalities in brain activity and connectivity during the resting state in patients with chronic traumatic axonal injury and examine the relationship of these abnormalities with deficits in structural connectivity and global cognitive outcome. The general conclusion extracted from the results of the studies performed here is that, although after TAI there are clear widespread patterns of white matter damage and cortical atrophy, the results of these studies seem to suggest that long-term cognitive sequelae after diffuse TBI follows a predominant frontal pattern of alteration. Disrupted frontal white matter structural connectivity and the basal activity of the frontal lobe at rest are the main alterations explaining cognitive outcome after diffuse and chronic TBI.

[eng] Background: In recent years it has become recognized that schizophrenia is associated with cognitive impairment, which affects particularly executive function and memory. Although cognitive impairment implies brain damage or dysfunction, little is known about the relationship of cognitive impairment in schizophrenia to the structural and functional brain abnormalities that characterize the disorder. Aims and hypothesis: This study aimed to identify structural and/or functional brain abnormalities associated with cognitive impairment in schizophrenia. According to the literature reviewed, the general hypothesis is that the cognitive deficits of schizophrenic patients are reflected in both structural and functional brain changes. Accordingly, we expect that patients with cognitive impairment will have more GM reductions and more dysfunctional patterns of brain activity than patents without such deficits. Method. structural MRI and voxel-based morphometry was carried out in 26 cognitively impaired and 23 cognitively preserved schizophrenia patients, plus 39 matched controls. Nineteen cognitively impaired patients, 18 cognitively preserved patients and 34 controls also underwent fMRI during performance of a working memory task. Some subjects, partly corresponding to the ones participating in the neuroimaging study, were also administered a battery of tests of different cognitive functions. Altogether, 25 participants with schizophrenia and relatively preserved cognitive function, 29 with impaired functions and 22 healthy participants were included in this part of the study. Results: The participants with cognitive impairment in executive functioning and memory also perform worse on visuospatial and language tasks when compared to other patients intact in memory and executive function and controls. These differences are independent of estimated premorbid IQ. No differences were found between the cognitively intact and cognitively impaired groups in lateral ventricular volume or whole brain volume. Voxel-based morphometry also failed to reveal clusters of significant difference in either GM or WM volume between the two patient groups. However, during performance of the n-back task, the cases with schizophrenia and impaired cognitition showed hypoactivation compared to the cognitively intact patients in DLPFC among other brain regions. Conclusions: This study provides evidence that structural brain abnormality in schizophrenia is a function of having the disorder, not the cognitive impairment that accompanies it. In contrast, a substantial part of the task-related functional imaging abnormality appears to reflect cognitive impairment., [spa] MARCO: Los déficits cognitivos son una característica conocida de la esquizofrenia. Sin embargo, poco se sabe sobre su relación con las anormalidades cerebrales tanto estructurales como funcionales que caracterizan dicha enfermedad. OBJETIVOS: Identificar las alteraciones estructurales y/o funcionales asociadas al déficit cognitivo en la esquizofrenia. MÉTODO: Se adquirió imágenes de resonancia magnética funcional (RMf) y morfometría basada en el vóxel (VBM) en 26 participantes que tenían alteraciones cognitivas y 23 que presentaban una cognición relativamente preservada, todos con esquizofrenia, así como en 39 controles apareados. Diecinueve de quienes presentaban déficits cognitivos y 18 de los preservados cognitivamente y 24 controles también ejecutaron una tarea n-back de memoria de trabajo durante la adquisición de imágenes de RMf. RESULTADOS: No se encontró diferencias entre los participantes con cognición preservada y quienes presentaban déficits cognitivos en el volumen de los ventrículos laterales ni tampoco en el volumen cerebral total. La VBM tampoco mostró clústers con diferencias significativas entre ambos grupos en el volumen de sustancia blanca y gris. Sin embargo, durante la realización de la tarea n-back de memoria de trabajo, los participantes con alteración cognitiva presentaron hipoactivación en la corteza prefrontal dorsolateral, entre otras regiones, en relación a quienes mostraban una cognición preservada. CONCLUSIONES: No se encontró evidencia de que los déficits cognitivos de la esquizofrenia sean una función de anormalidades cerebrales estructurales que acompañan a la enfermedad sino que se asociarían con un funcionamiento cerebral alterado.

