Abstract

Nilotinib, a tyrosine kinase inhibitor that targets the Abelson tyrosine kinase (c-Abl) signaling pathway, is FDA-approved to treat chronic myeloid leukemia. Nilotinib has properties indicative of a possible utility in neuroprotection that have prompted exploration of repurposing the drug for the treatment of Alzheimer's disease (AD) and Parkinson's disease (PD). AD is a progressive age-related neurodegenerative disorder characterized by the deposition of extracellular amyloid-β plaques and intracellular neurofibrillary tangles. It is incurable and affects approximately 50 million patients worldwide. Nilotinib reduces c-Abl phosphorylation, amyloid-β levels, and dopaminergic neuron degeneration in preclinical AD models. This study explores the effects of nilotinib on amyloid processing and mitochondrial functioning in the SH-SY5Y human neuroblastoma cell line. SH-SY5Y cells were exposed to nilotinib (1, 5, and 10 µM). Real-time PCR and immunoblot analysis were performed to quantify the expression of genes pertaining to amyloid-β processing and neuronal health. Nilotinib did not significantly change APP, BACE1, or ADAM10 mRNA levels. However, BACE1 protein was significantly increased at 1 µM, and ADAM10 was increased at 10 µM nilotinib without affecting APP protein expression. Further, nilotinib treatment did not affect the expression of genes associated with neuronal health and mitochondrial functioning. Abstract

Despite progress in treating rheumatoid arthritis, this autoimmune disorder confers an increased risk of developing cardiovascular disease (CVD). Widely used screening protocols and current clinical guidelines are inadequate for the early detection of CVD in persons with rheumatoid arthritis. Traditional CVD risk factors alone cannot be applied because they underestimate CVD risk in rheumatoid arthritis, missing the window of opportunity for prompt intervention to decrease morbidity and mortality. The lipid profile is insufficient to assess CVD risk. This review delves into the connection between systemic inflammation in rheumatoid arthritis and the premature onset of CVD. The shared inflammatory and immunologic pathways between the two diseases that result in subclinical atherosclerosis and disrupted cholesterol homeostasis are examined. The treatment armamentarium for rheumatoid arthritis is summarized, with a particular focus on each medication's cardiovascular effect, as well as the mechanism of action, risk–benefit profile, safety, and cost. A clinical approach to CVD screening and treatment for rheumatoid arthritis patients is proposed based on the available evidence. The mortality gap between rheumatoid arthritis and non-rheumatoid arthritis populations due to premature CVD represents an urgent research need in the fields of cardiology and rheumatology. 9. Preparing Reading Specialists to Become Competent Travelers in Urban Settings

Abstract

This article poses an alternative to traditional teacher education methods of preparing reading specialists in university programs focused solely on outcomes. An equally important issue to explore is that of process and how candidates are prepared. Studying outputs with limited attention to process factors leaves teacher educators with incomplete action steps and ambiguous mandates for change. Using qualitative applications, this case study of one university's preparation program for reading specialists reflects a phenomenological stance centered on the nature of the preparation process. Through methods of naturalistic inquiry--including field observations, interview data, written evaluations and use of applied research--the course instructor participates as the researcher, embracing the notion that "we can only know what we experience by attending to perceptions and meanings that awaken our conscious awareness." Because of that awakening, the author hopes that useful modifications to programs are made, which in turn lead to increased readiness of those who are prepared to teach in urban settings.

