Cerebrospinal fluid (CSF) analysis very frequently makes the difference to the diagnosis, not only in relation to infections but also in other diseases of the nervous system such as inflammatory, demyelinating, neoplastic and degenerative diseases. The authors review some practical and important features of CSF analysis in infectious diseases of the nervous system, with regard to acute bacterial meningitis, herpetic meningoencephalitis, neurotuberculosis, neurocryptococcosis, neurocysticercosis and neurosyphilis.

Analysis on cerebrospinal fluid (CSF) in neurological diagnosis has always been considered to be a strong point among the main complementary examinations in Brazil. The present paper reviews the main events in the history of CSF in the neurological sciences, with emphasis on the founders of several CSF schools in our country from the beginning of the 20th century to the present time.

The "hygiene hypothesis" postulates an inverse relationship between the prevalence of parasitic infections and the frequency of multiple sclerosis (MS). Objective: It was to study whether antibodies against parasites could be demonstrated more frequently in blood serum from MS patients with oligoclonal bands (OCB) than from MS patients without OCB. Methods: We studied serum samples from 164 patients who had previously been analyzed to investigate OCB. Parasitic antibodies were studied through unidimensional electrophoresis of proteins on polyacrylamide gel against Taenia antigens, searching for antiparasitic specific low molecular weight antibodies and also for antiparasitic nonspecific high molecular weight antibodies. Results: Two of the 103 patients with no evidence of OCB had antibodies of low molecular weight and 59 of them had antibodies of high molecular weight. Of the 61 patients with evidence of OCB, one showed antibodies of low molecular weight and 16 showed antibodies of high molecular weight. Conclusion: Antiparasitic antibodies are detected with similar frequency in MS patients with OCB and in MS patients without OCB.

The question of whether HIV-1 RNA in cerebrospinal fluid (CSF) is derived from viral replication in the central nervous system or simply reflects the transit of infected lymphocytes from the blood compartment has long been a matter of debate. Some studies found no correlation between CSF and plasma viral load, whereas others did. The lack of a correlation between the two compartments suggests that the presence of HIV-1 RNA is not simply due to the passive passage of the virus from blood to CSF but rather due to intrathecal replication. To evaluate the correlation between plasma and CSF HIV-1 RNA levels and to identify situations in which there is no correlation between the two compartments, seventy patients were prospectively studied. The association between CSF and plasma viral load was evaluated in the total population and in subgroups of patients with similar characteristics. A correlation between the CSF and plasma compartments was observed for patients undergoing highly active antiretroviral therapy (HAART), those with a CD4 T lymphocyte count lower than 200 cells/mm³, and those with increased CSF protein content. On the other hand, no correlation was observed for patients without adequate virological control, who had a CD4 count higher than 200 cells/mm³ and who did not use HAART. The correlation between the two compartments observed in some patients suggests that CSF HIV-1 RNA levels may reflect plasma levels in these subjects. In contrast, the lack of a correlation between the two compartments in patients who were not on HAART and who had normal CSF proteins and a poor virological control possibly indicates compartmentalization of the virus in CSF and, consequently, plasma-independent intrathecal viral replication.