[eng] Parkinson’s disease (PD) is currently the second most common neurodegenerative disorder after Alzheimer’s disease (AD), affecting 100.000 people in Spain and more than 4 million people worldwide. Being a movement disorder, impairment of motor functions is considered to be the hallmark symptom of PD. However, cognitive dysfunction is now recognized as a common feature, being present since early disease stages and affecting the majority of patients as the disease progresses. In line with this, one of the most common cognitive deficits found in PD patients are verbal semantic and phonemic fluency deficits. Although there are still no established biomarkers for PD, neuroimaging methods offer the possibility of mapping the neurobiological substrates of motor and cognitive impairment in these patients. Amongst the available neuroimaging techniques, structural and functional magnetic resonance imaging (MRI, fMRI) produces images of the brain that provide quantitative values of cerebral atrophy and brain dysfunction, respectively. These techniques are relevant because they shed light into neurodegeneration processes occurring in PD compared to normal aging as well as those pathological processes that specifically underlie cognitive decline in these patients. In addition to neuroimaging, another method that allows studying brain networks and their disruption in PD is transcranial direct current stimulation (tDCS), which, in addition to modulating functional connectivity, provides a tool for cognitive and motor rehabilitation. This thesis comprises three different studies that had two main goals. On one hand, it was aimed at characterizing the brain’s structural and functional correlates of semantic and phonemic fluency cognitive deficits in patients with PD using MRI, fMRI and tDCS methods. On the other hand, given the lack of a comparison of the relative contribution of different existing MRI-based techniques to characterize cortical degeneration in PD, specific changes in different characteristics of the cortical gray matter (GM) layer were assessed to determine whether brain atrophy in PD can be attributed to changes in cortical thickness, cortical folding or GM volume. The first study of this thesis showed that semantic fluency is associated with GM density in cortical frontal and temporal areas in PD patients, suggesting this function relies both on executive functions and semantic memory processes. The second study showed that by stimulating the left dorsolateral prefrontal cortex with tDCS, significant enhancement of functional connectivity occurs in both semantic and phonemic fluency networks in PD, in addition to a significant improvement of phonemic fluency functions. The third and final study showed that amongst the most widely used neuroimaging analysis techniques to assess cortical changes in PD, cortical thickness seems to be the most sensitive, followed by cortical folding and finally GM volume as assessed by voxel-based morphometry. To conclude, the results that were found in this thesis shaded light into previously unstudied issues related to PD, increasing the knowledge on some of the cerebral pathological changes that affect patients with this disorder. In addition they encourage the use of tDCS to explore brain networks in PD and suggest that surface-based techniques such as cortical thickness are useful to assess GM changes occurring in these patients., [spa] Actualmente, la enfermedad de Parkinson (EP) es el segundo trastorno neurodegenerativo más frecuente después de la enfermedad de Alzheimer. Al ser un trastorno del movimiento, el síntoma principal de la EP es el deterioro de las funciones motoras. Sin embargo, la disfunción cognitiva actualmente se reconoce como una característica más de la EP, que está presente desde las fases iniciales de la enfermedad y que afecta a la mayoría de los pacientes a medida que la enfermedad va avanzando. Entre los déficits cognitivos más comunes en la EP se encuentra el deterioro de la fluidez semántica y fonológica que normalmente se atribuye al deterioro de las funciones ejecutivas. Aunque todavía no se han establecido biomarcadores definitivos para la EP, los métodos de neuroimagen como la resonancia magnética estructural y funcional (RM, RMf) y los métodos de estimulación cerebral no invasiva como la estimulación por corriente directa (ECT) ofrecen la posibilidad de estudiar los sustratos neurobiológicos del deterioro cognitivo en estos pacientes. Esta tesis está formada por tres estudios. El primer estudio de esta tesis demostró que la fluidez semántica está asociada con la densidad de sustancia gris en áreas corticales frontales y temporales en los pacientes con EP, lo que sugiere que esta función se basa tanto en las funciones ejecutivas como en la memoria semántica. El segundo estudio mostró que la ECT sobre la corteza prefrontal dorsolateral izquierda, da lugar a un aumento significativo de la conectividad funcional en las redes neuronales de la fluidez semántica y fonológica, además de una mejora significativa de las funciones de fluidez fonológica en la EP. El tercer y último estudio mostró que entre las técnicas de análisis de neuroimagen más utilizadas para evaluar los cambios corticales en la EP, el grosor cortical parece ser el más sensible, seguido por el plegado cortical y el volumen de sustancia gris. Para concluir, los resultados que se encontraron proporcionan información relevante sobre temas que previamente no se habían estudiado en la EP, aumentando el conocimiento sobre algunos de los cambios cerebrales patológicos que afectan a los pacientes con este trastorno

OBJETIVO: Determinar los cambios en la función de flexibilidad cognitiva y en el flujo sanguíneo cerebral regional asociado, inducidos por el tratamiento con risperidona, en pacientes naive que inician un trastorno esquizofrénico. HIPÓTESIS: Los pacientes naive que inician un trastorno esquizofrénico presentarán durante la realización de una tarea de flexibilidad cognitiva un déficit en el rendimiento y un patrón de perfusión cerebral alterado. Tras el tratamiento tendrá lugar una mejoría de los índices de perfusión alterados y en el rendimiento durante la ejecución de la tarea, observándose una relación entre ambas variables. MÉTODO: Se obtuvieron imágenes SPECT de 8 sujetos de sanos en situación basal y durante la ejecución del subtest de flexibilidad de la batería TAP (Test of Attention Performance). Paralelamente se obtuvieron imágenes de SPECT de 8 sujetos naive con un primer episodio esquizofrénico, en situación de activación durante la realización de la misma tarea, antes y tras 6 semanas de tratamiento con risperidona. RESULTADOS: El grupo de sujetos sanos mostró una activación de la sección dorsal del giro cingulado anterior relacionada con la realización de la tarea, y se observaron correlaciones significativas entre el rendimiento en flexibilidad y los índices de perfusión correspondientes. En la condición pretratamiento, los pacientes mostraron un déficit en el rendimiento en la tarea de flexibilidad y una disminución del nivel de perfusión de las áreas cerebrales mencionadas, pero a diferencia de los controles sanos, no se observaron correlaciones entre la ejecución y el flujo sangineo cerebral obtenidas con los sujetos sanos. Tras el tratamiento con risperidona, se observó una mejoría significativa en el rendimiento en la tarea y en el nivel de perfusión de la sección dorsal del cingulado anterior, así como una mayor relación entre el rendimiento en la tarea y los índices de perfusión correspondientes., “STUDY OF REGIONAL CEREBRAL BLOOD FLOW WITH 99MTC-ECD SPECT DURING PERFORMANCE ON A FRONTAL ACTIVATION TASK BEFORE AND AFTER RISPERIDONE TREATMENT IN DRUG-NAIVE PATIENTS WITH SCHIZOPHRENIA” TEXT: The aim of this study was to examine the changes in cognitive flexibility and associated cerebral blood flow in the anterior cingulate lobe of drug-naïve patients with first-episode schizophrenia who were treated with atypical antipsychotics for 6 weeks. Single photon emission computed tomography (SPECT) images were obtained from 8 healthy subjects both at rest and while performing the flexibility subtest of the TAP (Test for Attentional Performance). SPECT images were obtained in parallel from 8 first-episode drug-naive schizophrenic patients while they were performing the same task both before and after 6 weeks of risperidone treatment. In the control group, an increase in the perfusion indices of the dorsal section of the anterior cingulate gyrus was observed in activation condition. Task performance was altered and the level of perfusion of the brain region related to the task execution was significantly decreased in the patients at baseline. After treatment, there was a significant improvement in both task performance and the level of perfusion of the dorsal section of the anterior cingulate. We conclude that treatment with risperidone improves cognitive flexibility, and there was a relationship between such improvements and normalisation of perfusion indices of the involved brain areas.

