Resumen Objetivo Presentar nuestra experiencia en el seguimiento clinico de pacientes sometidos a bypass gastroileal. Metodo Descripcion de los 6 casos en seguimiento por nuestra unidad tras haberse sometido a bypass gastroileal en otro centro privado. Resultados Los 6 pacientes presentaron complicaciones, destacando el fallecimiento de una paciente por desnutricion severa y la necesidad de reconversion quirurgica de otra por el mismo motivo, la inestabilizacion de la diabetes mellitus tipo 1 de otra paciente y el deficit de vitaminas liposolubles en el 100% de los mismos. Conclusiones Son escasas las publicaciones que avalan la seguridad del bypass gastroileal como tratamiento de la obesidad. En nuestra experiencia es una tecnica asociada a una alta tasa de complicaciones graves. Al ser una tecnica por ahora no estandarizada, consideramos que estos casos debieran intervenirse en centros donde exista un equipo multidisciplinar experto en el manejo de las posibles complicaciones, con un seguimiento estrecho por cirujanos y endocrinologos.

To present our experience in the clinical follow-up of patients undergoing a gastric bypass.Description of six cases under follow-up by our unit after undergoing a gastric bypass at another private centre.The 6 patients presented complications, the most notable being the death of one patient due to severe malnutrition and the need for revision surgery in another for the same reason, destabilisation of type 1 diabetes mellitus in another patient and fat-soluble vitamin deficiency in all of them.There are few publications that support the safety of gastric bypass as a treatment for obesity. In our experience, it is a technique associated with a high rate of serious complications. As it is a technique that is not yet standardised, we consider that these cases should be operated on in centres where there is a multidisciplinary team with expertise in the management of possible complications, with close follow-up by surgeons and endocrinologists.

Objectives: We aimed to describe and compare the complications associated with different percutaneous radiologic gastrostomy (PRG) techniques. Methods: A retrospective and prospective observational study was conducted. Patients who underwent a PRG between 1995–2020 were included. Techniques: A pigtail catheter was used until 2003, a balloon catheter without pexy was used between 2003–2009 and a balloon catheter with gastropexy was used between 2015–2021. For the comparison of proportions, X2 tests or Fisher’s test were used when necessary. Univariate analysis was performed to study the risk factors for PRG-associated complications. Results: n = 330 (pigtail = 114, balloon-type without pexy = 28, balloon-type with pexy = 188). The most frequent indication was head and neck cancer. The number of patients with complications was 44 (38.5%), 11 (39.2%) and 54 (28,7%), respectively. There were seven (25%) cases of peritonitis in the balloon-type without-pexy group and 1 (0.5%) in the balloon-type with-pexy group, the latter being the only patient who died in the total number of patients (0.3%). Two (1%) patients of the balloon-type with-pexy group presented with gastrocolic fistula. The rest of the complications were minor. Conclusions: The most frequent complications associated with the administration of enteral nutrition through PRG were minor and the implementation of the balloon-type technique with pexy has led to a decrease in them.

In order to develop evidence-based recommendations and expert consensus for the nutritional management of patients with short bowel syndrome (SBS), we conducted a systematic literature search using the PRISMA methodology plus a critical appraisal following the GRADE scale procedures. Pharmacological treatment with antisecretory drugs, antidiarrheal drugs, and somatostatin contributes to reducing intestinal losses. Nutritional support is based on parenteral nutrition; however, oral intake and/or enteral nutrition should be introduced as soon as possible. In the chronic phase, the diet should have as few restrictions as possible, and be adapted to the SBS type. Home parenteral nutrition (HPN) should be individualized. Single-lumen catheters are recommended and taurolidine should be used for locking the catheter. The HPN's lipid content must be greater than 1 g/kg per week but not exceed 1 g/kg per day, and omega-6 fatty acids (ω6 FAs) should be reduced. Trace element vials with low doses of manganese should be used. Patients with chronic SBS who require long-term HPN/fluid therapy despite optimized treatment should be considered for teduglutide treatment. All patients require a multidisciplinary approach and specialized follow-up. These recommendations and suggestions regarding nutritional management in SBS patients have direct clinical applicability.Con el fin de desarrollar recomendaciones basadas en la evidencia y el consenso de expertos para el manejo nutricional de los pacientes con síndrome de intestino corto (SIC), realizamos una búsqueda bibliográfica sistemática utilizando la metodología PRISMA junto a una valoración crítica siguiendo los procedimientos de la escala GRADE. El tratamiento farmacológico con fármacos antisecretores, antidiarreicos y somatostatina contribuye a reducir las pérdidas intestinales. El apoyo nutricional se basa en la nutrición parenteral; sin embargo, la ingesta oral y/o la nutrición enteral deben introducirse lo antes posible. En la fase crónica, la dieta debe tener las menores restricciones posibles y adaptarse al tipo de SIC. La nutrición parenteral domiciliaria (NPD) debe individualizarse. Se recomiendan catéteres de un solo lumen y se debe utilizar taurolidina para bloquear el catéter. El contenido de lípidos de la HPN debe ser superior a 1 g/kg por semana, pero no debe exceder 1 g/kg por día, y debe reducirse el ácido graso omega-6 (AG ω6). Deben utilizarse viales de oligoelementos con dosis bajas de manganeso. Los pacientes con SIC crónico que requieren NPD/fluidoterapia a largo plazo a pesar del tratamiento optimizado deben considerarse para el tratamiento con teduglutida. Todos los pacientes requieren un abordaje multidisciplinar y un seguimiento especializado. Estas recomendaciones y sugerencias con respecto al manejo nutricional de los pacientes con SIC tienen aplicabilidad clínica directa.

