Placental blood flow analysis is complicated by having both maternal and fetal flow components. Using the Fast Fourier Transform (FFT) of the umbilical venous pulse wave spectra (PW) envelope, we could simultaneously assess maternal/fetal blood flow in the placenta and investigate if normal and intrauterine growth restriction (IUGR)/pre-eclamptic pregnancies could be distinguished.This retrospective study included normal gestations (N = 11) and gestations with IUGR, pre-eclampsia, or both (N = 13). Umbilical vein PW were acquired and spectral envelopes were identified as a function of time and analyzed by FFT. Base-10 logarithms of the ratios of the maternal/fetal spectral peaks (LRSP) were compared in normal and IUGR/pre-eclamptic populations (two-tailed t-test). Body mass index (BMI), gestational age at scan time, placental position, and weight-normalized umbilical vein blood volume flow (two-tailed t-test, analysis of variance [ANOVA] analysis) were tested. P .05 was considered significant.The LRSP for normal and IUGR/pre-eclamptic pregnancies were 0.141 ± 0.180 and -0.072 ± 0.262 (mean ± standard deviation), respectively (P = .033). We detected differences between normal gestations and combinations of LRSP and weight-normalized umbilical venous blood flows. Placental effects based on LRSPs and blood flow may act synergistically in cases with both pre-eclampsia and IUGR (P = .014). No other significant associations were seen.In this preliminary study, we showed that umbilical venous flow contains markers related to placental maternal/fetal blood flow, which can be used to assess IUGR and pre-eclampsia. When coupled with umbilical cord blood flow, this new marker may potentially identify the primary causes of the two conditions.

This review summarizes the current state of non-invasive diagnosis of non-alcoholic fatty liver disease (NAFLD). This begins with a brief discussion of blood-based analysis (serum biomarkers) then progresses through various imaging modalities (imaging biomarkers) including magnetic resonance (MR), computed tomography (CT), and ultrasound-based imaging methods. The review concludes with comment on the advantages, disadvantages, and prospects of commercially available modalities and the impact they may have on diagnosis and management of patients with NAFLD.

Measurement of blood flow to the brain in neonates would be a very valuable addition to the medical diagnostic armamentarium. Such conditions such as assessment of closure of a patent ductus arteriosus (PDA) would greatly benefit from such an evaluation. However, measurement of cerebral blood flow in a clinical setting has proven very difficult and, as such, is rarely employed. Present techniques are often cumbersome, difficult to perform and potentially dangerous for very low birth weight (VLBW) infants. We have been developing an ultrasound blood volume flow technique that could be routinely used to assess blood flow to the brain in neonates. By scanning through the anterior fontanelles of 10 normal, full-term newborn infants, we were able to estimate total brain blood flows that closely match those published in the literature using much more invasive and technically demanding methods. Our method is safe, easy to do, does not require contrast agents and can be performed in the baby's incubator. The method has the potential for monitoring and assessing blood flows to the brain and could be used to routinely assess cerebral blood flow in many different clinical conditions.

Background In March 2020, the Mount Sinai Health System Inflammatory Bowel Disease (IBD) center reported an increase in telephone call volume, with many IBD patients expressing anxiety about being on immunosuppressive agents during the COVID-19 pandemic. Consistent with GI society and CDC recommendations, we leveraged the Rx.Universe platform (Rx.Health, New York, NY) to rapidly design and deliver a population-based digital navigation program (DNP) to provide outreach, remote COVID-19 symptom monitoring, triage, and Telehealth to IBD patients. Methods After identifying all IBD patients seen in our IBD center from Electronic Health Records (Epic Systems), we “bulk prescribed” the DNP (Rx.Health, New York NY) to 6100 patients’ smartphones. Patients were asked to reply to the prompt if they had new or worsening COVID-19 symptoms and opted-in to regular digital monitoring through an electronic patient reported outcome (ePRO) instrument. Patient data was screened by our clinical coordinators, who directly contacted patients via phone calls and scheduled testing and Telehealth visits with IBD practitioners when appropriate. Results Of the 6100 patients who were sent the DNP, 1829 patients opted-in to be regularly monitored using text-based electronic patient reported outcome (ePRO) instruments. Of those who responded affirmatively, 145 patients were identified requiring additional medical attention and were triaged using Telehealth visits. Compared to patients who chose not to opt-in, patients who opted-in were more likely to be female, white, married, on biologics, and had high inflammatory markers (Table 1). Conclusion As demonstrated by the 30% of patients who opted-in to regular COVID-19 symptom monitoring, a digital navigation program population approach is an effective and efficient approach to provide continuity of care and to mitigate COVID-19 exposure in a high-risk, immunosuppressed IBD population. This scalable approach serves as a model for providing high quality, remote monitoring to patients during COVID-19 and beyond, as well as achieving “Treat to Target” goals.

The measurement of blood volume flow is critical in many clinical applications. The current state-of-the-art for volume flow quantification uses pulsed-wave ultrasound, using a method that is neither accurate nor reliable. In this work, we implemented and demonstrated the feasibility of a 3D-based volume flow method on a Philips 3D xMATRIX ultrasound probe (XL14-3). To improve the workflow, an optimal scan sequence termed “iSTIC” was adopted for 4D color data acquisition. The acquisition time to obtain >20 volumes with high spatio-temporal resolution was controlled to within 15 seconds, which makes it feasible for clinical practice. This framework and repeatable measurements for volume flow estimation were tested on a customized flow phantom. Our experimental results showed comparable measurements between the 3D method and flow meter reference on the flow phantom. Under both constant and pulsatile flow conditions, we tested the performance of 3D volume flow by varying the color gain and pump flow rate. In constant flow, for the two aforementioned parameter changes, the measurements showed average biases of −2.7% and 2.9%, respectively. The corresponding coefficient of variation were 3.5% and 2.9%. Under pulsatile flow, the corresponding average biases were 3.3% and −3.6%, with coefficients of variation of 3.2% and 5.0%, respectively. These results indicate the potential of the 3D implementation to be a quick, reliable and accurate volume flow estimation technique and ready for the next step of clinical investigations.

