Abstract

Prostate cancer (PCa) is a major public health concern for men globally and the most commonly diagnosed cancer among men in the European Union (EU). Despite large trials suggesting benefits from early detection of PCa, risks of overdiagnosis and overtreatment are evident. In 2022, the EU Commission proposed introducing prostate-specific antigen (PSA) testing for men in an organised setting, in combination with magnetic resonance imaging (MRI) scanning as a follow-up test to minimise these risks. PRostate cancer Awareness and Initiative for Screening Europe (PRAISE-U) is a pilot study evaluating the implementation of a risk-stratified population-based approach to PCa screening in Ireland, Lithuania, Poland, and two areas in Spain (Galicia and Manresa) for feasibility, efficacy, and cost effectiveness. As per the protocol designed for the pilots, men aged 50–69 yr residing within the catchment area of the study sites will be invited to participate. Those consenting to participate will undergo PSA testing, and men with PSA >3 ng/ml will undergo risk stratification before MRI and, if necessary, after MRI before undergoing biopsy. A collaborative user board comprising health care professionals, patients, and decision-makers will be formed to provide stakeholder input throughout the study. PRAISE-U will be evaluated on three major pillars: analysis of clinical and programme outcomes, psychosocial impact, and cost effectiveness. A set of key performance indicators (KPIs) has been developed to be piloted in the PRAISE-U pilot sites. The KPIs will serve to assess the performance and outcomes of risk-stratified PCa screening at each site. A REDCap database will be used to collect and manage pseudoanonymised data from the pilot sites. Ethics approval was obtained from each pilot site. The PRAISE-U pilot implementation is expected to commence in the 3rd quarter of 2024 for 12 mo and provide valuable data on the implementation outcomes of a risk-stratified screening approach across Europe. The findings is expected to inform the development of an optimised screening strategy with an acceptable benefit to harm ratio.

Abstract

Background Periodontitis is a chronic inflammatory non-communicable disease (NCD) characterised by the destruction of the tooth-supporting apparatus (periodontium), including alveolar bone, the presence of periodontal pockets, and bleeding on probing.Objectives To outline, for family doctors, the implications of the association between periodontal and systemic diseases; to explore the role of family doctors in managing periodontitis as an ubiquitous non-communicable disease (NCD).Methods The consensus reports of previous focused collaborative workshops between WONCA Europe and the European Federation of Periodontology (using previously undertaken systematic reviews), and a specifically commissioned systematic review formed the technical papers to underpin discussions. Working groups prepared proposals independently, and the proposals were subsequently discussed and approved at plenary meetings.Results Periodontitis is independently associated with cardiovascular diseases, diabetes, chronic obstructive pulmonary disease, obstructive sleep apnoea, and COVID-19 complications. Treatment of periodontitis has been associated with improvements in systemic health outcomes. The article also presents evidence gaps. Oral health care professionals (OHPs) and family doctors should collaborate in managing these conditions, including implementing strategies for early case detection of periodontitis in primary medical care centres and of systemic NCDs in oral/dental care settings. There is a need to raise awareness of periodontal diseases, their consequences, and the associated risk factors amongst family doctors.Conclusion Closer collaboration between OHPs and family doctors is important in the early case detection and management of NCDs like cardiovascular diseases, diabetes mellitus, and respiratory diseases. Strategies for early case detection/prevention of NCDs, including periodontitis, should be developed for family doctors, other health professionals (OHPs), and healthcare funders. Evidence-based information on the reported associations between periodontitis and other NCDs should be made available to family doctors, OHPs, healthcare funders, patients, and the general population.

Abstract INTRODUCTION Disease‐modifying therapies (DMTs) for Alzheimer's disease (AD) will increase diagnostic demand. A non‐invasive blood‐based biomarker (BBBM) test for detection of amyloid‐β pathology may reduce diagnostic barriers and facilitate DMT initiation. OBJECTIVE To explore heterogeneity in AD care pathways and potential role of BBBM tests. METHODS Survey of 213 healthcare professionals/payers in US/China/UK/Germany/Spain/France and two advisory boards (US/Europe). RESULTS Current diagnostic pathways are heterogeneous, meaning many AD patients are missed while low‐risk patients undergo unnecessary procedures. Confirmatory amyloid testing (cerebrospinal fluid biomarkers/positron emission tomography) is utilized in few patients, resulting in diagnostic/treatment delays. A high negative‐predictive‐value test could streamline the diagnostic pathway by reducing unnecessary procedures in low‐risk patients; supporting confirmatory testing where needed. Imminent approval of DMTs will increase need for fast and reliable AD diagnostic tests. DISCUSSION An easy‐to‐use, accurate, non‐invasive BBBM test for amyloid pathology could guide diagnostic procedures or referral, streamlining early diagnosis and DMT initiation. Abstract