Aim: to communicate the home parenteral nutrition (HPN) data obtained from the HPN registry of the NADYA-SENPE group (www.nadya-senpe.com) for the year 2019. Material and methods: a descriptive analysis of the data collected from adult and pediatric patients with HPN in the NADYA-SENPE group registry from January 1 to December 31, 2019. Results: a total of 283 patients (51.9 % women), 31 children, and 252 adults from 47 Spanish hospitals were registered, which represents a prevalence rate of 6.01 patients per million inhabitants for year 2019. The most frequent diagnosis in adults was "palliative oncological" and "others" (21.0 %). In children, it was Hirschsprung's disease together with necrotizing enterocolitis, alterations in intestinal motility, and chronic intestinal pseudo-obstruction, with 4 cases each (12.9 %). The first reason for the indication was short-bowel syndrome in both children (51.6 %) and adults (37.3 %). The most used type of catheter was tunnelled both in children (75.9 %) and in adults (40.8 %). Sixty-eight episodes ended, all in adults, and the most frequent cause was death (54.4 %); 38.2 % were switched to oral. Conclusions: the number of collaborating centers and professionals in the NADYA registry is increasing. The main indications and reasons for HPN termination remain stable.Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadya-senpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD.

Background: home parenteral nutrition (HPN) is a technique that allows increasing the survival of patients with intestinal failure. Aim: to study the evolution of home parenteral nutrition over the last 8 years in our center. Methods: a retrospective study of adult patients receiving HPN between 2011 and 2019. Study variables are expressed as frequency and mean ± SD (range). Parametric, non-parametric tests, and a survival analysis (p0.05) were applied. Results: the use of HPN showed a substantial increase (up to 154 %). A total of 76 patients received HPN, 76.32 % had an oncological pathology, and consequently the main indication was intestinal obstruction (56.58 %). Average duration was 11.12 ± 23.38 months (0.17-139.17). In all, 44.74 % experienced one or several complications, primarily catheter-related bacteremia (CRB) (27.63 %), and metabolic disorders (40.79 %), mainly of hepatic type. CRB rate was 2.25/1,000 days of HPN. The main cause of death was the underlying disease. Overall survival in case of benign conditions at 1, 3, and 5 years was 65.5 %, 53.6 %, and 40.2 %, respectively. In case of malignancy overall survival at 3 months was 55.1 %, at 6 months 28.7 %, and at 1 years 19.1 %. Conclusions: oncological disease was the primary condition that led to an indication in our center. It is the underlying pathology what determines prognosis, which must be taken into account at the time of selecting patients. The BRC rate obtained is susceptible to improvement, so it is essential that health education and a correct technique be encouraged.Introducción: la nutrición parenteral domiciliaria (NPD) es una técnica que permite incrementar la supervivencia de los pacientes con fallo intestinal. Objetivos: estudiar la evolución de la NPD en los últimos 8 años en nuestro centro. Métodos: estudio retrospectivo de pacientes adultos con NPD entre 2011 y 2019. Las variables se expresan como frecuencias y media ± DE (rango). Se realizaron pruebas paramétricas, no paramétricas y análisis de la supervivencia (p0,05). Resultados: el uso de NPD mostró un sustancial incremento (hasta un 154 %). Un total de 76 pacientes recibieron NPD, el 76,32 % tenían patología oncológica y, en consecuencia, la principal indicación fue la obstrucción intestinal (56,58 %). La duración media fue de 11,12 ± 23,38 meses (0,17-139,17). El 44,74 % experimentaron una o varias complicaciones, siendo las principales la bacteriemia relacionada con el catéter (BRC) (27,63 %) y las metabólicas (40,79 %), fundamentalmente de tipo hepático. La tasa de BRC fue de 2,25/1000 días de NPD. La principal causa de muerte fue la enfermedad de base. La supervivencia global en caso de patología benigna a los 1, 3 y 5 años fue del 65,5 %, 53,6 % y 40,2 %, respectivamente. En caso de patología maligna, la supervivencia global a los 3 meses fue del 55,1 %, a los 6 meses del 28,7 %, y al año del 19,1 %. Conclusiones: la enfermedad oncológica fue la principal patología que motivó la indicación en nuestro centro. Es la patología de base la que determina el pronóstico, lo que debe tenerse en cuenta en la selección de pacientes. La tasa de BRC obtenida es susceptible de mejora, por lo que es fundamental reforzar la educación sanitaria y la técnica correcta.