The Vegetative State (VS) is a clinical condition characterized by wakefulness in the absence of awareness of self or environment. A person in a VS retains autonomic functions but exhibits no evidence of voluntary responses to stimulation nor evidence of language comprehension or expression. Some VS patients may evolve to a Minimally Conscious State (MCS), demonstrating inconsistent but reproducible evidence of awareness. The differential diagnosis between these two conditions, as well as between them and other disorders such as coma or locked-in syndrome, is challenging and prone to errors. Neuroimaging techniques can shed light on this process by providing complementary information that cannot be obtained from a bedside examination. Using functional magnetic resonance imaging (fMRI) several authors have demonstrated that some VS and MCS patients may retain residual cognitive functions. However, to date, no functional or structural neuroimaging study has identified clear biomarkers that allow for differentiation between these two groups of patients.The present doctoral thesis consists of four studies with the general aim to examine the neuroanatomical and neurofunctional bases of the impairment of consciousness in VS and MCS patients. Firstly, we focused on the assessment of brain responses to speech in a group of VS and MCS patients by means of fMRI. Secondly, we combined fMRI, diffusion tensor imaging (DTI) and clinical and neuropsychological assessment to study the cerebral and clinical changes of a VS patient from the time of VS through to recovery. Finally, we applied DTI and high-resolution T1-weighted structural MRI to in-vivo characterize the neuropathological patterns of VS and MCS patients and explore possible differences between these two groups of patients. In the third study we focused on the subcortical white matter, brainstem and thalami, following post-mortem works. Based on the results obtained in that study, in the fourth study we focused on the thalamus and its global and regional atrophy.Taken as a whole the four studies included in this thesis contribute to a better characterization of the cerebral mechanisms that construct awareness and the structural and functional bases of the DOC. They further support the use of fMRI to detect residual cognitive function that may remain unobservable in bedside examinations of these patients. They also highlight the potential of the combination of structural and functional neuroimaging techniques in their study as well as their possible future use in the estimation of outcome. Furthermore, our results provide for the first time evidence that VS and MCS patients can be reliably distinguished in vivo in base of structural measures, suggesting that MRI techniques could be combined with other measures in the clinical field to help in the solution to the problem of misdiagnosis., "El estado vegetative y el estado de mínima conciencia: función cerebral, conectividad y anormalidades estructurales".El estado vegetativo (EV) se caracteriza por la pérdida de conciencia de uno mismo y el medio. En la evolución del EV algunos pacientes progresan a un estado de mínima conciencia (EMC), comenzando a mostrar signos de conciencia, aunque de manera fluctuante e inconsistente. El diagnóstico diferencial entre estos dos estados es problemático y está asociado a un alto porcentaje de error. Desde el campo de la neuroimagen se han intentado aportar herramientas que permitan obtener información relevante para el proceso diagnóstico. Sin embargo, ningún estudio de neuroimagen funcional o estructural ha conseguido identificar marcadores que permitan diferenciar claramente entre estos dos grupos de pacientes. La presente tesis doctoral consta de cuatro estudios cuyo objetivo partida era examinar las bases neuroanatómicas y neurofuncionales de la alteración de la conciencia en el EV y el EMC. En primer lugar nos centramos en la evaluación de las respuestas cerebrales al lenguaje mediante la RMf en un grupo de pacientes en estos dos estados. En segundo lugar, combinamos técnicas de RMf, imagen con tensor de difusión (ITD) y exploraciones clínicas y neuropsicológicas para estudiar los cambios que acompañaron la recuperación de la conciencia en un paciente en EV. Por último, nos centramos en el estudio de ITD, así como de imágenes estructurales de alta resolución potenciadas en T1 para caracterizar en vivo los patrones neuropatológicos presentes en pacientes en EV y EMC y explorar posibles diferencias entre estos dos subgrupos. Tomada en conjunto, la presente tesis doctoral contribuye a una mejor caracterización de los mecanismos cerebrales implicados en la conciencia y las bases estructurales y funcionales de sus alteraciones. Nuestros resultados apoyan la utilidad de la RMf para detectar funciones cognitivas residuales en pacientes en EV y EMC. Así mismo, destacan el potencial de la combinación de técnicas de neuroimagen funcional y estructural en el estudio de estos pacientes. Por último, proporcionan por primera vez evidencia de que los pacientes en EV y en EMC pueden ser diferenciados en base a medidas estructurales, sugiriendo que estas técnicas podrían ser combinadas con otras medidas clínicas para ayudar a solucionar los problemas diagnósticos existentes.

