Introduction: Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNEN) represent roughly 1–2% of all colorectal malignancies. Given the rareness and heterogeneity of these mixed tumors, recognition and accurate diagnosis remain a challenge. In the absence of established guidelines, they are treated according to the standard of care for pure neuroendocrine carcinomas or adenocarcinomas from similar sites of origin. Case Presentation: We herein report a case of a rectal MiNEN in a 55-year-old male. He underwent colonoscopy for rectal bleeding and mucus emission, which revealed a vegetating lesion located approximately 8 cm from the anal verge, corresponding to a moderately differentiated low-grade adenocarcinoma of the rectum. Computed tomography scan and magnetic resonance imaging uncovered the presence of lung, lymph node, and subcutaneous implant metastases. The biopsy of the cutaneous implant showed neuroendocrine carcinoma Ki-67 90%. The patient underwent systemic chemotherapy. Conclusion: High-grade MiNEN tumors are the most commonly encountered in clinical practice and have an aggressive biological behavior. Little is known about the genetic drivers of this neoplasm and its pathogenesis remains controversial. Clinical and pathological awareness of this rare entity is a key step to design future targeted therapies and improve treatment options. The aim of this case report is to further our understanding regarding the clinical presentation, radiological features, pathology, management, and prognosis of MiNEN.

ABSTRACT Adenosine is an antiarrhythmic drug that slows conduction through the atrioventricular node and acts as a coronary blood vessel dilator. This case report highlights two unusual life-threatening events following the use of adenosine to revert supraventricular tachycardia in a structurally normal heart: non-sustained polymorphic ventricular tachycardia and myocardial infarction. A 46-year-old woman presented to the emergency department with a two-hour history of palpitations and was diagnosed with supraventricular tachycardia. Vagal maneuvers were ineffective, and after intravenous adenosine administration, the patient presented with chest pain and hypotension. The rhythm degenerated into non-sustained polymorphic ventricular tachycardia and spontaneously reverted to sinus rhythm with ST elevation in lead aVR and ST depression in the inferior and anterolateral leads. The patient spontaneously recovered within a few minutes. Despite successful arrhythmia reversal, the patient was admitted to the intensive care unit because of an infarction without obstructive atherosclerosis. This report aims to alert emergency physicians about the potential complications associated with supraventricular tachycardia and its reversal with adenosine.

Abstract Background Since the identification of multiple therapeutic targets, as is the case of anaplastic lymphoma kinase (ALK) translocation, the paradigm of treating patients with non-small cell lung cancer (NSCLC) has improved. In order to guarantee the possibility of longer survival outcomes with a better quality of life we must invest in the determination, in suitable time, of the consensual biomarkers and in the availability of the best treatments to our patients. Case presentation We present a case of a caucasian male in his fifth decade of life, non-smoker, who highlights the complex journey of ALK-positive patients. This particular case, demonstrates the efficacy and tolerability of the new ALK target therapies, allowing our patients to maintain their routines without compromising the effectiveness of the therapy. Conclusion Focusing on the reality of ALK positive patients and the impact that this therapy has on the daily lives of our patients, we can contribute to the awareness of this specific pathology.

ABSTRACT Objective: This study aimed to describe the clinical characteristics of the pediatric population with COVID-19 in an Emergency Department (ED) of a Brazilian general hospital. Methods: Epidemiological and clinical data of patients younger than 20 years old were collected from patients’ medical records from February 2020 to July 2021. Most of the epidemiological data described pertains to hospitalized patients. We also reviewed coinfections, treatment, and outcomes and compared the first and second waves of COVID-19. Results: We identified a total of 1303 episodes of SARS-CoV-2 infection. The median time from symptom onset to diagnosis was three days. Symptoms were present in 92.3% of the patients. The most common symptoms were fever (45.2%), nasal congestion/discharge (44.2%), and cough (39.4%). Chest radiography and tomography were performed in 7.7 and 3.3% of cases, with abnormal findings in 29.7 and 53.4%, respectively. Hospital admissions occurred in 3.5% of patients, mainly in the presence of comorbidities, in children under five years old and in those who presented to the ED during the first wave of COVID-19. Coinfection with a viral agent was identified in 20% of the 71 cases tested in this study, and a positive rapid test for Streptococcus pyogenes was found in 8% of the 174 cases tested, with no impact of these coinfections on hospitalization. Conclusions: We found that COVID-19 was a mild disease in most children in our study population, with most hospitalizations and readmissions occurring during the first wave of COVID-19.

