26. Exploring Factors That Influence the Efficacy of Functional Communication Training

Abstract

Understanding factors that influence the efficacy of functional communication training has both practical and conceptual benefits. The current study extended research in this area by exploring data from 95 consecutive applications of functional communication training with extinction across two independent clinics. We selected candidate predictor variables based on conceptual analysis, conducted preliminary exploratory analyses, and then selectively applied quantitative methods that are used in precision medicine to examine their accuracy and predictive utility. Treatment outcomes were better when challenging behavior was maintained by a single function than they were when it was maintained by multiple functions; however, these differences were most apparent among cases with an escape function. We also analyzed within-session responding to explore the potential influence of unprogrammed establishing operations on decrements in treatment efficacy. Our within-session measure only distinguished responders from nonresponders when escape was one of the multiple functions. Additional research is needed to validate these findings with an independent sample and to address a number of clinical conceptual issues.

Abstract

Rate-setting is a problematic area for newcomers to translation and established practitioners alike. Survey data generally support the view that translators feel underpaid and that money matters remain a chief ethical and pragmatic concern, but appropriate guidance is almost entirely absent from introductory textbooks on the translation profession and documentation prepared by industry associations remains unsatisfactory. Focusing on the translation industry in the United Kingdom, this conceptual paper explores constraints that limit price formation practices, and argues that translators feel under threat from disruptive technologies, Uberisation, and non-professional translation, now more than ever. We explore the complex interaction between status, internal and external perceptions, and regulation, and illustrate their push-pull relationship with rate-setting within a range of industry ‘educators’, uncovering the ways in which translators themselves, translation associations, and academic institutions directly and indirectly impact upon rate-setting practices. The article concludes by considering potential channels to buoy status and improve rate-setting practices in the translation industry.

41. Single Case Design Research in Special Education: Next Generation Standards and Considerations

Abstract

Single case design has a long history of use for assessing intervention effectiveness for children with disabilities. Although these designs have been widely employed for more than 50 years, recent years have been especially dynamic in terms of growth in the use of single case design and application of standards designed to improve the validity and applicability of findings. This growth expanded possibilities and inspired new questions about the contributions this methodology can make to generalizable knowledge about intervention in special education. In this paper, we discuss and extend previous standards for studies using single case designs (i.e., Horner et al., 2005). We identify new suggestions for internal validity, generality and acceptability, and reporting. We also provide considerations for single case synthesis and discuss the complexities of assessing accumulating evidence for a given practice.

Abstract

Background Infection with the novel severe acute respiratory syndrome coronavirus 2 has been associated with a hypercoagulable state. Emerging data from China and Europe have consistently shown an increased incidence of venous thromboembolism (VTE). We aimed to identify the VTE incidence and early predictors of VTE at our high-volume tertiary care center. Methods We performed a retrospective cohort study of 147 patients who had been admitted to Temple University Hospital with coronavirus disease 2019 (COVID-19) from April 1, 2020 to April 27, 2020. We first identified the VTE (pulmonary embolism [PE] and deep vein thrombosis [DVT]) incidence in our cohort. The VTE and no-VTE groups were compared by univariable analysis for demographics, comorbidities, laboratory data, and treatment outcomes. Subsequently, multivariable logistic regression analysis was performed to identify the early predictors of VTE. Results The 147 patients (20.9% of all admissions) admitted to a designated COVID-19 unit at Temple University Hospital with a high clinical suspicion of acute VTE had undergone testing for VTE using computed tomography pulmonary angiography and/or extremity venous duplex ultrasonography. The overall incidence of VTE was 17% (25 of 147). Of the 25 patients, 16 had had acute PE, 14 had had acute DVT, and 5 had had both PE and DVT. The need for invasive mechanical ventilation (adjusted odds ratio, 3.19; 95% confidence interval, 1.07-9.55) and the admission D-dimer level ≥1500 ng/mL (adjusted odds ratio, 3.55; 95% confidence interval, 1.29-9.78) were independent markers associated with VTE. The all-cause mortality in the VTE group was greater than that in the non-VTE group (48% vs 22%; P = .007). Conclusions Our study represents one of the earliest reported from the United States on the incidence rate of VTE in patients with COVID-19. Patients with a high clinical suspicion and the identified risk factors (invasive mechanical ventilation, admission D-dimer level ≥1500 ng/mL) should be considered for early VTE testing. We did not screen all patients admitted for VTE; therefore, the true incidence of VTE could have been underestimated. Our findings require confirmation in future prospective studies.

In 1919, the doors of Youngstown's Butler Institute of American Art were opened for the first time. Dubbed'the lighthouse of culture,'both the beautiful marble museum and the artwork inside were the gift of 19th-century industrialist Joseph G. Butler, Jr., in what was the crowning achievement of a long life. Butler earned his successes with hard work, a competitive spirit and business savvy. He earned a fortune in the iron and steel industry crowded by such figures as Andrew Carnegie, Henry Frick and Charles Schwab. Butler also took on politicians, promoted American interests, preserved American history and spearheaded projects to improve his community. To friends and admirers, he was affectionately referred to as'Uncle Joe.'This biography chronicles Butler's early life through his career in the iron and steel industry, detailing his contributions to the art world, his philanthropic endeavors and his accomplishments as an author and historian.

Abstract

Phosphoinositide-3-kinase δ (PI3Kδ) is a critical regulator of cell growth and transformation and has been explored as a therapeutic target for a range of diseases. Through the exploration of the thienopyrimidine scaffold, we have identified a ligand-efficient methylation that leads to remarkable selectivity for PI3Kδ over the closely related isoforms. Interrogation through the Free-Wilson analysis highlights the innate selectivity the thienopyrimidine scaffold has for PI3Kδ and provides a predictive model for the activity against the PI3K isoforms.
