Background: There is ongoing debate around rate versus rhythm control strategies for managing atrial fibrillation (AF), however, much of the data comes from Western cohorts. Kerala‐AF represents the largest prospective AF cohort study from the Indian subcontinent. Objectives: To compare 12‐month outcomes between rate and rhythm control strategies. Methods: Patients aged ≥18 years with non‐transient AF were recruited from 53 hospitals across Kerala. Patients were stratified by rate or rhythm control. The primary outcome was a composite of all‐cause mortality, arterial thromboembolism, acute coronary syndrome or hospitalization due to heart failure or arrhythmia at 12 months. Secondary outcomes included bleeding events and individual components of the primary. Predictors of the composite outcome were analysed by logistic regression. Results: A total of 2901 patients (mean age 64.6 years, 51% female) were included (2464 rate control, 437 rhythm control). Rates of the primary composite outcome did not differ between groups (29.7% vs 30.0%; p =.955), nor did any component of the primary. Bleeding outcomes were also similar (1.6% vs 1.9%; p =.848). Independent predictors of the primary composite outcome were older age (aOR 1.01; p =.013), BMI <18 (aOR 1.51; p =.025), permanent AF (aOR 0.78; p =.010), HFpEF (aOR 1.40; p =.023), HFrEF (aOR 1.39; p =.004), chronic kidney disease (aOR 1.36; p <.001), and prior thromboembolism (aOR 1.31; p =.014). Conclusion: In the Kerala‐AF registry, 12‐month outcomes did not differ between rate and rhythm control cohorts. [ABSTRACT FROM AUTHOR]

Objectives: The purpose of the study was to assess the precision of cone beam computed tomography (CBCT) in comparison with panoramic radiography in determining the anatomical relationship of inferior alveolar nerve (IAN) with the impacted mandibular third molar. Materials and Methods: Twenty patients diagnosed with the following panoramic radiographic markers: darkening of the root, interruption of white line of mandibular canal, diversion of mandibular canal, and narrowing of the roots suggesting a close relationship of roots with the mandibular canal were selected and underwent an additional CBCT to assess the proximity of IAN to mandibular third molar roots. All patients were assessed for loss of sensation or neurosensory deficit in the chin and lower lip during postoperative period by objective and subjective methods. Results: Twenty patients with an average age of 25.4 years (21–39 years) with 21 impacted mandibular third molars were included in this sample. It was found that after the removal of impacted third molars, IAN was not visible in any of the cases and postoperative objective and subjective neurosensory tests showed no signs of neurosensory disturbances. Conclusion: The study found that CBCT had limited usefulness in neurovascular bundle exposure prediction, prior to surgical removal of impacted mandibular third molars. The accuracy of radiographic markers in conventional panoramic radiography to predict neurovascular exposure was also limited.

Reconstruction of craniofacial bony defects has always been a challenging task for the surgeons over the years. The science of reconstructing such defects is of at most importance to craniofacial and plastic surgeons due to its relevance in facial aesthetics function as well as prerequisite procedure for continuing other surgical procedures. The main goal of the reconstruction of the craniofacial defects is to reduce the morbidity by restoring the facial form and aesthetics, as well as a good function of the facial structures by achieving a reasonable occlusion and articulation. Although significant improvements have occurred during the last few decades, challenges still exist as to what type of reconstruction to be carried out with regard to techniques and the type and quality of materials of choice to be used. As decades progressed, the advancement in surgical techniques and the variety of reconstruction methods have definitely improved the quality of life. This article reviews the method of bony reconstruction of craniofacial defects using autologous human bone marrow stem cells and autologous bone grafts and its modification, which includes much recent tissue engineering techniques and regenerative medicine, thereby replacing older techniques by biological substitutes, which can restore improve and maintain orofacial function and aesthetics.