La enfermedad de Parkinson (EP), la enfermedad de Parkinson con Demencia (EPD) y la Demencia con Cuerpos de Lewy (DCL) presentan una alteración del metabolismo de la alfa-sinucleína que provoca la formación de agregados proteicos llamados Cuerpos de Lewy. Las sinucleinopatías son la segunda causa de demencia degenerativa después de la enfermedad de Alzheimer. Se caracterizan neuropatológicamente por inclusiones de alfa-sinucleína en el tronco cerebral, núcleos subcorticales, áreas neocorticales y límbicas, y clínicamente por demencia, alteraciones atencionales, parkinsonismo y comúnmente alucinaciones visuales.Dado que la DCL y la EPD presentan un solapamiento clínico, se ha argumentado que pueden representar la misma enfermedad. La DCL se diagnostica cuando la demencia ocurre antes o de manera paralela al parkinsonismo, mientras que el término EPD es usado para describir la demencia que ocurre en el contexto de una EP bien establecida.La presente tesis doctoral consta de dos estudios los objetivos de los cuales fueron establecer las diferencias en sustancia gris cerebral entre DCL y EPD mediante la técnica de análisis voxel-based morphometry aplicada a imágenes de resonancia magnética potenciada en T1, así como determinar las diferencias en perfil cognitivo en estos grupos de pacientes. Asimismo, en el primer estudio, se evaluó la relación entre las estructuras cerebrales y las funciones cognitivas en estos pacientes, y en el segundo estudio, se evaluaron los correlatos neuroanatómicos de las alucinaciones visuales, así como su correlación con el funcionamiento cognitivo en este grupo de pacientes.Para ello se estudiaron un total de 12 pacientes diagnosticados de DCL, 16 pacientes con EPD y 16 controles sanos apareados por edad y sexo, mediante una secuencia de RM 3D potenciada en T1 adquirida en un escáner de 1.5 Tesla y una evaluación neuropsicológica y conductual.Las conclusiones que se derivan de este estudio son las siguientes:Los pacientes con DLC y EPD presentan un patrón diferente de atrofia regional. En comparación con los controles, los pacientes con EPD presentan mayor reducción en áreas parieto-occipitales y los pacientes con DCL en áreas frontales, temporales y parietales. Además, los pacientes con DCL tienen una mayor pérdida de sustancia gris que los EPD en diversas áreas frontales, en concreto en las circunvoluciones frontal inferior, frontal superior y premotora.En los análisis individuales, los pacientes con DCL y los pacientes con EPD presentan una pérdida de sustancia gris hipocampal bilateral, pero solo la frecuencia de la reducción hipocampal derecha fue significativamente mayor en DCL que en EPD.La presencia de alucinaciones visuales se relaciona con una reducción de la sustancia gris en regiones frontales en ambos grupos, pero en diferentes áreas. En los pacientes con DCL en la circunvolución frontal inferior y en la EPD en la región orbitofrontal. Asimismo, la severidad y frecuencia de las alucinaciones visuales correlaciona negativamente con el volumen de la circunvolución frontal inferior y el precuneus.En el perfil atencional, los pacientes con DCL presentan mayor distractibilidad, caracterizada por menor vigilancia, mientras que los pacientes con EPD presentan mayor impulsividad, reflejada por errores intrusivos y perseverativos.En el grupo con DCL, el déficit de memoria visual se correlaciona con un decremento del volumen del área temporal medial derecha (hipocampo y amígdala) y los déficits atencionales correlacionan con una pérdida del volumen del cingulado anterior, frontal inferior y prefrontal dorsolateral bilateralmente. En este mismo grupo, la presencia y severidad de las alucinaciones visuales se relaciona con una alteración de la fluencia verbal semántica, del control inhibitorio de la atención y de las funciones visuoperceptivas, mientras que en los pacientes con EPD se relaciona con déficits en memoria visual.La conclusión final, es que en los dos grupos con demencia, hay un patrón de pérdida de sustancia gris y un perfil cognitivo diferencial. Asimismo, cada patología presenta un patrón distinto de correlaciones entre sustancia gris y conducta.

Preterm birth is frequently associated with an increased risk of neurodevelopmental difficulties and of cognitive, behavioural and emotional problems during childhood. The present thesis comprises two MRI studies that demonstrated patterns of cerebral gray matter (GM) and white matter (WM) alterations and their cognitive correlates in children and adolescents who were born preterm.Our first study provided evidence of the persistence of diffuse WM abnormalities in adolescents who were born very preterm, and underlines their high frequency. The individual voxel-based morphometry analysis approach demonstrated that 80% of preterm subjects had WM abnormalities, the most frequently affected areas being the centrum semiovale and the posterior periventricular regions. These results suggest that WM reductions are common, even in those preterm subjects without motor impairment and who receive normal schooling. Volumetric and MRI-related cognitive outcomes suggest that cognitive processing speed is persistently impaired following early brain damage, despite the existence of developmental plasticity. Although the nature of the relationship between diffuse WM injury and cognitive/behavioural deficits is complex and not entirely understood, the results of our first study suggest that WM abnormalities are related with worse Performance IQ scores and slower processing speed. So, we can affirm that diffuse WM loss in preterm children plays an important role in long-term cognitive impairment. Our second study focussed on the investigation of preterm children with a low risk either of neurological deficit or of developmental difficulties. While the neurodevelopmental and cognitive outcome of high-risk preterm samples is well known, little research has been conducted into low-risk preterms, such as those born between 30-34 weeks of GA, with uncomplicated perinatal histories, normal cranial ultrasound scans and no obvious neurodevelopmental deficits. To our knowledge, this is the first study to investigate the brain volume characteristics of a low-risk preterm sample in childhood using an MRI approach and the first attempt to relate these measures to cognitive performance. This study demonstrated that low-risk preterm children are characterized by the presence of regional cortical GM volume reductions in the parietal and temporal lobes which correlate strongly with IQ. Moreover, preterm children also showed WM volume reductions that were concomitant with the GM loss in the parietal and temporal regions compared to full-terms.In summary, the body of results derived from this thesis provides evidence that preterm birth is associated with brain abnormalities and cognitive impairment in middle childhood and adolescence. Future studies are required to assess the impact of cognitive and behavioural function in middle childhood on later outcomes in preterm samples with low risk of neurodevelopmental deficits., Les seqüeles cognitives i conductuals associades al naixement prematur