12. Schizotypy, depression, and anxiety in physical and social anhedonia

Abstract

Social anhedonia is a more promising indicator of vulnerability to schizophrenia than physical anhedonia, both as assessed by Chapman scales. More broadly, the populations identified by these scales would have a propensity to different psychiatric disorders. This cross-sectional study examined the respective profiles of schizotypy, anxiety, and depression in French students with physical and social anhedonia, using psychometric and interview-based measures. Compared to controls (n=46), subjects with social anhedonia (n = 19) reported higher schizotypal scores for interpersonal, paranoid, disorganization, and cognitive/perceptual dimensions, whereas subjects with physical anhedonia (n = 35) had more extensive interpersonal deficits and paranoia. Both groups had more depressive and anxiety symptoms than controls, in particular subjects with social anhedonia. Keywords: physical anhedonia; social anhedonia; schizotypy; depression; anxiety

Abstract

Si, à l'origine, les maladies du corps et de l'esprit possédaient chacune leur autonomie, si elles connurent ensuite au xixe siècle un rapprochement conduisant à la neuropsychiatrie, la déflagration de mai 1968 contribua à les isoler de nouveau. L'émergence des sciences cognitives, et d'un esprit radicalement interdisciplinaire, a fait éclater ces replis dogmatique. C'est bien là l'esprit des neurosciences cognitives, où l'émotion et la cognition constituent deux phénomènes tellement rapprochés et intriqués que leur séparation, si elle peut garder un intérêt au niveau de l'enseignement et de la pratique, n'a plus de sens au niveau de l'identification des mécanismes physiopathologiques. Leurs moyens d'investigation sont les mêmes, qu'il s'agisse de l'imagerie cérébrale, de la génétique cognitive, de la modélisation, voire de la cognition sociale. On verra dans cet ouvrage une illustration répétée de l'intrication de ces deux disciplines lorsqu'il s'agit de s'intéresser aux dysfonctionnements qu'elles étudient l'une et l'autre. C'est bien ici le mérite du programme Cognitique d'avoir su fédérer des communautés à l'origine isolées pour créer une nouvelle mouvance de recherche. La cognition réparée ?

Abstract

Failure to obtain convincing results in psychiatric genetics can partly be attributed to the fact that progress in molecular biology and genetic epidemiology has not been followed by an equivalent development in phenotypic description. Instead of relying entirely on classical nosological approaches, we argue that identifying more homogeneous forms of diseases through a 'candidate symptom approach' among affected subjects and an endophenotype approach that identifies sub-clinical traits among non-affected relatives might yield better results. Examples where these strategies have already been fruitful when applied to complex diseases are presented in this review. Focusing on vulnerability traits might stimulate the redefinition of traditional psychiatric syndromes and help to bridge the gap between clinical and experimental approaches.

Abstract

A comparison of male and female French smokers and their non-smoking counterparts reveals that the former score higher on estimation of sensation seeking such as boredom susceptibility, experience seeking and disinhibition. Most people smoke to increase stimulation, especially when they are bored, and this, along with the desire to seek new experiences, makes them vulnerable to nicotine addiction. Men scored less than women on the experience seeking subscale.

International audience; La remédiation cognitive assistée par ordinateur (RCAO), dans laquelle le patient est utilisateur direct de la technologie, s’est étendue à diverses pathologies psychiatriques dont la dépression persitante. Son objectif est de restaurer un fonctionnement de qualité par l’intermédiaire de la stimulation interactive des fonctions cognitives altérées. La pratique de la RCAO nécessite que les usagers s’engagent véritablement dans une relation avec l’ordinateur, or ceux-ci sont peu habitués à utiliser les nouvelles technologies au sein de leur « prise en charge ». La médiation de la relation entre le patient et l’ordinateur par un thérapeute s’avère donc nécessaire afin de renforcer la confiance du sujet dans cet outil, de soutenir sa motivation et d’accompagner le développement de la métacognition qui constitue un pivot pour l’impact des résultats sur la vie quotidienne. Bien que la communication non verbale soit modifiée par la présence de l’ordinateur, les aspects empathiques peuvent se développer au cours des rencontres et permettre l’instauration d’une alliance thérapeutique. Nous précisons les modalités d’intervention en RCAO appliquée à la dépression, les enjeux du numérique dans ce contexte et le rôle du thérapeute au sein de ce dispositif de soin.

47. Fear of falling: efficacy of virtual reality associated with serious games in elderly people

Focusing on the psychological dimension of this pathology (phobic reaction to walking), we looked at how virtual reality associated with serious games can be used to treat this pathology.Methods: Participants with fear of falling were randomly assigned to either a treatment group or a waiting list. The therapy consisted of 12 weekly sessions of virtual reality exposure therapy associated with serious games.Results: Sixteen participants were included. The mean age of the treatment group was 72 years and that of the control group was 69 years. Participants’ scores on the fear of falling measure improved after treatment with virtual reality associated with serious games, leading to a significant difference between the two groups.Conclusion: Virtual reality exposure therapy associated with serious games can be used in the treatment of fear of falling. The two techniques are complementary (top-down and bottom-up processes). To our knowledge, this is the first time that a combination of the two has been assessed. There was a specific effect of this therapy on the phobic reaction. Further studies are needed to confirm its efficacy and identify its underlying mechanism.
