We examine a two-person game we call Will-Testing in which the strategy space for both players is a real number. It has no equilibrium. When an infinitely large set of players plays this in all possible pairings, there is an equilibrium for the distribution of strategies which requires all players to use different strategies. We propose a class of spherical wavelet bases for the analysis of geophysical models and forthe tomographic inversion of global seismic data. Its multiresolution character allows for modeling with an effective spatial resolution that varies with position within the Earth. Our procedure is numerically efficient and can be implemented with parallel computing. We discuss two possible types of discrete wavelet transforms in the angular dimension of the cubed sphere. We discuss benefits and drawbacks of these constructions and apply them to analyze the information present in two published seismic wavespeed models of the mantle, for the statistics and power of wavelet coefficients across scales. The localization and sparsity properties of wavelet bases allow finding a sparse solution to inverse problems by iterative minimization of a combination of the $\ell_2$ norm of data fit and the $\ell_1$ norm on the wavelet coefficients. By validation with realistic synthetic experiments we illustrate the likely gains of our new approach in future inversions of finite-frequency seismic data and show its readiness for global seismic tomography., Comment: 43 pages, 11 figures, submitted to Geophysical Journal International

- Abstract
A major challenge of genome-wide association studies (GWASs) is to translate phenotypic associations into biological insights. Here, we integrate a large GWAS on blood lipids involving 1.6 million individuals from five ancestries with a wide array of functional genomic datasets to discover regulatory mechanisms underlying lipid associations. We first prioritize lipid-associated genes with expression quantitative trait locus (eQTL) colocalizations and then add chromatin interaction data to narrow the search for functional genes. Polygenic enrichment analysis across 697 annotations from a host of tissues and cell types confirms the central role of the liver in lipid levels and highlights the selective enrichment of adipose-specific chromatin marks in high-density lipoprotein cholesterol and triglycerides. Overlapping transcription factor (TF) binding sites with lipid-associated loci identifies TFs relevant in lipid biology. In addition, we present an integrative framework to prioritize causal variants at GWAS loci, producing a comprehensive list of candidate causal genes and variants with multiple layers of functional evidence. We highlight two of the prioritized genes, CREBRF and RRBP1, which show convergent evidence across functional datasets supporting their roles in lipid biology. © 2022 American Society of Human Genetics

Research on regulatory focus has often used hopes versus duties to operationalize promotion and prevention focus, respectively. The current research examined regulatory focus in terms of exploration versus self-control to determine whether people tend to bring different types of experiences to mind when thinking about these experiences. I used Linguistic Inquiry and Word Count software to analyze written descriptions of exploration and self-control and used t-tests to examine between-condition differences on word categories that participants used at least 0.5% of the time. Across two studies, descriptions of exploration had more positive emotional tone and used more insight words. In contrast, descriptions of self-control used more function words, more negative emotion words, including anger, more words about ingestion, and more words about power.

Background Nurse navigators are an emerging workforce providing care to people with multiple chronic conditions. The role of the navigators is to identify patients requiring support in negotiating their health care. Purpose A critical discourse analysis was used to examine qualitative data collected from nurse navigators and consenting navigated patients to identify key indicators of how nurse navigators do their work and where the success of their work is most evident. Discussion Nurse navigators help patients who have lost trust in the health system to re-engage with their interdisciplinary health care team. This re-engagement is the final step in a journey of addressing unmet needs, essential to hospital avoidance. Conclusion Nurse navigators provide a continuum of authentic and holistic care. To acknowledge the true value of nurse navigators, their performance indicators need to embrace the value-added care they provide.

Aim To understand the impact and causes of ‘Failure to Attend’ (FTA) labelling, of patients with chronic conditions. Background Nurse navigators are registered nurses employed by public hospitals in Queensland, Australia, to coordinate the care of patients with multiple chronic conditions, who frequently miss hospital appointments. The role of the nurse navigator is to improve care management of these patients. Evidence for this is measured through improvement in patient self-management of their conditions, a reduction in preventable hospital admissions and compliance with attendance at outpatient clinics. Failure to attend (FTA) is one measure of hospital utilisation, identifying outpatient appointments that are cancelled or not attended. Method The cohort for this study was patients with multiple chronic conditions, and nurse navigators coordinating their care. Data describing the concept of FTA were thematically analysed twelve months into this three year evaluation. Results Although the patient is blamed for failing to attend appointments, the reasons appear to be a mixture of systems error/miscommunication between the patient and the health services or social reasons impacting on patient’s capacity to attend. Themes emerging from the data were: access barriers; failure to recognise personal stigma of FTA; and bridging the gap. Conclusion The nurse navigators demonstrate their pivotal role in engaging with outpatient services to reduce FTAs whilst helping patients to become confident in dealing with multiple appointments. There are many reasons why a patient is unable to attend a scheduled appointment. The phrase ‘Failure to Attend’ has distinctly negative connotations and can lead to a sense of blame and shame for those with complex chronic needs. We propose the use of the neutral phrase “appointment did not proceed” to replace FTA.