BACKGROUND: Quantitative blood flow (QBF) measurements that use pulsed-wave US rely on difficult-to-meet conditions. Imaging biomarkers need to be quantitative and user and machine independent. Surrogate markers (eg, resistive index) fail to quantify actual volumetric flow. Standardization is possible, but relies on collaboration between users, manufacturers, and the U.S. Food and Drug Administration. PURPOSE: To evaluate a Quantitative Imaging Biomarkers Alliance–supported, user- and machine-independent US method for quantitatively measuring QBF. MATERIALS AND METHODS: In this prospective study (March 2017 to March 2019), three different clinical US scanners were used to benchmark QBF in a calibrated flow phantom at three different laboratories each. Testing conditions involved changes in flow rate (1–12 mL/sec), imaging depth (2.5–7 cm), color flow gain (0%–100%), and flow past a stenosis. Each condition was performed under constant and pulsatile flow at 60 beats per minute, thus yielding eight distinct testing conditions. QBF was computed from three-dimensional color flow velocity, power, and scan geometry by using Gauss theorem. Statistical analysis was performed between systems and between laboratories. Systems and laboratories were anonymized when reporting results. RESULTS: For systems 1, 2, and 3, flow rate for constant and pulsatile flow was measured, respectively, with biases of 3.5% and 24.9%, 3.0% and 2.1%, and −22.1% and −10.9%. Coefficients of variation were 6.9% and 7.7%, 3.3% and 8.2%, and 9.6% and 17.3%, respectively. For changes in imaging depth, biases were 3.7% and 27.2%, −2.0% and −0.9%, and −22.8% and −5.9%, respectively. Respective coefficients of variation were 10.0% and 9.2%, 4.6% and 6.9%, and 10.1% and 11.6%. For changes in color flow gain, biases after filling the lumen with color pixels were 6.3% and 18.5%, 8.5% and 9.0%, and 16.6% and 6.2%, respectively. Respective coefficients of variation were 10.8% and 4.3%, 7.3% and 6.7%, and 6.7% and 5.3%. Poststenotic flow biases were 1.8% and 31.2%, 5.7% and −3.1%, and −18.3% and −18.2%, respectively. CONCLUSION: Interlaboratory bias and variation of US-derived quantitative blood flow indicated its potential to become a clinical biomarker for the blood supply to end organs. © RSNA, 2020 Online supplemental material is available for this article. Video review for quality and education purposes has been a valued tool for decades. However, the use of this process dropped significantly after the development of the Health Insurance Portability and Accountability Act in the 1990s. Video review was recently reestablished at our institution. By working with our institutional legal counsel and risk management team, we have been able to create a video review process that complies with legal requirements. Literature on this subject has not described the process of obtaining video recordings. We aimed to review the process of obtaining high quality recordings in a secure manner. We hope that in the future, the data collected through our multidisciplinary review process will be helpful in improving quality of care for injured patients and providing coaching and feedback to learners, as well as improving our trauma education curriculum.

OBJECTIVES The purpose of this study was to compare hepatic stiffness on ultrasound (US) shear wave elastography (SWE) in patients with chronic graft-versus-host disease (GVHD) after allogeneic hematopoietic stem cell transplantation versus patients with no underlying liver disease. METHODS We performed a retrospective analysis of 4901 patients who underwent abdominal US examinations with adjunctive liver SWE between August 2014 and December 2016. Each patient was scanned supine with gentle breath holding on LOGIQ E9 (GE Healthcare, Waukesha, WI) or Epiq (Philips Healthcare, Andover, MA) US machines (3-6 MHz). Three to 10 measurements were made intercostally in the right hepatic lobe, following manufacturers' guidelines before release of the 2015 Society of Radiologists in Ultrasound consensus or the 2015 Society of Radiologists in Ultrasound consensus. The median and standard deviation of the shear wave velocity (SWV) were obtained. A 2-sample t test with the Welch approximation was used for statistical analysis. RESULTS Six patients had documented hepatic chronic GVHD or a high clinical suspicion of liver chronic GVHD. All had normal pretransplant liver function test results and no pretransplant or posttransplant hepatic infection. The control group, obtained from the same database, contained 10 patients with normal liver function test results, no abdominal pain, and no history of liver disease or conditions that may have caused liver stiffness changes. Objectives To investigate the association between umbilical vein blood volume flow and the condition of preeclampsia in an at-risk maternal patient cohort. Umbilical vein volume flow was quantified by a 3-dimensional (3D) sonographic technique that overcomes several limitations of standard sonographic flow measurement methods. Methods A total of 35 patients, each with a singleton pregnancy, were recruited to provide 5 patients with preeclampsia, derived as a subset from a 26-patient at-risk group, and 9 patients with normal pregnancies. An ultrasound system equipped with a 2.0-8.0-MHz transducer was used to acquire multivolume 3D color flow and power mode data sets to compute the mean umbilical vein volume flow in patients with normal pregnancies and preeclampsia. Results The gestational ages of the pregnancies ranged from 29.7 to 34.3 weeks in the patients with preeclampsia and from 25.9 to 34.7 weeks in the patients with normal pregnancies. Comparisons between patients with normal pregnancies and those with preeclampsia showed weight-normalized flow with a moderately high separation between groups (P = .11) and depth-corrected, weight-normalized flow with a statistically significant difference between groups (P = .035). Umbilical vein volume flow measurements were highly reproducible in the mean estimate, with an intrapatient relative SE of 12.1% ± 5.9% and an intrameasurement relative SE of 5.6% ± 1.9 %. In patients who developed pregnancy-induced hypertension or severe pregnancy-induced hypertension, umbilical vein volume flow suggested gestational hypertensive disorder before clinical diagnosis. Conclusions Results indicate that mean depth-corrected, weight-normalized umbilical vein volume flow is reduced in pregnancies complicated by preeclampsia and that volume flow may indicate hypertensive disorder earlier in gestation. Volume flow measurements are highly reproducible, and further study in a larger clinical population is encouraged to determine whether 3D volume flow can complement the management of preeclampsia and, in general, at-risk pregnancy.

OBJECTIVES We are studying a new method for estimating blood volume flow that uses 3-dimensional ultrasound to measure the total integrated flux through an ultrasound-generated Gaussian surface that intersects the umbilical cord. This method makes none of the assumptions typically required with standard 1-dimensional spectral Doppler volume flow estimates. We compared the variations in volume flow estimates between techniques in the umbilical vein. METHODS The study was Institutional Review Board approved, and all 12 patients gave informed consent. Because we had no reference standard for the true umbilical vein volume flow, we compared the variations of the measurements for the flow measurement techniques. At least 3 separate spectral Doppler and 3 separate Gaussian surface measurements were made along the umbilical vein. Means, standard deviations, and coefficients of variation (standard deviation/mean) for the flow estimation techniques were calculated for each patient. P

This study investigates on the molecular and mechanical markers of intestinal fibrosis in Crohn’s Disease using endoscopic photoacoustic imaging. The quantitative imaging results in vivo are validated by histology and microelastometry.