Background: Oral submucous fibrosis (OSMF) is a chronic progressively scarring disease of the oral cavity. Lycopene is a powerful antioxidant obtained from tomatoes and has the highest singlet oxygen quenching capacity and a high capacity of quenching other free radicals in vitro among dietary carotenoids. Hyaluronidase is a substance prepared from the testes and semen of mammals that modifies the permeability of connective tissue through the hydrolysis of hyaluronic acid. Objective: To evaluate the efficacy of lycopene and lycopene−hyaluronidase combination, and to compare the efficacy of lycopene and lycopene−hyaluronidase combination in the treatment of OSMF. Study Design: The study consisted of 45 patients with OSMF divided into three equal groups. Patients in Group A were given Lycored 16mg daily in two equally divided doses for 3 months. Patients in Group B were given LycoRed along with hyaluronidase intralesional injection of 1500 IU twice weekly for 3 months. Patients in Group C were given placebo capsules. Patients were evaluated after 3 months. The following parameters were recorded: mouth opening, visual inspection, palpatory findings, and burning sensation. Results: There was statistically significant change in mouth opening and burning sensation for lycopene and lycopene–hyaluronidase combination than in the placebo group in the treatment of OSMF, but the lycopene–hyaluronidase combination did not show any statistically significant change when compared with lycopene alone. Conclusion: Lycopene appears to be a very promising antioxidant in the management of oral submucous fibrosis, both in clinical and symptomatic improvement.

- Abstract
Maxillary sinus is the largest among the paranasal sinus which is of surgical clinical and anthropological significance. Classification of maxillary antrum based on shapes and sizes using panoramic radiography have not been developed till date. Different shapes and sizes of maxillary antrum were taken into notice while tracing orthopantomogram. As panoramic radiograph is the most commonly available and feasible technology, a deeper study of this topic felt compelling.: To classify maxillary antrum based on its shape, presence of septa, shape of the floor and presence of recess employing digital panoramic radiographs. A total of 1000 panoramic radiographs available in soft copies in Department of Oral Radiology, Sree Anjaneya Institute of Dental Sciences were analysed for developing this classification. The panoramic radiographs were taken using KODAK 8000 carestream machine with exposure parameters 73kVp, 12mA, 13.9 seconds. The shapes identified included cloudy, rectangle, round, trapezoidal, square, ovoidal, pentagonal, hexagonal. In which cloudy marked the highest and hexagonal the least. Among the maxillary sinuses observed a few showed septations. The sinus floor contours observed comprised scalloped, wavy, curved, straight and v-shaped. Out of which a few showed recess. This is a proposed new classification of maxillary antrum using Panoramic radiograph. From this study, eight distinct shapes of the maxillary sinus were identified, wherein cloudy type was predominant and hexagonal was the least commonest. Only a few had sepatations and recess. Six distinct shapes of floor were identified, of which scalloped marked the highest.

- Abstract
Oral microbiota is among the most assorted in the human body. In excess of 700 species have been distinguished in the mouth, and new sequencing techniques are permitting us to find significantly more species. The life structures of the oral cavity is not the same as that of other body locales. The oral cavity has mucosal surfaces (the tongue, the buccal mucosa, the gingiva, and the palate), hard tissues (the teeth), and exocrine organ tissue (major and minor salivary organs), all of which present novel elements for microbiota organization. Oral squamous cell carcinoma is quite possibly the most well-known danger and is the main source of dismalness and mortality.

- Abstract
Orthopantomograms of different individuals were studied to understand the prevalence of morphological variants in styloid process. New types of styloid processes were identified other than that found in the existing classification while proceeding with the study. Thus, a deeper examination of this topic felt interesting. To identify the anatomical variations of styloid process and its distribution according to age and gender on digital panoramic radiographs.A total of 1000 panoramic radiographs from the database were analyzed for the types and distribution. Langlais classification with modification is used for the study. The tapered and wavy forms were identified apart from Langlais classification of styloid process, in elongated category. Among total population under study elongated category were predominant compared to other types and gender wise distribution manifested the predilection of elongated category among male. Moreover, the elongated variant dominated among the older individuals than adult and younger age groups. Considering age as the parameter Type IV category prevails in younger age group.1):The two new morphological variants among the elongated variant of styloid process categorized as tapered and wavy were identified, which is to be included in existing classification. 2): Elongated variant is most predominantly seen, of which tapered variety is common among elongated variety, 86.6% was seen in females and 83.8% was seen in males; 3): Wavy variant was dominant in males with 16.1%, whereas in females it was 13.3%. 4): Contemplation of gender as parameter, manifested male predilection to elongated category. 5): Relatively high prevalence of type IV category was analyzed next to elongated variant among age group upto 20 years.

