Background: Numerous studies have examined the prevalence of Helicobacter pylori infection and clarithromycin (CLA) resistance rate of H. pylori. However, in South Korea, there is a lack of research analyzing specimens from local clinics and hospitals using molecular methods. This study aimed to assess the prevalence of H. pylori infection and CLA resistance across sex and age groups, as well as to explore regional variations in CLA resistance and its characteristics. Methods: Data from a laboratory information system from 2015 to 2018 were retrospectively analyzed to determine the prevalence of H. pylori infection and CLA resistance rate. The 23S ribosomal RNA genes of H. pylori were analyzed using a dual priming oligonucleotide-based multiplex polymerase chain reaction method. Results: The overall prevalence of H. pylori infection was 50.5%(12,000/23,773), with a significantly higher prevalence among males (53.5%) than females (47.0%). The CLA resistance rate was 28.3%, with a significantly higher rate among females (34.9%) than males (23.8%). Age group analysis revealed that the highest prevalence of H. pylori infection was among individuals in their 40s, whereas the highest CLA resistance rate was observed among those in their 60s. The CLA resistance rate exhibited an upward trend and varied among patients based on their place of residence, and A2143G mutation was the most prevalent across all regions. Conclusion: The prevalence of H. pylori infection and CLA resistance rate in Korea remain high and vary according to sex, age, and region. To effectively eradicate H. pylori, it is crucial to periodically monitor regional CLA resistance patterns and conduct CLA susceptibility testing before prescription.

Abstract Background Financial risk protection (FRP) is a key component of universal health coverage (UHC): all individuals must be able to obtain the health services they need without experiencing financial hardship. In many low-income and lower-middle-income countries, however, the health system fails to provide sufficient protection against high out-of-pocket (OOP) spending on health services. In 2018, OOP health spending comprised approximately 40% of current health expenditures in low-income and lower-middle-income countries. Methods We model the household risk of catastrophic health expenditures (CHE), conditional on having a given disease or condition—defined as OOP health spending that exceeds a threshold percentage (10, 25, or 40%) of annual income—for 29 health services across 13 disease categories (e.g., diarrheal diseases, cardiovascular diseases) in 34 low-income and lower-middle-income countries. Health services were included in the analysis if delivered at the primary care level and part of the Disease Control Priorities, 3rd edition “highest priority package.” Data were compiled from several publicly available sources, including national health accounts, household surveys, and the published literature. A risk of CHE, conditional on having disease, was modeled as depending on usage, captured through utilization indicators; affordability, captured via the level of public financing and OOP health service unit costs; and income. Results Across all countries, diseases, and health services, the risk of CHE (conditional on having a disease) would be concentrated among poorer quintiles (6.8% risk in quintile 1 vs. 1.3% in quintile 5 using a 10% CHE threshold). The risk of CHE would be higher for a few disease areas, including cardiovascular disease and mental/behavioral disorders (7.8% and 9.8% using a 10% CHE threshold), while lower risks of CHE were observed for lower cost services. Conclusions Insufficient FRP stands as a major barrier to achieving UHC, and risk of CHE is a major problem for health systems in low-income and lower-middle-income countries. Beyond its threat to the financial stability of households, CHE may also lead to worse health outcomes, especially among the poorest for whom both ill health and financial risk are most severe. Modeling the risk of CHE associated with specific disease areas and services can help policymakers set progressive health sector priorities. Decision-makers could explicitly include FRP as a criterion for consideration when assessing the health interventions for inclusion in national essential benefit packages.

This study aims to enable users of digitized cultural heritage artefacts to understand the correlation between artefacts and their context, discover new knowledge, and gain inspiration. We have developed data labelling and visualization technologies specific to cultural heritage so that users, such as curators, cultural heritage experts, and museum visitors, can easily and quickly access the information that they want through the correlation between artefacts. We came up with a web-based labelling tool for cultural heritage data and applied it to data on 45,692 artefacts from the National Museum of Korea. We also invented a web-based visualization tool that converts lists of attribute information in Excel spreadsheets into visualized content and edits the attributes. The meanings of thumbnails of visualized cultural heritage and that of connected lines, a search function, and visualized cultural heritage will allow users to quickly and easily access the cultural heritage information they are looking for.

