ZusammenfassungMitglieder der Österreichischen Gesellschaft für Pneumologie (ÖGP) beschreiben die erwartete Entwicklung der respiratorischen Gesundheit und zeigen Wege einer patientenorientierten und kosteneffizienten Versorgung für Österreich auf.Methoden: Im November 2017 trifft sich eine Gruppe von respiratorisch tätigen Ärzten, Pflegekräften und Physiotherapeuten, gemeinsam mit Vertretern von Selbsthilfe-Gruppen und Experten für Gesundheitsentwicklung, um die Themen Rahmenbedingungen für Lungengesundheit, Asthma und Allergie, COPD, Mukoviszidose und andere Krankheitsbilder der pädiatrischen Pneumologie, Infektionen in der Pneumologie, Schlafbezogene Atemstörungen, Interventionelle Pneumologie, Thorakale Onkologie, und Orphan Diseases zu bearbeiten.Ergebnisse: Respiratorische Erkrankungen sind extrem häufig und werden sehr oft durch individuelles Fehlverhalten (Zigarettenrauchen, Überernährung, körperliche Inaktivität) verursacht. Zudem wird für respiratorische Erkrankungen eine Zunahme der Prävalenz, aber eine Abnahme der Hospitalisierungen erwartet. Folgende Maßnahmen sind erforderlich, um den Herausforderungen der Zukunft zu begegnen.1. Maßnahmen des Screenings und der Fallfindung sollen für Lungenkarzinom und COPD umgesetzt werden.2. E-Health (Telemedizin, personalisierte Apps) soll vermehrt eingesetzt werden mit dem Ziel das Patientenmanagement zu erleichtern.3. Regionale Unterschiede der medizinischen Versorgung können durch E‑Health und Harmonisierung der Angebote der Gesundheitskassen reduziert werden.4. Patientenschulung und Awareness für respiratorische Erkrankungen soll verbessert werden (essentiell für Schlafstörungen, aber auch für andere respiratorische Erkrankungen)5. Die angebotene Versorgung soll interprofessionell, und auf der Grundlage erkrankungsspezifischer Boards wie bei Tumorboards (z. B. für interstitielle Lungenerkrankungen, Schlaf und Allergie) erfolgen.6. Die ambulante Rehabilitation im niedergelassenen Bereich kann einen großen Beitrag zur respiratorischen Gesundheit leisten.7. Das vermehrte Verständnis der Molekularbiologie wird die personalisierte Medizin und zielgerichtete Therapien (z. B. für Asthma, Lungenkarzinom) ermöglichen, aber auch Gesundheitskosten verändern.

SummaryThe Austrian Society of Pneumology (ASP) launched a first statement on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in May 2020, at a time when in Austria 285 people had died from this disease and vaccinations were not available. Lockdown and social distancing were the only available measures to prevent more infections and the breakdown of the health system. Meanwhile, in Austria over 13,000 patients have died in association with a SARS-CoV‑2 infection and coronavirus disease 2019 (COVID-19) was among the most common causes of death; however, SARS-CoV‑2 has been mutating all the time and currently, most patients have been affected by the delta variant where the vaccination is very effective but the omicron variant is rapidly rising and becoming predominant. Particularly in children and young adults, where the vaccination rate is low, the omicron variant is expected to spread very fast. This poses a particular threat to unvaccinated people who are at elevated risk of severe COVID-19 disease but also to people with an active vaccination. There are few publications that comprehensively addressed the special issues with SARS-CoV‑2 infection in patients with chronic lung diseases. These were the reasons for this updated statement. Pulmonologists care for many patients with an elevated risk of death in case of COVID-19 but also for patients that might be at an elevated risk of vaccination reactions or vaccination failure. In addition, lung function tests, bronchoscopy, respiratory physiotherapy and training therapy may put both patients and health professionals at an increased risk of infection. The working circles of the ASP have provided statements concerning these risks and how to avoid risks for the patients.

Scientific Members of the Austrian Society of Pneumology describe the expected development in respiratory health and provide guidance towards patient-oriented and cost-efficient respiratory care in Austria.Methods: In November 2017, respiratory care providers (physicians, nurses, physiotherapists) together with patient's advocacy groups and experts in health development, collaborated in workshops on: respiratory health and the environment, bronchial asthma and allergy, COPD, pediatric respiratory disease, respiratory infections, sleep disorders, interventional pneumology, thoracic oncology and orphan diseases.Results: Respiratory disease is extremely prevalent and driven by ill-health behavior, i.e. cigarette smoking, over-eating and physical inactivity. For the majority of respiratory diseases increased prevalence, but decreased hospitalizations are expected.The following measures should be implemented to deal with future challenges:1. Screening and case-finding should be implemented for lung cancer and COPD.2. E-health solutions (telemedicine, personal apps) should be used to facilitate patient management.3. Regional differences in respiratory care should be reduced through E‑health and harmonization of health insurance benefits across Austria.4. Patient education and awareness, to reduce respiratory health illiteracy should be increased, which is essential for sleep disorders but relevant also for other respiratory diseases.5. Respiratory care should be inter-professional, provided via disease-specific boards beyond lung cancer (for ILDs, sleep, allergy)6. Programs for outpatient's pulmonary rehabilitation can have a major impact on respiratory health.7. Increased understanding of molecular pathways will drive personalized medicine, targeted therapy (for asthma, lung cancer) and subsequently health care costs.

