Abstract

OBJECTIVE: To demonstrate hypercoagulability with a benchtop viscoelastic monitor in cats with congestive heart failure (CHF) and/or aortic thromboembolism (ATE) compared to controls. METHODS: 97 cats were enrolled throughout this prospective observational cohort study from September 2022 through October 2023. Cats were grouped by diagnosis of CHF, ATE, ATE plus CHF, or controls. Enrollment required diagnosis of heart disease and no previous antithrombotic therapy. The results of viscoelastic testing with the benchtop viscoelastic coagulation monitor (VCM Vet [VCM]; Entegrion) were compared between groups using factorial analysis of variance. RESULTS: Cats with heart disease had significantly higher clot times when compared to controls (control: mean, 285.3 [SD, 172.6]; CHF: mean, 391.7 [SD, 106.8]; ATE: mean, 415.9 [SD, 109.2]; and ATE plus CHF: mean, 368.6 [SD, 232.6]). Heart disease cats were noted to have significantly lower 45-minute lysis index values (control: median, 100 [range, 93 to 100]; CHF: median, 99 [range, 89 to 100]; ATE: median, 98 [range, 88 to 100]; and ATE plus CHF: range, 98 [91 to 100]). 39. Correlation of gross urine color with diagnostic findings in male cats with naturally occurring urethral obstruction.

Abstract

Seventy-five male cats with urethral obstruction were prospectively enrolled to evaluate gross urine color at urinary catheter placement for correlation with diagnostic findings. Cats with darker red urine were more likely to be azotemic (serum creatinine concentration >2.0 mg/dl [177 µmol/l]), and urine color correlated well with serum creatinine and serum potassium concentrations. Darker urine color was negatively correlated with urine specific gravity. Urine color was not associated with the presence or absence of lower urinary tract stones on radiographs or ultrasound. Cats with darker red urine at the time of urinary catheter placement are likely to have more significant metabolic derangements and may require more aggressive supportive care. Abstract

Patients with congenital insensitivity to pain and anhidrosis (CIPA), caused by mutations in the NTRK1 gene, can be difficult to diagnose because of their variable presentation, the lack of simple diagnostic tests, and the paucity of cases reported in North America. We describe a 1-year-old infant who had tooth loss and palmar hyperkeratosis as the primary manifestations of CIPA. He was initially evaluated by a pediatric dentist and epidermal dysplasia syndromes were considered, but insensitivity to pain was suspected after a skeletal survey revealed an unrecognized skull fracture. Nerve conduction studies were normal, as was his response to subdermal histamine injection. Sequence analysis of his NTRK1 gene revealed 2 mutations: 1 mutation is novel, while the other has been described previously in a patient of northern European descent. An antibody directed against NTRK1 revealed persistent expression in keratinocytes, consistent with the mutations in this patient. Skin biopsy specimens revealed a lack of epidermal and sweat gland innervation. Immunohistochemistry of skin biopsy specimens, together with routine nerve conduction studies, can provide quick and reliable confirmation if CIPA is clinically suspected. 44. Altering enhancer–promoter linear distance impacts promoter competition in cisand in trans

Abstract

In Drosophila, pairing of maternal and paternal homologs can permit trans-interactions between enhancers on one homolog and promoters on another, an example of a phenomenon called transvection. When chromosomes are paired, promoters in cisand in transto an enhancer can compete for the enhancer’s activity, but the parameters that govern this competition are as yet poorly understood. To assess how the linear spacing between an enhancer and promoter can influence promoter competition in Drosophila, we employed transgenic constructs wherein the eye-specific enhancer GMRis placed at varying distances from a heterologous hsp70promoter driving a fluorescent reporter. While GMRactivates the reporter to a high degree when the enhancer and promoter are spaced by a few hundred base pairs, activation is strongly attenuated when the enhancer is moved 3 kb away. By examining transcription of endogenous genes near the point of transgene insertion, we show that linear spacing of 3 kb between GMRand the hsp70promoter results in elevated transcription of neighboring promoters, suggesting a loss of specificity between the enhancer and its intended transgenic target promoter. Furthermore, increasing spacing between GMRand hsp70by just 100 bp can enhance transvection, resulting in increased activation of a promoter on a paired homolog at the expense of a promoter in cisto the enhancer. Finally, cis-/trans-promoter competition assays in which one promoter carries mutations to key core promoter elements show that GMRwill skew its activity toward a wild-type promoter, suggesting that an enhancer is in a balanced competition between its potential target promoters in cisand in trans.To activate their correct genes, enhancers must choose among potential target promoters in their chromosomal neighborhood. In Drosophila, extensive pairing between homologous chromosomes in somatic cells means this choice also involves potential promoter partners in trans on a nearby chromosome. Bateman and Johnson find that changing the linear distance between an enhancer-promoter pair can alter the specificity of an enhancer for its intended targets, with increasing distances leading to promiscuous activation of competing promoters both in cis and in trans.

46. Determination of normal intra-abdominal pressure using urinary bladder catheterization in clinically healthy cats.

Abstract

Objective– To establish a reference interval for intra-abdominal pressure (IAP) measured by urinary bladder catheterization in normal cats and determine if IAP is affected by observer variation, volume of saline instillation before measurement, or subject variables of gender, positioning, body condition score, and sedation. Design– Prospective experimental study. Setting– Private referral center. Animals– Twenty healthy adult cats. Interventions– Sedation with butorphanol, midazolam, and propofol for catheterization of the urinary bladder and measurement of IAP. Measurements and Main Results– A 5-Fr red rubber urinary catheter was placed under sedation, and IAP was determined using a water manometer with the cats in right lateral and sternal recumbency. Three readings were taken in each position by 2 observers. The cats were allowed to recover with the urinary catheter in place, and IAP was measured in each cat while they were awake in right lateral and sternal recumbency. Conclusions– In this population of clinically healthy cats, median (interquartile range) IAP taken over all measurements was 7.00 cm H2O (5.23–8.83 cm H2O). There was no statistical difference between observers or subject gender. Factors associated with a statistically significant increase in IAP were right lateral compared with sternal recumbency ( P=0.002), being awake compared with sedated ( P<0.001), having a higher body condition score ( P=0.01 and 0.001), instillation of a higher volume of saline into the bladder for measurement ( P<0.001 ), and struggling during awake measurements ( P<0.001). Alternating hemiplegia of childhood (AHC) is typically distinguished from familial hemiplegic migraine (FHM) by infantile onset of the characteristic symptoms and high prevalence of associated neurological deficits that become increasingly obvious with age. Expansion of the clinical spectrum in FHM recently has begun to blur the distinction between these disorders. We report a novel ATP1A2mutation in a kindred with features that bridge the phenotypic spectrum between AHC and FHM syndromes, supporting a possible common pathogenesis in a subset of such cases. Mutation analysis in classic sporadic AHC patients and in an additional five kindreds in which linkage to the ATP1A2locus could not be excluded failed to identify additional mutations.
