1. Factors associated with HIV status disclosure to partners and its outcomes among HIV-positive women attending Care and Treatment Clinics at Kilimanjaro region, Tanzania.

Abstract

BackgroundSub Saharan Africa continues to be the epicenter of HIV with 70% of people living with HIV globally. Women form nearly 60% of those living with HIV. Studies have shown disclosure of one's HIV status is important in HIV prevention, in increasing partners who are tested and getting into care early as well as in improving retention in PMTCT and ART programs. This study aimed to determine the prevalence, factors and outcomes of HIV status disclosure to partners among HIV-positive women attending HIV care-and-treatment clinics (CTCs) at Kilimanjaro region, northern Tanzania.MethodsA cross-sectional study was conducted from January to June 2014 in 3 out of the 7 districts of Kilimanjaro region. The study population was HIV-positive women aged 15-49, who were attending for routine HIV care at 19 selected clinics. Face-to-face interviews were conducted with consenting women to collect necessary information. Multivariate logistic regression analyses were used to determine the independent predictors of HIV status disclosure to partner.ResultsA total of 672 HIV-positive women in Moshi municipal, Hai and Mwanga districts were enrolled. Of them, 609 HIV-positive women reported to have a regular partner. Prevalence of serostatus disclosure to partners was 66%. Of the 400 who had disclosed; 56% did so within the first month of knowing their HIV status. In a multiple logistic regression model, HIV serostatus disclosure was higher among women who: were married/cohabiting (AOR = 4.16, 95% CI: 2.39-7.25; pConclusionIn this setting still a third of the HIV-positive women (34%) fail to disclose their HIV- serostatus to partners. Interventions to impart skills in communication and negotiation between partners may help in improving disclosure of HIV. Efforts to involve men in general sexual and reproductive health including couple counseling and testing will contribute in improving disclosure and communication on HIV among partners.

Abstract

The cellular response to radiation insult and studies have been carried out to investigate aspects of the cytoskeleton and the force response of the cell when probed by an AFM. Confirmed for the first time that there was a statistically significant difference for the PNT2 and PC3 cell lines in response to probing with the AFM tip, and that time was eliminated a possible influencing factor in the short term (1 hour) for the force response. Showed that the Hertz model is not sufficient for distances greater than 500nm due to the strain hardening effect for biological cells and that the biological cells non-linear force response becomes marked after the 500nm region. The orientation of actin was investigated and a bimodal variation was statistical significant, although the larger tendency was for a 90 degree separation there was indications that earlier theoretical work by Pollard, 2008 was present. The importance of the contact point when considering the cell lines PNT2, DU145 and PC3 and greater than 500nm indentation is shown and four different methods are tested and the most robust of these chosen as the method for the distance and cell lines involved. That being ‘line projection’ method created by the author. A method that normalises the data for AFM force curves is presented, the method minimises the contact point error at the same time and therefore provides biologists with a way to test cell lines using standard normal population tests.

Abstract

We study the periodic forced response of a system of two limit cycle oscillators that interact with each other via a time delayed coupling. Detailed bifurcation diagrams in the parameter space of the forcing amplitude and forcing frequency are obtained for various interesting limits using numerical and analytical means. In particular, the effects of the coupling strength, the natural frequency spread of the two oscillators and the time delay parameter on the size and nature of the entrainment domain are delineated. The system is found to display a nonlinear response on certain critical contours in the space of the coupling strength and time delay. Time delay offers a novel tuning knob for controlling the system response over a wide range of frequencies and this may have important practical applications., Comment: 20 pages, 15 figures. Abstract

Experimental observations of time delay induced amplitude death in a pair of coupled nonlinear electronic circuits that are individually capable of exhibiting limit cycle oscillations are described. In particular, the existence of multiply connected death islands in the parameter space of the coupling strength and the time delay parameter for coupled identical oscillators is established. The existence of such regions was predicted earlier on theoretical grounds in [Phys. Rev. Lett. 80, 5109 (1998); Physica 129D, 15 (1999)]. Abstract