constitueixen actualment un dels principals temes d'interès pediàtric. Les imatges per ressonància magnètica (RM) han resultat una eina interessant per a l'estudi de les lesions cerebrals associades a la prematuritat, encara que els correlats entre les troballes en RM i els indicatius del neurodesenvolupament estan poc investigats.L'interès general d'aquesta tesi doctoral se centra en l'estudi de les bases neuroanatòmiques (patrons d'alteració de substància grisa i blanca cerebral) relacionades amb el rendiment cognitiu que presenten els nens i els adolescents que han nascut prematurs. Amb aquest objectiu s'han usat tècniques volumètriques de neuroimatge basades en imatges de RM, així com avaluacions cognitives i de conducta en dues mostres de nens i adolescents (edats mitges de 9 i 14 anys respectivament) amb antecedents de part prematur. L'objectiu del primer estudi va ser investigar les relacions entre el rendiment cognitiu general i la integritat (concentració) de la substància blanca en una mostra d'adolescents que van néixer molt prematurs. Segons el nostre coneixement, cap estudi previ no havia usat la tècnica de la morfometria basada en el vòxel (VBM de l'anglès voxel-based morphometry) per analitzar els possibles correlats entre la substància blanca cerebral i els processos cognitius relacionats amb el quocient d'intel·ligència (QI) manipulatiu i amb mesures de velocitat de processament en una mostra d'adolescents amb antecedents de part prematur. A més a més, aquest va ser el primer estudi en usar una anàlisi de comparacions individuals amb la VBM a fi d'avaluar i establir els diferents patrons d'afectació de la substància blanca per a cada adolescent nascut prematur.Mentre que el desenvolupament neurològic i cognitiu dels prematurs d'alt risc ha rebut molta atenció, pràcticament no hi ha estudis amb referència als prematurs amb baix risc a desenvolupar dèficits neurològics o cognitius. Els prematurs de baix risc es defineixen com aquells nascuts amb una prematuritat moderada (30-34 setmanes de gestació), sense evidències de complicacions neonatals associades, amb resultats d'ultrasons cranials normals i sense dèficits obvis del desenvolupament motor, cognitiu i social. Fins a la realització de la nostra segona investigació, cap altre estudi no havia investigat les característiques volumètriques d'una mostra de prematurs de baix risc mitjançant RM (tècnica VBM), ni havia relacionat aquestes mesures amb l'execució cognitiva.Les conclusions d'aquesta tesi derivades de l'estudi I (I-III) i de l'estudi II (IV-VI) son:I. Els adolescents amb antecedents de prematuritat, sense evidències d'alteracions en la substància blanca segons una inspecció visual neuroradiològica de les imatges de RM, presenten amb una freqüència elevada reduccions de concentració de substància blanca en comparació amb els adolescents nascuts a terme, usant la tècnica de neuroimatge voxel-based morphometry. II. Els resultats dels anàlisis de patrons d'alteració de la substància blanca cerebral en prematurs adolescents mostren que el centre semioval i les regions periventriculars posteriors son les àrees més freqüentment afectades. Aquests resultats suggereixen que aquestes alteracions en substància blanca son altament comunes en subjectes amb antecedents de part prematur, i que persisteixen fins a l'adolescència, després d'un llarg període de maduració cerebral.III. Els adolescents amb història de part prematur presenten una afectació major en el QI Manipulatiu que en el QI Verbal. Tanmateix, l'afectació difusa de la substància blanca cerebral té un efecte important en el rendiment cognitiu dels prematurs, sobretot en tasques d'organització perceptiva i de velocitat de processament.IV. Els nens prematurs amb baix risc de desenvolupar dèficits del neurodesenvolupament presenten unes reduccions volumètriques de substància grisa cerebral tant globals com regionals localitzades principalment al lòbul temporal.V. Els nens prematurs de baix risc presenten decrements de volum de la substància blanca adjacents a les regions d'afectació de la substància grisa, encara que a només a les regions temporals esquerres van assolir la significació estadística. Amb aquests resultats, queda palès que els nens prematurs de baix risc mostren una afectació principalment en la substància grisa cortical, amb reduccions associades, però no tant manifestes, de substància blanca. La integritat de les substàncies grisa i blanca cerebral en la infància estan relacionades amb variables neonatològiques tals com l'edat de gestació i el pes al néixer. VI. El rendiment cognitiu dels nens prematurs de baix risc està caracteritzat per dèficits en la comprensió verbal i en el raonament perceptiu. Tanmateix, els nens prematurs de baix risc no mostren problemes de conducta o afectius en comparació amb els nens nascuts a terme.Finalment, destacar que són necessaris estudis futurs que avaluïn els efectes de la prematuritat per se en el desenvolupament i la morfologia cerebral així com també en relació amb els subseqüents correlats cognitius.

According neuropathological research limbic changes occur in Parkinson's disease with and without dementia. Structural MRI have reported atrophy of limbic structures including amygadala and hippocampus, in both demented and non-demented PD patients; and a progression of limbic atrophy over time. In addition, atrophy of paralimbic areas (i.e. paracingulate gyrus) and limbic association cortex (i.e orbitofrontal cortex) has also been reported in PD. Functional studies have also reported limbic dysfunctions in Parkinson's disease. PET studies reported that orbitofrontal and amygdalar presynaptic dopaminergic functions are altered in early PD and a significant decrease in the metabolism of the medial OFC in a follow-up study of patients with early PD. fMRI studies showed an abnormal amygdalar response in PD while recognizing fearful faces and lack of activation in the right hippocampus and amygdala while perceiving odorant stimuli inside the scanner. Neuropsychological studies pointed out that the functions that are known to depend on the integrity of limbic system are impaired in Parkinson's disease even in the early stages of the disease. Specifically, it has been reported and impairment in verbal memory, recognition of facial expressions of emotions, decision-making and olfactory function. However, the neuroanatomical and neurofunctional correlates of these dysfunctions are not known or are poorly investigated. For memory functions, hippocampal and amygdalar volumes have been reported to correlate with memory impairment. For decision making and recognition of emotions there are no studies of its correlates; and for olfactory dysfunctions one study reported a correlations between this impairment and a reduced fractional anisotropy in the cerebellum.The general aim of this thesis is to investigate the neuroanatomical and neurofunctional correlates of declarative memory, decision-making, recognition of emotions and olfactory dysfunctions in PD. We hypothesized that all these dysfunctions are due to the limbic degenerative changes associated with PD. We used structural