Background Early antiretroviral therapy (ART) restricts the size of the human immunodeficiency virus (HIV) reservoir in infants. However, whether antiretroviral (ARV) prophylaxis given to exposed vertically infected children exerts similar effects remains unknown. Methods We measured total and integrated HIV DNA, as well as the frequency of CD4 T cells producing multiply spliced RNA (msRNA) after stimulation (inducible reservoir) in vertically infected Thai infants. Eighty-five infants were followed longitudinally for up to 3 years. We compared the size of the reservoir in children who received continuous ARV prophylaxis since birth vs those who never received or discontinued prophylaxis before initiating ART. We used samples from a cross-sectional cohort of 37 Thai children who had initiated ART within 6 months of life to validate our findings. Results Before ART, levels of HIV DNA and the frequencies of cells producing msRNA were significantly lower in infants who received continuous ARV prophylaxis since birth compared to those in whom ARV prophylaxis was discontinued or never initiated (P < .020 and P < .001, respectively). Upon ART initiation, total and integrated HIV DNA levels decayed significantly in both groups (P Conclusions Neonatal ARV prophylaxis with direct transition to ART durably limits the size of the HIV reservoir.

IntroductionOnce-weekly subcutaneous semaglutide, a glucagon-like peptide-1 analog, is approved in the USA as an adjunct to diet and exercise for adults with inadequately controlled type 2 diabetes (T2D) to improve glycemic control and reduce the risk of major adverse cardiovascular events in people with T2D and established cardiovascular disease. The Semaglutide Unabated Sustainability in Treatment of Type 2 Diabetes (SUSTAIN) phase III clinical trial program demonstrated the efficacy and safety of once-weekly subcutaneous semaglutide; however, determining its effectiveness in a real-world setting could support decision-making by clinicians, payers and policy makers in routine clinical practice.Research design and methodsSEmaglutide PRAgmatic (SEPRA) is an ongoing open-label, randomized, pragmatic clinical trial designed to compare the effects of once-weekly subcutaneous semaglutide versus standard of care in US health-insured adults with T2D and physician-determined inadequate glycemic control. The primary end point is the proportion of participants achieving glycated hemoglobin (HbA1c) ResultsBetween July 2018 and March 2021, 1278 participants were enrolled from 138 study sites across the USA. At baseline, 54% were male with mean±SD age 57.4±11.1 years and body mass index 35.7±8.0 kg/m2. Mean diabetes duration was 7.4±6.0 years and mean HbA1c was 8.5±1.6%. At baseline, concomitant antidiabetes medications included metformin, sulfonylureas, sodium-glucose co-transporter-2 inhibitors, and dipeptidyl peptidase-4 inhibitors. The majority of participants had hypertension and dyslipidemia. The trial design was self-assessed using the PRagmatic Explanatory Continuum Indicator Summary-2 tool by the study steering group and was scored 4–5 in all domains suggesting a highly pragmatic study.ConclusionsSEPRA, a highly pragmatic ongoing study, will provide data on the effects of once-weekly subcutaneous semaglutide in a real-world setting when used during routine management of T2D.Trial registration numberNCT03596450.Trial registration number

The success of the shock and kill strategy for the HIV cure depends both on the reactivation of the latent reservoir and on the ability of the immune system to eliminate infected cells. As latency reversal alone has not shown any impact in the size of the latent reservoir, ensuring that effector CTLs are able to recognize and kill HIV-infected cells could contribute to reservoir reduction. In this study, we investigated which functional aspects of human CTLs are associated with a better capacity to kill HIV-infected CD4+ T cells. We isolated Gag- and Nef-specific CTL clones with different TCR sequences from the PBMC of donors in acute and chronic infection. High-affinity clonotypes that showed IFN-γ production preserved even when the CD8 coreceptor was blocked, and clones with high Ag sensitivity exhibited higher efficiency at reducing the latent reservoir. Although intrinsic cytotoxic capacity did not differ according to TCR affinity, clonotypes with high TCR affinity showed a better ability to kill HIV-infected CD4+ T cells obtained from in vivo–infected PBMC and subjected to viral reactivation. Strategies aiming to specifically boost and maintain long-living memory CTLs with high TCR affinity in vivo prior to latency-reversing treatment might improve the efficacy of the shock and kill approach to reduce the latent reservoir.