Resumo A doença valvar cardíaca é um problema de saúde crescente no mundo. Os pacientes com valvopatia podem apresentar diversas emergências cardiovasculares. O manejo desses pacientes é um desafio no departamento de emergência, principalmente quando a condição cardíaca prévia é desconhecida. Atualmente, recomendações específicas para o manejo inicial são limitadas. A presente revisão integrativa propõe uma abordagem baseada em evidência, de três etapas, desde a suspeita de valvopatia à beira do leito até o tratamento inicial das emergências. A primeira etapa é a suspeita de uma condição valvar subjacente com base nos sinais e sintomas. A segunda etapa consiste na tentativa de confirmação diagnóstica e avaliação da gravidade da valvopatia com exames complementares. Finalmente, a terceira etapa aborda as opções diagnósticas e terapêuticas para insuficiência cardíaca, fibrilação atrial, trombose valvar, febre reumática aguda, e endocardite infecciosa. Além disso, apresentamos imagens de exames complementares e tabelas para apoio aos médicos.

Com o aumento da necessidade energética acompanhando o crescimento da população mundial tem-se tornado maior a preocupação com poluição, aquecimento global e queimadas. Essa preocupação incentiva investimentos e pesquisas no âmbito do desenvolvimento sustentável, sendo a geração de dióxido de carbono uma das maiores problemáticas na indústria de energia, com a queimada de combustíveis fósseis e seu forte impacto no aquecimento global. O dióxido de carbono é um gás que participa do efeito estufa e tem contribuído para o aumento da temperatura global, aumento no nível do mar e diminuição das geleiras. Para reduzir as emissões de CO2 na atmosfera estão sendo utilizadas tecnologias de captura, transporte, sequestro e armazenamento de dióxido de carbono. Este trabalho busca fazer uma revisão dos tipos dessas tecnologias, analisar sua aplicabilidade e esclarecer os principais conceitos.

ABSTRACT Chest pain is a frequent, potentially life-threatening condition in the emergency department and requires immediate investigation and treatment. This case report highlights a rare differential diagnosis of pleuritic chest pain: epipericardial fat necrosis. A 29-year-old man presented with normal clinical evaluation, electrocardiography, point-of-care ultrasound, and unremarkable laboratory tests. The initial hypothesis was acute pleuritis. Chest radiography revealed peri-cardiac nonspecific findings, and computed tomography revealed epicardial fat necrosis. Despite the rarity of this condition, accurate diagnosis allows for better practices. An algorithm for a diagnostic approach is proposed.

Background: The Brazilian National Solid Waste Policy establishes the shared responsibility of all actors involved in e-waste generation and management; however, some conflicts of interest need interventions and approaches for preventing them. Objective: This paper proposes using a graph model for conflict resolution (GMCR) decision support system to simulate the analysis and resolution of realistic e-waste management conflicts. Method: A systematic literature review focused on e-waste management, shared responsibility and conflict management was conducted, and a graph model for conflict resolution (GMCR) decision support system was applied to generate a framework to address this context. Results: The need for commitment agreements promoted by government institutions in partnerships with companies involved in the process is essential since the principle of shared responsibility requires educational actions, favoring efficiency in the reverse logistics recovery procedures for e-waste. Understanding the interconnected causes of conflicts and their facets is crucial for effective resolution and prevention, aiding comprehension, focused interventions, and evidence-based decision-making for transformative change amidst conflicting stakeholder objectives in the case of WEEE management. These results can be helpful for academics and practitioners working in this area.

Background: This paper proposes a risk analysis of transport requalification projects in the urban mobility problem caused by a mining disaster related to irregular rock salt extraction in the city of Maceió, Brazil. Methods: The model is composed of three main steps: problem definition, risk management, and decision analysis. For this purpose, we used the Picture Fuzzy-Delphi method for data collection and experts’ judgment elicitation and the Delphi method was used to assess the problem without interference from others. In addition, we used Picture Fuzzy Sets (PFSs) to incorporate uncertain information in the decision-making process. Results: The results of the proposed model demonstrated consistency and relevance to the discussion. The application of methods shows the risks of the project based on a general perspective. It evaluates the sustainability tripod: economic, environmental, and social points of view, assessing the occurrence risk and intensity of the risk. Conclusions: The main objective of the work was achieved; however, some limitations of this study are related to the methods used to assess risks and the options of projects of requalification available at the moment of data analysis. This paper contributes because it systematizes the risk management of projects related to requalification in urban mobility.

Cyanosis is typically a sign of a potentially life-threatening condition in the emergency department and requires immediate workup and treatment. This case report highlights the diagnostic reasoning and clinical approach to cocaine- and volatile nitrite–induced methemoglobinemia (MHG). MHG is a rare, life-threatening cause of cyanosis. The diagnosis must be suspected in the emergency department in the presence of hypoxia and cyanosis disproportionate to cardiopulmonary repercussions and refractory to oxygen supplementation. Acquired causes are more prevalent than genetics, and recreational drugs should be highly suspected. Despite the rarity of this situation, cyanosis precipitants and the specificities of each hemoglobinopathy are reviewed in this article.