In order to develop evidence-based recommendations and expert consensus for nutrition management of patients undergoing bariatric surgery and postoperative follow-up, we conducted a systematic literature search using PRISMA methodology plus critical appraisal following the SIGN and AGREE-II procedures. The results were discussed among all members of the GARIN group, and all members answered a Likert scale questionnaire to assess the degree of support for every recommendation. Patients undergoing bariatric surgery should be screened preoperatively for some micronutrient deficiencies and treated accordingly. A VLCD (Very Low-Calorie Diet) should be used for 4–8 weeks prior to surgery. Postoperatively, a liquid diet should be maintained for a month, followed by a semi-solid diet also for one month. Protein requirements (1–1.5 g/kg) should be estimated using adjusted weight. Systematic use of specific multivitamin supplements is encouraged. Calcium citrate and vitamin D supplements should be used at higher doses than are currently recommended. The use of proton-pump inhibitors should be individualised, and vitamin B12 and iron should be supplemented in case of deficit. All patients, especially pregnant women, teenagers, and elderly patients require a multidisciplinary approach and specialised follow-up. Introduction: home parenteral nutrition (HPN) is a fundamental treatment for patients with intestinal failure who do not require hospitalization. We aimed to conduct an epidemiological and clinical practice analysis of HPN through a systematic review. Methods: the systematic review was conducted according to the PRISMA guidelines. A search was performed using the Healthcare Databases Advanced Search of PubMed® and EMBASE®, to identify articles which followed patients treated with HPN for at least 5 years, published between 2009 and 2019 in English or Spanish language. In addition, we manually retrieved other publications of interest. We excluded articles about subgroups of patients with a specific pathology, cancer or pregnant patients. We excluded studies collecting exclusively HPN complications. Results: a total of 267 references were identified, of which 9 met criteria (3 of pediatric population and 6 of adults). In adults, the main difference found between publications was the underlying pathologies. The most common indication was short bowel syndrome and the main cause of exitus was the underlying pathology. Most of the pediatric patients received this support in the first months of life. In addition, children showed a higher conversion rate to oral intake and a lower number of deaths when compared to adults. Conclusions: the use of HPN in cancer pathology is subject to wide geographic variability. It would be advisable to establish indication guidelines in patients with cancer and conduct quality studies, which provide rigorous and homogeneous information.Introducción: la nutrición parenteral domiciliaria (NPD) constituye el tratamiento de elección para pacientes con fallo intestinal que no requieren ingreso hospitalario. En esta revisión sistemática, nuestro objetivo fue realizar un análisis epidemiológico y de práctica clínica en NPD. Métodos: la revisión se realizó siguiendo la guía PRISMA. Para ello, se hizo una búsqueda bibliográfica en PubMed® y EMBASE® de los estudios publicados entre 2009 y 2019 en inglés o español que realizaran el seguimiento de pacientes con NPD durante al menos 5 años. Esta búsqueda se completó manualmente. Se excluyeron los artículos centrados únicamente en pacientes oncológicos, una patología específica o embarazadas o bien en complicaciones. Resultados: se identificaron 267 artículos, de los que cumplieron los criterios 9 (3 de población pediátrica y 6 de adultos). En adultos, la principal diferencia entre los estudios fue la patología de base. La indicación mayoritaria fue el síndrome de intestino corto y la causa de exitus, la enfermedad primaria. La mayor parte de la población pediátrica recibió este apoyo en los primeros meses de vida. Además, destaca el mayor porcentaje de conversión a vía oral y el menor número de fallecimientos frente a población adulta. Conclusiones: la utilización de la NPD en patología oncológica está sometida a una amplia variabilidad geográfica. Sería recomendable establecer directrices de uso en enfermos oncológicos y la realización de estudios de calidad que aporten información rigurosa y homogénea.

Objective: to analyze the characteristics of oncology patients with home parenteral nutrition HPD collected from the NADYA registry. Method: cross-sectional study of the data collected from the NADYA’s group registry related to home and outpatient artificial nutrition. Patients with HPD and explicit oncological diagnosis were selected. Data were collected from January 1, 2010 to December 31, 2016. Results: Within the 7 years studied, 1463 patients were collected, of which 599 had a cancer diagnosis (40.94%) with an increase of 43% from the beginning. [Purpose] Bariatric surgery is the method of choice for the management or treatment of obesity. Bariatric surgery brings about several physiological changes in the body and is associated with set of complications. The aim of this study is to provide guidelines on post bariatric surgery management based on consensus by the Spanish society for Obesity Surgery (Sociedad Española de Cirugía de la Obesidad) (SECO) and the Spanish Society for the Study of Obesity (Sociedad Española para el Estudio de la Obesidad) (SEEDO)., [Method] The boards proposed seven experts from each society. The experts provided the evidence and a grade of recommendation on the selected topics based on systematic reviews/meta-analysis. A list of clinical practical recommendations levels of evidence and grades of these recommendations was derived from the consensus statements from the members of these societies., [Results] Seventeen topics related to post-operative management were reviewed after bariatric surgery. The experts came with 47 recommendations and statements. The mean number of persons voting at each statement was 54 (range 36-76)., [Conclusion] In this consensus, we have designed a set of guidelines to be followed while managing patients after bariatric surgery. Expertise and knowledge of the clinicians are required to convey suitable considerations to the post-bariatric patients. There should also be extensive follow-up plans for the bariatric surgery patients.