Early initiation of antiretroviral therapy (ART) in acute HIV infection (AHI) is effective in limiting seeding of the HIV viral reservoir, but little is known about how the resultant decreased antigen load affects long-term antibody development after ART. We report here that Env-specific plasma antibody levels and antibody-dependent cellular cytotoxicity (ADCC) increased during the first 24 weeks of ART and correlated with antibody levels persisting after 48 weeks of ART. Participants treated in AHI stage 1 had lower Env-specific antibodies levels and ADCC activity on ART than those treated later. Importantly, participants who initiated ART after peak viremia in AHI developed elevated cross-clade ADCC responses detectable one year after ART initiation even though clinically undetectable viremia was reached by 24 weeks. These data suggest that there is more germinal center activity in the later stages of AHI and that antibody development continues in the absence of detectable viremia during the first year of suppressive ART. Development of therapeutic interventions that can enhance earlier development of germinal centers in AHI and antibodies after ART initiation could provide important protection against the viral reservoir that is seeded in early treated individuals.

The goal of this case series report is to demonstrate the predictability of providing immediate restorations with an implant specifically designed to achieve high primary stability, and to report on 12-month survival outcomes. Twenty-nine implants were placed in eight patients with various tooth-replacement needs, from single teeth to partially edentulous spans to fully edentulous mandibles. A new tapered-implant system (The Marc Nevins, Little Implant) was utilized that incorporates a self-tapping thread design with a thread pitch and thread lead to improve primary stability at insertion. All implants were placed with a minimum insertion torque of 45 Ncm. Four immediate implants and two early placements at extraction sites were included in the cohort. Twenty-seven implants were clinically successful at 1 year. Two implants in a healed ridge were found to have failed at 6 weeks postoperative in one patient wearing an ill-fitting removable partial denture. The radiographic crestal marginal bone levels were recorded in millimeters from the top of the implant platform to the first bone-to-implant contact. The mean crestal marginal bone level change was -0.57 ± 0.82 mm. This case series report demonstrates the ability of an implant designed for high primary stability to achieve successful integration in the presence of an immediate restoration for various clinical presentations, including single-tooth, multi-unit, and edentulous cases.

The expansion of immigration enforcement in the United States has increased the documentation requirements to which immigrants are subjected. A case in point is birth certificates, which are used to establish identity, nationality, age, and kin relationships in myriad US immigration cases. This development gives highly localized bureaucratic practices in immigrants’ countries of origin transnational implications. Based on fieldwork in a registry of vital records in El Salvador, interviews with Salvadoran officials, and legal work with immigrants in the United States, this article analyzes birth certificates’ use as immigration documents, focusing on the understandings of legality and authenticity that underpin their circulation. This analysis contributes to theorizing citizenship by detailing the ways that immigration enforcement practices in immigrants’ country of residence can make their relationship to their country of origin both more important (in that they need identity documents) and less accessible (due to distance).

Background: Little is known about the burden of atopic dermatitis (AD) encountered in US primary care practices and the frequency and type of skin care practices routinely used in children. Objective: To estimate the prevalence of AD in children 0 to 5 years attending primary care practices in the United States and to describe routine skin care practices used in this population. Design: A cross-sectional survey study of a convenience sample of children under the age of 5 attending primary care practices for any reason. Setting: Ten primary care practices in 5 US states. Results: Among 652 children attending primary care practices, the estimated prevalence of ever having AD was 24% (95% CI, 21–28) ranging from 15% among those under the age of 1 to 38% among those aged 4 to 5 years. The prevalence of comorbid asthma was higher among AD participants compared to those with no AD, namely, 12% and 4%, respectively (P Conclusions: Our study found a large burden of AD in the primary care practice setting in the US. The majority of households reported skin care practices that may be detrimental to the skin barrier, such as frequent bathing and the routine use of moisturizers with high water: oil ratios. Clinical trials are needed to identify which skin care practices are optimal for reducing the significant burden of AD in the community.