OBJECTIVE: To determine the relationship between Taenia antigen (TA) detection in cerebrospinal fluid (CSF) and magnetic resonance imaging (MRI) findings in patients with definite diagnosis of neurocysticercosis (NC). METHOD: Sixty-three patients with definite diagnosis of NC were submitted to a MRI of the brain, and to a CSF examination, with a meticulous search for TA by ELISA. RESULTS: TA detection was positive in 36 patients (57.1%). A total of 836 lesions were analyzed, greatly within the cerebral parenchyma (98.7 of the lesions). Intact or non-degenerating cysts were the most common evolutive phase observed (50.4% of all lesions), 22.1% were degenerating cysts and 19.5% calcified cysts. We observed a significant relationship between TA levels detected and the total number of lesions and the number of non-degenerating cysts, but not with calcified lesions. CONCLUSION: According to our results, we propose at least four important types of contribution: (1) TA detection may allow etiologic diagnosis in transitional phases of NC, with non-characteristic images; (2) in final stages of evolution of cysticercoids in the CNS, lesions may not appear on CT or MRI, and TA detection may contribute to a definite etiologic diagnosis; (3) TA detection may permit diagnosis of NC in some patients with previous negative tests for antibody detection in CSF; (4) TA detection may represent an accurate marker of disease activity in the epileptic form of NC.OBJETIVO: Determinar a relação entre a detecção de antígeno de Taenia (TA) no líquido cefalorraquidiano (LCR) e achados de ressonância magnética (RM) em pacientes com diagnóstico definitivo de neurocisticersose. MÉTODO: Sessenta e três pacientes com diagnóstico de NC foram submetidos a exame de RM e exame de LCR com pesquisa de antígeno de Taenia por método imunoenzimático. RESULTADOS: A detecção de TA foi positiva em 36 pacientes (57,1%). Um total de 836 lesões foram analizadas sendo 98,7% intraparemquimatosas, 50,4% dos cistos encontravam-se íntegros, 22,1% degenerados e 19,5% calcificados. Foi observada relação significativa entre a presença dos níveis de TA detectados com o número total dos cistos e também com o número de cistos íntegros. Não foi observada relação com cistos calcificados. CONCLUSÃO: (1) a detecção de TA permite o diagnóstico etiológico em formas transicionais na NC com imagem pouco característica; (2) em estágio evolutivo final de um cisticerco no sistema nervoso, este pode não aparecer na tomografia computadorizada ou RM sendo a presença do antígeno importante para confirmação diagnóstica; (3) a detecção do TA permite também o diagnóstico de NC nos casos em que as reações inumológicas são negativas; (4) a detecção do TA representa um marcador de atividade da doença nas formas epiléticas da NC.

The frequency of oligoclonal bands (OCB) restricted to the cerebrospinal fluid (CSF) from patients with multiple sclerosis (MS) varies widely in different populations. The objective of this study was to determine the frequency of these OCB in a group of MS patients in the city of São Paulo. Techniques used to detect OCB consisted of isoelectric focusing followed by immunoblotting. Oligoclonal bands were found in 49 (54.4%) out of 90 patients with clinically definite MS; in (31.2%) of the 16 patients with clinically isolated syndrome; in 7 (17.9%) of 39 patients with inflammatory disorders of the central nervous system (IDCNS), and in none of the individuals with no neurological condition (control group). The specificity of the method was 100% when compared to the control group and 82.1% when compared to the IDCNS group. These results suggest that the frequency of CSF OCB is much lower in Brazilian MS patients from São Paulo city than that reported in MS series from Caucasian populations.A frequência da detecção de bandas oligoclonais (BOC) restritas ao líquido cerebrorraquidiano (LCR) em pacientes com esclerose múltipla (EM) varia amplamente em diferentes populações. O objetivo deste estudo foi determinar a frequência destas BOC em pacientes com EM em amostra de população da cidade de São Paulo. A técnica utilizada para a detecção das BOC foi a focalização isoelétrica, seguida do immunoblotting. BOC foram detectadas: em 49 (54,4%) de 90 pacientes com EM clinicamente definida; em 5 (31,2%) de 16 pacientes com síndrome clínica isolada; em 7 (17,9%) de 30 pacientes com doenças inflamatórias do sistema nervoso central (DISNC); e em nenhum indivíduo sem doença neurológica. A especificidade do método foi 100% quando comparada ao grupo controle e 82,1% quando comparada ao grupo de DISNC. Estes resultados sugerem que a freqüência de BOC no LCR é mais baixa em pacientes da cidade de São Paulo portadores de EM do que aquelas descritas em populações caucasianas.