Objectives To determine whether contrast-enhanced sonographic quantitative perfusion parameters can detect bowel wall fibrosis in the setting of mixed inflammatory and fibrotic lesions in a Crohn disease animal model. Methods This study was approved by the institutional Committee on the Use and Care of Animals. Multiple (range, 1–5) 2,4,6-trinitrobenzenesulfonic acid–ethanol enemas were used to create intestinal inflammatory lesions with variable fibrosis in female Lewis rats. Low–mechanical index contrast-enhanced sonography was performed 3 days after the final enema using a 0.2-mL bolus of sulfur hexafluoride microbubbles injected through a tail vein. Contrast-enhanced sonographic data were analyzed with software that converts video data into echo-power (linearized) data. Colorectal lesions were scored for histopathologic inflammation and fibrosis; bowel wall collagen was quantified by Western blotting. The Spearman correlation was used to assess associations between contrast-enhanced sonographic quantitative parameters and bowel wall collagen; the Kruskal-Wallis test was used to compare continuous results between histopathologic groups. Results Thirty-one animals were included in our analysis. Animals were placed into 3 histopathologic cohorts: (1) severe bowel wall inflammation/minimal or no fibrosis (n = 11); (2) severe bowel wall inflammation/moderate fibrosis (n = 9); and (3) severe bowel wall inflammation/severe fibrosis (n = 11). Western blotting showed a significant difference in bowel wall collagen between histopathologic cohorts (P = .0001). There was no correlation between any contrast-enhanced sonographic quantitative parameter and bowel wall collagen (P > .05). There was no difference between histopathologic cohorts for any contrast-enhanced sonographic quantitative parameter (P > .05). Conclusions Contrast-enhanced sonographic quantitative perfusion parameters failed to effectively detect bowel wall fibrosis in the setting of superimposed inflammation in a Crohn disease animal model.

The aim of this study was to evaluate the feasibility of ultrasound strain elastography (SE) for the assessment of resting biceps brachii muscle (BBM) stiffness in patients with Parkinson's diseases (PD). From May 2014 to December 2014, we prospectively performed SE of BBM in 14 patients with PD and 10 healthy controls. Based on the Unified Parkinson's Disease Rating Scale for scoring muscle rigidity (UPDRS, part III), muscle rigidity scores in 14 patients with PD included 3 patients with high rigidity (UPDRS III-IV) and 11 patients with low rigidity (UPDRS I-II). Ultrasound strain was represented by the deformation of the BBM and subcutaneous soft tissues that was produced by external compression with a sand bag (1.5 kg) tied onto an ultrasound transducer. Deformation was estimated with two-dimensional speckle tracking. The difference in strain ratio (SR, defined as mean BBM strain divided by mean subcutaneous soft tissue strain) between PD and healthy controls was tested by unpaired t test. The correlation between SR and muscle rigidity score was analyzed by Pearson correlation coefficient. The reliability of SR in assessment of BBM stiffness was tested using intraclass correlation coefficient. In our result, the SR in PD and healthy controls measured 2.65±0.36 and 3.30±0.27, respectively. A significant difference in SR was noted between the healthy controls and PD (P=.00011). A negative correlation was found between SR and UPDRS rigidity score (r=-0.78). Our study suggests that the SR of BBM to reference tissue can be used as a quantitative biomarker in assessing resting muscle stiffness associated with muscle rigidity in PD.

Over the past decade, there has been increasing evidence that ultrasound elastography may aid in the imaging evaluation and management of pediatric and adult Crohn’s disease patients. Multiple studies have shown that ultrasound-derived intestinal stiffness measurements can suggest the presence of bowel wall fibrosis in areas of stricturing disease, even in the setting of superimposed inflammation. Such knowledge can help guide the appropriate medical and surgical management of these patients. Furthermore, semiquantitative and quantitative elastographic methods may predict response to medical therapy, show evidence of progressive bowel damage over time, and potentially predict ensuing complications, such as impending bowel obstruction or penetrating complications. This chapter will review the different ultrasound elastography techniques for assessing the bowel, published evidence supporting the use of these techniques in Crohn’s disease patients, potential roles in clinical practice, and likely challenges and obstacles to future clinical use.

Introduction Post-stroke spasticity, a leading cause of disability, affecting 12 million stroke survivors in the world. Local muscle injection of Botulinum Toxin (BoNT) is a common treatment for post-stroke spasticity, which is lack of quantitative assessment in the effectiveness of the treatment. Commonly used Modified Ashworth Scale in clinical assessment is subjective. Ultrasound elastography has proven to be useful in the assessment of muscle mechanical properties. The aim of the study was to evaluate the feasibility of ultrasound elastography to assess the efficacy of BoNT treatment for post-stroke spasticity of the upper limb. Methods We received IRB approval and written inform consents prior to the ultrasound scanning. In this prospective study, we performed ultrasound elastography (ultrasound strain elastography and ARFI based shear wave elastography) of the spastic biceps muscle in seven subjects (5 men and 2 women, mean age 45 years) who underwent biceps muscle BoNT injection for post-stroke spasticity of the upper limb. We measured ultrasound elastography parameters including axial strain ratio (Strain ratio=muscle strain/reference subcutaneous tissue strain), longitudinal strain ratio, and shear wave velocity of the spastic biceps muscle immediately prior to and 17-30 days (mean 22 days) after BoNT injection treatment. Statistical analyses: Two-tailed paired t-test was used to examine the difference in ultrasound elastography parameters of the treated muscles before and after BoNT injection, and the results was showed with box-and-whicker plots. Spearman rank correlation coefficient (rs) was applied to analyze the correlation of ultrasound elastography parameters to clinical assessment with Modified Ashworth Scale (MAS) and Tardieu Scale (TS). Results The difference in ultrasound elastography parameters before and after BoNT injection was significant (p Conclusion Our results suggest that ultrasound elastography is feasible to assess the effectiveness of BoNT treatment for post-stroke spasticity of the upper limb. Further study on validating ultrasound elastography in post-stroke spasticity management in large population is warranted.