- Abstract
Chromium is a harmful heavy metal to the environment due to the toxicity induced by it to plants and other living organisms. High concentration of Cr in soil poses severe toxicological problems ecosystem. Phytoremediation using different plants is an economical and environment-friendly method for removing Cr from soil. The addition of chelating agents augments the phytoextraction using plants. The present study aimed to augment the Cr phytoremediation capacity of Amaranthus virdis, a predominant plant species in the Cr-contaminated open dumpsites of Bangalore. . Phytoextraction of Cr by Amaranthus viridis was studied in the presence of different chelating agents viz. ethylenediaminetetraacetic acid (EDTA), citric acid (CA), growth promoting hormone- indoleacetic acid (IAA) and NPK fertiliser. A. viridis grown under different concentrations (5, 10 and 20 mg/Kg) of Cr were treated with 0.5g EDTA/Kg of soil, 0.5g CA/Kg of soil, 1mg IAA/Kg of soil and NPK (125 mg of nitrogen, 45 mg of phosphorous and 156 mg of potassium per Kg of soil). Results indicated that CA, at 10 mg/kg Cr supply, induced the highest uptake (up to 29.25 µg/plant). Furthermore, the study revealed that CA amendment induced maximum Cr uptake in A. viridis at all levels of Cr supply as compared to other amendments. This was due to the increased solubility of Cr in the presence of citric acid and the amelioration of oxidative stress due to Cr to plants by citric acid. This study inferred that the non-hyperaccumulating plant, A. virdis could be used as a phytoremediator for Cr in the presence of citric acid in the places where it is grown abundantly.

Aims: Limited data on the uptake of guideline‐directed medical therapies (GDMTs) and the mortality of acute decompensated HF (ADHF) patients are available from India. The National Heart Failure Registry (NHFR) aimed to assess clinical presentation, practice patterns, and the mortality of ADHF patients in India. Methods and results: The NHFR is a facility‐based, multi‐centre clinical registry of consecutive ADHF patients with prospective follow‐up. Fifty three tertiary care hospitals in 21 states in India participated in the NHFR. All consecutive ADHF patients who satisfied the European Society of Cardiology criteria were enrolled in the registry. All‐cause mortality at 90 days was the main outcome measure. In total, 10 851 consecutive patients were recruited (mean age: 59.9 years, 31% women). Ischaemic heart disease was the predominant aetiology for HF (72%), followed by dilated cardiomyopathy (18%). Isolated right HF was noted in 62 (0.6%) participants. In eligible HF patients, 47.5% received GDMT. The 90 day mortality was 14.2% (14.9% and 13.9% in women and men, respectively) with a re‐admission rate of 8.4%. An inverse relationship between educational class based on years of education and 90 day mortality (high mortality in the lowest educational class) was observed in the study population. Patients with HF with reduced ejection fraction and HF with mildly reduced ejection fraction who did not receive GDMT experienced higher mortality (log‐rank P < 0.001) than those who received GDMT. Baseline educational class, body mass index, New York Heart Association functional class, ejection fraction, dependent oedema, serum creatinine, QRS > 120 ms, atrial fibrillation, mitral regurgitation, haemoglobin levels, serum sodium, and GDMT independently predicted 90 day mortality. Conclusion: One of seven ADHF patients in the NHFR died during the first 90 days of follow‐up. One of two patients received GDMT. Adherence to GDMT improved survival in HF patients with reduced and mildly reduced ejection fractions. Our findings call for innovative quality improvement initiatives to improve the uptake of GDMT among HF patients in India. [ABSTRACT FROM AUTHOR]