COVID-19 outbreak has significantly changed all aspects of general practice in Europe. This article focuses on the academic challenges for the discipline, mainly in the field of education, research, and quality assurance. The efforts of the European Region of the World Organisation of National Colleges, Academies, and Academic Associations of General Practitioners/Family Physicians (WONCA Europe) to support academic sustainability of the discipline in the time of pandemic are presented. Medical education was affected by the pandemic, threatening both its productivity and quality. Emerging new educational methods might be promising, but the results of their rapid implementation remain uncertain. A relatively small number of publications related to COVID-19 and general practice is available in the medical literature. There is a shortage of original data from general practice settings. This contrasts with the crucial role of GPs in fighting a pandemic. COVID-19 outbreak has opened widely new research areas, which should be explored by GPs. Maintaining the quality of care and safety of all patients during the COVID-19 pandemic is the utmost priority. Many of them suffer from poor access or inadequate management of their problems. Rapid implementation of telemedicine brought both threats and opportunities. The COVID-19 pandemic also challenged doctors’ safety and well-being. These aspects will require discussion and remedy to prevent deterioration of the quality of primary care. WONCA Europe is making a multi-faceted effort to support GPs in difficult times of the pandemic. It is ready to support future efforts to uphold the integrity of family medicine as an academic discipline.

Abstract

Patients with chronic obstructive pulmonary disease (COPD) suffer from voice changes with respect to the healthy population. However, two issues remain to be studied: how long-term speech elements such as prosody are affected; and whether physical effort and medication also affect the speech of patients with COPD, and if so, how an automatic speech-based detection system of COPD measurements can be influenced by these changes. The aim of the current study is to address both issues. To this end, long read speech from COPD and control groups was recorded, and the following experiments were performed: (a) a statistical analysis over the study and control groups to analyse the effects of physical effort and medication on speech; and (b) an automatic classification experiment to analyse how different recording conditions can affect the performance of a COPD detection system. The results obtained show that speech—especially prosodic features—is affected by physical effort and inhaled medication in both groups, though in opposite ways; and that the recording condition has a relevant role when designing an automatic COPD detection system. The current work takes a step forward in the understanding of speech in patients with COPD, and in turn, in the research on its automatic detection to help professionals supervising patient status.

Lifestyle changes and a healthy diet are still the cornerstone in the clinical management of these patients as approved medications are presently lacking.

Abstract

Background: Colorectal cancer (CRC) is the third most commonly diagnosed malignant cancer and the fourth leading cause of cancer-related death in the world. The role of family doctors in the diagnosis and management of gastrointestinal disorders, including screening for CRC, is crucial. Aim of the study: To examine the appropriateness of colonoscopy referrals in a primary healthcare setting. Material and methods: The clinical reports of all colonoscopies ordered during 2017 at a primary care center (317 in total) were examined. The reasons for referral, the findings of the colonoscopies, and the pathology of identified polyps were extracted from the reports and summarized. Results: The patients included 161 males (50.8%) and 156 females (49.2%) with a mean age of 63.6 years. The most frequent reasons for ordering a colonoscopy were a personal history of polyposis/CRC (n = 94, 29.7%), rectal bleeding (n = 57, 18%) and anemia (n = 35, 11%). Only 67 (21%) of the colonoscopies performed showed a normal result, whereas 250 (79%) had abnormal findings. The main findings of the colonoscopies were polyps (n = 119, 37.5%), diverticulosis (n = 61, 19.2%) and hemorrhoids (n = 37, 11.7%). The main results for polyp pathology were tubular adenoma with low-grade dysplasia (n = 70, 58.8%), hyperplastic polyp (n = 27, 22.7%), adenocarcinoma (n = 9, 7.6%) and inflammatory polyp (n = 4, 3.4%). Colorectal cancer was found in a total of 14 (4.4%) Conclusions: patients.The referral for colonoscopy by family doctors is appropriate.

Abstract

BACKGROUND Individuals from rural areas are increasingly using social media as a means of communication, receiving information, or actively complaining of inequalities and injustices. OBJECTIVE The aim of our study is to analyze conversations about rural health taking place on Twitter during a particular phase of the COVID-19 pandemic. METHODS This study captured 57 days’ worth of Twitter data related to rural health from June to August 2021, using English-language keywords. The study used social network analysis and natural language processing to analyze the data. RESULTS It was found that Twitter served as a fruitful platform to raise awareness of problems faced by users living in rural areas. Overall, Twitter was used in rural areas to express complaints, debate, and share information. CONCLUSIONS Twitter could be leveraged as a powerful social listening tool for individuals and organizations that want to gain insight into popular narratives around rural health.