Background and objectives: by means of this update, the GARIN working group aims to define its position regarding the dietary treatment of patients with chronic kidney disease (CKD). In this area there are many aspects of uncertainty. Material and methods: bibliographical review and specific questions in advance were discussed and answered at a meeting in the form of conclusions. Results: the therapeutic action must be individualized and taking into account the degree of renal failure that the patient presents and their comorbidities. Regarding nutritional medical therapy, our group proposes three different levels of action, in which the recommendations of protein intake, fiber, fatty acids or potassium are different. In addition, we suggest using the phosphorus/protein ratio concept in adjusting the diet of the patient with CKD. We give recommendations regarding treatment in diabetes and artificial supplementation. Conclusions: these recommendations about dietary issues in patients with CKD can add value to clinical work.Introducción y objetivos: en el tratamiento dietético de los pacientes con enfermedad renal crónica (ERC) existen muchas áreas de incertidumbre. El grupo de trabajo GARIN tiene como objetivo definir su posición en este campo. Material y métodos: revisión bibliográfica previa y reunión presencial en la que se discutieron y contestaron preguntas específicas sobre el tema. Adipose tissue (AT) dysfunction in obesity is commonly linked to insulin resistance and promotes the development of metabolic disease. Bariatric surgery (BS) represents an effective strategy to reduce weight and to improve metabolic health in morbidly obese subjects. However, the mechanisms and pathways that are modified in AT in response to BS are not fully understood, and few information is still available as to whether these may vary depending on the metabolic status of obese subjects. Malnutrition is a common complication in cancer patients and can negatively affect the outcome of treatments. This study aimed to reach a consensus on nutritional needs and optimize nutritional care in the management of cancer patients at a national level.A qualitative, multicenter, two-round Delphi study involving 52 specialists with experience in nutritional support in cancer patients was conducted.Regarding the presence of malnutrition, 57.7% of the participants stated that 30% of the patients had malnutrition at the time of diagnosis, 40.4% considered that 31-50% had malnutrition during cancer treatment, and 26.9% that 50% at the end of the treatment. Forty percent of participants believed that the main objective of nutritional treatment was to improve quality of life and 34.6% to improve tolerability and adherence to chemotherapy. The quality nutritional care provided at their centers was rated as medium-low by 67.3%. Enteral and parenteral nutrition was administered to less than 10% and less than 5% of patients in 40.4 and 76.9% of cases, respectively. In relation to nutritional screening at the time of diagnosis, 62.9% of participants considered than screening to assess the risk of malnutrition was performed in 30% of patients.There is an important variability in the management of cancer patient nutrition, which is associated with the absence of a national consensus on nutritional support in this field. Given the incidence of nutritional disorders in cancer patients, a specialist in clinical nutrition (regardless of his/her specialty) should be integrated into the strategic cancer plan.

To communicate HPN data obtained from the HPN registry of the NADYA-SENPE group (www.nadya-senpe.com) for the year 2016.Descriptive analysis of the data collected from adult and pediatric patients with HPN in the NADYA-SENPE group registry from January 1st, 2016 to December 31st, 2016.There were 286 patients from 42 Spanish hospitals (54.2% women), 34 children and 252 adults, with 294 episodes, which represent a prevalence rate of 6.16 patients / million inhabitants / year 2016. The most frequent diagnosis in adults was "palliative cancer" (25.8%), followed by "others". In children it was "motility alterations" with 6 cases (17.6%), Hirschsprung's disease and necrotising enterocolitis, both with 5 children (14.7%). The first indication was short bowel syndrome in both children (64.7%) and adults (37.3%), followed by intestinal obstruction in 28.6% adults and 14.7% in children. The most frequently used type of catheter was tunnelled in both children (70.6%) and adults (37.9%). The most frequent complication in adults was infection related to the catheter, which presented a rate of 0.48 infections / 1,000 days of NPD. During this period, 71 episodes ended in adults and the main cause was death (57.7%) followed by resuming the oral route (31%).There is a progressive increase of centers and professional collaborators in the registry who report patients receiving parenteral nutrition at home. The main indications of HPN and the motive for ending have remained stable.

Resumen En la actualidad contamos con escasas herramientas terapeuticas eficaces para combatir la creciente prevalencia de obesidad y comorbilidades asociadas. La cirugia bariatrica es el unico tratamiento que ha demostrado su efectividad a largo plazo. Sin embargo, la alta complejidad tecnica de la misma, junto a las caracteristicas de los propios pacientes, implican un elevado riesgo quirurgico e importante coste economico. Esto esta llevando al desarrollo de nuevos procedimientos por via endoscopica con un menor riesgo clinico y coste economico, manteniendo los beneficios en cuanto a morbimortalidad, incluso pudiendo servir de elemento «puente» previamente a la cirugia en los casos en que esta sea inevitable, pero permitiendo una perdida de peso y control de comorbilidades que mejoren el riesgo prequirurgico. El objeto de nuestra revision es el analisis de las tecnicas endoscopicas mas relevantes en estos momentos.

Objetivo: analizar las características de los pa cientes oncológicos con nutrición parenteral do miciliaria (NPD) recogidos en el registro NADYA. Método: estudio transversal de los datos recogi dos en el registro del grupo de nutrición artificial domiciliaria y ambulatoria NADYA de pacientes con NPD y diagnóstico oncológico explicito, desde el 1 de enero de 2010 al 31 de diciembre de 2016. Resultados: en los 7 años estudiados se han recogido 1463 pacientes, de los que 599 han tenido un diagnóstico de cáncer (40,94%) con un incremento del 43% desde el inicio. La edad media fue de 57 años IIQ [46 – 64] registrán dose 5 niños. La mayoría de los pacientes eran oncológicos paliativos (51,3%). Se registraron 3,18 complicaciones sépticas relacionadas con el catéter por cada 1000 días de nutrición, con mayor frecuencia en el grupo de enteritis rádica (ANOVA p

