Abstract: PLATO (PLAnetary Transits and Oscillations of stars) is ESA's M3 mission designed to detect and characterise extrasolar planets and perform asteroseismic monitoring of a large number of stars. PLATO will detect small planets (down to <2 R_(Earth)) around bright stars (<11 mag), including terrestrial planets in the habitable zone of solar-like stars. With the complement of radial velocity observations from the ground, planets will be characterised for their radius, mass, and age with high accuracy (5 %, 10 %, 10 % for an Earth-Sun combination respectively). PLATO will provide us with a large-scale catalogue of well-characterised small planets up to intermediate orbital periods, relevant for a meaningful comparison to planet formation theories and to better understand planet evolution. It will make possible comparative exoplanetology to place our Solar System planets in a broader context. In parallel, PLATO will study (host) stars using asteroseismology, allowing us to determine the stellar properties with high accuracy, substantially enhancing our knowledge of stellar structure and evolution. The payload instrument consists of 26 cameras with 12cm aperture each. For at least four years, the mission will perform high-precision photometric measurements. Here we review the science objectives, present PLATO's target samples and fields, provide an overview of expected core science performance as well as a description of the instrument and the mission profile at the beginning of the serial production of the flight cameras. PLATO is scheduled for a launch date end 2026. This overview therefore provides a summary of the mission to the community in preparation of the upcoming operational phases.

3. Higher Education Leadership Challenges: Importance Instructional Leaders Understanding and Utilizing Emotional Intelligence to Connect and Enhance Student Academic Success

Abstract: This qualitative study examined the impact of Emotional Intelligence (EI) attributes in higher education instructional leaders and its implications on student success. Drawing from Goleman's (1995) framework of EI, which includes self-management, self-awareness, social awareness, and relationships, the study investigated how educators utilized these attributes during lectures. The context is an urban public university in Southern Texas focused on undergraduate and graduate instructional leaders. The research uncovered challenges these leaders face in promoting student success through effective EI practices in the classroom. Instructional leaders with high EI are equipped to understand and respond to the needs of their students motivating their students by tapping into their emotions and connecting the material to their interests and aspirations. Additionally, their ability to encourage and create a positive learning environment significantly impacted students' confidence and engagement with the material. The three critical themes highlighted--are motivation for students, encouragement to students, and fostering a positive environment of effective instructional leadership. Analyzing the responses of instructional leaders through open questionnaires and focus group sessions, research gain valuable insights into how EI influenced teaching practices and, ultimately, student academic success. Abstract: This project aims to develop an interactive control system for an autonomous service robot using an ROS (robot operating system). The system integrates an intuitive web interface and an interactive chatbot supported by Google Gemini to enhance the control experience and personalization for the user. The methodology includes the integration of an API (application programming interface) to access a database storing user preferences, such as speed and frequent destinations. Furthermore, the system employs facial recognition, people groups’ recognition, and adaptive responses from the chatbot for autonomous navigation, ensuring a service tailored to the individual needs of each user. To validate the proposal, it was implemented on an autonomous service robot, integrated into a motorized wheelchair. Tests demonstrated that the system effectively adjusts the wheelchair’s behavior to user preferences, resulting in safer and more personalized navigation. The use of facial recognition and chatbot interaction provided more intuitive and efficient control. The developed system significantly improves the autonomy and quality of life for wheelchair users, proving to be a viable and efficient solution for autonomous and personalized control. The results indicate that integrating technologies like ROS, intuitive web interfaces, and interactive chatbots can transform the user experience of autonomous wheelchairs, better meeting the specific needs of users.

