Background: The use of convalescent plasma (CP) has been considered for its immunological mechanisms that could benefit patients in moderate and severe stages of COVID-19. This study evaluated the safety and efficacy of the use of donor CP for COVID-19. Material and methods: A double-blind, randomized controlled clinical trial was conducted from May to October 2020. Thirty-nine participants with moderate (II) and severe (III) stages of COVID-19 confirmed by RT-PCR were included. The study randomization rate was set at 3:1. CPs were chosen for application with a neutralizing antibody titer of ≥1:32. Results: We observed a significantly lower 21-day post-transfusion mortality HR: 0.17 (95.0% CI [0.07–0.45, p < 0.001]) in the group receiving CP compared with the control group; protective units (PU) in the group receiving convalescent plasma after seven days were significantly higher (512 (32–16,384) vs. 96 (32–256), p = 0.01); the PAO2/FIO2 index showed a significant improvement in the group receiving CP (251.01 (109.4) vs. 109.2 (62.4), p < 0.001, in the control group). Conclusion: CP is safe and effective, as it decreased mortality in the CP group compared with the control group.

In SARS-CoV-2, there is an overactivation of the immune system that triggers systemic hyperinflammation that causes lung damage; therefore, the use of convalescent plasma (CP) has been considered for its immunological mechanisms that could benefit patients in moderate and severe stages of the disease. This study evaluated the safety and efficacy of the use of convalescent donor plasma for COVID-19 to reduce mortality in patients with SARS-CoV-2 stage II (moderate) and stage III (severe) disease.Material and methodsA double-blind, randomized controlled clinical trial was conducted from May 20 to December 10, 2020. Patient follow-up included assessment by Sequential Organ Failure Assessment (SOFA) score and use of the Oxygenation Index (PAO2/FIO2) index and monitoring of blood markers, such as C Reactive Protein (CRP), D-Dimer (DD), ferritin, IgG antibody titers against SARS-COV-2, and inflammatory cytokines at days three and seven post-treatment.ResultsWe observed a significantly lower 21-day post-transfusion mortality HR: 0.17 [95.0% CI 0.07-0.45, p2/FIO2 index showed a significant improvement in the group receiving convalescent plasma 251.01 ± 109.4 vs 109.2 ± 62.4, pConclusionConvalescent plasma is safe and effective, as it decreases mortality in the convalescent plasma group compared to the control group.

The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, https://doi.org/10.17605/OSF.IO/GEHFX). Methods: In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung–Knapp–Sidik–Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. Results: A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. Conclusions: Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. The current neuronavigation techniques increase safety and surgeon confidence during neurosurgical procedure performance. However, its real usefulness remains in integrating multimodal information from advanced magnetic resonance imaging, as tractography (DTI), functional studies that evaluate motor and sensitive language, motor function (BOLD techniques with different paradigms), and nuclear medicine. At the operating room, the fusion of sonographic information acquired in real-time with the predefined plan increase the chance to achieve gross-total resection of primary brain tumors. Combining these different image modalities with brain mapping and motor stimulation information in selected cases is possible, increasing surgery safety. In this review, we present our experience with multimodal neuronavigation to treat brain tumors in pediatric patients.

Brain tumor resection in pediatric patients constitutes a real challenge. In order to improve survival and to preserve neurological function, we will further on describe our experience with awake craniotomy and functional mapping for brain tumor resection in pediatric patients. Although our experience with this technique was relatively short, we did not observe complications, and a gross total resection was successfully achieved in all cases. In the postoperative period we did not find any new deficiency in our patients. We observed functional recovery - motor and sensitive aphasia, motor strength improvement in hemiplegic patients, and recovery of neurodevelopmental milestones during follow-up. In our experience, the use of awake craniotomy and brain mapping for brain tumor resection in pediatric patients is truly safe and reliable.

Primary sclerosing cholangitis (PSC) is a rare idiopathic condition with immunopathogenic mechanisms where there is chronic progressive destruction of the biliary tree. Secondary sclerosing cholangitis (SSC) is clinically comparable to PSC, but is caused by specific processes which directly damage the biliary tree; examples include recurrent pancreatitis, bile duct malignancy, congenital bile duct abnormalities. A new cause of SSC has been described during or following significant critical illness associated with severe respiratory insufficiency, vasopressor requirement, shock and sepsis. This condition rapidly progresses to cirrhosis, frequently requiring liver transplantation for definitive management.La colangitis esclerosante primaria (CEP) es una condición idiopática rara con mecanismos inmunopatogénicos en la que existe una destrucción crónica y progresiva del árbol biliar. La colangitis esclerosante secundaria (CES) es clínicamente comparable a la CEP, pero está causada por procesos específicos que dañan directamente el árbol biliar; ejemplo de esto son enfermedades como la pancreatitis recurrente, las neoplasias de la vía biliar y las anomalías congénitas de la vía biliar, entre otras. Una nueva causa de CES ha sido descrita durante o después de una enfermedad crítica, en muchas ocasiones relacionada con las intervenciones de soporte o con condiciones de ingreso propias de la unidad de terapia intensiva, como son la ventilación mecánica invasiva, los vasopresores, el estado de choque y la sepsis. Esta condición progresa rápidamente a cirrosis, y con frecuencia requiere un trasplante de hígado para su manejo definitivo.

Miguel de Cervantes Saavedra fue hijo de un cirujano itinerante. Escribio una de las obras maestras de la literatura universal, El Ingenioso Hidalgo Don Quijote de la Mancha, considerada la primera novela moderna y solo la Biblia la rivaliza por el numero de traducciones a diferentes lenguas e idiomas. La lectura del Quijote por un medico le hace dar un viraje a nuestra profesion. En sus paginas se tratan diversos y variados aspectos medicos, que van desde la descripcion de diferentes enfermedades, tratamientos y heridas de batalla, al perfil psicologico de sus personajes. La capacidad de Cervantes para la descripcion de hechos medicos y enfermedades es notoria, derivada de la osbservacion de la actividad de su padre y de la lectura de libros de medicina. Don Quijote representa un mundo idealizado, ficticio y con claro comportamiento psicotico. A pesar de haber sido escrito hace cuatro siglos, El Quijote esta lleno de referencias a diferentes especialidades de la medicina cuyo contenido sigue siendo de interes. El objetivo de este trabajo es describir brevemente el entorno medico que vivio Cervantes y su impacto en El Quijote. Miguel de Cervantes Saavedra was the son of an itinerant surgeon. He wrote a masterpiece of the world literature, El Ingenioso Hidalgo Don Quijote de la Mancha. It is rivalled only by the Bible for the number of languages into which it has been translated. This masterpiece of the world literature is considered to be the first modern novel. For the physician, a reading of the Quijote is likely to veer in a medical direction. Various medical matters arise in the course of the novel, ranging from medical illness, treatments and battle wounds to the psychological profile of their characters. Cervantes´s excellent nosological ability is evident when he describes symptoms and signs of illness, derived from being a witness to his father´s medical practice and reading medical books. Don Quixote representing an idealized and fictional world with clear psychotic features. Although Cervantes wrote it four centuries ago, Don Quixote contains plenty of references to different specialities of medicine, and many of the ideas and concepts reflected in it are still of interest. The aim of this paper is to describe briefly the medical environment that surrounded Cervantes and its impact on the Quixote.