Situated within the Irving K. Barber Learning Centre at the University of British Columbia (UBC) in Vancouver, Canada, the Chapman Learning Commons (CLC) has been offering academic transition and learning support programs targeted to first year students since 2002. A recent addition to our suite of services is the Profs-in-Commons program which invites faculty members to conduct their office hours and host events in the CLC. The program has been an important initiative for the Learning Commons and the UBC campus community because it encourages student–faculty interaction outside of the classroom; it increases student’s attendance in course-based office hours – hosted by faculty members and it leverages the status of libraries as neutral, collaborative, and community-oriented learning spaces. The program is grounded in student engagement research consistently showing that students’ transition to university is greatly enhanced when they foster academic connections with faculty members. Profs-in-Commons also responds to research into best practices for how to support student transition to university academic environments. This chapter will elaborate on the theoretical foundations of the Profs-in-Commons program, share how the UBC-Vancouver Profs-in-Commons program was initiated and is sustained, and discuss the program’s benefits and challenges.

Plasmacytoid dendritic cells (pDCs) are robust producers of IFNα and one of the first immune cells to respond to SIV infection. To elucidate responses to early HIV-1 replication, we studied blood pDCs in 29 HIV-infected participants who initiated antiretroviral therapy during acute infection and underwent analytic treatment interruption (ATI). We observed an increased frequency of partially activated pDCs in the blood before detection of HIV RNA. Concurrent with peak pDC frequency, we detected a transient decline in the ability of pDCs to produce IFNα in vitro, which correlated with decreased phosphorylation of IFN regulatory factory 7 (IRF7) and NF-κB. The levels of phosphorylated IRF7 and NF-κB inversely correlated with plasma IFNα2 levels, implying that pDCs were refractory to in vitro stimulation after IFNα production in vivo. After ATI, decreased expression of IFN genes in pDCs inversely correlated with the time to viral detection, suggesting that pDC IFN loss is part of an effective early immune response. These data from a limited cohort provide a critical first step in understanding the earliest immune response to HIV-1 and suggest that changes in blood pDC frequency and function can be used as an indicator of viral replication before detectable plasma viremia.

BackgroundAtopic dermatitis (AD) is a common, chronic skin disorder often beginning in infancy. Skin barrier dysfunction early in life serves as a central event in the pathogenesis of AD. In infants at high risk of developing AD, preventative application of lipid-rich emollients may reduce the risk of developing AD. This study aims to measure the effectiveness of this intervention in a population not selected for risk via a pragmatic, randomized, physician-blinded trial in the primary care setting.MethodsInfant–parent dyads are recruited from a primary care practice participating through one of four practice-based research networks in Oregon, Colorado, Wisconsin, and North Carolina. Eligible dyads are randomized to the intervention (daily use of lipid-rich emollient) or the control (no emollient) group (n = 625 infants in each) and are followed for 24 months. The primary outcome is the cumulative incidence of physician-diagnosed AD and secondary outcomes include caregiver-reported measures of AD and development of other atopic diseases. Data collection occurs via chart review and surveys, with no study visits required. Data will be analyzed utilizing intention-to-treat principles.DiscussionAD is a common skin condition in infants that affects quality of life and is associated with the development of other atopic diseases. If a safe intervention, such as application of lipid-rich emollients, in the general population effectively decreases AD prevalence, this could alter the guidance given by providers regarding routine skin care of infants. Because of the pragmatic design, we anticipate that this trial will yield generalizable results.Trial registrationClinicalTrials.gov:NCT03409367. Registered on 11 February 2018.

Permanently shadowed regions (PSRs) at the poles of the Moon are potential reservoirs of trapped volatile species, including water ice. Knowledge of the distribution and abundance of water ice at the poles provides key scientific background for understanding the evolution of volatiles in the Earth-Moon system and for human exploration efforts. The Lunar Reconnaissance Orbiter Camera (LROC) acquired images of the terrain within PSRs to search for indications of water ice. In addition, the LRO Miniature Radio-Frequency (Mini-RF) instrument acquired S-band radar observations to further characterize these regions. Specifically, the m-chi decomposition was used to assess the distribution of materials within and around PSRs based on the type of backscatter. Double-bounce backscatter is indicative of water ice, but could also be produced by randomly distributed blocks at the wavelength scale. To ascertain whether these signatures are due to water ice or blocks, we quantified the abundance of detectable blocks in areas with double-bounce backscatter using the LROC Narrow Angle Camera (NAC). Block populations were measured for a suite of craters with different ages, sizes, and radar characteristics. For fresh craters, a correlation between block size, block density and double-bounce backscatter was found. Within PSRs exhibiting double-bounce backscatter, no blocks were found. Additionally, no albedo variations were observed at PSRs, in contrast to observations of PSRs on Mercury. While the possibility of water ice in some lunar craters still exists, these results indicate that they are likely small-scale, and that the observed radar anomalies at PSR-bearing craters are most likely due to the presence of wavelength-scale blocks.