30. Aspects of the volunteering experience associated with well-being in older adults

- Abstract
ISSUES ADDRESSED: While the psychological health benefits of older people's engagement in formal volunteering are well-documented, there is limited research assessing how volunteering may produce these favourable outcomes. To guide the development of volunteer positions that optimise outcomes, this study examined (a) which aspects of the volunteering experience are most strongly associated with favourable psychological health among older adults and (b) whether relationships between these aspects and psychological outcomes are moderated by sociodemographic characteristics. METHODS: A sample of 293 volunteers aged 60+ years (69% female, mean age = 73.33 years) completed measures assessing their engagement in volunteering, various aspects of their volunteer experience, and their psychological health. RESULTS: Being adequately appreciated for one's contribution, perceived social and mental intensity of the volunteer role, and believing that others benefit from one's volunteering efforts were found to be associated with higher levels of psychological health. CONCLUSIONS: Ensuring the contributions of volunteers are explicitly recognised, providing opportunities for engagement in social and cognitive activities, and informing volunteers of the benefits others experience constitute potential means of enhancing volunteer outcomes and may assist with volunteer retention. SO WHAT?: This study builds upon the limited research assessing which aspects of the volunteering experience are most strongly associated with favourable psychological health among older adults. The findings point to the potential importance of four specific aspects of the volunteer experience in enhancing psychological health outcomes.

ISSUE ADDRESSED: Population oral health (OH) improvements depend on successful, coordinated execution of oral health promotion (OHP) programs by both oral and general health professionals with key competencies (skills, abilities, knowledge and values). This study explored multidisciplinary professionals' perspectives of the competencies required for the successful implementation of a community-based OHP program called Smiles 4 Miles (S4M) in early childhood settings in Victoria, Australia. METHODS: Convenience sampling was used to recruit multidisciplinary professionals working in the S4M early childhood health promotion program in Victoria. Semi-structured focus groups were conducted with program managers/coordinators (n = 26) from 21 S4M sites and the state-wide program coordination team (n = 5). Focus groups explored OHP competency needs, capacity to promote child OH and strategies for enhancing OHP competencies. The competencies identified through focus groups were then compared to the International Union for Health Promotion and Education (IUHPE) competencies framework. RESULTS: Strategies to enhance individual and organisational OHP competencies included intersectoral collaborations; working in multidisciplinary teams; support networks and partnerships; sharing skills and expertise between health professionals. The OHP competencies identified by the participants were consistent with key IUHPE domains including ethical values and health promotion knowledge base underpinning, enabling change, advocacy for health, mediating through partnerships, communication, leadership, assessment, planning, implementation, evaluation and research. CONCLUSION: A multidisciplinary workforce based in community settings can play key and complementary roles in OHP and widen avenues for oral disease prevention. SO WHAT?: Integrated collaborative workforce models involving multidisciplinary professionals beyond the OH sector can more effectively support efforts to address the burden

White mold disease (WM) of common bean (Phaseolus vulgaris L.), caused by Sclerotinia sclerotiorum (Lib.) de Bary, reduces crop yield and quality throughout the United States. The development of cultivars with resistance to WM would be facilitated by the identification and use of molecular markers linked to resistance genes. The objectives of this research were (i) to characterize WM reaction in a recombinant inbred line (RIL) population derived from a cross between resistant and susceptible germplasm, (ii) to validate the effect of a previously reported quantitative trait locus (QTL) for WM resistance, and (iii) to locate additional QTL associated with WM resistance. A RIL population that consisted of 94 lines was derived from a cross between G122 (resistant) and CO72548 (susceptible). The population was evaluated for WM reaction in three greenhouse tests and one field environment, and for molecular markers throughout the genome. Two RIL were identified with higher resistance levels (P < 0.05) than the resistant parent G122. A previously reported QTL on linkage group B7 was significant (P < 0.01) n single-factor analysis of variance, but not with composite interval mapping. Five QTL for resistance to WM were found (likelihood odds ratio [LOD] > 2.7) on linkage groups B1, B2b, B8, and B9. The QTL were contributed from both parents and together accounted for 48% of the phenotypic variation ([R.sup.2]). For field resistance, one QTL ([R.sup.2] = 12%) on linkage group B8 was detected. These results confirm polygenic resistance to WM in common bean.

