Introduction. Castleman's disease (CD) or angiofolicullar lymph node hyperplasia is a rare pathologic process characterized by non-neoplastic reactive proliferation of lymphoid tissue. Mimicking clinical and laboratory signs of infection, it could be a great diagnostic problem for an infectologist. Case report. We report a case of a 39-year old man who was initially clinically suspected to have an infectious central nervous system (CNS) affection, having most similar appearance to neurotuberculosis. Malignancy with bone metastases and lymphoma were also among many possible diagnoses. The patient was later histologically confirmed to have Castleman's disease, analyzing the enlarged inguinal lymph node, which was the key point in rejecting the suspicion of malignancy and tuberculosis. By further analyses, the patient was diagnosed to have mixed connective tissue disorder (MCTD). Vasculitis of mesencephalon and thalamus was detected by magnetic resonance imaging. Conclusion. CD with CNS involvement is very rare as well as CD with MCTD association, making this case even more unique. This case report underlines the importance of definitive histological diagnosis in patients with lymphadenopathia associated with systemic involvement and the need of additional immunological and radiological examinations, as well.

Background/Aim. The era of direct-acting antiviral (DAA) regimen in the treatment of chronic hepatitis C virus (HCV) started in 2011. The aim of this study was to assess the antiviral efficacy and safety of DAA regimen, ombitasvir (OBV)/paritaprevir (PTV)/ritonavir (r) + dasabuvir (DSV) + ribavirin (RBV), in patients with chronic HCV infection, genotype 1. Methods. The real-life data were collected. The study was multicentric and included seven infectious diseases and hepatology departments in Serbia. A total of 21 patients were enrolled in the OBV/PTV/r + DSV + RBV early access program, 20 of which were previously treated with pegylated interferon + RBV, while 1 was treatment-naive. All patients received the adequate doses of these antiviral drugs. RBV was not given to the patients with HCV genotype 1b infection according to the therapeutic protocol. For the majority of patient, the treatment duration lasted for 12 weeks. For the patients with liver cirrhosis, who were infected with HCV genotype 1a, the duration of treatment was 24 weeks. Viremia was assessed at four points in time: at baseline, 4 weeks after the treatment beginning (rapid viral response, RVR), 12 or 24 weeks after the treatment beginning (end of treatment response – ETR) and 12 weeks after the end of treatment (sustained viral response – SVR). SVR, as a confirmation of the absence of HCV was considered as endpoint of successful treatment. Results. Complete RVR, ETR and SVR were achieved in 64.71%, 85.71% and 95.24% of the patients, respectively. Only 3 patients had mild adverse effects which did not required dose reduction. Conclusion. The treatment of the patients with a chronic HCV infection with OBV/PTV/r + DSV + RBV resulted in excellent antiviral activity and tolerability.

Introduction. One of the more uncommon etiological factors responsible for the development of acute pancreatitis (AP) is hypercalcemia. Hyperparathyroidism (HPT), as a cause of hypercalcemia, is responsible for 1.5–13% of AP according to a number of studies. A mechanism of the development of AP in hyperparathyroidism is still unclear. Case report. We presented a 47-year-old female patient, who had five episodes of AP in total before the etiological factors were finally determined. The patient had certain comorbidities which were considered to be potential causes of AP. She had chronic renal insufficiency (she was on a regular hemodialysis program), systemic lupus erythematosus and mioma uteri. She used to regularly take an antiepileptic drug (combination of sodium valproate and valproic acid). During the fifth episode of AP, the serum calcium level was for the first time elevated to twice the normal value. Level of parathyroid hormone was several times higher. A static scintigraphy found hyperplasia or hyperfunctional adenoma of the right inferior and superior parathyroid glands. Abdominal multislice computed tomography (MSCT) scan verified the enlargement of the entire pancreas, as well as the presence of heterogeneous structures with diffuse amorphous calcifications. The lytic lesions in the pelvic bones could be seen in both sides. Parathyroidectomy was being postponed by an endocrine surgeon because of the poor overall condition of the patient. In the next period the patient had five more episodes of AP. The condition was significantly contributed by increasingly more frequent and longer episodes of metrorrhagia. Despite all therapeutic measures that were taken, systemic inflammatory response syndrome (SIRS) developed, and fatal outcome occurred. Conclusion. In case of recurrent pancreatitis, hyperparathyroidism is to be considered even if a significant elevation of serum calcium is not present. This is especially the case for patients with chronic renal insufficiency or impaired vitamin D metabolism, who have a higher risk of secondary hyperthyroidism.

