The estimation of joint contact forces in musculoskeletal multibody dynamics models typically requires the use of expensive and time-consuming technologies, such as reflective marker-based motion capture (Mocap) system. In this study, we aim to propose a more accessible and cost-effective solution that utilizes the dual smartphone videos (SPV)-driven musculoskeletal multibody dynamics modeling workflow to estimate the lower limb mechanics. Twelve participants were recruited to collect marker trajectory data, force plate data, and motion videos during walking and running. The smartphone videos were initially analyzed using the OpenCap platform to identify key joint points and anatomical markers. The markers were used as inputs for the musculoskeletal multibody dynamics model to calculate the lower limb joint kinematics, joint contact forces, and ground reaction forces, which were then evaluated by the Mocap-based workflow. The root mean square error (RMSE), mean absolute deviation (MAD), and Pearson correlation coefficient (ρ) were adopted to evaluate the results. Excellent or strong Pearson correlations were observed in most lower limb joint angles (ρ = 0.74 ~ 0.94). The averaged MADs and RMSEs for the joint angles were 1.93 ~ 6.56° and 2.14 ~ 7.08°, respectively. Excellent or strong Pearson correlations were observed in most lower limb joint contact forces and ground reaction forces (ρ = 0.78 ~ 0.92). The averaged MADs and RMSEs for the joint lower limb joint contact forces were 0.18 ~ 1.07 bodyweight (BW) and 0.28 ~ 1.32 BW, respectively. Overall, the proposed smartphone video-driven musculoskeletal multibody dynamics simulation workflow demonstrated reliable accuracy in predicting lower limb mechanics and ground reaction forces, which has the potential to expedite gait dynamics analysis in a clinical setting. Osteoporosis results in low-trauma fractures affecting millions globally, in particular elderly populations. Despite the inclusion of physical activity in fracture prevention strategies, the optimal bone-strengthening exercises remain uncertain, highlighting the need for a deeper understanding of lower limb joint loading dynamics across various exercise types and levels. This study examines lower limb joint loading during high-impact exercises across different intensities. A total of 40 healthy, active participants were recruited (mean ± SD: age of 40.3 ± 13.1 yr; height 1.71 ± 0.08 m; and mass 68.44 ± 11.67 kg). Motion capture data and ground reaction forces of 6 different exercises: a self-selected level of walking, running, countermovement jump, squat jump, unilateral hopping, and bilateral hopping were collected for each participant. Joint reaction forces were estimated using lower body musculoskeletal models developed in OpenSim. Running and hopping increased joint forces compared to walking, notably at the hip (83% and 21%), knee (134% and 94%), and ankle (94% and 77%), while jump exercises reduced hip and ankle loading compared to walking (36% and 19%). Joint loading varied with exercise type and intensity, with running faster increasing forces on all joints, particularly at the hip. Sprinting increased forces at the hip but lowered knee and ankle forces. Higher jumps intensified forces on all joints, while faster hopping reduced forces. The wide variation of lower limb joint loading observed across the exercises tested in this study underscores the importance of implementing diverse exercise routines to optimize overall bone health and strengthen the musculoskeletal structure. Practitioners must therefore ensure that exercise programs include movements that are specifically suitable for their intended purpose. This study aimed to quantify how different high-impact exercises affect lower limb joint loading dynamics. Forty healthy, active participants with an average age of 40 yr performed 6 exercises: walking, running, jumping, and hopping, at various intensities. Results showed that running and hopping increased joint forces compared to walking, particularly at the hip and knee. Conversely, jumping exercises reduced hip and ankle loading. Toe-in gait is a pathology in which the child walks and turns the foot inward instead of pointing straight ahead. The alignment of the lower limb structure changes in this disease, increasing the incidence of knee and hip osteoarthritis. This study aimed to determine the kinematic and joint loading in subjects walking with a toe-in gait pattern. This study selected two groups of subjects: normal subjects and those with toe-in gait due to an increased femoral head anteversion angle (each group consisted of 15 subjects). A Qualisys motion analysis system and a Kistler force plate were used to record the motions and forces applied to the leg while walking. OpenSim software (version 3.3) was used to analyze the range of motion, moments, muscle forces, and joint contact forces in both groups of subjects. The mean values of stride length for normal subjects (1.1 ± 0.141 m) and those with toe-in gait (0.94 ± 0.183 m) differed significantly. The mediolateral component of the ground reaction force decreased significantly in the toe-in gait group compared to normal subjects (p -value = 0.05). The peak force of most of the hip joint muscles increased significantly in those with toe-in gait compared to normal subjects (p -value < 0.05). The results of this study showed that those with toe-in gait, due to an increase in femoral head anteversion angle, only had a change in rotation of the pelvic and hip joint. There was no significant difference between walking speed and most ground reaction force components between normal subjects and those with toe-in gait. As the peaks of most of the hip joint muscles increased significantly in the toe-in gait group, this increased joint contact forces (especially the anteroposterior component of the hip joint and the mediolateral component of the knee joint), which may ultimately increase the incidence of hip and knee joint osteoarthritis. Cerebral palsy (CP) includes a group of neurological conditions caused by damage to the developing brain, resulting in maladaptive alterations of muscle coordination and movement. Estimates of joint moments and contact forces during locomotion are important to establish the trajectory of disease progression and plan appropriate surgical interventions in children with CP. Joint moments and contact forces can be estimated using electromyogram (EMG)-informed neuromusculoskeletal models, but a reduced number of EMG sensors would facilitate translation of these computational methods to clinics. This study developed and evaluated a muscle synergy-informed neuromusculoskeletal modelling approach using EMG recordings from three to four muscles to estimate joint moments and knee contact forces of children with CP and typically developing (TD) children during walking. Using only three to four experimental EMG sensors attached to a single leg and leveraging an EMG database of walking data of TD children, the synergy-informed approach estimated total knee contact forces comparable to those estimated by EMG-assisted approaches that used 13 EMG sensors (children with CP, n = 3, R2 = 0.95 ± 0.01, RMSE = 0.40 ± 0.14 BW; TD controls, n = 3, R2 = 0.93 ± 0.07, RMSE = 0.19 ± 0.05 BW). The proposed synergy-informed neuromusculoskeletal modelling approach could enable rapid evaluation of joint biomechanics in children with unimpaired and impaired motor control within a clinical environment. The ankle joint is one of the most involved joints in uphill walking. Furthermore, it is well known that toe walking increases the external dorsiflexion moment in the first half of stance during level walking. However, the effects of different foot-strike patterns on plantar flexor muscle forces, ankle joint forces, and other lower limb joint and muscle forces are unknown. Do foot-strike patterns during 18° uphill walking affect lower limb sagittal joint angles and moments, as well as joint contact and muscle forces? This study was based on a data subset from