Abstract

Background/Study Context: The goal of the study was to identify and characterize latent profiles (clusters) of cognitive functioning in centenarians and the psychometric properties of cognitive measures within them. Methods: Data were collected from cross-sectional, population-based sample of 244 centenarians (aged 98 to 108, 15.8% men, 20.5% African American, 38.0% community-dwelling) from 44 counties in northern Georgia participating in the Georgia Centenarian Study (2001-2008). Measures included the Mini-Mental State Examination (MMSE), Severe Impairment Battery (SIB), Wechsler Adult Intelligence Scale-III Similarities subtest (WAIS), Hand Tapping, Behavioral Dyscontrol Scale (BDS), Controlled Oral Word Association Test (COWAT), and Fuld Object Memory Evaluation (FOME). The Global Deterioration Rating Scale (GDRS) was used to independently evaluate criterion-related validity for distinguishing cognitively normal and impaired groups. Relevant covariates included directly assessed functional status for basic and instrumental activities of daily living (DAFS), race, gender, educational attainment, Geriatric Depression Scale Short Form (GDS), and vision and hearing problems. Results: Results suggest two distinct classes of cognitive performance in this centenarian sample. Approximately one third of the centenarians show a pattern of markedly lower cognitive performance on most measures. Group membership is independently well predicted (area under the curve [AUC]=.83) by GDRS scores (sensitivity 67.7%, specificity 82.4%). Membership in the lower cognitive performance group was more likely for individuals who were older, African Americans, had more depressive symptoms, lower plasma folate, carriers of the apolipoprotein E (APOE) c4 allele, facility residents, and individuals who died in the 2 years following interview. Conclusions: In a population expected to have high prevalence of dementia, latent subtypes can be distinguished via factor mixture analysis that provide normative values for cognitive functioning. Abstract

This study provides, for the first time, normative data on cognitive functioning and physical performance, health and health behaviors, and diseases from a population-based sample of 244 centenarians and near-centenarians (M age = 100.5 years, range 98-108, 84.8% women, 21.3% African American) from the Georgia Centenarian Study. Data are presented by the four key dimensions of gender, race, residence, and educational attainment. Results illustrate the profound range of functioning in this age group and indicate considerable differences as a function of each dimension. Bivariate models generally suggest that cognitive functioning and physical performance is higher for men than women; whites than African Americans; community than facility residents; and those with more than high school education than those with less than high school education. Multivariate models elaborate that differences in educational attainment generally account for the largest proportion of variance in cognitive functioning and residential status generally accounts for the largest proportion of variance in physical performance measures. 18. Eating Behaviors, Mental Health, and Food Intake Are Associated With Obesity in Older Congregate Meal Participants

Abstract

The relationship between eating behaviors, food intake, and mental health and the occurrence of obesity in older adults has rarely been investigated. Therefore, the objective of this study was to establish the associative links of these factors with two measures of obesity: class I obesity as indicated by body mass index (OB-BMI; BMI ≥30 kg/m2) and class I obesity as indicated by waist circumference (OB-WC; WC ≥43 inches for men and ≥42 inches for women). Older adults participating in the Older American's Act congregate meal program (N = 113, mean age = 74 years, 74% female, 45% African American) were assessed. Eating behaviors (cognitive restraint, uncontrolled eating, and emotional eating), food group choices (sweets, salty snacks, and fruits), and mental health indices (depression, anxiety, and stress) were recorded by questionnaire and related to measured occurrence of OB-BMI and OB-WC. In a series of multivariate logistical regression models, we found cognitive restraint to be consistently and robustly associated with both measures of obesity. In the fully adjusted model, cognitive restraint, consumption of sweets, anxiety, and lack of depression were associated with OB-WC. In summary, we found an association of obesity with abnormal eating behaviors, certain food group intakes, and mental health symptoms in this population. These findings may guide the development of future weight management interventions in a congregate meal setting.