Home parenteral nutrition (HPN) is a technique that has allowed the survival in the community of those patients with serious diseases resulting in an intestinal failure that made their nutrition impossible by other methods. It is indicated if there is a documented intestinal failure (understood by the reduction of the intestinal function to the minimum to the point that intravenous supplementation is required to maintain health and/or growth) with impossibility for oral or enteral exclusive nutrition, provided that there is the possibility of managing the patient at home and that there is no short-term survival expectancy. It requires taking into account the patient's quality of life, family environment and the capacity of the patient and/or their caregivers to be trained for HPN therapy. In low prevalence health topics, as intestinal failure, where the available scientific evidence is of poor quality, consensus documents add value in decision-making. Furthermore, HPN is a complex process and, although there is extensive experience in its application and even clinical practice guidelines, in daily practice there are uncertainties about its suitability, usefulness, rational use and associated costs. For this reason, this document of consensus has been carried out, using the GRADE method. With this document we intend to define our position with regard to the current use of HPN in our country and answer several controversial questions related to this treatment.La nutrición parenteral en domicilio (NPD) es una técnica que ha permitido la supervivencia en la comunidad de aquellos pacientes con enfermedades graves resultantes en un fallo intestinal que hacía imposible su nutrición por otros métodos. Esta está indicada si existe un fallo intestinal documentado (entendido como la reducción de la función intestinal al mínimo hasta el punto de que se requiere suplementación intravenosa para mantener la salud y/o el crecimiento) con imposibilidad para la nutrición exclusiva por vía oral/enteral, posibilidad de manejo del enfermo en el domicilio y que no exista una expectativa corta de supervivencia, teniendo en cuenta la calidad de vida del paciente, el entorno familiar y la capacidad del paciente y/o de sus cuidadores de entrenamiento para la terapia de NPD.En los problemas de salud poco prevalentes, como es el fallo intestinal, en los cuales la evidencia científica disponible es de baja calidad, los documentos de consenso/expertos aportan valor en la toma de decisiones. Se suma a ello que la NPD es un proceso muy complejo y, a pesar de que existe una amplia experiencia en su aplicación e incluso guías de práctica clínica, en la práctica diaria se plantean incertidumbres acerca de su conveniencia, utilidad, uso racional y costes asociados. Por este motivo se ha realizado este documento de consenso, utilizando el método GRADE, con el que pretendemos definir nuestra posición con respecto al uso actual de la NPD en nuestro país y dar respuesta a una serie de preguntas que generan controversia en relación a este tratamiento.

Objetivo: Comunicar los datos del registro de Nutrición Parenteral Domiciliaria (NPD) del grupo de trabajo NADYA-SENPE del años 2015.Material y métodos: Recopilación de los datos de NPD del registro “on-line” del grupo de Nutrición Artificial Domiciliaria y Ambulatoria (NADYA) desde el 1 de enero de 2015 al 31 de diciembre de 2015.Resultados: Se registraron 236 pacientes, con 243 episodios de NPD procedentes de 40 hospitales. Lo que representa una tasa de 5,08 pacientes/millón de habitantes/ año 2015. La patología más frecuente en los adultos fue “otros” (26,3%) seguido por “oncológico paliativo” (21,6%). La complicación más frecuente fue la séptica relacionada con el catéter que presentó una tasa de 0,53 infecciones/1000 días de NPD. Finalizaron 64 episodios, la principal causa fue el fallecimiento (43,7%) y el ‘paso a la vía oral’ (32,8%).Conclusiones: constatamos el aumento de los centros y profesionales colaboradores, dando respuesta a la cantidad progresivamente mayor de pacientes con soporte nutricional parenteral en domicilio. Se mantienen estables las principales indicaciones para el establecimiento de NPD y las causas de finalización del tratamiento.

Summary Introduction Gastrostomy feeding tube insertion has become a common procedure as it enables patients who require long term enteral feeding. Conventional surgical gastrostomies were the only way of gaining enteral access in patients in which it is not possible to pass an endoscope or a nasogastric tube required for endoscopic or radiological gastrostomies, and in patients in which certain anatomical abnormalities contraindicate performing these techniques. As conventional surgical gastrostomies are associated with high morbidity, especially gastric leakage around the tube, percutaneous laparoscopic assisted gastrostomy (PLAG) may be a better way to gain enteral access. Material and methods Observational study of a prospective cohort of 224 patients on whom a gastrostomy was performed for nutritional support between January 2009 and October 2015 at Virgen del Rocio University Hospital in Seville. The types of gastrostomies carried out were: percutaneous endoscopic gastrostomy (PEG, n = 106), percutaneous radiological gastrostomy (PRG, n = 89), conventional surgical gastrostomy, Open Stamn or Laparoscopic Janeway (SG, n = 9) and percutaneous laparoscopic assisted gastrostomy (PLAG, n = 20), technique that we describe in detail. Short and long term complications are described. Results Many more complications were seen in the conventional gastrostomy group than in the other three groups, especially leakage of gastric content around the tube, with burning and irritation of the skin (66% compared with 2.83% in PEG and 0% in PLAG and PRG). The group with the highest proportion of patients completely free of complications was PLAG (75%), whilst in the conventional surgical gastrostomy group, no patient was completely free of complications. Conclusions We found lower complication rate in PLAG than any other technique. We believe that PLAG could be preferred technique for patients on whom it is not possible to perform PEG or PRG, as it is safe and easy.

Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadya-senpe. com) del año 2017. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2017. Resultados: se registraron 308 pacientes (54, 5% mujeres), 38 niños y 270 adultos, procedentes de 45 hospitales españoles, en total 312 episodios, lo que representa una tasa de prevalencia de 6, 61 pacientes/millón de habitantes/año 2017. El diagnóstico más frecuente en adultos fue “oncológico paliativo” (25, 6%), seguido de “otros”. En niños fue la enfermedad de Hirschsprung, con seis casos (15, 8%). El primer motivo de indicación fue síndrome de intestino corto tanto en niños (55, 3%) como en adultos (33, 7%). El tipo de catéter más utilizado fue el tunelizado tanto en niños (74, 3%) como en adultos (38, 2%). Finalizaron 81 episodios; la causa más frecuente fue el fallecimiento (62, 9%) y que pasaron a vía oral (34, 7%). Conclusiones: se mantiene el incremento progresivo de centros y profesionales colaboradores en el registro de pacientes que reciben NPD. Las principales indicaciones de NPD y de motivo de finalización se mantienen estables. The first indication was short bowel syndrome in both children (55.3%) and adults (33.7%). The most frequently used type of catheter was tunneled in both children (73.4%) and adults (38.2%). Ending 81 episodes, the most frequent cause was death (62.9%) and transition to oral feeding (34.7%). Conclusions: the progressive increase of collaborating centers and professionals in the registry of patients receiving NPD is maintained. The main indications of HPN and the motive for ending have remained stable.