Being able to measure 3-D flow velocity and volumetric flow rate effectively in the cardiovascular system is valuable but remains a significant challenge in both clinical practice and research. Currently, there has not been an effective and practical solution to the measurement of volume flow using ultrasound imaging systems due to challenges in existing 3-D imaging techniques and high system cost. In this study, a new technique for quantifying volumetric flow rate from the cross-sectional imaging plane of the blood vessel was developed by using speckle decorrelation (SDC), 2-D high-frame-rate imaging with a standard 1-D array transducer, microbubble contrast agents, and ultrasound imaging velocimetry (UIV). Through SDC analysis of microbubble signals acquired with a very high frame rate and by using UIV to estimate the two in-plane flow velocity components, the third and out-of-plane velocity component can be obtained over time and integrated to estimate volume flow. The proposed technique was evaluated on a wall-less flow phantom in both steady and pulsatile flow. UIV in the longitudinal direction was conducted as a reference. The influences of frame rate, mechanical index (MI), orientation of imaging plane, and compounding on velocity estimation were also studied. In addition, an in vivo trial on the abdominal aorta of a rabbit was conducted. The results show that the new system can estimate volume flow with an averaged error of 3.65% ± 2.37% at a flow rate of 360 mL/min and a peak velocity of 0.45 m/s, and an error of 5.03% ± 2.73% at a flow rate of 723 mL/min and a peak velocity of 0.8 m/s. The accuracy of the flow velocity and volumetric flow rate estimation directly depend on the imaging frame rate. With a frame rate of 6000 Hz, a velocity up to 0.8 m/s can be correctly estimated. A higher mechanical index (MI = 0.42) is shown to produce greater errors (up to 21.78±0.49%, compared to 3.65±2.37% at MI = 0.19). An in vivo trial, where velocities up to 1 m/s were correctly measured, demonstrated the potential of the technique in clinical applications.

To our knowledge, this phenomenon has not been previously reported in chronic GVHD and suggests potential utility of SWE for diagnosis and monitoring of disease progression and the treatment response in this cohort of patients.

Objectives The aim of the study was to assess the feasibility of ultrasound strain imaging in characterizing the biceps brachii muscle in chronic poststroke spasticity. Methods We prospectively analyzed strain imaging data from bilateral biceps brachii muscles in 8 healthy volunteers and 7 patients with poststroke chronic spasticity. Axial deformations of the biceps brachii muscle and overlying subcutaneous tissue were produced by external compression using a sandbag (1.0 kg) attached to a transducer. The lengthening and shortening of the biceps brachii muscle and subcutaneous tissue were produced by manual passive elbow extension (from 90° to 0°) and flexion (from 0° to 90°), respectively. We used offline 2-dimensional speckle tracking to estimate axial and longitudinal strain ratios (biceps brachii strain/subcutaneous tissue strain), and the longitudinal tissue velocity of the biceps brachii muscle. Statistical analyses included analysis of variance for testing differences in strain imaging parameters among healthy, nonspastic, and spastic biceps brachii muscles, the Bonferroni correction for further testing differences in US strain imaging among paired groups (healthy versus spastic, nonspastic versus spastic, and healthy versus nonspastic), and the Pearson correlation coefficient for assessing the intraobserver reliability of performing strain imaging in stroke survivors. Results The differences in strain imaging parameters between healthy and spastic and between nonspastic and spastic biceps brachii muscles were significant at both 90° elbow flexion and maximal elbow extension (P .05). The intraobserver reliability of performing strain imaging in stroke survivors was good (r = 0.85; P Conclusions Ultrasound strain imaging seems to be feasible for characterizing the biceps brachii muscle in chronic poststroke spasticity.

The change of tissue elasticity has been recognized as a biomarker of many disease conditions. Elastography has been investigated by observing the strain and stress correlation or shear wave propagation in tissue. The strain measurement can be achieved via speckle tracking in ultrasound (US) and optical modalities whereas the stress in deep tissue cannot be directly measured. Assuming that the collapsing of the vasculature could reflect the stress exerted on a tissue volume, the photoacoustic (PA) signals of the hemoglobin content within the vasculature could be an alternative measurement of the stress. This study investigates the strain-PA correlation with phantoms and a rat model in vivo. Parallel PA-US imaging was achieved by combining a compact linear US array and fiber optics delivering 720nm illumination. The phantom study simulated the vasculature with a piece of sponge soaked in ink, and the surrounding tissue with porcine gel with varied elasticity. The strain was generated by pushing the PA-US probe against the phantom surface. A correlation of 0.9 was found between the strain within the sponge and the PA signal changes. The rat model possesses inflammatory and fibrotic intestinal strictures comparable to those in the Crohn’s disease patients. Crohn’s disease (CD) is a chronic autoimmune disease of the intestinal tract affecting 700,000 people in the United States. The pathology of CD is characterized by obstructing intestinal strictures due to inflammation (with high levels of hemoglobin), fibrosis (with high levels of collagen), or a combination of both. The accurate characterization of the intestinal strictures is critical, as the fibrotic intestinal strictures have to be removed surgically. Currently, there is no imaging modality that can differentiate the fibrotic and inflammatory strictures. Standard diagnosis by endoscopic biopsy suffers from the post-procedure complications, and limited sampling locations and depth. Combining the optical spectroscopy and ultrasound (US) imaging, photoacoustic (PA) imaging is an ideal tool for resolving the molecular components of the intestinal strictures. This study investigates the feasibility of differentiating the fibrotic and inflammatory intestinal strictures using PA-US parallel imaging in a rat model in vivo. A linear US array was used to acquire US and PA imaging transcutaneously. PA imaging with endoscopic and transcutaneous illumination was attempted in 12 and 10 animals, respectively. To compare the capability of ultrasound strain and Doppler parameters in the assessment of renal allograft interstitial fibrosis/tubular atrophy (IF/TA), we prospectively measured ultrasound corticomedullary strain (strain) and intra-renal artery Doppler end-diastolic velocity (EDV), peak systolic velocity (PSV) and resistive index (RI) in 45 renal transplant recipients before their kidney biopsies. We used 2-D speckle tracking to estimate strain, the deformation ratio of renal cortex to medulla produced by external compression using the ultrasound transducer. We also measured Doppler EDV, PSV and RI at the renal allograft inter-lobar artery. Using the Banff scoring system for renal allograft IF/TA, 45 patients were divided into the following groups: group 1 with ≤5% (n = 12) cortical IF/TA; group 2 with 6%-25% (n = 12); group 3 with 26%-50% (n = 11); and group 4 with50% (n = 10). We performed receiver operating characteristic curve analysis to test the accuracy of these ultrasound parameters and duration of transplantation in determining26% cortical IF/TA. In our results, strain was statistically significant in all paired groups (all p0.005) and inversely correlated with the grade of cortical IF/TA (p0.001). However, the difference in PSV and EDV was significant only between high-grade (26%, including 26%-50% and50%) and low-grade (≤25%, including5% and 6%-25%) cortical IF/TA (p0.001). RI did not significantly differ in any paired group (all p0.05). The areas under the receiver operating characteristic curve for strain, EDV, PSV, RI and duration of transplantation in determining26% cortical IF/TA were 0.99, 0.94, 0.88, 0.52 and 0.92, respectively. Our results suggest that corticomedullary strain seems to be superior to Doppler parameters and duration of transplantation in assessment of renal allograft cortical IF/TA.