previous publications, analysing uphill walking on an 18° ramp at a preset speed of 1.1 m/s in 18 male participants (34 limbs analyzed, 27 ± 5 years). Participants were divided into two groups based on their foot-strike pattern at initial contact: heel (HC) and forefoot (FC). Lower limb sagittal joint angles and moments as well as joint contact and muscle forces were assessed. Differences between the groups were assessed using two-sample t-tests. FC showed increased soleus and gastrocnemius muscle forces as well as ankle joint forces during loading response and mid stance compared to HC. The soleus muscle force impulse was 51.1% higher in the FC group than in the HC group (p < 0.001). On the other hand, FC had a lower absolute centre of mass vertical displacement and reduced knee and hip joint, as well as iliopsoas and hamstring muscle force impulses. In terms of plantar flexor and ankle joint loading, it is advantageous to exhibit a heel strike pattern. Purpose: Toe-in gait is defined as a style of walking in which the foot turns inward. It may be caused by an increase in femoral bone anteversion, tibia torsion, and metatarsus adductus. There are some conservative treatment approaches used to correct this condition. This review aimed to determine the effects of the toe-in gait on joint loading, kinematics, and kinetic parameters while walking. Moreover, it sought to determine the efficiency of various conservative treatments used to correct the condition. Materials and Methods: A literature search was conducted in the following databases: PubMed, Institute for Scientific Information (ISI), Web of Science database, EBSCO, and Embase, using the following keywords in toe, toe-in, toeing, in-toe, pigeon toe, and conservative treatment published between 1950 and 2021. The quality of the studies was evaluated using the Down and Black tool. Results: A total of 13 papers on the impact of toe-in gait on joint contact force, kinematics, kinetic parameters, and conservative approaches to management were found. The quality of the studies varied between a score of 11 and 22. The toe-in gait influences the joint contact forces and kinematics of the joints, especially the hip and pelvis. The effects of conservative treatment on the toe-in gait appear to be controversial. Conclusion: As the toe-in gait influences the joint contact force, it may increase the incidence of degenerative joint diseases. Therefore, treatment is recommended. However, there is no strong evidence on the efficacy of conservative treatments, and there are no recommendations for the use of these treatments in subjects with toe-in gait. [ABSTRACT FROM AUTHOR]

Optimal implant placement in reverse total shoulder arthroplasty (rTSA) remains controversial. Specifically, the optimal glenoid inclination is unknown. Therefore, a cadaveric shoulder simulator with 3-dimentional human motion specific to rTSA was used to study joint contact and muscle forces as a function of glenoid component inclination. Eight human cadaver shoulders were tested before and after rTSA implantation. Scapular plane abduction kinematics from control subjects and those with rTSA drove a cadaveric shoulder simulator with 3-dimentional scapulothoracic and glenohumeral motion. Glenoid inclination varied from −20° to +20°. Outputs included compression, superior-inferior (S/I) shear, and anterior-posterior shear forces from a 6° of freedom load cell in the joint, and deltoid and rotator cuff muscle forces. Data were evaluated with statistical parametric mapping and t -tests. Inferior glenoid inclination (−) reduced S/I shear by up to 125% relative to superior inclination, with similar compression to the neutral condition (0°). Superior inclinations (+) increased the S/I shear force by approximately the same magnitude, yet decreased compression by 25% in the most superior inclination (+20°). There were few differences in deltoid or rotator cuff forces due to inclination. Only the middle deltoid decreased by approximately 7% for the most inferior inclination (−20°). Compared with native shoulders, the neutral (0°) rTSA inclination showed reduced forces of 30%-75% in the anterior deltoid and a trend toward decreased forces in the middle deltoid. Force demands on the rotator cuff varied as a function of elevation, with a trend toward increased forces in rTSA at peak glenohumeral elevation. Inferior inclination reduces superior shear forces, without influencing compression. Superior inclination increased S/I shear, while decreasing compression, which may be a source of component loosening and joint instability after rTSA. Inferior inclination of the rTSA glenoid may reduce the likelihood of glenoid loosening by reducing the magnitude of cyclic shear and compressive loading during arm elevation activities, although this may be altered by specific-subject body habitus and motion. Joint contact force is the actual force applied on the articular surface that could predict performance and injuries, but rarely reported for badminton sport. The study sought to calculate lower limb joint contact force and decelerative kinematics for badminton forward lunges. Fifteen badminton players performed backhand and forehand forward lunges in random order. The kinematic and kinetic data were input to scale a musculoskeletal model and solve inverse dynamics in the simulations. Outcome variables were compared between lunge conditions using repeated measures MANOVA. Forehand lunge produced higher compressional ankle contact force (p = 0.040, partial η2 = 0.14), faster touchdown hip abduction (p = 0.031, partial η2 = 0.16), and larger horizontal deceleration of the mass centre (p = 0.016, partial η2 = 0.19) and torso (p = 0.031, partial η2 = 0.16) compared to backhand lunge. Despite the statistical significance, we found that the increments of joint loading in forehand lunge were small (<5%) with limited effect size and could be attributed to the larger movement deceleration during braking. Although typically favorable in outcome, anatomic total shoulder arthroplasty (aTSA) can require long-term revision. The most common cause for revision is glenoid loosening, which may result from eccentric cyclic forces and joint translations. "Rocking" of the glenoid component may be exacerbated by the joint geometry, such as glenoid inclination and version. Restoration of premorbid glenoid inclination may be preferable, although laboratory and computational models indicate that both superior inclination and inferior inclination have benefits. This discrepancy may arise because previous studies were limited by a lack of physiological conditions to test inclination. Therefore, a cadaveric shoulder simulator with 3-dimensional human motion was used to study joint contact and muscle forces with isolated changes in glenoid inclination. Eight human cadaveric shoulders were tested before and after aTSA. Scapular-plane abduction kinematics from human subjects were used to drive a cadaveric shoulder simulator with 3-dimensional scapulothoracic and glenohumeral motion. Glenoid inclination was varied from −10° to +20°, whereas compressive, superior-inferior shear, and anterior-posterior shear forces were collected with a 6- df load cell during motion. Outputs also included muscle forces of the deltoid and rotator cuff. Data were evaluated with statistical parametric mapping repeated-measures analysis of variance and t tests. Inferior glenoid inclination (−10°) reduced both compressive and superior-inferior shear forces vs. neutral 0° inclination by up to 40%, and even more when compared with superior inclination (P <.001). Superior inclinations (+10° and +20°) tended to increase deltoid and rotator cuff forces vs. neutral 0° inclination or inferior inclination, on the order of 20%-40% (P ≤.045). All force metrics except anterior-posterior shear were lowest for inferior inclination. Most aTSA muscle forces for neutral 0° inclination were not significantly different from native shoulders and decreased 45% and 15% in the posterior deltoid and supraspinatus, respectively (P ≤.003). Joint translations were similar to prior reports in aTSA patients and did not differ between any inclinations or compared with native shoulders. Joint reaction forces were similar to those observed in human subjects with instrumented aTSA implants, providing confidence in the relative magnitude of our results. Inferior inclination reduces overall forces in the shoulder. Superior inclinations increase the muscle effort required for the shoulder to achieve similar motion, thus increasing the forces exerted on the glenoid component. These results suggest that a preference toward aTSA glenoid components in inferior inclination may reduce the likelihood of glenoid loosening by reducing excessive muscle and joint contact forces. Background. Abnormal joint loading is a contributor factor to develop and progress of osteoarthritic knee joint degeneration. Various approaches have been used to measure joint contact forces in the patients with knee osteoarthritis (OA). Evaluation of knee adduction moment based on inverse dynamic (ID) approach is a surrogate marker for medical compartment loading in people with knee OA. It is controversial whether joint loading increases in this group of subjects or not. Therefore, the aim of this study was to evaluate knee joint contact force in the subjects with OA based on OpenSIM modelling approach. Methods. 10 knee osteoarthritic patients and 10 normal subjects with comparable age, height an weight were recruited in this study. A motion analysis system with 7 highspeed camera and a Kistler force plate was used to record the motions of the subjects while walking and the force applied on the legs, respectively. OpenSim software was used to evaluate joint contact forces. Results. Although the mean value of the second peak of GRF in OA group decreased significantly compared to normal subjects (P-value = 0.0005), the mean value of the force applied on the leg in midstance phase increased significantly compared to normal subjects (P-value = 0.0005). The range of motion of ankle, hip and knee joints decreased in OA group compared to normal subjects (P-value < 0.05). The peaks of knee joint contact forces decreased significantly in OA subject (P-value > 0.001). Moreover, the peak of joint contact force of ankle in OA group was less than that of normal subjects. Conclusions. The results of this study showed that the loads on knee joint (both based on moments and knee joints contact force omponents) of the subjects participated in this study was significantly less than that of normal subjects. This may be due to use of compensatory mechanisms selected by the subjects. It is highly recommended to repeat the research on more number of the subjects with various severities of knee joint OA. Background: Individuals with chronic ankle instability (CAI) exhibit altered vertical ground reaction forces (vGRF), a laterally shifted center of pressure, and an inverted foot position during walking. These neuromechanical alterations are linked with altered ankle joint loading in this population. Vibration-based gait retraining improves center of pressure positioning but effects on neuromechanical variables influencing joint loading remains unknown.Research Question: Do patients with CAI exhibit altered vGRF and ankle joint contact forces (JCF) after receiving a single session of vibration-based gait retraining?Methods: Ten individuals with CAI underwent a single session of vibration-based gait retraining. Kinematic and kinetic data were collected during walking on an instrumental treadmill with force plates embedded in it. Following a baseline gait assessment without feedback, participants walked at a self-selected speed for 10 minutes while receiving feedback. Data was collected during an early (1 st and 2 nd minute) and late adaptation phase (9 th and 10 th minute) and, compared to baseline values. Impact and propulsive vGRF variables (i.e. peak, time to peak, and loading rate) were obtained. Musculoskeletal modeling was used to calculate ankle JCF variables (peak, impulse, and loading rate) during stance phase.Results: Propulsive vGRF and ankle JCF outcomes were significantly reduced during the early and late adaptation phases (p ≤ 0.039).Significance: These results indicate that vibration-based gait retraining can immediately reduce propulsive vGRF and ankle JCF and may represent a modality that could help restore appropriate ankle joint loading patterns in those with CAI. Background: Previous studies on different periacetabular osteotomy approaches for correction of hip dysplasia disagree on the time course of normalisation of muscle function postoperatively, some stating that especially hip flexor function is not fully normalised after 12 months. Purpose: The purpose of this study was to evaluate hip function during walking before, and 6 and 12 months after minimally invasive periacetabular osteotomy. Methods: Using conventional 3D inverse dynamics followed by static optimisation, we calculated hip net joint moment and angular impulse, as well as individual muscle forces and hip joint contact force, during walking for 32 patients with hip dysplasia and 32 matched controls. Results: None of the extensor and abductor measures were significantly different between controls and patients tested preoperatively, nor between any of the 3 time points patients were tested. For all of the flexor measures, patients' preoperative values were lower than controls', but had increased to values above the controls 6 months postoperatively. Conclusion: Hip muscle function during walking seemed normalised after 6 months after minimally invasive periacetabular osteotomy, while joint contact force did not fully normalise until 12 months postoperatively, perhaps because the hip joint structures need a longer time to heal than the muscles and a potential pain alleviating strategy was still in effect. Introduction: High heel shoes affect the knee joint and can cause arthritis in the tibiofemoral (TFJ) and patellofemoral joints (PFJ). There is a dearth of research investigating the contact forces of TFJ and PFJ. Therefore, the purpose of this study was to assess the effect of high heel shoes on muscle forces as well as TFJ, and PFJ contact forces during walking. Materials & Methods: A total of nine young females with mean age, height, and body mass of 21.34±3.83 years, 1.49±0.14 m, and 60.54±1.76 kg, respectively, were randomly included in this study. The walking variables in each case were measured while walking with 6-cm-heels, 3-cm-heels, and without shoes. All measurements were conducted on the right legs. The Qualisys motion analyzer and Kistler Force Plate were utilized to record data. The data were collected using OpenSim software to obtain muscular and joint contact forces. In addition, the data were analyzed in SPSS software. P-value less than 0.05 was considered statistically significant. Findings: The internal and external TFJ contact force during walking with 6-cm-heels was 3.66±4.07 N/BW, which was significantly higher than that during walking without shoes (P=0.05). The anterior-posterior PFJ contact force was obtained at 1.71±0.27N/BW, which was significantly greater than that during walking without shoes by 0.61 N/BW (P=0.001). Moreover, the vertical force of PFJ contact forces during walking with 6-cm-heels (6 cm) was significantly greater than that during walking without shoes (P=0.025). However, there were no significant differences between walking with 6- and 3-cm-heels regarding anterior-posterior, internal-external, and vertical TFJ and PFJ forces (P>0.05). Discussion & Conclusions: Walking with high heel shoes increased joint contact force in the medial compartment of TFJ and PFJ during the stance phase of the gait cycle. This increase in forces as cumulative forces can put the joint at the high risk of degeneration and arthritis. The TFJ and PFJ pain are warning signs of walking with high heel shoes. Given the harmful effect of high heels on TFJ and PFJ, it is recommended to use low heel shoes or flat ones.