The particularity of this case is the nutritional management that has managed to avoid the use of prolonged parenteral nutrition and possible complications by placing jejunal tube at the distal end in patients with short bowel. He received jejunal tube feeding laundry in the efferent loop terminal with decreased weight gain and subsequent reconstruction of intestinal transit debit proximal jejunostomy.Lo más característico del presente caso es el manejo nutricional del paciente, que ha logrado evitar la utilización de nutrición parenteral total de forma prolongada y sus posibles complicaciones, mediante la administración de una nutrición enteral a través de una sonda yeyunal en el extremo distal de una ileostomía en un paciente con intestino corto severo. Se trata de un hombre de 34 años de edad, con antecedentes de colecistectomía complicada con peritonitis postquirúrgica y dehiscencia, que a los dos años desarrolló un cuadro de obstrucción de intestino delgado, por lo que se realizó devolvulación de intestino delgado que se complicó con dos fugas intestinales en diferentes tiempos; tras la segunda fuga se realizó yeyunostomía lateral en doble cañón de escopeta a nivel de la dehiscencia; presentó altos débitos por el asa aferente de la yeyunostomía terminal; durante el ingreso se insertó una sonda de poliuretano para alimentación enteral por asa eferente de la yeyunostomía. Recibió nutrición enteral por sonda yeyunal colada en el asa eferente con disminución del débito de yeyunostomía proximal terminal y ganancia de peso con posterior reconstrucción del tránsito intestinal.

La anorexia nerviosa es la enfermedad psiquiatrica mas frecuente entre las mujeres jovenes, y se caracteriza por la realizacion de dietas estrictas con perdida significativa de peso y un miedo desproporcionado a su ganancia. Esta enfermedad conlleva multiples complicaciones derivadas tanto de la desnutricion que origina como de los metodos empleados para la perdida de peso. El tratamiento de esta afeccion exige un abordaje multidisciplinario y especializado, que se puede efectuar en distintos niveles asistenciales tanto ambulatorio como hospitalario, dependiendo de la situacion clinica de los pacientes. Durante el proceso de renutricion, en cualquiera de los niveles asistenciales, se pretende la recuperacion de un peso minimo saludable, la normalizacion de la conducta alimentaria asi como la correccion de las secuelas fisicas y psicologicas de la malnutricion. El tratamiento inicial debe enfocarse hacia la restauracion del peso, y para alcanzar los objetivos propuestos se seleccionara la via de acceso de alimentacion mas apropiada; la via oral siempre sera la de eleccion, y se optara por la nutricion artificial solo en situaciones de falta de cooperacion o de incorrecta progresion ponderal durante el tratamiento. Si es preciso puede recurrirse a la nutricion artificial, y la nutricion enteral es preferible a la parenteral. Una vez iniciada la realimentacion, la progresion en la alimentacion se realizara de manera individualizada, y se incrementara progresivamente el aporte calorico hasta alcanzar los objetivos de peso propuestos. Es imprescindible el estricto control hidroelectrolitico, metabolico y fisico durante la fase inicial de la realimentacion para evitar y diagnosticar complicaciones que pueden aparecer, como el sindrome de renutricion.

When assessing a patient nutritional support the state of the gastrointestinal tract is the most important point to consider, whenever possible we should choose the enteral nutrition (EN) over parenteral nutrition (PN) and recognized by its various advantages. The percutaneous gastrostomy (GRP) were introduced in clinical practice as an alternative to surgical gastrostomy (GQ). The speed, simplicity, low cost, low morbidity and low mortality of these techniques has allowed its rapid development.The feeding tubes types used were the balloon-retained devices catheter, the pig tail and the ballon-retained with gastropexia (gastropexia). It is a retrospective study of all patients who were placed GPR in the Virgen del Rocío University Hospital, Sevilla, between September 1996 and September 2013, which aims to study is to describe the characteristics of patients and the complications presented for different types of enteral feeding tubes used. GPR 186 were performed in 176 patients (135 males (76.70%), with an average duration of 303.6 days, the most frequent diseases were cancers of the head and neck 49.46%. The types of feeding tubes used were pigtail 118 (63.44%), balloon-retained devices 22 (11.83%), and gastropexia 46 (24.73%). The most common early complications in the pigtail group were early purulent exudate and early bleeding (2.5 and 3.4% respectively), while in the balloon catheter group the initial output of the probe was most frequent early complication (13.7%), none of these complications were observed in the group of gastropexy. The most common late complication was obstruction probe. The GPR is a safe technique with lower mortality of 1%; Low frequently of early and late complications. A better understanding of this technique can reduce the frequency of complications.A la hora de valorar el soporte nutricional en un paciente el estado del tracto gastrointestinal es el punto más importante a considerar, siempre que sea posible deberíamos escoger la nutrición enteral (NE) sobre la nutrición parenteral (NP) por sus variadas y reconocidas ventajas. Las gastrostomías percutáneas (GRP) fueron introducidas en la práctica clínica como alternativa a la gastrostomía quirúrgica (GQ). La rapidez, simplicidad, bajo costo, reducida morbilidad y escasa mortalidad de estas técnicas ha permitido su rápido desarrollo. Este es un estudio retrospectivo de los tipos de sonda (las sondas de balón, las de rabo de cerdo y la de balón con gastropexia) colocadas en el Hospital Universitario Virgen del Rocío, Sevilla entre Septiembre de 1996 y Septiembre 2013, el objetivo es describir las características de los pacientes y las complicaciones presentadas según los diferentes tipos de sondas utilizadas. Se realizaron 186 GRP en 176 pacientes (135 varones (76,70%), con una duración promedio de 303,6 días, las patologías más frecuente fueron los cánceres de cabeza y cuello 49,46%. Los tipos de sonda utilizadas fueron sonda rabo de cerdo 118 (63,44%), de balón 22 (11,83%), y sonda de balón con gastropexia 46 (24,73%). Las complicaciones tempranas más frecuentes en el grupo de rabo de cerdo fueron el exudado purulento precoz y el sangrado precoz (2,5 y 3,4% respectivamente), mientras que en el grupo de sonda con balón la salida inicial de la sonda fue la complicación temprana más frecuente (13,7%), ninguna de estas complicaciones se han observado en el grupo de gastropexia. La complicación tardía más frecuente fue la obstrucción de la sonda. La GPR es una técnica segura con una mortalidad menor de 1%; Con menor frecuencia de complicaciones tempranas y tardías. El mejor conocimiento de estas técnicas permite disminuir la frecuencia de complicaciones.

