Abstract

Background. The apolipoprotein E (ApoE) e4 polymorphism is linked to increased mortality rates, Alzheimer's disease, and cardiovascular disease in older people, but previous studies have largely failed to detect an effect on self-reported mobility disability. We hypothesized that poor performance on mobility-related tests may provide a better measure of effects, and we aimed to estimate the extent to which the ApoE e4 allele increases risks of poor performance on measured mobility and self-reported mobility disability compared to e3/3, in a medium-sized population cohort. Methods. Data were from 1262 people at baseline older than 65 years from the Longitudinal Aging Study Amsterdam (LASA), followed up for 6 years. Age- and sex-adjusted logistic regression models were used to explore associations. Results. At baseline, those individuals with an e4 allele had an odds ratio of 2.26 (95% confidence interval, 1.31-3.90) for poor performance on gait speed testing ( Conclusions. The ApoE e4 polymorphism is associated with a substantial excess of mobility limitation. The impact is detectable by performance testing, but not by self-reports. Poor results on mobility performance tests may provide a phenotype of ageing.

Abstract

Percentile age norms for ages 55 to 85 using overlapping intervals at specified age midpoints are presented for the sum scores of sections A and B of Raven's Coloured Progressive Matrices (RCPM). The representative age and gender stratified sample (N = 2,815) used is derived from the Longitudinal Aging Study Amsterdam (the Netherlands). As RCPM scores appear to be strongly associated with education, percentile norms for three educational levels are presented: low (0-9 years), middle (10-15 years) and high (16 years and more).

Abstract

Correlation between Mini-Mental State (MMSE) behavior and age, academic background medical factors and cognitive function is analyzed in a population-directed elderly group. Evaluation of individual and combined impact of these factors on MMSE performance reveals the dependence of demographic features on diminished cognitive processing. Logistic regression analysis facilitates such an evaluation. Influence of age and academic background on MMSE performance is consistent and independent of age- and knowledge-associated health factors and neuromotor functioning. These factors affect cognitive functioning.

Abstract

In Western populations, the most common cause of dementia is Alzheimer's disease. The second most common cause is vascular disease, in which the brain tissues suffer from the consequences of inadequate and disrupted blood supply. Vascular dementia is the most common cause of dementia among Oriental peoples. The prevalence and risk factors for vascular dementia are not as widely studied as they are in Alzheimer's disease. At least in part, this is due to greater uncertainties in the diagnosis of vascular dementia. Research has been completed in which data from major European studies of dementia have been re-analyzed to provide estimates on the prevalence and risk factors for vascular dementia. The data indicate that the rate of vascular dementia rises sharply with age. The prevalence of the condition is greater among men than women. This is consistent with the sex differences observed in stroke, which is also related to cerebrovascular disease. When only patients from 80 to 89 were considered, the researchers found that a decline in the prevalence of vascular dementia occurred from 1957 to 1972. The age-specific prevalence of vascular dementia was, with one unexplained exception, similar for all women regardless of the European nation of residence. Among men, the prevalence of vascular dementia varied from place to place. The prevalence of vascular dementia was unusually low in Cambridge, England for both sexes; among men, the prevalence was greater in Lund, Sweden and in Finland. Abstract

Correlation between symptoms of depression and risk of Alzheimer's disease (AD) have been investigated in older people with more than 8 years of education to see whether specific symptoms of depression that predict AD can be identified. In the Amsterdam Study of the Elderly, a community-based study, a sample of 3,147 nondemented persons with normal cognition were studied. Depressed mood and subjective bradyphrenia seem to be indicative of subclinical AD in this group and AD may become apparent in a relatively short period of time vs other groups. Memory complaints are associated with depression and with dementia. Confounding effects of memory complaints were of interest.

Abstract

Objective: Results of previous studies suggest that memory complaints may predict cognitive decline and dementia among elderly people in whom cognitive impairment is already apparent. However, cognitive decline is often a gradual process, and elderly people may notice that their memory deteriorates before mental status tests are able to detect any change in cognitive functioning. Therefore, the authors hypothesized that memory complaints would predict incident Alzheimer's disease in elderly subjects with no signs of cognitive impairment. Method: In the community-based Amsterdam Study of the Elderly, a sample of 3,778 nondemented persons, 65 to 84 years old, was selected and divided into two cognitive categories: normal (Mini-Mental State scores of 26-30) and borderline and impaired (Mini-Mental State scores less than 26). At baseline, the presence or absence of memory complaints was assessed. At follow-up, incident cases of Alzheimer's disease were diagnosed in a two-step procedure. Results: After an average of 3.2 years, 2,169 persons were reevaluated, of whom 77 had incident Alzheimer's disease. Multivariate logistic regression analyses showed that memory complaints were associated with incident Alzheimer's disease in subjects with normal baseline cognition but not in subjects with impaired baseline cognition. Conclusions: The findings of this study suggest that memory complaints are a relatively strong predictor of incident Alzheimer's disease in older persons in whom cognitive impairment is not yet apparent. Furthermore, they suggest that older persons may be aware of a decline in cognition at a time when mental status tests are still unable to detect a decline from premorbid functioning.