19. Vitamin D Status Is Associated With Grip Strength in Centenarians

Abstract

Low serum concentrations of 25-hydroxyvitamin D (25(OH)D) have been associated with poor physical function in older adults, but few, if any, studies have examined this relationship in the very old. Therefore, the purpose of this study is to examine this relationship in the very old. Serum 25(OH)D concentrations were obtained from 194 centenarians and near centenarians (98 years and older). The associations between 25(OH)D concentrations and measures of physical function were evaluated with unadjusted and adjusted regression models. We found that 35% of centenarians had 25(OH)D concentrations less than 50 nmol/L. Adjusted mean grip strength was lower for centenarians with 25(OH)D concentrations less than 75 nmol/L than for centenarians with higher concentrations (P<0.05). However, there were no differences in the Georgia Centenarian Study (GCS) Composite Scale, a global measure of physical function, between those with higher and lower 25(OH)D concentrations. We conclude that low 25(OH)D concentrations are associated with poor grip strength, but not GCS Composite Scale, in the very old. Considering the high burden of poor physical function in older adults, understanding the relationship between vitamin D and different measures of physical function, including strength, becomes increasingly important.

AbstractHearing loss has been associated with poor vitamin B12status in some, but not all studies. This study examined a possible relationship between age-related hearing loss and poor B12status in 93 older adults using different indices of B status. Hearing loss was defined as pure-tone average threshold > 25 decibel hearing level. Participants with methylmalonic acid (MMA) > 271 nmol/L at baseline received 1,000 μg/d, and those with MMA <271 nmol/L were randomly assigned to receive 0,25, or 100 Ltg/d of B12. In a series of logistic regression analyses, compared with participants with normal hearing, those with impaired hearing had a significantly higher serum mean MMA concentrations in the best and the worst ears and a higher prevalence of elevated MMA (> 271 nmol/L) in the worst ear only. Thus, elevated MMA concentration may be associated with hearing loss in older adults. However, short-term B12supplementation was unrelated to improvements in hearing status in B12-deficient individuals.

Abstract

AbstractA national probability sample in their 60s (32%), 70s (38%), and 80s (30%) completed a telephone survey (N = 475). Subjective measures indicated that perceived milk intolerance occurred more often in Blacks vs. Whites (35% vs. 17%) and women vs. men (21% vs. 10%). Compared to milk tolerant individuals, milk intolerant individuals were more likely to “not try at all” to consume enough calcium (46% vs. 31%), and more likely to avoid milk during adolescence (30% vs. 15%) and currently (56% vs. 23%). Milk intolerant elders appear at risk for low calcium intakes because of their long history of milk avoidance and lack of concern about consuming enough calcium as adults.

Abstract

AbstractThis study compares the dietary patterns of centenarians (n = 73) with sexagenarians (n = 76) and octogenarians (n = 83). A Daily Diet Diary was developed to code the responses of the 24-hour dietary recalls. Centenarians and octogenarians had higher consumption frequencies of milk (P < 0.001) and grains (P < 0.01) and fewer (P < 0.05) skipped breakfasts than sexagenarians. However, centenarians also consumed high fat foods, such as whole milk (P < 0.001) and biscuits (P < 0.01), and coffee (P < 0.05) more frequently than the two younger cohorts. Several of the variations found in the food patterns among the three age groups may reflect cohort differences, rather than true antecedents for longevity.

Abstract

Aim: To determine any correlation between magnetic resonance spectroscopy (MRS) pattern of high-grade glioma before, during, and after radiotherapy (RT) with overall survival (OS) and progression-free survival (PFS)., Patients and Methods: Twenty-six patients prospectively underwent surgery and RT to 60 Gy. MRS was performed before RT, at week 4 of RT, and 2 months post-RT. Normalized and relative metabolite ratios were evaluated. Patients were grouped according to similar evolving MRS patterns and analyzed for differences in OS and PFS., Results: Significant decreases in tumor choline/N-acetyl-aspartate and normalized choline were observed from baseline to post-RT. After a median follow-up of 22.9 months, patients with >40% decrease in normalized choline from week 4 during RT to 2 months post-RT had a significantly worse median OS (9.1 months vs. not reached, p<0.00001) and PFS (5.8 vs. 19.8 months, p=0.0018)., Conclusion: The change in normalized choline at 2 months post-RT was highly prognostic for PFS and OS. This may allow more individualized response-based treatment.