Background: The posterior tibialis tendon dysfunction (PTTD) is typically associated with progressive flatfoot deformity, which could be alleviated with foot orthosis. However, the evaluation of tibialis posterior (TP) weakness on lower limb mechanics of flatfoot adults with foot orthoses is scarce and requires further investigation.Research Question: This study aimed to examine the effects of TP weakness on lower limb mechanics in flatfoot adults with foot orthosis through gait analysis and musculoskeletal modelling.Methods: Fifteen young adults with flatfoot were recruited from University to perform a gait experiment with and without foot orthoses. Data collected from the motion capture system were used to drive the musculoskeletal modelling for the estimation of the joint force and extrinsic muscle forces of the lower limb. A parametric analysis was conducted by adjusting the TP muscle strength from 40 % to 100 %. Two-way repeated measures ANOVA was used to compare the peak extrinsic foot muscle forces and joint forces among different levels of TP weakness and insole conditions.Results: TP weakness significantly increased ankle joint force superoinferiorly (F = 125.9, p < 0.001) and decreased anteroposteriorly (F = 125.9, p < 0.001), in addition to a significant increase in the muscle forces of flexor hallucis longus (p < 0.001) and flexor digitorum longus (p < 0.001). Besides, the foot orthosis significantly reduced most peak muscle forces whilst significantly reduced the second peak knee force and peak ankle force compared to the control condition (F = 8.79-30.9, p < 0.05).Significance: The increased extrinsic foot muscle forces (flexor hallucis longus and flexor digitorum longus) and ankle joint forces in the TP weakness condition indicated that TP weakness may induce compensatory muscle activation and attenuated joint load. Back squats are a common strengthening exercise for knee and hip musculature. However, repetitive loaded movements like backs squats result in high patellofemoral joint loading and therefore may contribute to the development of common overuse injuries. Thus, it is important to understand how changing parameters such as squat depth or load influences patellofemoral loading. This study investigated differences in patellofemoral loading when experienced female lifters squatted to three depths (above parallel, parallel, and below parallel) and with three loads (unloaded, 50%, and 85% of depth-specific one repetition maximums). Patellofemoral joint reaction forces (pfJRF) and stresses (pfJS) were calculated from biomechanical models incorporating knee extensor moments (KEM) and joint angles. Peak KEMs displayed a depth-by-load interaction such that within each depth, as load increased so did peak KEM. However, within each load, the effects of depth were different. Peak pfJRF also increased with load and was higher at below parallel than above or parallel depths. Peak pfJS also displayed a depth-by-load interaction, increasing with load within a given depth, and being greatest at the below parallel depths within a given load. Longitudinal bone growth is regulated by mechanical forces arising from physical activity, whose directions and magnitudes depend on activity kinematics and intensity. This study aims to investigate the influence of common physical activities on proximal femoral morphological tendency due to growth at the femoral head growth plate. A subject-specific femur model based on magnetic resonance images of one able-bodied 6-year old child was developed, and the directions of hip contact force were described as load samples at a constant magnitude. Finite element analysis was performed to predict growth rate and growth direction, and expected changes in neck-shaft angle and femoral anteversion were computed corresponding to circa 4 months of growth. For most loading conditions, neck-shaft angle and femoral anteversion decreased during growth, corresponding to the femur's natural course during normal growth. The largest reduction in neck-shaft angle and femoral anteversion was approximately 0.25° and 0.15°. Our results suggest that most common physical activities induce the expected morphological changes in normal growth in able-bodied children. Understanding the influence of contact forces during less common activities on proximal femoral development might provide improved guidelines and treatment planning for children who have or are at risk of developing a femoral deformity. Gait biomechanics after anterior cruciate ligament (ACL) injury are associated with functional outcomes and the development of posttraumatic knee osteoarthritis. However, biomechanical outcomes between patients treated nonoperatively compared with operatively are not well understood. The primary purpose of this study was to compare knee joint contact forces, angles, and moments during loading response of gait between individuals treated with operative compared with nonoperative management at 5 years after ACL injury. Forty athletes treated operatively and 17 athletes treated nonoperatively completed gait analysis at 5 years after ACL reconstruction or completion of nonoperative rehabilitation. Medial compartment joint contact forces were estimated using a previously validated, patient‐specific electromyography‐driven musculoskeletal model. Knee joint contact forces, angles, and moments were compared between the operative and nonoperative group using mixed model 2 × 2 analyses of variance. Peak medial compartment contact forces were larger in the involved limb of the nonoperative group (Op: 2.37 ± 0.47 BW, Non‐Op: 3.03 ± 0.53 BW; effect size: 1.36). Peak external knee adduction moment was also larger in the involved limb of the nonoperative group (Op: 0.25 ± 0.08 Nm/kg·m, Non‐Op: 0.32 ± 0.09 Nm/kg·m; effect size: 0.89). No differences in radiographic tibiofemoral osteoarthritis were present between the operative and nonoperative groups. Overall, participants treated nonoperatively walked with greater measures of medial compartment joint loading than those treated operatively, while sagittal plane group differences were not present. Statement of clinical relevance: The differences in medial knee joint loading at 5 years after operative and nonoperative management of ACL injury may have implications on the development of posttraumatic knee osteoarthritis. Background: There is little knowledge that explains how forces are applied on knee, hip and L5-S1 joint between the sound and prosthetic leg in unilateral Below-Knee Amputation (BKA), therefore the aim of this study was to assess the interaction of knee, hip and L5-S1 joint contact forces between sound and prosthetic leg in patients with unilateral BKA during walking. Methods: Five patients with BKA were recruited in this study. A Qualisys motion system captured with seven cameras and a Kistler force plate were used to record kinematics and kinetics variables of walking. The forces applied on knee, hip and L5-S1 joint contact forces (JCF) were calculated by using OpenSIM software. SPSS software was used to analyze data at an alpha set point of 0.05. Results: The propulsive and second peaks of ground reaction forces applied on sound leg were significantly higher than on prosthetic leg (P0.05). Conclusion: The results of this study showed that the meaningful JCF applied on the sound leg was more than that of prosthetic leg. Insignificant increases in JCF on the sound leg during life can create cumulative forces on the knee and low-back and endanger these joints of the risk of knee OA and chronic low-back pain. Balanced forces applied on sound and prosthetic leg is important, if this is the case, so indicating using a proper application of socket pin and prosthetic feet may have beneficial impact on sound side loads.

Background: Subtalar fusion usually leads to a satisfactory clinical outcome by eliminating the motion of the subtalar joint but can cause an aggravation of osteoarthritis after the subtalar fusion. Previous studies have investigated the effect of subtalar fusion in static testing using cadaver limbs, but there was no evidence of an aggravation of osteoarthritis.Research Question: The objective was to investigate the differences in foot joint kinematics and kinetics during a standing pose and walking with and without subtalar fusion, using a musculoskeletal simulation.Methods: Full-body joint kinematics, ground reaction force, and foot pressure of the healthy subjects were recorded during walking using an optical motion capture system. The models with and without subtalar fusion were constructed using the AnyBody Modeling System (AnyBody Technology, Aalborg, Denmark). The range of motion and contact forces in the individual foot joints with and without simulated subtalar fusion in healthy subjects were estimated using computational simulation and compared using the Wilcoxon signed-rank test. The change of motion in the Chopart's joint was observed.Results: Normalized to the subject's body weight (BW), the average maximum contact forces in the tibiotalar and Lisfranc joints during walking were significantly increased by 2.6 and 0.9 BW with the simulated subtalar joint fusion, respectively. The simulated subtalar joint fusion increased joint contact forces significantly during walking, which can increase the risks of secondary arthritis in the adjacent joints.Significance: The subtalar joint fusion increased the joint contact forces in adjacent joints during walking implying that the fusion can increase the risk of secondary injuries in adjacent joints in the foot. Introduction: Longer steps with load carriage is common in shorter Soldiers when matching pace with taller Soldiers whereas shorter steps are hypothesized to reduce risk of injury with load carriage. The effects of load carriage with and without step length manipulation on loading patterns of three commonly injured structures were determined: Achilles tendon, patellofemoral joint (PFJ) and medial tibiofemoral joint (mTFJ).Materials and Methods: ROTC Cadets (n = 16; 20.1 years ± 2.5) walked with and without load carriage (20-kg). Cadets then altered preferred step lengths ±7.5% with load carriage. Achilles tendon, PFJ and mTFJ loads were estimated via musculoskeletal modeling.Results: Large increases in peak Achilles tendon load (p < 0.001, d = 1.93), Achilles tendon impulse per 1-km (p < 0.001, d = 0.91), peak mTFJ load (p < 0.001, d = 1.33), and mTFJ impulse per 1-km (p < 0.001, d = 1.49) were noted with load carriage while moderate increases were observed for the PFJ (peak: p < 0.001, d = 0.69; impulse per 1-km: p < 0.001, d = 0.69). Shortened steps with load carriage only reduced peak Achilles tendon load (p < 0.001, d = -0.44) but did not reduce Achilles impulse per km due to the resulting extra steps and also did not reduce peak or cumulative PFJ and mTFJ loads (p > 0.05). Longer steps with load carriage increased PFJ loads the most (p < 0.001, d = 0.68-0.75) with moderate increases in mTFJ forces (p < 0.001, d = 0.48-0.63) with no changes in Achilles tendon loads (p = 0.11-0.20).Conclusion: A preferred step length is the safest strategy when walking with load carriage. Purpose: The negative heel rocker shoes help reduce the plantar pressure in patients with diabetes, but their effects on the other lower limb joints are unknown. Accordingly, the current study aimed at evaluating the effect of negative heel rocker shoes on the moment and the contact forces applied on lower limb joints of diabetic patients while walking. Methods: A total of 10 patients with diabetes mellitus and 10 healthy individuals were enrolled to this quasi-experimental study. Musculoskeletal system modeling was initiated using OpenSIM and Visual 3D. After noise elimination and data processing, the independent t test was done. The statistical analyses were done by SPSS 19 and significance level was considered at less than 0.05. Results: Vertical contact force of hip joint in the experimental group (1.17 Newton/body weight) was significantly higher than that of the control group (P=0.04). In the experimental group, the anterior-posterior contact forces of hip joint (P=0.01), knee joint (P=0.01), and ankle joint (P=0.04) were 1.3, 1.5, and 0.47 N/BW, respectively, which were significantly higher than those of the control group. In the experimental group, the internal-external contact forces of hip joint (P=0.01) and knee joint (P=0.01) were 1.31 and 0.15 N/BW, respectively; which were significantly lower than those of the control group. Conclusion: Using negative heel rocker shoes is associated with decrease in the vertical contact force of ankle joint. Heel rocker shoes increase vertical and anterior-posterior forces of proximal lower limb joints (hip and knee). The increased contact force in proximal lower limb joints, especially hip joint, may increase the risk of joint injury and disruption. Thus, more attention should be paid to prescribe negative heel rocker shoes for the patients with diabetes.

