Improved long-term disease prognosis has led to the otolaryngologist sometimes seeing a remote history of congenital esophageal atresia repair. We report a case of esophageal foreign body impaction with post congenital esophageal atresia repair over 10 years after the fact. A 13-year-old boy reported a globus feeling—a ‘lump in the throat’—and dysphagia after eating fried chicken. Despite diagnostic difficulty, we identified and removed a chicken bone through esophagogastroduodenoscopy 2 days later without complications. The chicken bone was 98 mm long. This impaction after such repair is due both to anastomotic stricture and to esophageal dysmotility. Such impaction is usually accidental, and its recurrence should be prevented by working with the subject and caretakers.

The original NRTL model is modified for the correlation of ternary liquid-liquid equilibria. The ternary expression of the modified NRTL model includes three additional ternary parameters and the ternary terms vanish when a ternary system degenerates to a binary. The ability of the modified NRTL model has been evaluated in the calculations of ternary vapor-liquid-liquid equilibria and quaternary liquid-liquid equilibria.

Extracts containing gymnemic acids, which were extracted from the leaves of Gymnema sylvestre (GS) as nine fractions, were evaluated for their effects on a high K(+)-induced contraction of guinea-pig ileal longitudinal muscles, on glucose transport mediated by the difference of glucose-evoked transmural potential difference (delta PD) in the inverted intestine of guinea-pig and rat, and on blood glucose in rat. Among nine fractions obtained by high performance liquid chromatography from the extract, f-2 and f-4 strongly suppressed the high K(+)-induced contraction of the ileal muscle, f-3 and f-5 did so moderately, and f-8 and f-9 did so weakly, whereas the other fractions did not affect it. The degree of suppression of high K(+)-induced contraction by f-2 at 74% was almost the same as that of f-4 at 67%, at concentrations of 0.1 mg/ml. The suppressed contraction by f-2 or f-4 was recovered by adding 5.5 mM pyruvate. The delta PD increased by 5.5 mM glucose in the inverted intestines of guinea-pig and rat were equally suppressed by 0.1 mg/ml of f-2 or f-4 to 40%. In a rat sucrose tolerance test, f-2 and f-4 suppressed the elevation of blood glucose level. Both f-2 and f-4 suppressed the contraction of guinea-pig ileal longitudinal muscle, interfered with the increase in delta PD induced by glucose in the inverted intestines of guinea-pig and rat, and inhibited the elevation of blood glucose level. In conclusion, it is suggested that some of the extracts containing gymnemic acids from GS leaves suppress the elevation of blood glucose level by inhibiting glucose uptake in the intestine.

ラッカー用の硝酸繊維素の酢酸エステル溶液に種々の可塑剤を加えたラッカーで, 細かいガラス粉末を塗布して不透明としたガラス板上に1μ 以下の薄膜をつけた。比較的高粘度の硝酸繊維素とポリエチレングリコール-ジ-2-エチルへキソエート(P.E.G.H.と略す)を約0.03mol/l含むラッカーを用いて薄膜を作ると,光沢ある斑点が薄膜上に数多くあらわれた。リン酸エステルあるいはフタル酸エステル系可塑剤を用いたときは,この斑点はほとんどあらわれなかった。この現象を確かめるために種々の実験をおこなった。まず硝酸繊維素の酢酸アミル溶液に上記の可塑剤を添加し,可塑剤量と溶液の粘度変化の関係を測定した。可塑剤の粘度は,溶液粘度にくらべて相当低いにもかかわらずP.E.G.H.を約0.002mol/l添加すると粘度は約0.5%増加した。また水面上にラッカーを滴下して形成した薄膜が,重錘によって破断するまでのくぼみを測定した。上記の異常粘度を示した組成のラッカーを用いてつくった薄膜のくぼみは,酢酸アミル溶液単独でつくった薄膜のくぼみより減少した。さらにこのラッカーを1Mc の超音波で処理して, ガラス粉末塗布のガラス板上に薄膜を形成すると, 光沢斑点は消失し,ラッカーを放置してふたたび薄膜をつくると光沢斑点は再現した。このときラッカーの粘度を測定すると超音波処理によって0.7%程度低下し,放置すると復元した。この斑点現象は重合度にも関係し,低粘度硝酸繊維素では認められないか,または非常に細かくなる。以上の諸現象は硝酸繊維素分子の集合体あるいはミセルと可塑剤の相互作用によって,ミクロの凝集体がラッカー中に生じた結果によるものと推論した。赤外吸収曲線からも,硝酸繊維素と可塑剤が相互作用を及ぼしていることが認められた。また金属反射顕微鏡を用いて水面上でのラッカーによる薄膜形成の様相を追跡し,斑点現象を一つのモデルを仮定することによって説明した。

ニトロセルロース薄膜にあらわれる不均一な光沢斑点に及ぼす可塑剤の溶解力の影響を調べた。第1にニトロセルロース-酢酸イソアミル-可塑剤3成分系のn-ヘキサンによる濁りの析出点を測定し,このn-ヘキサン量からMooreの方式により,混液のニトロセルロースに対する溶解パラメーターδmを計算し,可塑剤の溶解力について考察した。第2に数種の可塑剤についてn-ヘキサン希釈価と,Hugginsの定数k'および極限粘度[η]との関係を検討した。第3に可塑剤を含むニトロセルロースフィルムのヤング率を測定し,溶解力のよい可塑剤を含むフィルムは溶解力の悪い可塑剤を含むフィルムよりやわらかく,フィルムのかたさが光沢斑点と関係のあることを確かめた。以上の実験結果から,可塑剤がニトロセルロースに吸着されること,および可塑剤の溶解力が低いほど薄膜が形成される過程で,光沢斑点を生じやすいことを推論した。
