Abstract

This paper presents an overview of the second edition of the HEad and neCK TumOR (HECKTOR) challenge, organized as a satellite event of the 24th International Conference on Medical Image Computing and Computer Assisted Intervention (MICCAI) 2021. The challenge is composed of three tasks related to the automatic analysis of PET/CT images for patients with Head and Neck cancer (H&N), focusing on the oropharynx region. Task 1 is the automatic segmentation of H&N primary Gross Tumor Volume (GTVt) in FDG-PET/CT images. Task 2 is the automatic prediction of Progression Free Survival (PFS) from the same FDG-PET/CT. Finally, Task 3 is the same as Task 2 with ground truth GTVt annotations provided to the participants. The data were collected from six centers for a total of 325 images, split into 224 training and 101 testing cases. The interest in the challenge was highlighted by the important participation with 103 registered teams and 448 result submissions. The best methods obtained a Dice Similarity Coefficient (DSC) of 0.7591 in the first task, and a Concordance index (C-index) of 0.7196 and 0.6978 in Tasks 2 and 3, respectively. In all tasks, simplicity of the approach was found to be key to ensure generalization performance. The comparison of the PFS prediction performance in Tasks 2 and 3 suggests that providing the GTVt contour was not crucial to achieve best results, which indicates that fully automatic methods can be used. This potentially obviates the need for GTVt contouring, opening avenues for reproducible and large scale radiomics studies including thousands potential subjects.

Abstract

Simple Summary: Immunotherapy has been introduced as a standard of care for several cancers, and its use is on the rise. However, the enhanced immune system frequently causes immune-related adverse events. Depending on the affected organ and tissue and the severity of the toxicity, immunotherapy is generally held, and steroids or immunosuppressive agents are introduced, impacting cancer treatment. Therefore, prompt identification of toxicity at an early stage and multidisciplinary management are mandatory. Since imaging is crucial to guide clinicians in managing immune-related adverse events, this imaging-based review presents the most frequent complications identifiable on imaging. Clues for identification and the most important differential diagnoses are discussed to enhance knowledge among imaging specialists and clinicians regarding these complications. Immunotherapy has revolutionized oncology care, improving patient outcomes in several cancers. However, these therapies are also associated with typical immune-related adverse events due to the enhanced inflammatory and immune response. These toxicities can arise at any time during treatment but are more frequent within the first few months. Any organ and tissue can be affected, ranging from mild to life-threatening. While some manifestations are common and more often mild, such as dermatitis and colitis, others are rarer and more severe, such as myocarditis. Management depends on the severity, with treatment being held for >grade 2 toxicities. Steroids are used in more severe cases, and immunosuppressive treatment may be considered for non-responsive toxicities, along with specific organ support. A multidisciplinary approach is mandatory for prompt identification and management. The diagnosis is primarily of exclusion. It often relies on imaging features, and, when possible, cytologic and/or pathological analyses are performed for confirmation. In case of clinical suspicion, imaging is required to assess the presence, extent, and features of abnormalities and to evoke and rule out differential diagnoses. This imaging-based review illustrates the diverse system-specific toxicities associated with immune checkpoint inhibitors and chimeric antigen receptor T-cells with a multidisciplinary perspective. Clinical characteristics, imaging features, cytological and histological patterns, as well as the management approach, are presented with insights into radiological tips to distinguish these toxicities from the most important differential diagnoses and mimickers—including tumor progression, pseudoprogression, inflammation, and infection—to guide imaging and clinical specialists in the pathway of diagnosing immune-related adverse events. Abstract

Background: Quality and reproducibility of radiomics studies are essential requirements for the standardisation of radiomics models. As recent data-driven respiratory gating (DDG) [18F]-FDG has shown superior diagnostic performance in lung cancer, we evaluated the impact of DDG on the reproducibility of radiomics features derived from [18F]-FDG PET/CT in comparison to free-breathing flow (FB) imaging. Methods: Twenty four lung nodules from 20 patients were delineated. Radiomics features were derived on FB flow PET/CT and on the corresponding DDG reconstruction using the QuantImage v2 platform. Lin's concordance factor (Cb) and the mean difference percentage (DIFF%) were calculated for each radiomics feature using the delineated nodules which were also classified by anatomical localisation and volume. Non-reproducible radiomics features were defined as having a bias correction factor Cb < 0.8 and/or a mean difference percentage DIFF% > 10. Results: In total 141 features were computed on each concordance analysis, 10 of which were non-reproducible on all pulmonary lesions. Those were first-order features from Laplacian of Gaussian (LoG)-filtered images (sigma = 1 mm): Energy, Kurtosis, Minimum, Range, Root Mean Squared, Skewness and Variance; Texture features from Gray Level Cooccurence Matrix (GLCM): Cluster Prominence and Difference Variance; First-order Standardised Uptake Value (SUV) feature: Kurtosis. Pulmonary lesions located in the superior lobes had only stable radiomics features, the ones from the lower parts had 25 non-reproducible radiomics features. Pulmonary lesions with a greater size (defined as long axis length > median) showed a higher reproducibility (9 non-reproducible features) than smaller ones (20 non-reproducible features). Conclusion: Calculated on all pulmonary lesions, 131 out of 141 radiomics features can be used interchangeably between DDG and FB PET/CT acquisitions. Radiomics features derived from pulmonary lesions located inferior to the superior lobes are subject to greater variability as well as pulmonary lesions of smaller size. [ABSTRACT FROM AUTHOR]

Abstract

BackgroundAlthough the role of image-based AI in cancer research has been substantial, its impact on the clinical side has been limited so far. Physicians’ trust in AI, and its wider acceptability, has been significantly lower owing to its “black-box” nature, which raises liability questions concerning its use in the clinical context. MethodsTo comprehend the barriers in AI’s adoption, and to inform the future discourses in the human-centric and ethical design of AI, we designed and conducted semi-structured interviews with 7 imaging experts in the oncological domain. ResultsData saturation was achieved despite the small sample size, gathering concordant emerging needs and recommendations. Our findings demonstrate the divergent nature and focus of clinical and research practices, with differing AI needs. AI is afforded a peripheral, and yet a crucial role of a “decision help”, which can enable oncologists and related imaging specialists (i.e. radiologists, radiation-oncologists and nuclear medicine physicians) to push the boundaries of biological reasoning in treating cancers. Furthermore, our interviewees emphasized the need to embody ethics and liability in designing AI systems, and the development of educational opportunities for AI and cancer experts to enable an integrative vision of image-based AI. To this end, specific design guidelines are provided to inform both the Human-Centered Design and AI researchers in order to meaningfully address the contextually-sensitive concerns and challenges around the adoption of intelligent interactive technologies in cancer care. ConclusionsThe existing impact of AI in the clinical practices is limited as compared to the clinical research. In the future, AI is afforded a peripheral role of a ‘decision helper’ which might enable doctors to better understand the peculiarities and subtleties of cancers, and support them in developing novel treatment methods. Finally, in order to develop physicians’ trust in AI and its wider acceptability in clinical oncology, designers would have to address the ethical and liability concerns in relation to the use of AI systems.