Introduction: Specific learning disorder (SLD) is a much common concept widely discussed nowadays. Specifically, it is a neurodevelopmental disorder found in children and adults. According to the Diagnostic and Statistical Manual of Mental Disorder (DSM)-V, SLD stands for a group of disorders that impedes the ability to learn or to use the requisite academic skills. It comprises impairment in reading, writing, and mathematical ability. Aim of the Review: This systematic review aimed to identify the prevalence and pattern of learning disability among children of India. Methods: This systematic review and meta-analysis followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and is also registered under PROSPERO vide CRD Number: CRD42021286224. The literature search was done using Boolean operators in databases such as PubMed, ProQuest, Cochrane, ScienceDirect, Google Scholar, and websites such as ResearchGate, Academia, and so on. The literature search was focused on Indian studies conducted between 2000 and 2020 (20 years). The initial search identified 122 articles across the databases. Following the screening process and removing the duplicate files, 20 articles were included in the review. According to the predetermined protocol, the researcher independently searched, reviewed, collected, extracted, and evaluated the requisite data and relevant information from each research article. Each individual research article was assessed for its quality using the "critical appraisal tool for use in systematic reviews addressing questions of prevalence." In the current review, considering the heterogeneity between the studies with regard to sample selection, geographical location, and instrument used for data collection, a random-effect model was adopted. The statistical evaluations were done through OpenMeta (Analyst). Results and Discussion: The prevalence of learning disability among Indian children ranges from 2.16% to 30.77% across the studies. The pooled prevalence of LD among children and adolescents is estimated to be 10.70% (95% confidence interval [CI]: 7.10% to 14.3%), and the median age was 6-12 years. The prevalence of dyslexia among children and adolescents is estimated to be 6.20% (95% CI: 4.40% to 7.90%; I2 = 97.01% P < 0.001), while the prevalence of dysgraphia is estimated to be 6.30% (95% CI: 3.60% to 9%; I2 = 97.83% P < 0.001). Moreover, the prevalence of dyscalculia is much lower and it is estimated to be 4.90% (95% CI: 2.60% to 7.20%; I2 = 97.59% P < 0.001). The finding of this review is consistent with another review by the National Survey of Children's Health, USA, which estimated the lifelong prevalence of learning disability to be 9.7% in children (USA) from 3 to 17 years of age. Conclusion: The finding of the review explains that learning disability affects quite a large number of Indian Children. The information from the current review may set a benchmark to assess the disease burden of the country and it will be a reference for resource planning and health-care policymaking. Aims Data on the burden of acute heart failure (AHF) admissions, practice patterns, and outcomes are rare from India and other low- and middle-income countries. We aimed to describe the baseline characteristics, guideline-directed medical therapy (GDMT) prescribing patterns and 90-day mortality rates in patients admitted with AHF in Kerala, India. Methods and results The Cardiology Society of India-Kerala Acute Heart Failure Registry (CSI-KHFR) is an observational registry from 50 hospitals in Kerala, India, with prospective follow-up. Consecutive patients with AHF, who consented to participate, were enrolled. The 2016 European Society of Cardiology criteria were used for the diagnosis of AHF. Kaplan–Meier survival analysis and Cox-proportional hazard models were used for data analysis. The variables in the MAGGIC risk score were used in the multivariable model. A total of 7507 patients with AHF (37% female) participated in the CSI-KHFR. The mean age was 64.3 (12.9) years. More than two-third had reduced ejection fraction (EF) (67.5%). Nearly one-fourth (28%) of patients with heart failure (HF) with reduced EF received GDMT. Overall, in-hospital and 90-day mortality rates were 7% and 11.6%, respectively. Prescriptions of different components of GDMT were independently associated with 90-day mortality. Conclusion The CSI-KHFR recorded an in-hospital and 90-day mortality of 7% and 11.6%, respectively. Only one of four patients received GDMT. AHF mortality was independently associated with GDMT initiation. Quality improvement initiatives that focus on increasing GDMT prescription may improve the survival of HF patients in India. [ABSTRACT FROM AUTHOR]

A comparative field studies on seven municipal dumpsites namely Agara 1 (12.917°N , 77.639°E), Agara 2 (12.922°N, 77.639°E), HSR depot (12.919°N, 77.644°E) , Koramangala Church (12.934°N, 77.626°E) , Koramanagla BDA (12.931°N, 77.625°E), Garvebhayipalya (12.897°N, 77.638°E) and Sanjay Gandhi hospital (12.891°N, 77.601°E), and its adjoining non-dump sites were conducted to understand their soil characteristic features and the vegetation pattern. Soil characteristics were presented in terms of the physicochemical parameters and the vegetation patterns were presented in terms of the dominance using the ecological parameter Important Value Index (IVI). Soils at the dump sites showed higher mean electrical conductivity and pH values as compared to the non-dump sites. Though the mineral content showed higher mean value in the dump sites (except chloride), there is no significant variation in the higher total soluble solutes between dump and non-dump sites(P>0.05) As per ANNOVA there was highly significant variation in the heavy metal content between dump and non dumpsites (P<0.01).. With respect to vegetation analysis though 50 different species found across locations only 10 species viz Alternatheria sessile, Amaranthus spinose, Caesalpinia pulcherima, Ipomea acumilanata, Ipomea evolvulus, Parthenium hysterophorous Pisum sativum, Ricinis communis, Sida rombifolia and Solanum lycopersicum were found consistent across all locations irrespective of the seasons. Among these, A. sessile, R. communis and A. spinosa were found dominant based on the IVI values across seven locations which further can be studied for their potential for phyto remediating the land pollutants such as heavy metals.