Abstract

Death is medically described as the irreversible loss of essential characteristics and requires irreversible loss of capacity for consciousness and capacity to breathe. Death certification has legal, ethical, philosophical, biological, cultural, and religious characteristics and consequences. Its diagnosis is based on the best available scientific evidence: somatic, circulatory and/or neurological criteria. Even if we accept the determination of death is “scientific” and a set of criteria for its diagnosis have been established, death diagnosis still differs among countries. It is for this reason and sporadic cases like the one occurred recently in Spain, that “artistic” aspects of medicine and the mystery of the extensively described condition catalepsy revive. Catalepsy is a medical condition characterized by inactivity, decreased responsiveness to external stimuli, and a tendency to maintain an immobile posture (muscular rigidity). This condition may be associated with psychotic disorders, nervous system drug toxicity, and other conditions. Moreover, its influence in artistic literature and mass media have led to the consideration of catalepsy as a cornerstone in the confluence of medicine, art, mystery, science and society.

After the first weeks of vaccination against SARS-CoV-2, several cases of acute thrombosis were reported. These news reports began to be shared frequently across social media platforms. The aim of this study was to conduct an analysis of Twitter data related to the overall discussion. The data were retrieved from 14 March to 14 April 2021 using the keyword ‘blood clots’. A dataset with n = 266,677 tweets was retrieved, and a systematic random sample of 5% of tweets (n = 13,334) was entered into NodeXL for further analysis. Social network analysis was used to analyse the data by drawing upon the Clauset–Newman–Moore algorithm. Influential users were identified by drawing upon the betweenness centrality measure. Text analysis was applied to identify the key hashtags and websites used at this time. More than half of the network comprised retweets, and the largest groups within the network were broadcast clusters in which a number of key users were retweeted. The most popular narratives involved highlighting the low risk of obtaining a blood clot from a vaccine and highlighting that a number of commonly consumed medicine have higher blood clot risks. A wide variety of users drove the discussion on Twitter, including writers, physicians, the general public, academics, celebrities, and journalists. Twitter was used to highlight the low potential of developing a blood clot from vaccines, and users on Twitter encouraged vaccinations among the public.

However, two issues remain to be studied: how long-term speech elements such as prosody are affected; and whether physical effort and medication also affect the speech of patients with COPD, and if so, how an automatic speech-based detection system of COPD measurements can be influenced by these changes. The aim of the current study is to address both issues. To this end, long read speech from COPD and control groups was recorded, and the following experiments were performed: (a) a statistical analysis over the study and control groups to analyse the effects of physical effort and medication on speech; and (b) an automatic classification experiment to analyse how different recording conditions can affect the performance of a COPD detection system. The results obtained show that speech—especially prosodic features—is affected by physical effort and inhaled medication in both groups, though in opposite ways; and that the recording condition has a relevant role when designing an automatic COPD detection system. The current work takes a step forward in the understanding of speech in patients with COPD, and in turn, in the research on its automatic detection to help professionals supervising patient status. Abstract

The article describes evaluation of the first year of implementation of a process-management project and a quality-improvement programme linked to economic incentives carried out at CAPSE (Consortium of Primary Healthcare of Eixample, Barcelona City, Catalonia, Spain).To evaluate the changes and to describe the experience of a variable payment scheme linked to quality objectives in two primary healthcare centres in Spain.Data from a variable payment scheme for professionals, where the variable payment consisted of three parts, were analysed. The three areas included in the variable payment were the results of management by objectives, performance evaluation, and participation in a quality-improvement programme. Parallel to this the actions of continuous quality improvement that had been proposed during 2006 were described and evaluated.Participation among personnel in the quality-improvement programme was high, including 96% of doctors and 100% of nurses. Seventy-two improvement actions were proposed and accepted; 56% of the improvement actions took place within the established period of time, 18% did not finish within the deadline and 26% could not be accomplished for various reasons.The following areas needing improving were detected: process-management training for healthcare professionals, process orientation to the patients, the need to develop a communication plan, the selection of process and outcomes indicators, appropriate use of information systems and the time spent in implementation of the quality-improvement programme.Our preliminary results are encouraging. More studies and comparison of similar experiences are required before widespread use of this system can be recommended.