The current opioid crisis in the United States is a major problem facing health-care providers, even at the end of life. Opioids continue to be the mainstay treatment for pain at the end of life, with the prevalence of pain reported in up to 80% of patients and tends to increase as one gets closer toward the end of life. In the past year, 20.2 million Americans had a substance use disorder (SUD) and SUDs are disabling disorders that largely go untreated. In addition, the coexistence of both a mental health and SUD is very common with the use of opioids often as a means of chemical coping. Most hospice programs do not have standardized SUD policies/guidelines in place despite the increasing concerns about substance abuse within the United States. The goal of this article is to review the literature on this topic and offer strategies on how to manage pain in patients who have active SUD or who are at risk for developing SUD in those dying on hospice.

Objective and Design: A randomized, open-label pilot study in individuals treated with antiretroviral therapy (ART) since acute HIV infection (AHI) with a regimen including a histone deacetylase inhibitor to induce HIV from latency and control HIV replication during subsequent treatment interruption (TI). Methods: Fifteen participants who initiated ART at AHI were randomized to vorinostat/hydroxychloroquine/maraviroc (VHM) plus ART (n ​= ​10) or ART alone (n ​= ​5). The VHM arm received three 14-day vorinostat cycles within 10 weeks before TI. ART was resumed for plasma viral load (VL) ​> ​1,000 HIV RNA copies/mL. BACKGROUND: Staphylococcus aureus bacteraemia is a common cause of severe community-acquired and hospital-acquired infection worldwide. We tested the hypothesis that adjunctive rifampicin would reduce bacteriologically confirmed treatment failure or disease recurrence, or death, by enhancing early S aureus killing, sterilising infected foci and blood faster, and reducing risks of dissemination and metastatic infection.METHODS: In this multicentre, randomised, double-blind, placebo-controlled trial, adults (≥18 years) with S aureus bacteraemia who had received ≤96 h of active antibiotic therapy were recruited from 29 UK hospitals. Patients were randomly assigned (1:1) via a computer-generated sequential randomisation list to receive 2 weeks of adjunctive rifampicin (600 mg or 900 mg per day according to weight, oral or intravenous) versus identical placebo, together with standard antibiotic therapy. Randomisation was stratified by centre. Patients, investigators, and those caring for the patients were masked to group allocation. The primary outcome was time to bacteriologically confirmed treatment failure or disease recurrence, or death (all-cause), from randomisation to 12 weeks, adjudicated by an independent review committee masked to the treatment. Analysis was intention to treat. This trial was registered, number ISRCTN37666216, and is closed to new participants.FINDINGS: Between Dec 10, 2012, and Oct 25, 2016, 758 eligible participants were randomly assigned: 370 to rifampicin and 388 to placebo. 485 (64%) participants had community-acquired S aureus infections, and 132 (17%) had nosocomial S aureus infections. 47 (6%) had meticillin-resistant infections. 301 (40%) participants had an initial deep infection focus. Standard antibiotics were given for 29 (IQR 18-45) days; 619 (82%) participants received flucloxacillin. By week 12, 62 (17%) of participants who received rifampicin versus 71 (18%) who received placebo experienced treatment failure or disease recurrence, or died (absolute risk difference -1·4%, 95% CI -7·0 to 4·3; hazard ratio 0·96, 0·68-1·35, p=0·81). From randomisation to 12 weeks, no evidence of differences in serious (p=0·17) or grade 3-4 (p=0·36) adverse events were observed; however, 63 (17%) participants in the rifampicin group versus 39 (10%) in the placebo group had antibiotic or trial drug-modifying adverse events (p=0·004), and 24 (6%) versus six (2%) had drug interactions (p=0·0005).INTERPRETATION: Adjunctive rifampicin provided no overall benefit over standard antibiotic therapy in adults with S aureus bacteraemia.FUNDING: UK National Institute for Health Research Health Technology Assessment.