Background: Long duration walking, a commonly recommended treatment option for knee osteoarthritis (OA), may lead to increased knee joint loading.Research Question: To evaluate the effects of prolonged walking on dynamic knee joint stiffness and contralateral knee joint contact forces (KCFs) in individuals with unilateral symptomatic knee OA.Methods: Twenty-six older adults with knee OA completed a 45-minute bout of walking on a treadmill. Dynamic knee joint stiffness, estimated KCFs, measured ground reaction forces (GRFs), and simulated muscle forces were evaluated for both the symptomatic and asymptomatic limbs at 15-minute intervals using repeated measures, analysis of variance (ANOVA).Results: Dynamic knee joint stiffness during the early weight-acceptance phase of gait was significantly higher for the symptomatic limb throughout the 45-minute bout of walking. A significant increase in peak KCFs and simulated muscle forces were also observed during the weight-acceptance phase of gait for both limbs after 30 and 45 min of walking. Additionally, significantly elevated peak KCFs and muscle forces were observed during the late-stance phase of gait for the contralateral asymptomatic limb throughout the 45-minute bout of walking.Significance: Walking durations of 30 min or greater lead to increased knee joint loading. Additionally, the elevated dynamic knee joint stiffness observed for the symptomatic knee during the weight acceptance phase of gait appears to be unrelated to the knee joint loading profile. Finally, the greater KCFs during the late-stance phase of gait observed for the asymptomatic limb are consistent with previously demonstrated risk factors for OA development and progression. Purpose: The aim of this study was to examine the joint contact forces (JCF) between each limb as the LLD magnitude increases during walking activity. Methods: Eighteen male healthy subjects volunteered to participate in the experiment. Walking gait analysis was conducted with eight different levels of insole to simulate the LLD, starting from 0 cm until 4.0 cm with 0.5 cm increment. Qualisys Track Manager System and C-motion Visual 3D biomechanical tools were used to analyse the results. Four joints (ankle, knee, hip, and pelvis) of lower limb of two legs were investigated. The increment of insoles was placed on the right leg to represent the long leg. Results: The results suggest that the mean contact forces for all joints in the short leg were increased as the increment level increased. On the contrary, the mean contact forces in the long leg decreased when the LLD level increased. Among these four joints, JCF in hip shows a positive increment based on the ASI value. Means that hip shows the most affected joint as the LLD level increase. Abstract Standard musculoskeletal simulation tools now offer widespread access to internal loading conditions for use in improving rehabilitation concepts or training programmes. However, despite broad reliance on their outcome, the accuracy of such loading estimations, specifically in deep knee flexion, remains generally unknown. The aim of this study was to evaluate the error of tibio-femoral joint contact force (JCF) calculations using musculoskeletal simulation compared to in vivo measured JCFs in subjects with instrumented total knee endoprostheses during squat exercises. Using the early but common "Gait2392_simbody" (OpenSim) scaled musculoskeletal models, tibio-femoral JCFs were calculated in 6 subjects for 5 repetitions of squats. Tibio-femoral JCFs of 0.8–3.2 times bodyweight (BW) were measured. While the musculoskeletal simulations underestimated the measured knee JCFs at low flexion angles, an average error of less than 20% was achieved between approximately 25°–60° knee flexion. With an average error that behaved almost linearly with knee flexion angle, an overestimation of approximately 60% was observed at deep flexion (ca. 80°), with an absolute maximum error of ca. 1.9BW. Knee osteoarthritis is a major public health problem and adults with obesity are particularly at risk. One approach to alleviating this problem is to reduce the mechanical load at the joint during daily activity. Adjusting temporospatial parameters of walking could mitigate cumulative knee joint mechanical loads. The purpose of this study was to determine how adjustments to velocity and step length affects knee joint loading in healthy weight adults and adults with obesity. We collected three‐dimensional gait analysis data on 10 adults with a normal body mass index and 10 adults with obesity during over ground walking in nine different conditions. In addition to preferred velocity and step length, we also conducted combinations of 15% increased and decreased velocity and step length. Peak tibiofemoral joint impulse and knee adduction angular impulse were reduced in the decreased step length conditions in both healthy weight adults (main effect) and those with obesity (interaction effect). Peak knee joint adduction moment was also reduced with decreased step length, and with decreased velocity in both groups. We conclude from these results that adopting shorter step lengths during daily activity and when walking for exercise can reduce mechanical stimuli associated with articular cartilage degenerative processes in adults with and without obesity. Thus, walking with reduced step length may benefit adults at risk for disability due to knee osteoarthritis. Background: Although there are a few studies on kinetic and kinematic parameters of scoliotic subjects, it is still controversial whether gait performance of scoliotic subjects differs from that of normal subjects or not. Moreover, there is lack of information regarding joint contact force of scoliotic on convex and concave sides. Therefore, this study examined these issues. Method: Two groups of children (healthy and children with scoliosis, each group consisting of 5 subjects) participated in this study. The force applied on leg and motions of body parts were evaluated using a Kistler force plate and motion analysis system, respectively. Joint contact forces, muscles length were evaluated in both groups and on both sides (in scoliotic subjects) with OpenSim software. The difference in the parameters between healthy children and scoliotic subjects, and also concave and convex sides, was determined using the independent t test. P value was set at ≤ 0. 0 5. Results: The results of this study showed that there was no difference between the forces applied on the leg, range of motions of the joints (hip, knee, ankle, pelvic and trunk), muscles length and joint contact force, neither between normal and children with scoliosis, nor between concave and convex sides (p > 0. 