OBJECTIVES This study evaluated the diagnostic performance of transvaginal ultrasound (TVUS) shear wave elastography (SWE) for evaluating uterine adenomyosis and leiomyomas. METHODS Institutional Review Board approval was obtained for prospective enrollment of 34 premenopausal women with pelvic pain and/or bleeding between January 2015 and June 2016. TVUS SWE was performed with regions of interest in multiple uterine segments and shear wave velocities(SWVs) were recorded. Reference pelvic magnetic resonance examinations were performed and reviewed without access to the ultrasound results. RESULTS Continuous variables were analyzed using means, t tests, and analysis of variance. Magnetic resonance imaging revealed adenomyosis in 6 women (12 uterine segments) and leiomyomas in 12 women (28 segments). On a per-patient basis, mean SWV in 16 women with no adenomyosis or leiomyoma was 4.3 ± 1.7 m/s, compared with 5.7 ± 2.3 m/s in 18 women with a magnetic resonance diagnosis of myometrial pathology (P

The aim of the study was to assess the feasibility of ultrasound strain imaging in characterizing the biceps brachii muscle in chronic poststroke spasticity.We prospectively analyzed strain imaging data from bilateral biceps brachii muscles in 8 healthy volunteers and 7 patients with poststroke chronic spasticity. Axial deformations of the biceps brachii muscle and overlying subcutaneous tissue were produced by external compression using a sandbag (1.0 kg) attached to a transducer. The lengthening and shortening of the biceps brachii muscle and subcutaneous tissue were produced by manual passive elbow extension (from 90° to 0°) and flexion (from 0° to 90°), respectively. We used offline 2-dimensional speckle tracking to estimate axial and longitudinal strain ratios (biceps brachii strain/subcutaneous tissue strain), and the longitudinal tissue velocity of the biceps brachii muscle. Statistical analyses included analysis of variance for testing differences in strain imaging parameters among healthy, nonspastic, and spastic biceps brachii muscles, the Bonferroni correction for further testing differences in US strain imaging among paired groups (healthy versus spastic, nonspastic versus spastic, and healthy versus nonspastic), and the Pearson correlation coefficient for assessing the intraobserver reliability of performing strain imaging in stroke survivors.The differences in strain imaging parameters between healthy and spastic and between nonspastic and spastic biceps brachii muscles were significant at both 90° elbow flexion and maximal elbow extension (P .01). There was no significant difference in axial strain ratios at 90° of elbow flexion or longitudinal tissue velocities between healthy and nonspastic muscles (P .05). The intraobserver reliability of performing strain imaging in stroke survivors was good (r = 0.85; P .01).Ultrasound strain imaging seems to be feasible for characterizing the biceps brachii muscle in chronic poststroke spasticity.

Crohn's disease is a chronic autoimmune disease of the intestinal tract affecting 700,000 people in the United States. The pathology of Crohn's disease is characterized by obstructing intestinal strictures due to inflammation, fibrosis, or a combination of both. The identification of fibrosis in intestinal strictures is critical, as the fibrotic strictures are irreversible and have to be removed surgically. Fibrotic strictures are characterized by the increased collagen content in the submucosa below the inner surface of the intestine. The standard diagnostic procedure for the strictures is the endoscopic biopsy, which has limited diagnostic accuracy due to the superficial sampling. Conventional imaging modalities can identify intestinal strictures but cannot assess the molecular components within the strictures. In our previous studies, photoacoustic imaging (PAI) has demonstrated the capability to identify fibrosis in intestinal strictures by increased collagen content. The objective of this research is to further investigate the feasibility of identifying fibrosis in intestinal strictures using PAI in vivo.

OBJECTIVES—: To determine whether bowel wall fibrosis can be detected in freshly resected human intestinal specimens based on ultrasound-derived shear wave speed. METHODS—: Seventeen intact (>3-cm) bowel segments (15 small and 2 large intestine) from 12 patients with known or suspected inflammatory bowel disease were procured immediately after surgical resection. Ultrasound shear wave elastography of the bowel wall was performed by two methods (Virtual Touch Quantification [VTQ] and Virtual Touch-IQ [VT-IQ]; Siemens Medical Solutions USA, Inc, Mountain View, CA). Eighteen short-axis shear wave speed measurements were acquired from each specimen: 3 from the 9-, 12-, and 3-o’clock locations for each method. Imaging was performed in two areas for specimens greater than 10 cm in length (separated by ≥5 cm). A gastrointestinal pathologist scored correlative histologic slides for inflammation and fibrosis. Differences in mean shear wave speed between bowel segments with low and high inflammation/fibrosis scores were assessed by a Student t test. Receiver operating characteristic curve analysis was performed. RESULTS—: High-fibrosis score (n = 11) bowel segments had a significantly greater mean shear wave speed than low–fibrosis score (n = 6) bowel segments (mean ± SD: VTQ, 1.59 ± 0.37 versus 1.18 ± 0.08 m/s; P = .004; VT-IQ, 1.87 ± 0.44 versus 1.50 ± 0.26 m/s; P = .049). There was no significant difference in mean shear wave speed between high– and low–inflammation score bowel segments (P > .05 for both VTQand VT-IQ). Receiver operating characteristic curves showed areas under the curve of 0.91 (95% confidence interval, 0.67–0.99) for VTQ and 0.77 (95% confidence interval, 0.51–0.94) for VT-IQin distinguishing low– from high–fibrosis score bowel segments. CONCLUSIONS—: Ex vivo bowel wall shear wave speed measurements increase when transmural intestinal fibrosis is present.