Background: The delta neutrophil index (DNI), a prognostic and diagnostic marker for sepsis, is based on the leukocyte count. Platelet activation, similar to leukocyte activation, plays a crucial role in host defense against pathogens and may serve as a predictor of sepsis outcome. However, the combined evaluation of mean platelet component (MPC) and DNI has rarely been used to assess sepsis. Methods: To assess the prognostic and diagnostic validity of the simultaneous evaluation of DNI and MPC in cases of human febrile sepsis, we conducted measurements of cellular indices, including DNI and MPC, as well as molecular biomarkers, including procalcitonin (PCT) and C-reactive protein (CRP). This study was carried out in patients admitted to the emergency department with suspected sepsis. Results: Using a cutoff value of 2.65%, the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the DNI in sepsis were found to be 69%, 73.9%, 77.9%, and 64.1%, respectively. Furthermore, significant differences in DNI and MPC levels were observed between the sepsis and non-sepsis groups (6.7 ± 7.8% versus 2.1 ± 2.2% (p = 0.000) and 26.0 ± 1.9 g/dL versus 26.8 ± 1.4 g/dL (p = 0.002), respectively). Notably, there was a negative correlation between DNI and MPC, with the strength of the correlation varying based on the cause of sepsis. By setting the cutoff value of the DNI to 6.2%, its sensitivity, specificity, and NPV improved to 100%, 80.3%, and 100%, respectively, although the PPV remained at 10.6%. Conclusions: In our study, the DNI demonstrates superior effectiveness compared with other molecular biomarkers, such as CRP and procalcitonin, in distinguishing septic febrile patients from non-septic febrile patients. Additionally, a negative correlation exists between MPC and DNI, making MPC a valuable marker for differentiating the etiology of sepsis. These findings hold significant clinical implications, as DNI/MPC evaluation is a cost-effective and readily applicable approach in various impending sepsis scenarios. Notably, this study represents the first examination of the prognostic and diagnostic validity of employing the simultaneous evaluation of DNI and MPC in human cases of febrile sepsis.

The objective of this study was to investigate the efficacy of pulse transit time (PTT)-pulse wave analysis (PWA) fusion in cuff-less blood pressure (BP) trend tracking based on the wearable wrist ballistocardiogram (BCG). We constructed PTT and 36 candidate BCG PWA features based on the BCG and photoplethysmogram (PPG) signals acquired at the wrist. We performed a model-based analysis to select 6 candidate BCG PWA features most sensitive to BP. We aggregated the 6 BCG PWA features into novel predictors of diastolic, systolic, and pulse BP (DP, SP, and PP) orthogonal to PTT by covariance-maximizing dimensionality reduction. Then, we evaluated and compared the efficacy of BCG PTT and BCG PTT-PWA fusion in tracking the trend of DP, SP, and PP. Unique innovations of this study, generalizable to a range of physiological signals beyond the BCG, are (i) the systematic selection of PWA features based on model-based analysis and (ii) the development of PWA-based BP predictors orthogonal to PTT. Using the experimental wrist BCG and PPG signals collected from 23 human subjects, we demonstrated that (i) BCG PTT-PWA fusion may significantly outperform BCG PTT; (ii) PTT may play the primary role in tracking BP while BCG PWA features may still have an independent and complementary value (especially in tracking PP); and (iii) model-based selection of BCG PWA features can enhance the robustness of BP trend tracking based on BCG PTT-PWA fusion.

This study explores the effects of hydrothermal carbonization (HTC) on the fuel properties of empty fruit bunches (EFB) by varying the reaction temperatures between the range of 180–300 °C. The improved properties of hydrochars following HTC were achieved by analyzing the changes in the physical and chemical properties of EFB. Moreover, it can save energy during treatment processes, in addition to evaluating the improvement of the biofuel stability based on the equilibrium moisture content and agglomeration. The results showed that the chemical structure of EFB decomposed owing to dehydration and decarboxylation reactions, leading to increased carbon and fixed carbon concentrations in the obtained hydrochar; and thus, an increased calorific value. Hydrochar generated during HTC exhibited chemical properties similar to those of conventional coal fuel. The optimal HTC reaction temperature range was ~230–250 °C. Based on the X-ray fluorescence results, hydrochar produced during HTC had smaller K and Cl contents and a smaller degree of agglomeration than the original sample, indicating that clinker production can be reduced by the HTC of EFB. The results of this study have significance in the utilization of unused waste biomass as an energy source that can replace fossil fuels.

The study analyzed bibliometric data of papers published in Korea Citation Index (KCI) and Web of Science (WoS) journals from 2002 to 2021. After examining size differences of KCI and WoS domains in the number of authors, institutions, and journals to put publication and citations counts in perspective, the study investigated co-authorship patterns over time to compare collaboration trends of Korean and international scholars and analyzed the data at author, institution, and journal levels to explore how the influences of authors, institutions, and journals on research output differ across domains. The study also conducted frequencybased analysis of keywords to identify key topics and visualized keyword clusters to examine topic trends. The result showed Korean LIS authors to be twice as productive as international authors but much less impactful and Korean institutions to be at comparable levels of productivity and impact in contrast to much of productivity and impact concentrated in top international institutions. Citations to journals exhibited initially increasing pattern followed by a decreasing trend though WoS journals showed far more variance than KCI journals. Co-authorship trends were much more pronounced among international publication, where larger collaboration groups suggested multi-disciplinary and complex nature of international LIS research. Keyword analysis found continuing diversification of topics in international research compared to relatively static topic trend in Korea. Keyword visualization showed WoS keyword clusters to be much denser and diverse than KCI clusters. In addition, key keyword clusters of WoS were quite different from each other unlike KCI clusters which were similar.