OBJECTIVE: (1) To determine the concentration of Taenia antigens in the cerebrospinal fluid (CSF) of patients with neurocysticercosis (NC); (2) to establish its relationship with clinical activity of the disease and with classical variables of CSF. METHOD: A CSF examination was performed in one sample from 36 patients with definitive diagnosis of NC, including: quantitative and cytomorphological study, biochemical tests, immunological reactions for cysticercosis and Taenia antigens. The antibodies for antigens detection were obtained from the larval form of Taenia crassiceps, ORF strain. After intraperitoneal passage through female mice, a group of rabbits was immunized with vesicular fluid antigens. RESULTS: The Taenia antigen was detected in CSF from 17 patients (47.2%), especially in those patients with epileptic symptoms in the last 12 months. CONCLUSION: Taenia antigens presence in CSF have significant relationship with clinically active forms of NC, being a more sensitive marker than the classic eosinophil presence.OBJETIVO: (1) Determinar a concentração de antígenos de Taenia no líquido cefalorraquidiano (LCR) em doentes com neurocisticercose; (2) estudar sua relação com a atividade clínica da doença e com as variáveis clássicas do LCR. MÉTODO: Em 36 pacientes com diagnóstico definido de neurocisticercose foi realizado exame do LCR, com estudo citológico e citomorfológico, exame bioquímico, reações imunológicas para cisticercose e detecção de antígenos de Taenia. Os anticorpos para detecção desses antígenos foram obtidos a partir da forma larvar da Taenia crassiceps, cepa ORF. Após a inoculação e proliferação intraperitoneal dessa forma larvária em ratas, foi imunizado um grupo de coelhos com seu líquido vesicular. RESULTADOS: Em 17 pacientes (47,2%) foi detectado antígeno de Taenia, especialmente naqueles com manifestação epiléptica nos últimos 12 meses. CONCLUSÃO: A detecção de antígeno de Taenia guarda relação significativa com a vigência de formas clinicamente ativas, sendo, nestas formas, marcador mais sensível que a eosinofilorraquia.

Alzheimer's disease (AD) is pathologically characterized by the accumulation of amyloid plaques and tau-associated neurofibrillary tangles in the cerebral tissue. The search for antemortem biomarkers is intense including analysis of cerebrospinal fluid (CSF) beta-amyloid and tau proteins concentrations seeking for an accurate and early diagnosis. Levels of hyperphosphorylated tau at threonine 181 were measured in the CSF of 34 patients with AD (19 with senile AD - SAD and eight with presenile AD - PSAD) and seven with other dementias (OD). The levels of CSF phosphotau were significantly higher in the AD patients compared to OD (AUC 0.812), with no association with severity of dementia, age of onset, duration of the disease or scores in the Mini-Mental State Examination. There were no differences of phosphotau levels between SAD and PSAD patients. These findings corroborate some previous studies and indicate that CSF phosphotau may help to differentiate AD from other dementias.A doença de Alzheimer (DA) se caracteriza pelo achado anátomo-patológico de acúmulo de placas senis e emaranhados neurofibrilares associados à proteína tau no tecido cerebral. A pesquisa por marcadores biológicos antemortem está focada nas concentrações das proteínas beta-amilóide e tau no líquido cefalorraqueano (LCR) objetivando um diagnóstico mais precoce e acurado da doença. Os níveis de proteína tau hiperfosforilada no sítio 181 foram determinados no LCR de 34 pacientes com DA (19 com DA senil - DAS e oito com DA pré-senil -DAPS) e sete pacientes com outras demências (OD). Os níveis de fosfotau foram significativamente mais elevados em pacientes com DA quando comparados com OD (AUC 0,812), sem relação com gravidade da demência, idade de início, duração da doença e escores do Mini-Exame do Estado Mental. Não foram observadas diferenças entre os níveis de fosfotau em pacientes com DAS e DAPS. Estes achados corroboram os dados encontrados em estudos prévios e indicam que o nível de fosfotau no LCR dos pacientes pode colaborar na diferenciação da DA com outras demências.

O Departamento Científico de LCR (DC-LCR) da Academia Brasileira de Neurologia (ABN) recomenda a adoção do Termo de Consentimento Livre e Esclarecido (TCLE) como procedimento prévio à punção para coleta de LCR, tendo como finalidade o adequado esclarecimento dos pacientes quanto aos riscos do procedimento e às medidas de prevenção de complicações do exame. O documento final do TCLE foi resultado de consenso entre os membros de DC-LCR. O DC-LCR da ABN considerou também pertinente e importante a divulgação das normas legais relacionadas à pesquisas em seres humanos em nosso país.