- Abstract
Background Patients with complex chronic conditions experience fragmentation of care, unnecessary hospitalization and reduced quality of life, with an increased incidence of poor health outcomes. Aim(s) The aim of this paper was to explore how nurse navigators manage client care. This was achieved through an examination of narratives provided by the nurse navigator that evaluated their scope of practice. Method(s) All nurse navigators employed by Queensland Health were invited to participate in a study evaluating the effectiveness of the service. Eighty‐four self‐reported vignettes were thematically analysed to understand the work from the nurses’ perspectives. Results Two themes emerged from the vignettes. Theme 1, the layers of complexity, is comprised of three sub‐themes: the complex patient, the complex system and patient outcomes. Theme 2, professional attributes, has two sub‐themes: person‐centred care and clinical excellence. Conclusion Navigators innovatively integrate services and address the fragmented nature of the health system. They apply expert clinical and social skills, through consistent and robust communication, to meet the needs of those with multiple chronic conditions. Implications for nursing management Results provide insight into the new role, illuminating the work they achieve, despite system complexities.

Aim With increasing age and chronicity in populations, the need to reduce the costs of care while enhancing quality and hospital avoidance, is important. Nurse‐led co‐ordination is one such model of care that supports this approach. The aim of this research was to assess the impact that newly appointed Navigators have on service provision; social and economic impact; nurses’ professional quality of life and compassion fatigue; and analysis of the change that has occurred to models of care and service delivery. Design A concurrent mixed‐method approach was selected to address the research aims. Methods The research project was funded in July 2018 and will conclude in December 2020. Several cohorts will be studied including; patients assigned to a navigator, patients not assigned to a navigator, family members of patients assigned a navigator; and a sample sized estimated at 140 navigators. Discussion This study provides a comprehensive international longitudinal and mixed method framework for evaluating the impact of nurse navigators on quality of care outcomes for patients with chronic conditions.

BACKGROUND: Education for health literacy of Australian Aboriginal people living remotely is challenging as their languages and worldviews are quite different from English language and Western worldviews. Becoming health literate depends on receiving comprehensible information in a culturally acceptable manner. METHODS: The study objective was to facilitate oral health literacy through community education about scabies and strongyloidiasis, including their transmission and control, preceding an ivermectin mass drug administration (MDA) for these diseases. A discovery education approach where health concepts are connected to cultural knowledge in the local language was used. Aboriginal and non-Aboriginal educators worked collaboratively to produce an in-depth flip-chart of the relevant stories in the local language and to share them with clan elders and 27% of the population. RESULTS: The community health education was well received. Feedback indicated that the stories were being discussed in the community and that the mode of transmission of strongyloidiasis was understood. Two-thirds of the population participated in the MDA. This study documents the principles and practice of a method of making important Western health knowledge comprehensible to Aboriginal people. This method would be applicable wherever language and culture of the people differ from language and culture of health professionals.

Traditionally, Indigenous men and women maintained distinct gendered realities. Colonisation and the subsequent introduction of the patriarchal system altered these realities, negatively impacting on Indigenous men’s and women’s health and wellbeing in a cumulative and continuing way. This literature review provides an overview of gendered Indigenous perspectives of health and wellbeing, and discusses some of the intervention strategies in Australia that have attempted to address these issues. In providing a context for understanding gendered Indigenous health perspectives, this literature review discusses the place of Indigenous peoples in contemporary Australian society and the complex historical factors that inform the relationship between Indigenous and non-Indigenous peoples. The research involved a review of gendered Indigenous health literature involving a systematic search of peer-reviewed and grey literature, and government and non-government reports. This work commenced as a conversation between Indigenous researchers who were members of the National Indigenous Research and Knowledges Network (NIRAKN) Health and Wellbeing Node who are intimately aware of the ongoing crisis in Indigenous health and wellbeing. As we progressed our collaborative conversation, we agreed to focus on several specific areas and commenced with this review of the literature about gendered Indigenous health and wellbeing in order to gain a deeper understanding of what is known and what research has taken place. This work does not set out to include all work produced, but to open up the discussion on ways to offer a greater focus on and improve gendered Indigenous health and wellbeing. We offer this monograph as a contribution to the larger conversation that needs to be had, to develop a broader understanding of Indigenous gendered health and wellbeing and what needs to happen in bringing about positive health outcomes for Indigenous peoples in Australia. Future research is needed to not only describe the situation of gendered Indigenous health and wellbeing, but to involve Aboriginal and Torres Strait Islander women and men in the ways that health services serve this community. More work is needed to build strong evidence of what works in improving gendered Indigenous health outcomes. This research project was funded by the National Indigenous Research and Knowledges Network (NIRAKN) (ARC ID: SR120100005), the Healing Foundation (via NIRAKN to the Health and Wellbeing Node) and CQUniversity and involved a review of gendered Indigenous health literature.
