Introducción: Las tecnologías 4.0 han logrado la obtención de información en tiempo real sobre los pacientes, el personal médico ha podido tomar decisiones basadas en datos científicos, además de lograr de manera más eficiente el ahorro de tiempo y recursos. Objetivo: Explorar el desarrollo y la implementación de las tecnologías 4.0 en el sector de la salud, determinando sus principales beneficios y dificultades, para de esta manera tener una mayor aproximación teórica a este fenómeno industrial. Metodología: Revisión bibliográfica sistemática de artículos científicos en español e inglés desde 2021 a 2023. Se analizaron más de 20 artículos de bases de datos como SCOPUS, Redalyc, Science Direct, incluyendo artículos originales y estudios de caso, observacionales y descriptivos. Resultados: varios análisis médicos realizados por IAs demostraron eficiencia en detección de afecciones, como metástasis pulmonares y de cáncer de mama, también al momento de crear algoritmos de predicción como muertes por sobredosis. Conclusiones: La aplicación de tecnologías 4.0 en el sector de la salud ha sido evidencia del gran avance científico, las inteligencias artificiales; han mejorado el proceso de intervencionismo médico, en emisión de diagnósticos, medidas preventivas y planes de tratamiento.

El presente artículo propone el análisis y la mejora del sistema eléctrico y la inyección del vehículo Suzuki Forsa 1 del Instituto Tecnológico Superior Oriente. Se emplean diversas herramientas de diagnóstico y desmontaje, como el dinamómetro, el software RaceTec R1000 y un multímetro, para diagnosticar el comportamiento del vehículo, desmontar las piezas originales y montar las piezas mejoradas. Adicionalmente, se realiza la programación de los componentes eléctricos para potenciar la seguridad y el rendimiento del vehículo. La repotenciación demuestra la importancia del rendimiento del vehículo en buen estado y su contribución al aumento de potencia y seguridad. Los resultados prerrepotenciación indican un torque de 37,4 lb.pie a 3000 RPM y una potencia de 21,4 HP a 4000 RPM. Tras la repotenciación con inyección electrónica, se obtienen 60,8 lb.pie a 3500 RPM y 40,9 HP a 4000 RPM. En consecuencia, se concluye que la repotenciación mejora las condiciones de funcionamiento. Antes de la repotenciación, con el sistema a carburador y sistema eléctrico deteriorado, la velocidad máxima era de 130 km/h; después de la repotenciación se logra alcanzar los 150 km/h.

Abstract Background Bilateral ureteropelvic junction obstruction is a common cause of secondary hypertension in the pediatric population, often due to congenital malformation. On the other hand, it is less frequently encountered in the adult population and is usually due to an acquired condition, most commonly by a bilaterally obstructing nephrolithiasis causing hydronephrosis and subsequent hypertension. The aim of this study was to investigate and highlight the underlying mechanisms by which acute bilateral ureteropelvic junction obstruction causes hypertensive crisis and why early detection and prompt treatment are necessary to mitigate the effects of elevated blood pressure on target organs. Case presentation A 41-year-old African American man with hypertensive cardiomyopathy presented with anuria. He was found to have elevated blood pressure with evidence of target organ damage on laboratory examination, demonstrated by sudden elevation of his serum creatinine level. He was initially treated with oral and intravenous antihypertensives, with minimal improvement. The work-up was unremarkable apart from the imaging finding of acute bilateral ureteropelvic junction obstruction from obstructing nephrolithiasis causing hydronephrosis. Bilateral ureteral stents were placed for decompression, with resolution of the hypertensive crisis and improvement of renal function. Conclusion This case highlights the importance of prompt diagnosis and treatment of underlying acute bilateral ureteropelvic junction obstruction to mitigate the deleterious effects of sudden blood pressure elevation on target organs.

This article describes the linkage methods that will be used in the Decennial Census Digitization and Linkage project (DCDL), which is completing the final four decades of a longitudinal census infrastructure covering the past 170 years of United States history. DCDL is digitizing and creating linkages between nearly a billion records across the 1960 through 1990 U.S. censuses, as well as to already-linked records from the censuses of 1940, 2000, 2010, and 2020. Our main goals in this article are to (1) describe the development of the DCDL and the protocol we will follow to build the linkages between the census files, (2) outline the techniques we will use to evaluate the quality of the links, and (3) show how the assignment and evaluation of these linkages leverages the joint use of routinely collected administrative data and non-routine survey data.