Background The adoption of electronic health records (EHR) has created an opportunity for multicenter data collection, yet the feasibility and reliability of this methodology is unknown. The aim of this study was to integrate EHR data into a homogeneous central repository specifically addressing the field of adult congenital heart disease (ACHD). Methods Target data variables were proposed and prioritized by consensus of investigators at five target ACHD programs. Database analysts determined which variables were available within their institutions' EHR and stratified their accessibility, and results were compared between centers. Data for patients seen in a single calendar year were extracted to a uniform database and subsequently consolidated. Results From 415 proposed target variables, only 28 were available in discrete formats at all centers. For variables of highest priority, 16/28 (57%) were available at all four sites, but only 11% for those of high priority. Integration was neither simple nor straightforward. Coding schemes in use for congenital heart diagnoses varied and would require additional user input for accurate mapping. There was considerable variability in procedure reporting formats and medication schemes, often with center-specific modifications. Despite the challenges, the final acquisition included limited data on 2161 patients, and allowed for population analysis of race/ethnicity, defect complexity, and body morphometrics. Conclusion Large-scale multicenter automated data acquisition from EHRs is feasible yet challenging. Obstacles stem from variability in data formats, coding schemes, and adoption of non-standard lists within each EHR. The success of large-scale multicenter ACHD research will require institution-specific data integration efforts.

There is currently no validated model explaining the variability of sexual expression. This has created a scenario where sexuality, as a construct, is purely intuitive. Sexuality educators have frequently presented the Circles of Sexuality, a model that contends that sexuality is a combination of intimacy, sensuality, sexual health/behaviors, sexual identity, and sexualization. Adapting photovoice methodology and using the social media site Tumblr, the current analysis used this model to assess the social conceptualization of sexuality. Results indicate that the circles are unequally represented, with intimacy underrepresented and sexual identity overrepresented. Implications for sexuality educators, clinicians, and researchers are discussed.

How do you effectively train and assess student staff in a learning commons environment? How do you foster a student-led approach while maintaining accurate and high-level service? How do you create an environment where student staff are engaged and motivated to succeed? Peer-to-peer service models are fundamental to many learning commons environments and contribute to student success. Many student-delivered services in learning commons compliment programs traditionally offered exclusively by professional staff such as librarians, IT professionals, learning specialists or student affairs personnel. In such service models, students are the front line contact and the need for knowledgeable assistance and accurate referrals remains paramount. This article presents the findings of a study that investigated how training and assessment is approached with student staff in a learning commons environment. Learning commons coordinators and supervisors from across North American shared how they train students (methods and content), approach ongoing professional development of student staff, and how they monitor or assess the overall quality and accuracy of their student service models. The survey results and tangible examples offer insights and strategies for fostering an engaged student team, driven to deliver a high level of service.

This paper explores the Australian public's understanding of volunteering from a media perspective. A content analysis of 137 news articles published in The Australian newspaper revealed that volunteering in Australia is framed in terms of eight perspectives by nine claim makers. Since media-led images of volunteering tend to influence peoples' decision to volunteer, the findings have several implications for organizers of social development projects involving volunteers.