To determine whether ultrasound strain zero-crossing elasticity measurement can be used to discriminate moderate cortical fibrosis or inflammation in renal allografts, we prospectively assessed cortical hardness with quasi-static ultrasound elastography in 38 renal transplant patients who underwent kidney biopsy from January 2013 to June 2013. With the Banff score criteria for renal cortical fibrosis as gold standard, 38 subjects were divided into two groups: group 1 (n = 18) with ≤25% cortical fibrosis and group 2 (n = 20) with >26% cortical fibrosis. We then divided this population again into group 3 (n = 20) with ≤ 25% inflammation and group 4 (n = 18) with >26% inflammation based on the Banff score for renal parenchyma inflammation. To estimate renal cortical hardness in both population divisions, we propose an ultrasound strain relative zero-crossing elasticity measurement (ZC) method. In this technique, the relative return to baseline, that is zero strain, of strain in the renal cortex is compared with that of strain in reference soft tissue (between the abdominal wall and pelvic muscles). Using the ZC point on the reference strain decompression slope as standard, we determined when cortical strain crossed zero during decompression. ZC was negative when cortical strain did not return or returned after the reference, whereas ZC was positive when cortical strain returned ahead of the reference. Fisher's exact test was used to examine the significance of differences in ZC between groups 1 and 2 and between groups 3 and 4. The accuracy of ZC in determining moderate cortical fibrosis and moderate inflammation was examined by receiver operating characteristic analysis. The intra-class correlation coefficient and analysis of variance were used to test inter-rater reliability and reproducibility. ZC had good inter-observer agreement (ICC = 0.912) and reproducibility (p = 0.979). ZCs were negative in 18 of 18 cases in group 1 and positive in 19 of 20 cases in group 2 (p ≪ 0.001), and were positive in 18 of 20 cases in group 3 and negative in 17 of 18 cases in group 4 (p ≪ 0.001). The area under the receiver operating characteristic curve was 0.992 ± 0.010 for fibrosis and 0.988 ± 0.021 for inflammation. ZC had 100% sensitivity and 95% specificity when zero strain was used as the cutoff value to determine moderate cortical fibrosis and 94% sensitivity and 90% specificity for inflammation. ZC is a new strain marker that could be straightforward to interpret and perform, making it a potentially practical approach for monitoring progression of cortical fibrosis or inflammation in renal allografts.

To quantitatively assess the contrast-to-noise ratio (CNR) of green tagging and standard color flow images in displaying fast flow velocity, we retrospectively reviewed 20 cases of hemodynamically significant renal artery stenosis (RAS) detected by renal color Doppler ultrasound and confirmed with digital subtraction angiography. At the site of RAS, blood flow with high velocity that appeared as aliasing on color flow images was computationally analyzed with both green tagging and standard color mapping. To assess the difference in the CNR between normal background flow and the aliased signal as a function of visualizing aliasing between the two color mappings, we used GetColorpixels (Chongqing Medical University, Chongqing, China) to count the values in the color channels after segmenting color pixels from gray-scale pixels. We then calculated the CNR in each color channel-red, green, and blue (RGB)--in the aliasing region on green tagging and standard color mapping. The CNRs in the red, green and blue channels were 0.35 ± 0.44, 1.11 ± 0.41 and 0.51 ± 0.19, respectively, on standard color mapping, and 0.97 ± 0.80, 4.01 ± 1.36 and 0.64 ± 0.29, respectively, on green tagging. We used a single-factor analysis of variance and two-tailed t-test to assess the difference in CNR in each color channel between the two color mappings at the site of RAS. With these comparisons, there was no significant difference in the CNR in the red or blue channel between green tagging and standard color mapping (p > 0.05). However, there was a statistically significant difference in the CNR in the green channel between the two color mappings (p = 0.00019). Furthermore, the CNR measured in the green channel on the green tagging image was significantly higher than the CNRs in all other color channels on both color mapping images (p = 0.000). Hence, we conclude that green tagging has significantly higher visibility as a function of high-velocity flow than standard color mapping. The use of green tagging may improve the ability to detect RAS with color Doppler ultrasound.

Objectives To quantitatively assess the correlation between the corticomedullary strain ratio and cortical fibrosis in renal transplants. Methods Using quasistatic ultrasound elasticity imaging, we prospectively assessed the corticomedullary strain ratio in renal allografts of 33 patients who underwent renal transplant sonography and biopsy. Based on Banff score criteria for renal cortical fibrosis, 33 allografts were divided into 2 groups: group 1 (n = 19), with mild ( 26%) fibrosis. We used 2-dimensional speckle-tracking software to perform offline analysis of cortical and medullary strain induced by external compression by the ultrasound transducer. We then calculated the corticomedullary strain ratio (cortical normalized strain/medullary normalized strain; normalized strain = developed strain/applied strain [deformation from the abdominal wall to the pelvic muscles]). An unpaired 2-tailed t test was used to determine differences in normalized strain and the strain ratio between the groups. Receiver operating characteristic curve analysis was performed to determine the best strain ratio cutoff value for identifying moderate fibrosis. Results Normalized strain differed between the cortex and medulla (mean ± SD: group 1, 4.58 ± 2.02 versus 2.58 ± 1.38; P = .002; group 2, 1.71 ± 0.42 versus 2.60 ± 0.87; P = .0011). The strain ratio in group 1 was higher than in group 2 (2.06 ± 1.33 versus 0.70 ± 0.20; P = .0007). The area under the receiver operating characteristic curve was 0.964. The sensitivity and specificity of a strain ratio cutoff value of 0.975 for determining moderate fibrosis were 92.9% and 94.7%, respectively. Conclusions Strain values vary in different compartments of the kidney. The corticomedullary strain ratio on ultrasound elasticity imaging decreases with increasing renal cortical fibrosis, which makes it potentially useful as a noninvasive quantitative marker for monitoring the progression of fibrosis in renal transplants.

To determine if acoustic radiation force impulse elastography-derived bowel wall shear wave velocity (SWV) allows distinction of acutely inflamed from fibrotic intestine in a Crohn disease animal model.University Committee on the Use and Care of Animals approval was obtained. An acute inflammation Crohn disease model was produced by treating eight Lewis rats with a single administration of trinitrobenzenesulfonic acid (TNBS) enema, with imaging performed 2 days later in the surviving six rats. Colonic fibrosis in an additional eight Lewis rats was achieved by administering repeated TNBS enemas during 4 weeks, with imaging performed in the surviving seven rats 7 days later to allow acute inflammation resolution. Nine transcutaneous bowel wall SWV measurements were obtained from the colon in all rats without and with applied strain. Mean SWVs without and with applied strain were compared between animal cohorts by using the Student t test, and receiver operating characteristic (ROC) curves were created to assess diagnostic performance.Mean bowel wall SWVs were significantly higher for fibrotic versus acute inflammation cohort of rats at 0% (3.4 ± 1.1 vs 2.3 ± 0.5 m/sec; P = .047) and 30% (6.3 ± 2.2 vs 3.6 ± 0.9 m/sec; P = .02) applied strain. Both acute inflammation and fibrotic cohort of rats demonstrated linear increases in mean SWV with increasing applied strain, with significantly different mean slopes (P = .02) and y-intercepts (P = .02). The area under the ROC curve of the SWV ratio (mean SWV/applied strain) for differentiating histopathologically confirmed fibrotic from inflamed bowel was 0.971.Bowel wall SWV helps distinguish acutely inflamed from fibrotic intestine in a Crohn disease animal model.

