Seventeen kidneys removed from patients with renal carcinoma were submitted to ex vivo ultrasonography and then to serial pathological examination in an attempt to detect secondary tumours in apparently normal tissue. Ex vivo ultrasonography failed to reveal further tumours, but 2 lesions measuring 5 and 8 mm were found on pathological examination. These preliminary data suggest that intra-operative ultrasonography may be of little value in detecting secondary tumours in conservative surgery for renal carcinoma. [ABSTRACT FROM AUTHOR]

The goal of this retrospective study is to assess if diabetic patients are more likely to develop urinary tract infection (UTI) within the context of ureteral obstruction. 804 patients that had received an emergency treatment by double J placement for ureteral stone were selected between January 2004 and December 2014 at the Clinique Saint Pierre d'Ottignies, Ottignies-Louvain-La-Neuve,Belgium. They were divided in two groups : patients with UTI associated and the control group with the non infected ones. In the group of infected patients, 82 were diabetic whereas they were 46 in the control group. There was a significant difference regarding the presence of diabetes between the group of patients with UTI and the control group (p inferior to 0.001). This study demonstrates that diabetic patients are at higher risk of urinary tract infection in case of ureteral obstruction, thus an invasive treatment could be considered faster.

We report the case of a 28-year-old man who presented at the emergency department with recent left painful scrotal swelling, without history of genitourinary infection or trauma. On physical examination, left scrotal swelling with nodular palpation was noted. Contrast enhanced sonography demonstrated nodular vascularized thickening of the tunica vaginalis. Surgical exploration revealed multiples solid nodules of the vaginal wall, with, at frozen section analysis, fibroblastic tissue, vessels and chronic inflammation without malignity, suggestive of fibrous pseudotumor of the tunica vaginalis. We discuss the sonographic aspect of this rare entity and the difficulty to establish a diagno- sis of benignity without surgical exploration.

A 5-year-old girl with Ehlers-Danlos syndrome presented with acute abdominal pain and anuria caused by a spontaneous perforation of bladder diverticula. Conservative treatment was successful.

In order to try to enhance postoperative continence rates after radical retropubic prostatectomy, a fascial sling suspension was performed at the time of the operation in 75 patients (group 2). Postoperative continence was compared to that of the 63 previous patients (group 1). Complete continence rate were 43 of 75 (57%) in group 2 and 17 of 63 (27%) in group 1 (p = 0.0003) at 1 month, 57 of 75 (76%) in group 2 and 36 of 63 (57%) in group 1 (p = 0.0149) at 3 months, 65 of 75 (87%) in group 2 and 46 of 63 (73%) in group 1 at 6 months, 69 of 75 (92%) in group 2 and 50 of 63 (79%) in group 1 at 9 months, and 72 of 75 (96%) in group 2 and 51 of 63 (81%) in group 1 at 12 months. Fascial sling suspension resulted in an earlier return and more complete recovery of urinary continence in men undergoing radical prostatectomy.

The continent urinary diversion is the technique usually used to replace the urinary bladder after total cystectomy The formation of calculi in the pouch is a frequent late complication. We have treated successfully with extra corporeal wave shock lithotripsy (ESWL) two patients who have developed calculi in their pouch two years after realisation of an ileal urinary diversion (Indiana pouch). E.S.W.L. is a good and non invasive technique to cure those calculi with no danger for the continence of the pouch.

Purpose: A study was performed to ascertain if fascial sling suspension of the anastomosis after radical retropubic prostatectomy would enhance postoperative continence rates., Materials and Methods: A fascial sling suspension with a strip harvested from the fascia of the rectus muscle was performed at the time of radical retropubic prostatectomy in 30 patients (group 2). Postoperative continence was compared to that of 30 previous patients (group 1) operated on by the same surgeon., Results: Complete continence rates (no protection needed at any time) were 18 of 30 men (60%) in group 2 and 10 of 30 (33%) in group 1 (p = 0.069) at 1 month, 28 of 30 men (93%) in group 2 and 21 of 30 (70%) in group 1 (p = 0.042) at 3 months, 30 of 30 in group 2 (100%) and 27 of 30 (90%) in group 1 at 6 months, and 30 of 30 in group 2 (100%) and 28 of 30 (93%) in group 1 at 9 and 12 months., Conclusions: Fascial sling suspension resulted in an earlier return and more complete recovery of urinary continence in men undergoing radical prostatectomy.

Neuroendocrine carcinomas of the prostate is rare and usually concern patients with an initial diagnosis of adenocarcinoma of the prostate which become refractory to hormonal treatment. Here is reported the case of a patient who presented with a primary neuroendocrine tumor of the prostate. A review of the literature shows the differences with usual adenocarcinoma and the diagnostic and therapeutic implications.

We report 3 new cases of polyorchidism. Review of the literature allows two groups of anatomical variations to be identified: (1) where the Wolffian duct is duplicated longitudinally or absent, and (2) where the two testes are drained by the same epididymis and vas deferens. We postulate that the first group results from a primary anteroposterior division of the genital ridge, whereas the second group is the result of a transverse duplication.

We describe a case of a congenital arteriovenous fistula of the gluteal artery, a posterior branch of the left iliac artery, developing during pregnancy without postpartum regression after delivery. Surgical excision without preoperative embolisation was made possible by prior ligation of the gluteal artery.

In order to evaluate the impact of endourological techniques and extracorporeal lithotripsy on the management of ureteral calculi, we reviewed the charts of 838 patients requiring hospitalization for this pathology from 1982 till 1987. 314 patients were managed conservatively and 524 required intervention therapy: 52 with blind endoscopic techniques, 93 with open surgery, 122 with retrograde ureteroscopy, 168 with percutaneous extraction and 92 with ESWL. Evolution of the therapeutic strategy is discussed and our present approach to ureteral stone management is outlined.

A total of 47 percutaneous operations (endoureteropyelotomy) was performed for treatment of 41 congenital and 6 secondary cases of ureteropelvic junction obstruction between September 1983 and April 1988. Evaluation was available in 39 cases with a followup of 4 to 56 months (mean 16 months). Good results were obtained in 28 of 39 cases (72 per cent) and there were 3 failures (8 per cent). Eight patients (20 per cent) were symptomatically improved but they had residual obstruction on a diuretic study. Only 1 multioperated patient with a good early postoperative result has shown radiological deterioration at 1 year. Complications were managed conservatively, except for 1 case of intravascular coagulopathy. Endoureteropyelotomy is an effective alternative treatment for ureteropelvic junction obstruction but further evaluation definitely is warranted to define better its indications and risk factors.