We study the effects of time delayed linear and nonlinear feedbacks on the dynamics of a single Hopf bifurcation oscillator. Our numerical and analytic investigations reveal a host of complex temporal phenomena such as phase slips, frequency suppression, multiple periodic states and chaos. Such phenomena are frequently observed in the collective behavior of a large number of coupled limit cycle oscillators. Our time delayed feedback model offers a simple paradigm for obtaining and investigating these temporal states in a single oscillator.We construct a detailed bifurcation diagram of the oscillator as a function of the time delay parameter and the driving strengths of the feedback terms. We find some new states in the presence of the quadratic nonlinear feedback term with interesting characteristics like birhythmicity, phase reversals, radial trapping, phase jumps and spiraling patterns in the amplitude space. Abstract

We present a detailed study of the effect of time delay on the collective dynamics of coupled limit cycle oscillators at Hopf bifurcation. For a simple model consisting of just two oscillators with a time delayed coupling, the bifurcation diagram obtained by numerical and analytical solutions shows significant changes in the stability boundaries of the amplitude death, phase locked and incoherent regions. A novel result is the occurrence of amplitude death even in the absence of a frequency mismatch between the two oscillators. Similar results are obtained for an array of N oscillators with a delayed mean field coupling and the regions of such amplitude death in the parameter space of the coupling strength and time delay are quantified. We investigate the dynamical behaviour of two limit cycle oscillators that interact with each other via time delayed coupling and find that time delay can lead to amplitude death of the oscillators even if they have the same frequency. We demonstrate that this novel regime of amplitude "death" also exists for large collections of coupled identical oscillators and provide quantitative measures of this death region in the parameter space of coupling strength and time delay. 19. Measuring the digital divide among people with severe mental ill health using the essential digital skills framework.

Abstract

Aims: Amid the vast digitalisation of health and other services during the pandemic, people with no digital skills are at risk of digital exclusion. This risk might not abate by the end of the pandemic. This article seeks to understand whether people with severe mental ill health (SMI) have the necessary digital skills to adapt to these changes and avoid digital exclusion. Methods: Two hundred and forty-nine adults with SMI across England completed a survey online or offline. They provided information on their digital skills based on the Essential Digital Skills (EDS) framework, sociodemographic information, and digital access. This is the first time that the EDS is benchmarked in people with SMI. Results: 42.2% had no Foundation Skills, and 46.2% lacked skills for daily life (lacking Foundation or Life Skills). 23.0% of those working lacked skills for professional life (lacking Foundation or Work Skills). The most commonly missing skills were handling passwords and using the device settings (Foundation Skills) and online problem solving (Skills for Life). People were interested in learning more about approximately half of the skills they did not have. People were more likely to lack Foundation Skills if they were older, not in employment, had a psychosis-spectrum disorder, or had no Internet access at home. Conclusion: A significant portion of people with SMI lacked Foundation Skills in this objective and benchmarked survey. This points to a high risk for digital exclusion and the need for focused policy and tailored health sector support to ensure people retain access to key services and develop digital skills and confidence. To our knowledge, this is the first time this has been described using the EDS framework. Services, including the National Health Service (NHS), need to be aware of and mitigate the risks. 21. The Confidential Enquiry into Perioperative Equine Fatalities : a prospective multicentre cohort study of equine perioperative mortality

Abstract

The Confidential Enquiry into Perioperative Equine Fatalities (CEPEF) was a prospective multicentre cohort study of equine perioperative mortality within seven days of surgery. The study was designed to establish the incidence of equine perioperative morality and identify any predisposing risk factors for non-colic surgery. Records of 41,787 operations under general anaesthesia were submitted by 149 clinics from 19 countries from 1991 to 1996. The total mortality from perioperative complications was 1.9%. The death rate in the 35,927 non-colic operations was 0.95 and was 7.95 for the 5,833 colics. Twenty two different causes of death were identified for non-colics. The most common causes of death were cardiac arrest (32.82%), fracture (23.31%) and myopathy (7.06%). The signalment of the patients undergoing colic surgery were found to be very different from non-colics. Univariate analysis of non-colics for all types of death identified numerous risk factors which were associated with increased or decreased risk of death including age of the patient, drugs used for sedation and induction, clinic type, operation, and day of the week. The most frequent cause of death in equidae, intraoperative cardiac arrest (0.33%), was examined in greater detail in non-colics. Multivariable models were built using logistic regression for two groups of patients; all non colics and a second model for those who received only inhalation maintenance agents. The final model for both groups which included variables for age and gender demonstrated that sedative premedication, induction agent, type of clinic, starting time of anaesthesia, body position and day of the week were all associated with death from cardiac arrest.