This study measures the vascular wall shear rate at the vessel edge using decorrelation based ultrasound speckle tracking. Results for nine healthy and eight renal disease subjects are presented. Additionally, the vascular wall shear rate and circumferential strain during physiologic pressure, pressure equalization and hyperemia are compared for five healthy and three renal disease subjects. The mean and maximum wall shear rates were measured during the cardiac cycle at the top and bottom wall edges. The healthy subjects had significantly higher mean and maximum vascular wall shear rate than the renal disease subjects. The key findings of this research were that the mean vascular wall shear rates and circumferential strain changes between physiologic pressure and hyperemia that was significantly different between healthy and renal disease subjects.

Introduction: Diaphragm excursion measured via ultrasound may be an important imaging outcome measure of respiratory function. We developed a new method for measuring diaphragm movement and compared it to the more traditional M-mode method. Methods: Ultrasound images of the right and left hemidiaphragms were collected to compare speckle tracking and M-mode measurements of diaphragm excursion. Speckle tracking was performed using EchoInsight (Epsilon Imaging, Ann Arbor, Michigan). Results: Six healthy subjects without a history of pulmonary diseases were included in this proof-of-concept study. Speckle tracking of the diaphragm is technically possible. Unlike M-mode, speckle tracking carries the advantage of reliable visualization and measurement of the left hemidiaphragm. Conclusions: Speckle tracking accounted for diaphragm movement simultaneously in the cephalocaudad and mediolateral directions, unlike M-mode, which is 1-dimensional. Diaphragm speckle tracking may represent a novel, more robust method for measuring diaphragm excursion, especially for the left hemidiaphragm. Muscle Nerve, 2016

Local lung function is difficult to evaluate, because most lung function estimates are either global in nature, e.g. pulmonary function tests, or require equipment that cannot be used at a patient’s bedside, such as computed tomography. Yet, local function measurements would be highly desirable for many reasons. In a recent publication (Rubin et al. 2012), we were able to track displacements of the lung surface during breathing. We have now extended these results to measuring lung strains during respiration as a means of assessing local lung ventilation. We studied two human volunteers and 14 mice with either normal lung function or experimentally induced pulmonary fibrosis. The differences in strains between the control, normal mice and those with pulmonary fibrosis were significant (P < 0.0001), while the strains measured in the human volunteers closely matched linear strains predicted from the literature. Ultrasonography may be able to assess local lung ventilation in a clinical setting.

The Society of Radiologists in Ultrasound convened a panel of specialists from radiology, hepatology, pathology, and basic science and physics to arrive at a consensus regarding the use of elastography in the assessment of liver fibrosis in chronic liver disease. The panel met in Denver, Colo, on October 21-22, 2014, and drafted this consensus statement. The recommendations in this statement are based on analysis of current literature and common practice strategies and are thought to represent a reasonable approach to the noninvasive assessment of diffuse liver fibrosis.

Non-alcoholic fatty liver disease (NAFLD) is a common liver disease affecting 30% of the population in the United States. Biopsy is the gold standard for diagnosing NAFLD. Liver histology assesses the amount of fat, and determines type and extent of cell injury, inflammation and fibrosis. However, liver biopsy is invasive and is limited by sampling error. Current radiological diagnostic modalities can evaluate the 'physical' morphology in liver by quantifying the backscattered US signals, but cannot interrogate the 'histochemical' components forming these backscatterers. For example, ultrasound (US) imaging can detect the presence of fat but cannot differentiate steatosis alone from steatohepatitis. Our previous study of photoacoustic physiochemical analysis (PAPCA) has demonstrated that this method can characterize the histological changes in livers during the progression of NAFLD in animal models. In this study, we will further validate PAPCA with human livers. Ex vivo human liver samples with steatosis, fibrosis and cirrhosis will be scanned using optical illumination at wavelengths of 680-1700 nm and compared to histology results. In vivo study on human subjects with confirmed steatosis is planned using our PA-ultrasound (US) parallel imaging system based on Verasonic US imaging flatform with an L7-4 probe. 10 mJ/cm2 per pulse optical energy at 755 nm will be delivered to the skin surface, which is under the safety limit of American National Standard Institute. Preliminary study with ex vivo human tissue has demonstrated the potential of the proposed approach in differentiating human liver conditions.

Objectives Three-dimensional (3D) umbilical cord blood volume flow measurement with the intention of providing a straightforward, consistent, and accurate method that overcomes the limitations associated with traditional pulsed-wave Doppler flow measurement and provides a means by which to recognize and manage at-risk pregnancies.

Objectives The twinkling artifact is an emerging tool for identifying urinary tract calculi. The purpose of this prospective study was to evaluate the diagnostic accuracy of the twinkling artifact compared to unenhanced computed tomography in detecting urolithasis. Methods After Research Ethics Board approval, 51 patients with flank pain from the emergency department were enrolled between November 2009 and September 2010. Patients received an unenhanced computed tomographic scan with 1.25-mm raw data and reformatted 5-mm axial and 2-mm coronal images. Blinded assessment of the urinary tract was performed with gray-scale and color Doppler interrogation. The number of calculi, location, size, kidney distance from the skin, body mass index of the patient, and sonographic image parameters were recorded. Results There were 35 right-sided and 38 left-sided renal calculi, 14 right-sided and 21 left-sided ureteric calculi, and 6 bladder calculi (total, 114 calculi). Thirteen patients had no calculi. The average calculus size was 2.6 mm (range, 1-9 mm). There were 6 false-positive and 22 false-negative instances of twinkling artifacts. On gray-scale evaluation looking for an echogenic focus with shadowing, there were 8 false-positive and 40 false-negative findings. The positive predictive value (PPV) of the twinkling artifact for identifying calculi was 94%, and the sensitivity was 83%. The PPV of gray-scale sonographic shadowing was only 64.9%, and the sensitivity was 80.2%. Conclusions The twinkling artifact has a high PPV for detecting renal and urinary tract calculi. Evaluation for the twinkling artifact is a complementary technique to standard gray-scale shadowing of calculi and improves detection of urolithiasis on sonography.