We report the case of a 41-year-old man with iatrogenic Creutzfeldt-Jakob disease (CJD) acquired after the use of growth hormone (GH) obtained from a number of pituitary glands sourced from autopsy material. The incubation period of the disease (from the midpoint of treatment to the onset of clinical symptoms) was rather long (28 years). Besides the remarkable cerebellar and mental signs, the patient exhibited sleep disturbance (excessive somnolence) from the onset of the symptoms, with striking alteration of the sleep architecture documented by polysomnography. 14-3-3 protein was detected in the CSF, and MRI revealed increased signal intensity bilaterally in the striatum, being most evident in diffusion-weighted (DW-MRI) sequences. This is the second case of iatrogenic CJD associated with the use of GH reported in Brazil.

This paper analyzes the diagnosis aid of the dosage of lactate in the cerebrospinal fluid (CSF) in infectious diseases of the central nervous system (CNS). We analyzed prospectively 130 samples of CSF of 116 patients with diagnoses of infectious processes in the CNS. The 130 samples of CSF were divided into five groups: 28 samples of the control group, 40 of bacterial meningitis, 22 of viral meningitis, 16 of fungal meningitis and 24 of patients presenting acquired immune deficiency syndrome (AIDS). The concentration of lactate in the CSF was elevated in the group of patients with bacterial meningitis (average = 46.2 mg/dL), fungal meningitis (average = 27.3 mg/dL) and in the AIDS group (average = 23.5 mg/dL). In the control group and viral meningitis group the lactate content in the CSF presented the reference rates according to the employed method. The lactate dosage in the CSF presented a negative correlation with glycorrhachia and positive correlation with the cellularity and total proteins of the CSF. We conclude that the lactate dosage in the CSF, although unspecific, helps to distinguish the infectious processes of the CNS.

Neurocysticercosis (NC), the presence of Taenia solium metacestodes in tissues, is the most frequent and severe parasitic infection of the central nervous system. We investigated the presence of total IgE by an automated chemiluminescence assay in 53 paired cerebrospinal fluid (CSF) and serum samples from patients with NC (P) and in 40 CSF samples from individuals with other neurological disorders as the control group (C). Total IgE concentration ranged from 1.2 to 6.6 IU/ml (mean = 1.4 IU/ml, standard deviation-sd = 1.1 IU/ml) in 28.3% of CSF samples from the P group, a value significantly higher than for the C group (£1.0 IU/ml). The serum samples from the P group showed concentrations ranging from 1.0 to 2330.0 IU/ml (mean = 224.1 IU/ml, sd = 452.1 IU/ml), which were higher than the normal value cited by the manufacturer (

We assayed samples of cerebrospinal fluid (CSF), serum and saliva from patients with neurocysticercoses, control group and individuals with other parasitoses, by ELISA with Taenia crassiceps vesicular fluid antigen (Tcra) and Taenia solium total antigen (Tso) for the detection of antibodies. The sensitivity for IgG-Tcra was 100% for CSF and serum, and 32.0% for saliva; and for IgG-Tso 100% for CSF, 80.0% for serum and 24.% for saliva. Specificity was 100% for CSF and 80.0% for serum with both antigens, and 100% for saliva with Tcra and 87.5% with Tso. The sensitivity and specificity for IgA-Tcra was, respectively, 40.0% and 100% for CSF, 36.0% and 97.1% for serum, and 4.0% and 90.0% for saliva. IgE detection showed 24.0% sensitivity and 97.1% specificity for serum, with no detection in CSF samples. The search for antibodies revealed the presence of IgG > IgA > IgE in CSF, serum and saliva samples, with IgG being present in all phases of the disease, while IgA/IgE were more frequent in the inactive form.

During the past four decades the participation of women in medicine has increased dramatically. This study is focused on the women's participation in authorship of articles published in the Arquivos de Neuro-Psiquiatria, the official Journal of the Brazilian Academy of Neurology. The articles were analyzed according to the number of articles and sex of both first and the senior (last) authors. The data were collected from 1945 to 2005. A total of 950 articles were published in this period. The proportion of women serving as first authors increased from 2.8% to 36.6% and the proportion serving as senior authors increased from 2.8% to 23.8% (1945-2005).Nas últimas quatro décadas houve um aumento crescente na participação de mulheres na medicina. Este artigo apresenta a participação de mulheres como autoras de artigos publicados em Arquivos de Neuro-Psiquiatria, jornal oficial da Academia Brasileira de Neurologia. Foi analisada esta participação em quinquênios, a partir de 1945 até 2005. Foram publicados neste período 950 artigos. A participação de mulheres como primeiro autor apresentou aumento de 2,8% (1945) para 36,6% (2005) e como último autor (chefe de grupo) passou de 2,8% (1945) para 23,8% (2005).