MRI (T1-weighted MRI and DTI), functional MRI (fMRI) and neuropsychological testing to assess declarative memory, emotional processing and decision-making, and olfactory function. Firstly, we focused on the hippocampal atrophy putatively related to declarative memory dysfunctions and PD, and a possible neuroradiological marker for the evolution to dementia. This issue was investigated in papers I and II. Secondly, we aimed to investigate structural correlates of deficits in the recognition of emotions, decision-making and olfactory dysfunction reported early in the disease course by means of VBM and DTI (paper III and IV). Finally in paper V the functional correlates of recognition memory were assessed using fMRI.The main conclusions of this thesis, derived from the five studies, can be summarized as follows:1) The pattern of atrophy in non-demented PD patients affects the anterior region of the hippocampus and progresses to the posterior part in demented patients but preserves the middle part of this structure. This pattern and evolution is similar to that seen in Mild Cognitive Impairment of amnestic type and Alzheimer's disease.2) Declarative memory dysfunctions in PD depend on the atrophy of the head of the hippocampus. 3) Patients with visual hallucinations present progressive hippocampal atrophy and also show widespread atrophy involving the limbic, paralimbic and neocortical areas in agreement with the evolution towards dementia. In contrast, patients without VH only show gray matter loss in the motor regions and their cognitive functions remain spared.4) Early PD patients present gray matter loss in some limbic regions. We observed gray matter loss in the amygdala and orbitofrontal cortex but not in the anterior cingulate, enthorrinal cortex and accumbes nuclei. These results only partially support the stages proposed by Braak.5) Impairment in decision-making and recognition of facial expressions of emotions occurs at early stages of PD. These neuropsychological deficits are accompanied by degeneration of orbitofrontal cortex (OFC) and amygdala. Bilateral OFC reductions are associated with impaired recognition of emotions, and gray matter volume loss in left lateral OFC is related to decision-making impairment in PD.6) PD patients presented abnormal white matter microstructural changes in several brain regions including cortical and subcortical areas early in the disease course. However, only reduction of fractional anisotropy in the white matter of the central olfactory areas, specifically the white matter adjacent to the gyrus rectus, is associated with olfactory dysfunction in PD patients.7) There is a disruption of functional networks involved in memory and default mode in early PD., Está tesis está compuesta de 5 artículos originales y una revisión con un factor de impacto total de 14.441. A lo largo de los 5 estudios se abordan los correlatos neuroanatómicos y neurofuncionales de las disfunciones de memoria declarativa, emocionales y olfatorias en la enfermedad de Parkinson por medio de resonancia magnética estructural y funcional y una extensiva exploración neuropsicológica. En el primer y segundo estudio se aborda el estudio de la atrofia del hipocampo y su progresión a lo largo del tiempo (estudio transversal y longitudinal) mediante voxel-based morphometry. En el tercer estudio se abordan los correlatos de las disfunciones de toma de decisiones y reconocimineto de emociones en la enfermedad de Parkinson, encontrando una relación entre la atrofia de la corteza orbitofrontal y dichas funciones. En el cuarto estudio utilizamos diffusion tensor imaging para investigar las posible anormalidades microestructurales en el sistema central olfatorio y su relación con la identificación de olores en las fases inciales del Parkinson. Finalmente en el quinto estudio investigamos la conectividad funcional de areas cerebrales implicadas en la realización de una tarea de memoria, las cuales descubrimos que están alteradas desde las fases iniciales de la enfermedad.

La literatura científica demuestra diferentes anomalías cerebrales estructurales y funcionales en la esquizofrenia. El propósito de la presente tesis ha sido investigar las anomalías en la estructura, la función y la conectividad delhipocampo en relación con el deterioro de memoria declarativa de los pacientes con esquizofrenia. En el primer trabajo utilizamos dos técnicas cuantitativas complementarias: la "Voxel-based morphometry" (VBM) y la estereología para investigar diferencias regionales en la sustancia gris del hipocampo. Lamuestra estaba compuesta por 28 pacientes esquizofrénicos comparados con 33 sujetos sanos. Ambas técnicas demostraron una reducción bilateral en la densidad posterior del hipocampo, con una predominancia en la regiónposterior izquierda en el análisis estereológico. En el análisis de las correlaciones observamos una asociación significativa entre la variable de olvido y la sustancia gris del hipocampo anterior. Los resultados del primer estudio sustentan la hipótesis de una atrofia regional del hipocampo de los pacientes esquizofrénicos que podría explicar en parte los déficits de memoriafrecuentemente presentes en esta enfermedad.En el segundo trabajo se describe, por medio del análisis de imágenes de difusión mediante el uso de VBM y de un análisis manual de región de interés, la existencia de un deterioro en la sustancia blanca del lóbulo temporal depacientes con esquizofrenia. La muestra estaba constituida por 25 pacientes con esquizofrenia y 24 controles. Encontramos una reducción en los valores de FA (anisotropia fraccional) y un aumento de los de ADC (coeficiente medio dedifusión) en el cuerpo y los pilares posteriores del fornix. Hallamos además diferencias de género en el grupo de pacientes, presentando las pacientes valores de FA disminuidos en el genu del cuerpo calloso. En este estudio, elrecuerdo demorado correlacionó con los valores de FA del fornix de los pacientes. Los datos proporcionados por este segundo estudio sugieren una interrupción en la conectividad de la sustancia blanca del lóbulo temporal medial, lo que podría contribuir a elucidar el déficit de memoria declarativa de los esquizofrénicos.En el tercer trabajo investigamos la activación cerebral en 22 pacientes con esquizofrenia y 24 controles sanos durante la ejecución de dos tareas de memoria de reconocimiento: de palabras y de caras. Los pacientes mostraron en ambas tareas unos patrones cerebrales corticales y subcorticales difusos de hipoactivación respecto a los controles. En particular, en el análisis de región de interés se halló una hipoactivación bilateral del hipocampo con una preeminencia izquierda en el reconocimiento de las palabras y derecha en el caso de las caras. Los resultados del tercer estudio proveen evidencias de unaparticipación funcional del hipocampo en el déficit de memoria declarativa de laesquizofrenia.En síntesis, nuestros resultados en conjunto evidencian que en la esquizofrenia estarían presentes unas alteraciones estructurales y funcionales del hipocampo especialmente relacionadas con el deterioro de memoria declarativa de estos pacientes.