The loss of mobility due to spinal cord injury is a risk factor for weight gain. Despite the well-documented outcomes of bariatric surgery in outpatients, little information is available about the surgery in paraplegic patients. We present two cases of patients with morbid obesity and spinal cord injury. After several attempts to lose weight conservatively, were assessed by the multidisciplinary team of our hospital and finally intervened by laparoscopic gastric bypass. After surgery have been no post-surgical complications. The patient in case 1, after two years of follow-up, a weight of 84 kg (BMI 25.08 kg/m2). Case 2, after a month of surgery has reduced weight and stopped taking antihypertensive therapy. It 's available to bariatric surgery as an important option to consider if all non-surgical interventions fail is highlighted.La pérdida de la movilidad, como consecuencia de una lesión medular, es un factor de riesgo para el aumento de peso. A pesar de los resultados bien documentados de la cirugía bariátrica en pacientes ambulatorios, hay poca información disponible acerca de la cirugía en pacientes parapléjicos. Presentamos dos casos clínicos de pacientes con obesidad mórbida y lesión medular. Tras varios intentos para perder peso de forma conservadora, fueron valorados por el equipo multidisciplinar de nuestro hospital y finalmente se intervinieron mediante bypass gástrico por vía laparoscópica. El caso 1, tras dos años de seguimiento, presenta un peso de 84 kg (IMC 25,08 kg/m2). El caso 2, tras un mes de la cirugía ha reducido de peso y ha dejado de tomar tratamiento hipotensor. En ellos, destaca la disposición de la cirugía bariátrica como una opción a tener en cuenta si todas las intervenciones no quirúrgicas fracasan.

Percutaneous gastrostomy, is the procedure of choice to provide enteral access in patients requiring nutritional support in this way in the long run, relegating the surgical gastrostomy.We present three patients requiring percutaneous gastrostomy for nutritional support. In two cases was performed endoscopic gastrostomy and another one using interventional radiology.While performing percutaneous gastrostomy clinical incidents were not detected, but when trying the replacement of gastrostomy tubes, showed the presence of gastrocolic fistula that caused failure or turnover in one case, or abdominal pain and diarrhea in the two other cases.Despite being a safe technique, should be done a proper patient selection in order to minimize the potential complications that may occur, as gastrocolic fistula, recommending in doubtful cases test of image such CT (computerized Tomography).Introducción: La gastrostomía percutánea es el procedimiento de elección para proporcionar acceso enteral en pacientes que requieren soporte nutricional por esta vía a largo plazo, relegando a la gastrostomía quirúrgica. Material y métodos: Presentamos tres pacientes con indicación de gastrostomía percutánea para soporte nutricional. En dos casos se realizó la gastrostomía mediante endoscopia y en otro mediante radiología intervencionista. Resultados: Durante la realización de las gastrostomías no se detectaron incidencias clínicas, pero al intentar recambio de las sondas de gastrostomía, se evidenció la presencia de fístula gastrocólica que, ocasionaba imposibilidad del recambio en un caso, o cuadro de dolor abdominal y diarrea en los otros dos casos. Discusión: Pese a ser una técnica segura, la gastrostomía percutánea debe realizarse con una correcta selección de los pacientes para minimizar posibles complicaciones que puedan acontecer, como fístula gastrocólica, recomendando en casos dudosos la realización de prueba de imagen tipo TC (tomografía computerizada).