Estudo prospectivo incluindo 67 pacientes adultos com linfoma não-Hodgkin, considerados segundo o "Working Formulation". A população foi estudada como um todo, quer apresentasse ou não anticorpos anti-HIV no soro. Todos os pacientes foram submetidos a avaliação neurológica e o exame do líquido cefalorraqueano (LCR) foi realizado em 63 deles. Os achados neurológicos e do exame do LCR foram correlacionados. Mostraram-se significativas as associações: dor tóraco-lombar localizada e alterações do LCR (presença de células neoplásicas, aumento da concentração proteica e/ou aumento do teor de globulinas gama); anormalidade da força muscular nos membros inferiores e alterações do LCR nos pacientes HIV soro-negativos. Houve correlação entre o aparecimento de sinais de comprometimento dos nervos cranianos III, IV e VI e a detecção de células neoplásicas no LCR.

O envolvimento da doença do enxerto contra o hospedeiro crônica (DECH-C) no sistema nervoso central tem sido especulado. Há uma série de semelhanças clínicas e fisiopatológicas entre DECH-C e doenças auto-imunes, o que leva a questionar sobre a síntese intratecal de imunoglobulinas. Este estudo avalia esta síntese, em particular durante a DECH-C, de forma quantitativa e qualitativa, a fim de observar sua incidência e possível fisiopatologia. Foram estudadas amostras pareadas de LCR e soro de 33 pacientes com leucemia mielóide crônica submetidos a transplante de medula óssea (TMO) alogênico, com doador aparentado, HLA idêntico. As amostras foram coletadas nos períodos pré TMO, pós TMO e concomitante à DECH-C. Não foi evidenciada produção intratecal de IgG ou IgA nas várias fases do TMO. Apenas casos isolados evidenciaram síntese, inclusive de IgM, durante a DECH-C.The central nervous system involvement in chronic graft versus host disease (GVHD) has been suggested. Chronic GVHD resembles auto immune connective tissue disorders. In order to investigate the immunoglobulin intra blood brain barrier (BBB) synthesis during chronic GVHD, and contribute to understanding the pathophysiology of the disease, we studied 33 patients who underwent allogeneic bone marrow transplants (BMT) from HLA identical related donors. Immunoglobulin intra BBB synthesis was investigated quantitative and qualitatively. The samples were collected pre BMT, pos BMT and during chronic GVHD. There were no evidence of immunoglobulin intra BBB synthesis, and no oligoclonal bands were found. Only isolated cases suggested IgG and IgA intra BBB synthesis, and in one case IgM during GVHD.

A barreira hemato-encefálica (BHE) contribui para o isolamento imunológico do sistema nervoso central (SNC). Sua avaliação nunca foi realizada em pacientes submetidos a transplante de medula óssea (TMO). Neste estudo a integridade da BHE foi avaliada através das proteínas do LCR, de forma quantitativa, a fim de observar a incidência e entender a fisiopatologia da doença do enxerto contra o hospedeiro crônica (DECH-C) no SNC. Foram estudadas amostras pareadas de LCR e soro de 33 pacientes com leucemia mielóide crônica submetidos a TMO alogênico, de doador aparentado, HLA idêntico. As amostras foram coletadas nos períodos pré TMO, pós TMO e concomitante à DECH-C. Não foi evidenciada quebra de BHE durante a DECH-C em nenhum dos casos estudados.The blood-brain barrier (BBB) contributes to the central nervous system (CNS) immunological isolation. BBB has never been studied in patients who developed chronic graft-versus-host disease (GVHD) after allogeneic bone marrow transplants (BMT), from HLA identical related donors. BBB disruption was investigated through the cerebrospinal fluid (CSF) proteins, quantitative and graphically, in order to detect the incidence and possible pathophysiology of the CNS involvement in chronic GVHD. Thirty three CSF and matched serum samples from chronic myeloid leukemia patients were collected pre BMT, pos BMT and during chronic GVHD. There was no evidence of BBB disruption in any patient studied.