0 5). Conclusion: The findings revealed that scoliosis deformity with curve less than 4 0 ∘ does not have any significant effect on joint contact force, kinetic and kinematic parameters. The net force and moment of a joint have been widely used to understand joint disease in the foot. Meanwhile, it does not reflect the physiological forces on muscles and contact surfaces. The objective of the study is to estimate active moments by muscles, passive moments by connective tissues and joint contact forces in the foot joints during walking. Joint kinematics and external forces of ten healthy subjects (all males, 24.7 ± 1.2 years) were acquired during walking. The data were entered into the five-segment musculoskeletal foot model to calculate muscle forces and joint contact forces of the foot joints using an inverse dynamics-based optimization. Joint reaction forces and active, passive and net moments of each joint were calculated from muscle and ligament forces. The maximum joint reaction forces were 8.72, 4.31, 2.65, and 3.41 body weight (BW) for the ankle, Chopart’s, Lisfranc and metatarsophalangeal joints, respectively. Active and passive moments along with net moments were also obtained. The maximum net moments were 8.6, 8.4, 5.4 and 0.8%BW∙HT, respectively. While the trend of net moment was very similar between the four joints, the magnitudes and directions of the active and passive moments varied between joints. The active and passive moments during walking could reveal the roles of muscles and ligaments in each of the foot joints, which was not obvious in the net moment. Our purpose was to compare joint loads between habitual rearfoot (hRF) and habitual mid/forefoot strikers (hFF), rearfoot (RFS) and mid/forefoot strike (FFS) patterns, and shorter stride lengths (SLs). Thirty-eight hRF and hFF ran at their normal SL, 5% and 10% shorter, as well as with the opposite foot strike. Three-dimensional ankle, knee, patellofemoral (PF) and hip contact forces were calculated. Nearly all contact forces decreased with a shorter SL (1.2–14.9% relative to preferred SL). In general, hRF had higher PF (hRF-RFS: 10.8 ± 1.4, hFF-FFS: 9.9 ± 2.0 BWs) and hip loads (axial hRF-RFS: −9.9 ± 0.9, hFF-FFS: −9.6 ± 1.0 BWs) than hFF. Many loads were similar between foot strike styles for the two groups, including axial and lateral hip, PF, posterior knee and shear ankle contact forces. Lateral knee and posterior hip contact forces were greater for RFS, and axial ankle and knee contact forces were greater for FFS. The tibia may be under greater loading with a FFS because of these greater axial forces. Summarising, a particular foot strike style does not universally decrease joint contact forces. However, shortening one’s SL 10% decreased nearly all lower extremity contact forces, so it may hold potential to decrease overuse injuries associated with excessive joint loads. Radiocapitellar joint arthroplasty is a commonly performed procedure, which often leads to early failure or instability. Few studies assess the effect of radiocapitellar joint arthroplasty on the ulnohumeral joint. We hypothesized that static forces of contact (compressing cartilage, or cartilage relaxation contact force) would reveal the effect of varying radial head implant size and elbow position on the ulnohumeral joint. A minimally–invasive method of measuring cartilage relaxation contact force was utilized in 10 fresh-frozen human cadaveric specimens that did not require significant dissection or intraarticular sensor placement. Specimens were rigidly fixed in various positions of elbow flexion and forearm pronosupination with increasing radial head implant lengths. Uniaxial distracting forces were applied and displacement was repeatedly measured with resultant best-fit polynomial curves to determine inflections corresponding to the force required to overcome static cartilage relaxation as in previous work. Baseline mean (intra-cadaver) cartilage relaxation contact force was 11.8 N (standard error of the mean = 0.3) at 90° of elbow flexion and neutral rotation. There was little variation within specimens (Intraclass correlation coefficient > 0.94). Cartilage relaxation contact force increased at the ulnohumeral joint with radial head implant overstuffing (> 4 mm, P < 0.05) and elbow flexion (120°, P < 0.001). Pronosupination altered cartilage relaxation contact force in an implant-length independent manner (P < 0.05). Radiocapitellar joint arthroplasty implant length and elbow joint position independently contribute to increased cartilage relaxation contact force at the ulnohumeral joint. This further supports attempts at anatomic reconstruction of the radiocapitellar joint to prevent pathologic ulnohumeral joint loading. • Ulnohumeral joint cartilage relaxation contact force can be measured. • Ulnohumeral contact force increases with elbow supination and flexion. • Radial head arthroplasty overstuffing increases ulnohumeral contact force. • Ulnohumeral contact force can be measured without destroying static joint stabilizers. The incidence of knee and hip joint osteoarthritis in subjects with below knee amputation (BK) appears significantly higher compared to unimpaired subjects, especially in the intact side. However, it is controversial if constant higher loads on the sound side are one of the major factors for an increased osteoarthritis (OA) incidence in subjects with BK, beside other risk factors, e.g. with respect to metabolism. The aim wasto investigate joint contact forces (JCF) calculated by a musculoskeletal model in the intact side and to compare it with those of unimpaired subjects and to further elucidate in how far increased knee JCF are associated with increased frontal plane knee moments. A group of seven subjects with BK amputation and a group of ten unimpaired subjects were recruited for this study. Gait data were measured by 3D motion capture and force plates. OpenSim software was applied to calculate JCF. Maximum joint angles, ground reaction forces, and moments as well as time distance parameters were determined and compared between groups showing no significant differences, with some JCF components of knee and hip even being slightly smaller in subjects with BK compared to the reference group. This positive finding may be due to the selected ESAR foot. However, other beneficial factors may also have influenced this positive result such as the general good health status of the subjects or the thorough and proper fitting and alignment of the prosthesis. The surgical installation accuracy of the components in unicompartmental knee arthroplasty (UKA) is an important factor affecting the joint function and the implant life. Taking the ratio of the medial-lateral position of the femoral component relative to the tibial insert (a/A) as a parameter, and considering nine installation conditions of the femoral component, this study established the musculoskeletal multibody dynamics models of UKA to simulate the patients' walking gait, and investigated the influences of the medial-lateral installation positions of the femoral component in UKA on the contact force, joint motion and ligament force of the knee joint. The results showed that, with the increase of a/A ratio, the medial contact force of the UKA implant was decreased and the lateral contact force of the cartilage was increased; the varus rotation, external rotation and posterior translation of the knee joint were increased; and the anterior cruciate ligament force, posterior cruciate ligament force and medial collateral ligament force were decreased. The medial-lateral installation positions of the femoral component in UKA had little effect on knee flexion-extension movement and lateral collateral ligament force. When the a/A ratio was less than or equalled to 0.375, the femoral component collided with the tibia. In order to prevent the overload on the medial implant and lateral cartilage, the excessive ligament force, and the collision between the femoral component and the tibia, it is suggested that the a/A ratio should be controlled within the range of 0.427-0.688 when the femoral component is installed in UKA. This study provides a reference for the accurate installation of the femoral component in UKA.