Obesity is an important health concern worldwide. Spain has one of the highest pediatric obesity rates among European countries, and they are increasing, which mandates the development of innovative strategies aimed at reverting this trend and decreasing the health problems related to obesity and the considerable waste of resources foreseen for the upcoming years.To determine if an educational intervention from a health professional would yield an additional benefit in the acquisition of knowledge on nutrition. A second objective was to determine the prevalence of weight excess as well as the lifestyle habits in a sample of school students.Analytical, interventional, random, longitudinal, pilot study in a sample of 107 students aged 9-15 years. The weight, height, adherence to the Mediterranean diet, level of physical activity and sedentarism, and knowledge on feeding and healthy lifestyles were estimated through a questionnaire. In an intervention group (54 students) a short educational intervention was carried out by a health professional. Two months later, the knowledge on diet and lifestyle habits was reassessed in all the students.After the educational intervention, the students in the intervention group had better knowledge regarding feeding and healthy lifestyles than the control students, and this difference was statistically significant.the additional educational activities on healthy lifestyles within the scholar program given by a health professional may represent an additional benefit to the strategies aimed at decreasing pediatric obesity in our setting.Introducción: La obesidad es un importante problema de salud a nivel mundial. España tiene una de las cifras más altas de obesidad infantil de Europa y están aumentando, lo que obliga a desarrollar estrategias innovadoras que reviertan esta tendencia y disminuyan los problemas de salud asociados a la obesidad y el enorme consumo de recursos que se prevé en los años venideros. Objetivos: Determinar si una intervención educativa por parte de un profesional sanitario aporta un beneficio adicional en la adquisición de conocimientos en materia de nutrición. Secundariamente determinar prevalencia de exceso de peso así como los hábitos de vida en una muestra de escolares. Material y métodos: Estudio piloto longitudinal analítico y de intervención aleatorizado por bloques realizado en una muestra de 107 alumnos de entre 9-15 años. Se estimaron mediante encuesta peso, talla, adherencia a la Dieta Mediterránea, grado de actividad física y sedentarismo, y conocimientos en alimentación y hábitos de vida saludables. Sobre un grupo de intervención (54 alumnos) se realizó una intervención educativa breve por parte de un profesional sanitario. Dos meses después se reevaluaron los conocimientos de alimentación y hábitos de vida saludables en todos los alumnos. Resultados: Después de la intervención educativa los alumnos del grupo de intervención poseen más conocimientos relativos a alimentación y hábitos de vida saludables que los alumnos del grupo control, y esta diferencia es estadísticamente significativa. Conclusiones: Las actividades educativas de apoyo dentro del programa escolar de estilos de vida saludable por parte de profesionales sanitarios pueden suponer un beneficio adicional en las estrategias encaminadas a disminuir la obesidad infantil en nuestro medio.

Morbid obesity and its complications are an increasingly prevalent problem. Very low calorie diets (VLCD), providing between 450 and 800 kcal per day, are an option increasingly used. After proper patient selection, VLCD can result in significant weight loss in 8-16 weeks, contributing to improve control of chronic complications such as diabetes, hypertension, dyslipidemia (except for initial elevation of HDL cholesterol) and apnea-hypopnea syndrome. VLDC are increasingly used prior to bariatric surgery, showing a decrease in hepatic steatosis and visceral abdominal fat. Although the results of the different studies are controversial, preoperative use of VLDC may decrease the rate of perioperative complications, operative time and hospital length of stay. a drastic decrease in intake occurs after bariatric surgery, with risk of protein deficiency, which should be frequently corrected with supplementation by protein modules. Side effects such as cholelithiasis, hyperuricemia and bone loss among others should be monitored in patients undergoing this type of diet.La obesidad mórbida y sus complicaciones suponen un problema cada vez más prevalente. Para su abordaje terapéutico existen diferentes modalidades dietéticas, siendo las dietas de muy bajo valor calórico (DMBVC), que aportan entre 450 y 800 kcal diarias, una opción cada vez más utilizada. Tras una adecuada selección del paciente, pueden conseguir una importante pérdida ponderal en 8-16 semanas, contribuyendo a la mejora del control de las complicaciones crónicas como la diabetes, la hipertensión arterial, la dislipemia (salvo por elevación inicial de HDL colesterol) y el síndrome de apnea-hipopnea del sueño. Las dietas muy bajas en calorías son cada vez más utilizadas en el preoperatorio de la cirugía bariátrica, habiéndose demostrado una disminución en la esteatosis hepática y la grasa visceral abdominal. Aunque los resultados de los diferentes estudios son controvertidos, la utilización preoperatoria de estas dietas podrían disminuir la tasa de complicaciones perioperatorias, el tiempo quirúrgico de la intervención y la estancia hospitalaria. Tras la cirugía bariátrica se produce una disminución drástica en la ingesta, con riesgo de déficit proteico en la dieta, que deben ser corregidos con frecuencia con la suplementación mediante módulos de proteínas. Se debe monitorizar a los pacientes sometidos a este tipo de dietas, ante el riesgo de desarrollar efectos secundarios como colelitiasis, hiperuricemia y pérdida de masa ósea entre otros.

Few effective therapeutic tools are currently available to fight the increasing prevalence of obesity and its associated comorbidities. Bariatric surgery is the only treatment with proven long-term effectiveness, but is associated to a high surgical risk and significant economic costs because of its technical complexity and the characteristics of patients. This is leading to development of new endoscopic procedures with less clinical risks and economic costs, while maintaining the benefits in terms of morbidity and mortality, which could even serve as a bridging element before surgery in cases where this is unavoidable, allowing for preoperative weight loss and control of comorbidities in order to improve anesthetic risks and possible complications. The purpose of this review was to analyze the most relevant and promising endoscopic techniques currently available.

La obesidad mórbida y sus complicaciones suponen un problema cada vez más prevalente. Para su abordaje terapéutico existen diferentes modalidades dietéticas, siendo las dietas de muy bajo valor calórico (DMBVC), que aportan entre 450 y 800 kcal diarias, una opción cada vez más utilizada. Tras una adecuada selección del paciente, pueden conseguir una importante pérdida ponderal en 8-16 semanas, contribuyendo a la mejora del control de las complicaciones crónicas como la diabetes, la hipertensión arterial, la dislipemia (salvo por elevación inicial de HDL colesterol) y el síndrome de apnea-hipopnea del sueño.
