15. Machine Learning-Based COVID-19 Patients Triage Algorithm using Patient-Generated Health Data from Nationwide Multicenter Database

Abstract

A prompt severity assessment model of patients confirmed for having infectious diseases could enable efficient diagnosis while alleviating burden on the medical system. This study aims to develop a SARS-CoV-2 severity assessment model and establish a medical system that allows patients to check the severity of their cases and informs them to visit the appropriate clinic center based on past treatment data of other patients with similar severity levels. This paper provides the development processes of a severity assessment model using machine learning techniques and its application on SARS-CoV-2 patients. The proposed model is trained on a nationwide dataset provided by a Korean government agency and only requires patients' basic personal data, allowing them to judge the severity of their own cases. After modeling, the boosting-based decision tree model was selected as the classifier while mortality rate was interpreted as the probability score. The dataset was collected from all Korean citizens who were confirmed with COVID-19 between February, 2020 and July, 2021. The experiments achieved high model performance with an approximate precision of $0{\cdot}923$ and AUROC score of $0{\cdot}950$ [$95$% Tolerance Interval $0{\cdot}940$-$0{\cdot}958$, $95$% Confidence Interval $0{\cdot}949$-$0{\cdot}950$]. Moreover, our experiments identified the most important variables affecting the severity in the model via sensitivity analysis. The prompt severity assessment model for managing infectious people has been attained through using a nationwide dataset. It has demonstrated its superior performance by surpassing that of conventional risk assessments. Abstract

Abstract Background The hTERT (human telomerase reverse transcriptase) gene contains five variable number tandem repeats (VNTR) and previous studies have described polymorphisms for hTERT-VNTR2-2nd. We investigated how allelic variation in hTERT-VNTR2-2nd may affect susceptibility to prostate cancer. Methods A case-control study was performed using DNA from 421 cancer-free male controls and 329 patients with prostate cancer. In addition, to determine whether the VNTR polymorphisms have a functional consequence, we examined the transcriptional levels of a reporter gene linked to these VNTRs and driven by the hTERT promoter in cell lines. Results Three new rare alleles were detected from this study, two of which were identified only in cancer subjects. A statistically significant association between rare hTERT-VNTR2-2nd alleles and risk of prostate cancer was observed [OR, 5.17; 95% confidence interval (CI), 1.09-24.43; P = 0.021]. Furthermore, the results indicated that these VNTRs inserted in the enhancer region could influence the expression of hTERT in prostate cancer cell lines. Conclusions This is the first study to report that rare hTERT VNTRs are associated with prostate cancer predisposition and that the VNTRs can induce enhanced levels of hTERT promoter activity in prostate cancer cell lines. Thus, the hTERT-VNTR2-2nd locus may function as a modifier of prostate cancer risk by affecting gene expression.

29. A Korean version of the Oral Impacts on Daily Performances (OIDP) scale in elderly populations: Validity, reliability and prevalence

Abstract

Abstract Background This study aimed to develop a Korean version of the OIDP index for elderly people and to assess the levels of sociodental impacts in an older Korean population. Methods The OIDP index for elderly people was cross-culturally adapted from English into Korean and then the derived instrument was tested for reliability and validity. The study population was elderly (65+ year-old) residents of Gangneung City, South Korea. Twenty two of the 222 senior day centres were randomly selected. Results 687 people were invited and 668 participated in the study (response rate: 97.2%). The standardized Cronbach's alpha coefficient was 0.85. The OIDP related significantly with different subjective measures of oral and general health (p < 0.001). 62.9% of the people had oral impacts relating to one or more performances, with eating food being the most frequently affected performance (47.6%). More than 70% of people with oral impacts had up to 3 performances affected by oral health conditions. Conclusion The Korean OIDP index showed satisfactory validity and internal consistency reliability, confirming its appropriateness for use among older Korean people. The prevalence of oral health related impacts was high. Future studies should focus on the test-retest reliability and the sensitivity to change of the Korean OIDP.

39. Nanobubble‐Based Acoustical Bursting of Nuclear Membrane for Interleukin‐33 Release Induces Eosinophil‐Mediated Cancer Immunotherapy.

Abstract

Despite noteworthy advances in cancer immunotherapy, the understanding of effective strategies to harness eosinophils in anti‐cancer immunity, which is critical for cytotoxic T‐cell recruitment, remains limited. In particular, interleukin‐33, which is essential for eosinophil activation, is chromatin‐bound and lacks efficient extracellular release mechanisms for its intact form. Here, a nanobubble‐based acoustical cancer cell‐bursting strategy (NanoBurst) is reported to elicit eosinophil‐mediated cancer immunity by releasing damage‐associated molecular patterns (DAMPs). NanoBurst consists of polymeric nanoparticles encapsulating a gas precursor, designed to effectively disrupt cancer cell membranes upon exposure to ultrasound. This process triggers the efficient release of both nucleus‐derived (e.g., high‐mobility group nucleosome‐binding domain‐1, gasdermin E, interleukin‐33) and cytoplasm‐derived DAMPs, initiating the eosinophil‐mediated immune cascade. Consequently, NanoBurst induces direct tumoricidal effects by activating eosinophils and enhances anti‐cancer immunity by recruiting NK cells and cytotoxic T‐cells in vivo. When combined with a dipeptidyl peptidase‐4 (DPP4) inhibitor, an inducer of eosinophil‐mediated chemotaxis, NanoBurst effectively inhibits the growth of primary tumors and the metastasis to the lung in a CT26 tumor‐bearing mouse model. Notably, boosting eosinophil‐mediated anti‐cancer immunity with NanoBurst‐DPP4 inhibitor shows superior efficacy for tumor suppression compared to immune checkpoint inhibitors.