We report the case of a 27 year old man who presented to the emergency room of a hospital with headache, vomiting and an episode of loss of conciousness. A cranial CT scan was normal and the patient discharged. Ten hours later he came to the emergency room of our hospital with the same complaints. A technically difficult cisternal puncture in an anxious patient who moved during the needle introduction was done. The CSF sample showed 1600 intact red blood cells without other alterations. His headache worsened and after 6 hours he became drowsy, numb and exhibited decerebration signs. A new CT scan showed diffuse subarachnoid and intraventricular blood. An emergency angiogram demonstrated laceration of a left posterior-inferior cerebellar artery in its retrobulbar loop with a pseudoaneurysm. He was succesfully treated by surgical clipping without injury. Sixteen days later he was discharged with a normal neurological exam.Relatamos o caso de um homem de 27 anos que procurou o pronto atendimento de um hospital com cefaléia intensa, vômitos e um episódio de perda de consciência. Uma tomografia de crânio foi normal e o paciente foi dispensado. Dez horas após, o paciente procurou o setor de emergência do nosso hospital com as mesmas queixas. Uma punção suboccipital tecnicamente dificultada pela ansiedade do paciente que se movimentou durante a coleta foi realizada evidenciando amostra de LCR levemente hemorrágico. A análise do LCR mostrou presença de 1600 hemácias íntegras sem aumento de leucócitos nem alterações bioquímicas. Houve piora acentuada da cefaléia e após 6 horas apresentou sonolência, torpor e sinais de descerebração. Nova tomografia mostrou sangue no espaço subaracnóideo e nos ventrículos. Uma angiografia realizada de emergência demonstrou laceração da artéria cerebelar póstero-inferior esquerda com a formação de um pseudoaneurisma. O paciente foi de imediato operado tendo sido realizada a clipagem do pseudoaneurisma com sucesso. Dezesseis dias após o paciente teve alta com exame neurológico normal.

A determinação da atividade enzimática da adenosino-deaminase (ADA) no líquido cefalorraqueano (LCR) de pacientes com síndrome da imunodeficiência adquirida (AIDS) pode sinalizar, ainda que de modo inespecífico, a atividade de patologias associadas. Os níveis de atividade de ADA, estudados no LCR de 263 pacientes, estavam aumentados naqueles com patologias associadas em relação aos controles normais e em relação aos pacientes com AIDS que não apresentam alterações ao exame de LCR. No entanto, os níveis de ADA detectados no LCR não apresentaram diferença estatística ao se considerarem os seguintes grupos de pacientes distribuídos segundo os achados no próprio LCR: com anticorpos para toxoplasmose, sífilis ou citomegalovírus; com C. neoformans ou seus antígenos; com bacilos álcool-ácido-resistentes; com linfomas.