AbstractDuring the unconventional reservoir development, a proper modelling of the underground fracture networks and their effects on production is crucial for reservoir development potential and realistic economic analysis. Conventionally, the complex fracture system formed by hydraulic and natural fractures is extremely difficult to capture, let alone to numerically simulate it. Most importantly, the current best solution can only rely on the knowledge of the natural fractures from the geology and geophysics team and hydraulic fractures from the engineering team. Nevertheless, this solution fails to realize the dynamic stress regime variations when fracturing jobs are done within the horizontal wellbore. In this study, a variety of data source and modelling tools is harnessed to delineate a more realistic and representative discrete fracture network (DFN). The first step is to obtain the original natural fractures already depicted from geological and geophysical information and the statistical information regarding the spatial configurations of this DFN. Next, a new set of natural fractures is generated by an in-house natural fracture generator while preserving the spatial characteristics of the original natural fractures at the same time. Then, a combined DFN of the original natural fracture and newly generated natural fractures is accomplished. This combined DFN is then intensity-calibrated by the given microseismic cloud events, especially focusing on the near-wellbore region. Then, a displacement discontinuity method- (DDM-) based in-house hydraulic fracture propagation model is used to generate hydraulic fractures with complex boundaries, honoring the fracturing job logistics from the engineering team. After this step, an ultimate and highly representative DFN can be achieved. By applying this very novel workflow, DFN characterizations of both a single-well scenario and well-pad (3 wells) scenarios have been highly successful. Statistics such as the cluster-wise hydraulic fracture half-length, height, aperture, and numbers of activated/nonactivated natural fractures can be easily presented. Through the powerful numerical method called the embedded discrete fracture model (EDFM), production simulation and stimulated reservoir volume evaluation can be seamlessly studied. Extents of 3D drainage volumes can also be plotted with ease. Overall, a holistic picture regarding the unconventional reservoir’s underground DFN can be reliably depicted, using the proposed workflow.

Summary: Background: Culturally tailored interventions may reduce disparities in diabetes care. We conducted a nationally representative assessment of self-reported cultural competency measures of care among patients with diabetes in the United States. Methods: The 2017 National Health Interview Survey was queried for adults with self-reported diabetes. Sample weight-adjusted multivariable logistic regressions defined adjusted odds ratios and 95% confidence intervals of a positive response to each of the cultural competency measures while controlling for relevant sociodemographic variables. Findings: 2,448 adults were included in the analyses. Black, Latinx, and Asian respondents had greater odds of and individuals with the highest income level had lower odds of placing greater importance in sharing cultures with their provider. Black and Latinx individuals had lower odds of reporting encountering providers who shared or understood their cultures. Asians had lower odds of and respondents aged 40-64 and 65 years and older had greater odds of reporting frequently being treated with respect by their providers. Non-English speakers had lower odds of and individuals from higher income brackets had greater odds of reporting frequently receiving easy-to-understand information about their care. Blacks and respondents not part of the workforce had greater odds of reporting frequently being asked about their opinions or beliefs in care. Interpretation: Disparities in self-reported provider cultural competency measures exist among cultural minorities in the United States. Our findings may inform efforts to reduce disparities and improve care among minorities with diabetes. Funding: No funding was used in the preparation of this work.

As in conventional computing, memories for quantum information benefit from high storage density and, crucially, random access, or the ability to read from or write to an arbitrarily chosen register. However, achieving such random access with quantum memories in a dense, hardware-efficient manner remains a challenge. Here we introduce a protocol using chirped pulses to encode qubits within an ensemble of quantum two-level systems, offering both random access and naturally supporting dynamical decoupling to enhance the memory lifetime. We demonstrate the protocol in the microwave regime using donor spins in silicon coupled to a superconducting cavity, storing up to four weak, coherent microwave pulses in distinct memory modes and retrieving them on demand up to 2 ms later. This approach offers the potential for microwave random access quantum memories with lifetimes exceeding seconds, while the chirped pulse phase encoding could also be applied in the optical regime to enhance quantum repeaters and networks.

The purpose of this research is to develop a game popularity tracking system that can generate popularity values once every 24 hours using daily data from Google Analytics. Within the scope of this project, game popularity is defined as the fact that the quality of a certain game is liked by many people. There are 5 metrics that can be used to calculate game popularity: users count, unique page views (UPV), average time on page (ATP), the difference of unique page views, and average time on page from the day before. To generate the popularity values daily, the combination of weighted average, exponential moving average, and exponential smoothing algorithm are used. The final result of this project is a service that can run automatically and also provides an HTTP API that could fetch popularity values of games from gaming sites, sending the data in XML format for other services to use when needed. The service is built using Node.JS and other in-trend technologies such as Google Analytics API v4, OAuth 2.0, Express.JS, etc.

This reflective essay highlights the importance of project structure when teaching highly controversial topics. I review research that shows differing approaches to teaching highly controversial subjects and the impact of the approaches on learning outcomes. I then reflect on the outcomes of deploying a non-directive, peacekeeping approach to address the issue of police-involved shootings in a criminal justice class. Finally, I draw attention to ancillary issues that emerge in this project structure as well as how the results impacted my own thinking about this topic.