Abstract: Abstract Background Contemporary debates about drug pricing feature several widely held misconceptions, including the relationship between incentives and innovation, the proportion of total healthcare spending on pharmaceuticals, and whether the economic evaluation of a medicine can be influenced by things other than clinical efficacy. Main body All citizens should have access to timely, equitable, and cost-effective care covered by public funds, private insurance, or a combination of both. Better managing the collective burden of diseases borne by today’s and future generations depends in part on developing better technologies, including better medicines. As in any innovative industry, the expectation of adequate financial returns incentivizes innovators and their investors to develop new medicines. Estimating expected returns requires that they forecast revenues, based on the future price trajectory and volume of use over time. How market participants decide what price to set or accept can be complicated, and some observers and stakeholders want to confirm whether the net prices society pays for novel medicines, whether as a reward for past innovation or an incentive for future innovation, are commensurate with those medicines’ incremental value. But we must also ask “value to whom?”; medicines not only bring immediate clinical benefits to patients treated today, but also can provide a broad spectrum of short- and long-term benefits to patients, their families, and society. Spending across all facets of healthcare has grown over the last 25 years, but both inpatient and outpatient spending has outpaced drug spending growth even as our drug armamentarium is constantly improving with safer and more effective medicines. In large part, this is because, unlike hospitals, drugs typically go generic, thus making room in our budgets for new and better ones, even as they often keep patients out of hospitals, driving further savings. Conclusion A thorough evaluation of drug spending and value can help to promote a better allocation of healthcare resources for both the healthy and the sick, both of whom must pay for healthcare. Taking a holistic approach to assessing drug value makes it clear that a branded drug’s value to a patient is often only a small fraction of the drug’s total value to society. Societal value merits consideration when determining whether and how to make a medicine affordable and accessible to patients: a drug that is worth its price to society should not be rendered inaccessible to ill patients by imposing high out-of-pocket costs or restricting coverage based on narrow health technology assessments (HTAs). Furthermore, recognizing the total societal cost of un- or undertreated conditions is crucial to gaining a thorough understanding of what guides the biomedical innovation ecosystem to create value for society. It would be unwise to discourage the development of new solutions without first appreciating the cost of leaving the problems unsolved.

Abstract: Abstract Introduction Children may require durable central venous catheters (CVCs) for various reasons. CVC-related integrity complications are common and can often be repaired at the bedside to increase lifetime. Variability in repair techniques can lead to complications, including the need for repeat repair and Central Line Associated Blood Stream Infection (CLABSI). Methods The impact of an educational curriculum to standardize tunneled CVC repairs for trainees on a pediatric surgery service was studied, focusing on comfort level with tunneled CVC repair and to determine the impact on complication rates. Rotating trainees studied a dedicated audiovisual educational curriculum comprised of a video, educational slides, and a practical component from November 2020 through January 2022. Experience and comfort level with tunneled CVC repairs were assessed before and after the rotation. CVCs repaired during the duration of the study were evaluated and compared to the period prior. Results Forty-nine individuals completed the pre- and post-training survey. Respondents (34.7%, n = 17) most commonly reported one year of surgical experience, and (79.6%, n = 39) had never observed or assisted in a repair previously. Abstract: Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

Abstract: Broad-band images of the solar chromosphere in the Ca II H&K line cores around active regions are covered with fine bright elongated structures called bright fibrils. The mechanisms that form these structures and cause them to appear bright are still unknown. We aim to investigate the physical properties, such as temperature, line-of-sight velocity, and microturbulence, in the atmosphere that produces bright fibrils and to compare those to the properties of their surrounding atmosphere. We used simultaneous observations of a plage region in Fe I 6301-2 \r{A}, Ca II 8542 \r{A}, Ca II K, and H$\alpha$ acquired by the CRISP and CHROMIS instruments on the Swedish 1-m Solar Telescope. We manually selected a sample of 282 Ca II K bright fibrils. We compared the appearance of the fibrils in our sample to the Ca II 8542 \r{A} and H$\alpha$ data. We performed non-local thermodynamic equilibrium (non-LTE) inversions using the inversion code STiC on the Fe I 6301-2 \r{A}, Ca II 8542 \r{A}, Ca II K lines to infer the physical properties of the atmosphere. The line profiles in bright fibrils have a higher intensity in their K$_2$ peaks compared to profiles formed in the surrounding atmosphere. The inversion results show that the atmosphere in fibrils is on average $100-200$~K hotter at an optical depth log$(\tau) = -4.3$ compared to their surroundings. The line-of-sight velocity at chromospheric heights in the fibrils does not show any preference towards upflows or downflows. The microturbulence in the fibrils is on average 0.5 km s$^{-1}$ higher compared to their surroundings. Our results suggest that the fibrils have a limited extent in height, and they should be viewed as hot threads pervading the chromosphere.

