The application of lean thinking in service industry required high knowledge and experience of lean concepts to know what needs to be done and is complementary to strategic decision making (Maleyeff, Management Decision, 2006). However, the similarity of service industry and manufacturing are very small which can result in difficult to implement lean in specific organization. One of the difficulties is deciding which of the many lean tools to apply and when to apply them. Lean implementation is a transformational process and needs to support organizational development alongside process improvement (Jones D. a., 2000). This research presents a comprehensive lean approach, based on Value Stream Management, which make the possibilities to enhance the performance of a service, by determining and tackling its criticalities. The study is motivated by the growing need to extend lean concepts to the service industry and by a minimum availability of operating tools to support technical staff in this effort. To validate the approach, an application concerning UUM Parcel Centre submitting parcel process is presented. Result demonstrate the quality of the approach and confirm the significant improvements that can be obtained with the application of lean thinking in the service context (Liker, 2000). With the success of application of the VSM in UUM Parcel Centre process, three processes in the current state map had been combined into one single process using digital computing. This result in reducing 12 minutes in total lead time which is 8.7 percentage in the future state map. Hence, UUM Parcel Centre can be more advance and efficient with implementing elements of Industry 4.0 technologies.

The fear of suffering dementia may lead to people signing an Advance Euthanasia Directive to make provision for health care decisions in the event that he/she becomes unable to make those decisions. However, Advance Euthanasia Directives are rarely adhered to in the case of dementia because the symptoms of dementia conflict with the due care criteria; a person requesting euthanasia must be able to confirm this request at time of death and must be undergoing unbearable and hopeless suffering. Once dementia has progressed, the euthanasia ‘wish’ can no longer be confirmed, and assessing suffering in a person with dementia is nearly impossible. This means that for a euthanasia request to be successful you have to perform the euthanasia early enough, while the patient is still cognitively competent. The risk in doing so is that the patient may lose years of their life that could have been full of quality. Postponing euthanasia in dementia could result in euthanasia not being possible and the person with dementia living a life that they did not want. This paper addresses how to decide what ‘on time’ is when it comes to dying with dementia through literature review, information visualisation and public debate.

In this study, the authors report the findings from a study of the contributory factors leading to slips, trips and falls (STFs) amongst elderly passengers at train stations and how these are likely to change in the future over the medium to long term (the period 2035–2050). Their data draws on: stakeholder interviews with rail personnel and elderly passengers; a set of station observations carried out across the UK; and, a survey of the views of station managers. The findings point to a set of 22 contributory factors covering aspects of organisational, station environment and passenger (individual) influence on STFs. Amongst the factors which most concern station managers at the present and over the next few decades are: rushing behaviour on train platforms; the consumption of alcohol by passengers; aspects of station design (e.g. flooring); and, training for station staff as regard the risks of STFs. The authors summarise their findings in the form of a systems model which highlights priorities with regard to STFs in terms of all of the stakeholders taking part in the study. A final section discusses a set of issues which might form the basis for a future agenda for research and practice in this area.
