O tratamento dos transtornos alimentares (TA) geralmente exige uma abordagem multidisciplinar em que a farmacoterapia é adjuvante de abordagens psicológicas e nutricionais. Psicotrópicos são indicados para a maioria dos pacientes com TA para tratar as comorbidades e também os sintomas chamados nucleares. Progressos importantes estão ocorrendo nos últimos anos. Este artigo apresenta uma revisão das evidências atuais e perspectivas futuras para o tratamento farmacológico da anorexia nervosa, bulimia nervosa e do transtorno da compulsão alimentar periódica.The treatment of eating disorders (ED) usually involves a multidisciplinary approach and pharmacotherapy is adjunctive to psychological and nutritional interventions. Psychotropic agents are prescribed for most patients with ED to treat both the comorbid conditions and ED core symptoms. Important progresses have occurred in the last years. We present an overview of the current evidences and future directions in the pharmacological treatment of anorexia nervosa, bulimia nervosa and binge eating disorder.

Este artigo descreve os principais instrumentos e métodos utilizados para mensurar os aspectos psicopatológicos gerais e específicos dos transtornos alimentares. São revistos treze instrumentos auto-aplicáveis e duas entrevistas clínicas semi-estruturadas. Os autores apresentam para cada instrumento de avaliação uma breve descrição de sua estrutura, sua utilização preferencial e se existe versão para o português. Ao final do artigo, é apresentada uma sugestão de protocolo psicométrico para anorexia nervosa, bulimia nervosa e transtorno da compulsão alimentar periódica.

INTRODUÇÃO/OBJETIVOS: Descreve-se o processo de tradução e adaptação, para a língua portuguesa, da Binge Eating Scale (BES) -- Escala de Compulsão Alimentar Periódica (ECAP) --, que avalia a gravidade da compulsão alimentar periódica em indivíduos obesos. O objetivo foi traduzir, adaptar e avaliar a aplicabilidade da versão para o português da ECAP. MÉTODOS: Após cuidadoso processo de tradução e adaptação para a língua portuguesa, foi obtida uma versão final da ECAP. Para avaliar sua aplicabilidade, foi realizado um pré-teste em um grupo de 32 pacientes obesos com transtorno da compulsão alimentar periódica (TCAP) e que procuravam tratamento para emagrecer. RESULTADOS: Os pacientes compreenderam adequadamente os itens da ECAP. A média de pontuação da ECAP nos pacientes obesos com TCAP foi de 31,2 (±5,8). CONCLUSÃO: A versão final para o português da ECAP foi considerada adequada para uso clínico.

Purpose: A growing body of evidence has shown that electroencephalography (EEG) is an interesting method of assessing the underlying brain physiology associated with disordered eating. Using EEG, we sought to evaluate brain reactivity to hyper-palatable food cues in undergraduate students with disordered eating behavior (DEB). Methods: After assessing the eating behaviors of twenty-six undergraduate students using the Eating Attitudes Test (EAT-26), electroencephalographic signals were recorded while the participants were presented with pictures of hyper-palatable food. The current study used a temporospatial principal component analysis (PCA) approach to identify event-related potential (ERP) responses that differed between DEB and non-DEB individuals. Results: A temporospatial PCA applied to the ERPs identified a positivity with a maximum amplitude at 347 ms at the occipital–temporal electrodes in response to pictures of hyper-palatable food. This positivity was correlated with the EAT-26 scores. Participants with DEB showed reduced positivities in this component compared with those without DEB. Conclusion: Our findings may reflect greater motivated attention toward hyper-palatable food cues in undergraduate students with DEB. These results are an important step toward obtaining a more refined understanding of specific abnormalities related to reactivity to food cues in this population.

OBJECTIVE: To investigate the relationship between binge-eating episodes and a perception that body weight is above the ideal in a sample of customers interviewed at shopping malls in five Brazilian cities. METHODS: In 1999, data were collected over the course of one week (Monday-Friday only) at the largest shopping malls in the cities of Porto Alegre, Salvador, Fortaleza, Goiânia and Curitiba (two malls per city). A total of 2855 participants (917 men and 1938 women) were interviewed. Weight and height measurements were standardized. Binge-eating episodes were identified using a questionnaire including the following questions based on DSM-IV diagnostic criteria: "Have you ever eaten, in a period of two hours or less, an amount of food greater than that most people would eat?" and "If the answer was "yes", did you, during these episodes, feel unable to stop eating or to control how much you were eating?". RESULTS: The prevalence of binge-eating episodes was higher among overweight subjects (15.6%) compared with normal-weight subjects (9.9%) (p = 0.0001) and, among subjects who perceived their body weight to be above the ideal (men: 13.9%; women: 15.1%) compared with those who perceived their body weight to be ideal or below the ideal (men: 8%; women: 7%) (p < 0.0001). In the multivariate analysis adjusted for body mass index and demographic variables, binge-eating episodes were correlated with the perception that body weight is above the ideal only among women (OR: 1.8; 95% CI: 1.2-2.5). CONCLUSIONS: These findings suggest that the perception that body weight is above the ideal, regardless of overweight status, is associated with binge-eating episodes in women.