Abstract: Introduction The Advanced Neurovascular Access (ANA5, Anaconda Biomed) is a novel stroke thrombectomy catheter comprising a distal self‐expanding covered funnel designed to locally restrict blood flow in the Internal Carotid Artery (ICA) and, when used in conjuction with available stent retreiver, reduce clot fragmentation during clot ingestion. The ANAIS study aimed to investigate the safety and efficacy of the ANA5 device (‘Funnel’) in different deployment locations and aspiration conditions. Methods Prospective, single‐arm, multi‐center study to assess the safety and performance of ANA5 in combination with a stent retriever in patients with Acute Ischemic Stroke (ANAIS). An independent imaging corelab and Clinical Events Committee assessed the different predefined outcomes. Patients with anterior circulation vessel occlusion admitted within 24 hours from symptom onset were eligible. The primary efficacy endpoint was successful recanalization, defined as mTICI ≥2b in the target vessel within three passes of the Funnel device. The primary safety endpoint was a composite of symptomatic Intracranial Hemorrhage within 24 hours and any device related severe adverse event. The impact of the funnel deployment location in the different ICA segments (C1 to C4 or above) and the condition of the aspiration on angiographical outcome was also evaluated. Results A total of 39 subjects who complied with all inclusion/exlusion criteria were treated by 10 different physicians in 3 stroke centers. Mean age was 69.9 ±13.2 years, 52.5% women, and the median National Institutes of Health Stroke Scale on admission was 16.0 [12.5‐19.5]. Initial occlusion location was: TICA 25.0%, M1‐MCA 52.5% and M2‐MCA 22.5%. The rate of succesful reperfusion within 3 passes was 74.4% (29/39) (primary efficacy endpoint). First pass effect (FPE) mTICI 2b‐3 and 2c‐3 was 56.4% (22/39) and 48.7% (19/39), respectively. Zero sICHs at 24h or SADE at 5 days were reported (primary safety endpoint). At 90 days, the rate of modified Rankin Scale score 0‐2 was: 57.9% (22/38). The efficacy of the thrombectomy procedure using the Funnel increases when: 1. The funnel was deployed in the higher ICA segments (below ophtalmic artery) with diameter between 3.5‐5.0mm, ensuring full apposition of the funnel to the vessel wall: C1(n=10): 40.0% vs. C2 or higher (n=29): 86.2% (p < 0.002). 2. Continuous aspiration was applied during the full retrieval of the stentriever in the Funnel (89.3% (n=28) vs. 36.4% (n=11), p < 0.0005). 3. When both continuous aspiration and appropriate ICA location were achieved, the Clinical efficacy endpoint improved to 95.5% (n=22), and FPE mTICI 2b‐3 and 2c‐3 were 72.7% and 59.1% respectively. Conclusion The use of the next generation Funnel Device in the ANAIS study results in a high rate of successful recanalization with an optimal safety profile and favorable 90 days clinical outcomes. The performance improved when the Funnel was deployed above the C1 cervical segment of the ICA, and continuous aspiration was applied during the full stent retrieval.

Abstract: Abstract The International Staging System (ISS) and the Revised International Staging System (R-ISS) are commonly used prognostic scores in multiple myeloma (MM). These methods have significant gaps, particularly among intermediate-risk groups. The aim of this study was to improve risk stratification in newly diagnosed MM patients using data from three different trials developed by the Spanish Myeloma Group. For this, we applied an unsupervised machine learning clusterization technique on a set of clinical, biochemical and cytogenetic variables, and we identified two novel clusters of patients with significantly different survival. The prognostic precision of this clusterization was superior to those of ISS and R-ISS scores, and appeared to be particularly useful to improve risk stratification among R-ISS 2 patients. Additionally, patients assigned to the low-risk cluster in the GEM05 over 65 years trial had a significant survival benefit when treated with VMP as compared with VTD. In conclusion, we describe a simple prognostic model for newly diagnosed MM whose predictions are independent of the ISS and R-ISS scores. Notably, the model is particularly useful in order to re-classify R-ISS score 2 patients in 2 different prognostic subgroups. The combination of ISS, R-ISS and unsupervised machine learning clusterization brings a promising approximation to improve MM risk stratification.

This is a common situation in social robotics, due to the high variety of the sensory capacities of robots; hence, more flexible multimodal models are needed. In this context, we propose an adaptive and flexible emotion recognition architecture able to work with multiple sources and modalities of information and manage different levels of data quality and missing data, to lead robots to better understand the mood of people in a given environment and accordingly adapt their behaviour. Each modality is analyzed independently to then aggregate the partial results with a previous proposed fusion method, called EmbraceNet+, which is adapted and integrated to our proposed framework. We also present an extensive review of state-of-the-art studies dealing with fusion methods for multimodal emotion recognition approaches. We evaluate the performance of our proposed architecture by performing different tests in which several modalities are combined to classify emotions using four categories (i.e., happiness, neutral, sadness, and anger). Results reveal that our approach is able to adapt to the quality and presence of modalities. Furthermore, results obtained are validated and compared with other similar proposals, obtaining competitive performance with state-of-the-art models.