- Abstract
Nickel is an essential element for normal physiological functions in plants. At higher doses (>50 mg/kg, as per WHO) it is toxic to plants and humans, which can bring about oxidative stress affecting the physiological functions of plants and is also considered carcinogenic to human beings. To manage nickel pollution in environment, proper chemical or phytoremediation techniques are required. In this regard nickel accumulator plants would offer a cost effective and environmental friendly phytoremediation method. In the present study, the nickel phyto-accumulation potential of Amaranthus viridis from soil was evaluated to check the tolerance level and the impact on selected morphological parameters like total biomass, plant height, root length and number of leaves. Nickel uptake by A. viridis was studied from Ni contaminated soil amended with20, 40, 60, 80 mg/Kg of Ni exposure under controlled conditions. Toxic effects and tolerance of the plant to toxic doses of nickel was evaluated by correlating the uptake per gram of biomass with various parameters of plant like its height, biomass, root length and, number of leaves. Supply dependent maximum nickel uptake of 108 µg/gm and corresponding decrease in growth parameters were recorded up to 60 mg/Kg exposure. This study indicates the uptake of nickel by A. viridis increases with increase in supply up to 60mg/kg and beyond 60 mg/kg, the uptake decreases. The study also shows uptake of nickel per gram of biomass has a significant negative correlation mainly with parameters like plant height (R= -0.71 at 0.05 level of significance) and total biomass (R = -0.83 at 0.05 level of significance) where as other parameters like length of root and number of leavers are not significantly affected (P>0.05) with uptake of nickel per gram of biomass.

Patients in Group A were given Lycored 16 mg daily in two equally divided doses for 3 months.

SLEEP ,SNORING ,EXPERIMENTAL design ,YEAR ,DATABASE searching - Abstract
Sleep is equally important to the food we eat, the liquids we drink, or the safety of the children. Each living being needs sleep. It is the essential activity of the mind during each stage of development. Whereas inadequate sleep can lead to the development of various health issues. Although a generous group of literature has explored the relationship between sleep patterns and problems, comprehensive reviews and far-reaching conclusions are lacking. This systematic review was conducted to describe the sleep pattern or habits present in Children and also to identify its influence on the onset of various sleep problems among 3 to 18 years. Electronic databases were searched for articles published up to August 2019 and no limits for study designs were kept. The articles for review were obtained from databases like MEDLINE, PubMed, Cochrane, and Google Scholar along with a handful of references of experts using Boolean operators’ search criteria. The guidelines adopted were Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). The nighttime sleep duration was the most common sleep measure or variable in 73% of the articles. The average sleep duration (night time) is 9.811 hours (3 to 5 years), 9.082 hours (6 to12 years) and 7.944 hours (12 to 18 years). The result from this review suggested that sleep patterns tend to delay as the age increases leading to insufficient sleep and irregular bedtime schedules leading to the onset of some of the sleep problems like snoring, nightmare, bedwetting, etc. A few reviews suggested that it was difficult to define the sleep problem effectively as families vary greatly in their tolerance of their children’s sleeping habits. - Abstract
Aims: Heart Failure (HF) is a growing epidemic globally and a challenging situation particularly for a country like India. There is paucity of data in the Indian context and the Indian College of Cardiology National Heart Failure Registry (ICCNHFR) would be the first of its kind in understanding the aetiology, demographics and treatment pattern in the country. The rationale and design of the study is described. Methods and Results: This is a prospective observational registry study of Acute Decompensated Heart Failure (ADHF) across various hospitals in India. This would include data of consecutive adult patients admitted with acute heart failure in the hospitals. This study is from Sep 2019 to Sep 2021 and is expected to enroll about 10000 patients. The primary data would be to analyse the demographics, aetiology, comorbid conditions, inhospital mortality, 30 day and one year mortality and rehospitalisation. The secondary aim would be understand the diagnostic patterns and therapeutic options offered for these patients. Conclusion: The present registry is designed to analyse the national data on ADHF. This would identify gaps in HF care and therapy. This would help in improving quality of heart failure care in the country and better resource utilization. This will help in the reduction of mortality and morbidity rate related to HF. [ABSTRACT FROM AUTHOR]