O presente trabalho, além da revisão da literatura sobre quimiotipagem do C. neoformans, com novos dados sobre a epidemiologia da criptococose, teve por finalidade principal a caracterização das duas variedades desta levedura em pacientes com neurocriptococose, HIV + e HIV -. As variedades neoformans e gattii estão hoje bem definidas bioquimicamente, com o emprego do meio C.G.B., proposto por KWON-CHUNG et al. (1982) 24. O isolamento do C. neoformans var. gattii das flores e folhas do Eucalyptus camaldulensis e do Eucalyptus tereticornis, na Austrália, através dos trabalhos de ELLIS & PFEIFFER (1990)16 e PFEIFFER & ELLIS (1992)41, possibilitou investigações epidemiológicas das mais interessantes sobre este microrganismo, levedura capsulada a qual SANFELICE50, 51, na Itália, em 1894 e 1895 despertou a atenção do meio médico. BUSSE8, em 1894, descrevia o primeiro caso de criptococose humana sob a forma de lesão óssea, simulando sarcoma. As pesquisas nacionais sobre o assunto em foco foram destacadas, seguindo-se a experiência dos Autores com o meio de C.G.B. (L - canavanina, glicina e azul de bromotimol). Foi possível, através deste meio o estudo de 50 amostras de líquor, sendo 39 procedentes de aidéticos (78%) e 11 de não aidéticos (22%). De pacientes HIV+, 37 (74%) foram identificados como C. neoformans var. neoformans e 2 (4%) como C. neoformans var. gattii. Dos HIV- 8 ( 16%) foram classificados como C. neoformans var. neoformans e 3 (6%) como C. neoformans var. gattii. Através deste trabalho, evidencia-se a importância da neurocriptococose, principalmente entre os aidéticos, demonstrando-se mais uma vez o interesse do meio CGB na quimiotipagem do C. neoformans em suas duas variedades, ganhando em importância a demonstração de que duas espécies de eucalipto podem funcionar como "árvores-hospedeiras" para o Cryptococcus neoformans var. gattii.The purpose of this work was to collect the main information from the literature about the biotyping of Cryptococcus neoformans. The more up-to date research concerning the epidemiology of cryptococcosis comprising quite a few articles, mainly after the advent of AIDS, was also reviewed. The Cryptococcus neoformans varieties neoformans and gattii are well defined biochemically nowadays chiefly through the C.G.B. medium, according to KWON-CHUNG et al. (1982)24. The isolation of C. neoformans var. gattii from flowers and leaves of Eucalyptus camaldulensis and Eucalyptus tereticornis, specially in Australia, through the works of ELLIS & PFEIFFER (1990) 16 and PFEIFFER & ELLIS (1992) 41 permitted very interesting epidemiological investigations on C. neoformans, a capsulaled yeast by which SANFELICE 50, 51, in Italy (1894; 1895) attracted attention of medical class. BUSSE 8, in 1894, described the first human case of cryptococcosis under the presentation of a bone lesion simulating sarcoma. In this paper, the Brazilian researchers focused on this subject were pointed out, followed by the Author's experience with the C.G.B medium (L - canavanine, glycin and bromothymol blue) proposed by KWON-CHUNG et al. (1982) 24 with very good results. It was possible with such medium the study of 50 C.N.S. liquor samples, being 39 from AIDS patients (78%) and 11 from non-AIDS ones (22%). Thirty-seven out of the 39 HIV- positive patients (74%) were identified as C. neoformans var. neoformans and 2 (4%) as C. neoformans var. gattii. From the negative HIV, 8 (16%) were classified as C. neoformans var. neoformans and 3 (6%) as C. neoformans var. gattii. We could not perform the serotyping of the above referred samples. It is evident anyway that in Brazil there exist both varieties gattii and neoformans, agents of neurocryptococcosis, including AIDS patients. The importance of neurocryptococcosis, mainly among AIDS patients, is stressed here, showing once more the value of C.G.B. medium in the typing of C. neoformans in its two varieties. Also, it is of relevant importance the demonstration that some species of eucalyptus may act as "host-trees" of C. neoformans var. gattii.

Foram utilizadas 82 LCR de transplantados renais (24 pacientes), 43 LCR de pacientes com críptococose comprovada (controles positivos), 35 LCR de pacientes com outras doenças (histoplasmose, paracoccidioidomicose e infecções bacterianas) como controles negativos. Os primeiros foram cultivados em ágar Sabouraud com sementes de girassol e juntamente com os demais examinado pelo teste de látex para pesquisa de antígeno circulante de C. neoformans, qualitativamente. O teste de Coaglutinação foi realizado qualitativamente e quantitativamente, encontrando-se títulos até a diluição 1:2048. Não foram detectadas reações falso-positivas ou falso-negativas entre os controles. Como prova de valor diagnóstico demonstrou: sensibilidade - 92,1%; especificidade - 92,6% e eficiência - 92,3%. Provou também ser um teste rápido, exato e econômico, embora sua escolha dependa do pré-tratamento de LCR (80ºC por 3 a 5 minutos) e soros (diluição ou álcali-precipitação) para evitar autoaglutinação e aumentar a sensibilidade da reação.We tested 82 CSF of 24 renal transplanted patients (RT) with cerebral cryptococcosis, 8 CSF of asymptomatic RT patients, 43 CSF of proven cryptococcosis cases (positive control) and 35 CSF of patients with other diseases (histoplasmosis, paracoccidioidomycosis and bacterial infections) as negative control. The RT CSF were cultured in Sabouraud agar slant added with sunflower seeds and both control and RT CSF were qualitatively examined by cryptococcosis latex test (Crypto-LA test). The COA test was developed both qualitatively and quantitatively. The highest titre encountered was 1:2048. No false reactions appeared among the controls. The diagnostic value demonstrated by Galen and Gambino's method was: sensitivity - 92.1%; specificity - 92.6% and efficiency - 92.3%. Besides that, the COA proved to be quick, exact and cheap, but it depends on CSF and sera pre-treatment, in order to avoid autoagglutination and increase its sensibility.