Despite strong anti-smoking efforts, at least 12% of American women cannot quit smoking when pregnant resulting in more than 450,000 smoke-exposed infants born yearly. Smoking during pregnancy is the largest preventable cause of childhood respiratory illness including wheezing and asthma. Recent studies have shown a protective effect of vitamin C supplementation on the lung function of offspring exposed to in utero smoke in a non-human primate model and an initial human trial. Vitamin C to Decrease the Effects of Smoking in Pregnancy on Infant Lung Function (VCSIP) is a randomized, double-blind, placebo-controlled trial to evaluate pulmonary function at 3 months of age in infants delivered to pregnant smokers randomized to 500 mg/day of vitamin C versus placebo during pregnancy. Secondary aims evaluate the incidence of wheezing through 12 months and pulmonary function testing at 12 months of age. Women are randomized between 13–23 weeks gestation from clinical sites in Portland, Oregon at Oregon Health & Science University and PeaceHealth Southwest Medical Center and in Indianapolis, Indiana at Indiana University and Wishard Hospital. Vitamin C supplementation occurs from randomization to delivery. Monthly contact with participants and monitoring of medical records is performed to document medication adherence, changes in smoking and medical history, and adverse events. Pulmonary function testing of offspring occurs at 3 and 12 months of age and incidence of wheezing and respiratory illness through 12 months is captured via at least quarterly questionnaires. Ancillary studies are investigating the impact of vitamin C on placental blood flow and DNA methylation.

Objective: This pilot study aimed to provide supportive evidence for the acceptability and usefulness of the Meaning-Making intervention (MMi) in patients newly diagnosed with Stage III or IV ovarian cancer, and to provide estimates of parameters needed to design a full-scale study. Methods: A randomized controlled trial with 24 patients (12 experimental and 12 control) was conducted. Existential well-being (primary outcome), overall quality of life, distress, anxiety, depression and self-efficacy were measured. Results: Compared to the control group, patients in the experimental group had a better sense of meaning in life at one and three months post-intervention. Conclusion: The MMi seems a promising intervention for advanced cancer patients, and a full randomized controlled trial is warranted to further investigate its efficacy. BackgroundTransferring patients with CHD from paediatric to adult care has been challenging, especially across institutions. Within a single institution, some issues such as provider interaction, information exchange, or administrative directives should not play a significant role, and should favour successful transfer.ObjectiveWe studied patients who were eligible for transfer to the adult congenital heart disease service within our institution in order to identify factors associated with successful transfer to adult care providers versus failure to transfer.MethodsPatients above18 years of age with CHD who were seen by paediatric cardiologists before January, 2008 were identified through a patient-care database. Records were reviewed to determine follow-up between 2008 and 2011 and to determine whether the patient was seen in the adult congenital cardiology clinic, paediatric cardiology clinic, or had no follow-up, and statistical comparisons were made between groups.ResultsAfter reviewing 916 records, 229 patients were considered eligible for transition to adult congenital cardiology. Of these, 77 (34%) were transferred successfully to adult congenital cardiology, 47 (21%) continued to be seen by paediatric cardiologists, and 105 (46%) were lost to follow-up. Those who transferred successfully differed with regard to complexity of diagnosis, insurance, and whether a formal referral was made by a paediatric care provider. Only a small fraction of the patients who were lost to follow-up could be contacted.ConclusionWithin a single institution, with shared information systems, administrations, and care providers, successful transfer from paediatric to adult congenital cardiology was still poor. Efforts for successful retention are just as vital as those for transfer.

Examining the need for an eldercare policy is relevant and timely because the population is aging, a focus of care has shifted from institutional to community based, and informal caregivers, primarily women, are increasingly pressured to be responsible for eldercare. The purpose of the study is to examine the differences in the experiences of low-income and those who are not low-income informal caregivers. Three hundred questionnaires were mailed to past and present home care clients and 58 questionnaires were returned (19% response rate). This research revealed that low-income caregivers have increased needs for support and education from those who can afford to pay for their support services. Low-income caregivers experience significantly greater caregiver distress than do caregivers who are not low income. The unique needs of low-income caregivers must be considered in the formation of Canadian eldercare policy as increased health care privatization promotes the growing inequality in health care provision.