ANTECEDENTES: A etiologia dos transtornos alimentares (TA) ainda não se encontra estabelecida. Dentre os diversos fatores envolvidos, a hipótese de uma disfunção do sistema nervoso central tem sido explorada de diversas maneiras. Assim, a avaliação neuropsicológica de pacientes com TA tem por finalidade investigar a ocorrência de déficits cognitivos associados a estas patologias. OBJETIVOS: O objetivo deste artigo é avaliar o estado atual das investigações sobre a avaliação neuropsicológica em pacientes com TA. MÉTODOS: Foi realizada uma pesquisa bibliográfica - utilizando as seguintes bases de dados como estratégias de procura: MedLINE, PsychoINFO, LILACS e Cochrane Data Bank -, procurando artigos relacionados ao tema e publicados até o ano de 2004. RESULTADOS: A anorexia nervosa é o transtorno alimentar que conta com mais estudos neuropsicológicos e, de uma forma geral, os resultados apontam para déficits de atenção, déficits viso-espaciais e de viso-construção. Na bulimia nervosa, as alterações mais encontradas são déficits de atenção seletiva e das funções executivas. O transtorno da compulsão alimentar periódica ainda não conta com estudos neuropsicológicos até o momento. Após um tratamento bem-sucedido do transtorno alimentar, embora haja uma diminuição dos déficits cognitivos, algumas alterações cognitivas parecem persistir. CONCLUSÕES: Os transtornos alimentares parecem estar associados a algum grau de disfunção neuropsicológica, muito embora as funções específicas que se apresentam deficitárias variem entre os estudos encontrados, talvez em decorrência de variações metodológicas. O fato de que, após o tratamento, alguns pacientes apresentem uma melhora no funcionamento cognitivo poderia indicar que, em alguns casos, os déficits seriam funcionais. A ausência de melhora no funcionamento cognitivo de alguns pacientes, após diversas formas de intervenção, pode sugerir que estes déficits antecederiam o desenvolvimento dos transtornos alimentares, podendo, assim, contribuir para seu desenvolvimento ou para um pior prognóstico. O melhor delineamento do perfil cognitivo dos pacientes com transtorno alimentar torna-se importante para orientar abordagens terapêuticas mais seletivas.BACKGROUND: The pathophysiology of eating disorders is still unknown, with many factors possibly involved. The existence of a central nervous system (CNS) dysfunction is being investigated with particular interest. One of the most employed strategies to reach this goal is the evaluation of cognitive functioning of patients with eating disorders with neuropsychological tests. OBJECTIVE: To evaluate the current knowledge about the neuropsychology of ED. METHODS: We performed a review of several data bases (including MedLINE, PsychoINFO, LILACS and Cochrane Data Bank), using terms related to main theme of interest. The review comprised articles published up to January, 2004. RESULTS: Anorexia Nervosa (AN) was t­ ­ he most studied ED from the neuropsychological point-of-view, with studies tending to elicit attentive, visuo-spatial, and visuo-constructive deficits among such patients. On the other side, patients with Bulimia Nervosa (BN) exhibited déficits in the selective aspects of attention and in executive functions. As yet, there is no study covering the neuropsychological aspects of binge-eating disorder. After successful treatment, individuals show improvement of some cognitive deficits, while other seem to persist. CONCLUSIONS: The ED are possibly associated with a certain degree of neuropsychological dysfunction, even though there is no consesus with regard to which function is particularly impaired. The fact that some cognitive dysfunction tend to disappear after treatment argues in favor of the hypothesis that these are functional deficits. Other deficits, however, tend to persist, suggesting that they may precede the development of eating disorders or even contribute to their development or to a worse prognosis. The study of the neuropsychological aspects of ED may help tailoring more selective therapeutic approaches to patients suffering from these disorders.

The current study investigated the comprehension of mindfulness-based cognitive behavioral therapy (MBCT) by patients with resistant depression at the Psychiatry Institute of the Federal University of Rio de Janeiro, Brazil. This was the first time the model was used in the institution to treat these patients. In this study, 45 patients were invited to participate in a baseline session of MBCT that consisted in the explanation of the model and experimental exercises conducted by two experienced therapists. Twenty eight patients accepted to participate. At the end of the intervention, the patients completed a self-administered questionnaire designed by our ambulatory to assess their understanding of the method’s goals. Patients with anxiety disorder was also accessed for group comparison. More than 75% of the patients rated the intervention as comprehensible and workable. Compared to patients with depression, patients with anxiety had a better understanding of the mindfulness framework (6.5%) and the meaning of cognitive behavioral therapy (17.6%). Patients that completed the intervention described the baseline session of MBCT as comprehensive and acceptable. These results may allow possible future developments in the practice of mindfulness as a treatment applicable in many condition and settings even in the Brazilian context.

Objective: To synthesize the risk of eating disorder (ED) symptoms in Brazilian university students through a systematic review and meta-analysis. Secondary goals were to analyze whether any specific majors were related to higher ED risk and whether any regions of Brazil had higher proportions of college students at risk of ED. Methods: The procedures followed the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines, and a search was conducted in three electronic databases (MEDLINE, LILACS, and SciELO). Results: Thirty-three studies were included in the analysis, of which 14 were included in the meta-analysis. All included studies used self-report questionnaires, the most frequent of which was the Eating Attitudes Test (EAT-26). None of the studies used a structured interview to diagnose ED. A meta-analysis of studies with a cutoff ≥ 20 for the EAT-26 (n=5) found 14.9% (95%CI 12.8-17.2%) positive screenings, while those with a cutoff of t ≥ 21 (n=9) found 13.3% (95%CI 11.3-15.6%) positive screenings. There was a significantly higher proportion of positive screenings among nutrition majors than all other majors combined (26.5 and 20.5%, respectively). Conclusion: Nutrition students seem to be at higher risk of ED. Further research should investigate whether positive screenings translate to actual ED diagnoses.