Foram estudados 68 pacientes epilépticos, com disritmia occipital no eletrencefalograma. A investigação foi desenvolvida quanto ao tipo de crise, à idade de início da doença, ao tempo de doença, à freqüência das crises e à evolução com tratamento medicamentoso, relativamente às disritmias occipitais paroxísticas e contínuas. Os resultados obtidos foram comparados com as epilepsias de modo geral e analisados em índices percentuais, tendo os autores chegado às seguintes conclusões: 1) em 65 casos (94%) havia manifestações convulsivas, sendo que em 50 (73%) as convulsões eram secundárias; 2) em apenas 7% dos casos havia sintomas visuais precedendo as convulsões; 3) a doença teve início precoce nas epilepsias occipitais em relação às epilepsias em geral; 4) o tempo de doença foi menor nas epilepsias occipitais; 5) as formas mais severas da doença predominaram nas epilepsias em geral; 6) não houve diferenças marcantes, quanto à evolução, entre as epilepsias occipitais paroxísticas e contínuas; 7) a evolução global das epilepsias occipitais não foi tão favorável quanto a das epilepsias em geral.Sixty eight epileptic patients with occipital EEG foci were studied. The investigation was developped in cases with paroxistic and continuous electroencephalographic abnormalities, concerning the age of onset and time of disease, frequency of seizures and evolution with drug treatment. The results were compared with the epilepsies in general and analysed in percentual index. The authors draws the following conclusions: 1) 65 cases (94%) exhibit convulsive seizures; in 50 cases (73%) the convulsions were secondary; 2) only 7% of patients exhibit visual aura; 3) the disease developped earlier in the occipital epilepsy than in the epilepsy in general; 4) the duration of disease was smaller in the ocipital epilepsy; 5) the disease was less severe in the occipital epilepsy than in the epilepsy in general; 6) the evolution of the paroxistic and continuous occipital epilepsy was similar; 7) the evolution of the occipital epilepsy was worse than the epilepsy in general.

Abstract This article presents a review of the recommendations on supplementary exams employed for the clinical diagnosis of Alzheimer's disease (AD) in Brazil published in 2005. A systematic assessment of the consensus reached in other countries, and of articles on AD diagnosis in Brazil available on the PUBMED and LILACS medical databases, was carried out. Recommended laboratory exams included complete blood count, serum creatinine, thyroid stimulating hormone (TSH), albumin, hepatic enzymes, Vitamin B12, folic acid, calcium, serological reactions for syphilis and serology for HIV in patients aged younger than 60 years with atypical clinical signs or suggestive symptoms. Structural neuroimaging, computed tomography or - preferably - magnetic resonance exams, are indicated for diagnostic investigation of dementia syndrome to rule out secondary etiologies. Functional neuroimaging exams (SPECT and PET), when available, increase diagnostic reliability and assist in the differential diagnosis of other types of dementia. The cerebrospinal fluid exam is indicated in cases of pre-senile onset dementia with atypical clinical presentation or course, for communicant hydrocephaly, and suspected inflammatory, infectious or prion disease of the central nervous system. Routine electroencephalograms aid the differential diagnosis of dementia syndrome with other conditions which impair cognitive functioning. Genotyping of apolipoprotein E or other susceptibility polymorphisms is not recommended for diagnostic purposes or for assessing the risk of developing the disease. Biomarkers related to the molecular alterations in AD are largely limited to use exclusively in research protocols, but when available can contribute to improving the accuracy of diagnosis of the disease.