Diagnostic codes used in healthcare administration have been employed extensively in clinical research to identify target patient populations, including demonstration of important clinical outcomes among adults with congenital heart disease. However, little is known about the reliability of code-derived data in this context. We sought to determine the accuracy of International Classification of Disease-9th Revision (ICD-9) diagnoses and the reliability of retrieval algorithms in adults with congenital heart disease (ACHD). Pilot testing of a hierarchical algorithm to identify ACHD patients and determine their principle congenital diagnosis was performed. A revised algorithm was then applied retrospectively to a sample of all outpatients seen by providers who see general cardiology and ACHD patients. Using all ICD-9 codes available from any encounter, accuracy for detection and categorization of sub-types were compared to physician chart review. After initial testing on 334 patients, the revised algorithm was applied to 740 patients. The sensitivity and specificity for ACHD patient identification from this specialty clinic population were 99 and 88 %, respectively. Of 411 (56 %) non-ACHD patients, 49 were incorrectly categorized as ACHD by the algorithm. Of ACHD patients, 326 of 329 were correctly identified by diagnostic codes and categorization of ACHD defect sub-type was correct in 263 (80 %). Administrative data can be used for identification of ACHD patients based on ICD-9 codes with excellent sensitivity and reasonable specificity. Accurate categorization that would be utilized for quality indicators by ACHD defect type is less robust. Additional testing should be done using non-referral populations.

Minority physicians provide care in a manner that promotes patient satisfaction and meets the needs of an increasingly diverse U.S. population. In addition, minority medical school faculty bring diverse perspectives to research and teach cross-cultural care. However, men and women of color remain underrepresented among medical school faculty, particularly in the higher ranks. National data show that although the numbers of women in medicine have increased, minority representation remains essentially static. Studying minority women faculty as a group may help to improve our understanding of barriers to diversification. Six National Centers of Excellence in Women's Health used a variety of approaches in addressing the needs of this group. Recommendations for other academic institutions include development of key diversity indicators with national benchmarks, creation of guidelines for mentoring and faculty development programs, and support for career development opportunities.

Background Extended smoking cessation follow-up after hospital discharge significantly increases abstinence. Hospital smoke-free policies create a period of ‘forced abstinence’ for smokers, thus providing an opportunity to integrate tobacco dependence treatment, and to support post-discharge maintenance of hospital-acquired abstinence. This study is funded by the National Heart, Lung, and Blood Institute (1U01HL1053231). Methods/Design The Inpatient Technology-Supported Assisted Referral study is a multi-center, randomized clinical effectiveness trial being conducted at Kaiser Permanente Northwest (KPNW) and at Oregon Health & Science University (OHSU) hospitals in Portland, Oregon. The study assesses the effectiveness and cost-effectiveness of linking a practical inpatient assisted referral to outpatient cessation services plus interactive voice recognition (AR + IVR) follow-up calls, compared to usual care inpatient counseling (UC). In November 2011, we began recruiting 900 hospital patients age ≥18 years who smoked ≥1 cigarettes in the past 30 days, willing to remain abstinent postdischarge, have a working phone, live within 50 miles of the hospital, speak English, and have no health-related barriers to participation. Each site will randomize 450 patients to AR + IVR or UC using a 2:1 assignment strategy. Participants in the AR + IVR arm will receive a brief inpatient cessation consult plus a referral to available outpatient cessation programs and medications, and four IVR follow-up calls over seven weeks postdischarge. Participants do not have to accept the referral. At KPNW, UC participants will receive brief inpatient counseling and encouragement to self-enroll in available outpatient services. The primary outcome is self-reported thirty-day smoking abstinence at six months postrandomization for AR + IVR participants compared to usual care. Additional outcomes include self-reported and biochemically confirmed seven-day abstinence at six months, self-reported seven-day, thirty-day, and continuous abstinence at twelve months, intervention dose response at six and twelve months for AR + IVR recipients, incremental cost-effectiveness of AR + IVR intervention compared to usual care at six and twelve months, and health-care utilization and expenditures at twelve months for AR + IVR recipients compared to UC. Discussion This study will provide important evidence for the effectiveness and cost-effectiveness of linking hospital-based tobacco treatment specialists’ services with discharge follow-up care. From both ancient biblical times – where the motivation was the “desire for order and continuity” (Crenshaw 1998) – and the period from Aristotle and Socrates onwards, moral formation of children and the shaping of their character have always been regarded as a primary function of both parental nurture and formal education. The central concerns of ancient sages (Mesopotamia, Egypt, Israel) were about knowledge and religion. The aims of education, for Aristotle, were to teach the intellectual and moral virtues necessary for right action, the pursuit of what is noble and thus of happiness as the telos [τeλος] (natural end) of life (Hollinger 2002, pp. 46–49). At the beginning of the 19th century, “nearly everyone was persuaded that religion and morality were inseparable; so inseparable that moral education must be religious education, and that no sense of absolute obligation in conscience could be found apart from religion. That moral philosophers taught the contrary made no difference” (Chadwick 1975, p. 229). Key questions for philosophers, theologians, and educationists have always been: “What is good for men and women?”, “What does it mean to be fully human?”, “Why be moral at all?”, and “Why might one virtue, disposition or action be deemed more valid than another?” Naturalistic or anthropocentric answers to such questions lead to quite different ethical foundations for behaviour, than theistic ones. The former approach is typified by Protagoras’s well-known statement that “Man [sic] is the measure of all things.” Any values resulting from this worldview will draw on the compendium of human wisdom (such as the “Great Books” or “values clarification” approaches), and thus be subject to the relativism of ideological, temporal, political, and cultural forces. The logical end of this anthropocentric approach, however, is nihilism, for if there is nothing transcendent outside of ourselves “in which to ground standards of conduct and morality, then there can be no good and evil”. In the chilling words of Dostoevsky, all things become permissible and totalitarian darkness awaits (Charles 2002, p. 100). Background. Fluoroscopic micturating cystourethrography (MCU) is used for screening and grading of vesicoureteral reflux (VUR). It involves ionizing radiation. This study was designed to assess the efficacy of contrast-enhanced sonography in predicting the presence or absence of VUR. Objective. To compare an ultrasound contrast agent for detection of VUR in at-risk infants, and to compare these findings with fluoroscopic MCU with the aim of determining whether echo-enhanced sonography could be used instead of fluoroscopic MCU to identify neonates who do not have VUR, thus avoiding the use of radiation in this group. Materials and methods. From August 1999 to August 2000, 97 neonates (69 male, 31 female), aged 28–90 days (mean 48 days), referred for MCU and renal ultrasonography for investigation of VUR were recruited consecutively. Echo-enhanced sonography using stabilized microbubbles was followed immediately by fluoroscopic MCU. VUR was diagnosed if transient hyperechogenicity appeared within the pelvicalyceal system or ureter. The mean number of micturitions was 2.7 (range 1–6). Results. Reflux was detected in 19 kidneys (14 babies) by one or other technique. The findings were concordant in 181 kidneys (94.2%). Echo-enhanced sonography had a sensitivity of 64% (95% CI 35–87%), a specificity of 100% (95–100%), a positive predictive value of 100% (66–100%), and a negative predictive value of 94% (87–98%). Conclusions. The role of echo-enhanced sonography is limited at present in our neonatal population as a screening examination. Its ability to detect cases of high-grade reflux may make it an attractive alternative in follow-up of known cases of VUR, and may help to reduce radiation exposure in this group.

