Cancer-associated myositis (CAM) is defined as when cancer appears within 3 years of myositis onset. Dermatomyositis and seronegative immune-mediated necrotizing myopathy are the phenotypes mostly related to cancer. In general, treatment principles in myositis patients with and without CAM are similar. However, some aspects of myositis management are particular to CAM, including (a) the need for a multidisciplinary approach and a close relationship with the oncologist, (b) the presence of immunosuppressive and antineoplastic drug interactions, and (c) the role of the long-term immunosuppressive therapy as a risk factor for cancer relapse or development of a second neoplasm. In this review, we will also discuss immunotherapy in patients treated with checkpoint inhibitors as a treatment for their cancer.Studies on cancer risk in patients treated with long-term immunosuppressive drugs, in autoimmune diseases such as systemic lupus erythematosus or rheumatoid arthritis, and in solid organ transplant recipients have shed some light on this topic. Immunotherapy, which has been a great advance for the treatment of some types of malignancy, may be also of interest in CAM, given the special relationship between both disorders.Management of CAM is a challenge. BT8009 is a Bicycle® Toxin Conjugate (BTC™), a novel class of chemically synthesized molecules, comprising a bicyclic peptide targeting Nectin-4 tumor antigen, linked to cytotoxin (monomethyl auristatin E [MMAE]) via a valine-citrulline (val-cit) cleavable linker. Due to their small size, BTCs represent a unique therapeutic class, combining the pharmacology normally associated with a biologic with the pharmacokinetic properties of a small molecule. Here, we describe the preliminary monotherapy dose escalation results of the ongoing multicenter Phase I/II clinical trial (NCT04561362) that assesses the safety and tolerability of BT8009 administration in patients with advanced solid tumors associated with Nectin-4 expression, not exposed to enfortumab vedotin. Patients were treated until disease progression or intolerable toxicity, with tumor assessments for response per RECIST taken every two months. Nectin-4 expression levels were measured in tumor tissue. Thirty-four patients were initiated on weekly BT8009 (7 patients at 2.5 mg/m2, 20 patients at 5.0 mg/m2, and 4 patients at 7.5 mg/m2) or biweekly BT8009 (3 patient at 7.5 mg/m2). Baseline demographics were 62% patients were male, 44% and 56% were ECOG 0 and ECOG 1, respectively, median age was 65.5 years. BT8009 exhibits linear pharmacokinetics. The most common treatment-related adverse events were nausea (14 patients, 41%; ≥ G3 1 patient, 3%), fatigue (13 patients, 38%; ≥ G3 2 patients, 6%), anorexia (11 patients, 32%; ≥ G3 0 patients), constipation (10 patients, 29%; ≥ G3 1 patient, 3%) and anemia (10 patients, 29%; ≥ G3 3 patients, 9%). To date, BT8009 had low incidences of ocular disorders (3%), hyperglycemia (6%), neuropathy (6%), and rash (18%). There was 1 dose-limiting toxicity (G3 asthenia) reported at the 7.5 mg/m2 weekly dose level. Six patients (18%) experienced an SAE, with only 1 related SAE (vomiting). Treatment-emergent AEs resulting in treatment discontinuation, dose interruption, or reductions were observed in 2 patients (6%), 11 patients (32%), and 5 patients (15%), respectively. Preliminary confirmed responses observed in patients with urothelial carcinoma (UC) enrolled in the 5 mg/m2 cohort show 4/8 patients with a complete response (CR) or partial response (PR), including 1/8 patients with a CR and 3/8 patients with a PR, and 2/8 patients (25%) with stable disease (SD), reflecting a 50% overall response rate and a 75% disease control rate in UC patients for this cohort. BT8009 exhibits a promising preliminary tolerability profile and preliminary antitumor activity. The molecule will continue to be explored in the current dose-escalation/dose-expansion study of BT8009 monotherapy and in combination with nivolumab. Five drugs are currently positioned as the standard of care for the treatment of advanced or metastatic GIST patients. This is the result of continuous, deep understanding of KIT and PDGFRA GIST oncogenic drivers as well as the resistance mechanisms associated to tumor progression. However, the complexity of GIST molecular heterogeneity is an evolving field, and critical questions remain open. Specifically, the clinical benefit of approved and/or investigated targeted agents is strikingly modest at advanced stages of the disease when compared with the activity of first-line imatinib. Ripretinib is a novel switch-pocket inhibitor with broad activity against KIT and PDGFRA oncoproteins and has recently demonstrated antitumoral activity across phase I to phase III clinical trials. Therefore, ripretinib has emerged as a new standard of care for advanced, multi-resistant GIST patients. Based on this data, the Food and Drug Administration has granted in 2020 the approval of ripretinib for GIST patients after progression to imatinib, sunitinib and regorafenib. This, in turn, constitutes a major breakthrough in sarcoma drug development, as there have not been new treatment approvals in GIST for nearly a decade. Herein, we provide a critical review on the preclinical and clinical development of ripretinib in GIST. Furthermore, we seek to assess the biological and clinical impact of this new standard of care on the course of the disease, aiming to provide an insight on future treatments strategies for the next coming years. e20563 Background: MPM is a highly aggressive pleural tumor associated with asbestos exposure and with limited survival despite systemic therapy. Histology is a prognostic factor and recently CheckMate 743 trial demonstrated survival benefit of immunotherapy in first line with some differences in the efficacy of chemotherapy according to histology. However, randomized trials who led to the approval of antifolate in mesothelioma did not include analysis of outcomes by histology. The objective of this study is to characterize the impact of chemotherapy according to histology in p with MPM at our institution. Methods: We review 189 MPM p diagnosed at Vall d´Hebron University Hospital between November 2002 and April 2020. Associations between clinical variables and outcome were assessed with Cox regression models and survival data were calculated by the Kaplan-Meier method. Results: Patient’s characteristics: median age 68 years (y) (45-88 y), males: 70%, performance status (PS)1: 69%, asbestos exposure: 75%, epithelioid subtype: 76%. First line chemotherapy was offered to 85% of p (66% cisplatin-pemetrexed and 27% carboplatin-pemetrexed). Median overall survival (OS) in overall population was 21.3 m (95%CI17.2-24.3).