Cognitive impairment of different degrees and hallucinations are common complications of chronic Parkinson's disease (PD). The pathogenesis, pathophysiology, and relationship between cognitive deficits and presence of hallucinations are not fully understood but both impact severely on overall disability and quality of life of PD patients . Cognitive impairment and dementia are the most frequently reported risk factors for visual hallucinations (VH) in PD. Similarly, most studies have found a greater prevalence of psychosis in demented versus non-demented patients with PD. However, although the relationship between VH and cognitive dysfunction is well established, it seems that the emergence of visual hallucinations is not caused directly by the presence of general cognitive impairment because not all PD patients with dementia experienced VH and association studies reported several risk factors for the development of this symptom supporting a multicausal nature of this phenomenon. The data presented in this dissertation intend to give some clues towards a better understanding of the cerebral basis and neuropsychological deficits associated with dementia and visual hallucinations in Parkinson's disease patients. The general aims of this thesis were:1)To study in vivo structural brain changes associated with dementia and visual hallucinations in Parkinson's disease patients. 2)To cognitively characterize a group of non-demented PD patients with VH in a cross-sectional and longitudinal study.For this purpose, we carried out five studies examining cognitive functions and structural brain characteristics in PD patients using neuropsychological and MRI methods. The first and the second investigations evaluated a sample of PD patients with and without dementia. The three last studies were carried out with non-demented PD patients with and without visual hallucinations. The results showed that the the pattern of cerebral atrophy found in demented PD and PD patients with VH extends well beyond the frontostriatal circuits traditionally described to be reponsible for cognitive symptoms in PD. We found grey matter atrophic changes affecting temporal and occipital regions in demented PD patients and in nondemented hallucinating PD patients. These findings stress the importance of the posterior cortical structures in the presence of cognitive impairment and VH in PD patients. We also observed a close relationship between the presence of hallucinations and progressive neuropsychological impairment. The cognitive impairments in hallucinating PD patients were observed in complex visual functions (visuoperceptive-visuospatial skills and visual memory) as well as semantic processing (such as interpretation of perceptual information). This neuropsychological pattern is in agreement with the atrophic changes found in our MRI study. The gray matter volume reductions involved not only secondary visual association areas but also tertiary areas implicated in the integration of semantic information., La enfermedad de Parkinson (EP) es un trastorno neurodegenerativo que se caracteriza por síntomas motores y no motores. Entre los síntomas no motores, la presencia de deterioro cognitivo y de síntomas psicóticos, concretamente alucinaciones se sitúan entre las complicaciones mas prevalentes. Ambas complicaciones tienen un gran impacto en el grado de incapacidad que sufre el paciente y por ende en su calidad de vida. La presencia de deterioro cognitivo y demencia se han identificado como los factores de riesgo mas frecuentemente asociados con el desarrollo de alucinaciones visuales (AV) en la EP. Asimismo, la mayoría de estudios han encontrado una mayor prevalencia de psicosis en pacientes con EP dementes respecto a los no dementes. Sin embargo, aunque la relación entre AV y disfunción cognitiva esta bien establecida, parece que la emergencia de AV no está causada directamente por la presencia de deterioro cognitivo pues no todos los pacientes con demencia experimentan AV y estudios de asociación apoyan una naturaleza multifactorial del fenómeno. El propósito general de esta tesis era lograr un mejor entendimiento de la base cerebral y de los déficits neuropsicológicos asociados a la presencia de demencia y AV en la EP. Los objetivos generales de los estudios que conformaron esta tesis fueron los siguientes:1) Estudio de los cambios estructurales cerebrales asociados a la presencia de demencia y alucinaciones visuales en la enfermedad de Parkinson.2) Caracterización cognitiva a través de un estudio transversal y de seguimiento a un grupo de pacientes con EP no dementes los cuales presentaban historia de alucinaciones visuales complejas.Para llevar a cabo estos objetivos fueron diseñados cinco estudios los que examinaron funciones cognitivas y características cerebrales estructurales en pacientes con EP utilizando para ello tests neuropsicológicos estandarizados y diferentes técnicas de resonancia magnética estructural. El primer y el segundo estudio evaluaron una muestra de pacientes con EP con y sin demencia. Los tres siguientes estudios fueron llevados a cabo en un grupo de pacientes con EP no dementes los cuales presentaban historia de AV complejas.Los resultados mostraron que el patrón de cambios estructurales cerebrales en pacientes con EP y demencia se extendía más allá de los circuitos fronto-estriatales descritos clásicamente como responsables de los déficits cognitivos de la EP. Fueron hallados cambios atroficos afectando a la corteza cerebral temporal y occipital en los pacientes con EP y demencia y en los pacientes con EP no dementes con historia de AV. Esos hallazgos enfatizan la importancia de las estructuras corticales posteriores en la explicación de la presencia de deterioro cognitivo y alucinaciones visuales en la EP. Además fue observada una estrecha relación entre presencia de alucinaciones visuales y deterioro cognitivo progresivo. El deterioro cognitivo en los pacientes con alucinaciones visuales fue observado en funciones visuales complejas y de procesamiento semántico (interpretación perceptual de la información). Este patrón neuropsicológico estaría de acuerdo con nuestros hallazgos de resonancia magnética estructural en los cuales se evidenciaron cambios cerebrales de tipo atrófico en áreas visuales secundarias y áreas terciarias implicadas éstas últimas en la integración de la información semántica.