30. Ideological constraints on Australian education, 1945-1975

Abstract

Four major Australian educational policies adopted between 1945 and 1975 have fallen short of the objectives set for them. This study is intended to describe those policies and to account for their shortfall. The central theme is that the four policies, 'selective', 'comprehensive', 'open', and 'community' education can be seen as manifestations of broader 'ideologies': 'elitism', 'egalitarianism', 'individualism', and 'collectivism' respectively. It is argued that this ideological source of educational ‚Äö prescription has been a constraint in the pursuit of policy objectives. A characteristic of 'ideology' as defined for this study is that some of the central beliefs of the system of values and beliefs of which it is constituted are uncorroborated or in error. It is shown that at the time at which these four policies were adopted by Australian education authorities the beliefs which they were based upon had not been the subject of any rigorous investigation and thus reflected ideological commitments rather than the application of tested educational knowledge. The uncorroborated methodological beliefs in the four prescriptions can be seen as distinguished by either 'rationalist' or 'romanticist' characteristics, that is, as derived from either the exercise of untested reason or, alternatively, from faith in emotional response as means to valued objectives. Several other influential educational prescriptions adopted or advocated since 1945 can be categorised under one or other of these two social methodologies. The concept of the 'dialectic' in social theory is briefly examined as a means of accounting for the frequent changes in recent Australian educational policy and for the light it might throw upon the disappointing achievement associated with this instability. To conclude, a general outline is given of an approach to educational innovation which emphasises an 'applied' science perspective. Translated into administrative terms this is seen to imply the advisability of a flexible application of 'action research' for increased effectiveness of future educational prescriptions.

Abstract

Establishing the abundance and physical properties of regolith and boulders on asteroids is crucial for understanding the formation and degradation mechanisms at work on their surfaces. Using images and thermal data from NASA's Origins, Spectral Interpretation, Resource Identification, and Security-Regolith Explorer (OSIRIS-REx) spacecraft, we show that asteroid (101955) Bennu's surface is globally rough, dense with boulders, and low in albedo. The number of boulders is surprising given Bennu's moderate thermal inertia, suggesting that simple models linking thermal inertia to particle size do not adequately capture the complexity relating these properties. At the same time, we find evidence for a wide range of particle sizes with distinct albedo characteristics. Our findings imply that ages of Bennu's surface particles span from the disruption of the asteroid's parent body (boulders) to recent in situ production (micrometre-scale particles).

Abstract

Background: The multimorbid burden and use of systemic immunosuppressants in people with psoriasis may confer greater risk of adverse outcomes of coronavirus disease 2019 (COVID-19), but the data are limited. Objective: Our aim was to characterize the course of COVID-19 in patients with psoriasis and identify factors associated with hospitalization. Methods: Clinicians reported patients with psoriasis with confirmed/suspected COVID-19 via an international registry, Psoriasis Patient Registry for Outcomes, Therapy and Epidemiology of COVID-19 Infection. Multiple logistic regression was used to assess the association between clinical and/or demographic characteristics and hospitalization. A separate patient-facing registry characterized risk-mitigating behaviors. Results: Of 374 clinician-reported patients from 25 countries, 71% were receiving a biologic, 18% were receiving a nonbiologic, and 10% were not receiving any systemic treatment for psoriasis. In all, 348 patients (93%) were fully recovered from COVID-19, 77 (21%) were hospitalized, and 9 (2%) died. Increased hospitalization risk was associated with older age (multivariable-adjusted odds ratio [OR] = 1.59 per 10 years; 95% CI = 1.19-2.13), male sex (OR = 2.51; 95% CI = 1.23-5.12), nonwhite ethnicity (OR = 3.15; 95% CI = 1.24-8.03), and comorbid chronic lung disease (OR = 3.87; 95% CI = 1.52-9.83). Hospitalization was more frequent in patients using nonbiologic systemic therapy than in those using biologics (OR = 2.84; 95% CI = 1.31-6.18). No significant differences were found between classes of biologics. Independent patient-reported data (n = 1626 across 48 countries) suggested lower levels of social isolation in individuals receiving nonbiologic systemic therapy than in those receiving biologics (OR = 0.68; 95% CI = 0.50-0.94). Abstract