Objective To determine whether there is an association between hypermobility and sports injury.Methods A quantitative observational approach using a cross-sectional survey was adopted. Individuals were identified as hypermobile or not. All participants were asked to complete two questionnaires: one asking demographic information and the other injury-specific. Fisher’s exact test was used for statistical analysis.Results 114 individuals participated in the study, 62 women and 52 men. 26% of the participants were hypermobile. There was no significant association between hypermobility and sports injury (p=0.66). There was a significant increase in joint and ligament sprain among the non-hypermobile (NH) group covering all sports (p=0.03). Joint dislocation was found exclusively among hypermobile individuals. The duration of injury in hypermobile individuals was higher than NH. The use of oral painkillers or anti-inflammatories in the semiprofessional group was greater than the general population.Conclusion Hypermobility is relatively common among individuals, and there is a lot of anecdotal evidence associating it with increased rates of injuries. This project finds that NH individuals are more likely to sustain a ligament or joint sprain in sports. This is due to increased joint laxity and flexibility preventing injury. There were important limitations to this study which will be addressed in further work. These include assessing for pauciarticular hypermobility and focusing on one sport to investigate its association with sports injury in those who are hypermobile or not. It would also be important to focus on one specific joint, assessing its flexibility and association with injury.

INTRODUÇÃO: A cicatriz do umbigo é a única do corpo aceita culturalmente, cuja dismorfia ou ausência chama a atenção negativamente. As distorções dessa cicatriz, que ainda ocorrem com frequência nas abdominoplastias, determinaram a elaboração de inúmeros procedimentos técnicos com a finalidade estética de imitar o seu aspecto natural. Apresentamos mais uma opção sobre neo-onfaloplastia no decurso das plásticas de abdome, mediante uma técnica de fácil execução, com efeito estético próximo ao umbigo natural e sem cicatrizes cutâneas. MÉTODO: No decurso da abdominoplastia, o umbigo e o seu pedículo são amputados e a derme da pele na área de projeção do neoumbigo é suturada diretamente no plano da fáscia muscular abaixo, após divulsão tecido adiposo local para produzir a necessária umbilicação sem cicatriz. RESULTADOS: No período de nove anos, foram operados 316 pacientes com a utilização desta técnica, com constante aspecto natural e mínimos efeitos pouco estéticos.INTRODUCTION: Under the cultural aspect the umbilicus is the only scar accepted in the body, its dysmorphia or absence is noted negatively. Distortions of this scar are still frequent in the abdominoplasties determined by several different aesthetic technical procedures to imitate its natural aspect. The scope of this presentation is another attempt to obtain a neoomphaloplasty based on an easy technical procedure, with a quite natural final aspect and without scars. METHODS: During the abdominoplasty, the umbilicus and its stalk are amputated and the dermis of the skin over the future projection is sutured directly to the fascia below, after the local fat excesses be divulsioned to produce the natural navel aspect without scar. RESULTS: Along the last nine years, 319 patients were operated by this technique, with a natural aspect and minimal unaesthetic effects.

Microorganisms secrete into their extracellular environment numerous compounds that are required for their survival. Many of these compounds could be of great interest for biotechnology applications and their genes used in synthetic biology design. The secreted proteins and the components of the translocation systems themselves can be scrutinized in-depth by the most recent proteomic tools. While the secretomes of pathogens are well-documented, those of non-pathogens remain largely to be established. Here, we present the analysis of the exoproteome from the marine bacterium Ruegeria pomeroyi DSS-3 grown in standard laboratory conditions. We used a shotgun approach consisting of trypsin digestion of the exoproteome, and identification of the resulting peptides by liquid chromatography coupled to tandem mass spectrometry. Three different proteins that have domains homologous to those observed in RTX toxins were uncovered and were semi-quantified as the most abundantly secreted proteins. One of these proteins clearly stands out from the catalogue, representing over half of the total exoproteome. We also listed many soluble proteins related to ABC and TRAP transporters implied in the uptake of nutrients. The Ruegeria pomeroyi DSS-3 case-study illustrates the power of the shotgun nano-LC-MS/MS strategy to decipher the exoproteome from marine bacteria and to contribute to environmental proteomics.

Procedural Content Generation is the automatic process for generating game content in order to allow for a decrease in developer resources while adding to the replayability of a digital game. It has been found to be highly effective as a method when utilized in rougelike games, of which Hotline Miami 2: Wrong Number shares a number of factors. Search based procedural content, in this case, a genetic algorithm, allows for the creation of levels which meet with a number of designer set requirements. The generator proposed provides for an automatic creation of game content for a commercially available game: the level design, object placement, and enemy placement.

Bacteria from the Roseobacter clade are abundant in surface marine ecosystems as over 10% of bacterial cells in the open ocean and 20% in coastal waters belong to this group. In order to document how these marine bacteria interact with their environment, we analyzed the exoproteome of Phaeobacter strain DSM 17395. We grew the strain in marine medium, collected the exoproteome and catalogued its content with high-throughput nanoLC-MS/MS shotgun proteomics. The major component represented 60% of the total protein content but was refractory to either classical proteomic identification or proteogenomics. We de novo sequenced this abundant protein with high-resolution tandem mass spectra which turned out being the 53 kDa RTX-toxin ZP_02147451. It comprised a peptidase M10 serralysin domain. We explained its recalcitrance to trypsin proteolysis and proteomic identification by its unusual low number of basic residues. We found this is a conserved trait in RTX-toxins from Roseobacter strains which probably explains their persistence in the harsh conditions around bacteria. Comprehensive analysis of exoproteomes from environmental bacteria should take into account this proteolytic recalcitrance.