ObjectiveThe effectiveness of health education on adolescents has been questioned, along with a growing body of empirical studies documenting the absence of behavioral changes after the intervention. However, evidence on its impact on other crucial health domains, besides health practices, is lacking. We evaluated the causal effects of a school-based health education program on adolescents' multidimensional psychological health factors.DesignA cluster-randomized controlled trial.MethodsWe conducted a cluster-randomized controlled trial involving 140 lower secondary schools in Vietnam. After stratifying by district, schools were randomized 1:1 to either treatment or control groups. Students enrolled in the treatment schools received monthly stand-alone health education in five topics by school teachers at the class level, but control group students did not receive any intervention. The primary outcomes of the study were students' non-cognitive skills, life satisfaction, aspirations gap, and the Health-Related Quality of Life at five-month follow-up. We estimated the intention-to-treat effects with the panel fixed effects model using student panel data.ResultsOf the 6,477 students enrolled at baseline, 2,958 (92%) treated and 2,967 (91%) control students completed the follow-up survey five months after baseline data collection from October to December 2018. Compared with controls, health education led to improved treatment school students' self-efficacy (p-value = 0.013), presumed life satisfaction five years from the present (p-value = 0.001), aspirations gap for a socially and mentally healthy future (p-value = 0.036), and the Health-Related Quality of Life (p-value = 0.036).ConclusionA school-based health education program enhanced students' non-cognitive skills, life satisfaction and aspirations gap, and the Health-Related Quality of Life significantly. This study proposes essential psychological factors that should be taken into account when evaluating the effectiveness of a health education program in resource-limited settings.

Sepsis is an emergent infectious disease and a leading cause of death despite immediate intervention. While Delta neutrophil index (DNI) and myeloperoxidase (MPO) are known as a prodiagnostic marker of sepsis, the preclinical evidence of the best marker of sepsis is unclear. For this, using a well-designed cecal ligation and puncture (CLP)-induced sepsis mouse model, we comparatively measured the level and cost-effectiveness of sepsis biomarkers such as DNI, myeloperoxidase (MPO), procalcitonin (PCT), and tumor necrosis factor-alpha (TNF-α). First, we found that the optimal time point for early detection is at 6 h, 24 h post-CLP. Strikingly, the peak level and fold change of DNI was revealed at 24 h, further showing the best fold change as compared with other biomarker levels. Given the fold change at 6, 24 h, PCT was next to DNI. Third, a cost-effectiveness survey showed that DNI was the best, with PCT next. Further, DNI level was moderate positively associated with PCT (ρ = 0.697, p = 0.012) and TNF-α (ρ = 0.599, p = 0.040). Collectively, these data indicate that DNI in CLP-induced sepsis mice is as effective as the existent inflammatory biomarkers such as MPO, PCT and TNF-α to predict the prognosis of sepsis. This might have clinically important implications that DNI is cost effective, thus quickly and rationally applying to diverse types of imminent sepsis regardless of species. This might be the first report on the validity of DNI in preclinical CLP-induced murine sepsis.

Background: Helicobacter pylori infection in children causes gastrointestinal symptoms and iron deficiency anemia. This study aimed to investigate trends in H. pylori stool antigen (HpSA) positivity in children and the relationship between HpSA test results and anemia.Methods: We analyzed the results of 2,762 HpSA tests and the correlation of hemoglobin and ferritin with HpSA in patients aged 0-18 years from 2008 to 2014 at a tertiary care center. Additionally, we prospectively evaluated HpSA test results and correlation with hemoglobin in 352 specimens obtained from five centers.Results: From 2008-2014, the mean positive rate of the HpSA test was 5.8%, with a high of 9.1% in 2012 and a low of 2.3% in 2013. We propose a framework based on the 5W1H model-based metadata for remote diagnosis in virtual reality (VR). For this purpose, we suggest unique metadata composed of Point of Interest (POI)-extended anchor (xAnchor)-content for a context-aware service in virtual and augmented reality. We define the attributes of the metadata based on the 5W1H context for information retrieval according to the context in a remote diagnosis. Second, we propose the ontology-based linker metadata that express the relations between AR scenes and that retrieve external information. Moreover, we suggest heritage building information metadata for information retrieval according to context. For evaluation, we created a geo-tagged content tool and a remote diagnosis VR application. We conducted focus-group interviews and heuristic evaluations for remote diagnosis in VR to verify the methodology of this study. As a result, we found that experts were most satisfied with the functions that provide the contextualized information. This study contributes to the geospatial metadata for a context-aware service in VR/AR as well as the remote diagnosis framework to overcome the time-consuming problem of the existing remote diagnosis.

There are the national science and technology standard classification system used in Korea, the classification according to the standard classification system for educational organization units, and the Korean standard education classification by the National Statistical Office. It is not suitable for calculation or evaluation, and classification is still mixed depending on the purpose of use. Therefore, in this study, the current status of academic standard classification, issues related to the standard classification system such as research foundation associations and research institutes, and issues related to the academic standard classification through the analysis of existing prior research issues, etc. As a result of the research, first, it is necessary to maintain and strengthen the linkage of the academic classification system, such as maintaining the linkage between the relevant departmental classification systems and strengthening the linkage with the relevant classification system, as a result of analysis of major issues in the academic standard classification system, and the systematic improvement cycle of the revision process and management system and settings are required.