Introduction: Alpha-synuclein (ASYN) accumulation is considered to be one of the key factors in Parkinson's disease (PD) pathogenesis. Posttranslational modifications, including oxidation and nitration, can increase aggregation of ASYN. Alterations of adenosine monophosphate-activated protein kinase (AMPK) signaling have been shown in several neurodegeneration models, including PD. Aim: The aim of this study was to investigate the role of AMPK in the action of modified recombinant wt (wild-type) ASYN on differentiated SH-SY5Y cells. Materials and methods: All experiments were conducted in all-trans retinoic acid-differentiated human neuroblastoma SH-SY5Y cells, treated with 3 different types of modified recombinant wt ASYN (oligomeric, nitrated and dopamine-modified ASYN). Activation of AMPK and Raptor, together with conversion of LC3-I to LC3-II, were monitored using immunoblotting. Cell viability was assessed using crystal violet assay. Results: Treatment with modified recombinant wt ASYN (oligomeric, nitrated and dopamine-modified ASYN) led to substantial decrease in number of viable differentiated SH-SY5Y cells. Western blot analysis showed decreased levels of AMPK and its downstream kinase Raptor, together with an increase in the conversion of LC3-I to LC3-II, upon treatment with recombinant ASYN. Pharmacological activator of AMPK (AICAR) significantly increased the number of viable cell in the presence of all 3 types of modified recombinant wt ASYN. Conclusion: Modified forms of recombinant wt (wildtype) ASYN (oligomeric, nitrated and dopamine-modified ASYN) induced decrease in number of viable differentiated SH-SY5Y cells, accompanied by inactivation of AMPK/Raptor signalling pathway and autophagy induction. Pharmacological activation of AMPK improved cell survival, indicating a protective role of AMPK in neurotoxicity of extracellular ASYN.

Abuse of anabolic-androgenic steroids (AAS) produces side effects in different tissues, with oxidative stress linked to their pathophysiology, being involved in fibrosis, cellular proliferation, and tumorigenesis. The aim of this study was to examine the acute effects of nandrolone decanoate (ND) on oxidative stress in isolated rat heart. The hearts of male Wistar albino were excised and perfused according to the Langendorff technique at gradually increasing coronary perfusion pressures (40-120 cmH2O). The hearts were perfused with ND at doses of 1, 10 and 100 μM. Oxidative stress markers, including the index of lipid peroxidation (thiobarbituric acid reactive substances (TBARS)), nitric oxide (nitrites; NO2-), the superoxide anion radical (O2-) and hydrogen peroxide (H2O2) were measured in the coronary venous effluent. Our results showed that acute effects of ND do not promote the production of reactive oxygen species (ROS). Our finding pointed out that the highest concentration of ND may even possess some anti-oxidative potential, which should be examined further.

Introduction. The triple therapy which consists of one of the protease inhibitor plus pegylated interferon and ribavirin (P/R) is the standard of care for the treatment of chronic hepatitis C virus (HCV) genotype 1(G1) infection both in treatment-naпve and experienced patients. Objective. The aim of this study was to analyze the efficacy and tolerability of this regime in hospital practice in Serbia. Methods. From July 2012 to October 2012, 20 previously treated patients with advanced fibrosis and HCV G1 infection were included in the triple antiviral regimen in six referral centers in Serbia. All patients were treated with response guide therapy (RGT) regime according to the boceprevir treatment protocol. During the 4-week lead-in period all patients received peginterferon plus ribavirin. After the lead-in period boceprevir was added in the dosage of 800 mg three times a day orally. The subsequent treatment varied according to virologic response and fibrosis. During the therapy HCV RNA level was measured at week 4, 8, 12, 24 of the treatment for the assessment of virologic response profile. All patients who completed therapy were assessed at the end of the treatment and at the end of an additional 24-week treatment-free period for a sustained virologic response (SVR). Results. The total of 20 patients with advanced fibrosis was treated. Among patients with an undetectable HCV RNA level at week 8 the rate of SVR was 100%. No patient with decrease in the HCV RNA level

Background/Aim. The most important ethiology factor of chronic liver disease that progresses into terminal insufficiency is hepatitis C virus (HCV) infection. Intravenous (iv) drug abuse is the main cause for spreading HCV. Thus the therapy for such patients is of extreme importance in reducing the incidence of the disease. The aim of the study was to establish efficacy of a combined therapy with peginterferon alpha-2a and ribavirin in iv opiate substances abusers having chronic HCV infection in relation to sex, age, genotype and level of fibrosis and duration of HCV infection before the treatment. Methods. Thirty one iv opiate substances abusers with chronic hepatitis C (HHC) were enrolled in the examination. The patients were divided according to the genotype into two groups. The patients with genotypes 1 and 4 (n = 18) were treated for 48 weeks, while those with genotypes 2 and 3 (n = 13) for 24 weeks. PCR HCV RNA, genotype determination and liver biopsy were done to each patient. Results. A stabile virological response was achieved in 93.5% of the patients, so the therapy demonstrated statistically significant efficacy i. v. opiate substances abusers with HHC (p < 0.001). There was no statistically significant difference in therapeutic response among patient groups formed according to the genotype, sex, duration of the disease and level of fibrosis (p > 0.05). Conclusion. Therapy of of iv opiate substances abusers with HHC has its specificities, and these patients need special treatment. Efficacy of the therapy was equivalent in patient groups formed according to the sex, genotype, level of fibrosis and duration of HCV infection. A combined therapy with peginterferon alfa 2a and ribavirin has high level of success in the treatment of these patients.

