13. Wound Complications Following Cleft Repair in Children with Van der Woude Syndrome

Abstract

Van der Woude syndrome (VWS; OMIM 119300) is an autosomal-dominant condition associated with clefts of the lip and/or palate and lower lip pits and is caused by mutations in interferon regulatory factor 6 (IRF6). The standard of practice for children born with cleft lip/palate is surgical repair, which requires proper wound healing. We tested the hypothesis that children with VWS are more likely to have wound complications after cleft repair than children with nonsyndromic cleft lip/palate (NSCLP). Furthermore, we hypothesized that children with VWS have more surgical procedures. A retrospective, case-controlled study was performed. Seventeen children with VWS and 68 matched controls with NSCLP were scored for the presence of wound complications after cleft repair, for the severity of complications, and for number of surgeries from age 0 to 10. Of the 17 children with VWS, 8 had wound complications. Of 68 controls, 13 had wound complications (P = 0.02). Of 8 wound complications in the VWS group, 6 were major, whereas of 13 complications in the control group, 9 were major (P = 0.04). Most wound complications were fistulae and occurred in isolated cleft palate and bilateral cleft lip. The mean number of surgeries in the VWS group was 3.0 compared with 2.8 in the control group (P = 0.67). Our studies suggest that children with VWS have an increased risk for wound complications after cleft repair compared with children with NSCLP. Furthermore, these data support a role for IRF6 in wound healing.

14. Adding Live-Streaming to Recorded Lectures in a Non-Distributed Pre-Clerkship Medical Education Model.

Abstract

Background: Live-streaming video has had increasing uses in medical education, especially in distributed education models. The literature on the impact of live-streaming in non-distributed education models, however, is scarce. Objectives: To determine the attitudes towards live-streaming and recorded lectures as a resource to pre-clerkship medical students in a non-distributed medical education model. Methods: First and second year medical students were sent a voluntary cross-sectional survey by email, and were asked questions on livestreaming, recorded lectures and in person lectures using a 5-point Likert and open answers. Results; Of the 118 responses (54% response rate), the data suggested that both watching recorded lectures (Likert 4.55) and live-streaming lectures (4.09) were perceived to be more educationally valuable than face-to-face attendance of lectures (3.60). While responses indicated a statistically significant increase in anticipated classroom attendance if both live-streaming and recorded lectures were removed (from 63% attendance to 76%, p =0.002), there was no significant difference in attendance if live-streaming lectures were removed but recorded lectures were maintained (from 63% to 66%, p=0.76). Conclusion: The addition of live-streaming lectures in the pre-clerkship setting was perceived to be value added to the students. The data also suggests that the removal of livestreaming lectures would not lead to a statistically significant increase in classroom attendance by pre-clerkship students. [ABSTRACT FROM AUTHOR]

Abstract

Eighteen human intestinal microbial strains were tested for the presence of cytochrome P450, an enzyme involved in detoxifying and toxifying various xenobiotics. The polymerase chain reaction (PCR) technique was used to amplify conserved regions within the gene. From the human intestinal microflora Eubacterium aerofaciens, a DNA fragment of the expected size could be amplified, for which a percentage homology of 20% was determined. Southern blot analysis, using Pseudomonas putida P450 CAM probes, showed reactions with probes at 45 and 55°C. The carbon monoxide (CO) difference spectrum produced a P450-like profile for E. aerofaciens, similar to P. putida P450 CAM. The presence of cytochrome P450 in human intestinal flora may influence the expression of hepatic cytochrome P450s. [ABSTRACT FROM AUTHOR]

25. Sexual Offender, Sexual Abuse Victim, and Generalist Population Therapists' Perceptions of Permissive Parent-Child Sexual Boundaries and Altered Perceptions of Self, Others, and Adaptation to the World as a Result of Vicarious Trauma

Abstract

The purpose of this study was to determine if therapist levels (therapists who treat sexual offenders, sexual abuse victims, and general population clients) differ in terms of therapists overall perceptions of permissive parent-child sexual boundaries and therapists altered perceptions of self, others, and adaptation to the world as a result of vicarious trauma. Participants in this study consisted of graduate level therapists holding membership in one of three professional organizations: (a) Association for Treatment of Sexual Abusers (ATSA), (b) American Professional Society on the Abuse of Children (APSAC), (c) American Mental Health Counselors Association (AMHCA). Data were collected using four instruments: (a) Trauma and Attachment Belief Scale (TABS; Pearlman, 2003), (b) Secondary Traumatic Stress Scale (STSS; Bride, 1999), and (c) Permissiveness of Parent- Child Sexual Boundary Scale (PPCSBS), and (d) demographic questionnaire. The multivariate analysis of variance (MANOVA) was used to analyze the data.Significant differences in the levels of therapists were found based upon professional organization membership. However, examination of the effect size (.05) found the differences between the therapist levels to be quite small and unimpressive. ATSA members reported significant differences in increased disrupted cognitive schemas (as measured by TABS) compared to APSAC members and AMHCA members. ATSA members scored significantly higher on the TABS subscales for Other-Safety, Other-Trust, and Other-Esteem. AMHCA members reported significant differences in permissive parent-child sexual boundaries behaviors (as measured by PPCSBS) compared to APSAC and ATSA members. AMHCA members reported sexual boundary behaviors to be appropriate for only younger age children, while the APSAC and ATSA members reported sexual boundary behaviors to be appropriate for both younger age and slightly older age children. No significant differences in the levels of therapists were found based upon the number of hours per week treating sexual offender and sexual abuse victim clients together. However, supplemental analyses found increased TABS subscale scores of Other-Safety, Other-Trust, and Other-Esteem for therapists treating sexual offender clients 20 or more hours per week. No significant differences in the levels of therapists were found based upon the number of years of clinical experience. Lastly, examination of the PPCSBS instrument found the scale to be unrelated to the TABS and STSS. The TABS and STSS were found to be highly correlated instruments of vicarious trauma. Thus it appears that therapists' perceptions of permissive parent-child sexual boundaries are unrelated to symptoms of vicarious trauma.