The Ikaros family of transcription factors is important for T cell development and leukemogenesis. In mice, homozygous expression of an Ikaros isoform that is unable to bind DNA leads to a complete block in T cell development, while heterozygous expression of the same Ikaros isoform results in leukemogenesis. To better understand the roles of Ikaros family members in human and murine T cell development, we used quantitative RT-PCR and Western blot analysis to determine the mRNA and protein levels of Ikaros family members in FACS-purified murine and human thymocytes. Allogeneic bone grafts are frequently sterilized by means of ionizing radiation. We investigated the effects of ionizing radiation on both quasistatic and impact mechanical properties of human bone. Specimens from four paired femora of four donors received doses of 29.5 kGy ("standard," frequently used by tissue banks), 94.7 kGy ("high"), or 17 kGy ("low") of ionizing radiation. Young's modulus was unchanged by any level of radiation. Radiation significantly reduced bending strength, work to fracture, and impact energy absorption; in each case, the severity of the effect increased from low to standard to high doses of radiation. Work to fracture was particularly severely degraded; specimens irradiated with the high dose absorbed only 5% of the energy of the controls. Radiation, even at relatively low doses, makes the bone more brittle and thereby reduces its energy-absorbing capacity. We suggest that because the level of radiation required to produce an acceptable level of viral inactivation (90 kGy) produces an unacceptable reduction in the mechanical integrity of the bone, low levels of radiation, sufficient to produce bacterial safety, should be used in conjunction with biological tests to ensure viral safety.