Background/Aim. Hepatitis C virus infection (HCV) is a complex disease, most commonly chronicle (80-85%). The aim of this research was to determinate the level of the liver damage in the patients cansed by HCV in conjunction with consuming ethyl alcohol. Methods. The research included 15 patients with chronic HCV infection supported by the misuse of ethyl alcohol, as well. The diagnosis of C infection hepatitis was proved using the ELISA test and PCR method. Results. The results of the study showed the liver damage by both HCV infection and ethyl alcohol, which was verified by the presence of biochemical changes and patohystological processing of the patients (liver biopsy and prosection). Patohystological changes were at the level of liver cirrhosis and carcinoma (2 patients). There was a signficant difference between the two subgroups (p < 0.001) regarding the examined values γ-GT, PLT and PTV. The basic therapeutic procedure was to introduce this category of patients into alcohol abstinence, and, in a few patients, to apply the antivirus therapy, as well. Conclusion. Based on the number of the examined patients (n = 15), we could conclude that a prolonged ethyl alcohol misuse with the presence of HCV infection was in a correlation with the liver disease progression.

Early diagnosis of the central nervous system (CNS) infections is a precondition of their successful treatment. However, the essential standard examination of the cerebrospinal fluid (CSF) is sometimes neither specific enough to define their basic nature, nor sufficient to differentiate them from processes of non-infectious origin. Supposing that the released surface molecules of activated immunocompetent cells could better define the character of inflammatory reaction, the levels of soluble CD4 antigens (sCD4) were determined with enzyme-immunosorbent test in the CSF of the patients with various CNS diseases. Background. The presence of lymphocytes within the liver parenchyma is related to immunologically mediated liver damage in chronic hepatitis C. The aim of the study was to make histological, histochemical, and immunocytochemical assessment of liver biopsy specimens in patients with chronic hepatitis C virus (HCV) infection. Methods. Biopsy specimens of 20 patients with chronic HCV disease were analyzed, using standard staining procedures to verify histologic liver lesions, as well as immunoenzymatic staining with monoclonal antibodies to detect CD4+ T-lymphocytes, B-lymphocytes, and macrophages. Results. Micromorphologic characteristics of chronic active viral hepatitis C were present in all the patients, differing, hower, by the level of their activity. Dominant changes were found within the portal space, consisting of mononuclear lympho-plasmocytic infiltration and macrophages. Immunocytochemical investigation of mononuclear and macrophageal infiltration showed the correlation between micromorphological findings and the degree of the activity. Conclusion. The presence of lymphocytic and macrophageal infiltration within the hepatic tissue directly correlated with the intensity of the liver damage. Analysis of the population of cellular infiltrate in the liver together with the monitoring of viremia level and the level of hepatocyte necrosis, could be useful tools for elucidation of the pathogenesis of chronic hepatitis C.

Introduction. Listeria monocytogenes is the third most frequent cause of bacterial meningitis in adults. It commonly affects persons with defective cell-mediated immunity or advanced age, and only a few patients with no underlying predisposition have been reported. Case report. We presented an previously healthy, 18-year-old man with typical clinical features of meningitis. On the account of earlier treatment with ceftriaxone and cerebrospinal fluid finding, an assumption of partially treated bacterial meningitis was made. The initial treatment with vancomycin and ceftriaxone, substituted on day 4 with meropenem, did not produce any clinical effect. On day 6 Listeria monocytogenes was isolated and, even as late as that, the administration of ampicillin was followed by complete recovery of the patient. Conclusion. In younger, immunocompetent individuals, in spite of the existent diagnostic and therapeutic problems, the subacute course of Listeria monocytogenes meningitis provides enough time for appropriate treatment and favorable disease outcome.

Tularaemia, as a rare disease, suprised medical workers in 1999 and 2ooo when it was detected and diagnosed in 31 patients. In 1999 patients were registered in villages of the Sokobanja community, and in 2ooo also in other communities in the district of Nish. Patients were peasants, mostly housewives. Glandular form of the disease was dominant. The main clinical symptoms were: tonsillitis, pain in the region of enlarged lymphatic glands and temperature, Bio-humoral syndrome presented accelerated sedimentation of erythrocytes and elevated fibrinogen values. Antibody titer of Francisella tularensis was extremely high in the majority of patients. The disease was cured in all patients: in 27 patients after the first therapy, in 4 subjects after recidivation and 5 patients were surgically treated.