Strain developed under quasi-static deformation has been mostly used in ultrasound elasticity imaging (UEI) to determine the stiffness change of tissues. However, the strain measure in UEI is often less sensitive to a subtle change of stiffness. This is particularly true for Crohn's disease where we have applied strain imaging to the differentiation of acutely inflamed bowel from chronically fibrotic bowel. In this study, a new nonlinear elastic parameter of the soft tissues is proposed to overcome this limit. The purpose of this study is to evaluate the newly proposed method and demonstrate its feasibility in the UEI. A nonlinear characteristic of soft tissues over a relatively large dynamic range of strain was investigated. A simplified tissue model based on a finite element (FE) analysis was integrated with a laboratory developed ultrasound radio-frequency (RF) signal synthesis program. Two-dimensional speckle tracking was applied to this model to simulate the nonlinear behavior of the straindevelopedinatargetinclusionovertheappliedaveragestraintothesurroundingtissues.Anonlinearempir- ical equation was formulated and optimized to best match the developed strain-to-applied strain relation obtained from the FE simulation. The proposed nonlinear equation was applied to in vivo measurements and nonlinear parameters were further empirically optimized. For an animal model, acute and chronic inflammatory bowel disease was induced in Lewis rats with trinitrobenzene sulfonic acid (TNBS)-ethanol treatments. After UEI, histo- pathology and direct mechanical measurements were performed on the excised tissues. The extracted nonlinear parameter from the developed strain-to-applied strain relation differentiated the three different tissue types with 1.96 ± 0.12 for normal, 1.50 ± 0.09 for the acutely inflamed and 1.03 ± 0.08 for the chronically fibrotic tissue. T-tests determined that the nonlinear parameters between normal, acutely inflamed and fibrotic tissue types were statistically significantly different (normal/ fibrotic (p 5 0.0000185), normal/acutely inflamed (p 5 0.0013) and fibrotic/acutely inflamed (p 50.0029)). This technique may provide a sensitive and robust tool to assess subtle stiff- ness changes in tissues such as in acutely inflamed bowel wall. We prospectively evaluated real-time ultrasound speckle tracking for monitoring soft tissue motion for image-guided radiotherapy. Two human volunteers and 1 patient with a proven hepatocellular carcinoma, who was being prepared for radiation therapy treatment, were scanned using a clinical ultrasound scanner modified to acquire and store radiofrequency signals. Scans were performed of the liver in the volunteers and the patient. In the patient, the speckle-tracking results were compared to those measured on a treatment-planning 4-dimensional computed tomogram with tumors contoured manually in each phase and with estimates made by hand on gray scale ultrasound images. The surface of the right lung and the prostate were scanned in a volunteer. The liver and lung surface were scanned during respiration. To simulate prostate motion, the ultrasound probe was rocked in an anterior-posterior direction. The correlation coefficients of all motion measurements were significantly correlated at all sites (P < .00001 for all sites) with 0 time delays. Ultrasound speckle-tracking motion estimates of tumor motion were within 2 mm of estimates made by hand tracking on gray scale ultrasound images and the 4-dimensional computed tomogram. The total tumor motion was greater than 20 mm. The angular displacement of the prostate was within 0.02 radians (1.1°) with displacements measured by hand. Speckle tracking could be used to monitor organ motion during radiotherapy.

Objectives Implanted mesh for inguinal hernia repair is often difficult to visualize with gray scale sonography and may present without the knowledge of the sonographer. We sought to evaluate the utility of the twinkling artifact produced by inguinal mesh to assist in mesh identification. Methods Two reviewers evaluated focused sonographic examinations of 44 inguinal regions, 24 of which had implanted inguinal mesh. The sonographic examinations consisted of static gray scale and color Doppler images with both linear and curvilinear array transducers. The presence of the twinkling artifact and visibility of the mesh were graded on a 4-point visibility scale. Results Inguinal mesh was not easily identified on gray scale imaging using either the curvilinear array (P = .5) or linear array (P = .5) transducer. The mesh was definitely seen in 3 of 24 inguinal regions using the linear array transducer and 2 of 24 inguinal regions using the curvilinear array transducer. In 79% of inguinal regions with mesh, the twinkling artifact was produced with the curvilinear array transducer only. The artifact was not elicited when using the linear array transducer. With the use of the curvilinear array transducer and the presence of the twinkling artifact, there was a significant chance of correctly identifying the presence of mesh (P Conclusions Standard gray scale imaging alone is not reliable when identifying inguinal mesh. The twinkling artifact was present in 79% of inguinal regions with mesh when evaluated with a low-frequency curvilinear array transducer.

To retrospectively correlate sonographic color Doppler twinkling artifact within the kidneys with unenhanced computed tomography (CT) in the detection of nephrolithiasis.Institutional review board approval was obtained for this retrospective HIPAA-complaint investigation, and the informed consent requirement was waived. Sonographic imaging reports describing the presence of renal twinkling artifact between January 2008 and September 2009 were identified. Subjects who did not undergo unenhanced abdominal CT within 2 weeks after sonography were excluded. Ultrasound examinations were reviewed by three radiologists working together, and presence, number, location, and size of renal twinkling artifacts were documented by consensus opinion. Sonographic findings were correlated with unenhanced CT (5-mm section width, no overlap) for nephrolithiasis and other causes of twinkling artifact. The number, location, and size of renal calculi at CT were documented.The presence of sonographic renal twinkling artifact, in general, had a 78% (95% confidence interval: 0.66, 0.90) positive predictive value for nephrolithiasis anywhere in the kidneys at CT. The true-positive rate of twinkling artifact for confirmed calculi at CT was 49% (73 of 148 twinkling foci), while the false-positive rate was 51% (75 of 148 twinkling foci). The overall sensitivity of twinkling artifact for the detection of specific individual renal calculi observed at CT was 55% (95% confidence interval: 0.47, 0.64).While renal twinkling artifact is commonly associated with nephrolithiasis, this finding is relatively insensitive in routine clinical practice and has a high false-positive rate when 5-mm unenhanced CT images are used as the reference standard.http://radiology.rsna.org/lookup/suppl/doi:10.1148/radiol.11102128/-/DC1.