Objectives Ceftaroline (active form of the prodrug ceftaroline fosamil) is a novel cephalosporin with activity against pathogens commonly associated with community-acquired pneumonia (CAP), including Streptococcus pneumoniae and Gram-negative pathogens. This randomized, double-blind, Phase III study evaluated the efficacy and safety of ceftaroline fosamil in treating patients with CAP. The primary objective was to determine non-inferiority [lower limit of 95% confidence interval (CI) ≥ -10%] of clinical cure rates achieved with ceftaroline fosamil compared with those achieved with ceftriaxone in the clinically evaluable (CE) and modified intent-to-treat efficacy (MITTE) populations. Methods Patients hospitalized in a non-intensive care unit setting with CAP of Pneumonia Outcomes Research Team (PORT) risk class III or IV requiring intravenous (iv) therapy were randomized (1:1) to receive 600 mg of ceftaroline fosamil iv every 12 h or 1 g of ceftriaxone iv every 24 h. Clinical cure, microbiological response, adverse events (AEs) and laboratory tests were assessed. FOCUS 2 registration number NCT00509106 (http://clinicaltrials.gov/ct2/show/NCT00509106). Results The study enrolled 627 patients, 315 of whom received ceftaroline fosamil and 307 of whom received ceftriaxone. Patients in both treatment groups had comparable baseline characteristics. Clinical cure rates were as follows: CE population, 82.1% (193/235) for ceftaroline fosamil and 77.2% (166/215) for ceftriaxone [difference (95% CI), 4.9% (-2.5, 12.5)]; and MITTE population, 81.3% (235/289) for ceftaroline fosamil and 75.5% (206/273) for ceftriaxone [difference (95% CI), 5.9% (-1.0, 12.7)]. Clinical cure rates for CAP caused by S. pneumoniae in the microbiological MITTE (mMITTE) population were 83.3% (35/42) and 70.0% (28/40) for ceftaroline fosamil and ceftriaxone, respectively. Ceftaroline fosamil and ceftriaxone were well tolerated, with similar rates of AEs, serious AEs, deaths and discontinuations due to an AE. The most common AEs for ceftaroline fosamil-treated patients were diarrhoea, headache, hypokalaemia, insomnia and phlebitis, and the most common AEs for ceftriaxone-treated patients were diarrhoea, insomnia, phlebitis and hypertension. Conclusions Ceftaroline fosamil achieved high clinical cure and microbiological response rates in patients hospitalized with CAP of PORT risk class III or IV. Ceftaroline fosamil was well tolerated, with a safety profile that is similar to that of ceftriaxone and other cephalosporins. Ceftaroline fosamil is a promising agent for the treatment of CAP.

Introduction: Efficacy and safety of the fixed-dose combination (FDC) of tiotropium (T), a long-acting muscarinic antagonist, + olodaterol (O), a long-acting β 2 -agonist, has previously been established in pivotal Phase III studies in COPD. The present studies (OTEMTO 1 and 2) evaluated FEV 1 and SGRQ score after 12 weeks of treatment with T+O FDC (2.5/5 µg; 5/5 µg) and T 5 µg compared to placebo (P) in patients with moderate to severe COPD. Methods: Two randomised, double-blind, parallel-group, Phase IIIb studies assessed the efficacy and safety of T 5 µg, T+O FDC 2.5/5 µg or 5/5 µg compared to P. Key inclusion criteria were age ≥40 years, diagnosis of COPD and post-bronchodilator FEV 1 ≥30 and Results: 1621 patients were evaluated. Patient characteristics were similar between OTEMTO 1 and 2: mean post-bronchodilator FEV 1 was 1.52 L and 1.55 L [55.3% and 54.8% predicted], respectively. Both studies met primary end points and significantly improved FEV 1 AUC 0–3 , trough FEV 1 and SGRQ with all treatments versus P (Table). There were no relevant safety findings. Conclusions: These studies show a consistent clinically meaningful improvement in SGRQ with T+O FDC 5/5 µg versus P, while confirming its effects on lung function parameters and safety. BackgroundTiotropium + olodaterol improves lung function and symptoms compared to monotherapies in chronic obstructive pulmonary disease (COPD). The OTEMTO 1 and 2 studies investigated the effects of tiotropium + olodaterol on lung function and health-related quality of life compared to placebo in patients with moderate to severe COPD.MethodsIn these two replicate, double-blind, parallel-group, placebo-controlled trials, patients were randomised to receive tiotropium + olodaterol 5/5 μg, 2.5/5 μg, tiotropium 5 μg or placebo for 12 weeks, via the Respimat® inhaler.