Introduction Neurocritical care focuses on the care of critically ill patients with an acute neurologic disorder and has grown significantly in the past few years. However, there is a lack of data that describe the scope of practice of neurointensivists and epidemiological data on the types of patients and treatments used in neurocritical care units worldwide. To address these issues, we designed a multicenter, international, point-prevalence, cross-sectional, prospective, observational, non-interventional study in the setting of neurocritical care (PRINCE Study). Methods In this manuscript, we analyzed data from the initial phase of the study that included registration, hospital, and intensive care unit (ICU) organizations. We present here descriptive statistics to summarize data from the registration case report form. We performed the Kruskal–Wallis test followed by the Dunn procedure to test for differences in practices among world regions. Results We analyzed information submitted by 257 participating sites from 47 countries. The majority of those sites, 119 (46.3%), were in North America, 44 (17.2%) in Europe, 34 (13.3%) in Asia, 9 (3.5%) in the Middle East, 34 (13.3%) in Latin America, and 14 (5.5%) in Oceania. Most ICUs are from academic institutions (73.4%) located in large urban centers (44% > 1 million inhabitants). We found significant differences in hospital and ICU organization, resource allocation, and use of patient management protocols. The highest nursing/patient ratio was in Oceania (100% 1:1). Dedicated Advanced Practiced Providers are mostly present in North America (73.7%) and are uncommon in Oceania (7.7%) and the Middle East (0%). The presence of dedicated respiratory therapist is common in North America (85%), Middle East (85%), and Latin America (84%) but less common in Europe (26%) and Oceania (7.7%). The presence of dedicated pharmacist is highest in North America (89%) and Oceania (85%) and least common in Latin America (38%). The majority of respondents reported having a dedicated neuro-ICU (67% overall; highest in North America: 82%; and lowest in Oceania: 14%). Conclusion The PRINCE Study results suggest that there is significant variability in the delivery of neurocritical care. The study also shows it is feasible to undertake international collaborations to gather global data about the practice of neurocritical care. There is limited information regarding epidemiological data, disease characteristics, variability of clinical care, and in-hospital mortality of neurocritically ill patients worldwide. We addressed these issues in the Point PRevalence In Neurocritical CarE (PRINCE) study, a prospective, cross-sectional, observational study. METHODS: We recruited patients from various intensive care units (ICUs) admitted on a pre-specified date, and the investigators recorded specific clinical care activities they performed on the subjects during their first 7 days of admission or discharge (whichever came first) from their ICUs and at hospital discharge. In this manuscript, we analyzed the final data set of the study that included patient admission characteristics, disease type and severity, ICU resources, ICU and hospital length of stay, and in-hospital mortality. We present descriptive statistics to summarize data from the case report form. We tested differences between geographically grouped data using parametric and nonparametric testing as appropriate. We used a multivariable logistic regression model to evaluate factors associated with in-hospital mortality. RESULTS: We analyzed data from 1545 patients admitted to 147 participating sites from 31 countries of which most were from North America (69%, N = 1063). Globally, there was variability in patient characteristics, admission diagnosis, ICU treatment team and resource allocation, and in-hospital mortality. Seventy-three percent of the participating centers were academic, and the most common admitting diagnosis was subarachnoid hemorrhage (13%). The majority of patients were male (59%), a half of whom had at least two comorbidities, and median Glasgow Coma Scale (GCS) of 13. Factors associated with in-hospital mortality included age (OR 1.03; 95% CI, 1.02 to 1.04); lower GCS (OR 1.20; 95% CI, 1.14 to 1.16 for every point reduction in GCS); pupillary reactivity (OR 1.8; 95% CI, 1.09 to 3.23 for bilateral unreactive pupils); admission source (emergency room versus direct admission [OR 2.2; 95% CI, 1.3 to 3.75]; admission from a general ward versus direct admission [OR 5.85; 95% CI, 2.75 to 12.45; and admission from another ICU versus direct admission [OR 3.34; 95% CI, 1.27 to 8.8]); and the absence of a dedicated neurocritical care unit (NCCU) (OR 1.7; 95% CI, 1.04 to 2.47). CONCLUSION: PRINCE is the first study to evaluate care patterns of neurocritical patients worldwide. The data suggest that there is a wide variability in clinical care resources and patient characteristics. Neurological severity of illness and the absence of a dedicated NCCU are independent predictors of in-patient mortality.