ABSTRACT Objective To analyze telemedicine diagnostic accuracy in patients with respiratory infections during COVID-19 pandemic compared to face-to-face evaluation in the emergency department. Methods Randomized, unicentric study between September 2020 and November 2020 in patients with any respiratory symptom (exclusion criteria: age >65 years, chronic heart or lung diseases, immunosuppressed). Patients were randomized 1:1 for brief telemedicine followed by face-to-face consultation or direct face-to-face evaluation. The primary endpoint was the International Classification of Diseases code. The secondary analysis comprised length of stay, diagnostic test ordering, medical prescription, and proposed destination. Results Ninety-eight patients were enrolled. The mean age was 36.3±9.7 years old, 57.1% were women, and 81.6% had diagnostic test ordered. Mean grouped by International Classification of Diseases code for upper respiratory tract infection, pharyngotonsillitis, and sinusitis showed no difference between study groups or secondary endpoints. The Telemedicine Group was representative of the population usually evaluated in this center. In the Telemedicine Group (n=48), 18.7% patients would be referred for evaluation at the emergency department. The distribution of diagnoses by telemedicine was 67.4% for upper respiratory tract infection, 2.3% for pharyngotonsillitis, and 0% for sinusitis, being statistically similar to the subsequent face-to-face assessment, respectively: 72.1%, 11.6% and 7% (Kappa 0.386 [95%CI: 0.112-0.66]; p=0.536). Telemedicine ordered COVID-19 molecular (RT-PCR) tests in 76.5% versus 79.4% in face-to-face evaluation (Kappa 0.715 [95%CI: 0.413-1]; p>0.999). Conclusion Diagnostic telemedicine consultation of low-risk patients with acute respiratory symptoms is not inferior to face-to-face evaluation at emergency department. Telemedicine is to be reinforced in the health care system as a strategy for the initial assessment of acute patients. ClinicalTrials.gov Identifier: NCT04806477

ABSTRACT Objective To analyze the impact of COVID-19 on emergency department metrics at a large tertiary reference hospital in Brazil. Methods A retrospective analysis of consecutive emergency department visits, from January 1, 2020, to November 21, 2020, was performed and compared to the corresponding time frame in 2018 and 2019. The economic viability analysis of a possible petroleum reservoir is based on the knowledge about its petrophysical properties, especially the porosity and its relationship with other properties, such as permeability and resistivity. In carbonate systems, the porosity presents a high degree of heterogeneity, which affects other quantities associated with it. In the present work, data of electrical resistivity, permeability, porosity by nuclear magnetic resonance (total porosity and its partitioning in micro, meso and macroporosity values, and irreducible water saturation) of five samples of Coquinas from the Morro do Chaves Formation (Sergipe-Alagoas Basin) extracted with vertical symmetry. The analysis of these data allowed the correlation between the permoporous pairs and the resistivity, evidencing the effect of the permoporous relations in the electric current flow, and between the percentage distribution of the porous space (micro - percentage values lower than 3.8%, meso - percentage values between 7.9 and 24.8%, and macroporosity, with percentage values higher than 71.5%) with permeability and irreducible water saturation, indicating a strong effect of mesoporosity on the observed values for those parameters. The authors present a checklist to be used at the last stage of a preoperative visit for esthetic plastic surgery composed of 29 yes/no questions, four blank spaces for entering data, and one question for ranking the level of risk of deep vein thrombosis. The criteria are divided into three tables relating to three areas: anesthesia, psychological aspects, and clinical risk factors. The answers are framed in four colors that identify the level of risk and suggest the degree of attention warranted. A fourth aggregate table contains reference sources which can be easily accessed during a medical visit via the Internet or a portable device equipped with a medical database software. The purpose of the checklist is to reduce the possibility of adverse events involving the surgical process by identifying the risk level of each item reviewed.Os autores apresentam um checklist para ser utilizado na última etapa da consulta pré-operatória de cirurgias plásticas estéticas, composto de 29 perguntas, com respostas do tipo sim e não, quatro espaços para inserção de dados e uma questão para classificação do nível de risco em trombose venosa profunda. Os quesitos são divididos em três tabelas relativas a três áreas: anestesia, aspectos psicológicos e riscos clínicos. As respostas são enquadradas em quatro cores, que identificam o nível de risco e sugerem o grau de atenção. Uma quarta tabela agregada contém fontes de referência de fácil acesso durante a consulta médica, pela Internet ou por aparelhos portáteis que contenham programas de dados que forneçam informações sobre medicações ou afecções. A finalidade do checklist é diminuir a possibilidade de ocorrências indesejáveis que envolvam o processo cirúrgico, pela identificação do nível de risco de cada item revisto.