The femoral head in subjects with leg calve perthes disease (LCPD) is generally considerably deformed. It is debatable whether this deformation is due to an increase in applied loads, a decrease in bone mineral density or a change in containment of articular surfaces. The aim of this study was to determine the influence of these factors on deformation of the femoral head. Two subjects with LCPD participated in this study. Subject motion and the forces applied on the affected leg were recorded using a motion analysis system ( Qualsis TM ) and a Kistler force plate. OpenSim software was used to determine joint contact force of the hip joint whilst walking with and without a Scottish Rite orthosis. 3D Models of hip joints of both subjects were produced by Mimics software . The deformation of femoral bone was determined by Abaqus . Mean values of the force applied on the leg increased while walking with the orthosis. There was no difference between bone mineral density (BMD) of the femoral bone of normal and LCPD sides ( p -value>0.05) and no difference between hip joint contact force of normal and LCPD sides. Hip joint containment appeared to decrease follow the use of the orthosis. It can be concluded that the deformation of femoral head in LCPD may not be due to change in BMD or applied load. Although the Scottish Rite orthosis is used mostly to increase hip joint containment, it appears to reduce hip joint contact area. Traditional approaches to the biomechanical analysis of movement are joint-based; that is the mechanics of the body are described in terms of the forces and moments acting at the joints, and that muscular forces are considered to create moments about the joints. We have recently shown that segment-based approaches, where the mechanics of the body are described by considering the effect of the muscle, ligament and joint contact forces on the segments themselves, can also prove insightful. We have also previously described a simultaneous, optimization-based, musculoskeletal model of the lower limb. However, this prior model incorporates both joint- and segment-based assumptions. The purpose of this study was therefore to develop an entirely segment-based model of the lower limb and to compare its performance to our previous work. The segment-based model was used to estimate the muscle forces found during vertical jumping, which were in turn compared with the muscular activations that have been found in vertical jumping, by using a Geers' metric to quantify the magnitude and phase errors. The segment-based model was shown to have a similar ability to estimate muscle forces as a model based upon our previous work. In the future, we will evaluate the ability of the segment-based model to be used to provide results with clinical relevance, and compare its performance to joint-based approaches. Downhill walking is a necessary part of daily life and an effective activity in post-operative rehabilitation following total knee arthroplasty. The purpose of this study was to determine differences in the behavior of total, medial, and lateral tibiofemoral compressive forces as well as knee extensor and flexor muscle forces between different limbs of patients with total knee arthroplasty (replaced, non-replaced) during downhill and level walking. Musculoskeletal modeling and simulation were implemented to determine muscle forces and tibiofemoral compressive forces in 25 patients with total knee arthroplasty. A 2 × 2 [Limb (replaced, non-replaced) × Slope (0°, 10°)] Statistical parametric mapping repeated measures analysis of variance was conducted on selected variables. Statistical parametric mapping did not identify any between-limb differences for compressive or muscle forces. Differences in joint compressive and muscle forces persisted throughout different intervals of stance-phase between level and downhill walking. Knee extensor muscle forces were distinctly greater during level walking for nearly all of stance phase. Knee flexor muscle force was greater during downhill walking for >60% of stance. Statistical parametric mapping did identify regions of significance between level and downhill walking that coincided temporally (near loading response and push off) with peak joint moment and joint compressive forces traditionally reported using discrete variable analyses. Downhill walking may be a safe and useful rehabilitation tool for post-knee arthroplasty rehabilitation that will not disproportionally load either the replaced or the non-replaced joint and where the quadriceps muscles can be strengthened during a gait-specific task. • Stance-phase knee joint compressive forces were evaluated during downhill walking. • Results revealed no between-limb differences, only between slope-differences. • Downhill walking may not disproportionally load either limb. • This task may be a useful gait-specific task for post-operative rehabilitation. Introduction: Several studies have investigated walking characteristics in hip dysplasia patients, but so far none have described all hip rotational degrees of freedom during the whole gait cycle. This descriptive study reports 3D joint angles and torques, and furthermore extends previous studies with muscle and joint contact forces in 32 hip dysplasia patients and 32 matching controls.Methods: 3D motion capture data from walking and standing trials were analysed. Hip, knee, ankle and pelvis angles were calculated with inverse kinematics for both standing and walking trials. Hip, knee and ankle torques were calculated with inverse dynamics, while hip muscle and joint contact forces were calculated with static optimisation for the walking trials.Results: No differences were found between the two groups while standing. While walking, patients showed decreased hip extension, increased ankle pronation and increased hip abduction and external rotation torques. Furthermore, hip muscle forces were generally lower and shifted to more posteriorly situated muscles, while the hip joint contact force was lower and directed more superiorly.Conclusion: During walking, patients showed lower and more superiorly directed hip joint contact force, which might alleviate pain from an antero-superiorly degenerated joint. A cohort of adult acquired flatfoot deformity rigid-body models was developed to investigate the effects of isolated tendon transfer with successive levels of medializing calcaneal osteotomy (MCO). Following IRB approval, six diagnosed flatfoot sufferers were subjected to magnetic resonance imaging (MRI) and their scans used to derive patient-specific models. Single-leg stance was modeled, constrained solely through physiologic joint contact, passive soft-tissue tension, extrinsic muscle force, body weight, and without assumptions of idealized mechanical joints. Surgical effect was quantified using simulated mediolateral (ML) and anteroposterior (AP) X-rays, pedobarography, soft-tissue strains, and joint contact force. Radiographic changes varied across states with the largest average improvements for the tendon transfer (TT) + 10 mm MCO state evidenced through ML and AP talo-1st metatarsal angles. Interestingly, 12 of 14 measures showed increased deformity following TT-only, though all increases disappeared with inclusion of MCO. Plantar force distributions showed medial forefoot offloading concomitant with increases laterally such that the most corrected state had 9.0% greater lateral load. Predicted alterations in spring, deltoid, and plantar fascia soft-tissue strain agreed with prior cadaveric and computational works suggesting decreased strain medially with successive surgical repair. Finally, joint contact force demonstrated consistent medial offloading concomitant with variable increases laterally. In a previous cadaver study, we directly measured the load acting on the trapeziometacarpal joint for increasingly greater key pinch forces. We noted that the joint load ranges from 2 kg to 4 kg during progressively greater key pinch from 0.5 kg to 1.5 kg. Using the same experimental approach, the aim of the current study was to measure and compare the load acting on the scaphotrapeziotrapezoid joint for the same levels of isometric key pinch force, and how it changes after trapeziometacarpal arthroplasty. We performed a cadaver study using 7 fresh-frozen, unembalmed adult forearms and hands (2 right and 5 left). Thumb pinch was simulated by loading the main actuator tendons involved in the key pinch grip (i.e., adductor pollicis, flexor pollicis longus, extensor pollicis longus, extensor pollicis brevis and abductor pollicis longus tendons). Measurements were made inside the joint using a force-sensing resistor sensor (Tekscan® FlexiForce™ force sensor). Before the trapeziometacarpal joint surgery, median load values recorded in the scaphotrapeziotrapezoid joint were 1.2 kg (IQR, 1.0-1.4), 1.6 kg (IQR, 1.6-2.5) and 2.4 kg (IQR, 2.3-3.4) during 0.5 kg, 1 kg and 1.5 kg key pinch, respectively. After the trapeziometacarpal arthroplasty, median joint contact forces did not change significantly relative to the original configuration. Our findings indicate that the loads measured in the scaphotrapeziotrapezoid joint during a simple key pinch are in fact lower