Conventional air quality monitoring networks typically tend to be sparse over areas of interest. Because of the high cost of establishing such monitoring systems, some areas are often completely left out of regulatory monitoring networks. Recently, a new paradigm in monitoring has emerged that utilizes low-cost air pollution sensors, thus making it possible to reduce the knowledge gap in air pollution levels for areas not covered by regulatory monitoring networks and increase the spatial resolution of monitoring in others. The benefits of such networks for the community are almost self-evident since information about the level of air pollution can be transmitted in real time and the data can be analysed immediately over the wider area. However, the accuracy and reliability of newly produced data must also be taken into account in order to be able to correctly interpret the results. In this study, we analyse particulate matter pollution data from a large network of low-cost particulate matter monitors that was deployed and placed in outdoor spaces in schools in central and western Serbia under the Schools for Better Air Quality UNICEF pilot initiative in the period from April 2022 to June 2023. The network consisted of 129 devices in 15 municipalities, with 11 of the municipalities having such extensive real-time measurements of particulate matter concentration for the first time. The analysis showed that the maximum concentrations of PM2.5 and PM10 were in the winter months (heating season), while during the summer months (non-heating season), the concentrations were several times lower. Also, in some municipalities, the maximum values and number of daily exceedances of PM10 (50 μg/m3) were much higher than in the others because of diversity and differences in the low-cost sensor sampling sites. The particulate matter mass daily concentrations obtained by low-cost sensors were analysed and also classified according to the European AQI (air quality index) applied to low-cost sensor data. This study confirmed that the large network of low-cost air pollution sensors can be useful in providing real-time information and warnings about higher pollution days and episodes, particularly in situations where there is a lack of local or national regulatory monitoring stations in the area. The aim of the study was to analyse relationship between PM2.5 and PM10 concentrations, traffic density and meteorological factors during the week with regular traffic regime and first week of COVID-19 lockdown in Novi Sad, Serbia. During the study period, which included working days and weekends, traffic emission contributions to PM were also determined. Obtained results have shown higher PM, pressure and emission contribution values, lower temperature, relative humidity values, and lower traffic counts for all vehicle categories during the COVID-19 week. A positive correlation was detected only during the first week, between PM2.5 and passenger vehicles and lightweight trucks, as well as between PM10 and all categories of vehicles. Background PM2.5 and PM10 concentrations were moderately correlated to total traffic during the first week as well. Very strong and moderate positive correlation was detected between PM2.5 and PM10 concentrations and temperature during COVID-19 week. The PM concentrations increased during COVID-19 week, but total traffic decreased by 31% on workdays and 42% on weekends, proving the impact of lockdown measures on traffic regime and intensity. Since relationship between PM2.5 and PM10 with different vehicle categories was confirmed only during first week, and PM and emission contribution concentrations were higher during COVID-19 week, a secondary emission source of PM was strongly indicated. Very strong and strong positive correlations of PM2.5 and PM10 with temperature during COVID-19 week have confirmed lower temperature impact on PM concentrations and, consequently, increased impact of heating, as an emission source, due to lockdown measures and people staying at their homes. • Geometric morphometrics was used for the first time for analysing rosette leaves of Sempervivum ciliosum and S. ruthenicum complexes. • Differences in the shape and size of the rosette leaves are significant between complexes and species. • The applied method is suitable for explaining morphological differences and taxonomical relations. • Geometric morphometrics can improve general knowledge of the genus Sempervivum. The yellow-flowered Sempervivum species from the Balkan Peninsula, tentatively divided into Sempervivum ciliosum (S. ciliosum, S. galicicum, S. jakucsii, S. klepa, S. octopodes) and S. ruthenicum complexes (S. kindingeri, S. leucanthum, S. ruthenicum, S. zeleborii), are well known for their pronounced phenotypic plasticity, especially in the vegetative parts, resulting in exceedingly complicate infrageneric taxonomy. Extensive studies dealing with the variability of qualitative and quantitative traits, such as the shape and size of the rosette leaves, were not conducted in the past. This study aimed to analyse the extent of morphological variability of the rosette leaves regarding their shape and size and evaluate their potential to resolve taxonomic doubts. For this purpose, geometric morphometrics, as a tool for analysing minute shape variability, was used to discriminate mentioned species. The variability of the rosette leaves was statistically analysed within 15 populations using univariate (ANOVA) and multivariate statistical analysis (MANOVA, PCA, DA). Obtained results indicated highly statistically significant differences in rosette leaf shape and size between analysed complexes and species. [ABSTRACT FROM AUTHOR]