FUNDAMENTO: Pacientes com anemia falciforme (FAL) apresentam freqüentemente episódios de dor precordial, possuem alterações eletrocardiográficas em repouso e exibem alterações da estrutura e das funções cardíacas. OBJETIVO: Avaliar o efeito dos episódios repetitivos de vaso-oclusão sobre a microcirculação coronariana. MÉTODOS: Pacientes estáveis com FAL (n = 10, cinco mulheres, 24,4 + 5,4 anos) foram submetidos a medida das velocidades de fluxo coronariano e da reserva de fluxo coronariano (RFC) na artéria coronária descendente anterior por meio de ecocardiografia transesofágica em estado basal e após hiperemia máxima, obtida com adenosina intravenosa. Esses pacientes foram comparados a pacientes com traço falciforme (TRA, n = 10, cinco mulheres, 27,7 + 3,2 anos), anemia ferropriva (FER, n = 8, oito mulheres, 26,6 + 5,2 anos) e grupo controle (NOR, n = 10, cinco mulheres, 26,3 + 6,3 anos). RESULTADOS: O grupo FAL apresentou aumento das velocidades de fluxo coronariano diastólico (p < 0,01) em estado basal e durante hiperemia máxima (67,3 + 14,0 cm/s e 198,2 + 37,9 cm/s, respectivamente), quando comparado aos três outros grupos (TRA, 34,4 + 11,9 cm/s e 114,7 + 36,4 cm/s; FER, 42,4 + 10,4 cm/s e 141,0 + 18,7 cm/s e NOR, 38,1 + 10,0 cm/s e 126,8 + 24,6 cm/s). Entretanto, a RFC foi normal no grupo FAL (3,0 + 0,7) e comparável (p = 0,70) aos demais grupos (TRA, 3,4 + 0,8; FER, 3,5 + 1,2 e NOR, 3,4 + 0,8). CONCLUSÃO: Apesar de maiores velocidades de fluxo coronariano já em estado basal e também durante hiperemia máxima, a RFC é normal na FAL, o que sugere integridade da microcirculação coronariana. Este estudo teve como finalidade avaliar macroscopicamente e histológicamente defeitos osteocondrais já cicatrizados, também conhecidos como reparos. Foram utilizados seis coelhos machos, adultos, albinos da raça Nova Zelândia. Defeitos cilíndricos osteocondrais de 3.2 mm de diâmetro por 4.0 mm de profundidade foram criados cirurgicamente em ambos côndilos femorais mediais. O cilindro osteocondral retirado do joelho esquerdo (defeito não tratado) foi implantado no joelho direito (joelho tratado). Comparou-se macroscopicamente e histológicamente ambos tipos de defeitos após doze semanas de evolução. A avaliação macroscópica de todos os defeitos mostrou evolução para reparos denominados biologicamente aceitáveis. O termo "biologicamente aceitável" foi utilizado para definir reparos, que à observação macroscópica, se apresentaram como tecido neo-formado semelhante à fibrocartilagem, brilhante, liso, firme, em continuidade com a cartilagem adjacente. Como todos os defeitos, tratados e não tratados, eram macroscopicamente semelhantes, realizou-se um estudo histológico comparativo para averiguar qual tipo de tecido de reparação se formava em ambos defeitos. Pela análise histológica dos reparos biologicamente aceitáveis, concluiu-se que houve formação de tecido cartilaginoso hialino nos defeitos tratados com enxerto autólogo e de tecido fibrocartilaginoso nos defeitos não tratados.The aim of this study was to evaluate macroscopically and histologically healed osteochondral defects, also known as repairs. Six adult, male, New Zealander White rabbits were used. Cylindrical osteochondral defects of 3.2 mm in diameter by 4.0 mm in depth were artificially created in the load-bearing surfaces of both medial femoral condyles. The osteochondral graft collected from the left knee (untreated defect) was implanted in the right knee (treated defect). Both defects were compared macroscopically ad histologically after twelve weeks. The macroscopic evaluation of all defects demostrated evolution in repairs called "biologically acceptable". The "biologically acceptable" term was used to define repairs, that at macrocospic observation, presented as neo-formed tissue similar to fibrocartilage, brilliant, smooth, firm, in continuity with the adjacent cartilage. As all the defects, treated and untreated, were macroscopically similar, a histological comparative study was made to verify which type of repair tissue was formed in the surface of both defects. By histologic analysis of the biologically acceptable repairs, the authors conclude that: treated defects with autologous grafts were filled with hyaline cartilaginous tissue and untreated defects were filled with fibrocartilaginous tissue.