than those measured inside the trapeziometacarpal joint. After trapeziometacarpal arthroplasty, the values are similar with no increase in load, suggesting that clinically asymptomatic scaphotrapeziotrapezoid radiographic involvement may not be a contraindication to arthroplasty., (Copyright © 2022 SFCM. Background The major complication in nonconforming total shoulder replacement (TSR) is glenoid loosening and is attributed to posteriorly directed humeral head translations. Whether the posterior translations observed clinically are induced by radial mismatch is unclear. The objective of our study was to explain the posterior glenohumeral translations observed clinically after TSR by determining the glenohumeral translation and contact force as a function of radial mismatch. We hypothesized that the posterior direction of glenohumeral translation during scaption would be related to the radial mismatch and that the joint contact force would increase as the radial mismatch increased. Methods A 6-degrees-of-freedom computational model of the glenohumeral joint was developed. We determined the muscle forces, joint contact force, and glenohumeral translation for radial mismatches from 1 mm to 20 mm with the shoulder positioned from 20° to 60° of scaption. Results As the radial mismatch increased, the contact location of the humeral head moved posteriorly and inferiorly. The middle deltoid force decreased by 3%, while the supraspinatus and infraspinatus muscle forces increased by 9% and 11%, respectively. The joint contact force remained relatively constant. Conclusions Increased posterior glenohumeral translations were observed with increased radial mismatch. Clinical observations of posterior translation may be attributed to the balancing forces of the middle deltoid, infraspinatus, and supraspinatus muscles. Lumbar disc herniation patients with increased pain exhibit greater gait asymmetry in stance time, swing time and single support time. Percutaneous endoscopic lumbar discectomy, as a minimally invasive surgical procedure has been used to treat patients with lumbar disc herniation. The objective of this study was to evaluate the immediate impact of the percutaneous endoscopic lumbar discectomy on gait asymmetry in spatiotemporal and kinetic parameters among lumbar disc herniation patients. Marker trajectories and ground reaction forces were measured during walking among 67 lumbar disc herniation patients and 15 healthy controls. Spatiotemporal gait parameters were analyzed via Visual3D. Muscle force and joint contact force were calculated with OpenSim. Gait asymmetry of those parameters were assessed with asymmetry index. After surgery, gait asymmetry in gait cycle time, step length, peak biceps femoris long head, tensor fasciae latae and rectus femoris muscle forces, and peak hip and knee joint contact forces reduced immediately. Postoperatively, increased gait cycle time and decreased step length were found in the affected side. Moreover, decreased peak biceps femoris long head, tensor fasciae latae and rectus femoris muscle forces, and peak hip joint contact force were observed in the contralateral side. These results suggested compensation strategy that biceps femoris long head, tensor fasciae latae and rectus femoris in the contralateral side were mainly used to compensate the affected side preoperatively in lumbar disc herniation patients, with less compensation between lower limbs after surgery, which may provide an insight into postoperative rehabilitation. • Gait asymmetry was estimated in lumbar disc herniation patients before and after surgery. • Gait asymmetry improved in lumbar disc herniation patients after surgery. • Postoperative lumbar disc herniation patients walked with less compensation. Although the conventional total knee arthoplasty (TKA) planning used 2D or 3D images of the patient’s lower extremity to determine bone alignment, it was shown that good bone alignment does not always guarantee successful results due to unbalanced joint contact forces and soft tissue tension. Thus, the joint contact force balance and soft tissue tension were pre-operatively calculated using finite element analysis (FEA). Although the results of FEA are accurate, the rigid body spring method (RBSM) has introduced for the joint contact force or pressure distribution due to the high costs for FEA. In this paper, an innovative method was described to quantify the pre- and post-operative knee contact force balance using various engineering techniques, including the free-form deformation (FFD) technique, and RBSM-based cartilage contact force and ligament tension estimation algorithm. Nine cases of TKA planning were performed with three different surgical options, including over-correction of the valgus ratio of the femur, tibial plateau cutting length, and MCL release to redistribute the medial and lateral contact force balance. Musculoskeletal simulation software and model repositories have broadened the user base able to perform musculoskeletal analysis and have facilitated in the sharing of models. As the recognition of musculoskeletal modeling continues to grow as an engineering discipline, the consistency in results derived from different models and software is becoming more critical. The purpose of this study was to compare eight models from three software packages and evaluate differences in quadriceps moment arms, predicted muscle forces, and predicted tibiofemoral contact forces for an idealized knee-extension task spanning −125 to +10° of knee extension. Substantial variation among models was observed for the majority of aspects evaluated. Differences among models were influenced by knee angle, with better agreement of moment arms and tibiofemoral joint contact force occurring at low to moderate knee flexion angles. The results suggest a lack of consistency among models and that output differences are not simply an artifact of naturally occurring inter-individual differences. Although generic musculoskeletal models can easily be scaled to consistent limb lengths and use the same muscle recruitment algorithm, the results suggest those are not sufficient conditions to produce consistent muscle or joint contact forces, even for simplified models with no potential of co-contraction. Typical inverse dynamics approaches to the calculation of muscle, ligament, and joint contact forces are based on a step-wise solution of the equations of motion. This approach is therefore limited in its ability to provide insight as to the muscular, ligamentous, and articular interactions that create joint stability. In this study, a new musculoskeletal model of the lower limb is described, in which the equations of motion describing the force and moment equilibrium at the joints of the lower limb are solved simultaneously using optimization techniques. The new model was employed to analyze vertical jumping using a variety of different optimization cost functions and the results were compared to more traditional approaches. The new model was able to find a solution with lower muscular force upper bounds due to the ability of the ligaments to contribute to moment equilibrium at the ankle and knee joints. Equally, the new model produced lower joint contact forces than traditional approaches for cases which also included a consideration as to ligament or joint contact forces within the cost function. This study demonstrates the possibility of solving the inverse dynamic equations of motion simultaneously using contemporary technology, and further suggests that this might be important due to the complementary function of the muscles and ligaments in creating joint stability. Traditional inverse dynamics approaches to calculating the inter-segmental moments are limited in their ability to accurately reflect the function of the biarticular muscles. In particular they are based on the assumption that the net inter-segmental moment is zero and that total joint moments are independent of muscular activity. Traditional approaches to calculating muscular forces from the inter-segmental moments are based on a consideration of joint moments which do not encapsulate the potential moment asymmetry between segments. In addition, traditional approaches may artificially constrain the activity of the biarticular muscles. In this study, an optimization approach to the simultaneous inverse determination of inter-segmental moments and muscle forces (the 1-step method) based on a consideration of segmental rotations was employed to study vertical jumping and contrasted with the more traditional 2-step approach of determining inter-segmental moments from an inverse dynamics analysis then muscle forces using optimization techniques. The 1-step method resulted in significantly greater activation of both the monoarticular and biarticular musculature which was then translated into significantly greater joint contact forces, muscle powers, and inter-segmental moments. The results of this study suggest that traditional conceptions of inter-segmental moments do not completely encapsulate the function of the biarticular muscles and that joint function can be better understood by recognizing the asymmetry in inter-segmental moments.