Abstract: La hepatitis alcohólica es la inflamación aguda del hígado secundaria al consumo de alcohol en cantidades hepatotóxicas, su fisiopatología está influida por diversos factores asociados. El diagnóstico tiene tres escenarios: la hepatitis alcohólica probable, posible y definitiva. La hepatitis alcohólica probable se basa en el cumplimiento de criterios diagnósticos clínicos y laboratoriales; la hepatitis alcohólica posible corresponde a casos de cumplimiento de estos criterios, pero con presencia de factores potencialmente confusores del diagnóstico, y la definitiva se sustenta sobre una base histopatológica. El síndrome de Budd-Chiari es una entidad poco frecuente, heterogénea y potencialmente letal, caracterizada por la presencia de trombos a nivel de las venas suprahepáticas, así como de la vena cava inferior. El diagnóstico definitivo está basado en pruebas de imagen. Por su parte, el síndrome de Budd-Chiari secundario es una entidad aún más infrecuente, poco estudiada, cuyo diagnóstico es difícil debido a su gran similitud al síndrome de Budd-Chiari primario, tanto clínica como en imágenes diagnósticas, por lo que se tiene que acudir a métodos diagnósticos de mayor complejidad e incluso invasivos. A continuación, se presenta un caso inusual de un paciente con consumo crónico de alcohol que presenta síndrome de Budd-Chiari secundario asociado a hepatitis alcohólica grave confirmada mediante biopsia hepática y sometido a múltiples estudios de imagen que descartaron la presencia de trombosis a nivel de las venas suprahepáticas.

Abstract: Abstract Objective Emergency general surgery (EGS) patients presenting with sepsis remain a challenge. The Surviving Sepsis Campaign recommends a 30 mL/kg fluid bolus in these patients, but recent studies suggest an association between large volume crystalloid resuscitation and increased mortality. The optimal amount of pre-operative fluid resuscitation prior to source control in patients with intra-abdominal sepsis is unknown. This study aims to determine if increasing volume of resuscitation prior to surgical source control is associated with worsening outcomes. Methods We conducted an 8-year retrospective chart review of EGS patients undergoing surgery for abdominal sepsis within 24 h of admission. Patients in hemorrhagic shock and those with outside hospital index surgeries were excluded. We grouped patients by increasing pre-operative resuscitation volume in 10 ml/kg intervals up to > 70 ml/kg and later grouped them into 30 ml/kg (p = 0.02). These groups had median qSOFA scores (1.0 vs. 1.0, p = 0.06). There were no differences in time to operation (6.1 vs 4.9 h p = 0.11), ventilator days (1 vs 3, p = 0.08), or hospital LOS (8 vs 9 days, p = 0.57). Relative risk regression correcting for age and physiologic factors showed no significant differences in mortality between the fluid groups. Conclusions Greater pre-operative resuscitation volumes were initially associated with significantly higher mortality, despite similar organ failure scores. However, fluid volumes were not associated with mortality following adjustment for other physiologic factors in a regression model. The amount of pre-operative volume resuscitation was not associated with differences in time to operation, ventilator days, ICU or hospital LOS.

Abstract: SPIRou is a near-IR \'echelle spectropolarimeter and high-precision velocimeter under construction as a next-generation instrument for the Canada-France-Hawaii-Telescope. It is designed to cover a very wide simultaneous near-IR spectral range (0.98-2.35 {\mu}m) at a resolving power of 73.5K, providing unpolarized and polarized spectra of low-mass stars at a radial velocity (RV) precision of 1m/s. The main science goals of SPIRou are the detection of habitable super-Earths around low-mass stars and the study of how critically magnetic fields impact star / planet formation. Following a successful final design review in Spring 2014, SPIRou is now under construction and is scheduled to see first light in late 2017. Abstract: Abstract The malignant energetic demands are satisfied through glycolysis, glutaminolysis and de novo synthesis of fatty acids, while the host curses with a state of catabolism and systemic inflammation. The concurrent inhibition of both, tumor anabolism and host catabolism, and their effect upon tumor growth and whole animal metabolism, have not been evaluated. We aimed to evaluate in colon cancer cells a combination of six agents directed to block the tumor anabolism (orlistat + lonidamine + DON) and the host catabolism (growth hormone + insulin + indomethacin). Treatment reduced cellular viability, clonogenic capacity and cell cycle progression. These effects were associated with decreased glycolysis and oxidative phosphorylation, leading to a quiescent energetic phenotype, and with an aberrant transcriptomic landscape showing dysregulation in multiple metabolic pathways. The in vivo evaluation revealed a significant tumor volume inhibition, without damage to normal tissues. The six-drug combination preserved lean tissue and decreased fat loss, while the energy expenditure got decreased. Finally, a reduction in gene expression associated with thermogenesis was observed. Our findings demonstrate that the simultaneous use of this six-drug combination has anticancer effects by inducing a quiescent energetic phenotype of cultured cancer cells. Besides, the treatment is well-tolerated in mice and reduces whole animal energetic expenditure and fat loss.