D'entre tots els canvis cognititus evidenciables durant l'envelliment les pèrdues de memòria són les més destacades. La dificultat per evocar noms, números de telèfon o recordar a quin lloc es col·loquen els objectes quotidians constitueixen queixes de memòria freqüents a mesura que avança l'edat (Bolla i col. 1991). L'alteració de la memòria a l'edat adulta genera alarma social degut, en part, al coneixement popular que pot tractar-se d'un símptoma inicial de demència (Junqué i Jurado, 1994). Malgrat diversos estudis indiquen que el baix rendiment cognitiu associat a l'envelliment pot ser un bon predictor de demència (Katzman i col., 1989; Fuld i col., 1990; Linn i col., 1995; Locascio i col., 1995; Masur i col., 1994; Jacobs i col., 1995), no sempre l'afectació de la memòria, ja sigui quant a queixes subjectives o objectives mitjançant proves neuropsicològiques, ha d'indicar un preludi de demència, ja que com s'ha comentat, es tracta d'una funció que mostra en part un cert declivi fisiològic amb l'edat. En aquest sentit, la recerca realitzada per a identificar variables biològiques i conductuals que permetin ajudar a diferenciar entre el declivi cognitiu associat a l'envlliment normal, d'una banda, i a les condicions patològiques, d'una altra, és un camp de creixent interès i producció científica.En la present tesi doctoral es pretenen caracteritzar algunes de les variables biològiques (troballes de neuroimatge i factors genètics) que influeixen en el perfil neuropsicològic d'una mostra de subjectes amb alteracions de memòria, no demenciats. La identificació de variables moduladores del rendiment cognitiu en poblacions d'aquestes característiques pot ser fins i tot d'interès clínic, donat que les baixes puntuacions en memòria constitueixen un dels primers indicadors de demència (Katzman i col., 1989; Fuld i col., 1990; Masur i col., 1994; Petersen i col., 1994; Linn i col., 1995; Locascio i col., 1995; Jacobs i col., 1995). En el present treball les persones amb problemes de memòria s'han seleccionat d'acord amb els criteris de l'entitat AMAE (Crook i col., 1986) per ser la més ben establerta i la que ha generat més quantitat de recerca fins a l'actualitat (Crook i col., 1986; Soininen i col., 1994; Koivisto i col., 1995; Hànninen i col., 1995; Parnetti i col., 1996; Hànninen i col., 1997a, 1997b; Helkala i col., 1997; Nielsen i col., 1998; Palumbo i col., 1997; Palumbo i col., 1999). No obstant i degut a que l'ACLL també representa una entitat amb creixent interès científic (Petersen i col., 1995, 1999; Smith i col., 1998; Jack i col., 1999; Daly i col., 2000) s'han comparat algunes de les característiques genètiques i neuropsicològiques entre els subjectes AMAE que també poden rebre un diagnòstic de ACLL i els que no.

[spa] La memoria no es un proceso unitario. Se han definido diferentes subsistemas mnesicos que operan mediante circuitos neuroanatómicos distintos. Tradicionalmente, los estudios de la memoria se han centrado en el aprendizaje intencionado, especificando que el lóbulo temporal y las estructuras diencefálicas son esenciales para este tipo de memoria. Contrariamente, hay pocos estudios sobre el aprendizaje incidental y no han concretado las estructuras anatómicas implicadas en este aprendizaje. Ya que se considera que este último tipo de aprendizaje es un proceso mnésico automático de adquisición de la información, cabe presuponer la posible implicacion del estriado, de forma similar a lo que ocurre con el sistema motor. Se han seleccionado una muestra de 114 sujetos, 70 como pacientes y 44 pertenecen al grupo control de sujetos sanos. La muestra de pacientes está constituida por sujetos afectos de la enfermedad de Alzheimer (N=20), de la enfermedad de Huntington (N=21), pacientes con calcificaciones bilaterales en los ganglios basales (N=18) y pacientes con una lesión vascular unilateral en el estriado (N=11). Se ha diseñado una exploración neuropsicológica que ha evaluado funciones cognitivas generales, visoperceptivas y visoespaciales, aprendizaje intencionado, aprendizaje procedimental y funciones frontales en base a pruebas clínicamente estandarizadas. De forma expresa, se han diseñado tareas para evaluar el aprendizaje incidental del espacio, frecuencia y orden temporal. Nuestros resultados sugieren que el aprendizaje incidental puede estar controlado por circuitos frontoneoestriados bilaterales y puede utilizarse como un indicador del funcionamiento mnésico que permita ayudar a discriminar entre la demencia subcortical y cortical, considerando conjuntamente el funcionamiento de la memoria declarativa y procedimental.

Durant el transcurs de la historia s'han atribuït a l'epilèpsia causes naturals, místiques, o una combinació d'ambdues etiologies. El misticisme que sovint ha envoltat la seva etiologia seria degut a la presentació variada, terrible i temorosa amb la que es manifesta aquesta malaltia. De vegades, aquestes creences supersticioses han obstaculitzat, i fins i tot aturat, la recerca de la causa i tractament de l'epilèpsia. No obstant això, es poden trobar descripcions naturalístiques del trastorn datades l'any 400 abans de Crist, quan Hipócrates va argumentar que les estranyes sensacions, emocions i conductes asociadse a l'epilèpsia eren de fet símptomes d'un trastorn cerebral. Desafortunadament, la creença dels autors hipocràtics ha estat contínuament desestimada i enfosquida per les creences supersticioses. Fins i tot avui, amb els grans avenços en la comprensió de l'epilèpsia, algunes persones en mantenen els orígens místics. No és sorprenent, doncs, que el tractament de les persones amb epilèpsia hagi fluctuat també entre cures naturalistes (neuròlegs i psiquiatres) i espirituals (mags i religiosos). Amb la millor comprensió de l'origen i neuropatologia de l'epilèpsia, a partir del 1950, molta recerca s'ha centrat en les conseqüències cognitives, de personalitat i psicològiques de l'epilèpsia. Com a resultat, els psicòlegs en general, i els neuropsicòlegs en particular, han jugat un paper cada vegada més important en el seu estudi, avaluació i tractament. Amb la utilització de tests neuropsicològics sensibles, els neuropsicòlegs han identificat alteracions cognitives interictals en l'atenció, memòria, llenguatge, raonament i habilitats psicomotrius. Els estudis neuropsicològics de l'epilèpsia han contribuït no únicament a la comprensió de la seva neuropatologia sinó també al coneixement del funcionament cognitiu cerebral. Cal fer esment que les investigacions neuropsicològiques relacionades amb l'epilèpsia i el seu tractament quirúrgic i farmacològic, han permés aprofundir en el coneixement de la localització i funcionament de procesos cognitius tant importants per l'home com són la memòria i el llenguatge, i en la comprensió del funcionament de diferentes àrees cerebrals com els lòbuls frontals i temporals. Malgrat això, encara falta recórrer un llarg camí. Esperem que estudis com el present i d'altres posteriors ajudin a avançar en el coneixement de la naturalesa de l'epilèpsia, i en la identificació i tractament de les seves conseqüències neuropsicològiques, doncs aquesta és una de les millors maneres de comprendre els mecanismes amb els que el cervell regula la nostra conducta.
